应用钛镍记忆合金牵张成骨增高下颌牙槽嵴的初步研究   总被引:20,自引:2,他引:20  
目的 探讨应用钛镍记忆合金牵张成骨增高下颌牙槽嵴的可行性。方法 选用成年杂种犬10只,拔除两侧下颌全部前磨牙和第1磨牙,实验组1个月后用完全埋置的钛镍记忆合金牵张器完成牵张手术。在牵张手术前及术后1、5、13周测量术区颌骨高度,拍X线片,分别在牵张完成后1、3个月处死动物,进行组织学研究。结果 牵张器就位后骨块即开始牵张升起,术后1周达7.5~11.5mm,其后无明显变化。X线片可见牵张完成后1个月牵张区骨密度增高,有新骨生成;3个月骨密度与周围牙槽骨接近。组织学观察牵张区早期有胶原束形成,随后钙化成骨。结论 用钛镍记忆合金牵张器可成功牵张增高犬下颌后牙区牙槽嵴,新骨以膜内成骨方式生成。  相似文献

The simultaneous use of cleft reduction and maxillary advancement by distraction osteogenesis has not been applied routinely because of the difficulty in three-dimensional control and stabilization of the transported segments. This report describes a new approach of simultaneous bilateral alveolar cleft reduction and maxillary advancement by distraction osteogenesis combined with autogenous bone grafting. A custom-made Twin-Track device was used to allow bilateral alveolar cleft closure combined with simultaneous maxillary advancement, using distraction osteogenesis and a rigid external distraction system in a bilateral cleft lip and palate patient. After a maxillary Le Fort I osteotomy, autogenous iliac bone graft was placed in the cleft spaces before suturing. A latency period of six days was observed before activation. The rate of activation was one mm/d for the maxillary advancement and 0.5 mm/d for the segmental transport. Accordingly, the concave facial appearance was improved with acceptable occlusion, and complete bilateral cleft closure was attained. No adjustments were necessary to the vector of the transported segments during the activation and no complications were observed. The proposed Twin-Track device, based on the concept of track-guided bone transport, permitted three-dimensional control over the distraction processes allowing simultaneous cleft closure, maxillary distraction, and autogenous bone grafting. The combined simultaneous approach is extremely advantageous in correcting severe deformities, reducing the number of surgical interventions and, consequently, the total treatment time.  相似文献

BACKGROUND: The aim of this clinical study was to evaluate bone elongation and the effectiveness of distraction osteogenesis when utilizing an extraosseous distractor to treat cases of atrophic posterior mandible. METHODS: Fourteen surgical sites were evaluated in 10 healthy, non-smoking, female patients, with varying degrees of atrophic posterior mandible, who underwent surgery by alveolar bone distraction. The patients presented between 6 and 10 mm above the mandibular, inferior alveolar nerve. Panoramic radiographs were taken before surgery and at the beginning and end of the consolidation period. The extraosseous distractor was fixed to both the basal and the osteotomized bone. After a 7-day latency period, the patients rotated the distractor rod three times a day for 3 to 10 days (1 mm per day). The mobile segment was held in place for 8 to 12 weeks for bone consolidation. RESULTS: The results revealed a distraction range of 2.32 to 8.11 mm (mean distraction +/- standard deviation, 5.12 +/- 1.67 mm), which was less than the real, measured distance between the upper and lower miniplates of the distractor (5.58 +/- 1.62 mm). The bone segments showed between 0.03 and 2.53 mm (mean, 0.88 +/- 0.59 mm) resorption above the upper miniplate. The efficacy of the extraosseous distractor was calculated as between 30.41% and 94.58% (73.45% +/- 20.32%). CONCLUSIONS: Bone regeneration was obtained in all cases; however, when planning alveolar bone distraction using the extraosseous distractor, greater distraction should be performed to compensate for the inclination of the distractor rod and possible bone loss. This adjustment should increase the efficacy of distraction osteogenesis.  相似文献

