目的 研究《国际功能、残疾和健康分类》(ICF)在作业治疗中的理论架构、方法体系及其在临床实践中的系统应用。方法 基于ICF理论,结合世界作业治疗师联合会的相关政策文件,分析作业治疗发展的理论架构,探讨基于ICF框架下临床情境下系统化的作业治疗。结果 基于ICF构建作业治疗的理论架构与方法体系,明确现代康复服务中综合作业治疗干预的目标、原则与应用方法。作业治疗干预目标是通过多学科、跨领域地应用综合作业治疗方案,提高个体的活动和参与,使功能最佳化。作业治疗基于生物-心理-社会模式,坚持以人为本和功能定向的原则,在不同情景下实施个性化的干预方案。在临床实践中,建议系统应用世界卫生组织国际健康分类家族,构建系统化的作业治疗服务体系,即功能与需求分析(ICHI)-功能分类、评估及编码(ICF)-疾病分类、诊断及编码(ICD)-作业治疗干预(ICHI)。结论 构建了基于ICF的作业治疗理论架构与方法体系,建立了综合性的临床作业治疗应用模式,明确了以活动和参与为导向的作业治疗干预目标,提高了作业治疗的系统化、结构化、标准化及精细化水平。  相似文献

If we aim towards a comprehensive understanding of human functioning and the development of comprehensive programs to optimize functioning of individuals and populations we need to develop suitable measures. The approval of the International Classification, Disability and Health (ICF) in 2001 by the 54th World Health Assembly as the first universally shared model and classification of functioning, disability and health marks, therefore an important step in the development of measurement instruments and ultimately for our understanding of functioning, disability and health. The acceptance and use of the ICF as a reference framework and classification has been facilitated by its development in a worldwide, comprehensive consensus process and the increasing evidence regarding its validity. However, the broad acceptance and use of the ICF as a reference framework and classification will also depend on the resolution of conceptual and methodological challenges relevant for the classification and measurement of functioning. This paper therefore describes first how the ICF categories can serve as building blocks for the measurement of functioning and then the current state of the development of ICF based practical tools and international standards such as the ICF Core Sets. Finally it illustrates how to map the world of measures to the ICF and vice versa and the methodological principles relevant for the transformation of information obtained with a clinical test or a patient-oriented instrument to the ICF as well as the development of ICF-based clinical and self-reported measurement instruments.  相似文献

Abstract

Background: Vocational rehabilitation is implemented to ensure and sustain return to work. Different healthcare professionals are involved in the process of vocational rehabilitation and physical therapists are in the forefront of providing vocational rehabilitation services. A comprehensive conceptual framework is required, so appropriate and adequate vocational rehabilitation can be provided by physical therapists. The international classification of functioning, disability, and health (ICF) by the World Health Organization can provide such a framework.

Objective: This paper will discuss recent developments in the field where the ICF has been used in vocational rehabilitation, how such usage will benefit physical therapy practice, and priority areas that need to be addressed in the near future.

Discussion: The ICF has been used in multiple ways in vocational rehabilitation in which physical therapists can play a major role. To better understand and measure work disability, the ICF can be used as a basis for a conceptual definition of vocational rehabilitation and as a tool to develop or select functioning domains and measurement instruments relevant to vocational rehabilitation.

Conclusion: The ICF is a viable option for physical therapists to understand and facilitate the process of vocational rehabilitation towards better return to work outcomes. Physical therapists, as one of the primary health professionals engaged in vocational rehabilitation, will benefit from using a reference framework like the ICF in their clinical practice and research.  相似文献

PURPOSE: This paper describes the conceptual foundation and systematic framework of the International Classification of Functioning, Disability, and Health (ICF) as a mechanism for understanding the course and consequences of various health related states. The specific application of the ICF with persons with cognitive dysfunction is also presented. METHOD: A comprehensive literature review related to the conceptualization of the ICF, its classification scheme and coding process is presented. Information on cognitive disorders including prevalence, functional manifestations and the assessment of a person's cognitive functioning and the applicability of the ICF's holistic classification and coding of cognitive dysfunction within the components of body structure and function, activity and participation, and environmental attributes is also reviewed. CONCLUSION: The ICF has the potential to classify and interpret cognitive deficits on a global level and thereby reflects upon the overall health and functioning of the individual in major life activities. The coding system systematically organizes measures related to the person's cognitive status and the resulting functional outcomes. The ICF focuses on individuals' performance of activities in all aspects of life and validates the independence and well-being of persons with disabilities making it an important instrument to be used by rehabilitation professionals.  相似文献

