Objective To study the immediate conservative breast surgical reconstruction by trans-ferring the same lateral latissimus dorsal myocutaneous flap (LDM) for the treatment of stage Ⅱ and stage Ⅲ breast cancer, combined with neoadjuvant chemotherapy. Methods Breast cancer patients in stage Ⅱ and Ⅲ with pathological diagnosis by core needle biopsy, had undergone immediate conservative breast reconstruction surgery with transferring the latissimus dorsal myocutaneous (LDM). We scored the reconstructed breast twice at one month after operation and completion of radiation. The final score was decided using the mean value of the twice evaluating score. Kaplan-Meier survival was used to analyze the survival of 18 cases with traditional mastectomy. Results The tumor size ranged from 30 mm to 55 mm before neoadjuvant chemotherapy, and ranged from 25mm to 45 mm after neoadjuvant chemotherapy. The median weight of the tumor specimens in breast conservative operation was 140 g (90 g to 220 g). A-mong the 18 patients, 16 cases had more than 3 scores with satisfied cosmetics (93.33 % ). Dorsal subcu-taneous seroma in donation area was observed in 5 of 18 patients (27.78 %) and dorsal incision dehiscence was observed in 2 of 18 patients (11.11%). None of the patients developed recurrence except that 1 pa-tient (5.55 %) had distance metastasis (bone metastasis} in a mean 22-months follow-up. The Kaplan-Meier survival curve in breast conservation reconstruction patients had no different from that of traditional mastectomy patients at the same time. 目的:总结良性重症终末期肝病肝移植术后免疫抑制剂的应用经验,探讨个体化用药方案的价值。方法:回顾分析2002年4月—2010年8月术前终末期肝病模型(MELD)评分≥25分的179例肝移植病例资料,分为3个阶段:第一阶段(2002年4月—2004年12月)65例,第二阶段(2005年1月—2007年12月)64例,第三阶段(2008年1月—2010年8月)50例。第一阶段采用他克莫司(Tac)+吗替麦考酚酯(MMF)+甲基强的松龙(MP)的常规三联用药方案,第二阶段采用减量的三联用药方案,第三阶段采用个体化免疫抑制方案。比较3个阶段存活率、感染死亡率和排斥反应发生率。结果:第一、第二、第三阶段患者的存活率呈逐渐升高的趋势;患者感染死亡病例数占相应阶段病例总数比率为27.7%、17.2%、8.0%,差异有统计学意义(P〈0.05);患者感染死亡病例数占相应阶段死亡病例数比率为85.7%、64.7%、44.4%,呈下降趋势,第三阶段与第一阶段比较,差异有统计学意义(P〈0.05);第三阶段与第二阶段排斥反应发生率差异无统计学意义(P〉0.05),与第一阶段比较排斥反应发生率虽然轻度升高(P〈0.05),但是存活率却由67.7%提高到82.0%。结论:良性重症终末期肝病肝移植术后免疫抑制剂的个体化应用,减少了用药量,降低了感染死亡率,有利于提高患者存活率。  相似文献

目的探讨终末期肝病模型(MELD)评分评估终末期肝病患者行肝移植术后,受者短期预后、肝移植的手术时机以及MELD与肝脏病理的关系。方法对30例肝移植病例进行回顾性分析,比较术后随访30天后存活组(12例)与死亡组(18例)的术前MELD评分,以MELD分值25和30为界线将病例分组,比较存活率以及肝脏病理,分析大块、亚大块肝细胞坏死与非大块、亚大块肝细胞坏死病例的MELD分值。结果所有患者术前MELD评分平均值为28.92±13.45,术后随访3个月总生存率为40%,其中存活组与死亡组术前MELD评分分别为21.56±11.83和33.82±12.43(P〈0.05);以MELD评分25为界将患者分为两组,术后3个月存活率为63.6%和26.3%(P〈0.05);以MELD评分30为界将患者分为两组,术后3个月存活率为53.3%和26.7%(P〉0.05):大块、亚大块肝细胞坏死组与非大块、亚大块肝细胞坏死组患者术前MELD值相比有显著差异,分别为22.38±12.69和33.28±12.41(P〈0.05)。结论MELD评分可评估肝移植受术者的短期预后,肝移植受者MELD评分值在25分时比30分时行肝移植术更有意义,MELD评分与肝细胞坏死面积有关。  相似文献

