Introduction Since digital subtraction angiography (DSA) carries a low risk of morbidity, and is associated with patient discomfort and higher cost, our objective was to determine whether high-resolution 3-D time-of-flight MR angiography (TOF-MRA) at 3 T may replace DSA in the follow-up of patients after coiling of an intracranial aneurysm. Methods This prospective study included 50 consecutive patients with a ruptured and subsequently coiled intracranial aneurysm. All patients were followed up at a mean of 14 months after coiling with DSA and high-resolution 3-D TOF-MRA at 3 T generating 0.02 mm3 isotropic voxels. One examiner used DSA and TOF-MR angiograms to assess the need for and risk of retreatment; these data were used to calculate intermodality agreement. Another two examiners independently assessed aneurysm occlusion by DSA and TOF-MRA according to the Raymond scale; these data were used to calculate interobserver agreement. Results Discrepancies between DSA and TOF-MRA were found in three patients (intermodality agreement κ = 0.86). While DSA indicated complete aneurysm occlusion, TOF-MRA showed small neck remnants in the three patients. Coils on all DSA projections obscured these three neck remnants. Interobserver agreement was higher for DSA (κ = 0.82) than for TOF-MRA (κ = 0.68), which was in part due to the complexity of the information provided by TOF source images and reconstructions. Conclusion 3-D TOF-MRA at 3 T is not only an adjunctive tool but is ready to replace DSA in the follow-up of patients with previously coiled intracranial aneurysms. Additional DSA may only be performed in complex and not clearly laid out aneurysms.  相似文献

Following coil embolization of intracranial aneurysms, many centers perform at least one digital subtraction angiography (DSA) continuing with time-of-flight magnetic resonance angiography (TOF-MRA). Angiographic computed tomography (ACT) provides high-resolution data from a rotational acquisition of a c-arm-mounted flat panel detector. This study evaluates possible advantages of applying ACT in aneurysm follow-up. In 22 patients DSA examinations with a rotational acquisition were performed. Rotational data were processed into an isotropic high-resolution volume. TOF-MRA was performed the day before DSA. Three experienced neuroradiologists performed a rating of the occlusion rate and a subjective method comparison. Weighted κ statistics were calculated to assess the level of interobserver agreement. Compared to DSA, the diagnostic value of ACT as well as of TOF-MRA was rated to be inferior, although the sensitivity of detecting residual necks was higher with both techniques. Compared to TOF-MRA, ACT achieves favorable ratings only in aneurysms after stent-remodeling. Interobserver agreement was high for all techniques. Ratings of the occlusion rate correlated highly between all observers (r > 0.85, p < 0.001, respectively). In selected patients ACT can add valuable diagnostic information to DSA. TOF-MRA remains a highly sensitive method for aneurysm follow-up.  相似文献

Purpose

The objective of this study was to determine the value of high-field magnetic resonance imaging and to clarify the characteristics of each image among three-dimensional gradient echo (3D-GRE), two-dimensional spin echo (2D-SE) and inversion recovery (2D-IR) sequences used as contrast-enhanced T1-weighted images for stereotactic irradiation treatment planning of sellar lesions.

Materials and methods

Pulse sequences of 2D-SE and 3D-spoiled gradient recalled acquisition in the steady state (3D-SPGR) using GRE at 1.5 T and 2D-IR and 3D-fast SPGR (3D-FSPGR) at 3 T after injection of contrast material were acquired for 14 small pituitary tumors. As quantitative methods, signal-to-noise ratios (SNR) and contrast-to-noise ratios (CNR) were evaluated using a region-of-interest analysis.

Results

There was no significant difference in SNR between 1.5-T SPGR and 3-T FSPGR, while 3-T IR was superior to 1.5-T SE. The 2D-SE and -IR provided significantly better CNR than 3D-GRE between tumor and normal structures.

