Objectives:

Evaluation and reduction of dose are important issues. Since cone beam CT (CBCT) has been established now not just in dentistry, the number of acquired examinations continues to rise. Unfortunately, it is very difficult to compare the doses of available devices on the market owing to different exposition parameters, volumes and geometries. The aim of this study was to evaluate the spans of effective doses (EDs) of ten different CBCT devices.

Methods:

48 thermoluminescent dosemeters were placed in 24 sites in a RANDO® head phantom. Protocols with lowest exposition parameters and protocols with highest exposition parameters were performed for each of the ten devices. The ED was calculated from the measured energy doses according to the International Commission on Radiological Protection 2007 recommendations for each protocol and device, and the statistical values were evaluated afterwards.

Results:

The calculation of the ED resulted in values between 17.2 µSv and 396 µSv for the ten devices. The mean values for protocols with lowest and highest exposition parameters were 31.6 µSv and 209 µSv, respectively.

Conclusions:

It was not the aim of this study to evaluate the image quality depending on different exposition parameters but to define the spans of EDs in which different CBCT devices work. There is a wide span of ED for different CBCT devices depending on the selected exposition parameters, required spatial resolution and many other factors.  相似文献

Objectives

To characterize the dose distribution for a range of cone beam CT (CBCT) units, investigating different field of view sizes, central and off-axis geometries, full or partial rotations of the X-ray tube and different clinically applied beam qualities. The implications of the dose distributions on the definition and practicality of a CBCT dose index were assessed.

Methods

Dose measurements on CBCT devices were performed by scanning cylindrical head-size water and polymethyl methacrylate phantoms, using thermoluminescent dosemeters, a small-volume ion chamber and radiochromic films.

Results

It was found that the dose distribution can be asymmetrical for dental CBCT exposures throughout a homogeneous phantom, owing to an asymmetrical positioning of the isocentre and/or partial rotation of the X-ray source. Furthermore, the scatter tail along the z-axis was found to have a distinct shape, generally resulting in a strong drop (90%) in absorbed dose outside the primary beam.

Conclusions

There is no optimal dose index available owing to the complicated exposure geometry of CBCT and the practical aspects of quality control measurements. Practical validation of different possible dose indices is needed, as well as the definition of conversion factors to patient dose.  相似文献

Objectives:

The purpose of the study is to evaluate the effectiveness of thyroid shielding in dental CBCT examinations using a paediatric anthropomorphic phantom.

Methods:

An ATOM® 706-C anthropomorphic phantom (Computerized Imaging Reference Systems Inc., Norfolk, VA) representing a 10-year-old child was loaded with six thermoluminescent dosemeters positioned at the level of the thyroid gland. Absorbed doses to the thyroid were measured for five commercially available thyroid shields using a large field of view (FOV).

Results:

A statistically significant thyroid gland dose reduction was found using thyroid shielding for paediatric CBCT examinations for a large FOV. In addition, a statistically significant difference in thyroid gland doses was found depending on the position of the thyroid gland. There was little difference in the effectiveness of thyroid shielding when using a lead vs a lead-equivalent thyroid shield. Similar dose reduction was found using 0.25- and 0.50-mm lead-equivalent thyroid shields.

Conclusions:

Thyroid shields are to be recommended when undertaking large FOV CBCT examinations on young patients.  相似文献

Objectives:

To compare the effective dose to patients from temporomandibular joint examinations using a dental CBCT device and a multislice CT (MSCT) device, both before and after dose optimization.

Methods:

A Promax® 3D (Planmeca, Helsinki, Finland) dental CBCT and a LightSpeed VCT® (GE Healthcare, Little Chalfont, UK) multislice CT were used. Organ doses and effective doses were estimated from thermoluminescent dosemeters at 61 positions inside an anthropomorphic phantom at the exposure settings in clinical use. Optimized exposure protocols were obtained through an optimization study using a dry skull phantom, where four observers rated image quality taken at different exposure levels. The optimal exposure level was obtained when all included criteria were rated as acceptable or better by all observers.