目的 探讨肝移植治疗多囊肝病中的效果和经验.方法 回顾分析我中心2000年1月至2008年12月9例多囊肝行肝移植的病例,对患者术前MELD评分、肝肾功能,术中输血、失血,手术时间、无肝期以及术后并发症、存活时间等方面进行总结.结果 9例患者术前MELD平均(16±9)分,5例同时患有多囊肾,除1例出现肝硬化外其他8例无明显肝功能损害但因明显的压迫症状而严重影响生活质量,3例有肾功能异常需要透析.术中平均输血(1800±1600)ml,失血(3500±2600)ml,平均手术时间(7.2±1.5)h,无肝期(52.7±15.4)min.术后3例分别因腹腔出血、急性排斥反应及循环衰竭导致多器官衰竭而早期死亡;6例患者均存活1年以上,现最长存活时间8年.本组1年和2年存活率分别为77.8%和66.7%.结论 肝移植是治疗多囊肝疾病的有效方法,比较其他的肝移植受者手术时间长,失血量较大,手术难度较高,但预后良好.  相似文献

The number of patients dying while on the liver transplantation (LT) waiting list (WL) has continued to increase in recent years as a result of severe shortage of organs. Therefore, it is important to evaluate the existing models that predict death on the WL and to determine the independent predictors of death. The study cohort comprised 152 adult patients listed for LT in our centre over a period of 2 years (January 2001 to January 2003). The 12-month survival rate has been calculated by Kaplan-Meier method. The survival analysis performed by Cox proportional hazard model has evaluated the three parameters which compose the model for end-stage liver disease (MELD) score. Forty-four patients (28.9%) died while listed for LT. The survival rate was 92% at 3 months, 80% at 6 months and 69% at 12 months. Median survival was not reached. MELD score was found to be an excellent predictor of death at 12 months on our WL--c-statistic (area under curve) 0.84. In our survival analysis, only international normalized (prothrombin) ratio (INR) and serum creatinine were identified as an independent predictors of death (P < 0.0001). A new simplified version of the MELD score, which does not include serum bilirubin, is proposed and its c-statistic as predictor for death on the WL at 12 months is 0.86, as good as the original MELD score, when evaluated on our list. There is a fourfold increase in mortality on our WL for LT between 3 and 12 months after the inclusion. A simplified version of the MELD score, using only serum creatinine and INR might be taken into account when predicting 12 months mortality on WL with longer waiting time, but it has to be confirmed by other prospective studies.  相似文献

The aim of this study was to explore the feasibility of emergency right lobe adult-to-adult living-donor liver transplantation (LDLT) for high model for end-stage liver disease (MELD) score severe hepatitis. Consecutive 10 high MELD score severe hepatitis patients underwent emergency right lobe adult-to-adult LDLT in our hospital from April to December 2007. The MELD score was 34.50 ± 2.088. The outcomes of these recipients were retrospectively analyzed. Among them, eight cases of ABO blood group were identical and two cases compatible, one case was Rh negative. Two recipients died and the rest of the recipients and all donors are safe; perioperative and 2-year survival rate was 80%. The mean graft-recipient weight ratio (GRWR) was 1.27% ± 0.25%, and graft volume to recipient standard liver volume ratio (GV/ESLVR) was 56.7% ± 6.75%. Of the 10 patients, three received right lobe grafts with middle hepatic vein (MHV), four without MHV, three without MHV but followed by V and VIII hepatic vein outflow reconstruction. An encouraging outcome was achieved in this group: elevated serum creatinine, serum endotoxin, decreased serum prothrombin activity, and Tbil returned to normal on postoperative days 3, 7, 14, and 28, respectively. One-year survival rate was 80%. Outcomes of emergency right lobe adult-to-adult LDLT for high MELD score severe hepatitis were fairly encouraging and acceptable. Emergency right lobe adult-to-adult LDLT is an effective and life-saving modality for high MELD score acute liver failure patients following severe hepatitis.  相似文献

