目的探讨肝功能指标血清中γ谷氨酰胺转肽酶(GGT)与血压水平的相关性。方法采用横断面研究方法,选取邯郸地区2 372名在岗男性煤矿工人为研究对象,通过职业健康检查获取资料,包括年龄、性别、职业史、家族史、生活史等,测量其身高、体重指数(BMI)、收缩压(SBP)、舒张压(DBP)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、空腹血糖(FPG)、丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、血红蛋白(Hb)等。将所入选的受检者根据血压水平(mm Hg)分为3组,正常血压组(n=957):SBP120、DBP80;高血压前期组(n=958):SBP 120~139、DBP;高血压组(n=457):SBP≥140、DBP≥90。按血清中GGT水平四分位数分为4组,以SPSS 17.0统计软件分析其血清中GGT与SBP、DBP、FPG、TG、TC、HDL-C、BMI之间的相关性。结果①该人群的血清中GGT随血压水平的升高而逐渐升高,差异有统计学意义(P0.01);②年龄、BMI、腰围、TG、TC、FPG、Hb随血压水平的升高而逐渐升高,差异有统计学意义(P0.01);HDL-C随血压升高而逐渐降低,差异有统计学意义(P0.05)。③根据GGT水平四分位组比较,随着GGT水平升高,年龄、SBP、DBP、TC、TG、FPG、Hb、ALT、AST、腰围和BMI均升高,差异均有统计学意义(P0.01),HDL-C随GGT升高而降低,差异有统计学意义(P0.01)。④以血清中GGT水平为因变量,年龄、TG、TC、HDL-C、FPG、Hb、SBP、DBP、ALT、AST为自变量进行逐步回归分析显示,GGT可显著影响SBP、TG、TC、FPG、Hb,未发现对DBP有独立影响。结论血清中GGT水平与血压呈显著正相关,是收缩压的独立风险因素。  相似文献

ObjectiveTo explore the alterations of serum lipid m etabolism of Type 2 diabetes mellitus(DM) and its relationship with cardiovascul ar diseases.MethodsA 1∶1 case-control study in 100 newly diagnose d cases of Type 2 diabetes mellitus and 100 controls was conducted,8 indices of serum lipid metabolism were measured.ResultsThe means of TG and apo B in the group of obesity and overweight were significantly higher than those i n the normal weight group,and the means of HDL-C and Lp(a) were the reverse.BMI was significantly positively correlated with TG and apo B,but negatively correl ated with HDL-C and Lp(a).WHR was significantly positively correlated with TC,T G,LDL-C and apo B,whereas negatively correlated with HDL-C.The group with high er TC,LDL-C and apo E had a significantly higher proportion of hypertension,and the group with higher HDL-C had a significantly lower proportion of hypertensi on and CAD.ConclusionsThis study indicated that the dyslipidemia of type 2 diabetes mellitus(especially with obesity),such as low HDL-C,high TG,LD L-C and apo E might be a major cause of AS and cardiovascular diseases.  相似文献

目的分析2型糖尿病(T2DM)高尿酸血症的临床及生化特征,并探讨血尿酸(UA)与代谢综合征(MS)组分数目的关系。方法采用横断面研究方法,将547例T2DM患者按UA分为高尿酸血症组和正常尿酸组;按具有代谢综合征组分的数目不同分为4组(MS1,MS2,MS3及MS4),分析临床资料。结果T2DM并高尿酸血症组的体重指数(BMI)、收缩压(SBP)、舒张压(DBP)、空腹血糖(FPG)、肌酐(Cr)、甘油三酯(TG)、总胆固醇(TC)、糖化血红蛋白(HbAlc)明显高于正常尿酸组(P〈0.05,P〈0.01),高密度脂蛋白胆固醇(HDL-C)明显低于正常尿酸组(P〈0.05),低密度脂蛋白胆固醇(LDL-C)2组间差异无统计学意义(P〉0.05);MS1—4组间UA、BMI、SBP、DBP、Cr、TG、TC、LDL—C、HDL-C差别均有统计学意义(P均〈0.05),年龄、FPG差别无统计学意义;UA与性别、TG、HbAlc、HDL-C、Cr有线性回归关系(P〈0.05,P〈0.01),其中与HbAlc、HDL-C负相关,TG对UA影响最大。结论随着代谢综合征组分数目的增加,T2DM患者UA增加,异常TG对UA影响最大,因此UA水平升高是代谢综合征的危险因素。  相似文献

