Incidental colonic focal lesions detected by FDG PET/CT   总被引:9,自引:0,他引:9  
OBJECTIVE: The aim of this study was to assess the performance of FDG PET/CT for the detection of colonic lesions, especially advanced neoplasms (villous or >10-mm adenomas, carcinomas). Because of 18F FDG accumulation in adenomatous polyps, PET using FDG can detect early premalignant colorectal lesions. MATERIALS AND METHODS: FDG PET/CT studies performed for a 1-year period in 1,716 consecutive patients with various malignant diseases, except colorectal cancer, were retrospectively reviewed. PET images obtained 1 hr after FDG injection and non-contrast CT images used for attenuation correction were fused for analysis. Of 45 patients showing intense focal colonic FDG uptake, 20 patients (with 21 foci) underwent a colonoscopic investigation, and, when necessary, polyp resection. The intensity of FDG uptake was quantified using the standardized uptake value (SUV(max)). RESULTS: The FDG colonic foci were associated with 18 colonoscopic abnormalities in 15 patients, with no colonic abnormality detected in five patients (false-positive [FP] results). Histopathologic findings revealed advanced neoplasms in 13 patients (13 villous adenomas and three carcinomas) and two cases of hyperplastic polyps. A difference in the mean SUV(max) was found between FP and true-positive colonic FDG foci but was not statistically significant (p = 0.14). CONCLUSION: Presence of a focal colonic FDG uptake incidental finding on a PET/CT scan justifies a colonoscopy to detect (pre-)malignant lesions. The fusion of PET and CT images allows an accurate localization of the lesions. PET/CT is a useful tool to differentiate pathologic from physiologic FDG uptake.  相似文献

Colonic adenomas constitute 70-80% of all colorectal polyps, and their clinical significance relates primarily to their relationship with colorectal cancer. The malignant potential of the polyps detected by FDG-PET is unknown, as not all the colonic lesions identified by FDG-PET represent colorectal malignancies. The purpose of this study was to investigate the rate of FDG-PET positivity within colonic villous adenomas. A pathology database search was performed to identify all patients diagnosed with colonic villous adenoma between June 1, 1996 and December 1, 2000. Patients with a pathologic diagnosis of colonic villous adenoma and who also had a FDG-PET study up to 1 month before colonoscopy were included in this study. FDG-PET findings were compared with pathological features. Of more than 4,000 patients, six patients were diagnosed with colonic adenoma on subsequent colonoscopy following FDG-PET study. Based on the pathological findings, these 6 patients had a total of 2 villous and 9 tubulovillous adenomas. Five of the 6 patients showed foci of increased FDG uptake in the region of the colon that corresponded to the villous adenoma(s) detected on colonoscopy, which accounted for a true-positive rate of 83.3% (5/6 subjects). Focal lesions in the colon seen on FDG-PET examinations need to be investigated further, even though some of these will prove to be villous adenomas rather than colorectal carcinomas. Future studies in a larger number of patients are needed to evaluate the relationship of histopathological features of colonic polyps and detectability of these lesions by FDG-PET.  相似文献

目的 探讨老年结肠息肉的临床特点及其与癌变的关系.方法 对我院经结肠镜检出的498例结肠息肉患者的临床特点进行回顾性分析,比较老年组(60岁以上)与中青年组(60岁以下)结肠息肉的特点.结果 腺瘤性息肉的检出率老年组(69.7%)显著高于中青年组(37.6%)(P<0.01);虽然结肠息肉以左半结肠为多,但在老年组腺瘤性息肉在结肠各部位的检出率并无显著性差异;绒毛状腺瘤不典型增生发生率和严重程度显著高于管状腺瘤.结论 老年人大肠息肉中的腺瘤性息肉患病率高,癌变机会大,老年人应尽量行全结肠检查,发现腺瘤性息肉,特别是绒毛状腺瘤不管大小应完整切除,减少癌变的机会.  相似文献

