OBJECTIVE: To study photorefractive keratectomy (PRK) or laser in situ keratomileusis (LASIK) after clear lens extraction (CLE) with intraocular lens (IOL) implantation for hyperopia or astigmatism. DESIGN: Retrospective, noncomparative interventional case series. PARTICIPANTS: Sixty-five eyes (55 subjects) had CLE with posterior chamber IOL implants for hyperopia up to 12.25 diopters (D); 31 eyes were retreated with PRK, and 34 eyes were retreated with LASIK for residual ametropias. INTERVENTION: For PRK and LASIK, the refractive surgery was performed with the slit-scanning excimer laser Nidek EC-5000, Nidek Co., Tokyo, Japan. MAIN OUTCOME MEASURES: Manifest refraction, best-spectacle and uncorrected Snellen visual acuity, haze, and halos were evaluated before surgery and at 1, 3, 6, and 12 months postoperative. RESULTS: Forty-seven eyes were evaluated at the 12-month postoperative examination: 96% of these eyes had spherical equivalents (SE) within +/-2 D of emmetropia, 79% of eyes had SE within +/-1 D of emmetropia and 51% of eyes had SE within +/-0.50 D of emmetropia. Eighty-five percent of the eyes at 12 months postoperative had uncorrected visual acuity of 20/40 or better, and 46% of eyes had uncorrected visual acuity of 20/20 or better. Eighty-seven percent of the eyes at 12 months postoperative had uncorrected visual acuity within 1 Snellen line of their initial best spectacle-corrected visual acuity (BSCVA) before all treatment. No eye lost 2 Snellen lines of BSCVA at 3, 6, or 12 months after PRK or after LASIK. CONCLUSIONS: IOL implantation for CLE, although an invasive technique, resulted in better refractive outcomes without laser-related clinical complications after PRK or LASIK adjustment.  相似文献

OBJECTIVE: Photorefractive keratectomy (PRK) and laser in situ keratomileusis (LASIK) outcomes were compared at 1, 3, 6, and 12 months after surgery. DESIGN: Retrospective, nonrandomized, comparative study. PARTICIPANTS: One hundred seven LASIK-treated eyes (58 patients) and 107 PRK-treated eyes (91 patients) having myopia between -1 and -9.50 diopters (D). All LASIK-treated eyes were analyzed retrospectively and matched with PRK-treated eyes having sphere within +/-0.25 D, +/-1 D of cylinder, and +/-7 years of age. INTERVENTION: For PRK and LASIK, the refractive surgery was performed with the slit-scanning excimer laser Nidek EC-5000, (Nidek Co. Tokyo, Japan) MAIN OUTCOME MEASURES: Manifest refraction, best-spectacle and uncorrected Snellen visual acuity, haze, halos, and keratometry were evaluated before surgery and up to 12 months after surgery. RESULTS: Seventy percent of eyes were evaluated at the 12-month postoperative exam. Of these eyes, 83% of LASIK cases and 86% of PRK cases had uncorrected visual acuities of 20/20 or better. Refractions within +/-0.5 D represented 78% of the LASIK eyes and 83% of the PRK eyes at that follow-up. Patients who underwent LASIK reported halos twice as often as patients who underwent PRK using a subjective scale. The odds ratio of high halos for LASIK versus PRK was 3.50 (95% confidence interval, 1.89-6.46; P<0.0001). At 1 month after surgery, 64% of the LASIK eyes were within +/-0.50 D compared with 77% of the PRK eyes. No eye lost 2 Snellen lines of best corrected visual acuity at 6 or 12 months after surgery. Ten PRK eyes (9.3%) and three LASIK eyes (2.8%) were retreated. CONCLUSIONS: PRK and LASIK achieved equal refractive outcomes at all postoperative follow-ups, but LASIK patients were twice as likely to experience halos.  相似文献

PURPOSE: To evaluate the results of laser in situ keratomileusis (LASIK) for the correction of low and moderate myopia and astigmatism using the Nidek EC-5000 excimer laser with a modified algorithm. METHODS: Three hundred and ninety-six eyes of 257 patients with myopia ranging from -1.50 to -6.00 D and astigmatism of no more than 4.00 D were included. Manifest spherical equivalent refraction, uncorrected and spectacle-corrected visual acuity were assessed at 1 month (357 eyes, 90%), 3 months (220 eyes, 56%), and 6 months (168 eyes, 42%) after LASIK. RESULTS: Mean preoperative spherical equivalent refraction was -3.60 +/- 1.27 D and mean cylinder was -1.01 +/- 1.08 D. Mean postoperative spherical equivalent refraction was -0.12 +/- 0.31 D and mean cylinder was -0.19 +/- 0.33 D. At 6 months follow-up, 158 eyes (94%) had a mean postoperative spherical equivalent refraction within +/- 0.50 D of emmetropia; 168 eyes (100%) were within +/- 1.00 D. Uncorrected visual acuity was 20/20 or better in 81% (136 eyes) and 20/40 or better in 100%. Two eyes (1.2%) lost 1 line of spectacle-corrected visual acuity due to a decentered ablation. No eye lost 2 or more lines of spectacle-corrected visual acuity. CONCLUSION: Our modified LASIK algorithm with the Nidek EC-5000 excimer laser resulted in safe and predictable outcomes for treatment of low and moderate amounts of myopia and astigmatism.  相似文献

