The impact of aortic endografts on renal function   总被引:2,自引:0,他引:2  
OBJECTIVE: To determine the impact on late postoperative renal function of suprarenal and infrarenal fixation of endografts used to treat infrarenal abdominal aortic aneurysm (AAA). METHODS: Retrospective analysis of 277 patients treated from 2000 to 2003 with three different endografts at two clinical centers. Five patients on dialysis for preoperative chronic renal failure were excluded. Group IF of 135 patients treated with an infrarenal device (Medtronic AneuRx) was compared with group SF of 137 patients treated with a suprarenal device (106 Cook Zenith and 31 Medtronic Talent). Renal function was evaluated by calculating preoperative and latest postoperative creatinine clearance (CrCl) using the Cockcroft formula. Patients who developed a >20% decrease in CrCl were considered to have significantly impaired renal function. RESULTS: There were no significant differences in patient age, sex, aneurysm size, preoperative risk factors, dose of intra- and postoperative contrast, or baseline CrCl (IF: 69.3 mL/min, SF: 71.7 mL/min, P = .4). Follow-up time of 12.2 months was the same in both groups. CrCl decreased significantly during the follow-up period in both groups (IF: 69.3 mL/min to 61.7 mL/min, P < .01; SF: 71.7 mL/min to 64.9 mL/min, P < .03). Postoperative CrCl (IF: 61.7 mL/min, SF: 64.9 mL/min, P = .3), and the rate of CrCl decrease during the follow-up period (IF: -10.9%, SF: -9.5%, P = .2) was not different between the two groups. The number of patients with a >20% decrease in CrCl was not different between the two groups (IF: n = 35 [25.9%], SF: n = 41 [29.9%], P = .46). However, the magnitude of decrease in CrCl in patients with renal impairment was greater in patients treated with suprarenal fixation endografts (SF: -39%) compared with those treated with infrarenal endografts (IF: -31%, P = .005). This greater degree of renal impairment was not due to identifiable differences in preoperative risk factors, age, or baseline CrCl. No patients in these series required dialysis. CONCLUSIONS: Regardless the type of endograft used, there is a 10% decrease in CrCl in the first year after endovascular aneurysm repair. Suprarenal fixation does not seem to increase the likelihood of postoperative renal impairment. Decline in renal function over time after endovascular aortic repair is probably due to multiple factors, and measures known to be effective in protecting kidneys should be considered for these patients. Long-term follow-up with measurement of CrCl, along with renal imaging and regular blood pressure measurements, should be performed to detect possible late renal dysfunction. Prospective studies comparing suprarenal versus infrarenal fixation are needed to confirm those results.  相似文献

OBJECTIVES: This study was performed to determine whether there is deterioration in renal function during follow-up in patients who have undergone endovascular aneurysm repair (EVAR), as recommended by the device manufacturers; to determine whether suprarenal fixation correlates with impairment of renal function; and to explore the potential implication of life-long surveillance of renal function with contrast-enhanced computed tomography. METHODS: One hundred forty-six consecutive patients underwent EVAR at our institution. Data from 113 of these patients who were free from preoperative renal insufficiency or postoperative renal disease were analyzed. Fifty-three patients received infrarenal (IR) fixation devices, and 60 patients received suprarenal (SR) fixation devices. All SR fixation devices were placed under investigational device exemption protocols. The average follow-up was 688 days. Sixty-five consecutive patients who had undergone open repair of an abdominal aortic aneurysm (AAA) served as the control group. RESULTS: Preoperative creatinine concentration, intraoperative blood loss, contrast volume, and number of contrast-enhanced procedures were not significantly different between the IR and SR groups. Two renal artery occlusions (1 SR, 1 IR; P=NS) were identified, and 8 renal infarcts (5 SR, 3 IR; P=NS). There was an increase in mean creatinine concentration in the open AAA, IR, and SR fixation groups at each time point in the analysis. Mean elevation in creatinine concentration at 12, 24, and 36 months was 0.10, 0.10, and 0.04 mg/dL, respectively, for open AAA repair; 0.20, 0.21, and 0.28 mg/dL for IR fixation; and 0.15, 0.21, and 0.12 mg/dL for SR fixation. At life table analysis, renal impairment at 36 months was seen in 36% +/- 9% of patients in the IR group, 25% +/- % of patients in the SR group, and 19% +/- 6% of patients in the open AAA group (P=.04 for IR fixation vs open AAA repair). CONCLUSIONS: A decrease in kidney function is seen after EVAR, regardless of fixation level, that is independent of renal disease and renal arterial occlusion. In patients with normal renal function the site of proximal fixation does not affect postoperative creatinine concentration. The decrease in renal function is likely related to the repetitive administration of contrast agent.  相似文献