目的 探讨金昌队列单抗-HBs阳性人群肝功酶异常与脂质代谢指标的关系。方法 整群抽取2011年6月~2013年12月进入金昌队列且排除各种肝病史的单抗-HBs阳性者14 467例作为研究对象,通过实验室血清学检测相关酶学及脂质代谢指标。结果 该人群丙氨酸氨基转移酶(alanine aminotransferase,ALT)、天门冬氨酸氨基转移酶(aspartate aminotransferase,AST)、谷氨酰转肽酶(gamma-glutamyltransferase,GGT)异常检出率分别为21.35%、17.45%、15.01%,均为男性高于女性(均有P<0.001)。趋势性χ2检验分析显示,随着甘油三酯(triglyceride,TG)、总胆固醇(total cholesterol,TC)、低密度脂蛋白(low density lipoprotein cholesterol,LDL-C)、体质指数(body mass index,BMI)升高,ALT、AST、GGT异常检出率升高(均有P<0.001);随着高密度脂蛋白(high density lipoprotein cholesterol,HDL-C)升高,ALT、AST、GGT异常检出率下降(均有P<0.001),具有剂量反应关系。尿酸(uric acid,UA)异常组ALT、AST、GGT异常检出率均高于正常组(均有P<0.001)。结论 金昌队列单抗-HBs阳性人群ALT、AST、GGT异常检出率均为男性高于女性,且与BMI、TG、TC、LDL-C、HDL-C存在剂量反应关系。  相似文献

目的 探讨杭州市某社区人群血尿酸水平与代谢综合征(MS)各组分肥胖、血压、血糖和血脂的相关性.方法 对2013年杭州市四季青社区参加体检的18岁以上居民2 288人进行调查,分别按该人群中男性和女性血尿酸水平的四分位数法将男女性各分为4组,测定体重指数(BMI)、腰围(WC)、收缩压(SBP)、舒张压(DBP)、空腹血糖(FPG)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)和尿酸(UA)等指标.结果 在男性人群中,SBP、DBP、WC、TC和TG均随着血UA水平的升高而显著升高(P<0.05),而HDL-C则显著降低(P<0.05);在女性人群中,年龄、SBP、DBP、WC、FPG、TC、TG和LDL-C均随着血UA水平的升高而显著升高(P<0.05),而HDL-C则显著降低(P<0.05);logistic回归分析显示,在男性人群中,BMI、SBP和TG与血UA水平升高呈显著正相关(P<0.05);在女性人群中,BMI、TG和FPG与血UA水平升高呈显著正相关(P<0.05).结论 高尿酸血症与中心性肥胖、血压高、血糖高及血脂异常等MS组分之间有密切关系,血UA水平与女性的相关性更强.因此,有效地控制血UA水平是预防和治疗MS的重要途径之一.  相似文献

目的 探讨脂肪肝患者血清瘦素水平变化及其与胰岛素抵抗之间的关系。方法 用放免法测定 5 7例脂肪肝患者及 33例正常对照组患者血清瘦素 (LEP)水平 ,同时测定体重指数(BMI)、空腹血糖 (FPG)、空腹胰岛素 (FINS)、血脂 ,采用稳态模式 (HOMA)评价胰岛素抵抗。结果 (1)脂肪肝患者LEP水平、BMI、FPG、FINS、HOMA、胆固醇 (TC)、甘油三酯 (TG)、低密度脂蛋白 (LDL-C)均高于对照组 (P <0 .0 1、P <0 .0 5 )。 (2 )女性LEP水平明显高于男性 (P <0 .0 1)。 (3)LEP与BMI、FINS、TC、TG、LDL -C、HOMA呈正相关 ;多元线性回归分析显示 ,脂肪肝患者HOMA、FINS、BMI是LEP的主要相关因素。结论 脂肪肝患者存在高瘦素水平及瘦素抵抗、高胰岛素血症及胰岛素抵抗 ,高瘦素水平与胰岛素抵抗密切相关。  相似文献

目的:了解非酒精性脂肪肝患者的临床特点。方法:分析130例非酒精性脂肪肝(NAFLD组)患者和50例健康对照者(对照组)的临床资料,包括身高、年龄、性别、体质量、腰围、血压、空腹血糖(FPG)、甘油三酯(TG)、总胆固醇(TC)、谷丙转氨酶(ALT)、尿酸(UA)、高密度脂蛋白(HDL-C)及低密度脂蛋白(LDL-C)等。结果:NAFLD患者高发年龄在40-60岁之间,女性随年龄增长发病率逐渐升高。NAFLD患者体质量、体质量指数、腰围、血压、FPG、TG、LDL-C、ALT、UA水平均明显高于对照组,而HDL-C水平明显低于对照组,差异均有统计学意义(P〈0.05)。结论:及早进行临床生化和超声检查,同时改变不良生活方式,可以有效预防NAFLD的发生发展。  相似文献