Results:

The effective dose from a bilateral examination was 184 µSv for Promax 3D and 113 µSv for LightSpeed VCT before optimization. Post optimization, the bilateral effective dose was 92 µSv for Promax 3D and 124 µSv for LightSpeed VCT.

Conclusions:

At optimized exposure levels, the effective dose from CBCT was comparable to MSCT.  相似文献

Objectives

As a first step in developing a protocol for multidimensional sialography using cone beam CT (CBCT), the objective of this study was to compare the effective radiation doses from sialography of the parotid and submandibular glands using plain radiography and CBCT.

Methods

The effective doses were calculated from dose measurements made at 25 selected locations in the head and neck of a radiation analogue dosimeter (RANDO) phantom, using International Commission on Radiological Protection 2007 tissue weighting factors.

Results

The effective dose (E) changed in relationship to changes in CBCT field of view (FOV), peak kilovoltage (kVp) and milliamperage (mA). Specifically, E decreased from a maximum of 932 μSv (30 cm FOV, 120 kVp, 15 mA) to 60 μSv (15 cm FOV, 80 kVp, 10 mA) for a parotid gland study and to 148 μSv (15 cm FOV, 80 kVp, 10 mA) for a submandibular study. The collective series of plain radiographs made during sialography of the parotid and submandibular glands yielded effective doses of 65 μSv and 156 μSv, respectively. The plain parotid gland series included one panoramic, two anterior–posterior skull and four lateral skull radiographs, whereas the submandibular gland series included one panoramic, one standard mandibular occlusal and four lateral skull radiographs.

Conclusion

The effective doses from CBCT examinations centred on the parotid and submandibular glands were similar to those calculated for plain radiograph sialography when a 15 cm FOV was chosen in combination with exposure conditions of 80 kVp and 10 mA.  相似文献

Objectives:

To demonstrate the feasibility of GafChromic® XR-QA2 (ISP Corp., Wayne, NJ) as a dosemeter when performing measurements of the effective dose from three cone beam CT (CBCT) units and to compare the doses from examinations of three common dental clinical situations. A second aim was to compare the radiation doses for three digital panoramic units with the doses for the CBCT units.

Methods:

The CBCT units used were Veraviewepocs 3De® (J Morita MFG Corp., Kyoto, Japan), ProMax® 3D (Planmeca, Helsinki, Finland) and NewTom VGi® (Quantitative Radiology, Verona, Italy). GafChromic XR-QA2 films were placed between the selected layers of the head and neck of a tissue-equivalent human skull (RANDO® phantom; The Phantom Laboratory, Salem, NY). The exposure parameters were set using the automatic exposure control function of the units. Depending on the availability, medium and smaller field of view (FOV) scanning modes were used. The effective dose was estimated using the 2007 International Commission on Radiological Protection formalism.

Results:

The lowest effective dose of a CBCT unit was observed for ProMax 3D, FOV 4 × 5 cm (10 μSv), the highest for NewTom VGi, FOV 8 × 8 cm—high resolution (129 μSv). The range of effective doses for digital panoramic machines measured was 8–14 μSv.

Conclusions:

This study demonstrates the feasibility of using radiochromic films for dental CBCT and panoramic dosimetry.  相似文献

Objective

The aim of this study was to evaluate the influence of thyroid collars on radiation dose during cone beam CT (CBCT) scanning.

Methods

Average tissue-absorbed dose for a NewTom 9000 CBCT scanner (Quantitative Radiology, Verona, Italy) was measured using thermoluminescent dosemeter chips in a phantom. The scans were carried out with and without thyroid collars. Effective organ dose and total effective dose were derived using International Commission on Radiological Protection 2007 recommendations.

Results

The effective organ doses for the thyroid gland and oesophagus were 31.0 µSv and 2.4 µSv, respectively, during CBCT scanning without a collar around the neck. When the thyroid collars were used loosely around the neck, no effective organ dose reduction was observed. When one thyroid collar was used tightly on the front of the neck, the effective organ dose for the thyroid gland and oesophagus were reduced to 15.9 µSv (48.7% reduction) and 1.4 µSv (41.7% reduction), respectively. Similar organ dose reduction (46.5% and 41.7%) was achieved when CBCT scanning was performed with two collars tightly on the front and back of the neck. However, the differences to the total effective dose were not significant among the scans with and without collars around the neck (p = 0.775).

