Background

Mixed dentition space analysis methods using regression equations, namely, Moyers’ analysis and Tanaka-Johnston analysis are commonly used around the world. However, the applicability of these analyses among different racial groups have been questioned. The primary objective of this study was to assess the applicability of the Moyers’ and Tanaka-Johnston analyses among Nepalese Mongoloids and to develop regression equations for the same population if needed.

Methods

One hundred (50 males and 50 females) pre-treatment study models of the Nepalese Mongoloid patients undergoing orthodontic treatment were retrieved from the archives of the department of Orthodontics. The mesiodistal widths of mandibular incisors and widths of canines and premolars of all 4 quadrants were measured by a single investigator using a digital caliper to the nearest 0.01?mm. Predicted widths of canines and premolars were obtained using standard Moyers’ and Tanaka-Johnston analyses and then compared with the measured widths.

Results

The measured widths of canines and premolars were significantly different from the predicted widths obtained from Moyers’ and Tanaka-Johnston analyses. Strong and positive correlations were found between the sum of mesiodistal widths of mandibular incisors and the sum of mesiodistal widths of canines and premolars in males (0.73 for maxillary arch and 0.68 for mandibular arch) and females (0.64 for maxillary arch and 0.79 for mandibular arch).

Conclusions

The Moyers’ and Tanaka-Johnston analyses did not accurately predict the mesiodistal width of unerupted canines and premolars for Nepalese Mongoloid population. Hence, new regression equations have been developed for this population. However, validation studies should be conducted to confirm the applicability and accuracy of these equations.

’ (dot), which denote an implant and a single standing implant, respectively, are utilized to construct the Panoramic Implant Notation System (PIN System), which allows graphical representation of implants and implant-supported prostheses. The use of the PIN System is described by means of its application to common dental implant situations. While the Universal Numbering System is applied principally, examples of use of FDI Two-Digit Notation and Palmer Notation are also illustrated.ConclusionBy employing the PIN System, implant positions as well as information pertaining to the type and extent of implant prosthodontics can be easily visualized, and, therefore, practically described, which leads to the avoidance of misunderstanding and ambiguity in communication.  相似文献

Objectives

The purpose of this study was to estimate the fatigue life of premolars restored with two dental ceramics, lithium disilicate (LD) and polymer infiltrated ceramic (PIC) using the numerical method and compare it with the published in vitro data.

Methods

A premolar restored with full-coverage crown was digitized. The volumetric shape of tooth tissues and crowns were created in Mimics®. They were transferred to IA-FEMesh for mesh generation and the model was analyzed with Abaqus. By combining the stress distribution results with fatigue stress–life (S–N) approach, the lifetime of restored premolars was predicted.

Results

The predicted lifetime was 1,231,318 cycles for LD with fatigue load of 1400 N, while the one for PIC was 475,063 cycles with the load of 870 N. The peak value of maximum principal stress occurred at the contact area (LD: 172 MPa and PIC: 96 MPa) and central fossa (LD: 100 MPa and PIC: 64 MPa) for both ceramics which were the most seen failure areas in the experiment. In the adhesive layer, the maximum shear stress was observed at the shoulder area (LD: 53.6 MPa and PIC: 29 MPa).

Significance

The fatigue life and failure modes of all-ceramic crown determined by the numerical method seem to correlate well with the previous experimental study.  相似文献

Purpose

The aim of this study was to investigate and quantify the effect of preparation design parameters on a premolar restored with two different CAD/CAM ceramic crowns by three-dimensional finite element analysis (FEA).

Methods

A restored human first premolar was digitized by a micro-CT scanner and a 3D model was created by a medical image processing software (Mimics). Following segmentation, dentine and ceramic were extracted by a surface meshing software (3-matic). Models with different preparation designs with three convergence angles (6°, 12° and 20°) and two preparation heights (3.1 mm and 4.1 mm) were produced. Mesh generation for models was performed in IA-FEMesh software with a lithium disilicate glass ceramic (LD, E = 95.9 GPa) and a polymer-infiltrated ceramic (PIC, E = 30.1 GPa) as the restorative materials. A 5-mm diameter stainless steel hemisphere was employed as an indenter. Twelve models were analyzed numerically in Abaqus?.

