Practice and theory of "delayed" embolectomy. A 22-year perspective   总被引:1,自引:0,他引:1  
A study was made of delayed embolectomy in 45 patients (55 limbs). In 5 patients (11%) the cause of arterial embolization was rheumatic heart disease, and 40 patients (89%) suffered from atherosclerotic cardiovascular disease. The study was divided into 3 phases: I (1960-1964), II (1965-1974) and III (1975-1981). In phases II and III surgery was carried out using the Fogarty catheter technique. In 5 patients the embolus was located in the upper extremity. Seven patients died and 8 major amputations were performed within 30 days of surgery. Use of the Fogarty catheter technique and persistent anticoagulant therapy effected 71.8% limb salvage in phase II and 91.6% limb salvage in phase III. Successful revascularization was achieved in 70.9% of the limbs in which peripheral emboli had occurred on an average of 2.7 days prior to surgical intervention. A theoretical basis for the late development of the acute embolic onset is presented.  相似文献

During the past decade from May 1978 to July 1988, intraaortic balloon pumping (IABP) was used in a total of 108 patients following open-heart surgery at our department. Eleven complications depend on IABP have occurred in nine of theses patients, i.e. five of circulatory disorder on account of ischemia of the lower extremity ipsilateral to balloon catheter insertion, three of ischemia of the abdominal viscera, one of aorto-esophageal fistula, one of gas embolism resulting of balloon rupture and one of damage of the abdominal aorta caused by a Fogarty balloon catheter. Five of these patients died of these complications depend on IABP. Especially, four of five patients died of vascular accidents. These complications were due to operation of the balloon catheter or the driving unit, bat that was no complication related to insert of the balloon catheter. Then, we have to paid more careful attention to driving the intra-aortic balloon.  相似文献

摘要:目的 探讨急性下肢动脉缺血的有效治疗措施及并发症的防治方法。方法 回顾性分析32例急性下肢动脉缺血患者的诊治经验。4例行非手术治疗,28例行急诊手术,其中25例行股动脉切开Fogarty 导管取栓术(7例同时行血管成型术),3例取栓后行截肢术。结果 非手术组死亡1例,手术组无死亡病例。3例截肢,28例肢体得以保存。 结论 急诊股动脉切开取栓术为治疗急性下肢动脉缺血的首选治疗方案。该方法有助于降低病死率和截肢率。  相似文献

OBJECTIVE: to describe an intraoperative technique using a Fogarty balloon to treat arterial spasm following vascular bypass and endarterectomy. DESIGN: prospective case control study. SUBJECTS AND TREATMENT: twenty-two patients following femorodistal bypass surgery and one patient following carotid endarterectomy, with arterial spasm in the distal run-off on completion angiography, were treated with Fogarty balloon dilatation. MATERIALS: Fogarty balloon catheter (Baxtertrade mark). RESULTS: twenty-three patients (100%) with arterial spasm were successfully treated by Fogarty balloon as demonstrated on completion angiography. No complications were seen. CONCLUSION: this simple technique removes vascular spasm rapidly and produces an excellent angiographic result.  相似文献

