Introduction

The aim of this paper is to present our experience with femoral press-fit fixation in anterior cruciate ligament reconstruction using bone-patellar tendon-bone autograft.

Methods

The patient population was randomly placed in two groups: group A (58 patients), who underwent femoral screw fixation; group B (62 patients), who underwent femoral press-fit fixation.

Results

At last follow-up 9.2% of patients were lost; 28% of patients in group A and 64% of patients in group B had excellent International Knee Documentation Committee score (grade A); 66% of patients in group A and 32% of patients in group B had good International Knee Documentation Committee scores (grade B). The difference was statistically significant (p?<?0.05).

Conclusions

Femoral press-fit fixation of bone- patellar tendon- bone autograft provides stable fixation at low cost, it ensures unlimited bone-to-bone healing and high primary stability, avoiding the disadvantages of hardware and the need for removal in case of revision.  相似文献

Summary The endoscopic single incision technique for ACL reconstruction with a femoral half-tunnel may lead to a graft/tunnel mismatch and subsequent protrusion of the block from the tibial tunnel. The typical tibial fixation with an interference screw is not possible in these cases. Fixation with staples in a bony groove inferior to the tunnel outlet can be used as an alternative technique. Current literature does not provide biomechanical data of both fixation techniques in a human model. This study was performed to evaluate primary biomechanical parameters of this technique compared to a standard interference screw fixation of the block. 55 fresh-frozen human cadaver knee joints of a younger age (mean age: 44 years) were used. Grafts were harvested from the patellar tendon midportion with bone blocks of 25 mm length and 9 mm width. A 10 mm tibial tunnel was drilled from the anteromedial cortex to the center of the tibial insertion of the ACL. 3 different sizes of interference screws (7 × 30, 9 × 20 and 9 × 30 mm) were chosen as a standard control procedure (n = 40). For tibial bone-block fixation the graft was placed through the tunnel, the screw was then inserted on the cancellous or the cortical surface respectively. 15 knees were used for staple fixation. A groove was created inferior to the tunnel outlet with a chisel. The bone block was fixed in this groove with 2 barbed stainless steel staples. Tensile testing in both of the groups was carried out under axial load parallel to the tibial tunnel in a Zwick-testing-machine with a velocity of 1 mm/sec. Dislocation of the graft and stiffness were calculated at 175 N load. Maximum load to failure using interference screws varied between 506 and 758 N. Load to failure using staples was 588 N. Dislocation of the graft ranged between 3.6 and 4.7 mm for interference screw fixation and was 4.2 mm for staples. With both fixation techniques, the recorded failure loads were sufficient to withstand the graft loads which are to be expected during the rehabilitation period. Staple fixation of the bone block outside of the tunnel resulted in fixation strength comparable to interference screw fixation.   相似文献

[目的]研究肌腱-骨块自体移植物嵌压固定法重建前交叉韧带的组织形态学转归.[方法]以8只山羊膝关节为实验模型,用阶梯状联合钻1次成形建立股骨隧道呈倒置瓶颈状,以髌腱-胫骨结节骨块为移植物,在股骨隧道内行嵌压固定;在胫骨端骨桥打结固定.术后4、8、12和16周取材,分别进行放射学、大体形态和组织切片检查,观察隧道宽度变化及移植物的组织学转归.[结果]术后各时间点放射检查未见隧道扩大;HE染色检查显示12周后有明显的腱-骨连接形成,移植骨块与隧道形成骨性愈合.[结论]嵌压固定法重建前交叉韧带有利于移植物的愈合.  相似文献

The anterior cruciate ligament (ACL) consists of two functional bundles that behave independently throughout the range of knee motion. Many two-bundle reconstruction techniques have been introduced to restore the function of the two bundles of the ACL. Generally, two femoral and two tibial tunnels are made during the surgery for a two-bundle ACL reconstruction. However, the procedure is technically demanding and time consuming. This paper describes one-tibial-two-femoral ACL double bundle reconstruction technique with a sextuple-stranded hamstring autograft. The anteromedial femoral tunnel is made using transtibial drilling technique and posterolateral femoral tunnel is made using outside-in technique. The two bundles in a single tibial tunnel are separated using biodegradable interference screw. Stable and adequate femoral fixation of the two bundles with a transtibial fixation and bioabsorbable screw can be obtained. This technique is relatively simple, and replicates the anatomy and differential behavior of the two native bundles of ACL more effectively.  相似文献