PURPOSE: To prospectively investigate with computed tomographic (CT) colonography the prevalence and size distribution of nonadenomatous polyps in asymptomatic adults and to compare the detection rates of adenomatous and nonadenomatous polyps. MATERIALS AND METHODS: A total of 1233 asymptomatic adults (mean age, 57.8 years; 505 women, 728 men) underwent same-day CT colonography and optical colonoscopy procedures. CT colonoscopy studies were interpreted prospectively with a primary three-dimensional approach immediately before optical colonoscopy. Statistical analysis was performed with the chi(2) test. Size, prevalence, and by-polyp detection differences were compared between adenomatous and nonadenomatous polyps. RESULTS: Seven hundred fifty-six (57.7%) colorectal polyps identified at optical colonoscopy in 410 (33.3%) patients were nonadenomatous; of these lesions, 622 (82.3%) were diminutive (相似文献

OBJECTIVE: Our purpose was to determine the clinical significance of polyps missed on CT colonography using histologic analysis and the natural history of colorectal polyps and to propose guidelines for follow-up colon surveillance based on CT colonographic findings. SUBJECTS AND METHODS. One hundred eighty-six men (age range, 40-87 years; mean, 62.3 years) underwent CT colonography immediately before conventional colonoscopy. All polyps detected on CT colonography were measured and imaged, and their segmental location was documented. All polyps detected on colonoscopy were measured, photographed, biopsied, and histologically analyzed. Results of CT colonography and conventional colonoscopy were compared with the final pathology reports. Conventional colonoscopy was used as the gold standard unless CT colonography showed a lesion measuring 10 mm or more that was not detected on conventional colonoscopy and had characteristics of a polyp. In these cases, follow-up conventional colonoscopy was offered. RESULTS: One hundred ninety-one polyps were detected on conventional colonoscopy. CT colonography prospectively detected 53 polyps. Histologic analysis of the polyps not detected on CT colonography showed that of those 5 mm or smaller, 58.1% were not adenomas, and of those measuring 6-9 mm, 42.8% were not adenomas. Both missed polyps at CT colonography of 10 mm or more were adenomas. Of the 22 polyps measuring 10 mm or more, three were not detected on conventional colonoscopy. Of these three, CT colonography showed a lesion having characteristics of a polyp, follow-up endoscopy confirmed the presence of the lesion, and histologic analysis showed a villous adenoma, a tubulovillous adenoma, and a tubular adenoma. CONCLUSION: If CT colonography shows no abnormality, follow-up screening in 5 years is recommended. If CT colonography detects a lesion smaller than 5 mm, follow-up imaging in 3-5 years is recommended. If CT colonography detects a lesion measuring 6 mm or more, endoscopy and polypectomy should be offered unless contraindicated.  相似文献

PURPOSE: To examine: (1) the feasibility of PET/CT colonography (PET/CTc) in patients with colorectal polyps; (2) the impact of metabolic information on CTc interpretation and, conversely, the impact of morphological information on PET characterisation of focal colorectal uptake. METHODS: Ten patients with colorectal polyps underwent PET/CTc, followed within 3 h by therapeutic conventional colonoscopy (CC). A radiologist and a nuclear medicine physician analysed the PET/CTc images. The agreement of morphological and metabolic information in the colon and rectum was evaluated. The sensitivity and specificity of PET, CT and PET/CT were calculated for colorectal polyps. RESULTS: Seventeen polypoid lesions were identified at CC: six< or =5 mm, six between 6 and 9 mm, and five > or =10 mm (four hyperplastic polyps, 11 tubular adenomas, one adenocarcinoma and one submucosal lipoma). A total of 20 scans (supine and prone) were performed in the ten patients: the agreement of morphological and metabolic information was excellent in 17 scans, good in two and moderate in one. PET/CTc showed a sensitivity of 91% for lesions > or =6 mm and a specificity of 100%. The metabolic information did not disclose any further polyps missed on CTc. The morphological information permitted correct classification of all eight instances of focal radiotracer uptake. CONCLUSION: PET/CTc is a feasible study. Adding a colonographic protocol to PET/CT images seems to allow correct characterisation of all cases of colorectal focal radiotracer uptake. The metabolic information does not seem to increase the accuracy of CTc.  相似文献