PURPOSE: To evaluate effectiveness, safety, predictability, and short-term stability of laser in situ keratomileusis (LASIK) using the LaserSight Compac-200 Mini excimer laser with software version 9.0, for all refractive errors. METHODS: One hundred fifty consecutive patients (300 eyes) that received bilateral LASIK for myopia, hyperopia, and astigmatism were studied prospectively. A new 9.0 software version applying a modified nomogram that takes advantage of bilateral surgery was used. Follow-up at 6 months was available for 267 eyes (89%). RESULTS: Six months postoperatively, 131 eyes (96.32%) in the low to moderate myopia group (-1.00 to -5.99 D; n=136) had a spherical equivalent refraction within +/-1.00 D, and 123 eyes (90.44%) were within +/-0.50 D of emmetropia. In the high to extreme myopia group (-6.00 to -25.00 D; n=114), 97 eyes (87.08%) had a spherical equivalent refraction within +/-1.00 D and 78 eyes (68.42%) were within +/-0.50 D of emmetropia. In the hyperopia group (+1.00 to +6.00 D; n=50), 44 eyes (88%) had a postoperative spherical equivalent refraction within +/-1.00 D, and 31 eyes (62%) were within +/-0.50 D of emmetropia. Mean change in spherical equivalent refraction at 6 months was less than -0.50 D in the low to high myopia groups and -1.16 +/- 0.55 D in the extreme myopia group. At 6 months follow-up, uncorrected visual acuity was 20/20 or better in 73 eyes (54%) in the low to moderate myopia groups and 21 eyes (18%) in the high to extreme myopia groups. In the hyperopia group at 6 months follow-up, uncorrected visual acuity was 20/20 or better in 31 eyes (62%) and 20/40 or better in 41 eyes (82%). Only two eyes had a temporary loss of two or more lines of spectacle-corrected visual acuity due to corneal folds that were surgically treated. Six months after LASIK, no eye had lost any lines of best spectacle-corrected visual acuity in this series. CONCLUSIONS: Our modified LASIK nomogram with the 9.0 software of the LaserSight 200 excimer laser (with a larger and smoother ablation pattern) resulted in safe and effective outcomes for the treatment of low to high myopia, astigmatism, and hyperopia.  相似文献

PURPOSE: To retrospectively analyze the safety and efficacy of hyperopic laser in situ keratomileusis (LASIK) treatment of eyes with primary hyperopia and consecutive hyperopia after initial myopic treatment. METHODS: Thirty-two eyes of 19 patients with primary hyperopia (group 1) and 37 eyes of 26 patients with consecutive hyperopia after initial myopic LASIK overcorrection (group 2) that had LASIK for hyperopia with the Hansatome microkeratome and VISX S2 Smoothscan excimer laser with 6 months' follow-up after surgery were analyzed. Uncorrected visual acuity, best spectacle-corrected visual acuity, fogged manifest refraction, and corneal topography with corneal irregularity measurement (CIM) were evaluated 1 month, 3 months, and 6 months after surgery. RESULTS: In group 1, the mean preoperative cycloplegic spherical equivalent was +4.0 +/- 4.5 diopters (D) (range, +1.5 to + 8.75 D) and the 6-month postoperative cycloplegic spherical equivalent was +0.26 +/- 1.74 D (range, -3.00 to +2.75 D). Fifty-three percent of eyes (n= 17) in group 1 were within 1 D of emmetropia. Sixty-six percent of eyes (n= 21) had uncorrected visual acuity of at least 20/40. Three eyes (9%) lost two lines of best spectacle-corrected visual acuity. Changes in uncorrected visual acuity, best spectacle-corrected visual acuity, spherical equivalent, and the CIM topographic index 6 months after surgery were statistically significant compared with the preoperative values. In group 2, the mean preoperative cycloplegic spherical equivalent was +1.58 +/- 0.35 D (range, +0.125 to +2.75 D), and the mean postoperative cycloplegic spherical equivalent was -0.48 +/- 0.46 (range, -2.75 to +0.38 D). Eighty-six percent of eyes (n= 32) were within 1 D of emmetropia. Eighty-four percent of eyes (n= 31) in group 2 had uncorrected visual acuity of at least 20/40. One eye (2.7%) lost two lines of best spectacle-corrected visual acuity. Complications included an epithelial nest that resolved 3 months after surgery in one eye in group 2. CONCLUSIONS: LASIK is a relatively safe treatment of primary hyperopia and hyperopia resulting from overcorrection after initial LASIK treatment of myopia (consecutive hyperopia). Patients with high hyperopia (>5 D) are at risk for loss of two lines of best spectacle-corrected visual acuity. A reduction in the level of attempted correction appears to be necessary in the treatment of consecutive hyperopia.  相似文献

