Objectives

The aims of our study were to determine levels of trefoil factor (TFF) peptides in saliva and oral mucosal tissues from patients with oral squamous cell carcinoma (OSCC), and to evaluate whether individual members of TFFs (TFF1, TFF2, and TFF3) might act as biomarkers of disease.

Materials and methods

Saliva samples were from 23 healthy subjects and 23 OSCC patients. Tissue samples were collected from 32 normal oral mucosa (NOM) and 32 OSCC biopsy specimens. ELISA and immunohistochemical methods were used to evaluate the expression of TFF1, TFF2, and TFF3 in saliva and oral mucosal tissues, respectively.

Results

Expression of TFF2 and TFF3 in oral mucosal tissues of OSCC patients was strongly downregulated when compared to healthy subjects (p?<?0.001 and p?=?0.002, respectively). However, there were no differences in levels of salivary TFF concentrations between OSCC patients and healthy subjects.

Conclusions

The present study extends previous observations, demonstrating the reduction of TFF2 and TFF3 expression in oral mucosal tissues of OSCC patients.

Clinical relevance

These findings suggest the clinical significance of TFF2 and TFF3 molecules as negative markers of tumor progression in OSCC. Quantification of TFF levels in saliva may not be optimal in terms of diagnostic or predictive value for OSCC derived from oral mucosa.  相似文献

Background

It has been estimated that 15%-20% of human tumours are driven by infection and inflammation, and viral infections play an important role in malignant transformation. The evidence that herpes simplex virus type 1 (HSV-1) could be involved in the aetiology of oral cancer varies from weak to persuasive. This study aimed to investigate by nested PCR (NPCR) the prevalence of HSV-1 in samples from normal oral mucosa, oral leukoplakia, and oral squamous cell carcinoma (OSCC).

Material and Methods

We investigated the prevalence of HSV-1 in biopsies obtained from 26 fresh, normal oral mucosa from healthy volunteers as well as 53 oral leukoplakia and 27 OSCC paraffin-embedded samples. DNA was extracted from the specimens and investigated for the presence of HSV-1 by nested polymerase chain reaction (NPCR) and DNA sequencing.

Results

HSV-1 was detected in 14 (54%) of the healthy samples, in 19 (36%) of the oral leukoplakia samples, and in 14 (52%) of the OSCC samples. The differences were not statistically significant.

Conclusions

We observed a high incidence of HSV-1 in healthy oral mucosa, oral leukoplakia, and OSCC tissues. Thus, no connection between OSCC development and presence of HSV-1 was detected. Key words:HSV-1, nested PCR, PCR.  相似文献

Objectives

The purpose of this prospective clinical study was to identify the bacterial spectra on the surface of oral squamous cell carcinomas (OSCC) in comparison to oral mucosa of patients with a higher risk to emerge an OSCC and a control group to determine their susceptibility to various common antibiotics.

Material and methods

Swabs from 90 patients, 30 patients of each group, were cultured on media for aerobes and anaerobes and tested with agar diffusion and Etest.

Results

The predominant pathogens of the normal healthy oral mucosa were aerobes. The ratio between aerobes and anaerobes was 2:1, balanced in risk patients and inverted in the OSCC group. Altogether, 1,006 isolates were cultured. The most frequent strains were 47 viridans streptococci, 30 Staphylococcus species, 14 Enterococcus faecalis, 36 Neisseria species, 14 Escherichia coli, and 23 other aerobes, 66 Peptostreptococcus species, 39 Fusobacterium species, and 34 Prevotella species. The resistance rates in the OSCC group were penicillin 40 %, ampicillin 57 %, doxycycline 23 %, clindamycin 47 %, and amoxicillin/clavulanic acid 20 %, but up to 100 % of pathogens were susceptible to azithromycin, telithromycin, levofloxacin, and moxifloxacin.