Conclusions

Three Tesla was found to be superior to 1.5 T in distinguishing tumors from the normal sellar structure. Optimal dose planning will utilize each advantage of imaging; 3D-GRE allows high-resolution acquisition and 2D-SE and -IR can offer better tissue contrast.  相似文献

This comparative study of tumour patients and volunteers aimed at differentiating liver parenchyma from neoplastic lesions by using localised 1H MRS at 3.0 T as an adjunct to MRI. In total 186 single-voxel proton spectra of the liver were acquired at 3.0 T using the body transmit receive coil. Consecutive stacks of breath-hold spectra were acquired in the PRESS technique at a short echo time of 35 ms and a repetition time of 2,000 ms. Processing of the spectra included spectral alignment with the software package SAGE and quantitative processing with LCModel. The resulting metabolite concentrations were presented in arbitrary units relative to the internal water. In general, the spectra showed four main groups of resonances originating from the methyl protons (0.8–1.1 ppm) and methylene protons of the lipids (1.1–1.5 ppm; 2.0–2.2 ppm) as well as the methyl protons of choline-containing compounds (CCC) at 3.2 ppm. Overall, the CCC and lipid values in malignant liver tumours showed no significant differences to liver parenchyma. On average, total lipid measurements in normal liver parenchyma increased with age, while those of the CCC did not show pertinent changes. Significant differences between the contents of CCC in malignant liver tumours and normal liver parenchyma were not observed, because in patients and volunteers normal liver tissue showed a large variability in the content of CCC.  相似文献

Objective

To explore the predictive value of parameters derived from diffusion-weighted imaging (DWI) and contrast-enhanced (CE)-MRI at different time-points during neoadjuvant chemotherapy (NACT) in breast cancer.

Methods

Institutional review board approval and written, informed consent from 42 breast cancer patients were obtained. The patients were investigated before and at three different time-points during neoadjuvant chemotherapy (NACT) using tumour diameter and volume from CE-MRI and ADC values obtained from drawn 2D and segmented 3D regions of interest. Prediction of pathologic complete response (pCR) was evaluated using the area under the curve (AUC) of receiver operating characteristic analysis.

Results

There was no significant difference between pathologic complete response and non-pCR in baseline size measures (p?>?0.39). Diameter change was significantly different in pCR (p?<?0.02) before the mid-therapy point. The best predictor was lesion diameter change observed before mid-therapy (AUC?=?0.93). Segmented volume was not able to differentiate between pCR and non-pCR at any time-point. The ADC values from 3D-ROI were not significantly different from 2D data (p?=?0.06). The best AUC (0.79) for pCR prediction using DWI was median ADC measured before mid-therapy of NACT.

Conclusions

The results of this study should be considered in NACT monitoring planning, especially in MRI protocol designing and time point selection.

The purpose of this study was to examine the differences in expert ratings of quality of magnetic resonance images (MRI) of the temporomandibular joint in 24 patients with suspected anterior disc displacement examined in randomized order at 1.5 and 3.0 T. Parasagittal (closed and opened mouth) and paracoronal sections were performed with a surface coil. Two experienced examiners blinded to patient status and type of MRI diagnosed the images according to position of condyle, position and changes in the signal, and disc shape. In addition, perceptibility of position and disc shape were assessed. A highly significant difference in the perceptibility of disc shape (P < 0.001) and position (P < 0.001) was obtained. With comparable examination sequences and identical resolution, the 3.0 T MRI of the temporomandibular joint increases the perceptibility of joint structures.  相似文献