Results

The results indicated that preparation height was found to be a major factor affecting stress distribution in different components. In all models, the maximum principal stress of the ceramic crowns was found in contact area against the indenter. This stress was lesser in the longer abutment than the shorter one and it was greater for LD ceramic. Convergence angle had limited effect on stress distribution of ceramic crown in all models.

Conclusions

The preparation height appeared to play a more important role in the stress distribution of ceramic crown than the convergence angle.  相似文献

Purpose

The purpose of this study was to examine whether the status of dentition is associated with incident functional disability in elderly people.

Methods

This prospective cohort study targeted community-dwelling Japanese adults of age ≥70?years (n = 838). Participants were classified into the following four groups in accordance with Miyachi’s Triangular Classification, which represents the status of dentition on the basis of numbers of remaining teeth and occlusal supports: Zone A, ≥10 occlusal supports; Zone B, 5–9 occlusal supports; Zone D, ≤4 occlusal supports and?≥11 remaining teeth and Zone C, ≤10 remaining teeth. Incident functional disability was defined by the first certification of long-term care insurance in Japan. Data regarding age, sex, body mass index, medical history, smoking, alcohol consumption, education, depressive symptoms, cognitive impairment, social support, history of fall, and subjective masticatory ability were collected.

Results

During follow-up for 5185 person-years, 305 participants experienced functional disability. Considering the follow-up data of? ≥3?years from baseline, participants in Zones C (hazard ratio [HR], 1.98; 95 % confidence interval [CI], 1.26–3.11) and D (HR, 2.50; 95 %CI, 1.54–4.05) were found to be more likely to develop functional disability than those in Zone A (p for trend = 0.002).

Conclusions

Status of dentition was associated with incident functional disability in an elderly Japanese population. The findings of this study suggest that maintenance of remaining teeth and retention of occlusal supports contribute to the prevention of functional disability.  相似文献

Objective

The aim of this study was to evaluate long-term fatigue behavior using an in vitro step-stress accelerated life test (SSALT), and to determine the crack initiation point using in silico finite element analysis for computer-aided designed and manufactured (CAD/CAM) molar crowns fabricated from three commercial CAD/CAM resin composite blocks: Cerasmart (CS; GC, Tokyo, Japan), Katana Avencia Block (KA; Kuraray Noritake Dental, Niigata, Japan), and Shofu Block HC (HC; Shofu, Kyoto, Japan).

Methods

Fifty-one mandibular first molar crowns luted on a resin core die were embedded in acrylic resin and covered with a polyvinyl chloride tube. Single compressive tests were performed for five crowns. SSALT was conducted for 36 crowns using three profiles and reliabilities at 120,000 cycles, and a Weibull analysis was conducted. The maximum principal strain of each CAD/CAM resin composite crown model was analyzed by three-dimensional finite element analysis.

Results

Fracture loads of CS and KA (3784 ± 144 N and 3915 ± 313 N) were significantly greater than that of HC (2767 ± 227 N) (p < 0.05). Fracture probabilities at 120,000 cycles were 24.6% (CS), 13.7% (KA), and 14.0% (HC). Maximum principal strain was observed around the mesiolingual cusps of CS and KA and the distobuccal cusp of HC.

Significance

CAD/CAM resin composite molar crowns containing nano-fillers with a higher fraction of resin matrix exhibited higher fracture loads and greater longevity, suggesting that these crowns could be used as an alternative to ceramic crowns in terms of fatigue behavior.  相似文献

Objective

Elucidate the influence of debonding on stress distribution and maximum stresses for intra-radicular restorations.

Methods

Five intra-radicular restorations were analyzed by finite element analysis (FEA): MP = metallic cast post core; GP = glass fiber post core; PP = pre-fabricated metallic post core; RE = resin endocrowns; CE = single piece ceramic endocrown. Two cervical preparations were considered: no ferule (f0) and 2 mm ferule (f1). The simulation was conducted in three steps: (1) intact bonds at all contacts; (2) bond failure between crown and tooth; (3) bond failure among tooth, post and crown interfaces. Contact friction and separation between interfaces was modeled where bond failure occurred. Mohr–Coulomb stress ratios (σMC ratio) and fatigue safety factors (SF) for dentin structure were compared with published strength values, fatigue life, and fracture patterns of teeth with intra-radicular restorations.

Results

The σMC ratio showed no differences among models at first step. The second step increased σMC ratio at the ferule compared to step 1. At the third step, the σMC ratio and SF for f0 models were highly influenced by post material. CE and RE models had the highest values for σMC ratio and lower SF. MP had the lowest σMC ratio and higher SF. The f1 models showed no relevant differences among them at the third step.

