BACKGROUND AND PURPOSE: Although digital subtraction angiography (DSA) provides excellent visualization of the intracranial vasculature, it has several limitations. Our purpose was to evaluate the ability of helical CT angiography (CTA) to help detect and quantify intracranial stenosis and occlusion compared with DSA and MR angiography (MRA). METHODS: Twenty-eight patients underwent CTA, DSA, and 3D time-of-flight (TOF) MRA for suspected cerebrovascular lesions. All three studies were performed within a 30-day period. Two readers blinded to prior estimated or calculated stenoses, patient history and clinical information examined 672 vessel segments. Lesions were categorized as normal (0-9%), mild (10-29%), moderate (30-69%), severe (70-99%), or occluded (no flow detected). DSA was the reference standard. Unblinded consensus readings were obtained for all discrepancies. RESULTS: A total of 115 diseased vessel segments were identified. After consensus interpretation, CTA revealed higher sensitivity than that of MRA for intracranial stenosis (98% versus 70%, P < .001) and occlusion (100% versus 87%, P = .02). CTA had a higher positive predictive value than that of MRA for both stenosis (93% versus 65%, P < .001) and occlusion (100% versus 59%, P < .001). CTA had a high interoperator reliability. In 6 of 28 patients (21%), all 6 with low-flow states in the posterior circulation, CTA was superior to DSA in detection of vessel patency. CONCLUSION: CTA has a higher sensitivity and positive predictive value than MRA and is recommended over TOF MRA for detection of intracranial stenosis and occlusion. CTA has a high interoperator reliability. CTA is superior to DSA in the evaluation of posterior circulation steno-occlusive disease when slow flow is present. CTA results had a significant effect on patient clinical management.  相似文献

PURPOSE: The aim of the study was to compare gadolinium-enhanced MRA (Gd-MRA), Computed Tomography angiography (CTA) and digital subtraction angiography (DSA) in the evaluation of carotid stenosis. MATERIAL AND METHODS: Thirty patients with US-proven internal carotid artery stenosis over 50%, underwent Gd-MRA, CTA and DSA. Gd-MRA was acquired using the keyhole technique. RESULTS: The overall agreement was 90% for Gd-MRA and 93% for CTA. Gd-MRA and CTA overestimated 5 and 4 cases of mild stenosis and 1 case each of moderate stenosis, respectively. Gd-MRA better delineated ulcerated plaques, tandem lesions and pseudo-occlusions. DISCUSSION AND CONCLUSIONS: Because of the high speed acquisition breath-hold is no longer necessary for Gd-MRA and more delayed phases were analysed when necessary. The main limitation of Gd-MRA was represented by insufficient volume coverage that allowed the visualisation of supraaortic vessels from the arch only in 57% of the cases. Calcified plaques were very well detected by CTA and not by Gd-MRA. In our experience Gd-MRA represents the second step non invasive tool in patients with US-proven carotid stenosis. This leads to avoid conventional angiography in most cases.  相似文献