Conclusions

Thyroid collars can effectively reduce the radiation dose to the thyroid and oesophagus if used appropriately.  相似文献

Objectives

The dosimetry of the Kodak 9500 cone beam CT (CBCT) unit (Carestream Health, Rochester, NY) was measured before and after installation of copper filtration.

Methods

Dosimetry of a pre-production Kodak 9500 CBCT unit was compared with a current production unit with 0.4 mm of added filtration and increased kVp. Thermoluminescent dosimeter 100 chips were placed at 24 locations in a RANDO (radiation analogue dosimetry) head phantom (Nuclear Associates, Hicksville, NY). Small, medium and large adult default exposure settings were used in separate dosimeter runs for large and medium field of view (FOV) examinations with both units. Equivalent dose and effective dose were calculated using International Commission on Radiological Protection (ICRP) 1990 and 2007 tissue weights.

Results

Estimations of risk using 2007 ICRP calculations increased by an average of 77% for large FOV scans and 125% for the medium FOV scans in comparison with 1990 calculations. With added filtration, effective dose for medium FOV examinations for default settings were: small adult 76 µSv, medium adult 98 µSv, and large adult 166 µSv. Effective doses for large FOV examinations were: small adult 93 µSv, medium adult 163 µSv, and large adult 260 µSv. Effective dose was reduced by an average of 43% in examinations made with increased filtration and adjusted kVp.

Conclusion

The manufacturer''s installation of additional filtration with the adjustment of kVp in the Kodak 9500 CBCT unit resulted in significant patient dose reductions for examinations at all adult default settings.  相似文献

Objective

The aim of this study was to assess the accuracy and reliability of cone beam CT (CBCT) images compared with multidetector CT (MDCT) images for the detection of surface osseous changes in temporomandibular joints (TMJs).

Methods

Naked-eye inspection of 110 sites in 10 TMJs from 5 dry human skulls provided the gold standard. Two radiologists interpreted the images. Sensitivity, specificity and kappa statistics were used for analysis.

Results

The sensitivities of both modalities were low and comparable whereas the specificities were high and comparable. Intraobserver reliabilities for CBCT (p=0.0005) and for MDCT (p=0.0001) showed significant agreement. Interobserver reliability was higher for CBCT than for MDCT.

Conclusion

CBCT and MDCT accuracy was comparable in detecting surface osseous changes with comparable intraobserver reliabilities. However, since CBCT requires less radiation exposure, it should be encouraged for imaging TMJ with suspected surface osseous changes.  相似文献

Objectives:To evaluate, in an in vitro study, the quantity of artefacts generated by two different restorative materials, and to determine the diagnostic accuracy of cone beam CT (CBCT) scans and periapical radiographs in identifying gaps in prosthetic crowns.Methods:A total of 30 teeth restored with metal-ceramic (n = 15) and all-ceramic (n = 15) crowns, properly adapted and with 0.30- and 0.50 mm gaps, underwent CBCT exams (with voxel sizes of 0.25 and 0.30 mm) and periapical radiographs. The artefacts generated by two different crowns were quantified and compared by the Mann–Whitney test. In addition, five examiners evaluated the presence or absence of gaps in periapical radiographs and CBCT images. The accuracy of tests was determined by the area under the receiver operatring characteristic curve and these values were compared by using the Kruskal–Wallis test.Results:There was no significant difference in artefact values between the different restorative materials and the different resolutions of CBCT images. Regarding the accuracy of the tests evaluated, periapical radiography and CBCT with voxel size 0.25 mm showed the best performance for smaller gaps (0.30 mm). For larger gaps (0.50 mm), all exams tested showed the same performance.Conclusions:Periapical radiography was still the most cost-beneficial method for the diagnosis of maladaptation in dental restorations. CBCT exams did not improve accuracy in detecting gaps in prosthetic crowns.  相似文献

Objective

The aim of this study was to compare the diagnostic capabilities of two-dimensional sialography with a novel three-dimensional technique using cone beam CT (CBCT).