Significance

FEA most closely predicted failure performance of intra-radicular posts when frictional contact was modeled. Results of analyses where all interfaces are assumed to be perfectly bonded should be considered with caution.  相似文献

Objective

The aim was to evaluate the effect of short glass-fiber/filler particles proportion on fracture toughness (FT) and flexural strength (FS) of an experimental flowable fiber-reinforced composite (Exp-SFRC) with two methacrylate resin formulations. In addition, we wanted to investigate how the fracture-behavior of composite restorations affected by FT values of SFRC-substructure.

Methods

Exp-SFRC was prepared by mixing 50 wt% of dimethacrylate based resin matrix (bisGMA or UDMA based) to 50 wt% of various weight fractions of glass-fiber/particulate filler (0:50, 10:40, 20:30, 30:20, 40:10, 50:0 wt%, respectively). FT and FS were determined for each experimental material following standards. Specimens (n = 8) were dry stored (37 °C for 2 days) before they were tested. Four groups of posterior composite crowns (n = 6) composed of different Exp-SFRCs as substructure and surface layer of commercial particulate filler composite were fabricated. Crowns were statically loaded until fracture. Failure modes were visually examined. The results were statistically analysed using ANOVA followed by post hoc Tukey’s test.

Results

ANOVA revealed that ratio of glass-fiber/particulate filler had significant effect (p < 0.05) on tested mechanical properties of the Exp-SFRC with both monomer systems. Exp-SFRC (50 wt%) had significantly higher FT (2.6 MPam1/2) and FS (175.5 MPa) (p < 0.05) compared to non-reinforced material (1.3 MPam1/2, 123 MPa). Failure mode analysis of crown restorations revealed that FT value of the substructure directly influenced the failure mode.

Significance

This study shows that short glass-fibers can significantly reinforce flowable composite resin and the FT value of SFRC-substructure has prior importance, as it influences the crack arresting mechanism.  相似文献

Objectives

To assess conceptual designs of dental posts consisting of polyetherimide (PEI) reinforced with carbon (C) and glass (G) glass fibers in endodontically treated anterior teeth.

Methods

3D tessellated CAD and geometric models of endodontically treated anterior teeth were generated from Micro-CT scan images. Model C-G/PEI composite posts with different Young’s moduli were analyzed by Finite Element (FE) methods post A (57.7 GPa), post B (31.6 GPa), post C (from 57.7 to 9.0 GPa in the coronal–apical direction). A load of 50 N was applied at 45° to the longitudinal axis of the tooth, acting on the palatal surface of the crown. The maximum principal stress distribution was determined along the post and at the interface between the post and the surrounding structure.

Results

Post C, with Young’s modulus decreasing from 57.7 to 9.0 GPa in the coronal–apical direction, reduced the maximum principal stress distribution in the restored tooth. Post C gave reduced stress and the most uniform stress distribution with no stress concentration, compared to the other C-G/PEI composite posts.

Significance

The FE analysis confirmed the ability of the functionally graded post to dissipate stress from the coronal to the apical end. Hence actual (physical) C-G/PEI posts could permit optimization of stress distributions in endodontically treated anterior teeth.  相似文献

Purpose

This study analyzed risk factors for post-loading implant loss in cases of implant-supported prostheses applied to edentulous jaws of Japanese patients.

Methods

In total, 245 dental implant fixtures placed in 54 edentulous jaws of 46 patients performed at Niigata University Hospital were retrospectively analyzed. Kaplan–Meier curves were used to estimate the cumulative survival rate (SR) of implants, and multiple Cox regression analysis was used to identify predictive factors of implant loss. The following risk factors for implant failure were examined: age, sex, survival time, implant length, implant location, smoking habit, bone density, bone augmentation, opposing dentition, loading period, and type of final restoration. The Cochran–Mantel–Haenszel test was used to examine difference in survival curves of the extracted predictors.

Results

Sixteen implants failed during the observation period (SR = 92.8 %). Multiple Cox regression analysis revealed that male sex [hazard ratio (HR) = 16.1; p = 0.007] and use of maxillary removable restorations (HR = 12.7; p < 0.000) were risk factors for implant failure. Other factors had no significant effect on implant failure. The SR of implants for males (SR = 86.9 %) was significantly lower than that for females (SR = 99.1 %). The SR of implants for maxillary removable restorations (SR = 76.4 %) was significantly lower than for maxillary fixed restorations (SR = 99.1 %) and mandibular fixed restorations (SR = 97.8 %).