目的:评价PRK术中应用低浓度(0.02g/L)丝裂霉素(PRK+LMMC)30s治疗低、中度近视眼术后疗效。

方法:选取我院接受屈光性角膜切削术(PRK)治疗的30例57眼低、中度近视患者,术中应用0.02g/L的MMC 30s,将术后6mo的随访结果与同期接受准分子激光原位角膜磨镶术(LASIK)治疗的疗效进行比较。

结果:术后6mo,PRK+LMMC治疗组等值球镜屈光度0.210D、等值球镜屈光度在±0.50D范围内的发生率71.9%、裸眼视力≥1.0者77.2%,平均最佳矫正视力1.19±0.18; LASIK治疗组等值球镜屈光度-0.017D,等值球镜屈光度在±0.50D范围内的发生率70.1%,裸眼视力≥1.0者72.7%,平均最佳矫正视力1.16±0.19,两组比较差异均无统计学意义(P>0.05)。术后6mo,PRK+LMMC治疗组没有出现超过1级的haze,所有眼最佳矫正视力均好于术前。LASIK治疗组1眼因角膜上皮植入使最佳矫正视力较术前下降3行,其余眼最佳矫正视力均好于术前。

结论:PRK术中应用0.02g/L的MMC 30s治疗低、中度近视同LASIK具有同样的视力结果,避免了LASIK瓣的并发症,不失为一个可取的手术程序。  相似文献

PURPOSE: To describe the off-label use of the Alcon LADARVision System for CustomCornea (LADARWave and LADARVision4000) in two patients, one for retreatment after laser in situ keratomileusis (LASIK) and one for retreatment after photorefractive keratectomy (PRK). METHODS AND RESULTS: Both patients were unhappy with the quality of their vision after initial refractive surgery. The first patient initially had LASIK to correct -4.75 D of myopia in each eye, and then had retreatment with CustomCornea. Before retreatment, manifest refraction was OD 0.50 -0.50 x 180 degrees (20/25 uncorrected) and OS -0.50 -0.75 x 180 degrees (20/40 uncorrected). Two months after retreatment with CustomCornea, his uncorrected visual acuity OD was 20/15, and 1 month after retreatment in his left eye, uncorrected visual acuity was 20/15. The second patient had PRK OD, with a postoperative uncorrected visual acuity of 20/25 and cycloplegic refraction of +1.00 -0.75 x 84 degrees. After CustomCornea retreatment, his 3.5-month postoperative uncorrected visual acuity was 20/15 and manifest refraction was +0.50 -0.50 x 180 degrees. CONCLUSION: The Alcon LADARVision System for CustomCornea was used effectively to retreat both a myopic LASIK and myopic PRK patient, primarily for subjective quality of night vision complaints.  相似文献

PURPOSE: To evaluate the effectiveness, predictability, and safety of laser in situ keratomileusis (LASIK) for correction of hyperopia and hyperopic astigmatism. METHODS: Fifty-four hyperopic eyes of 35 patients with a spherical equivalent refraction between +1.00 and +6.00 D were followed for at least 12 months following LASIK. All surgery was performed with the scanning Chiron Technolas Keracor 117C excimer laser. Data on uncorrected and spectacle-corrected visual acuity, predictability, stability of refraction, and complications were analyzed. RESULTS: At 12 months, the average residual refraction was +0.29 +/- 0.78 D; 83.3% of eyes (45 eyes) were in the range of +/- 1.00 D and 61.1% of eyes (33 eyes) were within +/- 0.50 D of emmetropia. Fifty eyes (92.6%) had uncorrected visual acuity of 20/40 or better and 34 (63.0%) eyes had 20/20 or better. One eye (1.9%) lost two lines of best spectacle-corrected visual acuity and two eyes (3.7%) gained two or more lines. Two patients (two eyes, 3.7%) had complaints of halos and one patient (one eye, 1.9%) had glare at 12 months after LASIK for hyperopia. CONCLUSIONS: LASIK was used to treat hyperopia from +1.00 to +6.00 D with good predictability and safety. Primary and second hyperopia require different nomograms, according to our experience.  相似文献

