Biochemical signs of hyperthyroidism, or even overt and possibly lethal clinical hyperthyroidism were reported in 2 severely iodine-deficient African countries (Zimbabwe and Democratic Republic of Congo, RDC) soon after the introduction of iodized salt. The 2 countries had access to iodized salt produced in Botswana, as well as 5 other countries in the region, namely Cameroon, Nigeria, Kenya, Tanzania, and Zambia. Therefore, a multicenter study was conducted in these 7 countries to evaluate whether the occurrence of iodine-induced hyperthyroidism (IIH) after the introduction of iodized salt was a general phenomenon or corresponded to specific local situations in the 2 affected countries. Two or 3 areas with a past history of severe iodine deficiency that had recently been supplemented with iodized salt were selected in each of the 7 countries. The prevalence of goiter was determined in 4423 schoolchildren in these areas and the concentration of urinary iodine in 2258. Salt factories and health structures were visited for the evaluation of the quality of iodized salt and the possible occurrence of IIH. The study showed that iodine deficiency had been eliminated in all areas investigated, and that the prevalence of goiter had markedly decreased since the introduction of iodized salt. This is a remarkable achievement in terms of public health. However, some areas were now exposed to iodine excess due mostly to a poor monitoring of the quality of the iodized salt and of the iodine intake of the population. In these areas or countries, IIH occurred only when the introduction of iodized salt had been of recent onset (<2 years), namely in Zimbabwe and RDC. In conclusion, the risk of IIH after correction of iodine deficiency is closely related to a recent excessive increment of iodine supply.  相似文献

目的 调查浙江省舟山市海岛地区食用非加碘盐的居民患甲状腺疾病状况及致甲状腺疾病的相关影响因素.方法 2008年在浙江省舟山市岱山县对737名食用非加碘盐的居民进行流行病学问卷调查、甲状腺B超检查、甲状腺功能及尿碘测定;同时抽查了183名8~10岁儿童(均为食用非加碘盐居民的子女)的尿碘.结果 舟山市岱山县食用非加碘盐的居民尿碘中位数(MUI)为122.2 μg/L,8~10岁儿童MUI为123.7μg/L;甲状腺肿、甲状腺癌、甲状腺功能亢进(简称甲亢)、亚临床甲状腺功能亢进(简称亚临床甲亢)和亚临床甲状腺功能减退(简称亚临床甲减)的患病率分别为39.9%、0.4%、0.4%、0.7%和0.8%.logistic回归分析显示,甲状腺肿患病率无性别差异(P>0.05),而年龄是甲状腺肿发生的危险因素(P<0.05);甲状腺肿、甲亢患病情况与饮食史、吸烟史、饮酒史、饮茶史、尿碘水平均无明显相关关系(P均>0.05).结论 舟山市海岛地区食用非碘盐居民碘摄入适量,但甲状腺肿和甲亢患病率较高.  相似文献

Iodine deficiency disorders (IDD) were prevalent in the Islamic Republic (IR) of IRAN before 1989, when the national salt iodization program with 40 mg l/k of salt was initiated. Despite a comprehensive IDD control program, less than 50% of the households in rural areas consumed iodized salt by 1994. A law for the mandatory production of iodized salt for households was passed in 1994. The purpose of this study was to evaluate goiter status and urinary iodine excretion 2 yr after this law was implemented. In each of 26 provinces, 30 groups of 40 schoolchildren, total 36,178, were examined for goiter and classified according to World Health Organization (WHO) classification. Urinary iodine excretion was measured in 2,917 children by digestion method. Goiter was endemic in all provinces, but the majority were small (grade 1) goiter. Median urinary iodine was 20.5 microg/dl 85.1% had urinary iodine > or =10 microg/dl. Median urinary iodine was above 13 microg/dl in all 26 provinces. In all provinces the percentage of schoolchildren with urinary iodine <5 microg/dl was less than 16%. In nine provinces the median urinary iodine was between 13 to 20 microg/dl; urinary iodine of their schoolchildren was <5 microg/dl in 10.8% and <2 microg/dl in 6-9%. No significant difference was observed between boys and girls or children of rural and urban regions in urinary iodine excretion. We conclude that 7 yr after the beginning of salt iodization and 2 yr following mandatory iodized salt consumption, urinary iodine excretion is adequate in schoolchildren; considering the data of the percent of households consuming iodized salt and programmatic setting of the IDD program, The IR of Iran has reached a sustainable control program for iodine deficiency.  相似文献