OBJECTIVES: This preliminary study examined the technical efficacy, safety, and cost of treating arterial occlusions with a single device that combines pharmacologic and mechanical thrombolysis. METHODS: The technical success, bleeding complications, and costs for the first 26 consecutive patients in whom lower extremity ischemia was treated with the Trellis infusion catheter (TIC) were analyzed. Procedure time, thrombolytic infusion time, technical success, bleeding complications (major and intracranial hemorrhage), interventional suite time, and 30-day amputation-free survival were evaluated. RESULTS: 15 of 26 patients (58%) who received treatment with the TIC had acute arterial occlusions, and 11 of 26 patients (42%) had nonacute arterial occlusions. Nineteen of 26 patients (73.1%) received treatment of an infrainguinal occlusion, and 7 of 26 patients (26.9%) received treatment of a suprainguinal occlusion. Lower extremity native arteries were treated in 18 of 26 patients (69%), and lower extremity bypass grafts in 8 of 26 patients (31%). The technical success rate with TIC treatment was 92%, and the 30-day amputation-free survival rate was 96%. There was no difference in technical success or amputation-free survival rate between acute versus nonacute arterial occlusions, native artery versus bypass grafts, and suprainguinal versus infrainguinal arterial occlusions. Procedure time was 2.1 +/- 0.9 hours, and infusion time was 0.3 +/- 0.2 hours. There were no bleeding complications; however, 3 of 26 patients (11.5%) required further intervention to treat distal embolization. The overall mean cost for patients with TIC treatment was $3216 +/- $1740. CONCLUSIONS: Early results of TIC treatment in patients with arterial occlusions suggest that it is as effective as traditional catheter-directed thrombolysis. Furthermore, there were no bleeding complications, likely the result of TIC requiring shorter procedure and infusion times.  相似文献

OBJECTIVE: Chronic ischemia of the upper extremity requiring surgical revascularization is an uncommon condition. We analyzed modes of presentation, methods of operative repair, and follow-up in all consecutive patients with chronic ischemia of the upper extremity requiring arterial bypass. METHODS: Data prospectively entered into a vascular registry was retrospectively analyzed for all patients undergoing upper extremity arterial bypass from January 1, 1990, to June 30, 2003. Simple thromboembolectomy procedures and bypasses to an outflow target more proximal than the brachial artery were excluded. RESULTS: We identified 20 patients. Their mean age was 57 years, and 11 (55%) were women. Eight (40%) had diabetes, and five (25%) had renal insufficiency. Indications included exercise intolerance in 11 patients (55%), tissue loss in six (30%), and rest pain in three (15%). The etiology of ischemia was atherosclerosis in seven patients (35%) and complications of iatrogenic or civilian trauma in 13 (65%). The brachial artery was used as the inflow in 13 patients (65%), the axillary in six (30%), and the ulnar in one (5%). Conduits used included the great saphenous vein in 11 patients (55%), arm vein in 7 (35%), and prosthetic in 2 (10%). Outflow targets included the brachial artery in 12 patients (55%), the radial in five (25%), and the ulnar in three (15%). There were no perioperative deaths. One graft (5%) occluded 相似文献

目的 探讨非创伤急性下肢缺血的外科治疗方法及影响预后的因素,对比急性动脉栓塞和急性动脉血栓形成的发病率和预后.方法 回顾性分析1999年7月至2007年12月手术治疗的154例急性下肢缺血病例,所有病例均行股动脉或胭动脉切开、Fogarty导管取栓术.单纯取栓128例,Fogarty导管取栓+内膜剥脱术8例,Fogarty导管取栓+人工血管或自体大隐静脉转流术13例,Fogarty导管取栓+一期截肢术5例.按照病因将病例分为急性动脉栓塞组(99例)和急性动脉血栓形成组(55例),对比两组发病率、截肢率、病死率及截肢高危因素.结果 急性动脉栓塞组男性发病率(39.4%)低于女性(60.6%)(P<0.05);急性动脉血栓形成组男性发病率(72.7%)高于女性(27.3%)(P<0.05).所有患者的截肢率为9.7%,院内病死率为11.7%.急性动脉栓塞组截肢率(5.1%)低于急性动脉血栓形成组(18.2%)(P<0.05),急性动脉栓塞组院内病死率(11.1%)与急性动脉血栓形成组(12.7%)相当(P>0.05).两组病例截肢的共同高危因素是肢体缺血时间,急性动脉血栓形成组截肢风险还与吸烟和糖尿病有关.结论 急性动脉栓塞男性发病率高于女性,急性动脉血栓形成女性发病率高于男性,急性动脉栓塞截肢率低于急性动脉血栓形成,而急性动脉血栓形成比急性动脉栓塞具有更高截肢风险.  相似文献