Methods

47 subjects underwent parotid or submandibular gland sialography over a 2 year period using both plain imaging and CBCT. Both image sets were anonymized and independently reviewed by three certified oral and maxillofacial radiologists blinded to the clinical data. McNemar''s χ2 test was used to determine differences between the two modalities for feature visualization and interpretation.

Results

CBCT outperformed plain imaging with respect to visualization of the gland parenchyma (p < 0.001) and identification of sialoliths (p = 0.02). Plain imaging outperformed CBCT for the identification of strictures (p = 0.04); however, the negative per cent agreement (“specificity”) between the two imaging modalities was 100%. Although both imaging modalities performed equally in identifying normal and abnormal sialographic examinations, CBCT demonstrated a high negative per cent agreement for normal glands and a high positive per cent agreement (“sensitivity”) for abnormal glands with inflammatory changes.

Conclusion

CBCT sialography allowed better visualization of gland parenchyma and identification of sialoliths. The high negative per cent agreement for strictures suggests that, if strictures are identified on CBCT images, then obstruction can be ruled in. Relative to plain images, the high negative per cent agreement for normal glands suggests that, if an abnormal finding is detected on CBCT images, then disease can be ruled in, and the high positive per cent agreement for glands with inflammatory changes suggests that inflammation can be ruled out if these changes are not seen on CBCT images.  相似文献

Objectives:

We compared the effective dose from panoramic radiography with that from cone beam CT (CBCT) using dose area product under adult and child exposure conditions.

Methods:

The effective doses of the cephalo, panorama, implant and dental modes of Alphard 3030 (Asahi Roentgen Ind., Co. Ltd, Kyoto, Japan) CBCT and the Jaw, Wide, Facial and temporomandibular joint modes of Rayscan Symphony (RAY Co., Ltd, Hwaseong, Republic of Korea) CBCT were compared with those of CRANEX® 3+ CEPH (Soredex Orion Corporation, Helsinki, Finland) panoramic radiography equipment under adult and child exposure conditions. Each effective dose was calculated using a conversion formula from dose area product meter measured values (VacuTec Messtechnik GmbH, Dresden, Germany). The conversion formulae used were suggested by Helmrot and Alm Carlsson and Batista et al, and they were applied with the tube voltage taken into consideration.

Results:

The maximum effective doses from the Alphard 3030 and Rayscan Symphony were 67 and 21 times greater than that from panoramic radiography, respectively. The ratios of the effective dose under the child setting to that under the adult condition were 0.60–0.62 and 0.84–0.95, and the maximum differences in effective doses between the adult and child exposure settings were equivalent to 27 and 4 times greater than a panoramic examination in the Alphard 3030 and Rayscan Symphony, respectively.

Conclusions:

The effective CBCT doses were higher than those of panoramic radiography. The differences in effective doses between the adult and child CBCT settings were dependent on equipment type and exposure parameters. Therefore, adequate mode selection and control of exposure as well as further research are necessary to minimize the effective dose to patients, especially for radiosensitive children.  相似文献

The purpose of this study is to explore the topical use of radiographic contrast agents to enhance soft-tissue contrast on cone beam CT (CBCT) images. Different barium sulphate concentrations were first tested using an airway phantom. Different methods of barium sulphate application (nasal drops, syringe, spray and sinus wash) were then tested on four volunteers, and nebulized iodine was tested in one volunteer. CBCT images were performed and then assessed subjectively by two examiners for contrast agent uniformity and lack of streak artefact. 25.0% barium sulphate presented adequate viscosity and radiodensity. Barium sulphate administered via nasal drops and sprays showed non-uniform collection at the nostrils, along the inferior and/or middle nasal meatuses and posterior nasal choana. The syringe and sinus wash showed similar results with larger volumes collecting in the naso-oropharynx. Nebulized iodine failed to distribute into the nasal cavity and scarcely collected at the nostrils. All methods of nasal application failed to adequately reach or uniformly coat the nasal cavity beyond the inferior nasal meatuses. The key factors to consider for optimum topical radiographic contrast in the nasal airway are particle size, flow velocity and radio-opacity.  相似文献