Iodine deficiency is still an important health care problem in the world. In Italy, as in most European countries, it is responsible for the development of mild to moderate endemic goiter. In 1995 we conducted a goiter survey in the Gubbio township, an area of Umbria region in Italy, close to the Appenine mountain chain. This study demonstrated a high prevalence of goiter in the middle schoolchildren population, indicating the presence of moderate endemic goiter. Soon after, a goiter prevention campaign aimed at implementing the consumption of iodinated salt was started. In 2001, a second survey was conducted in the middle schoolchildren (age 11-14 yr old) of Gubbio and neighbour townships. Eight hundred thirteen subjects were studied. Data obtained in 240 age-matched children, studied in the same area in 1995, were used for comparison to monitor changes 5 yr after the beginning of iodine prophylaxis. Thyroid volume was measured by ultrasonography. Gland volume was expressed in ml. A large population living in a iodine-sufficient area, previously reported by others, was used as control. Urinary iodine excretion was measured randomly in 20% of the children. The overall prevalence of goiter decreased between 1995 and 2001 from 29 to 8%. Goiter odds ratio (OR), corrected for age, was 4.0 (95% CI 2.8-5.9) for 1995 compared to 2001 (p<0.000). Mean thyroid volume in the matched populations was 7.6+/-2.5 ml in 1995 and 5.7+/-2.1 ml in 2001. Median iodine urinary excretion increased from 72.6 to 93.5 mug/l, at the limit of statistical significance. Living in a rural area, no consumption of iodized salt and familiarity for goiter represented independent risk factors for goiter development. This study was the first conducted in Umbria region and confirmed that an implementation campaign for iodized salt consumption is a simple and useful instrument to prevent endemic goiter and related diseases. A new survey to evaluate goiter prevalence in the same area 10 yr after the beginning of iodine prophylaxis is already planned.  相似文献

A cross-sectional study in two stages consisted of healthy children to assess the effect of iodine supplementation on a pediatric population with mild iodine deficiency in an ongoing program in the Province of Pontevedra, northwestern Spain. In the first survey (1984), 1565 schoolchildren and in the second survey (1995) 907 schoolchildren were randomly selected from the population. In January 1985, a mandatory consumption of iodized salt in our region was begun. In both surveys we studied prevalence of goiter, urinary iodine excretion, and prevalence of thyroid dysfunction. Similar prevalences of goiter were observed in both surveys, 3.7% versus 3.9%; however, significantly lower prevalence of Ib and II degree goiters were observed in the second survey. The mean iodine excretion was 88.6 +/- 73 microg/L (median 66.3) and 146.4 +/- 99 microg/L (median 115.7), p < 0.01 for the first and second surveys, respectively. Finally, the overall prevalence of thyroid dysfunction was similar in both surveys, 9.2% versus 7.0%; however, significantly lower prevalence of suppressed serum thyrotropin (TSH), considered as a marker of subclinical hyperthyroidism, was observed in the second survey when compared to the first, 0.1% versus 2%, p < 0.01. Our results are in agreement with the recent data from Denmark, where the prevention of subclinical hyperthyroidism occurring in the elderly as a consequence of longstanding mild iodine deficiency is the reason that the Danish finally started iodine supplementation on a national basis. In conclusion, long-term correction of mild iodine deficiency in a pediatric population has beneficial effects on the prevalence of high-degree goiters, and this correction reduces significantly the prevalence of subclinical hyperthyroidism. The present observation constitutes a strong argument for correcting even mild iodine deficiency.  相似文献

采用不同浓度碘盐及碘油胶丸对昆明地区4个自然村1750人进行地方性甲状腺肿(简称地甲肿)防治。2年后地甲肿患病率分别从9.60%,8.30%,18.90%,23.60%,降至3.23%,3.08%,5.44%,4.92%。尿碘均值升至113.08—200.29μgI/L。结果提示:1、轻病区选用1/10万,重病区选用1/5万碘盐防治地甲肿是可行的;2、贫困山区的重病区除选用1/5万碘盐外还在重点人群加用小剂量碘油胶丸;3、适当增加碘摄入量是防治环境污染地区地甲肿的有效方法。  相似文献