Patients with severe tibioperoneal disease are poor candidates for a distal bypass. Absence of a distal target, lack of conduit, or multiple medical problems can make these patients a prohibitive risk for revascularization. Acute on chronic ischemia in this group poses a greater challenge. Thrombolytic therapy for acute ischemia can be prolonged and carries a significant risk of bleeding if continued beyond 24 hours. However, if the ischemic limbs can be isolated from the systemic circulation, a higher dose of the lytic agent can be given with lower risk. These are the initial results of a series of 10 patients who underwent percutaneous isolated limb perfusion with a high dose of thrombolytics for severe ischemia. Ten patients (lower extremity 8 and upper extremity 2) presented with severe limb-threatening ischemia. Mean ankle/brachial index (ABI) was 0.15 for the lower extremity, and there were no recordable digital pressures in patients with upper extremity ischemia. No distal target was visible on the initial arteriogram. These patients were then taken to the operating room, and under anesthesia, catheters were placed in an antegrade fashion via femoral approach in the popliteal artery and vein percutaneously. For upper extremity, the catheters were placed in the brachial artery and vein. A proximal tourniquet was then applied. This isolated the limb from the systemic circulation. Heparinized saline was infused through the arterial catheter while the venous catheter was left open. A closed loop or an isolated limb perfusion was confirmed when effluent became clear coming out of the venous port. A high dose of thrombolytic agent (urokinase 500,000 to 1,000,000 U) was infused into the isolated limb via the arterial catheter and drained out of the venous catheter. After 45 minutes, arterial flow was reestablished. In 4 patients, Reopro((R)) was used in addition to thrombolytics. Postprocedure angiograms showed minimal changes, but patients exhibited marked clinical improvement. The ABI changed from 0.15 to 0.5 in the lower extremity and near-normal digital pressures in upper extremity ischemia. Limb salvage and symptomatic relief at 6 months was 90%. All patients except one were kept on anticoagulation postoperatively. No bleeding complications were observed from the procedure. Percutaneous isolated limb perfusion brought symptomatic relief to patients presenting with acute on chronic limb ischemia. This can be an alternate option for patients facing amputation with no revascularization options.  相似文献

The efficacy, safety, and effects on hemostasis and coagulation of two doses of human tissue-type plasminogen activator in patients with acute and subacute peripheral arterial occlusion were compared. Seven patients with lower extremity ischemia and one patient with upper extremity ischemia had peripheral arterial thromboses (five arteries, three grafts) confirmed by clinical history, physical examination, and angiography. The duration of occlusion ranged from 31 hours to 30 days (mean 11.9 days). Tissue-type plasminogen activator was infused via a catheter directly into the thrombus at a randomly assigned dose of 0.05 mg/kg/hr (n = 4) or 0.025 mg/kg/hr (n = 4). Thrombolysis was complete in seven patients and partial in one. Duration of infusion ranged from 1 hour to 21 hours (mean 7.4 hours). The low dose required a longer infusion than did the high dose, but they were both successful in achieving thrombolysis. The one patient with partial thrombolysis had abrupt discontinuation of infusion when extravasation through a recently endarterectomized femoral artery developed. Otherwise there were no significant complications from tissue-type plasminogen activator therapy. Secondary procedures to correct underlying arterial disease were performed in five of the seven patients (71%) who had complete thrombolysis. Even at low dosages, infusion of tissue-type plasminogen activator into arteries or bypass graft thrombus produced complete thrombolysis, and no major complications occurred. This allowed more systematic effects to diagnose and treat underlying arterial disease.  相似文献

