阴囊Paget's病22例临床分析

目的探讨阴囊Paget′s病的临床特点、诊治及预后。方法回顾性分析22例阴囊Paget′s病的临床资料并随访。结果 22例均获随访,存活20例。2例术后复发无转移,二次手术治疗,效果良好;1例术后复发伴转移,二次术后2年死亡;1例术后1年因胰腺癌死亡;余18例均未复发。结论阴囊Paget′s病是多被误诊的少见恶性肿瘤,早行病理活检是确诊该病的主要方法,彻底切除病灶并保证切缘阴性为首选治疗方式,早期治疗者预后较好。     

阴囊Paget病11例临床分析

目的探讨阴囊Paget病的临床特征、治疗方法及预后因素。方法回顾性分析11例阴囊Paget病的诊治资料，8例病灶仅累及单侧阴囊者行包括病灶的扩大切除术，1例复发者行双侧病灶扩大切除加右侧睾丸、附睾及精索切除，2例行下腹壁转移皮瓣术。结果11倒手术效果满意，术后病理检查均报告为阴囊Paget病。结论本病临床较罕见，易误诊为阴囊湿疹，需要及早经病理检查确诊，治疗上以外科手术治疗为主。     

阴囊Paget病一例

患者男，75岁，因“阴囊红斑瘙痒6年余，反复破溃糜烂伴恶臭半年”人院。入院体格检查：阴囊部见5cmX4cm皮肤破溃糜烂，局部见脓性渗出物，恶臭，少许干痂、脱屑，周围皮肤发红、粗糙，腹股沟区淋巴结无肿大，余正常。初步诊断阴囊Paget病，抗感染治疗后行根治性病灶切除术，送快速冰冻病理检查示阴囊Paget病，且皮肤切缘阴性。给予单纯阴囊皮肤缝合，术后给予加压包扎、抗炎换药等对症处理，I期愈合出院。随访1年，未出现复发及远处转移。     

阴股沟皮瓣在治疗外阴Paget病的应用

目的探讨阴股沟皮瓣在外阴Paget病治疗中的应用。方法对收治外阴Paget病5例,均行病变皮肤广泛切除后采用阴股沟皮瓣转移修复。结果5例外阴Paget病,4例病例一次手术修复,术后无复发,1例2年后复发,二次手术病灶切除,创面植皮修复,所有病例远期效果满意。结论阴股沟皮瓣修复外阴Paget病术后外阴外观、功能接近正常,皮瓣质地良好,创伤相对较小,是一种良好的修复方式。     

阴囊Paget病12例临床分析

阴囊Paget病临床较为少见，我院自1993年2月至2004年12月共收治12例阴囊Paget病患者，术前均经组织活检确诊，并行病灶广泛切除手术治疗，术后效果满意，现报告如下。     

阴囊Paget氏病（附2例报告）

阴囊Paget氏病在临床上较少见,常被误诊为阴囊湿疹,本文报告2例。病例介绍例1,男,53岁,因左侧阴囊搔痒、流水、结痂3年余,在外院诊断为阴囊湿疹久治不愈。查:左侧阴囊有φ3cm 溃疡灶,表面有少量脓苔,边缘增厚、结有痂皮。双侧腹股沟淋巴结不肿大。病理活检诊断为。Paget 氏病。住院后用HPD 光敏治疗,病灶有缩小,2月后局部皮损又现。活检仍见Paget 氏细胞残留。继用YAG 激光局部治疗,仍未愈。故在局     

阴囊皮肤鳞状上皮细胞癌并Paget病6例临床及病理特点分析

目的探讨阴囊鳞状细胞癌合并Paget病的诊断、治疗、发病因素及预后,提高对该病的认知及诊疗水平。方法回顾分析我院2006～2012年诊治的6例经病理证实为阴囊皮肤鳞状上皮细胞癌合并Paget病患者的临床资料。结果阴囊皮肤鳞状上皮细胞癌合并Paget病恶性程度较低,病程长,预后较好。结论早期诊断的重要方法是尽早行多点活检或手术,首选的治疗手段为病变处阴囊皮肤的早期广泛切除,并辅以放化疗等综合治疗。