OBJECTIVE: Suprarenal clamping in abdominal aortic surgery is associated with a significant risk of postoperative renal failure, general morbidity, and mortality. Arterial access for temporary shunting in these patients is difficult, and arterial perfusion techniques are typically complex. This study evaluated if renal perfusion with venous blood using a minimal pump setup and intermediate-level heparinization prevents a decline in postoperative renal function in patients requiring suprarenal clamping for aortic reconstructive surgery. METHODS: Renal perfusion was achieved using a roller pump supplied with venous blood from a central venous catheter under medium-level heparinization to feed two perfusion balloon catheters. The calculated glomerular filtration rate was observed for 10 postoperative days and compared with the rate in patients with suprarenal clamping without renal perfusion. RESULTS: From 2001 to 2007, 158 patients underwent surgical reconstruction involving the pararenal aorta through a midline abdominal incision. Renal perfusion was started in 2006 and was always attempted if suprarenal clamping was anticipated preoperatively. Twenty-six patients received renal perfusion, and 132 also requiring suprarenal clamping did not. Of the latter, 109 were included in the control group. Five patients died 相似文献

Endovascular surgery for abdominal aortic aneurysms (AAA) has been performed with different commercially available devices since more than 7 years. The technique of implantation even for ruptured aneurysm is feasible. Attention is now focused on complications and mid-term results. We present a single center experience over the last 7 years using nine different endografts. 653 patients with AAAs were treated endovascularly (158 tubes, 495 bifurcated) between August 1994 and September 2001. The total mortality rate in the follow-up period (median 37 months) was 6.1 % with a 30-day mortality of 0.76 % and a procedure-related lethality of 1.2 %. Tube endografts showed the highest endoleakage rate (37.3 %) due to secondary shortening, migration and material defects. Endoleakage was diagnosed in 18.2 % of unibody designed stentgrafts versus 28.3 % in modular grafts. Regarding proximal Type I endoleaks suprarenal fixation with uncovered stents seems to be of no further advantage in proximal graft fixation. The conversion rate was 6.6 % (n = 43), aneurysm rupture following endovascular repair occurred in 3 of these patients (0.45 %). Mortality in emergency conversion was 30 % compared to no deaths in elective cases. There was a surprisingly high rate of inadvertent renal infarctions (9 %) in follow-up CT scans, two times higher in suprarenal fixation (18 %) than in infrarenal position (7.4 %). In conclusion, we observed best results in unibody bifurcated stentgrafts, late failure of first generation endografts (Mintec(R)) and decreasing conversion rates over the last years (learning curve!). The clinical significance of a side branch endoleak and related maximal AAA diameter remains uncertain. The ideal graft has yet to be found, but improvements in design and implantation techniques evolve rapidly. Good results will be obtained in the future by patient selection and restrictive indications.  相似文献

BACKGROUND: Generous endograft oversizing has been associated with propensity for aortic neck dilation and subsequent device migration in endografts without suprarenal fixation. Effects of variable oversizing of endografts with suprarenal fixation have been poorly studied. METHODS: Three hundred fifty-one patients underwent endovascular AAA repair (EVAR) in a prospective multicenter trial using the Zenith AAA Endovascular Graft, a fully supported bifurcated 3-piece endograft with barb-enhanced suprarenal stent fixation. Blinded core-laboratory measurement of variables was prospectively recorded at predischarge and at 1, 6, 12, and 24 months after the procedure. Potential influence of endograft oversizing on subsequent aortic neck dilation (minor axis), aneurysm shrinkage (major axis), device migration, endoleak, rupture, open conversion, and death were retrospectively studied. Data are given as mean +/- SEM. RESULTS: Risk of endograft migration (>5mm) at 12 months was 2.3% (6/261). However, patients with endograft oversizing of >30% had a 14% (4/29) migration risk compared with those oversized < or =30% (0.9%, 2/232), P <.002. There was zero device migration by the SVS definition (>10 mm or clinical event). Device oversizing >30% was associated with decreased AAA sac shrinkage (48% vs 77%) and with increased sac enlargement (9.5% vs 0.6%) at 24 months when compared with oversizing of < or =30%, respectively (P =.001). Incidence rate of any endoleak at 12 and 24 months was 8.2% (21/256) and 7.1% (12/169), respectively. Oversizing of endografts by >30% was associated with an increased type II endoleak rate (11 vs 4.7%) that failed to reach statistical significance (P =.27). Aortic neck diameters increased significantly by 6 months (P <.001) but then stabilized through 24 months; the absolute changes at 1 (n = 298), 6 (n = 278), 12 (n = 264), and 24 months (n = 171) were 0.66 +/- 0.10 mm (3.0%), 1.32 +/- 0.11 mm (5.6%), 1.38 +/- 0.12 mm (5.9%), and 1.44 +/- 0.16 mm (6.1%), respectively. Linear regression analysis demonstrated no correlation between endograft oversizing and aortic neck dilation at 12 (P =.86) or 24 months (P =.64). CONCLUSIONS: Device migration and endoleaks were very infrequent after treatment with the Zenith AAA Endovascular Graft. However, endograft oversizing of >30% was associated with an approximately 14-fold increase in device migration (>5 mm) at 12 months and with a approximately 16-fold increased risk of AAA expansion at 24 months. Although further follow-up will be essential to assess whether these early associations continue, avoidance of excessive endograft oversizing is recommended.  相似文献