OBJECTIVES: Limb and pelvic ischemia are known complications after endovascular abdominal aortic aneurysm repair (EVAR). The objective of this paper is to present our experience with the incidence, presentation, and management of such complications. METHODS: Over 9 years 311 patients with aortic aneurysms underwent EVAR. A retrospective review identified 28 patients (9.0%) with ischemic complications. RESULTS: Among 28 patients with ischemic complications, 21 had lower extremity ischemia and 7 had pelvic ischemia: colon (n = 4), buttock (n = 2), and spinal cord (n = 2). Of the 21 patients with lower extremity ischemia, 15 had limb occlusions (71.4%), 3 due to embolization (14.7%) and 3 the result of common femoral artery thromboses (14.7%). Limb occlusions were manifested as severe acute arterial ischemia (n = 6), rest pain (n = 3), intermittent claudication (n = 5), and decreased femoral pulse (n = 1). Limb occlusions were managed with thrombectomy and stent placement (n = 4), femorofemoral bypass (n = 7), eventual explantation because of persistent endoleak (n = 1), and expectant management (n = 3). The 3 patients with occlusions managed expectantly all had intermittent claudication, which has subsequently improved. In the 6 patients with lower extremity ischemia due to embolization or common femoral artery injury presentation was acute, and embolectomy was performed, followed by femoral artery endarterectomy and patch angioplasty or placement of an interposition graft. One patient who had a prolonged postoperative course including cardiac arrest subsequently required distal bypass and ultimately above- knee amputation. Among the 7 patients with pelvic ischemia, 2 patients had unilateral hypogastric artery embolization before the original surgery. Among the patients with colonic ischemia, 3 were seen immediately postoperatively, and required colectomy and colostomy. Two patients who required urgent colectomy subsequently had multiple organ failure, and died in the perioperative period. One patient had abdominal pain 1 week after surgery, which was managed with bowel rest, with subsequent improvement. In 2 patients spinal cord ischemia developed immediately after surgery, which resulted in persistent paraplegia. Buttock ischemia developed in 2 patients, 1 of whom required fasciotomy because of gluteal compartment syndrome, and had transient renal failure. CONCLUSIONS: Ischemic complications are not uncommon after EVAR, and may exceed the incidence with open surgical repair. Limb ischemia is most often a result of limb occlusion, and can be successfully managed with standard interventions. Pelvic ischemia often results from atheroembolization despite preservation of hypogastric arterial circulation. Colonic and spinal ischemia are associated with the highest morbidity and mortality.  相似文献

目的:探讨急性肢体动脉栓塞取栓治疗效果的影响因素,提高治愈率。方法:回顾性总结分析1997年8月-2001年11月我们采用Fogarty导管对四肢急性动脉栓塞41例取栓治疗的情况。结果:41例中保肢成功26例,截肢8例,死亡7例。结论:术前肢体的缺血时间与缺血程度,术中血栓取出的彻底性,恢复血流后再灌注损伤与发生代谢性肌肾综合征以及患者的原发病与全身情况是影响动脉栓塞后手术治疗效果的主要因素。  相似文献

OBJECTIVE: Symptomatic arterial disease of the upper extremity is an uncommon problem. In this study, we evaluate our results with brachial artery reconstruction in patients who present with symptomatic atherosclerotic occlusive disease and compare this cohort's demographics with a similar group with lower extremity ischemia. METHODS: From 1986 to 1998, all patients presenting for upper extremity revascularization with chronic ischemia were prospectively entered into a vascular registry. Demographics, indications, outcomes, and patency were recorded. Patients presenting with embolus, pseudoaneurysm, or trauma were excluded. The Fisher exact and Student t tests were used to assess significance. RESULTS: Fifty-one (83%) bypass grafts were performed with autogenous conduit and the remainder with polytetrafluoroethylene. Indications included 18 (30%) patients with exertional arm pain, 35 (57%) with rest pain, and 8 (13%) with tissue loss. Twenty-five (45%) patients were male, 8 (14%) had diabetes, and 30 (54%) were smokers. The mean age was 58 years (range, 33-93). The operative mortality rate was 1.8%, and follow-up ranged from 1 to 140 months. Eight occlusions were identified, with six occurring early. Five of these were in women with a smoking history. Only one of the 26 reconstructions that did not cross a joint occluded, whereas bypass grafts that did cross a joint occluded more frequently. No other major complications were recognized. CONCLUSION: Arm revascularization for ischemia can be performed with reasonable mortality and morbidity rates. These patients may represent a different subgroup of atherosclerotic disease than those with lower extremity involvement: they are more commonly women and smokers and less likely to be diabetic.  相似文献