阴囊Paget′s病3例临床诊治报道

阴囊Paget′s病又称阴囊湿疹样癌、阴囊炎性癌,是发生于乳腺外的Paget′s病,相对少见[1-2]。好发年龄在50～70岁,病情进展缓慢,可长达数年至数十年,其恶性程度低于鳞癌,预后相对较好。现报道3例该病病例如下。 1临床资料 1．1病例介绍1．1．1病例1患者,男,63岁。发现阴茎根部皮肤新生物6个月,在外院按阴囊湿疹治疗无好转,于2012年11月9日入院治疗。阴茎根部右侧处皮肤颜色发红,压之褪色,局部新生物约1．5cm×1．0cn、1．0cm×1．0cm大小,表面有溃疡、分泌物。双侧腹股沟无淋巴结肿大。切取病灶活检,病理报告：阴囊Paget’s病。临床分期AI期。     

阴茎阴囊Paget病2例围术期护理体会

阴茎阴囊Paget病又称阴茎阴囊皮肤炎性癌或湿疹样癌,是较罕见的男性外生殖器皮肤的恶件肿瘤。河南焦作第二人民医院泌尿外科收治了2例阴茎阴囊Paget病患者,均行阴茎阴囊paget根治、腹股沟淋巴结清扫、创面修复术。术后经过精心治疗和护理,患者痊愈出院。现将护理体会报告如下。     

Extramammary Paget's disease of the penis and scrotum

A 64-year-old Chinese man presented to the Bellevue Hospital Center Department of Dermatology with a 2-year history of an enlarging mass in his suprapubic region, which measured 7 cm x 9 cm and involved the penis and scrotum. A biopsy specimen showed extramammary Paget's disease. Twenty years earlier, while living in China in 1983, this patient had a suprapubic skin cancer which was excised, and he received radiation to the region. Extramammary Paget's disease is a rare cutaneous adenocarcinoma of epidermal origin, which is frequently associated with adnexal carcinoma and internal malignant conditions. Clinically, extramammary Paget's disease is characterized by a red, moist, eroded plaque in the anogenital region. Extramammary Paget's disease usually behaves as a slow-growing intraepithelial adenocarcinoma; however, it may become invasive and may metastasize through dermal lymphatics. The treatment of choice is wide excision.     

聚束蛋白在基底细胞癌、鳞状细胞癌、乳腺外Paget病、黑色素瘤中的表达

目的 探讨聚束蛋白在基底细胞癌、鳞状细胞癌、乳腺外Paget病及黑色素瘤中的表达.方法 用Elivision免疫组化法检测13例基底细胞癌、24例鳞状细胞癌、10例乳腺外Paget病、10例黑色素瘤以及10例正常皮肤组织中聚束蛋白的表达情况,分析聚束蛋白与皮肤肿瘤侵袭及转移的相关性.结果 聚束蛋白在基底细胞癌、鳞状细胞癌表达显著高于正常上皮组织(P<0.05),聚束蛋白在黑色素瘤表达显著低于正常上皮组织(P<0.05).结论 聚束蛋白在基底细胞癌、鳞状细胞癌中高表达,提示可能与肿瘤的局部侵袭相关,聚束蛋白在黑色素瘤低表达,提示可能与肿瘤细胞在真皮内和机体内广泛播散相关.     

The role of topical immune response modifiers in skin cancer

Topical immune response modifiers include imiquimod and resiquimod. The mechanism of action of immune response modifiers is complex and not completely understood. It involves the stimulation of innate and cell-mediated immune responses through Toll-like receptor-mediated induction of cytokines. Imiquimod is approved for the treatment of actinic keratoses, superficial basal cell carcinomas and warts; it has also been documented to successfully treat other forms of skin cancer such as squamous cell carcinoma in situ, melanoma and extramammary Paget's disease.     

Immunomodulators for skin cancer

Therapeutic interventions to augment tumor antigenicity or increase the host's immune response against cancer cells include recombinant cytokines, immune modulators, vaccination with tumor antigens, T cell-based immunotherapy, and gene therapy. We describe the current role of the immunomodulators (up-regulators of the immune response) in the therapy of skin cancer (non melanoma skin cancer, melanoma, lymphoma, Kaposi sarcoma, and extramammary Paget's disease).     