Damage of graft fibers is a complication that can happen during arthroscopic reconstruction of the anterior cruciate ligament. It usually occurs while pulling the neoligament through the tibial tunnel, inserting the femoral screw, or doing additional notchplasty. Rupture of the anterior cruciate ligament (ACL) is a common orthopedic injury. Various graft options are available for the reconstruction of ruptured ACL. Using the hamstring muscle as an autograft was first described in 1934, and it remains a commonly harvested graft for ACL reconstruction. Hamstring autografts can be harvested using the traditional anteromedial approach or the newer posteromedial technique. An isolated semitendinosus tendon can be used or combined with the gracilis tendon. There are numerous methods for graft fixation, such as intra-tunnel or extra-tunnel fixation. This comprehensive review discusses the different hamstring muscle harvesting techniques and graft preparation options and fixation methods. An experimental study was conducted to compare the biomechanical characteristics of six currently available anterior cruciate ligament (ACL) reconstruction procedures with flexor digitorum profundus tendons. Forty porcine knees were divided into eight groups, of 5 knees each. In groups A, B, C, and D, the flexor tendon graft was fixed with sutures and an Endobutton, with 20-mm-wide polyester tapes and staples, with 10-mm-wide polyester tapes and an Endobutton, and with bone plugs and interference screws, respectively. In group E, the graft was fixed using a combined procedure of those in groups B and D. In group F, the graft was directly fixed with interference screws. In groups G and H, the bone-patellar tendon-bone graft was fixed with interference screws, and sutures, respectively. Each femur-graft-tibia complex specimen was tested with a tensile tester by anteriorly translating the tibia until failure. This study demonstrated that the biomechanical properties of the femur-graft-tibia complex reconstructed with the flexor tendon graft were significantly affected by synthetic fixation devices. Regarding the average maximal load of the groups with the flexor tendon graft, group B had the highest (893 N) and group C had the second highest (770 N). Groups E and A were in the third rank. Group F had the second lowest (312 N), and Group D had the lowest (230 N). The maximal load of group B was significantly greater (P < 0.01) than that of group G (656 N) with the bone-patellar tendon-bone grafts. As to clinical relevance, this study indicated that the flexor tendon graft can be an alternative substitute for the bone-patellar tendon-bone graft for ACL reconstruction, if we understand the biomechanical characteristics of each reconstruction procedure. 目的:探讨以自体骨栓肌腱结嵌压固定保留胫骨止点的胭绳肌腱重建膝前交叉韧带的临床效果及应用价值。方法:采用自体骨栓肌腱结嵌压固定保留胫骨止点的胭绳肌腱重建膝前交叉韧带断裂20例,男15例,女5例;年龄18~32岁,平均22岁;左、右膝各10例。取自体胭绳肌腱,保留肌腱的胫骨止点,两端编织缝合后预张。建立胫骨、股骨隧道,并制作胫骨骨桥结构,股骨隧道为内窄外宽结构。骨栓肌腱结嵌入股骨隧道内,牵引线带着肌腱结远端的腱束经股骨、胫骨隧道穿出,与胫骨止点上的肌腱交叉,在胫骨骨桥上打结并缝合固定。术后患膝以支具固定在屈曲45。位。结果:20例患者均获得随访,随访时间8—24个月,平均11个月。膝关节功能评估采用Lysholm功能评分标准,术前平均(61.5±4.6)分,终末随访时平均(92.5±3.7)分,差异有统计学意义,P〈0.05。结论:骨栓肌腱结嵌压固定保留胫骨止点的胭绳肌腱重建膝前交叉韧带的方法为生物学固定,避免使用高值耗材,降低了手术费用,且有利于腱-骨愈合。  相似文献

