Goniometric reliability in a clinical setting. Shoulder measurements   总被引:5,自引:0,他引:5  
The purpose of this study was to examine the intratester and intertester reliabilities for clinical goniometric measurements of shoulder passive range of motion (PROM) using two different sizes of universal goniometers. Patients were measured without controlling therapist goniometric placement technique or patient position during measurements. Repeated PROM measurements of shoulder flexion, extension, abduction, shoulder horizontal abduction, horizontal adduction, lateral (external) rotation, and medial (internal) rotation were taken of two groups of 50 subjects each. The intratester intraclass correlation coefficients (ICCs) for all motions ranged from .87 to .99. The ICCs for the intertester reliability of PROM measurements of horizontal abduction, horizontal adduction, extension, and medial rotation ranged from .26 to .55. The intertester ICCs for PROM measurements of flexion, abduction, and lateral rotation ranged from .84 to .90. Goniometric PROM measurements for the shoulder appear to be highly reliable when taken by the same physical therapist, regardless of the size of the goniometer used. The degree of intertester reliability for these measurements appears to be range-of-motion specific.  相似文献

The impact upper extremity impairments (UE) have on UE function in breast cancer survivors (BCS) is unclear. The purpose of this study was to evaluate the associations between upper extremity active range of motion (AROM), passive range of motion (PROM), and strength with self-reported function in BCS. BCS (n = 24) completed the Disabilities of Arm, Shoulder and Hand (DASH) and the Pennsylvania Shoulder Score (PSS). AROM and PROM of shoulder flexion, extension, external rotation (ER) at 0° and 90° of abduction, and internal rotation (IR) at 90° of abduction were measured using a digital inclinometer. Strength was measured using a hand-held dynamometer for scapular abduction and upward rotation, scapular depression and adduction, flexion, IR, ER, scaption, and horizontal adduction. All constructs of AROM, PROM, and strength were correlated with the DASH and PSS. DASH was moderately to highly correlated with 2 of 5 AROM, 2 of 5 PROM, and 6 of 7 shoulder strength measures. PSS was moderately to highly correlated with 2 of 5 AROM, 2 of 5 PROM, and 4 of 7 shoulder strength measures. Regression analysis showed that AROM explained 40% of the DASH scores and strength explained 20% of scores on the PSS. This study characterizes the impact that shoulder motion, flexibility, and strength losses have on shoulder function in BCS. Deficits in AROM and shoulder strength explained the greatest proportion of shoulder disability. Future clinical trials should consider incorporating AROM and strengthening techniques to improve shoulder use after breast cancer treatments.  相似文献

[Purpose] The aim of the present study was to identify the effects of an unstable support surface (USS) on the activities of trunk and lower extremity muscles during pulley-based shoulder exercise (PBSE). [Subjects] Twenty healthy college students were included in this study. [Methods] Surface EMG was carried out in twenty healthy adult men. The activities of trunk and lower extremity muscles performed during PBSE using a resistance of 14 kg on a stable or unstable support surface were compared. The PBSE included shoulder abduction, adduction, flexion, extension, internal rotation, and external rotation. [Results] On the unstable surface, the rectus abdominis and erector spinae showed significantly less activation during shoulder external rotation, but the extent of activation was not significantly different during other shoulder exercises. The external oblique and rectus femoris showed no significant difference during any shoulder exercises. The tibialis anterior showed significantly greater activation during all shoulder exercises, except flexion and extension. The gastrocnemius showed significantly greater activation during shoulder abduction, extension, and internal rotation. However, during shoulder adduction, flexion, and external rotation, the gastrocnemius showed no significant difference. [Conclusion] The use of USS to increase core stability during PBSE is probably not effective owing to compensatory strategies of the ankle.Key words: Electromyography, Core stability, Ankle strategy  相似文献

目的:观察膏摩结合等速训练治疗肩周炎的临床疗效。方法:将60名肩周炎患者随机分为观察组和对照组各30例,2组均采用膏摩治疗,观察组加用等速肌力训练。观察2组患者治疗前后肩关节前屈、后伸,外展角度、力矩峰值及VAS评分变化。结果:治疗3周后,2组患者肩关节前屈、后伸、外展角度及力矩峰值均较治疗前明显提高(P0.05),且观察组更优于对照组(P0.05);2组VAS评分均较治疗前明显下降(P0.05),且观察组更低于对照组(P0.05)。结论:膏摩治疗配合等速训练能够更好的减轻肩周炎患者的疼痛,增加肩关节活动度,增加肌力,并藉此改善患者上肢运动功能。  相似文献