目的 探讨终末期肝病模型(MELD)评分较高的良性终末期肝病患者的肝移植疗效.方法 回顾分析80例良性终末期肝病肝移植患者的资料,根据MELD评分的不同将患者分成两组,MELD评分≥30分的23例为高MELD评分组,MELD评分<30分的57例为低MELD评分组.分别比较两组患者手术时间、术中无肝期、术中血液制品输入量、术后重症监护病房(ICU)治疗时间和受者1年存活率,同时比较死亡患者和存活患者的临床资料,寻找导致术后死亡的危险因素.结果 高MELD评分组的手术时间、术中血液制品输入量、ICU治疗时间以及术后3个月内的死亡率明显高于低MELD评分组,差异有统计学意义(P<0.05),而术中无肝期和患者1年存活率,两组间的差异无统计学意义(P>0.05).死亡者和存活者相比较,MELD评分的差异无统计学意义(P>0.05),而术前机械通气、血清钠水平、持续性肝性脑病(重型)等方面的差异有统计学意义(P<0.05).结论 对于良性终末期肝病患者,单纯依靠MELD评分不足以准确判断患者肝移植术后的生存状态,高MELD评分者也可获得较好的肝移植结果,术前严重的低钠血症、重度肝性脑病以及机械通气是除MELD评分以外影响患者术后生存状况的危险因素.  相似文献

Women have lower glomerular filtration (GFR) than men for the same serum creatinine (Cr) value, not accounted for in model for end-stage liver disease (MELD). We compare male/female Cr, GFR (using MDRD formula) and respective MELD scores in 403 Cr measurements using standard (sCr), O'Leary modified (mCr) and Compensated (cCr) Jaffe and Enzymatic (eCr) in 158 liver disease patients, mCr in 208 liver transplantation (LT) candidates, and EDTA-Cr(51)-GFR in 38 other candidates for LT; considering each female as male, a 'corrected' Cr was derived. MELD scores were calculated for measured and "corrected" Cr in females. Median Cr and GFR in females were lower than males (p < 0.05). Both MDRD and EDTA-Cr(51) GFR were lower in females than males, despite lower Cr values. In females, each MELD score was lower than the corresponding MELD-corrected Cr (p < 0.001) with > or =three-point difference in liver disease patients: 25%[sCr]; 23%[mCr]; 11%[eCr]; and 14%[cCr]. In 65% of female LT candidates, two- or three-point difference was found. Females with liver disease have lower GFR than males for the same Cr value; correcting Cr increases MELD score by two or three points in 65% of female LT candidates. MELD score adjustment in females would ensure equal LT priority by gender.  相似文献

It has been shown that the model for end-stage liver disease (MELD) score is an accurate predictor of survival in patients with liver disease without transplantation. Four recent studies carried out in the United States have demonstrated that the MELD score obtained immediately prior to transplantation is also associated with post-transplant patient survival. Our aim was to evaluate how accurately the MELD score predicts 90-day post-transplant survival in adult patients with chronic liver disease in the UK and Ireland. The UK and Ireland Liver Transplant Audit has data on all liver transplants since 1994. We studied survival of 3838 adult patients after first elective liver transplantation according to United Network for Organ Sharing categories of their MELD scores (< or = 10, 11-18, 19-24, 25-35, > or =36). The overall survival at 90-days was 90.2%. The 90-day survival varied according to the United Network for Organ Sharing MELD categories (92.6%, 91.9%, 89.7%, 89.7%, and 70.8%, respectively; P < 0.01). Therefore, only those patients with a MELD score of 36 or higher (3% of the patients) had a survival that was markedly lower than the rest. As a consequence, the ability of the MELD score to discriminate between patients who were dead or alive was poor (c-statistic 0.58). Re-estimating the coefficients in the MELD regression model, even allowing for nonlinear relationships, did not improve its discriminatory ability. In conclusion, in the UK and Ireland the MELD score is significantly associated with post-transplant survival, but its predictive ability is poor. These results are in agreement with results found in the United States. Therefore, the most appropriate system to support patient selection for transplantation will be one that combines a pretransplant survival model (e.g., MELD score) with a properly developed post-transplant survival model.  相似文献

