DDD Pacing: An Effective Treatment Modality for Recurrent Atrial Arrhythmias   总被引:10,自引:0,他引:10  
We performed atrial EP studies (atrial substrate evaluation) on 10 patients. These patients had evidence of paroxysmal, sustained, recurrent atrial arrhythmias (7 men and 3 women with a mean age of 64 ± 15 years). All patients combined a brady-tachy syndrome; 7 patients had a sick sinus syndrome (SSS) and 3 patients a typical vagally induced atrial arrhythmia. No anti-arrhythmic drug was allowed in 3 patients with SSS, 1 drug failed in 4 patients and the combination of 2 drugs failed in 3 patients during the first to eighth years prior to pacemaker implantation. Atrial substrate evaluation was feasible in all these patients off anti-arrhythmic therapy and showed important abnormalities of atrial loco-regional conduction parameters and long refractory periods (RP). The remarkable point was, in 7 patients, a paradoxical improvement in intra-atrial conduction delay at rapid pacing rate. The DDD pacing mode was chosen in all patients. No technical problem occurred during implantation. Atrial pacing rate was programmed to be slightly higher than the mean diurnal heart rate calculated on Holter monitoring. After implantation, the mean follow-up period was 18 ± 25 months with an average of one Holter every 4 months during the first 2 years. The 7 patients who improved intra-atrial conduction at rapid pacing rate were controlled without drugs, 2 patients were controlled with 1 drug, and 1 patient with 2 drugs. Atrial pacing in the DDD mode in a selected group of patients prevents paroxysmal and drug-resistant atrial arrhythmias. Atrial substrate evaluation is a sensitive tool for assuring the long-term benefit of atrial pacing. In this subset of patients, maintenance of AV synchrony by DDD pacing is preferable to catheter ablation of the His bundle.  相似文献

Atrial arrhythmias (AA) are commonly encountered in DDD paced patients. Newer dual chamber pacemakers (PM) possess mode switching functions that convert pacing to an asynchronous mode when AAs are detected. The lack of a reliable mode switch leading to rapid, irregular ventricular responses may result from AA undersensing. To avoid this, the DDDR PM Chorum 7234 Eta Medical AA diagnosis is based on a statistical approach: the PM constantly compares arrhythmic and sinus cycles and, based on "strong" and "weak" criteria, provides for rapid or slower mode switch. The aim of the study was to evaluate the efficiency and reliability of these two criteria. Thirty-one patients with a Chorum 7234 implanted for AV block (11), sinus dysfunction (10), both (5), or hypertrophic obstructive cardiomyopathy (5) were evaluated at 24 hours and 1 month using the internal memory (IM) of the PM, surface 24-hour Holter recordings, and exercise testing. Interrogation of the IM on the first day of study showed that 8 patients had mode switching episodes, based only on the strong criterion confirmed by the surface Holter recording. At I month, the IM revealed mode switching episodes in 12 patients, 6 of whom had used the weak criterion. No inappropriate mode switching episode was recorded during exercise testing at the 1-month follow-up. These results confirm the reliability and efficiency of this algorithm as well as the requirement for a specific algorithm to compensate for transient loss of sensing during AA.  相似文献

The new DDDR pacemaker META DDDR utilizes a minute veritilation sensor based on transthoracic impedance measurements. The sensor determines the metabolic indicated interval, the atrioventricular (AV) delay and the postventricular atrial refractory period (PVARP). The baseline PVARP must be carefully selected to define nonphysiological tachycardias. If a Pwave falls within the PVARP the pacemaker will automatically switch to the VVIR mode. This behavior prevents tracking of paroxysmal atrial tachyarrhythmias (PAT). Twenty-eight patients with sinus node dysfunction (n = 20), AV junction ablation (n = 5), complete or intermittent AV block (n = 3); who received a META DDDR pacemaker were studied. The mean age was 65 ± 13 years. Results: mode switching (reversion) to VVIR was observed in 57% of the patients. Forty-two percent had episodes of mode switching to VVIR during a stress test four related to PAT, and seven to sinus tachycardia. Fifty percent had episodes of mode switching to VVIR during a 24-hour Holter, four related to PAT, three to retrograde P wave sensing, and two to sinus tachycardia. At the last follow-up, 20 of the 26 patients initially programmed to the DDDR mode remained in the DDDR mode, while five were reprogrammed to the DDD and one to the VVIR mode. Mode switching has a high sensitivity but a low specificity for PAT. It appears to be a useful approach to prevent rapid tracking of atrial tachyarrhythmias. Careful PVARP programming is critical to appropriate reversion behavior, but further modifications of the algorithm are needed to improve its performance.  相似文献