Decreased range of motion (ROM) due to disuse limits the older adult's ability to perform daily activities. The flexibility of five joints of 46 women (aged 65 to 89 years) was measured by goniometer before and after a 25-week exercise program. Three groups were formed: a control group who did no exercise (C) (n = 13), a group who exercised with light weights (EW) (n = 17), and a group who exercised with no weights (EN) (n = 16). Subjects who exercised (EN and EW) gained significantly greater ROM in ankle plantar flexion, shoulder flexion, shoulder abduction, and left neck rotation than C subjects. No significant differences were found between groups in hip flexion, right neck rotation, wrist flexion or extension, or ankle dorsiflexion. The only difference between exercise treatments was that the EN group gained significantly more ROM in shoulder abduction than the EW. The use of arm weights may have limited shoulder ROM during exercise, resulting in less improvement for shoulder abduction. Comparison with data from other studies suggests age-related loss in flexibility at the shoulder joint. Exercise generally increased shoulder ROM and may be capable of reversing loss in flexibility due to disuse.  相似文献

[Purpose] This study investigated the effect of hip position on muscle onset time during prone hip extension with knee flexion. [Subjects] The study included 21 healthy male volunteers. [Methods] Muscle onset times of the right gluteus maximus, right hamstrings, bilateral lumbar erector spinae, and bilateral lumbar multifidus were measured using surface electromyography during right hip extension with knee flexion in the prone position. Measurements were made with the hip in 3 positions: (1) neutral, (2) abduction, and (3) abduction and external rotation. [Results] Gluteus maximus onset relative to the hamstrings was significantly earlier with hip abduction and with hip abduction and external rotation compared with that with the hip in the neutral position. Gluteus maximus onset relative to the hamstrings was significantly earlier with hip abduction and external rotation compared with that with hip abduction. The bilateral multifidus and left lumbar erector spinae onset times relative to the hamstrings were significantly earlier with hip abduction and external rotation compared with those with hip abduction and with the hip in the neutral position. [Conclusion] Abduction and external rotation of the hip during prone hip extension with knee flexion is effective for advancing the onset times of the gluteus maximus, bilateral multifidus, and contralateral lumbar erector spinae.Key words: Prone hip extension with knee flexion, Hip joint position, Muscle onset time  相似文献

Background:Intrinsic factors including altered joint motion in the upper extremity may lead to altered biomechanics in tennis players and could result in symptoms of lateral elbow tendinopathy.Purpose:To compare upper extremity passive motion and elbow carrying angle between three groups of women: recreational tennis players with LET, non-symptomatic recreational tennis players, and a control group of non-tennis players.Study Design:Cross-sectional.Methods:A convenience sample of 63 women was recruited and placed into one of the three groups: non-symptomatic tennis players (NSTP), symptomatic tennis players (STP), and a control group. Elbow carrying angle, passive range of motion of the shoulder, elbow, forearm, and wrist were measured during a single session.Results:A significant difference was found between the groups for wrist flexion (p < 0.00), forearm pronation (p = 0.002), elbow flexion (p = 0.020) and extension (p = 0.460), as well as shoulder internal rotation (p < 0.00). No significant differences were found in other motions or carrying angle between the three groups (p =0.059). Post-hoc comparisons indicated that shoulder internal rotation and wrist flexion was less in both STP and NSTP groups compared with the control group. Elbow flexion and forearm pronation were greater in STP than the other two groups.Conclusion:Impairments including loss of shoulder internal rotation and wrist flexion and greater motion at the elbow and forearm were found in the UE of symptomatic tennis players. Evaluation of passive motion and muscle length should be performed prior to establishing a rehabilitation plan for symptomatic tennis players.Levels of Evidence:3  相似文献

This study was designed to quantify the range of upper limb joint motion required during the performance of a specific type of functional activity. Ten able-bodied men were studied as they performed three feeding tasks--eating with a spoon, eating with a fork, and drinking from a handled cup. Three shoulder joint rotations, one elbow joint rotation, one forearm joint rotation, and three wrist joint rotations were quantified simultaneously using a three-dimensional measurement system. It was found that the required ranges of motion for the feeding tasks were 5 degrees to 45 degrees shoulder flexion, 5 degrees to 35 degrees shoulder abduction, 5 degrees to 25 degrees shoulder internal rotation, 70 degrees to 130 degrees elbow flexion, from 40 degrees forearm pronation to 60 degrees forearm supination, from 10 degrees wrist flexion to 25 degrees wrist extension, and from 20 degrees wrist ulnar deviation to 5 degrees wrist radial deviation. Wrist rotation was also measured, but it was found to be negligible.  相似文献