Purpose: To avoid potential problems of double-bundle anterior cruciate ligament reconstruction (ACLR), various modifications have been reported. This study analyzed a novel technique of modified doublebundle (MDB) ACLR without implant on tibial side in comparison to single-bundle (SB) ACLR. Methods: Eighty cases of isolated anterior cruciate ligament tear (40 each in SB group or MDB group) were included. SB ACLR was performed by outside in technique with quadrupled hamstring graft fixed with interference screws. In MDB group, ACLR harvested tendons were looped over each other at the center and free ends whipstitched. Femoral tunnel was created by outside in technique. Anteromedial tibial tunnel was created with tibial guide at 55. The anatomic posterolateral aiming guide (SmithNephew) was used to create posterolateral tunnel. With the help of shuttle sutures, the free end of gracillis was passed through posterolateral tunnel to femoral tunnel followed by semitendinosus graft through anteromedial tunnel to femoral tunnel. On tibial side the graft was looped over bone-bridge between external apertures of anteromedial and posterolateral tunnel. Graft was fixed with interference screw on femoral side in 10 knee flexion. International Knee Documentation Committee (IKDC), Tegner score, Pivot shift and knee laxity test (KLT, Karl-Storz) were recorded pre- and post-surgery. At one year magnetic resonance imaging (MRI) was done. Statistical analysis was done by SPSS software. Results: Mean preoperative KLT reading of (10.00 ± 1.17) mm in MDB group improved to (4.10 ± 0.56) mm and in SB group it improved from (10.00 ± 0.91) mm to (4.80 ± 0.46) mm. The mean preoperative IKDC score in MDB group improved from (49.49 ± 8.00) to (92.5 ± 1.5) at one year and that in SB group improved from (52.5 ± 6.9) to (88.4 ± 2.6). At one-year 92.5% cases in MDB group achieved their preinjury Tegner activity level as compared to 60% in SB group. The improvement in IKDC, KLT and Tegner scale of MDB group was superior to SB group. MRI confirmed graft integrity at one year and clinically at 2 years. Conclusion: MDB ACLR has shown better outcome than SB ACLR. It is a simple technique that does not require fixation on tibial side and resultant graft is close to native ACL.  相似文献

Introduction: The purpose of our study was to evaluate and compare the primary fixation strength of a novel bioabsorbable two shell expansion bolt (EB) with that of a well-established interference screw-fixation technique in hamstring reconstruction of the anterior cruciate ligament. Materials and methods: Thirty calf tibia plateaus (age 5–6 months) were assigned to three groups: In group I (n=10) triple-stranded hamstring grafts were fixed with titanium interference screws (7 mm thread / 8 mm head × 25 mm). Specimens of group II (n=10) received bioabsorbable poly-L-lactide interference screws (8×23 mm). In group III (n=10), the grafts were fixed using bioabsorbable poly-D,L-lactide expansion bolts (5.8/8.5/10 mm × 35 mm). The tensile axis was placed parallel to the bone tunnel. The construction was then loaded until failure under a displacement rate of 1 mm per second. Results: There were no significant differences concerning the maximum pullout force (group I: 357 N ± 61; group II: 326 N ± 92; group III: 343 N ± 55). In case of the expansion bolt, we found the stiffness to be higher (61 N/mm) when compared to group I (48 N/mm), and group II (52 N/mm) (P<0.01 I vs. III). Using interference screws, we were able to demonstrate a strong correlation between torque and pullout forces (group I: r 2=0.7; group II: r 2=0.92). Ruptures of the suturing material occurred only in groups I and II. Conclusion: We conclude that hamstring graft fixation, using the presented expansion bolt, demonstrates fixation strength similar to the established screw fixation and can therefore be regarded as a reasonable alternative fixation method. Especially, since some specific disadvantages of screw fixation can be prevented by application of the bolt fixation.  相似文献

Background:

Double bundle anterior cruciate ligament (DBACL) reconstruction is said to reproduce the native anterior cruciate ligament (ACL) anatomy better than single bundle anterior cruciate ligament, whether it leads to better functional results is debatable. Different fixation methods have been used for DBACL reconstruction, the most common being aperture fixation on tibial side and cortical suspensory fixation on the femoral side. We present the results of DBACL reconstruction technique, wherein on the femoral side anteromedial (AM) bundle is fixed with a crosspin and aperture fixation was done for the posterolateral (PL) bundle.

Materials and Methods:

Out of 157 isolated ACL injury patients who underwent ACL reconstruction, 100 were included in the prospective study. Arthroscopic DBACL reconstruction was done using ipsilateral hamstring autograft. AM bundle was fixed using Transfix (Arthrex, Naples, FL, USA) on the femoral side and bio interference screw (Arthrex, Naples, FL, USA) on the tibial side. PL bundle was fixed on femoral as well as on tibial side with a biointerference screw. Patients were evaluated using KT-1000 arthrometer, Lysholm score, International Knee Documentation Committee (IKDC) Score and isokinetic muscle strength testing.

