Traumatic axial dislocation of the carpus in a 20-year-old man is described. This injury was accompanied by a crushing injury to the hand. The disruption pattern was different from those of previously reported cases. Despite the restoration of painless wrist motion postoperatively, grip strength remained below normal. Early accurate reduction, fixation, and range of motion exercise are the treatments of choice in such complex injuries.  相似文献

Wound healing is an intricate, tightly regulated process that is critical to maintaining the barrier function of skin along with preserving all other skin functions. This process can be influenced by a variety of modifiable and nonmodifiable factors. As wound healing takes place in all parts of the human body, this review focuses on cutaneous wound healing and highlights the classical wound healing phases. Abnormal-pronation, excessive-pronation, over-pronation, or hyper-pronation, are terms with a long historical use in both medical and research terminology pertaining to an abnormal movement of the foot. These terms are commonly used as a potential kinematic aetiology to explain the occurrence of multiple lower limb and foot pathologies. Yet despite this widespread use there is no clear definition to explain what the terms abnormal, excess, over, or hyper-pronation, mean. Without a strict definition the use of these terms as a patho-mechanical explanation of injuries, leaves a distinct lack of clarity and is meaningless in regards to being able to distinguish what movement may need clinical intervention. It is unlikely that hyper-pronation can be given a quantitative scale because individual anatomy and activity changes the necessary range of pronation within the foot. This paper attempts to give hyper-pronationa philosophically justifiable definition so that the term may be used in a more precise manner and avoid the confusion that presently exists clinically as well as in research literature.  相似文献

Most areas of respiratory medicine continue to use an Oslerian approach, based on signs and symptoms, in which the disease is the center of all activity. However, this paradigm is changing. Now that lung diseases have been recognized as heterogeneous and complex, we are moving toward more personalized, precise, patient-oriented medicine. The aim of this review was to define the current state of the knowledge on bronchiectasis, or, more accurately, the bronchiectasis syndrome, as a multidimensional, systemic, heterogeneous, complex disease. We explore the advances that have already been made, and above all the many steps that are still to be taken. We also propose some tools which might facilitate the application of these concepts in clinical practice, and help us continue our journey toward a more holistic view of this disease.  相似文献
