我国目前围产儿死亡的主要原因已从先天畸形转为围产期缺氧,同时这也是远期婴儿患病率及致残率的主要原因之一。孕妇与胎儿远程监护(maternal and feta ltelemonitoring)是将胎儿监护与远程通讯技术相结合的一项技术,使孕妇可以在有通讯网络的任何地方、任何时候,通过远程监护系统让医护人员实时地了解胎儿宫内储备情况,有无宫内缺氧等表现。其核心内容是把在家庭中监测到的胎心以声音或变化曲线图形传输到远程胎儿监护中心计算机上,通过人工和计算机分析做出准确判断,并结合病情提出合理建议,从而确保孕妇及胎儿平安的监护系统。我院较早引进了远程胎儿电子监护系统,对399例孕妇进行了远程胎儿监护,现就远程胎儿监护技术的临床应用及护理要点总结如下。 相似文献

IntroductionDisasters of any kind can affect public health severely. A shortage of health care specialists, such as physicians and nurses, during a disaster is a challenge for health care systems. The use of technology is 1 emerging strategy for addressing the continually increasing demand for care. Moreover, nurses may use technology in their roles. Therefore, the purpose of this study was to identify the application of telehealth, and more specifically telenursing care, in incidents and disasters.MethodsThis systematic review study was conducted on the basis of the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. English language international databases (PubMed, Scopus, Embase, Web of Science, and Google Scholar) were searched through November 2018. The quality of the studies was assessed using the International Narrative Systematic Assessment tool.ResultsOf the 5,759 titles identified in this search, 17 studies met the inclusion criteria. The important findings of this study were grouped into 3 main categories: clinical teams, disaster and communication types, and key outcomes of the telehealth programs used in disasters and incidents. We did not find any articles in the field of telenursing care during incidents.DiscussionProviding health care during a disaster is essential, and technology is of vital importance for such care. Because of the shortage of specialized nurses in disaster areas, the presence of such a group in the telehealth program will provide a new window for care. Universal design and assistive technology present advantages and disadvantages in accommodating the needs of people with disabilities. The best solution may be a combination of the two, using universal design wherever possible and commercially practical and using assistive technologies wherever it is necessary or provides sufficient additional advantage to the user. Three approaches are discussed for the individual who is unable to interact with their world: change the individual, provide them with tools they can use, or change the environment. Examples of each are illustrated using personal workstations and shared, public, and encountered systems. The final decision may rest on commercial practicality, and several new technologies are explored. Ultimately, we need to continue to move forward both on the universal design and the assistive technology fronts if we are to address the needs of people with disabilities and those who are aging. 相似文献

Telenursing was instituted as an effective mode for providing care to patients geographically distant from healthcare providers. Using telecommunications and information technology, nursing care is provided remotely to individuals. Nurses recognize the value of telecare and telehomecare as essential components of telenursing that give patients easy access to high-quality care and eliminate costs and difficulties associated with travel to healthcare facilities. Patient satisfaction with telenursing is related to prompt quality care from professional nurses. Aims and Objectives
Interaction between caller and telenurse in telenursing is important for caller satisfaction and subsequent compliance. Despite this, satisfaction measures with focus on interaction in telenursing are scarce and rarely anchored in nursing theory. The aim was to evaluate the psychometric properties of the Telenursing Interaction and Satisfaction Scale (TISS) with focus on data quality, factor structure, convergent validity, and reliability.Methodological Design and Justification
This psychometric study was based on cross-sectional data.Research Methods, Instruments, and/or Interventions
Callers to the National Medical Advisory Service in Sweden (n = 616) completed the 60-item Telenursing Interaction and Satisfaction Questionnaire based on Cox's Interaction Model of Client Health Behavior. Twenty-five of these items were selected to form the TISS in four subscales according to the model. Data quality was evaluated in terms of missing data patterns and score distributions. The factor structure was evaluated with confirmatory factor analysis for ordinal data, convergent validity with Spearman correlations, internal consistency with ordinal alpha, scale reliability with composite reliability coefficients, and test–retest reliability with intraclass correlations.Results
The amount of missing data was acceptable and equally distributed. Data deviated significantly from a normal distribution. All response options were endorsed. The factor analysis confirmed the hypothesised four-factor structure; factor loadings ranged from 0.56 to 0.97 and factor correlations were high (0.88–0.96). Internal consistency (ordinal alpha = 0.82–0.97), scale reliability (0.88–0.99), and test–retest reliability (ICC = 0.77–0.86) were satisfactory for all scales.Study Limitations
The study design did not allow drop-out analysis.Conclusions
The TISS showed satisfactory psychometric properties in the study sample. It provides a measure that enables quantitative measurement of caller satisfaction with interaction in telenursing. 相似文献18.

ObjectivesThis study aimed to investigate the impact of telenursing on the hope and perceived self-efficacy of the mothers of premature infants after discharge from the neonatal intensive care unit.MethodThe study method was a clinical trial. The research population consisted of all the mothers with premature infants whose neonates had recently been discharged from NICU. Seventy subjects were selected through convenience sampling and randomly assigned to intervention and control groups. The intervention group subjects received telenursing training for four weeks, while in the control group, no intervention was offered. The data collection tools included the Perceived Maternal Parenting Self-Efficacy Questionnaire (PMPS-Q) and the Hope Scale of Mothers of Premature Neonates Admitted to the Intensive Care Unit. Data analysis was performed using an independent t-test, two-way repeated-measures ANOVA, and one-way repeated measures ANOVA.ResultsThe results showed that telenursing improves hope (P < 0.05) and perceived self-efficacy (P < 0.05) in the mothers of premature infants after discharge from the NICU.ConclusionAccording to the obtained results, it is suggested that the necessary measures be taken to establish distance nursing in the neonatal intensive care unit. As a result, mothers of premature infants and the country's health system can benefit from the advantages as mentioned earlier. 相似文献

按照"保基本、强基层、建机制"的新医改要求,为促进和提高基层医务人员的服务水平,研究设计了"远程医学教育系统"并在全国推广应用。本文对系统组成、软硬件标准、系统结构以及应用效果进行了阐述。迄今为止,已利用该系统对累计177800余人次医护人员提供了各类培训,在培训专业人才,提升医务人员业务技术水平方面发挥了重要作用。 相似文献