Methods:

Results:

The KT-1000 results were evaluated using paired t test with the P value set at 0.001. At the end of 1 year, the anteroposterior side to side translation difference (KT-1000 manual maximum) showed mean improvement from 5.1 mm ± 1.5 preoperatively to 1.6 mm ± 1.2 (P < 0.001) postoperatively. The Lysholm score too showed statistically significant (P < 0.001) improvement from 52.4 ± 15.2 (range: 32-76) preoperatively to a postoperative score of 89.1 ± 3.2 (range 67-100). According to the IKDC score 90% patients had normal results (Category A and B). The AM femoral tunnel initial posterior blow out was seen in 4 patients and confluence in the intraarticular part of the femoral tunnels was seen in 6 patients intraoperatively. The quadriceps strength on isokinetic testing had an average deficit of 10.3% while the hamstrings had a 5.2% deficit at the end of 1 year as compared with the normal side.

Conclusion:

Our study revealed that the DBACL reconstruction using crosspin fixation for AM bundle and aperture fixation for PL bundle on the femoral side resulted in significant improvement in KT 1000, Lysholm and IKDC scores.  相似文献

目的 前瞻性评估保留残端对于前交叉韧带重建临床疗效的意义.方法 2008年8月至2009年9月共70例有韧带残端存留的前交叉韧带损伤患者入选本研究组,随机分为保留残端组(n=35)和对照组(n=35).分别采取关节镜下保留残端重建前交叉韧带和切除残端的前交叉韧带重建手术技术,移植物均使用同种异体肌腱.术后随访分别进行膝关节功能评分(IKDC分级和Lysholm评分)、客观稳定性评估(Lachman试验、轴移试验和KT-1000测量)、本体感觉功能测量和二次关节镜手术探查.结果 70例患者中共61例(87%)获得随访,平均随访时间13.1个月.两组的功能评分无显著差异:Lysholm评分:保留残端组96.4分,对照组94.9分(P=0.71);IKDC分级中A和B级:保留残端组30例,对照组29例(P=0.586).两组的客观稳定性评估无显著差异:KT-1000测量的侧-侧差值:保留残端组1.69 mm,对照组1.65 mm(P=0.83);Lachman试验阴性例数:保留残端组29例,对照组28例(P=1.00);轴移试验阴性例数:保留残端组31例,对照组27例(P=0.225).本体感觉的角度重复试验结果无显著差异:保留残端组4.56°,对照组4.28°(P=0.522).二次手术探查时发现的移植物滑膜覆盖率无显著差异:保留残端组85%,对照组84.2%.结论 保留残端同时使用异体肌腱移植重建前交叉韧带,对术后膝关节主观功能评分、稳定性和本体感觉和移植物滑膜覆盖程度并无促进作用.
Abstract:
Objective To evaluate the clinical significance of arthroscopic anterior cruciate ligament (ACL)reconstruction using the remnant-preserved technique.Methods From August 2008 to September 2009,70 cases with the remnant of injured ACL were included in the trials,which were randomized into the remnant preservation(RP)group and the control group,35 cases in each group.All patients in the two groups underwent arthroscopic ACL reconstruction surgeries,with ACL-remnant preserving technique in RP group and ACL-remnant resection in control group,respectively.The injured ACL was reconstructed with allograft in all cases.Postoperative follow-up assessment included the International Knee Documentation Committee(IKDC)grading and Lysholm score,Lachman test,pivot shift test and KT-1000 measurement,proprioception measurements and the arthroscopic second look evaluation.Results Sixty-one(61/70,87%)cases were available for an average of 13.1 months follow-up assessment postoperatively.There were no significant differences between the RP and control group in functional outcome as evaluated with Lysholm score(96.4 vs.94.9,P = 0.71)and IKDC grading(cases with A and B gradings:30 vs.29,P = 0.586).Regarding objective stability,there were no differences between the 2 group in mean side-to-side difference of KT-1000(1.69 mm vs.1.65 mm,P =0.83),Lachman test(negative cases:29 vs.28,P = 1.00)and pivot shift test(negative cases:31 vs.27,P =0.225).There was also no difference between the groups in proprioception evaluation measured with angle repetitive test(4.56°vs.4.28°,P=0.522).During second look arthroscopic examination,the grafts synoveal coverage rates were found to be 85% in the RP group and 84.2% in the control group,without significant difference(P>0.05).Conclusions Arthroscopic ACL reconstruction with the remnant preserving technique using tendon allograft do not improve the postoperative knee-joint function scores,stability,proprioception and synovial coverage of grafts.  相似文献