OBJECTIVE: In many developing countries, children are at high risk for both goiter and anemia. Iron (Fe) deficiency adversely effects thyroid metabolism and reduces efficacy of iodine prophylaxis in areas of endemic goiter. The study aim was to determine if co-fortification of iodized salt with Fe would improve efficacy of the iodine in goitrous children with a high prevalence of anemia. DESIGN AND METHODS: In a 9-month, randomized, double-blind trial, 6-15 year-old children (n=377) were given iodized salt (25 microg iodine/g salt) or dual-fortified salt with iodine (25 microg iodine/g salt) and Fe (1 mg Fe/g salt, as ferrous sulfate microencapsulated with partially hydrogenated vegetable oil). RESULTS: In the dual-fortified salt group, hemoglobin and Fe status improved significantly compared with the iodized salt group (P<0.05). At 40 weeks, the mean decrease in thyroid volume measured by ultrasound in the dual-fortified salt group (-38%) was twice that of the iodized salt group (-18%) (P<0.01). Compared with the iodized salt group, serum thyroxine was significantly increased (P<0.05) and the prevalence of hypothyroidism and goiter decreased (P<0.01) in the dual-fortified salt group. CONCLUSION: Addition of encapsulated Fe to iodized salt improves the efficacy of iodine in goitrous children with a high prevalence of anemia.  相似文献

OBJECTIVE: Auditory disturbances may be present in iodine deficient children. The aim of this study was to determine the effect of long-term iodine supplementation on auditory thresholds in iodine deficient children. DESIGN AND METHODS: 70, 70 and 72 schoolchildren of an area of severe iodine deficiency were studied before intervention (1989), 3 yr after injection of 480 mg iodized oil (1992) and 7 yr after consumption of iodized salt (1999), respectively. Goiter was graded and serum T4, T3, TSH and thyroglobulin concentrations and urinary iodine levels were measured. Audiometry was performed with a pure tone audiometer. RESULTS: There was significant decrease in the prevalence and severity of goiter and serum TSH and thyroglobulin concentrations, and significant rise in serum T4 in 1992 and 1999, as compared to 1989. Before iodine supplementation, hearing was abnormal in 44% of schoolchildren, mean hearing threshold was 15.8 +/- 5.9 and in all children was >10 dB. Mean hearing threshold decreased to 10.2 +/- 4.6 and 10.0 +/- 5.9, 3 and 10 yr after intervention (p < 0.001). Forty seven and 62% of children had thresholds < 10 dB in 1992 and 1999, respectively. Hearing thresholds > 15 dB were detected in 46, 11 and 10% of schoolchildren in 1989, 1992 and 1999, respectively (p < 0.001). CONCLUSION: Continuous iodine supplementation permanantly improves the auditory thresholds of iodine deficient children.  相似文献

CONTEXT: To prevent goiter and nodular hyperthyroidism, iodine fortification (IF) of salt was introduced in Denmark in 1998. We prospectively registered all new cases of overt hyperthyroidism in two areas of Denmark before and for the first 6 yr after iodine fortification. METHODS: We used a computer-based register of all new cases of hyperthyroidism in two population subcohorts with moderate iodine deficiency (Aalborg, n = 310,124) and mild iodine deficiency (Copenhagen, n = 225,707), respectively. Data were obtained 1) before IF (1997-1998); 2) during voluntary IF (1999-2000); 3) during the early (2001-2002) period of mandatory IF; and 4) during the late (2003-2004) period with mandatory IF. RESULTS: The overall incidence rate of hyperthyroidism increased [baseline, 102.8/100,000/year; voluntary IF, 122.8; early mandatory IF, 140.7; late mandatory IF, 138.7 (P for trend <0.001)]. Hyperthyroidism increased in both sexes (P < 0.001) and in all age groups: 0-19, 20-39, 40-59, and 60+ yr (P for trend <0.001). The increase was relatively highest in young adults aged 20-39 yr: late mandatory IF (percent increase from baseline), age 20-39, 160%, P < 0.001; age 40-59, 29%, P < 0.01; age 60+ yr, 13%, P = not significant. CONCLUSION: Even a cautious iodization of salt results in an increase in the incidence rate of hyperthyroidism. Contrary to current concepts, many of the new cases were observed in young subjects, and are presumably of autoimmune origin. Furthermore, monitoring is expected to show a decrease in the number of elderly subjects suffering from nodular hyperthyroidism.  相似文献