Complications related to balloon catheter thrombembolectomy in 717 cases of arterial occlusion occurred in 4.3% (= 31) and are classified in 6 groups: 16 perforations resp. ruptures (2.2%), 2 AV-fistulas (0.3%), 10 dissections (1.4%), 3 aneurysms (0.4%), intimal lesion and catheter failure. Inadequate handling is probably responsible in the majority of cases. Intraoperative angiography usually permits to recognize the complication and displays the details required for additional procedures.  相似文献

A neonate with idiopathic brachial artery occlusion was recently treated at the San Bernardino County Medical Center. At birth, the right upper extremity was cold, pulseless and cyanotic. Arteriography demonstrated brachial artery occlusion, following which thrombectomy and postoperative heparinization resulted in a viable limb. This represents an unusual case of neonatal arterial occlusion with diagnostic preoperative arteriography and successful Fogarty thrombectomy. Possible etiological factors, treatment alternatives, and long term prognosis are discussed in this report.  相似文献

Background : The present study reviews the nature of upper gastrointestinal complications of hepatic arterial chemoperfusion at a tertiary referral centre for the treatment of hepatic malignancy. Methods : The patients involved in the present study all had major upper gastrointestinal (GI) haemorrhage and were undergoing hepatic arterial chemoperfusion. Results : Eight patients had major upper GI haemorrhage. Three of these patients were not referred for surgical management, and all three patients died. The five patients who were admitted or transferred to our unit and who underwent surgery all survived. Conclusions : These complications are probably caused by extravasation of 5-fluorouracil (5-FU) following thrombosis of the gastroduodenal artery. The resulting cavity may perforate into the hepatic artery, portal vein, duodenum or biliary tree. Surgeons and oncologists should be aware of these complications. If upper abdominal pain occurs, chemoperfusion should cease immediately and an urgent investigation, which may include catheter angiography, gastroscopy and computed tomography (CT) scanning. should be carried out to exclude an hepatic artery pseudo-aneurysm.  相似文献

目的探讨下肢动脉腔内成形治疗下肢动脉旁路移植后血管闭塞严重缺血的疗效。方法回顾分析2004年1月~2012年9月6例因下肢动脉旁路移植后血管闭塞出现严重缺血而接受下肢动脉腔内成形的临床资料。结果 6例技术均获成功,无并发症或严重心血管不良事件发生。随访时间1~79个月,平均23.5月。术后溃疡愈合,踝肱比值(ABI)0.90~1.07,患肢保存。结论初步观察,下肢动脉腔内成形治疗下肢动脉旁路移植后血管闭塞严重缺血,疗效满意。  相似文献

目的 探讨急性上肢动脉栓塞的合并疾病、栓塞部位、手术与非手术及治疗前缺血时间等因素与疗效的关系.方法 回顾1988年7月至2008年1月约20年问62例急性上肢动脉栓塞的临床资料,采用累加Logit回归模型分析数据,以年龄、性别、心功能情况、栓塞部位、手术与非手术治疗、缺血时间等为参数,分析这些因素与临床疗效的相关关系.结果 本组62例,男33例,女29例,年龄35~86岁,平均63.5岁,男女之间差异无统计学意义(P>0.05).62例中37例接受了Fogarty球囊导管取栓术,25例接受非手术治疗而给予溶栓、抗凝、祛聚治疗.其中55例(88.7%)的肢体缺血状态得到改善,截肢2例,死亡2例.累加Logit回归模型分析表明治疗开始时间、心功能情况、手术取栓对决定预后有统计学意义(P<0.01);而年龄、性别和栓塞部位对疗效无统计学意义(P>0.05).而非手术的25例中,23例缺血状态得到改善.结论 对于急性上肢动脉栓塞,8 h以内Forty导管取栓术是最有效的治疗方法 ,而对于无法耐受手术的高危患者,积极的药物治疗仍可能改善栓塞肢体的缺血状态.  相似文献