Estimation of mean-glandular dose (MGD) has been investigated in recent years due to the potential risks of radiation-induced carcinogenesis associated with the mammographic examination for diagnostic radiology. In this study, a new technique for immediate readout of breast entrance skin air kerma (BESAK) using high sensitivity MOSFET dosimeter after mammographic projection was introduced and a formula for the prediction of tube output with exposure records was developed. A series of appropriate conversion factors was applied to the MGD determination from the BESAK. The study results showed that signal response of the high sensitivity MOSFET exhibited excellent linearity within mammographic dose ranges, and that the energy dependence was less than 3% for each anode/filter combination at the tube potentials 25–30 kV. Good agreement was observed between the BESAK and the tube exposure output measurement for breasts thicker than 30 mm. In addition, the air kerma estimated from our prediction formula provided sufficient accuracy for thinner breasts. The average MGD from 120 Asian females was 1.5 mGy, comparable to other studies. Our results suggest that the high sensitivity MOSFET dosimeter system is a good candidate for immediately readout of BESAK after mammographic procedures.  相似文献

Objective: The Cavalieri principle of stereological methods is widely used to estimate the volume of structures. Recently in clinical practice, it has become common to use this approach for daily routine purposes. The Cavalieri principle provides quantitative and unbiased volume estimates which are independent of the observer. In the present study, the efficacy of using the Cavalieri principle to estimate the volume of sheep mandibular defects on cone beam CT (CBCT) scans was tested.Methods: 24 differently sized defects were created on 4 sheep mandibles. Before the defects were created, the outer boundaries of the defects were determined using plaster casts. CBCT scans of the defects were taken. The scans were reconstructed in the coronal plane and sections of 0.2 mm thickness with 0.2 mm and 0.4 mm intervals were obtained. The volume of each defect was estimated using the Cavalieri principle. The models were created using light-body silicone for the estimation of the actual volume of each defect. They were immersed in water using a pycnometer and the actual volumes were obtained on the basis of the Archimedean principle. The actual and estimated volumes of the defects were compared using the Wilcoxon signed-rank test.Results: The results showed that the volumes from the Cavalieri estimates did not differ from the actual volumes of the defects (P > 0.05).Conclusion: We concluded that the volume of mandibular defects can be accurately estimated using the Cavalieri principle on images from a CBCT scan.  相似文献

Objectives:

Few studies have described morphological deviations in obstructive sleep apnoea (OSA) patients on two-dimensional (2D) lateral cephalograms, and the reliability of 2D radiographs has been discussed. The objective is to describe the morphology of the cervical vertebral column on cone beam CT (CBCT) in adult patients with OSA and to compare 2D lateral cephalograms with three-dimensional (3D) CBCT images.

Methods:

For all 57 OSA patients, the cervical vertebral column morphology was evaluated on lateral cephalograms and CBCT images and compared according to fusion anomalies and posterior arch deficiency.

Results:

The CBCT assessment showed that 21.1% had fusion anomalies of the cervical column, i.e. fusion between two cervical vertebrae (10.5%), block fusions (8.8%) or occipitalization (1.8%). Posterior arch deficiency occurred in 14% as partial cleft of C1 and in 3.5% in combination with block fusions. The agreement between the occurrence of morphological deviations in the cervical vertebral column between lateral cephalograms and CBCT images showed good agreement (κ = 0.64).

Conclusions:

Prevalence and pattern in the cervical column morphology have now been confirmed on CBCT. The occurrence of morphological deviations in the cervical vertebral column showed good agreement between lateral cephalograms and CBCT images. This indicates that 2D lateral cephalograms (already available after indication in connection with, e.g. treatment planning) are sufficient for identifying morphological deviations in the cervical vertebral column. For a more accurate diagnosis and location of the deviations, CBCT is required. New 3D methods will suggest a need for new detailed characterization and division of deviations in cervical vertebral column morphology.  相似文献

Objectives:

To assess the influence of milliamperage and kilovolt peak (kVp) on the variability of cone beam CT (CBCT) voxel values.