Purpose: Successful endovascular repair of an abdominal aortic aneurysm (AAA) requires the creation of a hemostatic seal between the endograft and the underlying aortic wall. A short infrarenal aortic neck may be responsible for incomplete aneurysm exclusion and procedural failure. Sixteen patients who had an endograft positioned completely below the lowest renal artery and 37 patients in whom a porous portion of an endograft attachment system was deliberately placed across the renal arteries were studied to identify if endograft positioning could impact on the occurrence of incomplete aneurysm exclusion. Methods: Fifty-three patients underwent aortic grafting constructed from a Palmaz balloon expandable stent and an expandable polytetrafluoroethylene (ePTFE) graft implanted in an aorto-ilio-femoral, femoral-femoral configuration. Arteriography, duplex ultrasonography and spiral CT scans were performed in each patient before and after endografting to evaluate for technical success, the presence of endoleaks, and renal artery perfusion. Results: There was no statistically significant difference in patient demography, AAA size, or aortic neck length or diameter between patients who had their endografts placed below or across the renal arteries. However, significantly more proximal aortic endoleaks occurred in those patients with infrarenal endografts (P ≤.05). Median serum creatinine level before and after endografting was not significantly different between the 2 patient subgroups, with the exception of 2 patients who had inadvertent coverage of a single renal orifice by the endograft. Median blood pressure and the requirement for antihypertensive therapy remained the same after transrenal aortic stent grafting. Significant renal artery compromise did not occur after appropriately positioned transrenal stents as shown by means of angiography, CT scanning, and duplex ultrasound scan. Mean follow-up time was 10.3 months (range, 3 to 18 months). Patients who had significant renal artery stenosis (≥50%) before aortic endografting did not show progression of renal artery stenosis after trans-renal endografting. Two patients with transrenal aortic stent grafts had inadvertent coverage of 1 renal artery by the endograft because of device malpositioning, which resulted in nondialysis dependent renal insufficiency. In addition, evidence of segmental renal artery infarction (<20% of the kidney), which did not result in an apparent change in renal function, was shown by means of follow-up CT scans in 2 patients with transrenal endografts. Conclusion: Transrenal aortic endograft fixation using a balloon expandable device in patients with AAAs can result in a significant reduction in the risk of proximal endoleaks. Absolute attention to precise device positioning, coupled with the use of detailed imaging techniques, should reduce the risk of inadvertent renal artery occlusion from malpositioning. Long-term follow-up is essential to determine if there will be late sequelae of transrenal fixation of endografts, which could adversely effect renal perfusion. PURPOSE: This study was undertaken to assess the effect on renal function of open surgery and endovascular abdominal aortic aneurysm (AAA) repair with suprarenal fixation with the Zenith device. METHODS: Data for 279 patients with similar preoperative comorbid conditions were prospectively analyzed after AAA repair. One hundred ninety-nine patients underwent endografting with the Zenith AAA Endovascular Graft, which incorporates suprarenal fixation (Zenith standard risk group, ZSR), and 80 patients underwent open surgery (standard surgical risk group, SSR). Endovascular repair was also performed in 100 patients considered poor candidates for open repair (Zenith high risk group, ZHR). Serum creatinine concentration (SCr) and anatomic defects were assessed before the procedure, before discharge, and at 1, 6, 12, and 24 months in all patients who underwent endovascular repair, and before the procedure and at 1 and 12 months in patients who underwent open surgical repair (only SCr was measured before discharge). Renal function was also analyzed, with a creatinine clearance calculation (Cockcraft-Gault). Renal insufficiency was defined as an increase in SCr greater than 30% from a preoperative baseline value, any SCr concentration in excess of 2.0 mg/dL, or any need for dialysis. Cumulative renal infarction and arterial occlusion rates were calculated with computed tomographic, ultrasonographic, and angiographic data, and reported as cumulative values. RESULTS: Despite the initially superior renal function in the ZSR group at the pre-discharge evaluation (P =.01), there were no differences at 12 months with respect to rise in SCr greater than 30% (ZSR, 16%, vs SSR, 12%; P =.67), SCr rise greater than 2.0 mg/dL (ZSR, 2.5%, vs SSR, 3.4%; P =.66), incidence of renal artery occlusion (ZSR, 1%, vs SSR, 1.4%; P >.99), or infarction (ZSR, 1.5%, vs SSR, 1.4%; P >.99). Only one patient in each group required hemodialysis. Of note, both groups of patients demonstrated a reduction in creatinine clearance over 12 months, which then stabilized or improved by 24 months for ZSR patients. CONCLUSIONS: Renal dysfunction occurs in a subset of patients regardless of type of repair (open or endovascular with suprarenal fixation). The cause of renal dysfunction after open or endovascular repair with a suprarenal stent is probably multifactorial. The observed dysfunction occurs in a small number of patients, and the effect in the endovascular group (no data for the surgical group at 24 months) appears to be transient. The initial dysfunction, apparent in both groups over 12 months of follow-up, stabilizes or improves at 12 to 24 months.  相似文献