PURPOSE: The aim of this study was to compare the results of excimer laser photorefractive keratectomy (PRK) in patients who underwent PRK using the Summit Apex (Omnimed) excimer laser in one eye and the Nidek EC-5000 excimer laser in the other. METHODS: All consecutive patients who underwent PRK with the Summit Apex laser (Omnimed) in one eye and the Nidek laser (EC-5000) in the second and had at least 12 months of follow-up were included in this retrospective study (n=30). Uncorrected and spectacle-corrected visual acuity, final spherical equivalent refraction, and grade of subepithelial haze were compared. The average preoperative spherical equivalent refraction of eyes treated with the Summit laser was -6.00 D (range, -2.50 to -8.75 D), and for Nidek-treated eyes it was -5.57 D (range, -2.50 to -8.80 D). RESULTS: Forty-seven percent of Summit-treated eyes and 53% of Nidek-treated eyes had uncorrected visual acuity of 6/6 or better; 61% of Summit-treated eyes and 63% of Nidek-treated eyes had uncorrected visual acuity of 6/7.5 or better; 95% of Summit-treated eyes and 95% of Nidek-treated eyes had uncorrected visual acuity of 6/12 or better (difference not statistically significant). Seventy-three percent of eyes treated with the Summit laser and 80% of eyes treated with the Nidek laser had a postoperative refraction within +/-0.50 D of emmetropia; 97% of Summit-treated eyes and 87% of Nidek-treated eyes had a postoperative spherical equivalent refraction within +/-1.00 D of emmetropia; the difference between the two lasers was not statistically significant. However, the percent of eyes with persistent hyperopia was smaller in the Nidek group after 3 months (P=.0062) and after 6 months (P=.07) than in the Summit group. Videokeratography was not done. CONCLUSION: Both lasers were effective with relatively low side effects. No significant difference was found between the two lasers in postoperative uncorrected visual acuity or refractive outcome. Eyes operated with the Nidek laser had less persistent hyperopia and stabilized earlier.  相似文献

OBJECTIVE: To compare effectiveness, safety, and stability of excimer laser in situ keratomileusis (LASIK) and photorefractive keratectomy (PRK) for low-to-moderate myopia. DESIGN: Prospective, randomized, bilateral study. PARTICIPANTS: Thirty-three patients with a manifest refraction of -2.50 to -8.00 diopters (D) participated. INTERVENTION: For each patient, one eye received LASIK and the other received PRK. The first eye treated, and surgical method in the first eye, were randomized. Both eyes were treated by the same surgeon during the same operative session with a Summit Omnimed I laser (6-mm-diameter ablation) and a Chiron Automated Corneal Shaper. Follow-up was 90% at 1 and 2 years. RESULTS: At baseline, mean (+/-standard deviation) spherical equivalent manifest refraction was -4.80 +/- 1.60 D in LASIK-treated eyes and -4.70 +/- 1.50 D in PRK-treated eyes. At 1 day after surgery, 81% of patients (21 eyes) reported no pain in the LASIK-treated eye, whereas no patient (0%) reported being pain-free in the PRK-treated eye. At 3 to 4 days after surgery, 18 (80%) LASIK-treated eyes either improved or remained within 1 line of baseline spectacle-corrected visual acuity; only 10 (45%) PRK-treated eyes achieved this result. At 2 years after surgery, 18 (61%) LASIK- and 10 (36%) PRK-treated eyes achieved an uncorrected visual acuity of 20/20 or better, with no statistically significant difference in refractive outcome between the two techniques. Quantitative videokeratography showed more regularity after LASIK. Complications were similar in the two groups. Patients preferred LASIK by a margin of 2 to 1 at 1 year but showed no preference at 2 years. CONCLUSIONS: Using a 6-mm-diameter single-pass, large area ablation and an automated microkeratome to treat myopia of -2.50 to -8.00 D with 1.00 D or less astigmatism in 1994, the authors used LASIK to produce a higher percentage of eyes with an uncorrected visual acuity of 20/20 or better, more regular postoperative corneal topography, less postoperative pain, and more rapid recovery of baseline spectacle-corrected visual acuity than PRK. Both LASIK and PRK achieved successful correction of low-to-moderate myopia at 1 and 2 years after surgery.  相似文献

