We report a case of a primary malignant fibrous histiocytoma of the distal radius in a 46-year-old man. Treatment involved en bloc resection, reconstruction with a nonvascularized free fibular autograft, and wrist arthrodesis combined with adjuvant chemotherapy. At the 2-year follow-up evaluation the patient had a satisfactory outcome with complete radiographic union and no evidence of a local recurrence or metastasis. Resection combined with autogenous fibular grafting and adjuvant chemotherapy appears to be an acceptable method for treating malignant fibrous histiocytoma of the distal radius in this patient.  相似文献

Dislocation of the radial head in adults is uncommon. A simultaneous dislocation of the radial head and fracture of the ipsilateral distal end of radius with no other associated injuries is extremelyrare. As far as we know, such an injury after an unusual mode of injury has been seldom reported in the English literature. We report such a case without any associated injuries or comorbidity. Closed reduction was performed within two hours after injury and results were satisfactory. Immobilisation was continued for 3 weeks. Gradual mobilisation was started after removal of the plaster under the supervision of a physiotherapist. At 6 months’ follow-up, the patient had no residual pain at the elbow with full flexion & extension. Almost full supination with a restriction of last 10 degrees of pronation was achieved. There was no evidence of instability of the elbow.  相似文献

We report herein an extremely very rare case of primary benign fibrous histiocytoma of the stomach found in a 56-year-old man who presented with a 2-week history of nausea and anorexia. Gastrointestinal X-rays and endoscopy revealed a protruding lesion in the stomach, but biopsies failed to demonstrate the nature of the tumor. Histopathologic investigation of the resected tumor showed the typical features of benign fibrous histiocytoma: Consisting of an admixture of spindly fibroblast-like and roundish histiocyte-like cells arranged in a storiform pattern. Furthermore, immunohistochemical staining with alpha-1-antichymotrypsin was positive for histiocyte-like cells in the tumor. The patient has shown no evidence of recurrence in the 2 years of follow-up since his operation.  相似文献

Congenital dislocation of the radial head is rare, although it is the most common congenital anomaly of the elbow. A concomitant progressive cubitus valgus of the elbow has not previously been described in literature. We describe a case of an 8-year-old girl with an unilateral congenital radial head dislocation with a progressive cubitus valgus of 35°, caused by a prematurely closing physis of the lateral humeral condyle. This might be caused by an increased pressure on the lateral physis by the anteriorly dislocated radial head. As no complaints or limitations were present, treatment was non-operative with clinical observation, with satisfactory results after a follow-up of 18 months. A concomitant progressive cubitus valgus can be present in patients with a congenital radial head dislocation. Non-operative treatment can provide satisfactory results.  相似文献

A 9-year-old boy presented with anterior dislocation of the left elbow associated with a long-standing posterior dislocation of the radial head. Lack of full extension of the elbow joint was reported since birth. The dislocated elbow was reduced. No treatment was undertaken for the dislocated radial head. Radiographic evidence of dysplasia of the contralateral radial head provided a significant diagnostic criterion of a congenital malformation.  相似文献

Ipsilateral radial head dislocation with radial shaft fracture is a rare injury. A few cases have been reported in the literature. We report the case of a 39-year-old man who presented with a radial shaft fracture and ipsilateral dislocation of the radial head caused by hyperpronation trauma of the forearm with hyperflexion of the elbow. The dislocation was reduced by manipulation after open reduction of the radial shaft fracture, which was then fixed with a plate. The clinical and radiological outcome was good with 2 years follow-up.  相似文献

Bening fibrous histiocytoma is one of the most frequent benign tumors. Most tumors are found in the skin, particularly on the limbs. Tumor size is usually small. According to the WHO, a deep location is found for less than 1% of all benign fibrous histiocytomas. Most deep tumors occur in the subcutaneous tissue. Deep locations in other organs have also been reported. We report a case and discuss the pathological and clinical aspects of this rare tumor.  相似文献

The results of late surgery for symptomatic proximal migration of the radius after resection of the radial head for trauma are usually disappointing. Ulnar variance increases when the interosseous membrane is disrupted. Its integrity should be assessed to predict the results of further surgery. We describe a 29-year-old patient in whom distraction lengthening of the radius through an Ilizarov frame allowed him to regain pain-free function of the wrist and elbow after a complex Essex-Lopresti fracture dislocation and late symptoms of ulnar carpal impingement.  相似文献

We report a case of bilateral congenital dislocation of the radial head discovered at the time of a fracture of the ulna. The dislocation was missed initially and when considered as part of the Monteggia fracture, raised a problem concerning the therapeutic approach. Benign fibrous histiocytoma (BFH) is a rare skeletal tumor, accounting for approximately 1% of all surgically managed benign bone tumors. Spinal location is very rare, and its management can be challenging, involving excision of tumor and spinal reconstruction and stabilization. The prognosis is good after treatment.