Conclusions

Maxillary implants with removable restorations and male sex were risk factors for implant failure among Japanese edentulous patients.  相似文献

Aim

To assess the influence of the crown height, root length, crown-to-root ratio, and tooth type on the survival of teeth subjected to surgical endodontic retreatment and classified as periapically healed.

Methodology

A single operator performed endodontic microsurgery interventions between 2008 and 2018 on teeth with refractory apical periodontitis. The present analysis selected the teeth classified as “complete periapical healing” according to the scale suggested by Molven. The postoperative periapical radiographs and those taken at the last recall visit were analysed by two independent calibrated examiners, who measured crown height and root length in a blind manner. The crown-to-root ratio was calculated as the ratio of the two variables. The level of inter- and intra-operator agreement was tested with Bland–Altman plots with 95% limits of agreement. An independent statistician conducted a survival analysis using Kaplan–Meier plots and a log-rank test (α = 0.05) to assess the significance of the differences among the subgroups defined by the following criteria: (a) crown height <median vs. >median; (b) root length <median vs. >median; (c) crown-to-root ratio <1 vs. >1; (d) crown-to-root ratio <median vs. >median; (e) single-rooted teeth vs. multi-rooted teeth.

Results

At the end of the analysis, 42 patients were evaluated, each one contributing to the study with a single tooth. The mean follow-up period was 4.2 ± 2.4 years. Survival estimates were significantly improved for the teeth with roots longer than 8 mm, in comparison with that with shorter roots (p < 0.05). There were no statistically significant differences among the remaining considered subgroups.

Conclusions

Under the conditions of this retrospective study, teeth with longer residual roots after apical surgery exhibited better chances of survival when compared to teeth with roots shorter than 8 mm. The other considered variables did not seem to affect the survival of apically resected teeth.  相似文献

Objective

To assess fluoride concentration in drinking water which include tap water of 4 emirates - Abu Dhabi, Dubai, Sharjah and Ajman plus bottled water, commonly available soft drinks & juices in United Arab Emirates.

Methods

Five different samples of tap water collected from each of the four emirates of UAE: Ajman, Sharjah, Abu Dhabi and Dubai; twenty-two brands of bottled water and fifteen brands of popular cold beverages, purchased from different supermarkets in U.A.E were tested using ion selective electrode method and the fluoride concentration was determined.

Results

The mean fluoride content of tap water samples was 0.14 mg F/L with a range of 0.04–0.3 mg F/L; with Ajman tap water samples showing the highest mean fluoride content of 0.3 mg F/L. The mean fluoride content for both bottled drinking water and beverages was 0.07 mg F/L with a range of 0.02–0.50 mg F/L and 0.04–0.1 mg F/L respectively. Majority (68.2%) of the bottled water are produced locally within U.A.E while a few (31.8%) are imported.

Conclusions

The tap water, bottled water and beverages available in U.A.E show varying concentrations of fluoride, however none showed the optimal level necessary to prevent dental caries. Dental professionals in U.A.E should be aware of the fluoride concentrations before prescribing fluoride supplements to children.  相似文献

Objective

To determine oral health comprehension among parents of cerebral palsy (CP) children.

Methods

A self-administered questionnaire was utilized to obtain the required information. The study was conducted in two main centers for disabled children in Riyadh, Saudi Arabia.

Results

Parents of all 157 CP children registered in the two centers completed the questionnaire. Mothers mostly (86.6%) completed the questionnaire. Majority (98.7%) of the parents knew the importance of dental health for general health. More than two-third (70%) of the parents thought that teeth should be brushed thrice daily or after each meal. About three in every ten (29.9%) parents were not aware of the beneficial effect of fluoride in preventing dental caries; and very few (9.6%) were aware of water as a source of fluoride. Almost all (98.7%) the parents knew that sugary foods caused dental caries. Three-fourth (75.8%) of the parents were not aware of the possible harmful effects of bottled juices on teeth. There were no significant (p > 0.05) associations between the parental age/gender with any of the dependent variables.

Conclusion

Parents of CP children generally showed satisfactory oral health comprehension. However, they need further oral health education in several areas.  相似文献
