Self-reported outcome on hearing disability and handicap as well as overall health-related quality of life were measured after
hearing-aid fitting in a large-scale clinical population. Fitting was performed according to two different procedures in a
double-blind study design. We used a comparative procedure based on optimizing speech intelligibility scores and a strictly
implemented fitting formula. Hearing disability and handicap were assessed with the hearing handicap and disability inventory
and benefit of hearing aids with the abbreviated profile of hearing aid benefit. Effects on health-related quality of life
and depression were assessed with the EuroQol-5D questionnaire and the geriatric depression scale. We found that hearing-aid
fitting according to either procedure had a significantly positive effect on disability and handicap associated with hearing
loss. This effect lasted for several months. Only the effect on disability persisted after 1-year of follow-up. Self-reported
benefit from hearing aids was comparable for both fitting procedures. Unaided hearing disability was more pronounced in groups
of participants with greater hearing loss, while the benefit of hearing aids was independent from the degree of hearing impairment.
First-time hearing aid users reported greater benefit from their hearing aids. The added value from a bilateral hearing-aid
fitting was not significant. Overall health-related quality of life and incidence of depression did not alter after hearing-aid

AbstractObjective: The objective of this study was to explore technology use and its relationship to help-seeking for hearing impairment (HI) and success with hearing aids among older adults. Previous research had suggested a link between higher levels of technology use and hearing aid success. Design: General technology use was evaluated using a purposefully developed 25-item questionnaire. Twelve items related to everyday technology use (e.g. DVD player) and 13 related to advanced technology use (e.g. Bluetooth). Study sample: Four groups of older adults with HI participated in the study: (1) non-consulters (n = 49), (2) consulters (n = 62), (3) unsuccessful hearing aid owners (n = 61), and (4) successful hearing aid owners (n = 79). Results: Preliminary analyses revealed a main effect in the use of everyday and advanced technology across the four participant groups. However, it was found that age and living arrangements accounted for most of the variance in reported everyday technology use (p = .030; p = .029, respectively) and age and gender accounted for the variance in reported advanced technology use (p <.001; p = .040, respectively). For everyday technology, an increase in age and living alone were associated with decreased technology use and for advanced technology use, age and female gender were associated with decreased technology use. Conclusions: Although we hypothesized that technology use would be less amongst non-consulters and unsuccessful hearing aid owners, our findings did not support this prediction. This article aims to evaluate a hearing loss intervention versus an aging education intervention on activity engagement in the Aging and Cognitive Health Evaluation in Elders pilot (ACHIEVE-P). Forty adults (70–84 years) with hearing loss recruited from the Atherosclerosis Risk in Communities Study and de novo participated. Participants were randomized 1:1 to a best practices hearing intervention or a successful aging intervention. Hearing was measured with pure-tone audiometry. The Community Healthy Activities Model Program for Seniors questionnaire measured self-reported time engaging in activities at baseline and 6-month follow-up. At baseline, greater hearing loss was associated with reduced time per week on mental activities (−3.0 hours per 10 dB of hearing loss, 95% confidence interval: −5.8, −0.2). Mental activity engagement increased (mean: +1.3 hours, SD = 6.6) for the hearing intervention group but decreased (mean: −1.1 hours, SD = 4.8) for the aging education group (Cohen''s d : 0.41). Hearing loss may be associated with reduced engagement in mental activities. Whether hearing loss treatment impacts activity will be studied in the full-scale ACHIEVE trial. 相似文献

Studies have shown that elderly people with cognitive impairments benefit more from hearing aids with slower recovery times.ObjectiveTo study participation constraints and speech recognition in noise of elderly subjects equipped with hearing aids of different recovery times according to cognitive impairment status.MethodFifty subjects aged between 60 and 80 years were followed for four months. They were divided at first in groups of individuals without (G1; n = 24) and with (G2; n = 26) cognitive impairment based on results of the Alzheimer's Disease Assessment Scale - Cognitive Sub-scale test. Half the members of each group received hearing aids with faster recovery times and half got slower recovery aids, thus forming four groups: two without cognitive impairment (faster recovery - G1F; slower recovery - G1S) and two suspected for cognitive impairment (faster recovery - G2F; slower recovery - G2S). All subjects were interviewed, submitted to basic audiological assessment, asked to answer the Hearing Handicap Inventory for the Elderly questionnaire, and tested for speech recognition in noise. ANOVA, McNemar's test, and the Chi-square test were applied. The significance level was set at 5%.ResultsThere was significant improvement in participation constraint and speech recognition in noise with hearing aids alone. Sub-group G2F needed more favorable signal-to-noise ratios to recognize 50% of the speech in noise.ConclusionParticipation constraint and speech recognition in noise were improved regardless of recovery times or cognitive impairment status. 相似文献

Objective: To investigate factors influencing young people's motivation to reduce their leisure noise exposure, and protect their hearing health. Design: Questionnaires were conducted online to investigate young people's hearing health attitudes and behaviour. Items were developed using an integrated health promotion approach. The stage of change model was used to group participants in relation to their engagement with noise reduction behaviour. The health belief model was used to compare each group's perceptions of susceptibility and severity of hearing loss, as well as the benefits and barriers to noise reduction. Study sample: Results are presented for 1196 young Australians aged between 18 and 35 years. Results: Participants’ engagement with noise reduction behaviour was used to assign them to stage of change groupings: Maintenance (11%), Action (28%), Contemplation (14%), or Pre-contemplation (43%). Each group's responses to health belief model items highlighted key differences across the different stages of engagement. Conclusions: Future hearing health promotion may benefit from tailoring intervention activities to best suit the stage of change of individuals. Different information may be useful at each stage to best support and motivate young people to look after their hearing health. 相似文献

The United States does not ensure equitable access to hearing health care for all age groups, largely because these services are costly and even unobtainable in some places. Barriers to care are discussed within a context of the social determinants of health, under-representativeness of hearing-care professionals from historically marginalized communities, older adults and age-related hearing loss, and associated health conditions. The MarkeTrak 2022 study generated a sample of 15,138 respondents with information on 43,597 individuals. Data analysis revealed that self-reported hearing difficulty appears to increase with age with a rate of 12.4% for adults 18 years of age and older. A substantial proportion of individuals with hearing difficulty assumed that their problem was age-related, followed by exposure to loud sound and noise. Individuals with hearing difficulty were nearly three to four times more likely to have tinnitus, cognitive problems, and issues with balance and falling than those with no hearing problems. Self-reported hearing difficulty was lower for historically marginalized groups (7%) than for the White population (12%). Recommendations are presented to reduce the burden of hearing difficulty and hearing aid deserts for rural and urban populations. 相似文献