PURPOSE: To optimize scanning parameters for virtual colonoscopy utilizing a multislice Helical CT scanner in an in vitro study (using a homemade colonic phantom) and in a preliminary clinical study. MATERIAL AND METHODS: A colonic phantom was built using a plastic tube and 12 plastiline polyps were placed inside. The colonic phantom was studied with a multislice Helical CT scanner. Axial images were obtained with the phantom parallel to the long axis of the moving table (in order to simulate the evaluation of ascending and descending colon): oblique images were acquired with the phantom at 45 degrees relative to the long axis of the moving table (in order to simulate the evaluation of sigmoid colon and colonic flexures). Four different scanning protocols were tested: 1) slice collimation, 5 mm; slice width, 7 mm; table speed, 25 mm; reconstruction index, 5 mm; 2) slice collimation, 2.5 mm; slice width, 3 mm; table speed, 15 mm; reconstruction index, 3 mm; 3) slice collimation, 1 mm; slice width, 1.25 mm; table speed, 5 mm; reconstruction index, 1 mm; 4) slice collimation, 1 mm; slice width, 1.25 mm; table speed, 4 mm; reconstruction index, 1 mm. Quantitative analysis consisted in evaluation of the number of identified polyps and polyp size along the longitudinal axis. Qualitative analysis consisted in the evaluation of image artifacts and quality of 3D reconstructed images (step artifacts and polyp geometry distortion). This preliminary clinical study was performed in 12 patients (7 men and 5 women) who underwent multislice Helical CT colonography. We selected patients with clinical indications for conventional colonoscopy or after unsuccessful conventional colonoscopy. RESULTS: Multislice Helical CT colonography was 100% sensitive in the detection of all polyps and in all scanning protocols. With oblique scans, only a 3-mm polyp was missed during protocol 1 (sensitivity: 92%). Polyp geometry distortion was observed on longitudinal reconstructions, whereas no distortion was seen on axial images. Image quality was graded as optimal for protocols 2, 3, and 4; protocol 1 was graded as good on transverse scans and as poor on oblique scans. In our preliminary clinical study, two colonic carcinomas and three polyps were identified. CONCLUSIONS: At present, the introduction of multislice technology in virtual colonoscopy permits to improve spatial resolution and image definition. The actual clinical advantage, in terms of increased diagnostic accuracy, needs further investigation in larger clinical studies.  相似文献

The diagnostic accuracy of 2-[(18)F]-fluoro-2-deoxy- D-glucose positron emission tomography (FDG-PET) for the detection of (pre-)malignant lesions of the colon was compared with that of endoscopy. We selected a cohort of 39 patients [13 females and 26 males; mean age 62.3 years standard deviation (SD) 9.6 years] who underwent both FDG-PET and endoscopy (total of 44 procedures) in a 2-year period with a maximum interval between the examinations of 3 months (mean 30 days, SD 28 days). The underlying pathology was colorectal malignancies (24 patients), other malignancies (nine patients) and other disorders (six patients). Follow-up of resected colorectal cancer was the most common reason for the performance of endoscopy. In 19 patients FDG bowel uptake was interpreted as non-physiological, and in 18 patients abnormal findings (adenomatous polyps >3 mm or carcinoma) were detected by endoscopy. Compared with colonoscopy, FDG-PET had a sensitivity of 74% and specificity of 84%. The positive predictive value of FDG-PET was 78%. FDG-PET failed to detect small (diameter 3-10 mm) polyps in four patients. In nine cases abnormal FDG accumulation on PET imaging was the sole reason for performance of an endoscopic procedure. In these cases, endoscopy detected large adenomatous polyps in four patients and carcinomas in two patients, but no abnormalities were detected on endoscopy in the other three patients. There was a good correlation between the location of FDG uptake and endoscopy-positive lesions. FDG-PET is able to detect clinically relevant lesions of the colon. Our study suggests that it can be regarded as a useful adjunct in the non-invasive follow-up of patients with colorectal carcinomas.  相似文献