The implementation of the model for end-stage liver disease (MELD) score decreased mortality of those awaiting liver transplantation (LT); however, the impact of the MELD allocation system on the risk of chronic renal disease after LT remains unknown. We conducted a non-concurrent single-center cohort study of 174 patients undergoing LT at our center. We compared patients who underwent LT one year prior to MELD implementation (pre-MELD cohort) to those patients who underwent LT 1 year following MELD implementation (MELD cohort). All patients were followed for at least 2 years after LT. Stage 3 chronic renal disease (CRD-3) was defined by an estimated creatinine clearance (CL(Cr)) below 60 ml/min/1.73 m2, and stage 4 chronic renal disease (CRD-4) was defined by an estimated CL(Cr) below 30 mL/min/1.73 m2 according to the validated Modification of Diet and Renal Disease (MDRD) formula. Requirement of kidney transplantation and need for hemodialysis were also evaluated following LT. The pre-MELD cohort (n=97) and the MELD cohort (n=77) were comparable in baseline characteristics, prevalence of diabetes and hypertension, and immunosuppression. Mean calculated MELD score in the pre-MELD cohort was significantly lower than in the MELD cohort (16 vs. 19, P < 0.05). The estimated CL(Cr) at time of LT was lower in the MELD cohort compared with the pre-MELD cohort (75 vs. 95, P < 0.01). However, the incidence and prevalence of CRD-3 and CRD-4 at 6, 12, and 24 months after LT were comparable between the two cohorts. Need for kidney transplantation or hemodialysis after LT was comparable between the groups. In multivariate analysis, serum creatinine at LT was the only variable associated with the development of CRD-3 in the first 2 years after LT. In conclusion, the implementation of the MELD allocation system is not associated with increased mortality or occurrence of CRD-3 or CRD-4 in the first 2 years after LT.  相似文献

目的  探讨术后首次终末期肝病模型(MELD)评分及其衍生评分MELD联合血清钠(MELD-Na)评分、MELD联合血乳酸(MELD-Lac)评分对于肝衰竭患者肝移植术后早期生存率的预测能力。方法  回顾性分析135例肝衰竭肝移植受者的临床资料,根据术后28 d的生存情况分为早期生存组(110例)和早期死亡组(25例),比较两组患者的临床资料,采用受试者工作特征(ROC)曲线确定MELD评分、MELD-Na评分与MELD-Lac评分对肝衰竭患者肝移植术后早期生存率预测的最佳截取值,以评价不同评分预测肝衰竭患者肝移植术后早期生存率的能力。结果  两组患者术后首次MELD评分、MELD-Na评分、MELD-Lac评分比较,差异均有统计学意义(均为P < 0.05)。术后首次MELD评分、MELD-Na评分、MELD-Lac评分预测肝衰竭患者肝移植术后早期生存率的AUC分别为0.653 [95%可信区间(CI) 0.515~0.792]、0.648(95% CI 0.514~0.781)、0.809 (95% CI 0.718~0.900),最佳截取值分别为18.09、18.09、19.97,约登指数分别为0.398、0.380、0.525,灵敏度分别为0.680、0.680、0.840,特异度分别为0.720、0.700、0.690。MELD-Lac评分预测肝衰竭患者肝移植术后早期生存率的AUC大于MELD评分和MELD-Na评分,差异均有统计学意义(均为P < 0.05)。结论  术后首次MELD评分及MELD-Na评分对于肝衰竭患者肝移植术后早期生存率预测能力一般,而术后首次MELD-Lac评分是肝衰竭患者肝移植术后早期生存率更为可靠的预测指标。  相似文献