BACKGROUND AND PURPOSE:The cause of posterior reversible encephalopathy syndrome (PRES) is unknown. Two primary hypotheses exist: 1) hypertension exceeding auto-regulatory limits leading to forced hyper-perfusion and 2) vasoconstriction and hypo-perfusion leading to ischemia with resultant edema. The purpose of this study was to evaluate the catheter angiography (CA), MR angiography (MRA), and MR perfusion (MRP) features in PRES in order to render further insight into its mechanism of origin.MATERIALS AND METHODS:In 47 patients with PRES, 9 CAs and 43 MRAs were evaluated for evidence of vasculopathy (vasoconstriction and vasodilation), and 15 MRP studies were evaluated for altered relative cerebral blood volume (rCBV) in PRES lesions and regions. Visualization of vessels on MRA and toxicity blood pressures were compared with the extent of hemispheric vasogenic edema.RESULTS:Vasculopathy was present in 8 of 9 patients on CA (direct correlation to MRA in 3/6 patients). At MRA, moderate to severe vessel irregularity consistent with vasoconstriction and vasodilation was present in 30 of 43 patients and vessel pruning or irregularity in 7 patients, with follow-up MRA demonstrating reversal of vasoconstriction or vasodilation in 9 of 11 patients. Vasogenic edema was less in patients with hypertension compared with patients who were normotensive. Preserved normal length of the posterior cerebral artery (PCA) was commonly seen in patients with severe hypertension despite diffuse or focal vasoconstriction or vasodilation. In these patients, lengthier visualization of the distal PCA correlated with a lower grade of hemispheric edema (P = .002). Cortical rCBV was significantly reduced in 51 of 59 PRES lesions and regions compared with a healthy reference cortex (average 61% of reference cortex) with mild decrease in the remainder.CONCLUSION:Vasculopathy was a common finding on CA and MRA in our patients with PRES, and MRP demonstrated reduced cortical rCBV in PRES lesions. Vasogenic edema was reduced in patients with hypertension, and superior distal PCA visualization correlated with reduced hemispheric edema in patients with PRES and severe hypertension.

Neurotoxicity with development of posterior reversible encephalopathy syndrome (PRES) is commonly seen in association with cyclosporine and FK-506 immune suppression after transplantation (allogeneic bone marrow transplantation [allo-BMT], solid organ transplantation); preeclampsia and eclampsia; infection, sepsis, and shock; nonspecific medical renal disease; and in autoimmune conditions as well as after high-dose chemotherapy.1-13 The mechanism behind the development of PRES is yet unproved. Two broad theories have generally been considered.Severe hypertension with autoregulatory failure and hyperperfusion is often cited as the underlying mechanism. Alternatively, vasospasm has been demonstrated (catheter angiography [CA], MR angiography [MRA]), decreased cerebral blood flow noted (MR perfusion [MRP], single-photon emission CT [SPECT]) and the imaging appearance typically resembles a watershed distribution suggesting a mechanism related to brain hypoperfusion.1-3,5,9,13-20Given these opposing views, it was our opinion that parallel observations on CA, MRA, and MRP could render further insight into the state of brain perfusion in PRES. The accurate assessment of carotid artery disease is an important challenge for magnetic resonance (MR) angiography. Studies indicate that the detection and grading of stenosis and the evaluation of plaque morphology are all important steps in the clinical assessment of atherosclerosis. The prevalence of significant carotid artery stenosis in the elderly population and even in patients with symptoms of carotid artery disease is low; clinical risk seems to correlate more closely with plaque morphology and surface characterization than with the degree of stenosis. This highlights the importance of MR angiography and ultrasound, which can help characterize plaque morphology in addition to showing the degree of stenosis. The authors review the present understanding of plaque formation, comparisons of plaque imaging with conventional angiography, ultrasound, and MR angiography, and recent progress in MR angiography techniques. Several studies, including the North American Symptomatic Carotid Endarterectomy Trial and the European Carotid Surgery Trial, are discussed regarding the current objectives of carotid artery imaging. The sensitivity and specificity of plaque detection and morphologic evaluation continue to improve.  相似文献