CAZEAU, S., ET AL.: Dynamic Electrophysiology of Ventriculoatrial Conduction: Implications for DDD and DDDR Pacing. The behavior of ventriculoatrial conduction (VAC) during exercise remains unknown. In order to determine its characteristics and the consequences it might have on dual chamber pacemaker technology and programming, 17 patients underwent an electrophysiological study (EPS) of atrioventricular conduction (AVC) and of VAC during a protocol including three steps: supine rest, upright position, and finally during cycloergometric exercise; the measurements were done at progressively increasing pacing rates. During a preimplantation EPS, Wenckebach points AVC-W and VAC-W and conduction times, AVCT and VACT (as a function of pacing rate), were measured in ten consecutive patients using temporary leads and an external device. In another study, AVCT, VACT, AVC-W, and VAC-W were measured by telemetric recordings under identical conditions in seven patients implanted earlier with a DDD pacemaker. A 1/1 VAC was observed in 7/17 patients (41%) at rest, and in 13/17 patients (76%) at the end of the protocol; VAC was never observed in 4/17 patients [23%], but occurred in six of the ten patients initially free, three standing at rest and three on exercise. For all patients, the VAC behavior remained of “nodal” type, indicated by a progressive increase in VACT as pacing rate rose up to the VAC-W point. Neither the existence of exercise-induced VAC nor the maximal VACT-W could be predicted from AVC or VAC data obtained at rest. However, at the same pacing rates, standing up and exercise induced a shortening effect on VACT, and improved the VAC-W by an average of 33%. These results suggest that the electrophysiological behavior of VAC does not obey any general rule and cannot be predicted individually. It would thus appear unwise to base pacemaker mediated tachycardia (PMT) protection solely on long postventricular atrial refractory period (PVARP) programming in DDD patients. This work also revealed the potential risks of a rate responsive auto-adaptive PVARP algorithm as proposed in certain new devices.  相似文献

Prevention of Atrial Arrhythmias during DDD Pacing by Atrial Overdrive   总被引:11,自引:0,他引:11  
We evaluated the effect of atrial overdrive on the incidence of atrial arrhythmias (AA) in 22 patients (67 ± 9 years. 7 women, 15 men) with Chorus 6234 DDD pacemakers. Atrial overdrive was defined as a programmed paced rate 10 ppm faster than the mean ventricular rate stored for the last 24-hour period in the pacemaker memory. The protocol consisted of three phases of 1 month each. Phase I: observation after discontinuation of antiarrhythmic therapy. Phase II: arrhythmia analysis using the pacemaker memory after programming the lower rate to 55 ppm. The fallback function and histogram data were used to document the number and maximal duration of AA episodes as well as the total AA time in a month. Phase III: atrial overdrive. The mean ventricular heart rate was 65 ± 4 beats/min before atrial overdrive versus 75 ± 5 with atrial overdrive (P = 0.02). At the end of phase II, all patients presented with AA episodes (mean number per patient: 42 ± 78 in one month). In phase III (with atrial overdrive), 14 (64.6%) patients had no recorded AA (group A). In the other eight patients with persistent AA episodes in phase III (group B), there was a significant reduction in the number of AA episodes (90 ± 106 in phase II vs 38 ± 87 in phase III; P = 0.01), their total duration (166 ± 115 in phase II vs 92 ± 134 hours in phase HI; P = 0.03) and their maximal duration (121 ± 103 in phase II vs 85 ± 89 min; P = 0.04). Our short-term data suggest tliat atrial overdrive prevents or reduces A A episodes and demonstrate the feasibility and need of long-term studies to determine whether this benefit is sustained.  相似文献

