Comparison of left anterior oblique and geometric mean gastric emptying   总被引:2,自引:0,他引:2  
A left anterior oblique image (LAO) and the geometric mean of anterior and posterior counts have both been proposed as methods for acquiring gastric emptying data. Both approaches are used to correct for the changes in attenuation that occur as the depth of radiolabeled solids changes during gastric emptying. These two methods were compared by using a power exponential curve fit to calculate a lag phase, an equilibrium emptying rate, and a half-time for gastric emptying in 20 patients. There were no significant differences (mean +/- 1 s.e.m.) in the measured half-emptying time (115 +/- 10 versus 104 +/- 7 min) (p = 0.08) or rate of gastric emptying (0.015 +/- 0.002 versus 0.015 +/- 0.002 min-1) (p = 0.56) for LAO imaging versus the geometric mean. However, the LAO measurements of the lag phase were significantly higher (69 +/- 7 min) than the geometric mean (53 +/- 6 min) measurements (p = 0.004). This resulted in 4/20 (20%) of patients with normal geometric mean lag phase measurements who would have been reclassified as abnormal using the LAO method.  相似文献

Solid meal distribution patterns within the stomach, as well as gastric emptying were studied in 35 healthy subjects and 34 duodenal ulcer patients. Inspection of the serial scintiscans., recorded during each gastric emptying examination, revealed three different intragastric meal distribution patterns. Distinction between the three types of meal distribution within the stomach was facilitated by the use of the proximal: distal stomach width ratio (P:D), a parameter with a proven reproducibility. The P:D ratio less than 0.7 characterized Type III stomach, whereas 0.7 less than P:D less than 1 + 45 and P:D greater than 1.45 described Types I and II, respectively. Attention was focussed on the relationship of the intragastric meal distribution patterns to total stomach emptying. A significantly higher incidence of Type III stomach in women was accompanied by delayed gastric emptying and different emptying pattern when compared to men (T1/2 107.4 +/- 5.74 versus 84.2 +/- 3.21 min, mean +/- SEM p less than 0.001). Irrespective of the equally frequent occurrence of the relevant stomach types in normals and in duodenal ulcer patients, increased gastric emptying was observed in the latter group T1/2 90.8 +/- 3.30 versus 76.7 +/- 3.82 min, means +/- S.E.M., p less than 0.01). The necessity of establishing the normal range of gastric emptying parameters separately for men and women is concluded.  相似文献

The effect of MK-329, a potent, orally active, nonpeptidal cholecystokinin (CCK) antagonist, was measured on the gastric emptying rate of a solid meal in cats. External scintigraphy of cats that had been fed a meal of technetium-99m-(99mTc) labeled rabbit liver and light cream allowed the measurement of the emptying rates of either the liquid or solid portion of a meal under physiologic conditions. In cats, liquids emptied 2.6 times faster than solids [163 +/- 11 min vs. 62 +/- 3 min (mean +/- s.e.)]. At 3 or 10 mg/kg p.o., MK-329 gastric emptying was significantly accelerated, with the mean half-time of emptying being decreased by 34 +/- 11% (mean +/- s.e.) of the control half-times (p less than 0.02). Using only responders (five of six animals), mean half-time was decreased by 55 +/- 4% of the control half-times. CCK is important in regulating the emptying of solid food from the stomach because the CCK antagonist MK-329 accelerates that emptying.  相似文献

目的 观察13 C 辛酸呼吸试验测定糖尿病患者胃固体排空时间及其变异性 ,以及心自主神经病变和病理性胃排空状况对其的影响。方法  9例对照者和 15例糖尿病患者 [平均糖基化血红蛋白 (HbA1c) 7 8% ]1周内行 2次13 C 辛酸呼吸试验 ,采用非线性回归法计算半排空时间 (t1/ 2 )和缓慢期时间 (tlag) ,同时采用心自主神经功能试验判断有无心自主神经病变。结果 对照组t1/ 2 批内CV为 11 7%、tlag批内CV为 19 4% ,糖尿病患者t1/ 2 批内CV为 17 8%、tlag批内CV为 2 8 2 % ,对照者和糖尿病患者间差异无显著性。t1/ 2 和tlag的批内CV在糖尿病患者伴有和无心自主神经病变之间以及在正常胃排空时间和延迟性胃排空时间之间差异无显著性。结论 13 C 辛酸呼吸试验测定胃固体排空功能有较高的重复性 ,并不受心自主神经病变和病理性胃排空的影响。  相似文献