Injury to the anterior cruciate ligament (ACL) not only causes mechanical instability but also leads to a functional deficit in the form of diminished proprioception of the knee joint. “Functional” recovery is often incomplete even after “anatomic” arthroscopic ACL reconstruction, as some patients with a clinically satisfactory repair and good ligament tension continue to complain of a feeling of instability and giving way, although the knee does not sublux on clinical testing. Factors that may play a role could be proprioceptive elements, as the intact ACL has been shown to have significant receptors. Significant data have come to light demonstrating proprioceptive differences between normal and injured knees, and often between injured and reconstructed knees. ACL remnants have been shown to have proprioceptive fibers that could enhance functional recovery if they adhere to or grow into the reconstructed ligament. Conventionally the torn remnants are shaved off from the knee before graft insertion; modern surgical techniques, with remnant sparing methods have shown better outcomes and functional recovery, and this could be an avenue for future research and development. This article analyzes and reviews our understanding of the sensory element of ACL deficiency, with specific reference to proprioception as an important component of functional knee stability. The types of mechanoreceptors, their distribution and presence in ACL remnants is reviewed, and suggestions are made to minimize soft tissue shaving during ACL reconstruction to ensure a better functional outcome in the reconstructed knee.  相似文献

Several techniques of anterior cruciate ligament (ACL) double-bundle reconstruction have been introduced to improve the functional outcome and restore normal kinematics of the knee. Meanwhile, a remnant-preserving technique was developed to preserve the proprioception and to enhance the revascularization of the reconstructed ACL. We developed double-bundle ACL reconstruction technique using autogenous quadriceps tendon graft while preserving the remnant. With this technique, two femoral sockets and one tibial tunnel are made. To preserve the remnant of the ACL, the rotational direction of the reamer was set to counterclockwise just before perforation of the tibial tunnel. To pass the graft more easily without disturbance of the remnant, the graft passage was achieved through the tibial tunnel. We suggest that the remnant-preserving technique could be an effective alternative considering its mechanical stability as well as the proprioception and vascularization recovery in arthroscopic double-bundle ACL reconstruction.  相似文献

Treatment of tibial plateau fractures associated with anterior cruciate ligament (ACL) injuries can be challenging. Maintaining the joint surface and axis are two main principles for the management of patients with tibial plateau fractures. Herein, we describe a patient with tibial plateau fracture of Schatzker type VI associated with complete ACL rupture. Although in a two-stage reconstruction surgery for treatment of its rupture seem to be accepted by most orthopaedic surgeons in cases of ACL injuries, we treated our patient with ORIF in a one-stage ACL reconstruction surgery. At the 1-year follow-up, the patient gained full range of motion and was functioning well. We reviewed the literature on performing ORIF in a one-stage ACL reconstruction for the treatment of tibial plateau fractures, which seemed to be a reasonable management approach that supported its use for the treatment of this patient.  相似文献

目的:探讨膝后内侧入路金属空心螺钉治疗后交叉韧带胫骨止点撕脱骨折的疗效。方法:自2007年2月至2012年3月共收治36例后交叉韧带胫骨止点撕脱骨折患者,男28例,女8例;年龄16~57岁,平均35岁。均为新鲜闭合性骨折,屈膝90°位后抽屉试验阳性,后沉征阳性。患者均摄膝关节X线片,发现胫骨平台后侧有撕脱骨块,均取膝后内侧入路,用金属空心螺钉固定,术后指导功能锻炼。采用Lysholm膝关节功能评分评价疗效。结果:所有病例成功完成手术,并获随访,时间6~36个月,平均12个月。术后3个月骨折部位均愈合。根据Lysholm膝关节评价标准,优30例,良4例,可2例,平均(92.2±3.8)分。结论:膝后内侧入路金属空心螺钉治疗后交叉韧带胫骨止点撕脱骨折能较大限度恢复关节功能,是一种安全、简单、有效的手术方法。  相似文献