目的 观察食盐加碘对地方性甲状腺肿(地甲病)和甲状腺功能亢进症(甲亢)患病率的影响。方法 1989年调查了福州市(10454人)和武夷山市(1032人)两地共11486人的甲状腺肿和甲状腺功能亢进症的患病率,经过普遍食盐加碘10年后,又以同样方法对两地进行调查(福州3473人,武夷山2394人,两地共5867)。结果 普遍食盐加碘10年后,福州市的甲亢患病率从0.57%增至1.09%(P<0.01),武夷山市地甲病患病率由24.51%减至11.58%(P<0.01)。结论 普遍食盐加碘使地甲病患病率明显降低,但甲亢的患病率增加,这可能部分与碘摄入增多有关。  相似文献

Northwestern Greece was identified in the 1960s for its high prevalence of endemic goiter and iodine deficiency. Although iodized salt has been commercially available since then, a recent epidemiological survey of 3916 schoolchildren found that low-grade goiter is still prevalent in endemic proportions (21%). The aim of this study was to further assess the cause of goiter and the severity of iodine deficiency in children from this endemic area of Greece. Of the 800 children with clinically detectable goiter, 97 children (60 girls and 37 boys, 8-15 years) were recruited for determination of urinary iodine excretion, as well as assessment of thyroid volume and function and detection of antithyroid antibodies. The median urinary iodine concentration was 8.4 microg/dL, indicative of a mild iodine deficiency. Thyroid function was normal in all but 11 children who had subclinical hypothyroidism. Sixteen children (16.5%), including all those with subclinical hypothyroidism, were positive for antithyroid antibodies. Their median urinary iodine concentration (20.6 microg/dL) was higher compared to children who were negative for antibodies (7.4 microg/dL; p<0.001). The mean thyroid volume by ultrasonography (12.2+/-4.1 mL) was above the upper limit of normal for this age group. Thyroid volume was inversely related to the urinary iodine content in the children with negative antithyroid antibodies. Iodine deficiency is still prevalent in northwestern Greece although of mild severity and constitutes the primary cause of goiter among schoolchildren. However, it appears that autoimmune thyroiditis is emerging as a frequent cause of goiter in those children with sufficient iodine intake.  相似文献

Urinary iodine levels in children (6-12 years) living in three rural blocks and in the municipal urban area of Bardhaman District, West Bengal, were analyzed to compare the status of recent iodine nutrition in the rural and urban population of the district. Goiter, indicating previous iodine status, was simultaneously estimated. Iodine levels in salt samples, that provide insight into the usage of iodized salt, were estimated. Data indicated that 56.6% of urban children and 51.1% of rural children were biochemically iodine repleted and had urinary iodine excretion (UIE) levels > or = 10microg/dl. Urban children (29.4%) and rural children (37.1%) were found to have goiter. Eighty percent and 50% of the rural and urban salt samples, respectively, were found to have iodine levels below 10 ppm; with significant urban-rural differences. The results indicate that iodine repletion in the surveyed area needs continuous surveillance of the proper distribution and use of iodized salt.  相似文献

Recently we surveyed the thyroid function and TSH concentration of villagers in an endemic goiter area where iodized salt had been supplied for 25 years. We found that the serum FT3 and TSH (IRMA) level of villagers were higher and the FT4 level was lower than those of the controls, comparing with the RIA, which suggested that the inhabitants of the endemic goiter area had subclinical hypothyroidism based on the IRMA method for TSH assay. Therefore, we suggest that the best biochemical technique for monitoring the iodized salt prophylaxis program and the physiological response of villagers to iodine is measurement of serum TSH level with the ultrasensitive assay and FT4 level periodically.  相似文献