This study analyzed in three dimensions the longitudinal growth pattern of young patients with hemifacial microsomia (HFM) before and after mandibular distraction osteogenesis (DO). Six individuals with HFM (five boys and one girl; age at distraction, 12.5 +/- 2.4 years) were treated with similar procedures (surgery, type and direction of distraction, no functional orthodontic treatment before and after DO). Two individuals who did not undergo DO until late in their growth were used to compare growth patterns. In addition, one individual besides the six previously chosen sample in whom no DO was performed was also used to compare longitudinal growth patterns. Lateral and posteroanterior (PA) cephalograms were utilized preoperatively, spanning a period of 9 years. Computerized three-dimensional models were constructed from the lateral and PA cephalograms using a vector intercept algorithm. In the comparison group, for a period of 8 years on the affected side, the ramus height, body length, and total mandibular length increased at an average rate of 1.3, 1.9, and 3.0 mm per year respectively. On the unaffected side, the ramus height increased by 2.1 mm per year, 1.9 mm in the body, and 2.9 mm per year in total mandibular length. On average, the gonial angle on the affected side was increased by 1 deg per year, yet decreased by 1 deg per year on the unaffected side. The proportions between the affected to the unaffected side were maintained. In the six individuals 18 months after DO, it was found that the ramus height was reduced by 1.0 mm, whereas the body was found to resume its growth with a faster rate on the distracted side, maintaining its proportion. Angular changes demonstrated closing of the gonial angle on both the unaffected (0.5 deg) and distracted (3.5 deg) sides. Observed in three dimensions were the following: (1) On average, unoperated patients with isolated HFM tend to maintain their asymmetrical facial proportions and do not worsen substantially with time. (2) Different treatment effects were seen on the ramus, body, and total length of the mandible: changes in body length > ramus height > total length. (3) Eighteen months after DO, the correction was stable but with some degree of settling back from the initial overcorrection (< 5%). (4) Eighteen months after DO the mandibular body was found to have greater growth than the ramus. (5) Evaluating changes in three dimensions provides an improved understanding of the growth pattern and distraction effects on the mandible and its structural components. (6) Additional studies on the effect of mandibular distraction on other conditions involving mandibular deformities are required. In addition, the effect of various distractor devices should also be evaluated. Three-dimensional evaluation is recommended for improved accuracy.  相似文献

Distraction osteogenesis devices followed by hybrid functional appliance therapy and fixed orthodontic appliance therapy were used to correct a variety of maxillofacial skeletal and dental deformities in 4 patients. The patients underwent procedures to lengthen the mandibular ramus and body. Transcutaneous pins were used to activate the distraction devices. After achieving the desired skeletal position, the distraction devices were maintained for 2 to 3 months to allow ossification. Following distraction, functional orthodontic appliance therapy was initiated during growth to correct the cant of the occlusal plane by extrusion of teeth on the affected side for improved facial symmetry. Fixed orthodontic therapy was used for final occlusal adjustments. All patients achieved lengthening of their jaws and substantial improvement in facial symmetry and occlusion. The follow-up period for this group of 4 patients ranged from 18 to 36 months.  相似文献

目的 观察猴下颌骨牵引成骨的动态过程 ,探讨其成骨方式及机理。方法 恒河猴 10只行单侧下颌骨牵引 ,间歇期 5天 ,牵引速度 0 .5mm× 2次 /日 ,共 15天。于牵引后不同时期拍摄X线片 ,行四环素荧光双标记 ,将牵引区骨块标本平分 ,上部骨块制作不脱钙树脂切片 ,5u切片行甲苯胺蓝染色 ,2 0u切片荧光显微镜下测骨组织沉积率 ;下部骨块制作脱钙石蜡切片 ,HE染色。结果 牵引后早期 ,牵引区以平行于牵引方向排列的胶原纤维为主 ,可见纤细骨小梁及大量骨吸收陷窝 ,X线片上表现为低密度透光区。稳定期新骨逐渐由两端向中间形成 ,牵引后 4周 ,牵引中心充满平行排列的骨小梁 ,新骨密度仍低于正常 ;牵引后 6~ 12周 ,牵引区粗大的骨小梁呈网状排列 ,可见成熟哈佛氏系统 ,新骨表面形成骨皮质 ,X线片表现接近正常。牵引完成时骨组织沉积率为对照组的4.2倍 ,在稳定期逐渐减慢 ,但牵引后 12周时仍较对照高 40 .91%。结论 缓慢有规律地牵引下颌骨可明显促进新骨形成速度 ,延长骨段。牵引后足够时间的稳定期是新骨生成的必要条件。牵引区主要以膜内成骨方式形成新骨 ,成骨过程为新生与改建并存。  相似文献