目的 评估计算机辅助检测系统(CAD)设置不同的检测过滤器数值(SFV)时,在低剂量MSCT成像中对结肠病变的检测能力.方法 56例结肠癌和(或)结肠息肉患者行MSCT结肠成像扫描,依据结肠镜和外科手术结果,将病变分为4组:结肠癌、最长径≥10.0mm息肉、最长径5.1~9.9 mm息肉和最长径≤5.0 mm息肉,之后确定病变在CT图像上的部位及大小,作为评估结肠CAD系统检测病变的金标准.将CAD系统的SFV设为0.25、0.50、0.75和1.00共4个等级,分别检测CT结肠成像图像,记录CAD标注出的病灶的部位和大小,根据上述金标准评估CAD系统对结肠病变的检出率,采用x2检验比较不同SFV设置时CAD对各组病变的检出率.结果 56例患者共有159个阳性病灶,其中结肠癌为44个,最长径≥10.0mm息肉45个,最长径5.1~9.9 mm息肉32个,最长径≤5.0 mm息肉38个.将结肠CAD系统SFV分别设置为0.25、0.50、0.75和1.00时,病灶的检出率分别85.5%(136/159)、85.5%(136/159)、79.2%(126/159)和56.0%(89/159).SFV为0.25和0.50时,与SFV为1.00时,CAD对病灶检出率的差异有统计学意义(P<0.05).随着SFV数值的减低,病灶的检出率增高,假阳性数增加,但91.4%(138/151)~93.9%(31/33)的假阳性病灶很容易识别,仅有6.1%(2/33)~8.6%(13/151)的假阳性病灶,需借助MPR和3D仿真内镜识别.结论 在低剂量MSCT结肠成像中,结肠CAD系统可获得满意的病灶检出率,可调节CAD系统SFV数值,以便满足不同经验阅片者的需求.  相似文献

Spiral CT pneumocolon: applications, status and limitations   总被引:4,自引:0,他引:4  
CT pneumocolon is a promising new technique in the diagnosis and management of colon pathology. CT pneumocolon can detect (sensitivity >95%) and stage (accuracy 79%) colorectal cancer and is very accurate in the differentiation of malignant from benign colonic pathologies. It has excellent detection rates for polyps >10 mm in diameter. Several studies using 3D virtual colonoscopy have already proven its high sensitivity and specificity in polyp detection making this technique robust as a screening tool. The combined results for virtual colonoscopy, from all centres, show a sensitivity of >85% in the detection of polyps 10 mm or greater in size, 70-80% for 5-9 mm polyps and an overall specificity of 90%. CT pneumocolon is a safe, non-invasive and cost-effective method for detecting colonic carcinomas and adenomas and correctly identifying which patients need further colonoscopy. The technique is quick, well tolerated and non-operator dependent. It can also image the proximal colon when distal stenoses prevent endoscopic and barium examination. CT pneumocolon is able to identify the features and complications of inflammatory bowel disease. Further research is warranted to fully assess its impact in terms of a screening tool, acceptability, availability and cost benefit.  相似文献

目的:探讨PET/CT意外发现直结肠FDG局灶性高代谢灶的临床意义。方法回顾性分析34例PET/CT意外探测到直结肠局灶性高放射性摄取灶的病例,半定量分析局部病灶放射性摄取程度,均在1个月内行结肠镜检查,对表现为新生肿物病灶进行手术切除并做病理分析,将肠镜及病理结果作为金标准。所有临床资料及半定量分析数据汇总后行单因素方差分析。结果在所有34例结肠局灶性FDG高代谢病灶中,结肠镜及病理检查出恶性及癌前病变共22例,阳性预测值(PPV)为64.7%。其中8例为腺瘤,病灶SUVmax平均值为7.55±2.1,14例恶性病灶(9例腺癌,卵巢癌转移2例,间皮瘤转移2例,交界瘤转移1例),SUVmax平均值为7.62±4.87。结肠炎性改变4例(11.8%),SUV-max平均值为9.35±4.9,肠镜正常8例(23.5%),SUVmax平均值为7.4±1.75。癌前病变、恶性肿瘤、炎性病变及生理性摄取组间的单因素方差分析提示各组间无显著性差异,F=0.27,P=0.84,其中癌前病变组与恶性病灶组无显著性差异,P=0.968。结论18F-FDGPET/CT偶然发现直结肠局灶性高摄取病灶中癌前病变和恶性病变阳性率很高,单纯依靠SUVmax,不能进行良恶性鉴别,因此进一步结肠镜检查是有临床意义的。  相似文献