PURPOSE: To evaluate prospectively the long-term safety, efficacy, and visual performance following photorefractive keratectomy (PRK) for hyperopia using an erodible mask and axicon lens system. METHODS: Eighteen eyes of 9 patients with a mean preoperative spherical equivalent refraction of +2.26 +/- 0.82 D (range, +1.13 to +4.00 D) underwent PRK with the Summit Apex Plus excimer laser following manual scraping of the epithelium. Eyes were prospectively evaluated 1, 3, 6, 9, 12, 18, and 24 months following the procedure. Primary outcome variables included cycloplegic refraction and uncorrected visual acuity (UCVA). Visual performance was determined by contrast sensitivity measurements under scotopic (21 lux) and photopic (324 lux) conditions and best spectacle-corrected visual acuity (BSCVA) under scotopic, photopic, and glare conditions. RESULTS: For 18 eyes, 98.2% of the mean preoperative spherical equivalent refraction was corrected to +0.04 +/- 0.87 D (range, -1.38 to +2.00 D) at 24 months after PRK. Twelve eyes (67%) were within +/-0.50 D of attempted correction and 15 eyes (83%) were within +/-1.00 D. Stability within +/-0.50 D was achieved after 6 months. Two eyes (11%) experienced almost complete regression of the refractive effect. There was no statistically significant decrease in contrast sensitivity under scotopic or photopic conditions. (P > .05). Best spectacle-corrected visual acuity showed progressive improvement in the early postoperative period. By 24 months, 0 eyes (0%) lost 2 or more lines of BSCVA under scotopic and photopic conditions and 1 eye (5.5%) lost 2 or more lines under glare conditions. Fourteen eyes (78%) had grade 1 to 3 anterior stromal haze at 24 months which was characteristically mid-peripheral and did not adversely affect visual performance. CONCLUSION: Photorefractive keratectomy with the the Summit Apex Plus excimer laser for low to moderate hyperopia resulted in an effective reduction of hyperopia without compromising long-term visual performance. Stability and recovery of distance uncorrected and best spectacle-corrected visual acuity took approximately 6 months.  相似文献

PURPOSE: To compare uncorrected visual acuity and refractive error in patients undergoing photorefractive keratectomy (PRK) and laser in situ keratomileusis (LASIK) between 1 week and 6 months after surgery. METHODS: All eyes underwent PRK or LASIK with the VisX StarS2 excimer laser. We retrospectively analyzed data from 77 random eyes of 77 patients in the PRK group and 76 eyes of 76 patients in the LASIK group. All eyes had a low myopic refractive error (spherical equivalent range, -0.88 diopters (D) to -5.13 D; mean PRK. -2.8 +/- 0.20 D: LASIK, -2.5 +/- 0.22 D). Uncorrected visual acuity and manifest refractive error were evaluated 1 week, 1 month, and 6 months after surgery. RESULTS: Each eye undergoing PRK was paired with an eye undergoing LASIK for a similar level of spherical equivalent. Mean uncorrected visual acuity after 1 week was 0.85 +/- 0.06 (20/25, logMAR 0.12 +/- 0.04) for the PRK group and 1.01 +/- 0.06 (20/20, logMAR 0.01 +/- 0.03) for the LASIK group (p < 0.001). Mean spherical equivalent after 1 week was 0.23 +/- 0.12 D for the PRK group and -0.02 +/- 0.07 D for the LASIK group (p = 0.02). Mean uncorrected visual acuity after 1 month was 1.03 +/- 0.05 (20/20, logMAR 0.02 +/- 0.03) for the PRK group and 1.05 +/- 0.05 (20/20. -0.02 +/- 0.03) for the LASIK group (p = 0.16). Mean spherical equivalent after I month was 0.19 +/- 0.10 D for the PRK group and -0.02 +/- 0.09 D for the LASIK group. This difference was statistically significant (p = 0.02), but was unlikely to be clinically significant. Mean uncorrected visual acuity after 6 months was 1.05 +/- 0.06 (20/20, logMAR -0.01 +/- 0.03) for the PRK group and 1.06 +/- 0.05 (20/20, logMAR -0.14 +/- 0.03) for the LASIK group (p = 0.41). Mean spherical equivalent after 6 months was 0.02 +/- 0.08 D for the PRK group and 0.00 +/- 0.08 D for the LASIK group (p = 0.35). CONCLUSION: Uncorrected visual acuity 1 week after surgery is significantly better in eyes undergoing LASIK than in eyes undergoing PRK. Both procedures provide functional vision by 1 week after surgery. The difference does not relate to refractive error, which was similar between the two groups, but to differences in healing of the epithelium. By 1 month after surgery, there is no difference in mean uncorrected visual acuity between eyes that undergo PRK or LASIK for low myopia.  相似文献