A 49-year-old patient presented with an Angle Class III malocclusion with a partially edentulous mandible, as diagnosed by orofacial examination and radiographic and cephalometric analyses. The patient refused orthognathic surgery; therefore, the treatment plan included the preparation of all teeth and fabrication of provisional restorations to reestablish optimal occlusion. To allow for the placement of 3 implants, the edentulous posterior mandibular ridge was improved via alveolar distraction. The patient was recalled 3, 6, 12, and 24 months after prosthodontic treatment. The oral situation was stable and patient satisfaction was reported as high.  相似文献

We report a case of dentoalveolar rehabilitation employing the technique of alveolar distraction osteogenesis in a 25-year-old male patient with a right posterior mandibular defect that was a result of previous tooth extractions and consequent severe loss of alveolar bone. A new method for vector control during alveolar distraction osteogenesis was used. This method consisted of placing an orthodontic wire on an immediately loaded implant (in the right third molar position) and securing it by means of resin composite to the implant and the adjacent tooth. Implant positioning and soft tissue management were performed successfully. The described treatment strategy appears to have good potential for providing an ideal final position of the regenerated bone in cases where major tissue defects exist in the posterior mandible.  相似文献

The restoration of the edentulous atrophic anterior maxillary ridge has proved difficult. The absence of both gingival and bony alveolar tissue contributes to these problems. Numerous techniques to address these issues have been described. We report a case of a patient who sustained multiple facial injuries as a consequence of an accident, including the loss of the maxillary incisor teeth and associated alveolus. This patient underwent fixed implant supported prosthetic rehabilitation, following the use of distraction osteogenesis in the reconstruction of the edentulous atrophic anterior maxillary ridge.  相似文献

INTRODUCTION: When reconstructing the mandible after tumour resection with a fibular graft, the mandible is often vertically deficient, making placement of dental implants impossible. PATIENTS: Segmental vertical distraction of the reconstructed mandible was performed in nine patients following tumour surgery between February 1998 and 2001. Their age was 14-65 years (average 46.3); all underwent radiotherapy with a dose of up to 55.6 Gy prior to tumour resection. Mandibular discontinuity was repaired with a microvascular fibular bone graft. All grafts had a vertical bone deficit ranging from 9 to 12 mm when compared with the non-resected part of the mandibles. METHODS: All patients underwent segmental vertical distraction of the transplants. The distraction devices were applied intraorally. Distraction of 1.0 mm/day was performed using a Martin distractor (TRACK 1.5) followed by 12 weeks retention time. RESULTS: The increase of vertical bone height was stable and enabled placement of dental implants without any complications. CONCLUSION: Vertical distraction osteogenesis may become a common procedure in the treatment of alveolar ridge deficiency resulting from transplanting fibulae for mandibular reconstruction following tumour surgery.  相似文献

目的:探讨三焦点牵引成骨技术在下颌骨骨段缺损重建中的应用和在重建骨行种植义齿修复牙列缺失的可行性。方法:4例下颌体骨段切除患者,使用内置式三焦点牵引器同期行下颌骨缺损牵引成骨重建。牵引间歇期8天,牵引速度0.25mm×4次/d,稳定期4个月;1例于稳定期结束后在成骨区行种植义齿修复。结果:下颌骨牵引成骨最长45mm。牵引成骨区骨量充足,组织结构及硬度接近正常颌骨。牵引成骨区种植义齿可满足正常功能的需要。结论:三焦点牵引成骨技术可成功用于下颌骨骨段缺损的功能性重建,并可满足种植义齿修复的要求。  相似文献

When performing distraction osteogenesis, the osteotomy is normally applied to the cortical bone posterior to the mandibular second molar. We measured the topographic thickness of the cortical and trabecular bone of the mandibular ramus and at the mandibular canal (MC) to provide crucial anatomic data aimed at minimizing complications and elucidating the most appropriate site for placing the distractor. Forty sides of the mandibles were prepared from 20 Korean cadavers (10 men and 10 women with a mean age of 68 years). The specimens were scanned and reconstructed into three-dimensional images using a micro-computed tomography system. Coronal and horizontal sectional images of the mandibular ramus were taken at thickness intervals of 2 mm from the reconstructed three-dimensional images. Image analysis software was used to measure the thicknesses of the cortical and trabecular bone and to identify the locations of the MC within the body and the mandibular ramus on each section. The mean thicknesses of the buccal cortical plate, trabecular bone, and lingual cortical plate were 2.9 mm (men, 3.0 mm; women, 2.8 mm), 9.1 mm (men, 9.8 mm; women, 8.5 mm), and 2.2 mm (men, 2.3 mm; women, 2.1 mm), respectively. The distance from the buccal surface of the mandible to the MC increased from 5.3 to 10.0 mm (men, 5.3-10.0 mm; women, 5.3-9.1 mm) when moving progressively anterosuperior from the mandibular angle region. Morphometric analyses of the mandibular ramus can provide crucial data when performing mandibular osteotomy and locating an appropriate placement site for a distractor device during the distraction-osteogenesis procedure.  相似文献