AIM: To evaluate the value of administration of hyoscine-N-butyl-bromide (HBB) for image quality magnetic resonance imaging (MRI) of the prostate. METHODS: Seventy patients were retrospectively included in the study. Thirty-five patients were examined with administration of 40 milligrams of HBB (Buscopan ; Boehringer, Ingelheim, Germany); 35 patients were examined without HBB. A multiparametric MRI protocol was performed on a 3.0 Tesla scanner without using an endorectal coil. The following criteria were evaluated independently by two experienced radiologists on a five-point Likert scale: anatomical details (delineation between peripheral and transitional zone of the prostate, visualisation of the capsule, depiction of the neurovascular bundles); visualisation of lymph nodes; motion related artefacts; and overall image quality.RESULTS: Comparison of anatomical details between the two cohorts showed no statistically significant difference (3.9 ± 0.7 vs 4.0 ± 0.9, P = 0.54, and 3.8 ± 0.7 vs 4.2 ± 0.6, P = 0.07) for both readers. There was no significant advantage regarding depiction of local and iliac lymph nodes (3.9 ± 0.6 vs 4.2 ± 0.6, P = 0.07, and 3.8 ± 0.9 vs 4.1 ± 0.8, P = 0.19). Motion arte- facts were rated as "none" to "few" in both groups and showed no statistical difference (2.3 ± 1.0 vs 1.9 ± 0.9, P = 0.19, and 2.3 ± 1.1 vs 1.9 ± 0.7, P = 0.22). Overall image quality was rated "good" in average for both cohorts without significant difference (4.0 ± 0.6 vs 4.0 ± 0.9, P = 0.78, and 3.8 ± 0.8 vs 4.2 ± 0.6, P = 0.09). CONCLUSION: The results demonstrated no significant effect of HBB administration on image quality. The study suggests that use of HBB is not mandatory for MRI of the prostate at 3.0 Tesla.  相似文献

BACKGROUND AND PURPOSE: Three-dimensional time-of-flight (TOF) MR angiography has been evaluated in the follow-up of intracranial aneurysms treated with Guglielmi detachable coils (GDCs) with good results. Some of the studies used contrast material in addition to the 3D TOF MR technique and others did not. We assessed the usefulness of contrast material with 3D TOF MR angiography by comparing this sequence before and after contrast material injection. METHODS: Fifty-eight patients harboring a total of 71 cerebral aneurysms previously treated with GDCs were included in the prospective study. MR angiography (at 1.5 T) was performed with a 3D TOF sequence before and after injection of gadolinium-based contrast material. Features evaluated were presence and size of a neck remnant, parent and adjacent vessel patency, and venous overlap. Digital subtraction angiography was the standard of reference. RESULTS: Comparison of the techniques showed a good agreement in the detection of residual flow. Six cases of small residual neck were not detected with either the 3D TOF or the contrast-enhanced 3D TOF sequence. In one case of giant aneurysm, the extent of recanalization was more evident after contrast material administration. The use of contrast material did not help to show the parent and adjacent arteries. Venous overlap on contrast-enhanced 3D TOF angiograms did not affect image interpretation. CONCLUSION: In this series, the use of intravenous contrast material did not improve the ability of 3D TOF MR angiography to depict the presence of residual or recurrent aneurysms previously treated with endovascular coiling. In one giant aneurysm, use of intravenous contrast material did result in improved visualization of a residual aneurysm.  相似文献

PURPOSE: Multi-slice CT (MSCT) has great potential in evaluation of vascular structures. Our purpose was to investigate if there is any difference in detection of superiorly, inferiorly and horizontally directed small cerebral aneurysms (<5 mm) on MSCTA compared to digital subtraction angiography (DSA) or surgery. MATERIALS AND METHODS: One hundred and three consecutive patients who underwent MSCTA and DSA or surgery were included in the study. MSCTA and DSA results were evaluated independently by two different neuroradiologists who performed aneurysm detection, quantitation, and characterization using 2D multiplanar reconstructions, 3D maximum intensity projection and volume-rendered techniques. FINDINGS: MSCTA detected 49 small cerebral aneurysms (<5 mm) in 37 (36%) of 103 patients. The overall sensitivity, specificity, and accuracy of MSCTA for detecting small aneurysms were 0.85, 0.65, and 0.79, respectively. There was moderate agreement between MSCTA and DSA/surgery for detecting small aneurysms (kappa: 0.51). The sensitivity of detecting small aneurysms directed superiorly, inferiorly and horizontally was 0.94, 0.84, and 0.75, respectively. There was no statistically significant difference in detection between small aneurysms directed superiorly, inferiorly and horizontally on MSCTA (P > 0.05). CONCLUSION: The direction of small cerebral aneurysms is not important in diagnosis on multi-slice CT scanners, although the detection of small cerebral aneurysms with superior or inferior direction is slightly easier than the detection of horizontally directed aneurysms.  相似文献