BACKGROUND: In a questionnaire survey, long-distance flying staff of a charter airline reported significantly more dyspeptic symptoms than did short-haul crewmember and ground personnel (belching: 57% vs. 37%, bloating: 51% vs. 36%). To elucidate the reason for increased frequency of gastrointestinal symptoms during long-distance flights, we investigated the effects of altitude and diet on gastric emptying, cardiovascular function, and bodily complaints. METHODS: In a 2 x 2 repeated measurement design we simulated an 8-h flight in a hypobaric chamber in 16 healthy men subjected to 2 meal conditions (high fiber vs. low fiber) on separate days, and assigned to either a flight altitude of 2500 m (8200 ft) or 1000 m (3280 ft). The subjects were blinded toward altitude. Heart rate and gastrointestinal symptoms were taken every hour, and gastric emptying was assessed by 13C-octanoic acid breath-test. In a separate experiment, we examined the effect of the two test meals (2 g vs. 20 g of fiber) in 30 healthy men under conventional laboratory conditions and found no significant differences. RESULTS: At an altitude of 2500 m, heart rate was significantly increased independent of the dietary condition. Gastric emptying (T1/2) was significantly delayed at 2500 m (8200 ft) when a high-fiber meal was given (146.3 +/- 58.4 min low fiber vs. 193.9 +/- 54.3 min high fiber). The symptom score for gastric distension (mean: 1.33 +/- 0.3 vs. mean: 1.07 +/- 0.15) and bloating (mean: 1.82 +/- 0.47 vs. mean: 1.34 +/- 0.35) were also significantly increased at 2500 m for the high-fiber meal compared with the low-fiber meal. CONCLUSIONS: Flight altitude is a physiological load. In combination with a high-fiber diet, this induces significant delays in gastric emptying that may explain symptoms of cabin and cockpit crew and passengers on long-distance flights.  相似文献

Many radiopharmaceuticals and test meals that are used to measure gastric emptying are less than optimal. A vegetable-based solid meal, such as rice, labelled with a radiopharmaceutical that also has the capacity to measure gastric emptying of liquids, is likely to be ideal. The role of Technegas as a radioisotopic marker to measure gastric emptying of rice and liquids was evaluated. Technegas-labelled rice was incubated in 0.9% saline, 1 M HCl and simulated gastric fluid (3.2 g/l pepsinogen, pH 2-3) to assess stability of the label. In eight healthy volunteers gastric emptying of two meals - 200 g rice (370 kcal) and 75 g dextrose dissolved in 300 ml water (300 kcal), both labelled with 20 MBq of Technegas - was measured scintigraphically. Over 4 h, the average label stability was 93.7%+/-0.5% in 0.9% saline, 91.0%+/-0.4% in 1 M HCl and 93.6%+/-0.7% in simulated gastric juice. The lag phase was longer for rice than dextrose (25+/-7 min vs 4+/-2 min; P<0.05), but there was no difference in the post-lag emptying rate (2.1+/-0.3 kcal/min vs 1.7+/-0.2 kcal/min; P=0.2) between the two meals. We conclude that Technegas is a suitable radiopharmaceutical for measurement of gastric emptying of rice and nutrient-containing liquids.  相似文献