Conclusion

Gram-negative anaerobes play a decisive role in the development of postoperative infections in patients with OSCC. This tumor special type of colonization does not agree with the normal flora of the oral cavity. Clinical relevance: Biofilms on OSCC surfaces provide an important reservoir for anaerobic bacteria. As a consequence, a proposal for an antibiotic prophylactic regime should be given.  相似文献

Background. The effectiveness of folic acid supplementation in the periconceptional period for the prevention of cleft lip/cleft lip and palate (CLP) is contradictorily discussed. Genetically determined variants of enzymes of the folic acid metabolism could be part of the key to success or failure of folate supplementation. A mutation of the methylenetetrahydrofolate reductase (MTHFR) gene is suspected to be a risk factor for CLP. Methods. The blood samples of 66 CLP patients, their 88 relatives (without CLP), and 184 healthy controls were searched by polymerase chain reaction for mutations of MTHFR 677 C:T, MTHFR 1298 A :C and of the arylamine N-acetyltransferase (NAT1) gene [gene type NAT1°4 (wild type) or not]. Results. There was no significant difference in the number of MTHFR gene mutations (for 677 C:T and 1298 A :C) between the three groups (p~0.3), but for the NAT1 genes (p=0.033). The homozygote mutation was found more than twice as often in CLP patients (10.5%) and their relatives(10.6%) than in the healthy controls (4.35%). Discussion. Our results provide no evidence that the above MTHFR gene mutations are a risk factor for CLP. A NAT1 gene mutation instead could be a risk factor for CLP.  相似文献

Objective

The aim of this study was to investigate whether tumor-associated tissue eosinophilia (TATE) in early oral squamous cell carcinoma (OSCC) would aid in predicting occult lymph node metastasis.

Patients and methods

Seventy-one patients undergoing elective neck dissection for T1 and T2 OSCC were evaluated for clinical features, prognosis, and TATE. The degree of TATE in OSCC was statistically analyzed in relation to the clinicopathological features, tumor invasion, occult lymph node metastasis, and survival using χ 2 test and Kaplan–Meier method.

Results

Statistical analysis revealed that intense TATE was a significant feature (p?=?0.004) to predict occult lymph node metastasis in patients with early OSCC. All regional recurrences of the OSCC occurred in patients showing intense TATE.

Conclusions

These results suggest that intense TATE can be clinically used as a predictive factor for occult lymph node metastasis.

Clinical relevance

The presence of intense TATE is an adjunctive histopathological marker to reinforce the indication of elective neck dissection of the patients with early OSCC.  相似文献

Objectives

Overexpression of the histamine H1 receptor (H1R) has been described in a variety of tumor models, but experience in oral squamous cell carcinomas (OSCC) is not available. Current adjuvant treatment options for OSCC can be improved by the identification of new targets of therapy. Herein, we evaluated H1R expression in a large patient cohort of OSCC.

Materials and methods

H1R immunoexpression was evaluated in 191 cases of OSCC and two OSCC cell lines BICR56 and BICR3. Scanned images were digitally analyzed using ImageJ and the immunomembrane plug-in. The combined score of computer-assisted semiquantitative analysis was correlated with manually counted percentages of tumor cells by Kendall’s tau (т) correlation coefficient. Disease-free survival times were estimated using the Kaplan–Meier method and were compared by using the log-rank test. Multivariate analyses were performed using the Cox proportional hazards model.

Results

H1R was rarely expressed in OSCC but significantly related with advanced tumor stages (n?=?21/191, mean expression 63.5 % of cancer cells in positive tumor samples, 95 % confidence interval of the mean 53.5 to 73.6 %, p?=?0.006). Following univariate analysis, patients with H1R expression showed a significant poorer prognosis (p?=?0.0004). Multivariate analysis revealed H1R expression as an independent prognostic factor (p?=?0.0164). Expression of H1R in cancer cell lines was confirmed by specific staining of OSCC cell lines BICR56 and BICR3.

Conclusion

This is the first study focusing on H1R expression showing a significant poorer DFS rate in the H1R+ patient cohort. Based on these data, H1R activation may promote carcinogenesis in OSCC.

Clinical relevance

Investigation of H1R regulation and its antagonists shows a clear rationale for future supportive anticancer therapies in OSCCs.  相似文献

Objectives

Albumin has a known capability to modulate free serum concentrations of proteins produced by tumour cells. The technique of spin probe labelling of albumin followed by electron paramagnetic resonance (EPR) spectroscopy may allow identification of these structural and functional changes, which regularly occur as consequence of binding tumour metabolites as ligands. The aim of the present study was a proof of principle evaluation of EPR-analysis of peripheral blood samples as possible predictor for oral squamous cell carcinoma (OSCC).