Objective

To present imaging characteristics of the ankle at 7.0 T and to investigate the appearance and image quality of presumed pathologies of ankles without physical strain as well as of ankles after a marathon run in comparison to 1.5 T.

Materials and methods

Appearance of presumed pathologic findings and image quality of TSE (PD, T2, and STIR) and GRE sequences (MEDIC, DESS, and/or CISS) at 7.0 T and 1.5 T MRI were compared by two senior radiologists in consensus in two healthy controls without strain and in six marathon runners after a full-length marathon (eight males, mean age 49.1 years).

Results

Overall, 7.0 T MRI allowed for higher resolution images for most of the sequences while requiring comparable acquisition times and achieving high contrast images mainly in gradient echo sequences. Bursal or presumed peritendineal fluid and/or edematous tissue, which were found in seven of eight subjects, could be best appreciated with 7.0 T MEDIC. Other findings with sharper delineation at 7.0 T included cartilage defects (best: CISS), osseous avulsions, and osteophytes (best: DESS). Nevertheless, 1.5 T STIR imaging enabled assessment of a tibiotalar bone edema-like lesion in two runners, which was barely visible at 7.0 T using STIR, but not with any other sequence at 7.0 T including MEDIC (with frequency selective fat suppression). 7.0 T showed larger image quality variations with challenges especially in the TSE sequences.

Conclusion

Our initial results of ultra-high-field ankle joint imaging demonstrate the improved depiction of ankle anatomy, fluid depositions, and cartilage defects. However imaging of edema-like bone lesions remains challenging at ultra-high magnetic field strength, and TSE coverage in particular is limited by the specific absorption rate.  相似文献

The purpose of this study was to prospectively evaluate the safety of cardiac magnetic resonance (CMR) imaging at 3 T performed early (less than 14 days) after bare metal or drug-eluting coronary stent implantation in patients with acute myocardial infarction (AMI). Seventy-two consecutive patients with AMI treated by percutaneous revascularisation with a stent underwent CMR examination with a median delay of 6 days. Patients were followed-up for major adverse cardiac events, during hospitalisation and at 6 months. After CMR imaging, no acute stent thrombosis, death or repeated AMI were recorded at 6-month follow-up. Two symptomatic in-stent restenoses and two silent in-stent restenoses were recorded, at a mean delay of 106 days. In our population, we found a target revascularisation rate of 5.6%. This is consistent with the 6-month event rates after coronary artery stent (CAS) placement for AMI, evaluated by several studies. This preliminary clinical study supports the safety of 3-T CMR imaging performed early after coronary stent placement.  相似文献

Magnetic resonance angiography (MRA) for determining the degree of carotid stenosis prior to carotid endarterectomy is attractive because it does not have the high morbidity associated with conventional intra-arterial angiography. We assessed the interobserver variability in the estimation of the degree of stenosis amongst observers of different experience. In a prospective study, consecutive patients with transient ischaemic attacks and symptomatic tight carotid stenosis shown by Doppler ultrasound underwent conventional intra-arterial angiography and 2-D and 3-D time-of-flight MRA of the carotid bifurcations. The films of the processed MRA images were reviewed blind to other clinical and imaging data by eight observers of different levels of experience and coded for the presence and degree of stenosis. The stenosis on intra-arterial angiography was used as the reference standard. There was considerable variability between observers for estimation of the degree of stenosis on MRA. The observers' accuracy ranged from 41% (student) to 79% (experienced). From these estimations, excluding those of the student, it could be seen that up to 23% of patients who would have had an endarterectomy based on MRA should not have done so according to angiography, and up to 33% of patients who should have had an endarterectomy according to angiography would have inappropriately missed having an endarterectomy based on MRA results. Observer reliability of MRA processed images is not good, even in experienced hands. Using the source images and views of the circle of Willis might improve the accuracy, but a further study is required to assess this.  相似文献