BACKGROUND: Our aim was to determine whether suprarenal fixation in endografts compromises renal artery (RA) flow and whether subsequent RA intervention is precluded by the stent struts. METHODS: Prospectively acquired data from 104 patients with endovascular aneurysm repair (EVAR) of an abdominal aortic aneurysm using the Zenith endograft (Cook, Inc., Bloomington, IN) were analyzed. The Zenith device uses a 26-mm, uncovered, barbed Z stent for suprarenal function. RESULTS: No RA stenosis, occlusion, or infarction resulted from the suprarenal stent. In 3 of 104 (2.9%) patients, RA compromise (2 stenoses, 1 occlusion) was caused by impingement of graft material on the lowermost RA. The 2 RA stenoses were stented successfully at 1 and 7 months post-EVAR. Six of 104 (5.8%) patients developed late stenoses unrelated to the endograft: all were stented successfully from 19 to 36 months after EVAR. One patient with severe RA stenosis had balloon angioplasty pre-EVAR and then was stented electively 6 weeks post-EVAR. CONCLUSIONS: Our data show that the suprarenal fixation of the Zenith aortic endograft does not cause RA stenosis, occlusion, or infarction, nor does it preclude post-EVAR renal artery intervention.  相似文献

AIMS: Renal dysfunction occurs occasionally after the repair of abdominal aortic aneurysm (AAA), and preoperative renal function is considered as one of the potential causes. The present study was designed to evaluate and compare renal function and risk factors of AAA patients with those of hypertensive patients. METHODS: We prospectively examined 95 patients with AAA and 72 patients with essential hypertension (HT) without other cardiovascular diseases (CVD). Renal function, urinary albumin excretion (UAE) and renal scintigraphy were compared. Kidney size was calculated using ultrasonography. RESULTS: Serum creatinine and creatinine clearance in AAA patients was worse than in HT patients. Smoking status was more apparent in AAA patients. Renal artery stenosis occurred in 8 patients with AAA. Renal scintigraphy showed normal function in 19%, hypofunction in 69% and severe dysfunction in 12% of the AAA patients, and normal function in 42% and hypofunction in 58% of the HT patients (p < 0.0001). Multivariate linear regression analysis showed that renal function was related to age, UAE, CVD, smoking status and kidney size for all patients, UAE, CVD, smoking status and kidney size for AAA patients, and age and kidney size for HT patients. CONCLUSION: Renal function of AAA patients was worse than HT patients without other CVD. The risk factors for renal dysfunction were different between AAA and HT patients. These preoperative conditions may relate to the postoperative renal dysfunction seen in AAA patients.  相似文献

We examined data of 21 patients who were treated with selective perfusion of both renal arteries with 500 mL of 8 degrees C histidine-tryptophan-ketoglutarate (HTK) solution each for renal protection during aortic surgery. Only the data from aortic surgeries with unavoidable suprarenal aortic cross-clamping for juxtarenal or suprarenal abdominal aortic aneurysms (AAAs) or high Leriche syndrome accompanied with stenosis of renal arteries are presented. Five patients underwent immediate surgery because of perforation of an AAA; the other 16 patients went through elective surgeries. In three cases (14%) stenosis of the renal arteries was diagnosed; nevertheless, implantation of an aortorenal bypass was necessary in seven patients. In total, 14 aortorenal bypasses were implanted (five venous grafts and nine prosthesis grafts). Four (19%) patients needed catecholaminergic support to establish stable circulatory conditions; in two (9%) of these cases additional ischemia of the colon was observed and sigmoidectomy was performed. All of these four patients underwent immediate surgery, and one died after surgery because of severe sepsis. In four cases postsurgical renal insufficiency was observed. Three of these patients were admitted for emergency surgery because of their hemodynamic situation due to perforation of the AAA. None of the patients needed chronic dialysis after surgery. Whereas in all patients who underwent elective surgery the renal function remained stable as judged by postoperative serum creatinine values, in five out of seven patients with aortorenal bypass surgery the renal function improved. Perfusion with cold HTK solution offers an additional procedure to protect renal function in patients undergoing elective surgery with suprarenal cross-clamping of the aorta.  相似文献