目的 研究下颌骨牵引成骨术对下牙槽神经功能的影响。方法 对青年恒河猴 16只 ,全麻下行下颌角部骨截开术及牵引器置入术 ,术后第 5天开始以 0 5mm× 2次 /d的速度牵引 ,共15d。采用八导肌电图仪 ,表面电极在颏孔处刺激 ,针电极在卵圆孔附近记录 ,分别在术前、牵引完成后 0、2、4、6、9、12周时行感觉神经动作电位测试。结果 牵引完成时潜伏期较术前增加 2 2 18% ,其后逐渐降低 ,至牵引完成后 12周时仍较术前高 10 70 %。牵引完成时波幅降至术前的 2 8 5 4% ,牵引后 12周时恢复至术前的 99 84%。结论 下颌骨牵引成骨术对下牙槽神经的功能会产生暂时性影响 ,随着神经髓鞘和轴突的再生 ,神经功能可逐渐恢复正常。  相似文献

目的探讨采用内置式三焦点牵引器在犬下颌骨骨段缺损功能重建中牵引成骨的特点、规律。方法使用内置式三焦点牵引器对4只成年犬下颌骨骨段缺损进行牵引成骨重建。制作3cm下颌骨缺损区,同期植入内置式三焦点牵引器。牵引间歇期8天,牵引速度0.5mm×2次/天,稳定期3个月。结果下颌骨牵引成骨约30mm,牵引成骨过程中无感染、成骨不良等并发症。牵引完成后第3个月X线片、组织学观察及扫描电镜观察均可见新生骨的形成,新生骨的形态、组织结构接近正常下颌骨。结论三焦点牵引成骨技术重建的颌骨形态、组织结构和功能接近正常颌骨,牵引成骨区和压力成骨区局部应用rh-BMP2可以促进新骨的形成及钙化。  相似文献

In this article, a procedure involving 2-stage alveolar distraction osteogenesis using eccentric distraction devices for the augmentation of resorbed transplanted iliac bone following mandibular tumor resection is presented. A 6-month consolidation period was allowed between the first and second distractions, and endosseous implants were placed 4 months after the second distraction. Computerized tomographic images obtained before the implantation revealed that, 10 months after the first distraction, the bone generated still showed lower density compared with the basal bone, but the bone from both distractions showed enough maturity for implantation. It may be concluded that 2-stage alveolar distraction osteogenesis can be a useful and safe procedure for excessive alveolar lengthening if a sufficiently long consolidation period is allowed.  相似文献

目的 应用锥形束计算机体层摄影术(CBCT)观测并分析下牙槽神经管在下颌骨内的走行特点,探讨其分布规律,为临床手术提供解剖学依据.方法 采用单纯随机抽样法选取具备下颌后部CBCT 扫描数据的患者60 例,应用CBCT 自带的KaVo eXam Vision 软件测量颏孔区、前磨牙区、磨牙区下牙槽神经管与牙槽嵴顶、下颌骨颊舌侧骨板及下缘的平均距离,对各组测量项目进行成组t 检验比较,并观察其走行特点.结果 下牙槽神经管距下颌下缘小于距牙槽嵴顶的距离,在第二磨牙区距离下颌骨下缘最近;颏孔区至第一磨牙区间,下牙槽神经管距颊侧骨板距离小于距舌侧骨板距离,自第二磨牙后,下牙槽神经管距颊侧骨板距离则大于距舌侧骨板距离;性别及左、右侧下牙槽神经管距离差异无统计学意义(t=2.437,P > 0.05).结论 CBCT 扫描有利于更好了解下牙槽神经管的走行及结构特异性,对牙槽外科及种植外科手术方案的制定具有一定临床指导意义.  相似文献