PURPOSE: To examine the efficacy, predictability, stability, and safety of combined posterior chamber phakic intraocular lens (IOL) implantation and laser in situ keratomileusis (LASIK) in eyes with extreme myopia. METHODS: We analyzed the results of 67 eyes that received a posterior chamber hydrogel-collagen plate phakic IOL (STAAR Collamer Implantable Contact Lens) and also underwent secondary LASIK for the correction of extreme myopia. Mean follow-up was 3 months after the LASIK portion of the procedure (range, 1 day to 6 mo after LASIK). RESULTS: Mean preoperative spherical equivalent refraction was -23.00 +/- 3.60 D (range, -18.75 to -35.00 D), and mean refractive cylinder was 1.50 +/- 1.20 D (range, 0 to 5.00 D). Mean spherical equivalent refraction after IOL implantation and before LASIK was -6.00 +/- 2.80 D (range, -2.00 to -14.38 D) and mean refractive cylinder 1.50 +/- 1.10 D (range, 0 to 5.00 D). Mean postoperative spherical equivalent refraction at last examination after the LASIK portion of the two-part phakic IOL-LASIK procedure was -0.20 +/- 0.90 D (range, +1.75 to -5.13 D), and mean refractive cylinder was 0.50 +/- 0.50 (range, 0 to 2.25 D). Eighty-five percent (57 eyes) were within +/- 1.00 D and 67% (45 eyes) were within +/- 0.50 D of emmetropia at last examination. The refractions remained stable with a statistically insignificant change (P > .05 at each interval) during follow-up. Postoperative uncorrected visual acuity at last examination was 20/20 or better in 3% (2 eyes) and 20/40 or better in 69% (46 eyes). A gain of 2 or more lines of spectacle-corrected visual acuity was seen in 51 eyes (76%) and no eyes lost 2 or more lines of spectacle-corrected visual acuity at last examination. CONCLUSION: Combined posterior chamber phakic IOL implantation with the STAAR Collamer plate lens and LASIK (bioptics) is an effective and reasonably predictable method for correcting myopia from -18 to -35 D. PURPOSE: To evaluate safety, efficacy, predictability, and stability in the treatment of myopic astigmatism with laser in situ keratomileusis (LASIK) and photorefractive keratectomy (PRK) using the 200 Hz flying-spot technology of the LaserSight LSX excimer laser. SETTING: SynsLaser Clinic, Troms?, Norway. METHODS: This retrospective study included 110 eyes treated with LASIK and 87 eyes treated with PRK that were available for evaluation at 6 and 12 months, respectively. The mean preoperative spherical equivalent (SE) was -5.35 diopters (D) +/- 2.50 (SD) (range -1.13 to -11.88 D) in the LASIK eyes and -4.72 +/- 2.82 D (range -1.00 to -15.50 D) in the PRK eyes. The treated cylinder was 4.00 D in both groups. Eleven (8.5%) LASIK eyes and 8 (7.4%) PRK eyes had secondary surgical procedures before 6 and 12 months, respectively, and were excluded when the 6 and 12 month outcomes were analyzed. RESULTS: None of the eyes lost 2 or more lines of best spectacle-corrected visual acuity. Seventy-seven percent of the LASIK eyes and 78% of the PRK eyes achieved an uncorrected visual acuity of 20/20 or better; 98% in both groups achieved 20/40 or better. The SE was within +/-0.5 D of the desired refraction in 83% of the LASIK eyes and 77% of the PRK eyes; it was within +/-1.0 D in 97% and 98%, respectively. The cylinder correction had a mean magnitude of error of 0.04 +/- 0.31 D (range -0.96 to +0.85 D) in the LASIK eyes and 0.02 +/- 0.37 D (range -1.44 to +0.72 D) in the PRK eyes. Refractive stability was achieved at 1 month and beyond in the LASIK eyes and at 3 months and beyond in the PRK eyes. CONCLUSION: The outcomes of this study are comparable to those achieved with lasers that use small-beam technology with a lower frequency, as well as with other types of delivery systems. They suggest that the 200 Hz technology used in the LaserSight LSX excimer laser is safe, effective, and predictable and that with LASIK and PRK the results are stable when treating low to moderate myopia and astigmatism up to 4.0 D.  相似文献