OBJECTIVE: The aim of this study was to determine whether a new virtual colon dissection 3D visualization technique for CT colonography has a shorter analysis time and better sensitivity for detection of colonic polyps than interpretation of axial CT images. SUBJECTS AND METHODS. CT colonography was performed in 22 patients using 4-MDCT followed by conventional colonoscopy on the same day. The CT colonography data sets were analyzed by virtual colon dissection, which virtually bisects and unfolds the colon along its longitudinal axis to inspect the inner colonic surface for polyps. The same CT data sets were independently evaluated using axial interpretation. All data sets were independently interpreted by two radiologists in a blinded manner. RESULTS: Conventional colonoscopy revealed 31 colonic lesions in 20 patients. Twenty two of the lesions were smaller than 10 mm; nine were 10 mm or larger. Two of the original 22 patients were excluded, one because of residual stool and fluid and the other because of an impassable stenosing rectal wall cancer. For virtual colon dissection, the per-lesion sensitivity was 42% for observer 1 and 68% for observer 2; for axial interpretation, the respective sensitivities were 48% and 61%. For polyps 10 mm or larger, the respective sensitivities were 67% and 89% for virtual colon dissection and 89% and 100% for axial interpretation. The average time for reconstruction and analysis of virtual colon dissection was 36.8 min versus 29.2 min for axial images. Virtual colon dissection was feasible in both the supine and the prone positions in 45.5% of colonic segments, in either the supine or the prone position in 24.5%, and in neither position in 30% of segments. CONCLUSION: Although virtual colon dissection may facilitate detection of colonic polyps in isolated cases, its detection rate is not superior to axial interpretation, which is mainly attributable to failed rendering of insufficiently distended colonic segments or regions with residual feces. Virtual colon dissection is also the more time-consuming of the two procedures. With further improvement of path-finding and image segmentation, however, virtual colon dissection has the potential to be a useful interpretation tool for CT colonography.  相似文献

PURPOSE: To prospectively compare dark-lumen magnetic resonance (MR) colonography with conventional colonoscopy in the detection of colorectal polyps. MATERIALS AND METHODS: Local ethical committee approval and informed consent were obtained. One hundred consecutive patients (56 men, 44 women; mean age +/- standard deviation, 67.7 years +/- 14.7; range, 25-82 years) who were referred for conventional colonoscopy from January 2003 to January 2004 underwent MR colonography and conventional colonoscopy after standard precolonoscopic bowel cleansing. Colonoscopy was performed immediately after MR colonography. For MR colonography, the colon was filled with approximately 2000 mL of tap water. Imaging was performed with a 1.5-T MR unit with patients in the prone position. A T1-weighted three-dimensional volumetric interpolated breath-hold sequence was performed before and 75 seconds after intravenous administration of 0.2 mmol gadobenate dimeglumine per kilogram of body weight. Results of MR colonography were analyzed on a per-polyp and per-patient basis. Findings at colonoscopy were used as the reference for determining accuracy, sensitivity, specificity, and positive and negative predictive values of MR colonography. RESULTS: Of 100 patients recruited for study, 92 (52 men, 40 women; mean age, 61.5 years +/- 14.5; range, 25-82 years) underwent complete MR and conventional colonoscopy examinations. Forty-three of the 92 patients (47%) had normal findings at conventional colonoscopy. In the other 49 patients (53%), conventional colonoscopy depicted 107 polyps (82 adenomas, 25 hyperplastic polyps) and seven carcinomas. At per-polyp analysis, sensitivity of MR colonography in the detection of adenomatous polyps was 100% for polyps at least 10 mm in diameter and 84.2% for polyps 6-9 mm in diameter. At per-patient analysis, the accuracy of MR colonography was 93.1% (sensitivity, 89%; specificity, 96%) if detection of adenomatous polyps of all sizes was considered. CONCLUSION: Dark-lumen MR colonography is a promising modality with high accuracy for detecting colorectal polyps larger than 5 mm in diameter.  相似文献