Surgeons are switching to the four-bundled hamstring graft, composed of double-looped semitendinosus and gracilis tendons (DLSTG), to replace the torn anterior cruciate ligament. Mechanically, the DLSTG is superior; it is twice as strong and stiffer than a patella tendon graft, and the four bundles share load and mimic the function of the anteromedial and posterolateral bands of the native anterior cruciate ligament. Morbidity from tendon removal is minimal and by 3 months soreness disappears and isometric knee flexion strength returns to normal. It is safe for the patient to undergo aggressive rehabilitation without a brace and to return to sports activities at 4 months when the knee is reconstructed with a DLSTG graft. This report describes the rationale and technique for implanting the DLSTG in a femoral tunnel using rigid fixation instead of a compliant suture bridge. Fixation is achieved by looping the tendons over a post (Bone Mulch Screw) inside the femoral tunnel. The strength, stiffness, and biologic bond of the graft is enhanced by compaction of bone into the femoral tunnel thorugh a bore in the Bone Mulch Screw.  相似文献

The influences and mechanisms of the physiology,rupture and reconstruction of the anterior cruciate ligament(ACL)on kinematics and clinical outcomes have been investigated in many biomechanical and clinical studies over the last several decades.The knee is a complex joint with shifting contact points,pressures and axes that are affected when a ligament is injured.The ACL,as one of the intra-articular ligaments,has a strong influence on the resulting kinematics.Often,other meniscal or ligamentous injuries accompany ACL ruptures and further deteriorate the resulting kinematics and clinical outcomes.Knowing the surgical options,anatomic relations and current evidence to restore ACL function and considering the influence of concomitant injuries on resulting kinematics to restore full function can together help to achieve an optimal outcome.  相似文献

目的探讨关节镜下"8"字缝线与网兜缝线固定治疗前十字韧带(anterior cruciate ligament,ACL)胫骨止点撕脱骨折的早期临床疗效。方法回顾性分析2013年8月至2016年11月收治的37例ACL胫骨止点撕脱骨折并获得完整随访的患者,所有患者骨骺均已闭合。Meyers-McKeever-Zaricznyj骨折分型:Ⅱ型10例,Ⅲ型22例,Ⅳ型5例。固定方式采用"8"字缝线法25例,男17例,女8例,年龄(18.91±9.34)岁(范围14~36岁),以两根2号高强度缝线交叉成"8"字经胫骨骨道拉出固定骨折块;采用网兜缝线固定12例,男8例,女4例,年龄(19.63±7.85)岁(范围15~33岁),以三根2号高强度缝线编织成网兜经胫骨骨道拉出固定骨折块。手术前后采用Lysholm及国际膝关节评分委员会(International Knee Documentation Committee,IKDC)评分评价膝关节功能,通过Lachman试验及Pivot-shift试验评价膝关节稳定性,并记录手术时间、膝关节活动度及伸膝阻滞情况。结果"8"字缝线组随访(16.35±5.27)个月(范围10~22个月),网兜缝线组(14.06±7.18)个月(范围10~21个月)。末次随访时,"8"字缝线组的Lysholm、IKDC评分分别为(95.86±5.74)分、(90.53±4.61)分,网兜缝线组分别为(96.53±3.17)分、(92.15±5.54)分,两组差异无统计学意义(t=0.723,P=0.462;t=1.018,P=0.279);"8"字缝线组Lachman试验及Pivot-shift试验阴性率均为92%(23/25),网兜缝线组均为100%(12/12),两组差异无统计学意义(χ^2=0.904,P=0.265);网兜缝线组手术时间为(61.8±6.3)min,较"8"字缝线组的(43.5±5.9)min长,差异有统计学意义(t=2.714,P=0.025);"8"字缝线组伸膝受限(5°或以上伸膝受限)发生率为16%(4/25),网兜缝线组为0,两组差异有统计学意义(χ^2=0.450,P=0.032)。结论关节镜下"8"字缝线固定与网兜缝线固定技术治疗ACL胫骨止点撕脱骨折均可获得良好的早期膝关节功能和稳定性。网兜缝线固定技术可完整复位骨折块,术后伸膝阻滞发生率低,在治疗旋转Ⅲ型和粉碎Ⅳ型骨折中可能更具有优势;但有技术要求高、手术时间长的缺点。  相似文献
