Traumatic anterior dislocation of the hip

Summary A discussion is presented of traumatic anterior dislocation of the hip joint on the basis of three representative case reports. One patient had an iliac or pubic dislocation, the other two were obturator dislocations, including one with an associated femoral head fracture. All three patients were treated without surgery. After an average follow-up time of twenty-seven months, the results were excellent. Therepeutic guide-lines and recommendations are given for this infrequent type of injury.

A case of traumatic anterior dislocation of the hip is presented. Reduction was easily achieved but was unstable because of disruption of the iliofemoral ligament by detachment of the anterior inferior iliac spine. A stable reduction was achieved by operative reattachment of the anterior inferior iliac spine.

Summary An unusual traumatic anterior dislocation of the hip in a 47-year-old woman is reported. The mechanism of trauma behind anterior hip dislocations and their diagnosis are described and the prognosis of hip dislocations in general is mentioned.

Traumatic hip dislocation in childhood

Traumatic dislocation of the hip in childhood is uncommon and can be a consequence of minor trauma. The authors report a series of 35 dislocations in skeletally immature patients. Most were isolated posterior dislocations without acetabular lesions. In 75% of cases, reduction of the dislocation was easy. Nine children required surgery to remove interposed joint capsule and/or osteochondral fragments to achieve anatomic reduction. Outcomes were generally good, except in one patient in whom a displaced fracture of the femoral physis was followed by total head avascular necrosis. One case of partial necrosis had a satisfactory outcome. Epiphyseal necrosis, though uncommon, appeared to be inconsistent to prevent and hard to predict. Bone scan seems to be more effective than MRI for the detection of necrosis.     

Traumatic hip dislocation in childhood

Twelve children with hip dislocation were recorded from 18 hospitals of the GDR in a collective study conducted on behalf of the Working Group on Paediatric Traumatology. The closed approach was exclusively taken to all reductions, and all of them were performed within the twelve-hour limit. Avascular necrosis of the femoral head or other growth disorders were not observed. Relief periods were conspicuously differentiated, which gave rise to the proposal of unitisation of the therapeutic approach.     

Traumatic dislocation of the hip in children

In the current study, the incidence, presentation, and treatment of traumatic dislocation of the hip in children will be discussed. The complications and the possible pathologic changes behind the development of avascular necrosis are described. Most children have a good outcome after this injury, perhaps because a concomitant fracture is unusual. The results in children are significantly better than results in adults.

Three children aged 7, 9 and 11 were treated because of traumatic dislocation of the hip. There was one case of anterior and two cases of posterior iliac dislocation. The reduction was performed under general anaesthesia directly after admission. In two children derotational shoe for 6 weeks and a plaster for 3 weeks in third case were used in after treatment. The unweighing of the limb was maintained for 6 weeks more. Subsequently, the gradual weight bearing was allowed. Radiographic examination of two children 1.5 and 3.75 years after injury revealed normal hip joints. There was no restriction of physical activity of these children.