23例乳腺派杰氏病诊治体会

目的总结23例乳腺派杰氏(Paget’s)病患者的诊断及治疗。方法收集我院自1984～2004年23例经病理确诊的乳腺Paget’s病例，通过对病人的临床资料及治疗方法的分析，总结该病的诊治经验。结果乳腺Paget’s病以慢性乳头皮肤改变为主要特点，钼靶X线多显示导管区恶性改变。细胞学检查见到Paget’s细胞。术后7例失访，余16例随访时间最长13年，最短6月，其中3例出现术后远处转移。结论早期诊断及手术为主的综合治疗预后较佳。     

睾丸扭转诊治及探讨

目的:提高睾丸扭转的诊治水平,减少其误诊率。方法:对临床19例疑诊睾丸扭转病例进行总结,并针对病例分析。结果:2例经镇静治疗后自行复住。17例手术探查,15例证实为睾丸扭转,2例为急性附睾炎。15例睾丸扭转病人中10例为鞘膜外扭转,5例为鞘膜内扭转,发病至就诊时间2h～10d,彩色多普勒超声确诊率88.1%。19例均有睾丸绞痛症状,阴囊皮肤红肿.Prehn征阳性者为18例。结论:突发睾丸疼痛,尤其阴囊皮肤红肿者,应及早行彩色多普勒超声检查,疑为睾丸扭转者应尽早手术是避免睾丸坏死的关键。     

Pharmacotherapy of Paget's disease of bone

INTRODUCTION: Paget's disease of the bone is a common condition in some populations, affecting almost 10% of older adults in western Europe. Though frequently asymptomatic, it can sometimes result in severe disabilities. Timely intervention is now able to arrest disease progression in the majority of patients. AREAS COVERED: Studies describing the pharmacotherapy of Paget's disease are described in this review, based on a weekly Current Contents search for the term 'Paget' from 1995, and from personal knowledge and published references from earlier literature. EXPERT OPINION: Potent bisphosphonates are the cornerstone of current therapy and have been shown to produce biochemical remission, radiographic healing, pain relief and an improved quality of life. A single intravenous dose of zoledronate has produced the highest response rates and the most sustained remissions of any medication studied so far. It is not clear whether Paget's disease is cured in these patients, or whether they merely undergo a prolonged remission. As a result of this approach, we seldom need to retreat patients and our clinical experience is entirely consonant with the long-term follow-up results from the zoledronate trials, indicating that this treatment results in better clinical outcomes and greater quality of life for patients with Paget's disease.     

三联疗法治疗阴囊湿疹疗效观察

目的观察三联疗法治疗阴囊湿疹的疗效。方法将入选患者随机分为两组,治疗组（45例）予润燥止痒胶囊2.0g,3次/d,口服;联合马应龙麝香痔疮膏,外用,2次/d和曲安奈德益康唑乳膏,外用,2次/d;对照组（43例）：仅曲安奈德益康唑乳膏,外用,2次/d。两组均治疗2周。结果治疗组有效率为75.56%;对照组有效率为32.56%。两组比较差异有统计学意义（P〈0.05）。结论三联疗法治疗阴囊湿疹疗效显著,不良反应少。     

大汗腺癌伴乳房外Paget病一例

摘要：大汗腺癌伴乳房外 Paget 病临床罕见，本文通过回顾性分析 2013 年 5 月诊断的 1 例大汗腺癌伴乳房外 Paget病患者相关资料，探讨大汗腺癌伴乳房外Paget病的临床表现、组织病理改变及免疫组化染色特征表现。     

Zoledronic acid for Paget's disease of bone

Paget's disease of bone is a common skeletal disorder that often results in significant pain and disability. A number of bisphosphonates are currently available for treatment, however normalization of biochemical markers of bone turnover (the goal of treatment of Paget's disease) is rarely accomplished with currently available medications due to difficulty with administration, poor long-term adherence and possibly resistance to individual bisphosphonates. This review will focus on zoledronic acid, a potent intravenous bisphosphonate that appears to result in a higher percentage of patients with long-term normalization of markers of bone turnover than previously available drugs, thereby improving quality of life in patients with Paget's disease of bone.