PURPOSE: To determine the sensitivity and specificity of computed tomographic (CT) colonography for colorectal polyp and cancer detection by using colonoscopy as the reference standard. MATERIALS AND METHODS: Three hundred patients underwent CT colonography followed by standard colonoscopy. Bowel preparation consisted of magnesium citrate and polyethylene glycol. After colonic air insufflation, patients underwent scanning in the supine and prone positions with 3-mm collimation during a single breath hold. The transverse CT images, sagittal and coronal reformations, and three-dimensional endoluminal images were interpreted by two radiologists independently, and then a consensus reading was performed. CT colonographic findings were correlated with standard colonoscopic and histologic findings. RESULTS: The overall sensitivity and specificity of CT colonography for polyp detection were 90.1% (164 of 182) and 72.0% (85 of 118), respectively. By using direct polyp matching, the overall sensitivity was 69.7% (365 of 524). The sensitivity was 90% (74 of 82) for the detection of polyps 10 mm or larger, 80.1% (113 of 141) for polyps 5.0-9.9 mm, and 59.1% (178 of 301) for polyps smaller than 5 mm. The sensitivity was 94% (64 of 68) for the detection of adenomas 10 mm or larger, 82% (72 of 88) for adenomas 5.0-9.9 mm, and 66.9% (95 of 142) for adenomas smaller than 5 mm. CT colonography was used to identify all eight carcinomas. CONCLUSION: CT colonography has excellent sensitivity for the detection of clinically important colorectal polyps and cancer.  相似文献

Feasibility study of using air as a contrast medium in MR colonography   总被引:6,自引:0,他引:6  
AIM: To evaluate the feasibility of using air as intraluminal contrast medium in magnetic resonance (MR) colonography. MATERIALS AND METHODS: Twenty-two patients underwent MR colonography (MRC) using air before colonoscopy. All patients were imaged in prone and supine positions. Each colon was divided into five segments. Two radiologists reviewed the images for the degree of colonic distension, image quality and colonic lesions. The colonic lesions identified using MR were compared with those identified on colonoscopy. RESULTS: Eight men and 14 women with mean age of 55.5 years were studied. All patients completed the MR examination. One hundred and five out of 110 (95.5%) colonic segments were well distended in both prone and supine positions. Image quality was good in all cases. Colonoscopy detected 16 lesions: three > or = 1 cm, one 7 mm lesion and 12 lesions < or = 5 mm. All lesions 1 cm or larger were detected by MRC. CONCLUSION: MRC using air and the single-shot half-Fourier rapid acquisition with refocused echoes (RARE) technique is feasible. But in this small pilot study, it had low sensitivity for the detection of polyps less than 1 cm in diameter.  相似文献

CT colonographic data were processed by using a shape-based algorithm that depicts masses that protrude into the lumen. We studied nine shape criteria and three isosurface threshold settings. Results were compared with those of conventional colonoscopy performed the same day. RESULTS: There were 50 polyps (28 were > or =10 mm in size; 12, 5-9 mm; 10, <5 mm). The sensitivity with optimal settings for detecting polyps 10 mm or greater was 64% (18 of 28). Sensitivity improved to 71% (10 of 14) for polyps 10 mm or greater in well-distended colonic segments. Performance decreased for polyps less than 10 mm, poorly distended colonic segments, and other shape algorithms. There was a mean of six false-positive lesion sites per colon. These sites were reduced 39% to 3.5 per colon by sampling CT attenuation at the lesion site and discarding sites having attenuation less than a threshold. CONCLUSION: Automated detection of colonic polyps, especially clinically important large polyps, is feasible. Colonic distention is an important determinant of sensitivity. Further increases in sensitivity may be achieved by adding prone CT colonography.  相似文献

