We report a patient presenting with lichenoid plaques on exposed skin who had the metabolic features of porphyria cutanea tarda (PCT). Histology of lesional skin demonstrated a lichenoid inflammatory cell infiltrate in the upper dermis, while direct immunofluorescence revealed immunoreactive colloid bodies. Monochromator irradiation testing demonstrated photosensitivity in the visible spectrum consistent with porphyria. Solar-simulated irradiation induced a papular reaction with lichenoid histological changes. We propose that this atypical presentation of PCT may reflect a lichenoid tissue response to a porphyrin-mediated photochemical reaction.  相似文献

Epidermal basal cell damage in lichenoid tissue reactions (LTR) is considered to be the result of immunologic injury. In this study, we propose that LTR may be caused by local activation of Ia-reactive T cells. We have established allo-Iak-reactive helper T-cell clones and examined their behavior after adoptive transfer. We show that local transfer of 3 allo-Iak-reactive helper T-cell clones with different cross-reactivities and functions in vitro can cause delayed type hypersensitivity (DTH) reactions in vivo with antigen specificities identical to those demonstrated in vitro. Clone SK.1, when injected into appropriate recipients, caused massive dermal infiltrates of neutrophils and mononuclear cells. The latter were attracted to the epidermis and induced LTR-like basal cell degeneration which peaked at 72 h. Appropriate recipients were those strains of mice whose spleen cells were able to stimulate SK.1 cells to proliferate in vitro. Two other clones, SK.2.18 and SK.2.16, evoked significant DTH responses in their appropriate recipients, but the massive cellular infiltrates induced by either clone never invaded the epidermis or produced an LTR. The degeneration of epidermal cells caused by SK.1 cells did not correlate with the tested functions of this clone in vitro. The finding that only 1 of the 3 allo-Ia-reactive helper T-cell clones induced epidermotropic cellular infiltrates indicates that the infiltrative pattern of leukocytes in skin may depend on the particular T-cell clone that is activated.  相似文献

INTRODUCTION: Leflunomide (Arava) is an immunomodulator, recently introduced for systemic treatment of rheumatoid arthritis. We report the first case of lichenoid drug reaction due to this drug. CASE REPORT: A sixty-four year-old woman received leflunomide for rheumatoid arthritis. Two months after initiation of treatment, pruritus and lichenoid papules appeared on her hands and subsequently on her arms and her trunk, with a few bullous lesions. A skin biopsy was evocative for the diagnosis of drug induced lichenoid eruption. The treatment was stopped, and a wash out with colestyramine and topical corticotherapy resulted in dramatic improvement. No relapse was observed. Two months later, patch-tests with leflunomide diluted to 30 p. 100 in white petrolatum were negative. DISCUSSION: Side effects of leflunomide are frequent, generally benign for the cutaneous features. In our case, the delay, clinical and histological aspect and improvement on withdrawal of the drug emphasize the imputability of leflunomide. Few cases have been reported with others immunomodulators.  相似文献

Liver disease is now a well recognized association with lichen planus. Recent reports suggest that the hepatitis viruses may play a central role in this association. The present report describes a case of a lichenoid reaction to hepatitis B vaccination, which has only been rarely documented previously. A 16-year-old male developed a widespread pruritic erythematous eruption 5 weeks after receiving his second dose of hepatitis B vaccine. The eruption evolved to produce strikingly blue macules before spontaneously disappearing. A skin biopsy showed an intense lichenoid reaction. The hepatitis B surface antigen component of the vaccine is probably responsible for this reaction as well as for other rare immunologically mediated reactions to hepatitis B vaccination. These reactions may provide insight into the pathogenesis of the cutaneous features of natural hepatitis B infection.  相似文献

Determining the offending agent is complicated by the proliferation of lifestyle medications that the patient may not consider a medication. Lichenoid tissue reaction or interface dermatitis embrace several clinical conditions, the prototype of which is lichen planus and its variants, drug induced lichenoid dermatitis, special forms of lichenoid dermatitis, lichenoid dermatitis in lupus erythematosus, and miscellaneous disorders showing lichenoid dermatitis, the salient clinical and histological features of which are described to facilitate their diagnosis. As tattooing practices increase, delayed-type inflammatory reactions represent an uncommon adverse event to tattoo pigments. Different reaction patterns, such as eczematous, lichenoid, granulomatous and pseudolymphomatous reactions, have been previously reported, especially in association with metals contained in red tattoo pigments. We report a lichenoid papular reaction to an organic red tattoo ink, characterized by an intense mononuclear infiltrate dominated by CD8(+) T cells and CD56(+) lymphocytes and distributed in the superficial dermis around the red pigment and in the epidermis. Cytofluorimetric analysis of the lesional skin infiltrate confirmed the high frequency of cytotoxic CD8(+ )T cells and CD56(+)CD16(-) lymphocytes, most of which release type 1 cytokines. Chemical analysis of the red tattoo pigment confirmed its organic nature and the presence of intermediate reactive compounds. The lichenoid tissue reaction to red organic tattoo pigment showed the prototypical features of a cytotoxic inflammatory response to foreign substances (xenobiotics). The chemically unstable and reactive nature of modern tattoo pigments has to be taken into account by the clinician as well by the tattoo recipients.  相似文献

Fifty-one Kenyan patients with variable round plaques on the exposed parts, predominantly on the face, and characterized by central hyperpigmentation with annular hypopigmented borders, are described. The clinical and histological picture showed transitions, which ranged from lichen planus and lichenoid poikilodermatous syndromes to dermatitis. This syndrome corresponds largely with (sub-)tropical or actinic lichen planus as reported from the Middle East, where emphasis has been given mainly to a part of the described spectrum of transitions. We suggest that this syndrome should be considered as one of Pinkus' regional lichenoid syndromes which represent hybrids between lichen planus and other conditions rather than as a mere variant of lichen planus.  相似文献

First‐line anti‐tubercular therapy (ATT) is very effective in management of tuberculosis and is usually well tolerated. Varied spectrum of cutaneous adverse drug reactions is associated with ATT, of which lichenoid drug eruption (LDR) constitutes approximately 10% of the cases. However, LDR presenting as erythroderma is very rare. Here, we report a case of exfoliative dermatitis secondary to LDR which developed after 5 months of ATT.  相似文献