[Purpose] This study aimed to identify correlations among pelvic positions and differences in lower extremity joint angles during walking in female university students. [Subjects] Thirty female university students were enrolled and their pelvic positions and differences in lower extremity joint angles were measured. [Methods] Pelvic position, pelvic torsion, and pelvic rotation were assessed using the BackMapper. In addition, motion analysis was performed to derive differences between left and right flexion, abduction, and external rotation ranges of hip joints; flexion, abduction, and external rotation ranges of knee joints; and dorsiflexion, inversion, and abduction ranges of ankle joints, according to X, Y, and Z-axes. [Results] Pelvic position was found to be positively correlated with differences between left and right hip flexion (r=0.51), hip abduction (r=0.62), knee flexion (r=0.45), knee abduction (r=0.42), and ankle inversion (r=0.38). In addition, the difference between left and right hip abduction showed a positive correlation with difference between left and right ankle dorsiflexion (r=0.64). Moreover, differences between left and right knee flexion exhibited positive correlations with differences between left and right knee abduction (r=0.41) and ankle inversion (r=0.45). [Conclusion] Bilateral pelvic tilt angles are important as they lead to bilateral differences in lower extremity joint angles during walking.Key words: Pelvic position, Lower extremity joint angle, Walking  相似文献

ObjectivesTo determine whether ballistic resistance training is feasible, safe, and effective in improving muscle strength, power generation, and mobility in adults with neurologic conditions.Data SourcesNine electronic databases were searched from inception to March 2019 in addition to the reference lists of included articles.Study SelectionArticles were independently screened by 2 authors and were included if they were full-text; English-language articles published in a peer-reviewed journal; investigated ballistic resistance training for adults with a neurologic condition; and reported on feasibility, safety, strength, power, or mobility.Data ExtractionTwo authors independently extracted data. Study quality was assessed using the McMaster critical review form and the Physiotherapy Evidence Database scale.Data SynthesisThe search identified 1540 articles, with 13 articles describing 9 studies meeting the criteria for inclusion. Five studies were randomized controlled trials and 4 were cohort studies. Ballistic resistance training was feasible and safe with only 1 intervention-related adverse event reported. Findings indicated improvements in strength for hip abduction, leg press, knee flexion, and ankle dorsiflexion, but not for hip flexion, hip extension, knee extension, or ankle plantarflexion. Muscle power generation improved for hip flexion, hip abduction, leg press, knee extension, and knee flexion, but not for ankle plantarflexion. Treatment effect was positive for self-selected walking speed, with a standardized mean difference (SMD) of 0.69 (95% confidence interval [CI], 0.01-1.38) from 3 studies. However, fastest comfortable walking speed results were inconclusive with a SMD from 4 studies of 0.45 (95% CI, –0.01 to 0.91).ConclusionsBallistic training is safe and feasible for people with a neurologic condition. The effects on muscle strength, power generation, and mobility were found to be positive but not conclusive.  相似文献

OBJECTIVES: To identify the etiology of hemiplegic shoulder pain by arthrographic and clinical examinations and to determine the correlation between arthrographic measurements and clinical findings in patients with hemiplegic shoulder pain. DESIGN: Case series. SETTING: Medical center of a 1582-bed teaching institution in Taiwan. PARTICIPANTS: Thirty-two consecutive patients with hemiplegic shoulder pain within a 1-year period after first stroke were recruited. INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Clinical examinations included Brunnstrom stage, muscle spasticity distribution, presence or absence of subluxation and shoulder-hand syndrome, and passive range of motion (PROM) of the shoulder joint. Arthrographic measurements included shoulder joint volume and capsular morphology. RESULTS: Most patients had onset of hemiplegic shoulder pain less than 2 months after stroke. Adhesive capsulitis was the main cause of shoulder pain, with 50% of patients having adhesive capsulitis, 44% having shoulder subluxation, 22% having rotator cuff tears, and 16% having shoulder-hand syndrome. Patients with adhesive capsulitis showed significant restriction of passive shoulder external rotation and abduction and a higher incidence of shoulder-hand syndrome (P=.017). Those with irregular capsular margins had significantly longer shoulder pain duration and more restricted passive shoulder flexion (P=.017) and abduction (P=.020). Patients with shoulder subluxation had significantly larger PROM (flexion, P=.007; external rotation, P<.001; abduction, P=.001; internal rotation, P=.027), lower muscle tone (P=.001), and lower Brunnstrom stages of the proximal upper extremity (P=.025) and of the distal upper extremity (P=.001). Muscle spasticity of the upper extremity was slightly negatively correlated with shoulder PROM. Shoulder joint volume was moderately positively correlated with shoulder PROM. CONCLUSIONS: After investigating the hemiplegic shoulder joint through clinical and arthrographic examinations, we found that the causes of hemiplegic shoulder pain are complicated. Adhesive capsulitis was the leading cause of shoulder pain, followed by shoulder subluxation. Greater PROM of the shoulder joint, associated with larger joint volume, decreased the occurrence of adhesive capsulitis. Proper physical therapy and cautious handling of stroke patients to preserve shoulder mobility and function during early rehabilitation are important for a good outcome.  相似文献