ICF Core Sets are sets of categories out of the International Classification of Functioning, Disability and Health (ICF) which can serve as minimal standards for the assessment, communication and reporting of functioning and health for clinical studies, clinical encounters and multi-professional comprehensive assessment and management. The ICF Core Sets were developed in a formal decision-making and consensus process, integrating evidence gathered from preliminary studies and expert opinion. The Acute ICF Core Sets for patients with neurological, musculoskeletal and cardiopulmonary conditions are intended for use by physicians, nurses, therapists and other health professionals working in the acute hospital on medical, surgical or other units not specialised in rehabilitation. The Post-acute ICF Core Sets for geriatric patients and patients with neurological, musculoskeletal or cardiopulmonary conditions are intended for use by physicians, nurses, therapists and other health professionals involved in early post-acute rehabilitation. The Acute and Post-acute ICF Core Sets are first versions and need to be tested and validated in the patient and professional perspective and in different countries, regions, health care and provider settings.  相似文献

On June 9th 2011 the WHO World Report on Disability(WRD), called for by the World Health Assembly (WHA),was launched at the United Nations headquarters in NewYork. The WRD displays what has come to be known as the integrative model of functioning and disability as expressed in the International Classification of Functioning, Disability and Health (ICF). The present paper summarizes the representation of the role of rehabilitation in the WRD. It in particular highlights implications, perspectives and opportunities for Physical and Rehabilitation Medicine (PRM) and the International Society of Physical and Rehabilitation Medicine(ISPRM). The WRD acknowledges the genuine role of PRM and its contribution to enhancing a person’s functioning and participation in life. Challenges lie in the delivery of rehabilitation services in underserved parts of the world,ranging from the provision of timely, cost efficient and effective treatment, and the involvement of people with disability,family and care givers in the decision making process.In the present paper it is concluded that these challenges and the implementation of the WRD’s recommendations call upon multiple actors including ISPRM and for national rehabilitation strategies that can coordinate scarce resources effectively, especially in times of crisis such as disaster relief efforts.  相似文献

Rehabilitation medicine may be defined as the multi- and interdisciplinary management of a person's functioning and health. Rehabilitation medicine defines itself with respect to concepts of functioning, disability and health. Assessment and intervention management rely on this concept. The current framework of disability--the WHO International Classification of Functioning, Disability and Health (ICF)--providing a coherent view of health from a biological, individual and social perspective. However, ICF success will depend on its compatibility with measures used in rehabilitation and on the improvement of its practicability. Thus, it is expected to see the development of the ICF based on versions of currently used instruments and on the development of ICF Core Sets. The new language ICF is an exciting landmark event for rehabilitation. Key to building research capacity is the development of a qualified workforce. This requires the establishment of academic training programs and the creation of attractive research career opportunities. The adoption of the International Classification of Functioning, Disability and Health as unifying conceptual model for rehabilitation, the emergence of distinct scientific fields of human functioning and rehabilitation research, and the change to Bachelor and Master degrees in Europe provide opportunities to initiate academic training programs in human functioning and rehabilitation research. Applied training includes certificate programs in rehabilitation effectiveness and Masters and Doctoral programs in rehabilitation with specializations, e.g. in rehabilitation studies, management, education and rehabilitation counseling. Scientifically-oriented training includes Masters of Science and PhD programs in human functioning sciences and integrative rehabilitation sciences. There is also potential for collaborative Masters and Doctoral programs with the rehabilitation professions, movement sciences, psychology and the behavioral and social sciences. When initiating the process to develop these programs, one may learn from and co-operate with established programs in public health.  相似文献
