外伤性肾上腺出血误诊1例

肾上腺出血在腹部钝性伤中很少见 ,现将我们遇到的外伤性肾上腺出血 1例报告如下。男 ,34岁。因酒后骑摩托车摔伤 ,头部、胸腹部 1h就诊 ,CT表现为肋骨多发骨折并胸腔积液 ,右下肺创伤性温肺 ,右颞区硬膜外血肿并蛛网膜下腔出血。右肾上腺区显示 3.6cm× 3.0cm软组织肿块 ,CT值为 5 0Hu ,边缘清晰 ,密度均匀 ,CT诊断为右肾上腺肿瘤 ,良性无分泌功能 (图 1)。 1个月后复查CT见右肾上腺区椭圆形肿物缩小为 2 .0cm× 1.1cm ,CT值为 2 6 .6Hu ,密度减低以中心减低明显 (图 2 ) ,已怀疑为肾上腺出血。 2个月后CT平扫见肿物约为 1.3cm× 0 .…     

Perinephric hemorrhage secondary to adrenal myelolipoma case report

Spontaneous perinephric hemorrhage has been described as occurring secondary to a number of primary renal abnormalities, but rarely from other causes. We describe the computed tomography (CT) and ultrasound findings of this event resulting from hemorrhage from an adrenal myelolipoma.     

Evaluation of scrotal edema in a patient on peritoneal dialysis. A case report

A 50-year-old man placed on peritoneal dialysis for end stage renal disease developed unilateral (left) scrotal edema following infusion of dialysis fluid into the peritoneum. The left side of the scrotum and left inguinal canal were surgically explored and a left hydrocele was removed. Following surgery, the patient again demonstrated left scrotal edema during peritoneal dialysis. Tc-99m MAA (5.0 mCi, 185 mBq) was injected into a 2-liter bag of dialysis fluid, which was subsequently infused into the peritoneum. The communication between the peritoneal cavity and the left inguinal canal was demonstrated.     

MRI diagnosis of epidural cervical hematoma. A case report

The epidural hematoma of the spine is a seldom disease and a neurosurgical emergency. MRI allows an early diagnosis. Only surgery can provide a good anatomical and functional prognosis.

Neonatal infratentorial subdural hematoma contributing to obstructive hydrocephalus in the setting of therapeutic cooling: A case report

BACKGROUNDSymptomatic neonatal subdural hematomas usually result from head trauma incurred during vaginal delivery, most commonly during instrument assistance. Symptomatic subdural hematomas are rare in C-section deliveries that were not preceded by assisted delivery techniques. Although the literature is inconclusive, another possible cause of subdural hematomas is therapeutic hypothermia.CASE SUMMARYWe present a case of a term neonate who underwent therapeutic whole-body cooling for hypoxic ischemic encephalopathy following an emergent C-section delivery for prolonged decelerations. Head ultrasound on day of life 3 demonstrated a rounded mass in the posterior fossa. A follow-up brain magnetic resonance imaging confirmed hypoxic ischemic encephalopathy and clarified the subdural hematomas in the posterior fossa causing mass effect and obstructive hydrocephalus. CONCLUSIONThe aim of this report is to highlight the rarity and importance of mass-like subdural hematomas causing obstructive hydrocephalus, particularly in the setting of hypoxic ischemic encephalopathy and therapeutic whole-body cooling.

Intradural nerve root hematoma in the lumbar spine. A case report

Intradural nerve root hematoma of the lumbar spine is extremely rare and can cause compression of the cauda equina. This case, which presented with low back pain and radiation to both lower extremities, diagnosed as an intradural hematoma of nerve root by magnetic resonance imaging (MRI) and was totally removed successfully. Intradural nerve root hematoma can present with or without a history of trauma or blood dyscrasia, where MRI is the best imaging modality, and can be well treated by surgery.     

双侧肾上腺囊肿1例

患者男性,43岁。有高血压病史约6年,既往有腰椎间盘突出症。患者自觉腰部不适,到门诊就医行双肾MRI检查,MRI平扫:右肾上腺区见信号均匀大小约6.0cm×7.0cm类圆形T1WI低信号、T2WI高信号灶,边缘清楚,与右侧肾上腺体部关系密切,并压迫邻近组织移位(图1,2)。左肾上腺区见大小约2.0     

High-dose-rate brachytherapy in scrotal extramammary Paget's disease: A case report

PURPOSEExtramammary Paget’s disease (EMPD) is a rare but lethal intraepithelial malignancy without standardized guidelines concerning diagnostic or therapeutic approaches. We report a case of EMPD of the scrotum treated with excellent results using high-dose brachytherapy.METHODS AND MATERIALSA 76-year-old male originally presented in 2015 with pruritus and erythema of the right scrotum, biopsy proved to represent extramammary Paget’s disease. He was treated for a year with topical creams without sustained relief. In July 2016 he underwent a right hemiscrotectomy which revealed stage 1 EMPD of the right scrotum and the medial thigh with positive margins but no deep invasion. Brachytherapy was selected as the most appropriate treatment option and carried out in December 2016 using HDR with a H.A.M. applicator and CT treatment planning.RESULTSOn December 2021, at 5 years of clinical and pathological follow up, the patient remains NED with minimal skin toxicity.CONCLUSIONSHigh-Dose-Rate Brachytherapy appears to be a feasible treatment alternative as adjuvant therapy in patients with EMPD with incomplete resection.