OBJECTIVE: Transrenal fixation of abdominal aortic endografts may provide a more secure proximal attachment with few deleterious effects with respect to renal function. This study's purpose was to determine whether different metals used in two commercially available endografts (Cook Zenith and Medtronic Talent) result in different effects on renal function when placed across renal ostia. METHODS: A total of 140 consecutive patients, between August 2003 and April 2005, who underwent elective endovascular repair of an aortic aneurysm with a nitinol or stainless steel-based endograft with transrenal fixation were reviewed with a mean follow-up period of 5.5 months (range, 1-22 months). The main outcome variable was the percentage change in creatinine clearance (CrCl), which was determined before surgery and at the most recent follow-up. Multiple regression analysis was performed to analyze the contribution of various factors to any deterioration in renal function. RESULTS: The 140 patients were predominantly male (86%), with a mean age of 75 years (range, 56-92) and a mean aneurysm diameter of 62 mm (range, 42-110 mm). The mean intraoperative contrast use was 67 mL (range, 45-160 mL), and after surgery these patients received a mean of 3 surveillance computed tomographic scans (range, 1-7). Nineteen cases (13.6%) required deliberate accessory renal artery coverage. CrCl did not change significantly after surgery; the mean change in CrCl at the end of follow-up was a 2.5% decrease. There was no difference in the reduction in CrCl between endograft types. Only 14.3% of patients experienced a greater than 20% decrease in CrCl. Multiple regression analysis failed to show a relationship between change in renal function and age, sex, aneurysm diameter, infrarenal neck dimensions, endograft type, coverage of accessory renal arteries, intraoperative contrast volume, preoperative CrCl, and postoperative computed tomographic scans. Length of follow-up was an independent predictor (P = .04). CONCLUSIONS: Transrenal endograft fixation results in little deterioration in renal function during intermediate follow-up. The few patients who do experience a decline in renal function do so independently of any features related to the initial endovascular repair, and there is no difference in renal deterioration between endografts with nitinol and stainless steel transrenal bare metal stents.  相似文献

AIM: The measurement of intrarenal resistance indices (RIs) by doppler ultrasound plays an important role in the evaluation of renal transplant recipients. Although an elevated RI was initially considered to be specific for rejection, later studies revealed this parameter as a nonspecific marker of transplant dysfunction. In this study, we analysed Tc-99m DTPA renal scintigraphy findings in patients with increased RI during the early posttransplantation period. METHODS: This study included 22 patients with increased RI on doppler sonography during the first week after transplantation. Twenty-two recipients with uncomplicated early postoperative courses were used as a control group. An RI value >0.7 was considered pathologic. All patients underwent Tc-99m DTPA renal scintigraphy just after doppler sonography. In addition to visual interpretation of images, renogram curves were evaluated for patterns suggestive of acute tubular necrosis and acute rejection. Glomerular filtration rate (GFR) was calculated using computer software. Perfusion time-activity curves were assessed for the presence of peak and plateau patterns to calculate this ratio (P:PL). RESULTS: The mean value for P:PL in patients with increased RI and in the control group were 1.37 +/- 0.33 and 1.53 +/- 0.47, respectively (P < .05). The mean value for GFR was significantly lower in the patient group compared with control subjects. Six patients had normal perfusion and function (27%). Perfusion pattern and renogram changes were suggestive of acute tubular necrosis in 5 patients and acute rejection in 6 patients. These diagnoses were confirmed later with serial scintigraphic changes or biopsy results. Three patients had an accumulation pattern on the renogram suggesting partial obstruction. CONCLUSION: During the early posttransplantation period an increased RI on doppler sonography was seen in both normal functioning grafts and those with allograft dysfunction. Renal scintigraphy with perfusion and renogram patterns highly suggestive of specific allograft pathologies seemed to provide useful information to distinguish early postoperative renal allograft pathologies.  相似文献