[Purpose] The purpose of this study was to compare the intramuscular balance ratios of the upper trapezius muscle (UT) and the lower trapezius muscle (LT), and the intermuscular balance ratios of the UT and the serratus anterior muscle (SA) among prone extension (ProExt), prone horizontal abduction with external rotation (ProHAbd), forward flexion in the side-lying position (SideFlex), side-lying external rotation (SideEr), shoulder flexion with glenohumeral horizontal abduction load (FlexBand), and shoulder flexion with glenohumeral horizontal adduction load (FlexBall) in the standing posture. [Methods] The electromyographic (EMG) activities of the UT, LT and SA were measured during the tasks. The percentage of maximum voluntary isometric contraction (%MVIC) was calculated for each muscle, and the UT/LT ratios and the UT/SA ratios were compared among the tasks. [Results] The UT/LT ratio with the FlexBand was not significantly different from those of the four exercises in the side-lying and prone postures. The UT/SA ratio with the FlexBall demonstrated appropriate balanced activity. [Conclusion] In an anti-gravity posture, we recommend the FlexBand and the FlexBall for inducing balanced UT/LT and UT/SA ratios, respectively.Key words: Scapular muscle exercise, Balanced scapular muscle activity, Electromyography  相似文献

[Purpose] This study investigated the selective activation of the gluteus maximus during a prone hip extension with knee flexion exercise, with the hip joint in different positions. [Subjects] The subjects were 21 healthy, male volunteers. [Methods] Activities of the right gluteus maximus, right hamstrings, bilateral lumbar erector spinae, and bilateral lumbar multifidus were measured using surface electromyography during a prone hip extension with knee flexion exercise. Measurements were made with the hip joint in each of 3 positions: (1) a neutral hip joint position, (2) an abduction hip joint position, and (3) an abduction with external rotation hip joint position. [Results] Gluteus maximus activity was significantly higher when the hip was in the abduction with external rotation hip joint position than when it was in the neutral hip joint and abduction hip joint positions. Gluteus maximus activity was also significantly higher in the abduction hip joint position than in the neutral hip joint position. Hamstring activity was significantly lower when the hip was in the abduction with external rotation hip joint position than when it was in the neutral hip joint and abduction hip joint positions. OBJECTIVE: To determine normative values for isometric flexion/extension, abduction/adduction, and external/internal rotation strength ratios about the shoulder and to determine if these ratios are affected by age or gender. STUDY DESIGN: A cross-sectional study of 120 healthy volunteers (60 men, 60 women) aged 20 to 78 years. SETTING: Orthopedic research laboratory. METHODS: Flexion and extension strengths were measured isometrically using a Cybex II dynamometer at arm flexion angles of 30 degrees, 60 degrees, and 90 degrees. Abduction and adduction strengths were measured at 30 degrees, 60 degrees, and 90 degrees abduction. Internal and external rotation strengths were measured (1) with the arm abducted 15 degrees and neutral external/internal rotation and (2) with the arm abducted 90 degrees and externally rotated 30 degrees above the transverse plane. OUTCOME MEASURES: Isometric strength ratios for flexion/extension, abduction/adduction, and external/internal rotation. RESULTS: No statistically significant differences in agonist/antagonist strength ratios were found between dominant and nondominant sides or between genders. Age was associated with changes in strength ratios for measurements taken with the arm flexed or abducted 90 degrees. Posture was found to affect strength ratios. CONCLUSIONS: These data can serve as a normative reference for clinical use.  相似文献

Background

The varied roles of the subscapularis muscle as an internal rotator of the humerus, a shoulder abductor, a humeral head depressor and an anterior stabiliser may be a result of differing innervation and lines of torque between its superior and inferior components. The aims of the study were to investigate the differences in the level of muscle activation between the upper and lower subscapularis during abduction, flexion, internal and external rotation movements, and temporal characteristics during abduction and flexion.

Methods

Intramuscular electrodes recorded electromyographic muscle activity from the upper and lower subscapularis muscles of the dominant throwing arm of twenty-four normal subjects. Participants completed ten repetitions of four shoulder movements — abduction, flexion, internal rotation and external rotation. Muscle activity was expressed as a percentage of maximum voluntary isometric contraction.