PURPOSE: To compare three-dimensional (3D) time-of-flight (TOF) MR angiography, contrast-enhanced MR angiography, digital subtraction angiography (DSA), and rotational angiography for depiction of stenosis. MATERIALS AND METHODS: The study had Ethics Committee approval, and each patient gave written informed consent. Forty-nine patients (18 women, mean age, 67.2 years +/- 9.1 [+/- standard deviation], and 31 men, mean age, 63.1 years +/- 8.0) with symptomatic stenosis of internal carotid artery (ICA) diagnosed at duplex ultrasonography underwent transverse 3D TOF MR angiography with sliding interleaved kY acquisition and coronal contrast-enhanced MR angiography, followed by DSA and rotational angiography within 48 hours. MR angiography was performed at 1.5-T with a cervical coil. Contrast-enhanced MR angiograms were obtained after a bolus injection of 20 mL of gadobenate dimeglumine. Maximum ICA stenosis on maximum intensity projection and source images was quantified according to NASCET criteria. Correlations for 3D TOF MR angiography, contrast-enhanced MR angiography, DSA, and rotational angiography were determined by means of cross tabulation, and accuracy for detection and grading of stenoses were calculated. Data were evaluated with analysis of variance, Wilcoxon signed rank test, and McNemar test, all at significance of P < .05. RESULTS: Ninety-eight ICAs were evaluated at contrast-enhanced MR angiography, DSA, and rotational angiography, and 97 were evaluated at 3D TOF MR angiography. Correlations for contrast-enhanced MR angiography, 3D TOF MR angiography, and DSA relative to rotational angiography were r2 = 0.9332, r2 = 0.9048, and r2 = 0.9255, respectively. Lower correlation (r2 = 0.8593) was noted for contrast-enhanced MR angiography and DSA. Respective sensitivity and specificity for detection of hemodynamically relevant stenosis relative to rotational angiography were 100% and 90% for contrast-enhanced MR angiography, 95.5% and 87.2% for 3D TOF MR angiography, and 88.6% and 100% for DSA. Four of 31 severe stenoses were underestimated at DSA, and three were underestimated at contrast-enhanced MR angiography. Three severe stenoses were underestimated at 3D TOF MR angiography, and one was misclassified as occluded. Of 13 moderate (50%-69%) stenoses, one was overestimated at contrast-enhanced MR angiography, two were underestimated and three overestimated at 3D TOF MR angiography, and two were underestimated at DSA. CONCLUSION: DSA results in an underestimation of ICA stenosis compared with rotational angiography. Contrast-enhanced MR angiography correlates best with rotational angiography.  相似文献

The purpose of our experimental study was to assess the accuracy and precision of CT angiography (CTA), MR angiography (MRA) and rotational digital subtraction angiography (DSA) for measuring the volume of an in vitro aneurysm model. A rigid model of the anterior cerebral circulation harbouring an anterior communicating aneurysm was connected to a pulsatile circuit. It was studied using unenhanced 3D time-of-flight MRA, contrast-enhanced CTA and rotational DSA angiography. The source images were then postprocessed on dedicated workstations to calculate the volume of the aneurysm. CTA was more accurate than MRA (P=0.0019). Rotational DSA was more accurate than CTA, although the difference did not reach statistical significance (P=0.1605), and significantly more accurate than MRA (P<0.00001). CTA was more precise than MRA (P=0.12), although this did not reach statistical significance. Rotational DSA can be part of the diagnosis, treatment planning and support endovascular treatment of intracranial aneurysms. The emerging endovascular treatment techniques which consist of using liquid polymers as implants to exclude aneurysms from arterial circulation would certainly benefit from this precise measurement of the volume of aneurysms.  相似文献

Introduction

To prospectively compare of the diagnostic value of digital subtraction angiography (DSA) and time-of-flight magnetic resonance angiography (TOF-MRA) in the follow-up of intracranial aneurysms after endovascular treatment.

Methods

Seventy-two consecutive patients were examined 3?months after the embolization. The index tests included: two-dimensional DSA (2D-DSA), three-dimensional DSA (3D-DSA), and TOF-MRA. The reference test was a retrospective consensus between 2D-DSA images, 3D-DSA images, and source rotational DSA images. The evaluation included: detection of the residual flow, quantification of the flow, and validity of the decision regarding retreatment. Intraobserver agreement and interobserver agreement were determined.