BACKGROUND: Suprarenal fixation is widely used in endovascular aneurysm repair. Numerous small, underpowered studies have concluded that it does not increase the risk of renal impairment compared with infrarenal fixation. A recent meta-analysis demonstrated that renal infarction is more common with suprarenal fixation, but the effect on renal function remains unclear. METHODS: Electronic abstract databases, article reference lists, and conference proceedings were searched for series reporting renal function data after suprarenal fixation. There was considerable study heterogeneity with respect to key factors such as pre-existing renal dysfunction and length of follow-up. Authors were contacted to obtain individual patient data for a pooled reanalysis using standardized criteria. RESULTS: Of 46 potentially relevant citations, only 11 were eligible for inclusion in the meta-analysis. Complete data sets were available for four studies (1065 patients), with a median follow-up of 33 months. Kaplan-Meier curves were constructed for postoperative renal impairment in the suprarenal fixation and infrarenal fixation groups and compared by the log-rank test. Median time free of renal impairment was 38.5 months in the infrarenal fixation group compared with 32.4 months in the suprarenal fixation group (P = .0038). However, to account for significant methodologic differences, further analysis was required using a Weibull regression model fitted in open Bayesian inference using Gibbs sampling (BUGS). The pooled hazard ratio for deterioration of renal function after suprarenal fixation was 0.6 (95% confidence interval, 0.3-10). CONCLUSION: Currently available data are insufficient to determine the precise effect of suprarenal fixation on medium-term renal function. Conventional Kaplan-Meier analysis of the pooled data set suggested that suprarenal fixation increased the risk of renal dysfunction; however, the effect disappeared when sophisticated statistical modelling was performed to account for study heterogeneity. A randomised controlled trial of suprarenal fixation may resolve this issue.  相似文献

OBJECTIVES: Although suprarenal fixation may be of benefit during endovascular aneurysm repair (EVAR), its safety with regards to renal effects remains uncertain. To date, there has been no controlled study of the topic, with most reports relying upon single-center experiences that use heterogeneous patient populations and devices from different manufacturers. The purpose of this analysis was to evaluate the effect of suprarenal fixation on renal function by comparing homogeneous patient populations receiving EVAR grafts from a single manufacturer that are identical in design and delivery method, except for utilizing either suprarenal (SR) or infrarenal (IR) fixation. METHODS: During two pivotal US Food and Drug Administration trials, 283 patients underwent EVAR with the Powerlink bifurcated graft. The trials' inclusion and exclusion criteria and grafts were identical except for fixation scheme. Clinical, laboratory, and computed tomographic (CT) data were retrospectively reviewed. A comparison of preoperative, perioperative (1 to 7 days), and postoperative (>7 days) alterations in serum creatinine (SCr), creatinine clearance (CrCl), and blood pressure was performed. Renal adverse events were determined by CT scan and clinical chart review and included renal infarction, renal artery stenosis (either progressive or requiring renal stent placement), and renal artery occlusion. RESULTS: Both SR and IR groups demonstrated a significant increase in SCr and a decrease in CrCl over time. No significant difference in SCr or CrCl existed between groups during any time period. There were no differences in postoperative renal impairment (IR, 10.2%; SR, 7.6%, P = .634), the need for hemodialysis (IR, 0.7%; SR, 0%, P = 1.00), or systolic and diastolic blood pressure during subsequent follow-up between treatment groups. There was no significant difference in the number of renal adverse events detected by CT between the IR (10, 6.8%) and SR (3, 3.8%) groups (P = .550). CONCLUSION: Suprarenal fixation does not lead to a significant increase in acute renal events, renal impairment, or alteration in blood pressure compared with infrarenal fixation. Patients undergoing aneurysm repair with devices that use either suprarenal or infrarenal fixation develop progressive renal dysfunction over time. Further studies are needed to determine the long-term effects of suprarenal fixation on renal function and progression of renal artery stenosis.  相似文献

We evaluated the incidence, risk factors, and clinical consequences of renal microembolic events following endovascular aneurysm repair using suprarenal or infrarenal fixation. Pre- and postoperative (1 month) computed tomographic angiograms were reviewed for new renal perfusion defects. Suprarenal aortic and infrarenal neck thrombus load was classified by circumference involved and thrombus thickness. Serum creatinine was measured preoperatively, on the first postoperative day, and 1 month postoperatively. Among 136 patients, 8 (5.9%) had bilateral microembolic cortical defects. Patients with moderate or severe suprarenal thrombus were more likely to have renal microemboli than those with no or mild suprarenal thrombus (17% vs 0%; p < .001). Similarly, patients with moderate or severe infrarenal neck thrombus were more likely to have renal microemboli than those with no or mild infrarenal thrombus (9.6% vs 1.5%; p = .08). Severe infrarenal thrombus was independently predictive of microembolization (odds ratio 15.0; 95% confidence interval 1.6-142; p = .018). There was no statistically significant difference in the incidence of renal microembolization when comparing suprarenal and infrarenal fixation (8.2% vs 4.0%; p = .47). Changes in creatinine from baseline were not different in those with or without renal microemboli. Renal microembolization is an uncommon but distinct radiographic finding that is more associated with significant neck thrombus than fixation level.  相似文献

Objective

The influence of endovascular aneurysm repair (EVAR) on renal function is of high concern. The question whether stent graft fixation type plays a significant role in renal outcome after EVAR is still debated. However, other factors, such as repeated contrast medium exposure, should also be considered.