Material and methods

The present study is designed as gender-matched cohort. EPR was tested after retrieval of peripheral blood samples. The study group is represented by 32 patients with OSCC, and the control group consisted of 30 healthy patients.

Results

Overall analysis exhibited a diagnostic sensitivity of 72% (23/32 OSCC group) and a specificity of 80% (24/30 control group). Subgroup analysis revealed ten patients with elevated leukocytes (>10,000/μl; n?=?9 [OSCC group] and n?=?1 [control group]). After exclusion of patients with elevated white blood cell count, sensitivity considerably increased to 87% and specificity to 83%.

Conclusion

EPR analysis of peripheral blood samples might be appropriate to support the clinician in primary and follow-up diagnosis of potential tumours such as OSCC. Unfortunately, subgroup analysis characterises the method vulnerable to inflammation.

Clinical relevance

Nevertheless, our preliminary results are intriguing, as diagnosis of OSCC appears possible by simple peripheral blood examination. Thus, further appraisal of this novel method with inclusion of different tumour entities, systemic conditions and inflammation in a larger study population appears highly valuable.  相似文献

Background

Interleukin 8 (IL-8) is a pro-angiogenic, pro-inflammatory mediator that belongs to the family of chemokines. Due to its pro-angiogenic characteristic, it may play a vital role in tumour angiogenesis and progression.

Objectives

This study was designed to estimate the levels of salivary IL-8 in oral precancer and oral squamous cell carcinoma (OSCC) patients and compare them with healthy controls. The aim was to evaluate its efficacy as a potential biomarker for these diseases.

Materials and methods

Each group comprised 25 individuals. The salivary IL-8 levels were determined by enzyme-linked immunosorbent assay.

Results

The levels of salivary IL-8 were found to be significantly elevated in patients with OSCC as compared to the precancer group (p?<?0.0001) and healthy controls (p?<?0.0001). However, the difference in salivary IL-8 concentrations among the precancer group and controls was statistically non-significant (p?=?0.738).

Conclusions

Our results suggested that salivary IL-8 can be utilised as a potential biomarker for OSCC. Salivary IL-8 was found to be non-conclusive for oral premalignancy in this preliminary study. Hence, its possible role in transition from premalignancy to malignancy needs further research with larger sample sizes.

Clinical relevance

Saliva as a diagnostic biofluid offers a number of advantages over blood-based testing. The role of IL-8 in oral cancer if validated further by future research can provide an easy diagnostic test as well as a prognostic indicator for patients undergoing treatment. Therefore, if it’s role in tumourigenesis can be sufficiently assessed, it could open up new avenues to find out novel treatment modalities for oral cancer.  相似文献

Objectives

The β1 integrin (CD29) is a putative marker for cancerous epithelial stem cells. Cancer stem cells are essential to drive tumor growth, recurrence, and metastasis. We investigated the role of β1-integrin expression in the development of malignant phenotypes of oral squamous cell carcinoma (OSCC).

Materials and methods

Immunostaining was used to analyze the expression levels of β1 integrins in different types of cell colonies and tumor spheres. The results of cell viability and migration assays with and without siRNA knockdown of β1-integrin expression were compared. Cells expressing β1 integrins were evaluated for their tumorigenicity in mice. The expression of β1 integrins in human specimens of oral cancers at different clinical stages was semiquantified based on immunohistochemical staining of the β1-integrin protein.

Results

The expression level of β1 integrins in Meng-1 oral epidermoid carcinoma cells (OECM-1) cells was significantly higher in holoclonal colonies and tumor spheres compared to control cells. The knockdown of β1-integrin expression in OECM-1 cells reduced cell proliferation, migration, and tumor sphere formation. Beta-1 integrin (+) cells were more tumorigenic in the mouse xenograft model than β1 integrin (?) cells. In the human specimens, the expression level of the β1-integrin protein positively correlated with the clinical stage.

Conclusion

The expression of β1 integrin in OECM-1 cells is involved in the development of malignant phenotypes of OSCC.