Al though differences in exercise performance have been observed between different rate adaptive modes, the relative impact of atrioventricular (AV) synchrony and rate adaptation on quality of life (QOL) have not been determined. Thirty-three patients with either sinoatrial disease (18) or complete atrioventricular (AV) block (15) received DDDR pacemakers (16 minute ventilation sensing, 17 activity sensing). There were 11 males and 22 females, with a mean age of 66 ± 1 (range 39–78) years. The study was a double-blind, triple cross-over study comparing DDDR, DDD, and VVIR modes. At the end of each 8-week study period in each mode, QOL was assessed by a questionnaire evaluating patients' functional class (Classes I-IV), physical malaise inventory (41 items), illness perception (43 items), and overall QOL rating based on a 48 items measure covering different aspects of the patients' daily life adjustment. Two patients required early crossover from VVIR mode during the study. Patients experienced significantly fewer physi cal malaise such as temperature intolerance, dyspnea, and palpitations in the DDDR mode, compared with either DDD or VVIR pacing. DDDR pacing reduced the perception of illness in 5 of 43 items compared to VVIR pacing, and improved stamina and appetite compared to DDD pacing. The overall QOL score was 102 ± 2, 105 ± 2, 113 ± 2 in the DDDR, DDD, and VVIR modes, respectively, with a higher score indicating a poorer QOL (DDDR/DDD vs VVIR, P < 0.02). There was no change in functional classes between the three pacing modes. In conclusion, VVIR pacing has a lower QOL compared with DDD pacing, which can be further enhanced with rate augmentation.  相似文献

The importance of atrial stimulation is emphasized especially in the context of the natural history of SND. A review of electrocardiograms from 85 patients with atrial-triggered ventricular pacing (VAT, VDD, DDD) showed that various disturbances of rhythm were relatively common, and that the effects of an arrhythmia could be aggravated by this type of pacing. In certain circumstances even potentially dangerous ventricular extrasystoles were induced by the pacemaker. Abnormal triggering, sometimes with regular sinus rhythm, could also induce tachycardia. Our observations provided indications for a pacemaker design that would avoid such disturbing effects. The best available pacer for atrial-triggered ventricular pacing is the programmable DDD type.  相似文献

JUTZY, R.V., ET AL.: Comparative Evaluation of Rate Modulated Dual Chamber and VVIR Pacing. While dual chamber pacing is considered superior to VVI pacing at rest, there is a continuing debate as to the relative benefit of AV synchrony versus rate increase with exercise. To evaluate this question and to correlate different methods of evaluation, 14 patients with DDDR pacemakers were studied using serial treadmill exercise test with a CAEP protocol. Patients were exercised in DDD, DDDR, and VVIR modes. Echo-Doppler cardiac outputs were determined and pulmonary gas exchange was measured during exercise. There was a significant improvement in cardiac output with exercise in the DDDR versus VVIR modes, and in DDDR versus DDD modes in patients with chronotropic incompetence. There were small increases in exercise duration in DDDR versus VVIR modes, and small but consistent increases in VO, at all levels of exercise, though not statistically significant. In this group of patients, DDDR pacing was superior to VVIR pacing, and superior to DDD pacing when chronotropic incompetence was present.  相似文献

DDD pacemakers may have large cycle-to-cycle variations in rate at the upper rate limit because of 2:1 block or Wenckebach-type block. Rate smoothing was introduced as an option to eliminate these large variations. Now, DDDR pacemakers can produce similar electrocardiographic displays through a different mechanism that uses an activity sensor. This is termed "sensor-driven rate smoothing" because it occurs only when the activity sensor is driving the pacemaker. In the case described, as the atrial rate exceeded the maximum tracking rate and reverted to Wenckebach-type block, the RR interval varied only from 600 msec to 680 msec (13.3% rate-smoothing value) because of sensor-driven pacing. Maximal sensor-driven rate smoothing requires optimal programming of the rate response indicators. This sensor-driven rate-smoothing effect is an electrocardiographic manifestation that will undoubtedly be seen more frequently as DDDR devices come into widespread clinical use.  相似文献

We compared the clinical course of patients paced in VVIR versus DDDR mode to determine the most appropriate method of pacing following cardiac transplantation. Pacemaker implantation was required in 9 of 90 orthotopic cardiac transplants (10%). Indications included sinus bradycardia or sinus arrest (8 patients) and AV node dysfunction (1 patient). VVIR pacemakers were implanted in four patients and DDDR in five patients. DDDR patients : The mean P wave was 1.7 mV and the mean atrial stimulation threshold was 0.8 V (at 0.5 msec). During follow-up of 20 months, two atrial lead complications developed (29% of leads in 33% of patients). No lead complications were directly related to endomyocardial biopsy. VVIR patients : All four patients developed VA conduction with mean VA time 180 msec (160–240 msec). Two patients developed pacemaker syndrome. Conclusions : VA conduction and pacemaker syndrome may develop in cardiac transplant recipients paced in the VVIR mode. Dual chamber pacing is technically feasible and preferable following cardiac transplantation.  相似文献