目的研究成年男性血尿酸水平与代谢综合征的相关性。方法对2010月10日~2011年1月在某院进行体检资料完整者1 234例成年男性进行调查,测量身高、体重、收缩压(SBP)、舒张压(DBP),并检测血尿酸(UA)、空腹血糖(FPG)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)。按UA分为高尿酸组(HUA,268例)和正常尿酸组(对照组,n=966例),分析临床资料及与代谢综合征各组分的相关性。结果 (1)高尿酸血症检出率为21.72%。(2)高尿酸组TG、TC、LDL-C、体重指数(BMI)、SBP、DBP指标高于对照组,而HDL-C低于对照组,差异均有统计学意义(P﹤0.01)。(3)直线相关分析结果显示:UA与TG、TC、LDL-C、BMI、SBP、DBP呈正相关,而与HDL-C负相关,差异均有统计学意义(P﹤0.01),多元线性逐步回归分析结果显示:进入回归方程的有TG、BMI、DBP。(4)高尿酸组发生肥胖或超重、高血压、血脂紊乱检出率较正常组明显升高,差异均有统计学意义(P﹤0.01)。结论高尿酸血症与代谢综合征密切相关,应及早防治高尿酸血症及代谢综合征。  相似文献

目的分析非酒精性脂肪肝(NAFLD)患者和健康对照组的一般特征和临床指标,探讨其相关危险因素,为防治提供依据。方法根据B超检查诊断,体检患者分为正常对照组(4 086例)和NAFLD病例组(1 699例),比较两组人群的一般特征和各临床指标。结果各年龄段NAFLD的患病率有明显的性别差异。NAFLD病例组身体质量指数(BMI)、丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、总胆固醇(TC)、甘油三酯(TG)、空腹血糖(FBG)、尿酸(UA)、收缩压(SBP)、舒张压(DBP)均显著升高,而高密度脂蛋白胆固醇(HDL-C)显著降低(P0.05)。logistic回归分析显示BMI(OR为4.30)、TG(OR为1.94)、FBG(OR为1.86)、舒张压(OR为1.02)、HDL-C(OR为0.61)、LDL-C(OR为1.27)、ALT(OR为1.02)、AST/ALT(OR为0.40)进入方程,是独立危险因素(P0.05)。与NAFLD关系较为密切的指标是BMI、TG、FBG和LDL-C,可较好地预测NAFLD。结论 NAFLD患者具有肥胖、糖脂代谢紊乱(高FBG、高TG、高LDL-C、低HDL-C)、高血压、高尿酸的特征,且肝酶升高。BMI、FBG、TG、HDL-C、LDL-C、DBP、ALT是发生NAFLD的独立危险因素。另外,女性雌激素水平的降低可能是NAFLD的致病因素。  相似文献

目的分析不同营养状况青少年的脂代谢、糖代谢和血压状况,研究青少年肥胖与代谢综合征(MS)的关系。方法选择北京市东城区初中生966名,进行身体测量和血压、血脂、空腹血糖和胰岛素测定。结果不同营养状况青少年血压(SBP和DBP)、血脂(TC,TG,LDL-C)和空腹胰岛素(FINS)水平差异均有统计学意义(P值均〈0.05),均随体重指数(BMI)增加而上升;空腹血糖(FPG)差异无统计学意义(P〉0.05);除FPG,HDL-C外,其余MS指标异常检出率均为肥胖组〉超重组〉体重正常组(P值均〈0.05)。结论青少年肥胖与MS发生密切相关。肥胖青少年已经出现代谢紊乱,应重视并积极防治。  相似文献

目的揭示脂肪肝进展过程中伴随的机体代谢变化及其对人体的影响,引起人们对脂肪肝的重视并进行有效的干预。方法选择2年内连续2次在我处进行健康体检,经B超确诊脂肪肝程度进展的体检者414名作为本次研究的对象。应用配对t检验及卡方检验的方法,对研究对象的体质指数(BMI)、丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、空腹血糖(FPG)、血尿酸(UA)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL—C)等体检指标的水平及异常率进行比较。结果除FPG外,脂肪肝进展前后的BMI、TG、HDL—C、ALJT、AST、UA的水平及异常率均有统计学差异,P〈0.05;脂肪肝进展后BMI、TG、ALT、AST、UA的水平较前增高,HDL—C的水平较前降低,上述指标的异常率均呈现增高趋势。结论脂肪肝并非单纯的肝脏病变,脂肪肝的进展伴随着一系列代谢指标的改变,因而对脂肪肝的防治除了要重视对单纯性脂肪肝的早期干预,更要注重对代谢综合征的防治。  相似文献