Clinical relevance

Inhibitors for β1-integrin signaling may be suitable to become target-specific therapies for OSCC.  相似文献

Background

To analyze the presence of salivary Epstein-Barr virus (EBV) DNA in oral squamous cell carcinoma and potentially malignant oral disorders.

Material and Methods

Three groups were studied: Group 1 (12 oral squamous cell carcinomas (OSCC)), Group 2 (12 potentially malignant oral disorders (PMD)) and Group 3 (47 healthy controls). EBV DNA salivary analysis was performed by PCR.

Results

The highest percentage of positive salivary EBV DNA corresponded to the OSCC group (58.3%), followed by the PMD group (41.7%) and the controls (40.4%). The differences between groups were not statistically significant, however (p>0.05).

Conclusions

Salivary EBV DNA was more prevalent in OSCC than in PMD or the controls. Key words:EBV DNA, saliva, oral squamous cell carcinoma, oral leukoplakia.  相似文献

Objectives

Incontinentia pigmenti (IP) is an X-linked genodermatosis caused by a mutation of the IKBKG gene. The objective of this study was to present a systematic review of the dental and oral types of anomalies, to determine the total number and sex distribution of the anomalies, and to analyze possible therapies.

Materials and methods

We analyzed the literature data from 1,286 IP cases from the period 1993–2010.

Results

Dental and/or oral anomalies were diagnosed for 54.38% of the investigated IP patients. Most of the anomaly types were dental, and the most frequent of these were dental shape anomalies, hypodontia, and delayed dentition. The most frequent oral anomaly types were cleft palate and high arched palate. IKBKG exon 4–10 deletion was present in 86.36% of genetically confirmed IP patients.

Conclusions

According to the frequency, dental and/or oral anomalies represent the most frequent and important IP minor criteria. The most frequent mutation was IKBKG exon 4–10 deletion. The majority of dental anomalies and some of the oral anomalies could be corrected.

Clinical relevance

Because of the presence of cleft palate and high arched palate in IP patients, these two anomalies may be considered as diagnostic IP minor criteria as well.  相似文献

Purpose. The purpose of simultaneous chemoradiotherapy is to increase local-regional control and to decrease the incidence of distant metastases. Regimens containing cisplatin/5-FU chemotherapy are widely accepted as standard treatment in advanced head and neck cancer. Most studies reported promising response and survival data, but also severe mucosal toxicity. In recent years the newly developed drug Taxol demonstrated interesting activity in head and neck cancer as a single agent as well as in combination drug regimens. In the present outpatient phase II trial, we investigated the combination of Taxol/carboplatin with 40 Gy radiotherapy in a neoadjuvant setting of operable stage III/IV squamous cell carcinoma of the oral cavity and oropharynx. Patients and methods. Fifty-three patients were enrolled in this trial during the period from May 1998 to October 2000 and received five cycles weekly of Taxol (40 mg/m2) and carboplatin (AUC 1,5) with conventional radiotherapy (40 Gy). Within 3–4 weeks after chemoradiotherapy resection of the primary tumor and the regional neck nodes was performed. Results. Fifty-two patients were evaluable for toxicity and response. Complete response was observed in 31 of 52 patients (CR 60%), and partial remission was seen in 21 of 52 patients (PR 40%). In 30 of 52 patients complete pathologic response (pCR 58%) was documented in the resection specimens. The 1-, 2-, and 3-year overall survival rate was calculated as 84%. Conclusion. Our present results demonstrated impressive clinical and pathological response rates of concurrent Taxol/carboplatin and radiotherapy as a preoperative treatment modality in advanced oral and oropharyngeal cancer.  相似文献

Objectives

To gather preliminary data concerning the feasibility of using seven salivary mRNAs—IL-8; IL-1β; dual specificity phosphatase 1 (DUSP1); H3 histone family 3A (H3F3A); ornithin decarboxylase antizyme 1 (OAZ1); S100 calcium-binding protein P (S100P); and spermidine/spermine N1-acetyltransferase 1 (SAT1)—for detecting development of oral squamous cell carcinoma (OSCC) in oral lichen planus (OLP) patients and OSCC patients whose disease was in remission.