PURPOSE: Using gastric emptying scintigraphy the gastric retention rate is commonly calculated within a gastric region of interest (intragastric method). This technique may have significant limitations when left oblique anterior views are acquired, due in part to attenuation resulting from intragastric redistribution. To minimize these drawbacks, it was proposed to express the intragastric content as a percentage of the abdominal radioactivity (abdominal method). Our goal was to compare these two methods when anterior-posterior scanning is used. METHODS: Antero-posterior scintigraphic data of 272 consecutive patients were analysed by both methods. Retention rates were obtained by both observation and calculation by power exponential fit. Gastric emptying parameters (half-emptying time of solids (T(50,S)) and liquids (T(50,L)), lag phase (T(lag)) time and real emptying time (T(RE))), and quality of fit were also computed and compared. RESULTS: For solids, the intragastric method resulted in weakly higher experimental retention rates, whereas retention rates were quite similar for liquids. Differences between experimental and calculated retention rates were smaller for abdominal method, for both liquids and solids. As a result, values for the quality of fit were higher for the abdominal method. Significant differences were observed only for calculated T(50,S) (122+/-46 min vs. 124+/-48 min, mean difference 2+/-2 min, P<0.00001) and T(RE) (163+/-64 min vs. 168+/-68 min, mean difference 4.5+/-3.8 min, P<0.05), respectively, for the abdominal and the intragastric methods. However, the Bland-Altman statistical method revealed good agreements (<5% outliers). CONCLUSION: Intragastric and abdominal methods can be used indifferently to treat antero-posterior data of gastric scintigraphy.  相似文献

The aim of this study was to develop a scintigraphic test to measure gastric emptying and accommodation simultaneously. METHODS: Gastric emptying and accommodation were measured in healthy subjects. To determine gastric accommodation, the stomach was imaged with SPECT 20 min after intravenous administration of 185 MBq (5 mCi) (99m)Tc-pertechnetate. After ingestion of 11 MBq (300 micro Ci) (111)In-diethylenetriaminepentaacertic acid in a liquid nutrient drink or an (111)In-oxine-labeled egg sandwich, dual-isotope imaging assessed SPECT gastric dimensions and gastric emptying every 20 min up to 240 min. Gastric accommodation was calculated as the percentage change in planar (2-dimensional) gastric cross-sectional area (CSA) using a left anterior oblique planar projection and the percentage change in total SPECT gastric voxel counts (3-dimensional) compared with the baseline image. RESULTS: With the liquid nutrient drink (9 subjects), maximal mean CSA (158% +/- 12% of baseline; P < 0.05) occurred 40 min after meal ingestion, when only 69% +/- 3% of the radiolabeled liquid nutrient drink remained in the stomach. At 120 min, mean CSA was 125% +/- 8% of baseline, but only 35% +/- 3% of the liquid nutrient drink remained in the stomach. Using SPECT to measure 3-dimensional volumes, maximal gastric volume occurred 20 min after meal ingestion (189% +/- 25% of baseline). With the solid egg meal (10 subjects), maximal total CSA (159% +/- 13% of baseline) occurred immediately after meal ingestion; total CSA remained significantly increased above baseline for the first 3 h after ingestion of the egg meal, despite only 12% +/- 4% gastric retention at 3 h. Using SPECT to measure 3-dimensional volumes, maximal gastric volume occurred immediately after the meal (184% +/- 19% of baseline). CONCLUSION: This method permits simultaneous measurement of gastric emptying and accommodation. In healthy subjects, the gastric accommodation response is prolonged and persists despite nearly complete emptying of a liquid or solid meal.  相似文献

Several clinical studies have indicated that gastric emptying is delayed in patients with Roux-en-Y biliary diversion with vagotomy. In order to determine whether Roux-en-Y diversion without vagotomy also induces delayed gastric emptying, we have compared the effect of gastrectomy without vagotomy with either Billroth II or Roux-en-Y anastomosis on gastric emptying of a liquid and solid meal in 22 peptic ulcer patients. The emptying half-times (t1/2) for solid food were not significantly different, 54; 24-122 min (median and range) in the 11 patients with Billroth II gastrectomy, 68; 44-189 min in the 11 patients with Roux-en-Y gastrectomy, and 83; 27-114 min in 11 normal control subjects. However, the lag phase was significantly (p less than 0.01) shorter in the patients with Billroth II gastrectomy (5; 0-43 min) and Roux-en-Y gastrectomy (4; 2-12 min) than in the control subjects (10; 4-22 min). The t1/2's for the fluid meal were similar, 7; 4-49 min after Billroth II gastrectomy, 8; 4-32 min after Roux-en-Y gastrectomy, and 9; 3-20 min in the control subjects. In all subjects the lag phase for the fluid meal was very short, ranging from 0 to 3 min. It is concluded that Roux-en-Y diversion per se does not delay gastric emptying in man.  相似文献