Methods:

CBCT scans were obtained from radiographic phantoms in varying concentrations of dipotassium hydrogen phosphate solutions (200–1200 mg ml−1) under different protocols of milliamperage and kVp. In addition, scans were performed with and without a dental implant and exo-mass. The variability of CBCT voxel values was measured on each scan, and factorial analysis of variance and the post hoc Tukey test were performed (α = 0.05). Linear regression was performed to assess the relationship between voxel value variability and dipotassium hydrogen phosphate concentration.

Results:

milliamperage and the presence of a dental implant did not produce significant interference (p = 0.28 and 0.87, respectively) in voxel value variability. Scans at the highest kVp value presented a significant reduction (p ≤ 0.0001) in voxel value variability when only exo-mass was not present. Voxel value variability was not influenced by exo-mass in scans at the highest levels of milliamperage and kVp. The presence of exo-mass produced a significant reduction (p ≤ 0.0001) in voxel value variability in most of the scans. Higher concentrations yielded greater variations in voxel values in all scans, except for those operating at the highest levels of mAs and kVp.

Conclusions:

mAs did not influence the variability of CBCT voxel values; higher kVp reduced such variability when only the object was smaller than the field of view.  相似文献

Objectives

The validity of any measurement obtained through a cephalogram largely depends on the reproducibility of the cephalometric landmarks. The purpose of this study is to evaluate the influence of a programme of professional calibration (PPC) on the variability of landmark identification comparing conventional radiographs and cone beam CT (CBCT)-synthesized cephalograms.

Methods

5 graduate students in oral radiology identified 20 cephalometric landmarks from cephalograms generated from conventional radiographs (RADs), Ray-Sum CBCT-synthesized cephalograms (CBTs) and half-skull CBT (HSTs) from 10 patients. After a period of reinforcement on instruction and calibration with inter- and intraexaminer assessment of reproducibility (intraclass coefficient correlation scores > 0.75) for RADs, CBTs and HSTs obtained from 5 different patients, observers were asked to repeat the analysis of the first 10 patients under the same circumstances. Values in millimetres represented each landmark in a table of Cartesian co-ordinates (x- and y-axes).

Results

ANOVA showed significant reduction in variability levels after the PPC, and there were no differences among the methods of image acquisition. Repeated measures ANOVA indicated that the PPC accounted for reduction in variability levels in 14 of 20 landmarks.

Conclusions

The results suggest that a PPC has more influence than the type of image acquisition on variability of landmark identification based on two-dimensional cephalometric analysis. Cephalograms obtained from RAD or CBCT can be considered equivalent for clinical and experimental applications.  相似文献

Objectives:

To assess the efficacy of lead foils in reducing the radiation dose received by different anatomical sites of the head and neck during periapical intraoral examinations performed with digital systems.

Methods:

Images were acquired through four different manners: phosphor plate (PSP; VistaScan® system; Dürr Dental GmbH, Bissingen, Germany) alone, PSP plus lead foil, complementary metal oxide semiconductor (CMOS; DIGORA® Toto, Soredex®, Tuusula, Finland) alone and CMOS plus lead foil. Radiation dose was measured after a full-mouth periapical series (14 radiographs) using the long-cone paralleling technique. Lithium fluoride (LiF 100) thermoluminescent dosemeters were placed in an anthropomorphic phantom at points corresponding to the tongue, thyroid, crystalline lenses, parotid glands and maxillary sinuses.

Results:

Dosemeter readings demonstrated the efficacy of the addition of lead foil in the intraoral digital X-ray systems provided in reducing organ doses in the selected structures, approximately 32% in the PSP system and 59% in the CMOS system.

Conclusions:

The use of lead foils associated with digital X-ray sensors is an effective alternative for the protection of different anatomical sites of the head and neck during full-mouth periapical series acquisition.  相似文献