Materials and methods

Saliva samples were collected from five study groups (25 subjects/group): newly diagnosed OSCC, OSCC-in-remission, disease-active OLP, disease-inactive OLP, and normal controls. The salivary mRNA levels were determined by a pre-amplification RT-qPCR approach with nested gene-specific primers. Mean fold changes between each pair of study groups were analyzed by the Mann–Whitney U test.

Results

Salivary levels of OAZ1, S100P, and DUSP1 mRNAs were significantly higher in newly diagnosed OSCC patients, compared to: (1) normal controls (p?=?0.003; p?=?0.003; and p?<?0.001, respectively); (2) OSCC-in-remission (p?<?0.001; p?=?0.001; and p?<?0.001, respectively); (3) disease-active OLP (p?<?0.001; p?=?0.016; and p?<?0.001, respectively); and (4) disease-inactive OLP (p?=?0.043; p?<?0.001; and p?<?0.001, respectively). No significant differences were found in the levels of salivary IL-8, IL-1β, H3F3A, and SAT1 mRNAs between newly diagnosed OSCC patients and the normal controls (p?=?0.093, 0.327, 0.764, and 0.560, respectively).

Conclusion

Salivary OAZ1, S100P, and DUSP1 mRNAs are candidate biomarkers for detecting OSCC development in OSCC patients in remission and in OLP patients.

Clinical relevance

The results of this study serve as the basis for a further large-scale study which may lead to a non-invasive screening method for early detection of OSCC.  相似文献

Background. Tissue engineering of autogenous oral mucosa is a significant enrichment for various indications in oral and maxillofacial surgery. Besides preprosthetic surgery and prelamination of microvascular fasciocutaneous flaps, interdisciplinary work such as reconstruction procedures in cases of hypospadias and epispadias are the main indications for tissue-engineered oral mucosa. Results. One main advantage of this technique is the reduction of second surgical procedures. The morbidity is decreased and the quality of life may be significantly increased. The manufacture of tissue-engineered autogenous oral mucosa up to 15 cm2 requires a small biopsy of oral mucosa (4–8 mm3) and 40 ml of autogenous serum. Discussion. The clinical and morphological long-term follow-up after different surgical procedures with tissue-engineered oral mucosa establishes this technique as an excellent additional tool in oral and maxillofacial surgery.  相似文献

Aim. The aim of the present study was to investigate the effects of the oxygen-carrying hemoglobin solution DCLHb (diaspirin-crosslinked hemoglobin) on microvascular perfusion and tissue oxygenation in striated skin muscle after the induction of critical ischemia followed by reperfusion. Material and Methods. Using intravital fluorescence microscopy the functional capillary density was analyzed in the striated skin muscle of Syrian golden hamsters before the induction of a 4-h period of ischemia and again after 0.5 h, 2 h and 24 h of reperfusion (n=8 in each group). In other animals (n=8 in each group), the identical protocol was applied to determine tissue oxygenation by means of the multi-wire surface electrode (MDO, Eschweiler, Kiel, Germany). Animals in the treatment group (n=8) received a bolus infusion of 5 ml of DCLHb per kg of body wt. (10 g/dl; Baxter, Ill., USA) 15 min before reperfusion. Control animals (n=8) received equivalent volumes of isotonic saline (Braun, Melsungen, Germany). Results. Functional capillary density was dramatically reduced in control animals, while in DCLHb-treated animals significantly higher values were observed. Efficient restoration of tissue PO2 was also seen in DCLHb-treated animals and not in control animals. Conclusions. These results show that the oxygen-carrying solution DCLHb is significantly more efficient than the commonly used crystalloid solutions in restoration of tissue PO2 after ischemia–reperfusion. The use of this solution therefore appears promising as a means of protecting the tissue put at risk by ischemia from reperfusion damage.   相似文献

Introduction

The human epidermal growth factor receptor (her-2/neu) protooncogene encodes a membrane tyrosine kinase with homology to the epidermal growth factor receptor (EGFR). Amplification and protein overexpression have been identified in various solid tumors and a significant association with poor clinical outcome was reported. This investigation was performed to assess the frequency of her-2/neu overexpression and to compare these results with clinical outcome in OSCC.