Findings

The lower subscapularis was found to activate at a higher level than the subscapularis during abduction, flexion and external rotation movements and this was significant during concentric and eccentric phases of abduction and flexion (< 0.001). During internal rotation, upper subscapularis muscle activity mirrored that of lower subscapularis, with a mean difference of 1.14%. Neither upper nor lower subscapularis had onset data commencing prior to the abduction movement; however upper subscapularis activated significantly later than lower subscapularis (P = 0.018).

Interpretation

The lower subscapularis has significantly higher muscle activity during shoulder elevation and this might reflect its greater role as a humeral head depressor and anterior stabiliser.  相似文献

In isokinetic muscle testing of the shoulders of 31 normal volunteers, no statistical difference was found between dominant and nondominant shoulders, even though there was a consistent pattern of greater strength in the dominant shoulder. Internal rotation strength was greater than external rotation (p less than 0.01) by the ratio of 3:2 for both the fast and slow torque arm speed. Extension strength was greater than flexion (p less than 0.01) by a ratio of 5:4 at both speeds. Adduction strength was greater than abduction (p less than 0.01) by a ratio of 2:1 for both speeds. Overall, adduction strength was greatest, followed by extension, flexion, abduction, internal rotation, and external rotation. Male strength was greater than female, but the advantage decreased when normalized for lean body mass and exercise habit.  相似文献

[Purpose] This study aimed to clarify the relationship between the distance measurements in the Star Excursion Balance Test and participants’ posture and lower limb muscle strength. [Participants and Methods] Nine healthy male college students participated in this study. Star Excursion Balance Test distance was measured in both lower limbs by performing anterior, posterolateral, and posteromedial trials; measuring the maximum reach; and performing three-dimensional motion analysis to determine the posture at maximum reach. Isokinetic muscle strength for knee flexion/extension, hip flexion/extension, and hip adduction/abduction were measured using an isokinetic machine. [Results] The hip extension strength, reach side ankle dorsiflexion angles, stance side knee flexion, reach side knee flexion, and knee flexion strength were selected as significant explanatory variables in the anterior direction. For the posteromedial direction, hip adduction and hip extension strength, reach side hip flexion angle, and stance side hip flexion angle were selected. For the posterolateral direction, reach side knee flexion angle and stance side ankle dorsiflexion, knee flexion strength and reach side hip flexion angle were selected. [Conclusion] The related factors differed between the dominant and non-dominant legs even in the same reach direction.  相似文献

Limited range of motion (ROM) and subsequent soft tissue contractures resulting from spasticity are common developments in patients with acquired hemiplegia from cerebrovascular accidents. The purpose of this study was to examine the effects of a wrapping technique on ROM in patients with a spastic upper extremity. Four adult patients, postcerebrovascular accident of less than one year, received wrapping of the upper extremity for three hours, three times a week on alternating days for a total of two to four weeks. Baseline passive range-of-motion (PROM) measurements were established for shoulder flexion, shoulder abduction, shoulder external rotation, and wrist extension; these movements were commonly limited in all patients. The PROM was recorded after each wrapping session. The results showed a significant change in PROM for all motions in all patients (p = less than .01). In addition, all patients reported a decrease in pain in the upper extremity. Comparisons of videotapes of two patients at baseline and after 12 wrappings revealed an increase in ROM and a decrease in spasticity in the upper extremity during ambulation. These findings should encourage clinicians to experiment further with the wrapping technique and to report their findings.  相似文献

[Purpose] This study aimed to clarify the differences in scapular movement during flexion and abduction of the shoulder joint with different postures. [Participants and Methods] This study included 15 male participants. Their shoulder flexion and abduction and angles of the scapular upward rotation, scapular anterior tilt, scapular external rotation, and thoracic spine flexion were measured. Measurements were taken in three positions: the control, thoracic spine flexion, and thoracic spine extension positions using a three-dimensional motion capture system. [Results] In the shoulder flexion, the amount of change in the scapular external rotation was significantly greater in the thoracic flexion than in the thoracic extension. In shoulder abduction, the amount of change in the scapular anterior tilt and external rotation was significantly greater in the thoracic flexion than in the thoracic extension. A comparison of the scapular angles in shoulder flexion and abduction showed that the upward rotation, posterior tilt, and external rotation were significantly greater in abduction than flexion. [Conclusion] To avoid posture-induced incoordination of the scapula and thorax movement during shoulder elevation, postural adjustment of the thoracic spine based on the movements is necessary for the shoulder joint exercises.  相似文献