Methods

We performed a two-center, stratified-cohort case control study to evaluate the influence of last-generation abdominal endografts with suprarenal (SR) vs infrarenal (IR) fixation on renal function.

Results

From a total of 276 patients, 134 were treated with IR fixation (group A) and 142 with SR fixation (group B) stent grafts. There was no significant difference in intraoperative contrast medium use (mean 120.0 mL group A vs 104.8 mL; P = .087) between the two cohorts. Overall, 11.2% of the patients (31/276) showed a relevant decline (≥20%) of estimated glomerular filtration rate (eGFR) postoperative and 11.5% (31/269) after 12 months. Furthermore, 19/134 (14.2%) patients in group A and 12/142 (8.5%) patients in group B showed a postoperative decrease of eGFR ≥20% (P = .132). Comparing the 12-month follow up, there was also no significant difference between the two groups (group A, n = 18/134; group B, n = 13/135; P = .329). Patients with only one contrast-enhanced computed tomography scan postoperatively (4/102; 3.9%) showed significant less renal deterioration after 12 months compared with the rest of the study collectively (27/166; 16.9%; P = .002). Comparing IR vs SR fixation in these patients, there was no significant difference between the two groups. One patient (1/35; 2.9%) with IR fixation (group A) and 3/67 (4.5%) with SR fixation (group B) showed a decline in eGFR values of ≥20% after 12 months (P = 1.0).

Conclusions

Our study showed no significant difference in renal impairment between SR and IR fixation in EVAR for IR abdominal aortic aneurysm. However, significantly more renal deterioration was observed in patients with increased postoperative contrast medium expose. Therefore, alternatives such as contrast- enhanced duplex ultrasound or magnetic resonance imaging for EVAR surveillance should be considered.  相似文献

OBJECTIVE: It has been suggested that endovascular aneurysm repair (EVAR) in concert with serial contrast-enhanced computed tomography (CT) surveillance adversely impacts renal function. Our primary objectives were to assess serial renal function in patients undergoing EVAR and open repair (OR) and to evaluate the relative effects of method of repair on renal function. METHODS: A thorough retrospective chart review was performed on 223 consecutive patients (103 EVAR, 120 OR) who underwent abdominal aortic aneurysm (AAA) repair. Demographics, pertinent risk factors, CT scan number, morbidity, and mortality were recorded in a database. Baseline, 30- and 90-day, and most recent glomerular filtration rate (GFR) were calculated. Mean GFR changes and renal function decline (using Chronic Kidney Disease [CKD] staging and Kaplan-Meier plot) were determined. EVAR and OR patients were compared. CKD prevalence (>or=stage 3, National Kidney Foundation) was determined before repair and in longitudinal follow-up. Observed-expected (OE) ratios for CKD were calculated for EVAR and OR patients by comparing observed CKD prevalence with the expected, age-adjusted prevalence. RESULTS: The only baseline difference between EVAR and OR cohorts was female gender (4% vs 12%, P = .029). Thirty-day GFR was significantly reduced in OR patients (P = .047), but it recovered and there were no differences in mean GFR at a mean follow-up of 23.2 months. However, 18% to 39% of patients in the EVAR and OR groups developed significant renal function decline over time depending on its definition. OE ratios for CKD prevalence were greater in AAA patients at baseline (OE 1.28-3.23, depending upon age group). During follow-up, the prevalence and severity of CKD increased regardless of method of repair (OE 1.8-9.0). Deterioration of renal function was independently associated with age >70 years in all patients (RR 2.92) and performance of EVAR compared with OR (RR 3.5) during long-term follow-up. CONCLUSIONS: Compared with EVAR, OR was associated with a significant but transient fall in GFR at 30 days. Renal function decline after AAA repair was common, regardless of method, especially in patients >70 years of age. However, the renal function decline was significantly greater by Kaplan-Meier analysis in EVAR than OR patients during long-term follow-up. More aggressive strategies to monitor and preserve renal function after AAA repair are warranted.  相似文献

OBJECTIVE: To evaluate renal damage and impairment of renal function 1 yr after laparoscopic partial nephrectomy (LPN) with warm ischemia >30 min. METHODS: From July 2004 to June 2005, 18 patients underwent LPN with warm ischemia time >30 min. Kidney damage markers (daily proteinuria and tubular enzymes) and renal function (serum creatinine, cystatin C, and creatinine clearances) were assessed on postoperative days 1 and 5 and at 12 mo. Glomerular filtration rate (GFR) was evaluated before surgery and at 3 mo. Renal scintigraphy was performed before the procedure, at 5 d and at 3 and 12 mo postoperatively. Statistical analysis was performed using the Student t test and logistic regression analysis. RESULTS: In terms of kidney damage and renal function markers, the statistical analysis demonstrated that at 1 yr there was complete return to the normal range and no statistical difference between the values at the various time points. The GFR was not significantly different before and 3 mo after surgery. In terms of scintigraphy of the operated kidney, the values were 48.35+/-3.82% (40-50%) before the procedure, 36.88+/-8.42 (16-50%) on postoperative day 5 (p=0.0001), 40.56+/-8.96 (20-50%) at 3 mo (p=0.003), and 42.8+/-7.2% (20-50%) 1 yr after surgery (p=0.001). CONCLUSION: Our results demonstrate that kidney damage occurs during LPN when warm ischemia is >30 min. This damage is only partially reversible and efforts should be made to keep warm ischemia within 30 min.  相似文献