Material and methods

Archival biopsy specimens from 97 untreated OSCCs were evaluated using a polyclonal antibody A0485 (Dako). Only membrane staining intensity and pattern were evaluated according to the guidelines of the clinical trial assay recommendations (0–3+) for breast carcinoma. Score 0 and 1+ were interpreted as negative for HER-2/NEU protein overexpression and 2+ and 3+ as positive. FISH analysis with directly labeled probes for her-2/neu and chromosome 17 was performed on the same specimens. The ratio between her-2/neu and chromosome 17 signals was calculated after selection of 20–40 non-overlapping tumor cells. The tumor was considered amplified if the ratio was above 2.

Results

In 11 out of 97 biopsies (11.3%) membranous overexpression (score 2+ and 3+) of her-2/neu was shown by immunohistochemistry. FISH analysis in 42 cases revealed amplification in 14 cases. Concordance between immunohistochemistry and FISH was found in 86%. Clinical-pathological data as well as survival revealed no correlation with her-2/neu status.

Discussion

In spite of missing correlation between survival and her-2/neu overexpression in our study, the predictive value of the her-2/neu protooncogene in adjuvant therapy in OSCC needs further investigation.   相似文献

Objectives

Gene products, which show a significant association to cell proliferation and cell cycle control, are of high scientific interest, because genes as well as gene products could be possible targets for a specific therapeutic approach and eventually be prognostic markers.

Materials and methods

Cyclin D1 expression and amplification as well as the Ki-67 expression status were examined in a two tissue microarray analysis for head and neck squamous cell carcinoma (HNSCC) including 546 patients. A tumour site-specific analysis and a survival analysis of 222 oral squamous cell carcinoma (OSCC) patients were performed. Cyclin D1 amplification status was examined with fluorescence in situ hybridisation analysis, while cyclin D1 expression and Ki-67 expression status were examined with IHC.

Results

Amplification of the CCND1 gene and immunohistochemical expression of cyclin D1 and Ki-67 were examined in 546 tumours of the head and neck region in two tissue microarrays. CCND1 amplification was significantly more frequent in pharyngeal carcinomas (63 %) than in laryngeal (37 %) and oral (25 %) carcinomas. Among the 222 cases of OSCCs, both CCND1 amplification and cyclin D1 expression were significantly associated with overall survival of the patients (p?=?0.0127 and p?=?0.0004, respectively). Ki-67 expression was significantly associated with cyclin D1 expression and with amplification of the CCND1 gene (p?=?0.0002 and p?=?0.0015, respectively) but not with patient overall survival.

Conclusion

Our results suggest the prognostic value of CCND1 amplification and cyclin D1 expression for patients with OSCC and highlight the genetic differences in HNSCC of different subanatomic localisation.

Clinical relevance

Cyclin D1 expression and CCND1 amplification seem to have a prognostic value for OSCC. Further studies of HNSCC should always consider subanatomic genetic differences.  相似文献

Background. In our department panendoscopic examinations of the tracheobronchial system, the epi-, meso,- and hypopharynx, and the upper gastrointestinal tract are obligatory with regard to the staging of oral squamous cell carcinomas, the main interest being the exclusion of synchronous secondary carcinomas. The aim of this study was to present the pathological findings in 160 panendoscopies of patients with previously untreated oral squamous cell carcinomas. Study. A total of 17 synchronous carcinomas were found in 13 (8.2%) of all cases studied. They were situated in the esophagus (n=9), the stomach (n=1), the duodenum (n=1), the hypopharynx (n=1), the trachea (n=1), and the lung (n=4). Multiple carcinomas were found in three patients. The survival time of patients with a secondary carcinoma proved to be significantly worse than that of the other patients. The abuse of alcohol and tobacco causes a high amount of inflammatory changes in the aerodigestive system. In 27 cases (17%) ulcers of the stomach or duodenum were found, although the majority of the patients had large inflammatory mucosal alterations. In 49 cases (34.5%) a chronic tracheobronchitis could be found. Discussion. In our opinion there is an absolute indication for panendoscopic examinations of patients with oral squamous cell carcinomas. In cases with dysplastic mucosal findings these endoscopies have to be repeated on a regular basis.  相似文献