Atrial standstill (atrial paralysis) is a rare reason for permanent bradycardia. A case of atrial standstill is presented. A 35-year-old man had suffered from bradycardia since his childhood. For 2 years he had complaints (diminishment of his working capacity, and dyspnea occurred with effort) as well. On admission, a slow (38/min) junctional escape rhythm could be detected. There were no signs of atrial mechanical activity (atrial contraction) according to chest x ray, echocardiography, and the atrial pressure curve. The electrophysiological study revealed that the atria could not be electrically stimulated, and no P wave (A wave) could be recorded on right atrial electrograms. The patient received a rate responsive pacemaker. After pacemaker implantation, he became symptom-free; his working capacity improved markedly and his heart size decreased. Owing to the permanent bradycardia and the lack of atrial stimulation, the atrial standstill represents an indication for ventricular rate responsive pacing. Atrial standstill, permanent bradycardia, and the inability to stimulate the atrium are indications for ventricular rate responsive pacing.  相似文献

Dual chamber pacemakers were implanted in nine patients with permanent second or third degree AV block feight had complete retrograde block). Two identical exercise tests were performed after at least 1 month after implantation. During the first test (T1) the pacemaker was programmed to the DDD mode and heart rates were recorded every 15 to 30 seconds during exercise and 30 minutes after exercise. Following 30 minutes of rest, the implanted pacemaker was programmed to the VVT mode and driven by an external pacemaker via a skin electrode. The second exercise test (T2) was then performed and the rate of the external pacemaker was progressively changed to reproduce exactly the rate observed during T1 at the same exercise stress. Atrial natriuretic factor (ANF) levels were determined at rest, at regular intervals during exercise, and 30 minutes after exercise. ANF levels and release were statistically higher during rate matched ventricular, than DDD pacing. It is concluded that preservation of AV synchrony reduces ANF release induced by heart rate acceleration during exercise.  相似文献

A retrospective study of 252 patients who received a DDD pacemaker between October 1982 and December 1990 was performed. During a mean follow-up of 30 months, reprogramming to the VVI mode was necessary in 39 patients (15.5%). Technical problems causing downgrading occurred 15 times, of which 13 problems became permanent. A total number of 24 patients had sustained atrial arrhythmias, including 14 with atrial fibrillation and 10 with atrial flutter. In this group, conversion to sinus rhythm could be obtained in 38%. After 2 years, reliable DDD pacing was maintained in 86% of the surviving patients. The survival after 1 and 2 years was 94% and 89%, respectively, and was not influenced by arrhythmias or technical problems. We conclude that atrial arrhythmias including flutter are the most important reasons for reprogramming to the VVI mode, although in an important number of patients, predominantly those with flutter, restoration of AV synchrony can be obtained. The high number of patients with atrial flutter could imply some role for DDD devices offering the option of antitachycardia pacing. Reprogramming of the pacing mode did not influence mortality.  相似文献

A recent study of de Jongste has demonstrated the lengthening of short R-R intervals in patients with atrial fibrillation by right ventricular pacing. We have further analyzed the data from this study and specifically looked at the effect of right ventricular pacing on the R-R interval instability and heart rate. At the cost of only a slight increase in mean heart rate, a major reduction of the R-R interval instability can be obtained by right ventricular pacing. Based on these findings, we have developed and evaluated an automatic pacing rate algorithm, which continuously varies the stimulation rate in order to stabilize the otherwise irregular rhythm in patients with atrial fibrillation.  相似文献