天津市食盐加碘后甲状腺肿大率与碘营养状况的调查   总被引:14,自引:1,他引:13  
目的:调查和研究食盐加碘后天津市居民的甲状腺肿大率和碘营养状况,方法:在6个区共调查居民31530人,7-14岁在校儿童4415人,对调查对象进行甲状腺肿大率,尿碘浓度,食盐碘含量等项目的调查。结果:(1)食盐加碘后天津市居民的平均甲状腺肿大率为5.5%,8-10岁儿童平均甲状腺肿大率为19.7%,甲状腺肿大率较食盐加碘以前有了明显下降,但下降的速度较缓慢,(2)天津市食盐加碘以后各区居民的平均尿碘中位数为287.2ug/L,各区儿童尿碘中位数平均为271.6u/L,证明总体人群碘营养充足,(3)天津市居民总体盐碘含量超过20mg/kg水平,各区居民户盐含碘量有9.9%-41.1%超过60mg/kg,说明碘盐的含碘是偏高是一个新的现象。结论:经过3年左右的食盐加碘防治工作,天津市碘缺乏情况已得到控制。  相似文献

目的了解舟山市人群碘营养状况,为制定和完善科学补碘策略提供依据。方法随机抽取舟山市2县区中2个居委会(村)419名当地常住居民,共采集60份碘盐、7份水样、412份尿样分别进行盐碘、水碘、尿碘浓度检测,并对调查对象开展甲状腺超声检查,采集血样进行甲状腺功能检测。结果盐碘浓度的中位数为1.72mg/kg,仅有21份为合格碘盐,合格碘盐食用率为35%;水碘浓度均数为12.00μg/L;尿碘中位数为126.76μg/L;甲状腺异常73人,总异常率20.28%;甲状腺功能异常人数132人,总异常率为31.50%。结论舟山市居民的尿碘水平处于较适宜的水平,孕妇的尿碘水平明显低于WHO/UNICEF/ICCIDD推荐的标准。人群甲状腺异常率略高于省内平均水平,甲状腺功能异常率与全省水平无明显差异。  相似文献

Goiter rate, serum TSH, antithyroperoxidase (TPOAb), antithyroglobulin (TgAb) antibodies, and urinary iodine concentration (UIC) were evaluated 10-11 yr prior (1983-1984) and 5-6 yr after (1999-2000) national salt iodization in Iran. Pre- and post-iodization groups consisted of 465 and 1426 adults aged > or =20 yr, respectively, selected by random cluster sampling in Tehran province. Total, grade 1 and grade 2 goiter rates were 65.2, 53.1, and 12.1% in 1983-1984 vs 25.2, 15.5, and 9.7% in 1999-2000 (p<0.0001). Median serum TSH was 1.5 mIU/l in 1983-1984 vs 0.8 mIU/l in 1999-2000 (p<0.0001). Median TSH also decreased in 20-29, 30-39, 40-49, 50-59, and > or =60- yr-adults in 1983-1984 vs 1999-2000 (p<0.0001). In 1983-1984, positive TPOAb and positive TgAb were detected in 3.2 and 4%, respectively, using agglutination test. Corresponding values were 12.5 and 16.8% using immunoenzymometric assay in 1999-2000. Overt and subclinical hypothyroidism was present in 0 and 32.8/1000 in 1983-1984 vs 3.5 and 21.7/1000 in 1999-2000, respectively. Overt and subclinical hyperthyroidism was detected in 4.4 and 4.4/1000 in 1983-1984 vs 0.7 and 5.6/1000 in 1999-2000, respectively. Subclinical hypothyroidism in males was significantly more frequent in 1983-1984 vs 1999-2000 (odds ratio 5.02, 95% confidence interval 1.72-14.68; p=0.004). Salt iodization resulted in adequate UIC, decrease in serum TSH and subclinical hypothyroidism in males, and an increase in thyroid autoantibodies without significant change in thyroid abnormalities. Benefits of iodine supplementation far outweigh its hazards in Tehranian adults.  相似文献