PURPOSE: To evaluate the safety and efficacy of intrastromal corneal ring segments (ICRS, INTACS, Keravision) for the correction of corneal ectasia after laser in situ keratomileusis (LASIK). METHODS: In this prospective, noncomparative case series, INTACS were implanted in three eyes (two patients) that were ectatic after LASIK. Mean follow-up was 8.7 months (range, 8 to 10 mo). RESULTS: No intraoperative complications occurred. After INTACS implantation, uncorrected visual acuity ranged from 20/20 to 20/25 at 6 months and remained stable until 9 months. Mean postoperative spherical equivalent refraction was 0.50 D at 1 month and no significant changes appeared up to 9 months after INTACS implantation. After INTACS implantation, no eye lost any lines of Snellen spectacle-corrected visual acuity and two eyes gained from one to two lines. There was an increase in topographical regularity in all three eyes. CONCLUSION: Implantation of INTACS in eyes with corneal ectasia after LASIK resulted in good refractive outcome, absence of complications, and improvement in visual acuity.  相似文献

PURPOSE: To report the results of laser in situ keratomileusis (LASIK) in a series of adult patients with amblyopia. METHODS: A retrospective noncomparative review was performed on patients with amblyopia who underwent LASIK for correction of ametropia, using the Summit Krumeich Barraquer microkeratome and the Nidek EC-5000 excimer laser. Data were collected on uncorrected visual acuity, best spectacle-corrected visual acuity, manifest refraction, anterior segment evaluation, intraocular pressure, corneal topography, and dilated fundus examination (preoperative and postoperatively on day 1, months 2 and 6). RESULTS: Eight eyes of seven patients were included, with a mean patient age of 30 +/- 10 years (range 21 to 49 yr). Mean preoperative spherical equivalent refraction was -4.70 D (range -12.62 to +4.71 D) and the best spectacle-corrected visual acuity varied from 20/32 to 20/80. At 2 months after LASIK, mean spherical equivalent refraction was -0.37 +/- 0.60 D (range -1.37 to + 0.60 D), uncorrected visual acuity ranged between 20/20 to 20/30, and a mean gain of 3 Snellen lines (range 2 to 4 lines) was observed. All patients reported significant subjective improvement in their perception of vision. The visual acuity and subjective improvement were maintained throughout 6 months postoperatively. CONCLUSIONS: LASIK with the Nidek EC-5000 laser for correction of ametropia in adult amblyopic eyes provided encouraging results for visual acuity improvement beyond correction of the refractive error.  相似文献

PURPOSE: To evaluate the visual and refractive results of laser in situ keratomileusis (LASIK) for mixed and simple myopic astigmatism using bitoric ablation. METHODS: A retrospective study was performed in 65 eyes of 38 consecutive patients to evaluate uncorrected (UCVA) and best spectacle-corrected visual acuity (BSCVA) and cyclopegic and manifest refraction, before and 3 and 6 months after LASIK (Moria LSK-ONE microkeratome, Nidek EC-5000 excimer laser). RESULTS: At 3 and 6 months after LASIK, 40 eyes of 24 patients (64.5%) were available for follow-up examination. Mean age was 25.9 +/- 6.6 years (range 18 to 43 yr). Mean preoperative manifest spherical equivalent refraction was -1.40 +/- 0.80 D (range -3.80 to +0.50 D) mean preoperative cylinder was -3.30 +/- 1.30 D (range -1.00 to -6.00 D). At 6 months follow-up, mean manifest spherical equivalent refraction was +0.30 +/- 0.46 D (range -0.38 to +1.88 D), mean cylinder was -0.73 +/- 0.61 D (range -2.25 to 0 D). There was a 77.8% decrease in astigmatism magnitude. According to vector analysis, mean achieved vector magnitude was 80% of intended. Fifty percent (20 eyes) had a cylinder within +/- 0.50 D of emmetropia. Twenty-three eyes (57.5%) had a spherical component within +/- 0.50 D. Eighty-five percent (34 eyes) had postoperative UCVA of 20/40 or better. Ten percent (four eyes) lost two lines of Snellen BSCVA, whereas 35% (14 eyes) gained one or more lines. CONCLUSIONS: Bitoric LASIK with the Moria LSK-ONE microkeratome and Nidek EC-5000 excimer laser was effective and safe for the reduction of cylinder in mixed and simple myopic astigmatism. Moderate undercorrection of the cylinder was evident.  相似文献