目的: 探讨急性下肢缺血的救治方法。方法:回顾性总结4年来我科收治的96例(106条肢体)急性下肢缺血患者的临床资料。106条患肢均采用外科手术治疗;主要用Fogarty管在手术室或介入室DSA监测下行取栓,8例(12条肢体)患者在取栓的基础上进行解剖外人工血管转流。结果:术中髂、股、腘、胫动脉取栓成功70例(75条肢体),11例(12条肢体)腘动脉以远取栓不成功;8例(12条肢体)股动脉近心端受阻,远心端通畅者4例行腋-双股转流,4例行股-股人工血管转流。1例(1条肢体)因胸腹主动脉夹层造成下肢缺血行腹主动脉夹层开窗术。6例(6条肢体)行近端取栓加远端I期截肢。最终本组76例(85条肢体)救肢成功,成功率79.2%(76/96);11例(11条肢体)截肢,截肢率11.4%(11/96);9例(10条肢体)死亡,死亡率9.4%(9/96)。对76例(85条肢体)成功保肢者随访1~38个月,7例(7条肢体)因再栓塞或血栓形成再手术治愈。结论:取栓术是急性下肢缺血的有效治疗方法,DSA下监测取栓有助于提高疗效。手术的效果取决于肢体缺血的时间和程度以及并发症的处理。  相似文献

背景与目的:终末期肾病(ESRD)是各种原因导致的慢性肾脏疾病进展到终末期阶段,大部分ESRD患者选择血液透析肾脏替代治疗,而功能良好且通畅率令人满意的血管通路对其至关重要。上肢是患者构建血液透析通路的首选位置,但部分患者因各种原因存在上肢血管或中心静脉资源耗竭等问题,无法构建上肢血管通路。对此,可以考虑构建下肢人工血管移植物内瘘(AVG)作为患者长期血液透析的通路,本研究回顾性分析构建下肢AVG患者的临床资料与中远期随访结果,以期为此提供经验证据。 方法:回顾2014年3月—2018年11月南方医科大学南方医院血管外科32例构建下肢AVG的ESRD患者临床资料与随访资料,分析患者的相关临床指标,用Kaplan-Meier法计算患者术后的初级通畅率和次级通畅率。 结果:全组患者术后均能触及AVG震颤,围术期1例患者出现慢性心功能不全急性发作,予药物、加强透析治疗后未见明显好转后死亡;2例患者出现移植物血栓形成,切开导管取栓后造影未见吻合口及流出道静脉狭窄,术后AVG恢复通畅,顺利透析。所有患者围术期间无感染、透析通路相关性肢端缺血综合征(HAIDI)、假性动脉瘤等并发症发生。术后随访1~60个月,中位随访时间14.1个月。随访期间出现AVG狭窄13例(40.6%),闭塞10例(31.3%),经外科手术或腔内手术修复后恢复AVG通畅;人工血管移植物感染3例(9.4%),其中1例患者移植物局段感染,行人工血管移植物感染段切除,自体静脉移植术后顺利保留AVG并用于透析,2例患者因移植物全段感染予全段切除。期间未出现假性动脉瘤、HAIDI等并发症。患者术后初级通畅时间为(20.4±3.32)个月,1、2、3年初级通畅率为64.6%、44.7%、19.6%;次级通畅时间为(38.7±5.52)个月,1、2、3年次级通畅率为79.6%、79.6%、54.6%。 结论:下肢AVG经修复后可达到较满意的次级通畅率,对于无法建立上肢血管通路患者而言,是安全、有效的通路选择。规范的术前评估、手术及穿刺中严格的无菌操作以及规律的随访是保证其远期通畅的重要因素。  相似文献