Reports on the existence of a lag phase before solid-food gastric emptying are conflicting. We studied solid-phase gastric emptying in ten normal-weight male subjects using two opposed cameras and continuous monitoring. Each ingested a 300-g meal containing 99mTc-labeled liver pate. Identical computer-interfaced cameras continuously monitored gastric activity from anterior and posterior projections. Lag phase was determined by three techniques: (1) inspection of the emptying curve; (2) time to a 2% decrease in stomach activity; and (3) the time of visual appearance of duodenal activity. A short lag phase time was found using all methods, averaging 8.6 min. We concluded that a short solid meal lag phase exists that can be missed with conventional radionuclide gastric emptying methods not employing continuous measurements.  相似文献

An animal model of gastric emptying may have use in the study of gastric physiology and pharmacoscintigraphy. The pig has anatomy and physiology similar to that of humans. Our aim was to develop a model of gastric emptying in the pig. It was not possible to perform this study in conscious pigs; therefore, an anesthetic model was developed. METHODS: Fifteen studies were performed on 4 pigs (age, 2-6 mo; weight, 20-100 kg). After acclimatization and training, pigs were fasted overnight before the study. Pigs were anesthetized using inhaled isoflurane without the use of injected premedication agents. An orogastric tube was inserted for the administration of a liquid meal, which consisted of (99m)Tc-diethylenetriaminepentaacetic acid either in water (nonnutrient) or with dextrose (nutrient meal). The pig was laterally positioned to enable right lateral dynamic acquisition to be performed. Anesthesia was maintained at 2% +/- 0.5% isoflurane in 4 studies and 0.8% +/- 0.5% in 11 studies (4 nutrient, 7 nonnutrient). RESULTS: With 2% +/- 0.5% isoflurane, there was delayed gastric emptying with a mean 50% emptying time (+/-SEM) of 141 +/- 14 min. With 0.8% +/- 0.5% isoflurane, the liquid meal emptied in an exponential manner similar to that of humans, with mean 50% emptying times (+/-SEM) of 30 +/- 7 min (nutrient) and 31 +/- 4 min (nonnutrient). CONCLUSION: The results indicate that high-dose anesthesia inhibits gastric emptying, but with low-dose anesthesia a useful pig model of liquid gastric emptying can be developed.  相似文献

Using a dual-headed gamma camera and a standardized egg test meal labeled with 99mTc-sulfur colloid, the quantitative emptying of the total, proximal and distal stomach in five normal subjects was characterized. The same egg meal was given to the volunteers in 3 different forms: homogenized, and as 2.5 mm and 5.0 mm cubes on 3 separate occasions for a total of 15 studies. For the total stomach emptying, the lag phase and half emptying time (T1/2) obtained using a power exponential model were significantly shorter for the homogenized test meal than for the 2.5 mm and 5.0 mm cubed egg particles; the lag phases were 29 +/- 19 min (mean +/- SD) vs 55 +/- 26 (P less than 0.05) and 64 +/- 24 min (P less than 0.01), the T1/2's were 71 +/- 30 min vs 91 +/- 26 (P less than 0.05) and 104 +/- 30 min (P less than 0.05), respectively. For the proximal stomach, no statistically significant difference was observed among the 3 test meals; the T1/2s for the homogenized, 2.5 mm and 5.0 mm cubed meals were 65 +/- 26 min, 53 +/- 18 min and 64 +/- 20 min, respectively. For the distal stomach, both the peak activity (%) and time to peak activity (min) were significantly higher for the 2.5 mm cubes (43%, 54 min) and the 5.0 mm cubes (38%, 60 min) than for the homogenized eggs (16.4%, 30 min) (P less than 0.01).(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