Results

The sensitivity and specificity of residual flow detection ranged from 84.6?% (2D-DSA and TOF-MRA) to 92.3?% (3D-DSA) and from 91.3?% (TOF-MRA) to 97.8?% (3D-DSA), respectively. The accuracy of occlusion degree evaluation ranged from 0.78 (2D-DSA) to 0.92 (3D-DSA, Cohen??s kappa). The 2D-DSA method presented lower performance in the decision on retreatment than 3D-DSA (P?<?0.05, ROC analysis). The intraobserver agreement was very good for all techniques (???=?0.80?C0.97). The interobserver agreement was moderate for TOF-MRA and very good for 2D-DSA and 3D-DSA (???=?0.72?C0.94).

Conclusion

Considering the invasiveness of DSA and the minor difference in the diagnostic performance between 3D-DSA and TOF-MRA, the latter method should be the first-line modality for follow-up after aneurysm embolization.  相似文献

BACKGROUND AND PURPOSE: To assess chronological change in intracranial artery dissections with magnetic resonance imaging (MRI), MR angiography (MRA), and source images of MRA, and to determine whether the source images of MRA provide additional useful information to the combined evaluation of MRI and MRA. MATERIALS AND METHODS: Seven consecutive patients with intracranial artery dissections who were diagnosed by clinical history and conventional angiography were followed sequentially with MRI and MRA (mean follow-up duration, 8.8 months). Two observers independently reviewed the signal intensity of the arterial wall on T1-weighted images, luminal structures on MRA, and source images of MRA. RESULTS: In three (43%) of seven patients, the affected arterial wall had high signal intensity area from 4 to 62 days after onset on T-weighted images. Double lumen on MRA wasobserved only in one patient during the course of the study, whereas a definite low-intensity linear area in the lumen on source images of MRA was seen from 0 to 773 days after onset in all patients. When information from the source images of MRA was added to evaluation with both MRI and MRA, detectability increased to 100% from day 0 to day 3 and 67% from day 4 to day 30. CONCLUSION: The signal intensity of the dissected wall and the luminal structures on MRA and its source images vary according to chronological age. The use of source images from MRA in addition to the combined evaluation of MRI and MRA may provide more accurate diagnosis and follow-up study of intracranial artery dissections.  相似文献

RATIONALE AND OBJECTIVES: Cardiac magnetic resonance imaging (MRI) has been shown to be a robust and noninvasive method to assess left ventricular (LV) cardiac function. This study sought to assess volumes and mass calculated with MRI using fast techniques for acquisition and postprocessing, and to compare results in terms of cost-effectiveness with those of radionuclide angiography (RNA) or contrast angiography (CA). METHODS: Thirty-five patients and 15 healthy volunteers were studied. All patients underwent an MRI examination during the same period that they underwent ventriculography (26 patients) or radiography (25 patients). From 7 to 11 short-axis slices were acquired with a breath-hold fast-gradient echo-segmented sequence from apex to base. Contours were drawn with an automated border detection software. RESULTS: Ejection fraction (EF) correlated well between modalities (r = 0.77, P<0.001, for MRI and RNA; r = 0.72, P< 0.001, for MRI and CA). CONCLUSIONS: Cardiac MRI is a fast and accurate technique for estimation of LV volumes, EF, and mass.  相似文献

Summary 10 patients with an AVM of the brain were examined by magnetic resonance angiography (MRA), magnetic resonance imaging (MRI) and conventional cerebral angiography (CCA). From MRA in 7/10 patients important information about vascular supply could be provided; in 3 patients, all with small AVM's it could be only suspected. CCA revealed the vascularisation of the AVM's in all patients and showed additional hemodynamic aspects better than MRA. With MRI in all cases the AVM could be diagnosed, but only the involvement of the main cerebral vessels could be demonstrated; however, MRI is superior to MRA and CCA in showing the nidus and the involved brain structures.  相似文献

The value of angiography, sonography, and computed tomography as diagnostic methods for renal infarction is discussed. In all cases the infarct could be demonstrated angiographically, but not by sonography. In the case of a former infarct, the involved area appeared as a tumour mass by sonography and enhanced CT-scan showed contrast enhancement of the mass.  相似文献