Mode switching algorithms are commonly used to protect the ventricles against high rates induced by atrial tachycardia. In the case of atrial fibrillation (AF), the response of these algorithms depends on the quality of atrial sensing. The Chorum 7234 DDDR pacemaker uses a new mode switching algorithm, based on a statistical analysis of the atrial rhythm. It includes two criteria of diagnosis: "high" if more than 28 of 32 cycles are abnormally accelerated; and "low" if more than 36 of 64 cycles are abnormally accelerated. Methods: From a taped database of electrophysiological studies, episodes of AF lasting more than 2 minutes were selected. A tape recorder replayed the atrial signals into an external Chorum device. Each episode was replayed eight times with a programmed atrial sensitivity increasing from 0.4 –2.0 mV. For each criterion of diagnosis and each programmed sensitivity, the percentage of atrial sensing, the time to switching, and the mean ventricular rate were measured. Ten episodes of AF from 10 patients (9 men and 1 woman; ages 62 ± 16 years) were included: 1.95 ± 0.97 mV and 196 ± 64 ms. The sensitivity of the algorithm to diagnose atrial tachycardia reached 100%, for an atrial sensitivity set between 0.4 and 1.0 mV. The mean percentages of atrial sensed events were 74%± 18% and 46%± 9% for the "high" and "low" criteria, respectively. The mean diagnostic times were 28 ± 26 seconds and 68 ± 27 seconds, respectively. Sensing of < 23% of AF events resulted in failure to diagnose the arrhythmias by both algorithms. In the event of diagnostic failure, the mean ventricular pacing rate was 79 ± 9 ppm. Conclusion: Up to an atrial sensitivity of 1 mV, 100% of AF episodes were diagnosed. The Chorum mode switching algorithms are 100% reliable if > 45% of the AF waves are sensed. In the event of switching failure, the ventricle is protected by an average rate remaining below 80 ppm. Rate Adaptive Atrial Pacing in the Bradycardia Tachycardia Syndrome   总被引:1,自引:0,他引:1  
In 42 patients (26 men, 16 women; mean age 69 ± 10 years), who were paced and medicated with antiarrhythmic drugs for the bradycardia tachycardia syndrome, chronotropic response and AV conduction with rapid atrial pacing during exercise were studied. Patients were included if they had no second- or third-degree AV block, no complete bundle branch or bifascicular block, and a PQ interval ≤ 240 ms during sinus rhythm at rest. The interval between the atrial spike and the following Q wave (SQ) was measured in the supine position at rest with an AAI pacing rate of 5 beats/min above the sinus rate (SQ-R+5), and at the end of exercise with 110 beats/min (SQ-E110). Bicycle ergometry was performed using the Chronotropic Assessment Exercise Protocol with the pacemakers being programmed to AAI with a fixed rate of 60 beats/min. Chronotropic incompetence was defined as peak exercise heart rate: (1) < 100 beats/min; (2) < 75% of the maximum predicted heart rate; or (3) the heart rate at half the maximum workload < 60 + 2 beats/min per mL O2/kg per minute (calculated O2 consumption). During exercise, one patient developed atrial fibrillation. Chronotropic incompetence was present in 71 % (29/41) of the patients according to definition 2, and in 76% (31/41) according to definition 1 or 3. Ten out of 41 patients (24%) exhibited a second-degree AV block with atrial pacing at 110 beats/min at the end of exercise. Only 9 out of the remaining 31 patients (29%) showed a physiological adaptation of the SQ-E110, and 21 patients (68%) exhibited a paradoxical increase of the SQ interval with rapid atrial pacing at the end of exercise as compared to the SQ-R+5. These observations indicate that the pacing system to be used in most patients paced and medicated for the bradycardia tachycardia syndrome should be dual chamber, and the option of rate adaptation should be considered.  相似文献

Dual-chamber, rate-modulated pacemakers have recently become available. These devices, whose rate response is determined by the combined input from the intrinsic atrial rate and the sensor-driven rate, have led to some unexpected upper rate behaviors. In the case presented, with the maximum sensor pacing rate at 150 ppm, multiple episodes of apparent P wave tracking occurred above the maximum P wave iracking rate of 100 ppm. This behavior is explained by P waves thai inhibit sensor-driven atrial output; inhibition may result in variable maximum tracking rates that are equal to the current sensor-driven rate. This effect appears to have minimal physiological consequence. Its importance lies in its recognition as normal DDDR function so that searches for pacemaker malfunction will not be made.  相似文献

A New Pacing Algorithm for Overdrive Suppression of Atrial Fibrillation   总被引:8,自引:0,他引:8  
Constant rapid pacing may suppress arrhythmias, but it is usually poorly tolerated in the long term. We report a pilot study of a new pacing algorithm for overdrive suppression of atrial premature complexes (APCs) and atrial fibrillation (AF), which prevents postextrasystolic pauses and varies the pacing rate in response to the frequency of APCs. The algorithm was tested in a multiple crossover study for 24 hours in dual chamber pacemakers implanted in 70 patients. Comparison was made on ambulatory recordings between the number of atrial arrhythmias commencing with the algorithm active and inactive. In all cases, the algorithm functioned as designed. No patient was aware of its operation, and no malignant arrhythmias were induced. The 36 recordings that showed atrial arrhythmia were included for analysis. The effects of the algorithm were: APCs (estimated from pacemaker statistics) reduced in 18 patients, increased in 8 (P = 0.02); atrial salvos reduced in 12, increased in 4 (P = 0.041); and AF reduced in 11, increased in 8 (P = NS). In all patients with frequent AF (> 5 episodes in total), fewer episodes occurred when the algorithm was active. We conclude that the algorithm is safe and well tolerated, reduces atrial ectopic activity, and may reduce the frequency of sustained atrial fibrillation.  相似文献