The goal of this study was to compare magnetic resonance (MR) image quality at different field strengths for evaluating lesions in wrist and finger joints of patients with rheumatoid arthritis (RA) in order to determine whether the higher field strength provides diagnostic gain. The hand mainly affected in 17 RA patients was examined at 1.5 Tesla (T) and 3.0 T with comparable MR imaging (MRI) protocols. MR images were reviewed twice by two experienced radiologists using the Rheumatoid Arthritis MRI Scoring System (RAMRIS) of the OMERACT (Outcome Measures in Rheumatoid Arthritis Clinical Trials) group. Image quality was rated on a five-point scale using Friedmann’s test and Kendall’s W-test for statistical analysis. Image comparison revealed better image quality at higher field strength. Image quality of T1-weighted images was rated 14–22% better at 3.0 T compared with 1.5 T by both readers. Moreover, the rating for the T2-weighted-images acquired at 3.0 T was one point better in the five-point scale used. Inter-reader correlation for image quality, bone erosions/defects, edema and synovitis ranged between 0.6 and 0.9 at 3.0 T and between 0.6 and 0.8 at 1.5 T. Intra-reader correlation for these parameters was high at 0.8–1.0. MRI image quality of RA hands is superior at 3.0 T, while an acceptable image quality is achieved at 1.5 T, which improves the evaluation of extent of bone edema, synovitis and identification of small bone erosions.  相似文献

The purpose of this study was to assess the diagnostic accuracy of whole-body MRI (WB-MRI) at 1.5 T or 3 T compared with FDG-PET-CT in the follow-up of patients suffering from colorectal cancer. In a retrospective study, 24 patients with a history of colorectal cancer and suspected tumour recurrence underwent FDG-PET-CT and WB-MRI with the use of parallel imaging (PAT) for follow-up. High resolution coronal T1w-TSE and STIR sequences at four body levels, HASTE imaging of the lungs, contrast-enhanced T1w- and T2w-TSE sequences of the liver, brain, abdomen and pelvis were performed, using WB-MRI at either 1.5 T (n = 14) or 3 T (n = 10). Presence of local recurrent tumour, lymph node involvement and distant metastatic disease was confirmed using radiological follow-up within at least 5 months as a standard of reference. Seventy seven malignant foci in 17 of 24 patients (71%) were detected with both WB-MRI and PET-CT. Both investigations concordantly revealed two local recurrent tumours. PET-CT detected significantly more lymph node metastases (sensitivity 93%, n = 27/29) than WB-MRI (sensitivity 63%, n = 18/29). PET-CT and WB-MRI achieved a similar sensitivity for the detection of organ metastases with 80% and 78%, respectively (37/46 and 36/46). WB-MRI detected brain metastases in one patient. One false-positive local tumour recurrence was indicated by PET-CT. Overall diagnostic accuracy for PET-CT was 91% (sensitivity 86%, specificity 96%) and 83% for WB-MRI (sensitivity 72%, specificity 93%), respectively. Examination time for WB-MRI at 1.5 T and 3 T was 52 min and 43 min, respectively; examination time for PET-CT was 103 min. Initial results suggest that differences in accuracy for local and distant metastases detection using FDG-PET-CT and WB-MRI for integrated screening of tumour recurrence in colorectal cancer depend on the location of the malignant focus. Our results show that nodal disease is better detected using PET-CT, whereas organ disease is depicted equally well by both investigations.  相似文献