OBJECTIVE: The clinical significance of flat lesions in colorectal cancer screening remains uncertain. The purpose of this study was to investigate the frequency, histology, and virtual colonoscopy detection of flat lesions in an asymptomatic screening population. SUBJECTS AND METHODS: The morphology of all detected polyps was prospectively recorded as flat or polypoid (sessile or pedunculated) in 1,233 consecutive asymptomatic adults who underwent same-day virtual colonoscopy and optical colonoscopy. A flat morphology was defined as a shallow plaquelike broad-based lesion with a height of less than one half of its width. RESULTS: Of 344 polyps of 6 mm or greater confirmed at optical colonoscopy, 17 (4.9%) were labeled as flat at both virtual colonoscopy and optical colonoscopy; 17 (4.9%), at optical colonoscopy only; and 25 (7.3%), at virtual colonoscopy only, yielding 59 total lesions in 52 (4.2%) of 1,233 patients. Twenty-nine (49.2%) of 59 flat lesions were adenomatous, of which four measured 10 mm or greater and one 6- to 9-mm lesion was histologically advanced. None of the 148 diminutive flat lesions (< or = 5 mm) detected at optical colonoscopy was histologically advanced. Virtual colonoscopy prospectively detected 24 (82.8%) of 29 flat adenomas and 47 (80.0%) of all 59 flat lesions 6 mm or greater. In comparison, the sensitivity of virtual colonoscopy for the detection of polypoid adenomas and all polypoid lesions of 6 mm or greater was 86.2% (156/181, p = 0.58) and 81.0% (231/285, p = 0.86), respectively. CONCLUSION: Flat adenomas measuring 6 mm or greater are uncommon in a typical Western screening population, and advanced flat neoplasms are rare. The sensitivity of virtual colonoscopy for detecting flat lesions was similar to that of polypoid lesions. These results indicate that flat lesions are not a significant drawback for virtual colonoscopy screening.  相似文献

BACKGROUND: Azoxymethane (AOM) is a potent carcinogen that induces colorectal cancer and adenomas in rats. [(18)F]FDG-PET is a molecular imaging technique that is based on the elevated uptake and retention of radiolabeled glucose. At present, it is unknown at which stage FDG accumulation occurs during the adenoma carcinoma sequence. To address this issue, we studied the FDG uptake in AOM-induced rat colorectal adenocarcinoma (CRC) and correlated this with histopathological findings. METHODS: Seventy Fischer 344 rats were injected with AOM. Terminal autopsy took place 20-38 weeks after the first AOM injection. After [(18)F]FDG PET scanning, the rats were sacrificed, tissue [(18)F]FDG uptake was measured, followed by histopathological examination. RESULTS: Macroscopic examination revealed 21 tumors (7 located in the small bowel and 14 in the colon) in 19 rats. On histological examination, we found 10 colonic adenocarcinomas (the first being observed at Week 22) and 7 adenocarcinoma in the small bowel. In total, seven colon adenomas were found in five rats, six of which expressed high-grade dysplasia. The [(18)F]FDG accumulation in small intestine carcinomas was well beyond background accumulation (P<.0001). On PET scanning, two rats showed focal accumulation of the abdominal area, corresponding to small intestine carcinomas. CONCLUSION: Adenocarcinomas had a significantly higher [(18)F]FDG uptake than background bowel uptake. [(18)F]FDG uptake was lower in adenomas than in carcinomas. These data suggest that the AOM model allows the evaluation of intervention strategies with [(18)F]FDG uptake as a valid outcome measure.  相似文献

Patients at increased risk of colon cancer require strict colon surveillance. Our objective was to establish the efficacy of 2D axial CT colonography as a surveillance test when performed in routine clinical practice. Eighty-two patients at increased risk of colon cancer underwent CT colonography followed by conventional colonoscopy on the same morning. CT colonography studies were performed on a four-ring multidetector CT scanner (100 mAs, 120 kVp, 4×2.5 collimation) and were interpreted by two radiologists using 2D axial images only. Results were correlated with findings at colonoscopy. Note was made of subsequent histology reports from polypectomy specimens. A total of 52 polyps were detected at colonoscopy. Using 2D axial images alone, with no recourse to 2D multiplanar or 3D views, the sensitivity of CT colonography was 100, 33 and 19% for polyps larger than 9, 6–9 and smaller than 6 mm, respectively. Per-patient specificities were 98.8, 96 and 81.5%, respectively. Twenty-nine percent of polyps smaller than 1 cm were adenomatous and there were no histological features of severe dysplasia. CT colonography is a useful colon surveillance tool for patients at increased risk of colon cancer. It has a high specificity for identifying patients who should proceed to colonoscopy and polypectomy, while allowing further colon examination to be deferred in patients with normal studies. Using 2D axial images only, CT colonography can be performed as part of the daily CT workload, with a very low rate of referral for unnecessary colonoscopy.  相似文献
