开业护士(Nurse Practitioner)是高级临床护理注册护士(advanced practice registered nurse,APRN)中的一个角色,美国护士协会关于APRN的定义是这样描述的:APRN是获得高等教育并分别具有儿科、麻醉科、老年科、新生儿或精神科等领域专长的注册护士。它包括开业护士、临床护理专家、持证护理助产士、持证注册护士麻醉师。开业护士是承担高级临床护理角色的执业护士,并根据法律规定在一定范围内具有诊断、处方、转治建议的权限。主要在初级卫生保健场所或急症医院。结合我国现有的医疗护理现状,笔者认为开业护士的培养能从根本上缓解公民看病难、候诊时间长等热点问题,因此本文旨在介绍国外开业护士的发展,以此探讨开业护士在中国的开展提供依据。 相似文献

美国开业护士(NP)是具有硕士或博士学位的护士或具备执业资格的注册护士,能进行全面的患者评估,具备提出诊断、鉴别诊断、命令、执行、监督、解释诊断和实验室检查的能力。美国NP当前已有50年的发展历史,形成了清晰的核心能力框架,并在核心能力培养、发展和评价等方面都有较为深入的探索,而我国尚无对NP清晰的角色定义,也无统一的培养范式及相关岗位设置,且专业型护理研究生(MNS)仅为教育层面的概念,使MNS具备核心能力,胜任高级护理实践工作,仍需广泛探索。本文通过对美国NP核心能力框架、学位教育培养NP核心能力、继续教育发展NP核心能力和分层晋级计划评价NP核心能力进行综述,旨在为我国MNS能力培养及执业提供借鉴。 相似文献

PURPOSE: To provide the primary care provider (PCP) with a review of the clinical presentation, aggravating factors, and basic options for treatment of atopic dermatitis (AD). DATA SOURCES: Selected scientific literature and clinical experiences of the author. CONCLUSIONS: AD is a common, chronic skin condition that is frequently seen in both children and adults. Although AD can be challenging to treat, a number of therapeutic options exist to help alleviate the pruritus and rash that accompany the disorder. IMPLICATIONS FOR PRACTICE: The PCP can manage AD in most patients by prescribing avoidance measures, good skin care, antihistamines, and conservative topical medications. Patients with more severe disease may require aggressive therapies such as phototherapy, balneo-phototherapy, or systemic agents. 相似文献

Anabolic steroids are being used by athletes in a wide variety of sports in efforts to enhance their athletic performances. Steroid abuse is complex to evaluate due to the highly emotional subject matter and the limitations in researching anabolic steroids. This article has been written to heighten the practitioner's awareness of the problem of "sports doping" with anabolic steroids. It will provide practical information on possible consequences of steroid use and outline essential information to obtain through the history, physical exam and laboratory studies. Intervention strategies based on the three levels of prevention are described. With awareness of the problem of sports doping and knowledge of how to deal with it in primary care, the nurse practitioner can enhance the health care provided to aspiring athletes, athletes and retired athletes. 相似文献

Americans are increasingly using herbs and herbal preparations to treat a wide array of conditions ranging from colds to depression. Children and parents are ingesting herbs in greater quantities, as reflected by increased herb sales and telephone calls to poison information centers. Herbal preparations are being used for the management of insomnia, anxiety, and attention deficit hyperactivity disorders. This trend introduces interesting opportunities and dilemmas for the school nurse. This article provides an overview of the seven most common medicinal herbs (garlic, ginkgo, ginseng, Echinacea, chamomile, valerian, and feverfew). This report reviews the history of nurse practitioner reimbursement legislation specifically as it pertains to neonatal nurse practitioners (NNPs) in the tertiary care setting (inpatient/acute care). Barriers to reimbursement are reviewed, along with ways in which NNPs can continue to demonstrate their worth to the public and to health insurance providers. Specific steps are discussed on how hospital and health care providers are reimbursed for services rendered; obtaining a billing provider number is the initial step of establishing a billing service. Each state varies on how provider numbers are obtained and who is eligible to receive these numbers. Care must be taken to ensure that duplicate billing by different providers is avoided. The state's Medicaid Provider Manual is often helpful in determining which services are covered and which guidelines accompany each billable code. As tertiary care and/or follow-up providers, it is our responsibility to educate ourselves about the nurse practitioner reimbursement laws currently in effect and educate our legislators about our critical role in providing care to the sick inpatient population. Today's rapidly changing health care environment directly affects nurse practitioners. NPs will be practicing in increasingly competitive situations and with mounting pressures for cost-effective services. Ignoring the issues in the health care marketplace can threaten NP survival. This article will analyze the changes in the health care system, relate the economic effects of these changes to nursing practice, and apply a homeostatic model of interprofessional competitive behavior. In 1988, the National Organization of Nurse Practitioner Faculties appointed an ad hoc education committee (AHEC) to review existing nurse practitioner (NP) education curricula and to formulate national guidelines. An overview of the new NP guidelines are presented along with the philosophy, content, objectives, and competencies that are viewed as the foundation for NP education today. The guidelines are presented within the structure outlined in Brykczynski's (1985) research about NP practice. Thus, as a "product" of the Advanced Nursing Practice: Nurse Practitioner Curriculum Guidelines (NPCG), a new NP graduate for the 1990s could be described as one who demonstrates beginning competencies in primary health care theory and research within the five domains of practice identified by Brykczynski. 相似文献