Radionuclide gastric emptying studies are performed as a matter of clinical routine. Our aim was to evaluate the inter- and intra-individual variability and the reproducibility of gastric emptying studies in healthy young male volunteers using a single solid-phase, standard meal. The meal consisted of a pancake (500 KJ) tagged with technetium 99m sulphur colloid and no additional liquid. Continuous acquisitions of gastric activity in anterior projection were taken during 90 min, starting from the onset of the meal. Gastric emptying was evaluated three times in a 3-week period. Five different parameters were evaluated. Our results show that there is important inter- and intra-individual variability in normal volunteers. In spite of this variability, no significant difference between the different series of gastric emptying studies was observed.  相似文献

The gastric fundus and antrum probably play different roles in the emptying of solids and liquids in man, but there is little information about the intragastric distribution of food. We have used a new radionuclide technique to quantify proximal, distal and total stomach emptying of a digestible solid meal (100 g of 99Tcm-labelled liver/ground beef given with 150 ml of 10% dextrose) in 13 normal subjects. A proximal reservoir area was seen after consumption of the meal, and used to define the proximal stomach region. A line drawn immediately below this area divided the total stomach region into proximal and distal stomach. Emptying from the total stomach exhibited two phases--an initial lag period (median 41 min, range 21-57 min), followed by approximately linear emptying. Redistribution of food from the proximal to the distal stomach was a major component of the lag period in 11 of the 13 subjects. In the remaining two subjects, redistribution was rapid (proximal 50% emptying time of 4 and 10 min) and antral retention was the major component of the lag period. In seven subjects, a region of reduced activity (contraction band) was seen. The midpoint of this band closely approximated to the line used to divide proximal and distal stomach. We suggest that a contraction band may be responsible for the proximal gastric reservoir seen after meal consumption and plays an important role in the redistribution and emptying of digestible solid food from the stomach. There is considerable variation between normal subjects in the rate of transfer of digestible solid food from the proximal to the distal stomach.  相似文献

This study was undertaken to determine if men and women have similar rates of gastric emptying. Fifteen normal weight male subjects aged 20-53 yr (mean 30 yr) and 15 normal weight female subjects aged 20-45 yr (mean 32 yr) were studied. Each subject ingested an identical 300-g meal consisting of 150 g of beef stew and 150 g of orange juice. Technetium-99m-(99mTc) labeled liver pate was used as the radioactive marker for the solid phase; indium-111 DTPA [( 111In]DTPA) was the liquid marker. A scintillation camera and computer was used to measure the rate of emptying of the solid and liquid phases. Correction for radionuclide depth, decay, and crosstalk was performed. The half-emptying time for men was: solids 59.8 +/- 3.7 min, liquids 30.3 +/- 2.3 min, and for women was: solids 92.4 +/- 7.5 min, liquids 53.8 +/- 4.9 min. These differences were statistically significant (solids, p = 0.00097; liquids, p = 0.00038). We conclude that women empty both solids and liquids from their stomachs more slowly than men. Possible mechanisms for the difference in gastric emptying rates between men and women are discussed.  相似文献