PURPOSE: To compare visual function, safety, and higher-order aberrations (HOAs) after wavefront-guided laser in situ keratomileusis (LASIK) with the LadarVision CustomCornea (Alcon Laboratories, Inc.) and Star S4 CustomVue (Visx) laser systems. SETTING: John A. Moran Eye Center, University of Utah, Salt Lake City, Utah, USA. METHODS: Seventy-eight eyes of 39 patients with myopia with or without astigmatism were randomized for LASIK treatment in 1 eye with the CustomCornea laser; the other eye was treated with the CustomVue laser. Patients were followed for 6 months after surgery. The primary outcome measures were uncorrected visual acuity (UCVA), best spectacle-corrected visual acuity (BSCVA), manifest refraction, and changes in HOAs. RESULTS: At 6 months, the mean logarithm of the minimum angle of resolution (logMAR) UCVA was -0.0135 +/- 0.07 (SD) in the CustomVue group and 0.0417 +/- 0.12 in the CustomCornea group (P = .023). Eighty-eight percent of eyes in the CustomVue group had 20/20 or better UCVA compared with 67% in the CustomCornea group (P<.02). At 6 months, 91% of eyes in the CustomVue group and 79% in the CustomCornea group were within +/-0.50 diopter (D) of emmetropia (P<.1); 88% and 50%, respectively, were within +/-0.25 D (P<.001). Both platforms led to a small increase in total HOAs. The CustomVue system reduced trefoil and induced less of an increase in total HOAs, whereas the CustomCornea platform increased trefoil but induced less of an increase in spherical aberrations and coma. CONCLUSIONS: Both laser systems were effective, safe, and predictable. Wavefront-guided LASIK with the CustomVue system resulted in better visual acuity, with more eyes having 20/20 acuity than in the CustomCornea group.  相似文献

PURPOSE: To evaluate safety, efficacy, and predictability of photorefractive keratectomy (PRK) for hyperopic astigmatism of +1.75 to 00 D manifest refractive sphere and up to -2.50 D manifest refractive astigmatism using the VISX Star excimer laser system, version 2.5 software. METHODS: Treatment was performed on 32 eyes of 21 patients. Eighteen of 21 patients were 45 years of age or older. Manifest and cycloplegic refraction together with Pelli-Robson contrast sensitivity assessment was performed prior to surgery and 1, 3, 6, 12, and 24 months after treatment. RESULTS: Twenty-seven of 32 surgical procedures were reviewed 1 year after treatment (84%). Corneal epithelial healing was complete between day 4 and 10. Twelve months after treatment, 25 of 27 eyes (93%) achieved 20/40 or better uncorrected visual acuity and 19 eyes (70%) achieved 20/20. No patient lost two or more lines of Snellen visual acuity assessed 6 months and later after treatment. The mean spherical equivalent refraction was reduced from +2.90 at baseline to +0.10 D at 1 year and +0.40 D at 2 years; 65% of eyes had a refraction within +/- 0.50 D. Four patients had further treatment by laser in situ keratomileusis for undercorrection in three eyes and overcorrection in one eye. Pelli-Robson contrast acuity was significantly reduced 12 months after treatment from a mean 1.72 before to 1.66 after PRK (P = .02, t-test). CONCLUSIONS: PRK for hyperopia using the VISX Star excimer laser system was effective in the treatment of hyperopic astigmatism. Although no patient lost two or more lines of high contrast best spectacle-corrected Snellen visual acuity 1 year after treatment, there was a significant decrease in Pelli-Robson contrast acuity.  相似文献

OBJECTIVE: To describe the safety, effectiveness, and predictability of laser in situ keratomileusis (LASIK) for correcting residual myopia after primary photorefractive keratectomy (PRK). DESIGN: A retrospective, noncomparative case series. PARTICIPANTS AND INTERVENTION: Thirty-six consecutive eyes of 30 patients underwent LASIK after primary PRK. A Multiscan Schwind excimer laser was used for LASIK enhancement. MAIN OUTCOME MEASURES: Uncorrected visual acuity (UCVA), best spectacle-corrected visual acuity, refraction, videokeratography, and complications were determined before and after LASIK retreatment. Follow-up was at least 12 months. RESULTS: Before LASIK, 11.11% of eyes showed a UCVA of 20/40 or better. This increased to 94.44% 12 months after LASIK. A UCVA of 20/25 or better was achieved in 0% before and in 72.22% after retreatment. Refraction +/-0.5 diopters (spherical equivalent) represented 0% of eyes before and 77.78% of eyes after enhancement. Before LASIK, two eyes had significant haze. Haze remained in these two eyes and appeared in another eye. CONCLUSIONS: Laser in situ keratomileusis proved to be safe and effective for treating residual myopia after PRK. Care must be taken when considering LASIK retreatment in patients with significant haze after primary PRK.  相似文献