OBJECTIVE: In developing countries, many children are at high risk for both goiter and iron-deficiency anemia. Because iron deficiency may impair thyroid metabolism, the aim of this study was to determine if iron supplementation improves the response to oral iodine in goitrous, iron-deficient anemic children. DESIGN: A trial of oral iodized oil followed by oral iron supplementation in an area of endemic goiter in the western Ivory Coast. METHODS: Goitrous, iodine-deficient children (aged 6-12 years; n=109) were divided into two groups: Group 1 consisted of goitrous children who were not anemic; Group 2 consisted of goitrous children who were iron-deficient anemic. Both groups were given 200mg oral iodine as iodized oil. Thyroid gland volume using ultrasound, urinary iodine concentration (UI), serum thyroxine (T(4)) and whole blood TSH were measured at baseline, and at 1, 5, 10, 15 and 30 weeks post intervention. Beginning at 30 weeks, the anemic group was given 60mg oral iron as ferrous sulfate four times/week for 12 weeks. At 50 and 65 weeks after oral iodine (8 and 23 weeks after completing iron supplementation), UI, TSH, T(4) and thyroid volume were remeasured. RESULTS: The prevalence of goiter at 30 weeks after oral iodine in Groups 1 and 2 was 12% and 64% respectively. Mean percent change in thyroid volume compared with baseline at 30 weeks in Groups 1 and 2 was -45.1% and -21.8% respectively (P<0.001 between groups). After iron supplementation in Group 2, there was a further decrease in mean thyroid volume from baseline in the anemic children (-34.8% and -38.4% at 50 and 65 weeks) and goiter prevalence fell to 31% and 20% at 50 and 65 weeks. CONCLUSION: Iron supplementation may improve the efficacy of oral iodized oil in goitrous children with iron-deficiency anemia.  相似文献

目的 了解山东省当前盐碘水平状态下,碘缺乏地区重点人群碘营养状况及防治措施落实情况,为今后防治工作提供依据.方法 2007年,在山东省碘缺乏地区选择山区的岱岳、蒙阴,平原的莒县、高密,沿海的招远,共5个县(市、区)作为调查地区,在每个县(市、区)选择1个乡(镇)作为调查点.采用触诊、B超法进行8~10岁儿童甲状腺检查,直接滴定法检测其家中食用盐含碘量;砷铈催化分光光度法检测8~10岁儿童、孕妇、哺乳期妇女、2岁内婴幼儿及20~45岁育龄妇女5种重点人群的尿碘水平.结果 共检查514例8~10岁儿童甲状腺,触诊甲状腺肿大率为1.8%(9/514),B超检查甲状腺肿大率为1.2%(6/514);共检测501份食用盐,盐碘均值为30.95 mg/kg,碘盐覆盖率为94.6%(474/501),合格碘盐食用率为90.4%(453/501);共检测1707份尿样,尿碘中位数为216.7μg/L,8~10岁儿童、孕妇、哺乳期妇女、2岁内婴幼儿及20~45岁育龄妇女尿碘中位数分别为234.0、165.5、162.4、257.5、233.0μg/L.结论 山东省碘缺乏地区重点人群碘营养水平处于适宜或较适宜状态.表明目前盐碘水平能够满足山东省不同碘缺乏地区人群对碘的需求.

Objective To learn the iodine nutritional status of the vulnerable population with different iodine level under the current level of iodized salt in Shandong province and to offer prevention and cure measures.Methods Five groups of vulnerable population including school children aged 8 - 10, pregnant, lactation women, infants and women of childbearing age from mountain areas ( Daiyue, Mengyin counties ) , plain ( Luxian,Gaomi counties ) and coastal (Zhaoyuan county ) of five different iodine deficient areas were investigated in 2007.The thyroids of children aged 8 - 10 were checked by palpation and B ultrasound, their edible salt iodine level was detected by direct titration. The lever of urinary iodine of vulnerable population was examined by arsenic and cerium speetrophotometry. Results The goiter rates of 8 - 10 year-old were 1.8%(9/514) and 1.2%(6/514), respectively by palpation and B-ultrasonic. The mean iodine of 501 edible salt samples was 30.95 mg/kg. The coverage rate of iodized salt was 94.6% (474/501). The rate of qualified iodized salt was 90.4% (453/501). The median of urinary iodine was 216.7 μg,/L. The urinary iodine of school children aged 8 - 10, pregnant, lactation women, infants and women of childbearing age were 234.0, 165.5, 162.4, 257.5, 233.0 μg/L, respectively. Conclusions Current iodine nutritional level is basically appropriate in all groups of vulnerable people. The current iodine content of iodized salt could meet the needs of population from different iodine deficient areas of Shandong province.  相似文献