The aim of this study was to assess the influence of the normal menstrual cycle and of menopause on the gastric emptying rate of solids. Gastric emptying was studied in 15 premenopausal and ten postmenopausal women with an isotopic technique after the ingestion of a radiolabelled test meal. Premenopausal women were studied twice: within 1 week prior to menses and again 1 week after onset of menses. Postmenopausal women were studied only once. The emptying curves of the solid component of the meal fitted a linear model. The half-emptying time was 78±5 min during the follicular phase, 75 ± 7 min during the luteal phase and 76 ± 6 min in post-menopausal women (differences not statistically significant). The mean percentages of the meal retained in the stomach at different time intervals were also similar in the three groups. These results suggest that the menstrual cycle does not influence the gastric emptying rate of solids, which remains unchanged in relation to the follicular phase or after menopause. The gallbladder emptying response to sequential exogenous and endogenous cholecystokinin was measured by a noninvasive, nongeometric scintigraphic technique using a single dose of technetium 99m-labelled hepatobiliary agent. The mean latent period, ejection period, ejection fraction and ejection rate following 3 min infusion of 40 ng kg-1 of octapeptide of cholecystokinin were 2.8 +/- 0.7 min, 9.8 +/- 1.3 min, 34.0 +/- 9.9% and 3.5 +/- 2.0% per minute respectively. The corresponding values following 8 oz/70 kg of oral fatty meal were 15 +/- 6 min, 24.0 +/- 5 min (P less than 0.05), 55 +/- 9.1% and 2.8 +/- 0.6% per minute respectively. The total emptying for the combined stimuli was 71.4 +/- 14.0%. The wider range in latent and ejection period following fatty meal probably reflected the variations in time of the release into circulation and the duration of bioavailability of endogenous cholecystokinin. The lower rate of ejection was over compensated by longer duration of ejection resulting in higher degree of emptying following fatty meal. It is concluded that the gallbladder emptying pattern to exogenous and endogenous cholecystokinin can be studied sequentially using a single dose of technetium 99m-labelled hepatobiliary agent.  相似文献

AIMS: Dynamic antral scintigraphy (DAS), a non-invasive technique for the assessment of post-prandial gastric contractions, has been used to demonstrate abnormal contractility in several clinical conditions. The objective of the present study was to assess differences between solid and liquid meals regarding gastric contractions in healthy volunteers using DAS. METHODS: Ten healthy male volunteers were studied after ingesting solid or liquid meals [approximately 1670 kJ (approximately 400 kcal)] labelled with 99mTc phytate and administered in a random order. Gastric images were acquired for 120 min for gastric emptying half-time (T1/2) measurement. Dynamic (1 frame x s(-1)) images of the gastric antrum were acquired at 30 min intervals for 4 min for the assessment of antral contractility. RESULTS: Gastric emptying T1/2 values for solid and liquid meals were similar (58.1+/-19.06 min vs. 69.4+/-6.76 min; P=0.13). For the solid meal, average values for both frequency (3.08+/-0.15 cycles x min(-1) vs. 2.78+/-0.18 cycles x min(-1); P=0.003) and amplitude (33.94+/-5.2% variation vs. 24.09+/-7.37% variation; P=0.002) of antral contractions were significantly higher than those obtained with the liquid meal. For either of the test meals, none of the antral contractility variables correlated with gastric emptying T1/2. CONCLUSIONS: Dynamic antral scintigraphy is capable of detecting differences between solid and liquid meals concerning post-prandial gastric antral contractions under physiological conditions. The frequency and amplitude of gastric antral contractions after a solid meal are greater than after an exclusively liquid meal of similar calorie content, in spite of lack of a difference regarding gastric emptying.  相似文献

The study was designed to investigate the effect of trimebutine maleate, a drug used in both hyperkinetic and hypokinetic motility disorders, on gastric emptying in patients with non-ulcer dyspepsia having prolonged gastric emptying rates and to compare the parameters used for the determination of the lag period observed during the emptying of solid foods from the stomach. Gastric emptying was measured by the radionuclide technique. Twenty normal volunteers and 43 patients with non-ulcer dyspepsia participated in the study. Radionuclide imaging was performed by using a solid meal labeled with 99mTc-tin colloid. Of the patients with non-ulcer dyspepsia, 20 had prolonged gastric emptying. They were given three weeks of oral treatment with trimebutine maleate and had their radionuclide gastric emptying study repeated. Treatment with trimebutine maleate resulted in reduction in duration of the lag period and less retention of food at 100 minutes (p < 0.0005). After treatment with trimebutine maleate, no significant difference has been observed in the mean symptom score of patients with prolonged gastric emptying. Among the parameters used for the determination of the lag period, lag period determined by a mathematical equation (TLAG) has been found to be longer than the lag period determined by visual inspection of the images (VLAG) and there was correlation between the two parameters when the lag time was short.  相似文献