Vascular imaging, usually employing nephrotoxic contrast agents is relied upon for all aspects of endovascular AAA repair causing some to consider renal insufficiency a relative contraindication. We sought to determine if endovascular AAA evaluation and repair could be successfully accomplished by minimally or non-nephrotoxic modalities.Records and results for 98 consecutive patients undergoing endovascular AAA repair were reviewed. Patients requiring dialysis preoperatively were excluded (N=3).The average volume of iodinated contrast agent employed for intraoperative imaging was 152 cc (35-420 cc). Twenty patients (20%) had baseline renal insufficiency (serum creatinine > or =1.3 mg/dl). A rise in serum creatinine above baseline was observed in 23 (24%) patients following repair; for 15 (16%) this was permanent. Creatinine rise occurred in patients with both normal (15) and abnormal (8) baseline values (P=0.09). Rise in creatinine was independent of contrast volume employed and of the use of infrarenal vs suprarenal device fixation (P>0.05). Two (2%) patients required permanent dialysis, one of which had a normal baseline creatinine and unclear etiology for renal failure, the other had a baseline creatinine of 2 and required device placement over an accessory renal artery. Strategies to minimize the use of nephrotoxic contrast for patients with renal insufficiency included the use of MRA, rather than contrast-CT for pre and postoperative imaging (7, 35%) and use of Gadolinium rather than iodinated contrast for performance of intraoperative arteriography (5, 25%). Endovascular grafts were successfully designed and implanted based upon MRA as the sole preoperative imaging modality in every case in which it was attempted (7). Mortality was not significantly different between those with and without abnormal baseline renal function (P>0.05). Adverse events (access failures, arterial injuries, blood loss, endoleaks) were not significantly correlated with baseline renal insufficiency, rise in creatinine from baseline, use of MRA or intraoperative Gadolinium angiography (P>0.05).Pre- and postoperative evaluation and performance of endovascular AAA repair can be accomplished in patients with renal insufficiency without increasing the rate of mortality or adverse events employing a strategy which minimizes the use of nephrotoxic contrast agents, relying upon Gadolinium arteriography and MRA. Endovascular grafts can be successfully planned and followed employing MRA as the sole imaging modality.  相似文献

PURPOSE: Proximal endovascular aortic graft fixation and maintenance of hemostatic seal depends on the long-term stability of the aortic neck. Previous investigations of aortic neck dilation mostly focused on the infrarenal aortic diameter. Fenestrated and branched stent grafts facilitate suprarenal graft fixation and may thereby improve the long-term integrity of the aortic attachment site. For these devices, the natural history of the suprarenal aortic segment is also of interest. We investigated the natural history of the supra- and infrarenal aortic segment after open abdominal aortic aneurysm (AAA) repair. METHODS: For this retrospective analysis, we reviewed the preoperative and the initial postoperative as well as the most recent CT series that were obtained from 52 patients undergoing conventional repair of an infrarenal abdominal aortic aneurysm between January 1998 and December 2002. Measurements were performed using electronic calipers on a "split screen", allowing direct comparison of subsequent CT series at corresponding levels along the vessel. Main outcome measures were changes in postoperative measures of the supra- and infrarenal aortic diameters. RESULTS: The first postoperative exam was at a mean (+/-SD) of 7.0 +/- 3.5 months, and the final exams were at 44.4 +/- 21 months. Over this time period, the estimated rate of change in suprarenal diameter was 0.18 mm/ y with 95% confidence interval (CI) from 0.08 to 0.27. The estimated rate of change for the infrarenal diameter was 0.16 (95% CI: 0.05 to 0.27). A clinically relevant diameter increase of >or=3 mm was observed in seven patients (13%). There was evidence of larger diameter increases associated with larger AAA diameters (P = .003 and <.001 for suprarenal and infrarenal diameters), an inverted funnel shape (P = .002 and <.001), and marginal evidence of association with a history of inguinal hernia (P = .043 and .066). CONCLUSIONS: Although there is statistically significant evidence of increases in the supra- and infrarenal aortic diameters after conventional AAA repair, mean annual increases tended to be small and clinically relevant increases of 3 mm or more were observed in only a small proportion of cases.  相似文献
