目的探讨腹腔镜活体肝移植供肝切取手术的技术要点和应用前景。方法回顾性分析2015年9月至2016年10月四川大学华西医院肝脏肝移植外科12例腹腔镜下活体肝移植供肝切取术病人资料。分析12例供者及相应12例受者的手术及预后情况。结果 12例供者中,切取不包括肝中静脉的右半肝6例,左外叶3例,不包括肝中静脉的左半肝3例。术中失血量400(100~600 mL)。供者住院时间7(4~10 d)。所有供者术后均无并发症发生及围手术期死亡。12例受者术前移植物受体体重比(GRWR)为0.94%(0.54%~3.70%)。手术时间625(405~720 min)。术中失血量750(200~3000)mL。术后住院时间20(7~40)d。1例受者病人术后第7天发生消化道出血,保守治疗后出血停止。1例受者病人术后出现肺部感染,保守治疗后无效于术后第8天因呼吸衰竭死亡。其余受者病人术后顺利出院。结论随着技术的不断发展和器械的不断改进,腹腔镜活体肝移植供肝切取将有广阔的应用前景。  相似文献

目的 探讨婴幼儿亲属活体部分供肝肝移植中的供、受者的安全性.方法 2006年9月至2009年11月,行婴幼儿亲属活体部分供肝肝移植者8例,受者原发病均为先天性胆道闭锁.供者术前常规行三维CT观察和计算全肝及肝叶(段)体积及形状,磁共振胰胆管造影术(MRCP)了解胆道情况.根据婴幼儿受者腹腔容积切取合适肝叶(段)行肝移植术.受者术后给予抗排斥反应和预防感染等治疗;供者术后给予保肝和抑酸治疗.分析供、受者术前、术中及术后情况.结果 8对供、受者手术均获成功.6例供肝为左外侧叶,1例为S3肝段,1例为减体积S3肝段,供肝重量为(235.9±53.6)g(148~302 g),供肝重量与受者体重比(GW/RW)为(2.65±0.48)%(2.11%~3.36%).对供、受者随访3~40个月(中位数18个月),8例供者均存活,发生并发症2例(25%).8例受者中,死亡1例(12.5%),其他受者发生并发症13例次.结论 术前对供、受者行精确评估,切取合适供肝行肝移植术,术中精细操作,术后精心治疗可最大程度保证供、受者的安全.  相似文献

目的介绍成人间活体扩大右半肝移植治疗急性肝功能衰竭的临床经验。方法对1例42岁男性急性肝功能衰竭合并肝性脑病Ⅲ期患者行活体扩大右半肝移植治疗。其45岁姐姐为供者,CT评估供者包含肝中静脉的扩大右半肝体积为728.4cm^2(801g),供肝/受者体重比为1.3%。供肝之肝右、中静脉整形后与受者整形后之肝右静脉行端-侧吻合;供受者门静脉、肝动脉行端.端吻合。供肝胆管整形后与受者胆总管行端-端吻合。结果供、受者手术均成功。供者术后恢复顺利,受者术后8h恢复意识,14d后丙氨酸转氨酶、总胆红素等指标首次下降至正常水平。术后16d曾出现转氨酶明显升高,给予甲泼尼龙1000mg冲击治疗后恢复正常。随访至今,供受者已健康生存8个月,均未出现胆管、肝动脉及静脉回流等并发症。结论扩大右半肝移植在技术上完全可行。能为成人患者提供足够重量的移植物,尤其对于急性肝功能衰竭患者具有重要意义,术前精确的影像学评估,熟练的肝切除和肝移植技术是确保该类手术成功的关键因素。  相似文献

目的 总结活体右半供肝移植中,不含肝中静脉的右半供肝Ⅴ、Ⅷ段静脉回流的重建方法.方法 回顾性分析55例活体右半供肝移植中,不含肝中静脉的右半供肝Ⅴ、Ⅷ段静脉回流重建的临床资料.所有供者均通过了受者标准肝体积、供肝与受者体重比(GRWR)、供肝与受者标准肝体积比及供肝内血管解剖等指标的综合评估.供肝在切取、灌注及修整后,51例采用尸体髂静脉作为重建材料,其他4例分别采用受者的大隐静脉、曲张的脐静脉、肝内门静脉和肝静脉作为重建材料,以串联的方式重建供肝断面Ⅴ、Ⅷ段静脉回流.肝移植时,供肝肝右静脉与受者肝右静脉开口吻合,重建的Ⅴ、Ⅷ段静脉与受者肝中、肝左静脉汇合部吻合,供肝门静脉与受者门静脉右支或主干吻合.门静脉开放血流后依次重建肝动脉及胆道.术后对供、受者进行常规监测.结果 1例受者术后发生流出道梗阻,术后第43天死亡;1例受者术后第7天出现不明原因的急性重型肝组织坏死,行尸体供肝肝移植后痊愈.其余53例受者恢复顺利,术后4周时腹部CT检查显示重建的移植肝Ⅴ、Ⅷ段静脉回流通畅.55例供者术后均恢复顺利,术后2周出院.结论 不含肝中静脉的活体右半供肝Ⅴ、Ⅷ段静脉回流的重建在活体肝移植中是可行的,应选择合适的重建材料及手术方式.受者肝移植后临床效果良好.  相似文献

目的 探讨成人间活体肝移植供者评估、手术方式的选择及术后并发症分析.方法 收集2007年1月至2010年8月同一外科组施行的94例成人间活体肝移植的临床资料.受者年龄18 ~76岁,供者年龄19 ~60岁.94例活体肝移植手术方案包括:左半肝供肝移植2例,右半肝供肝移植92例,44例切取肝中静脉例,48例不切取肝中静脉.分析供受者术前评估、术后并发症及存活情况.结果 所有供者均恢复良好出院,供者并发症发生率为7.4%.随访截止于2011年5月31日,中位随访时间为37个月,死亡8例.供者1年存活率为95.7%,移植物存活率为94.7%.1例发生小肝综合征;1例因急性肝坏死行再次肝移植;24例(25.5%)经胆道造影或磁共振胰胆管成像检查发现胆道吻合口狭窄,但其中9例(9.6%)表现为肝功能异常.结论 活体肝移植是治疗终末期肝病的有效方法,精确的术前评估、合理手术方式选择,采用左半肝或右半肝供肝、含或不含肝中静脉的活体肝移植,在成人间活体肝移植中均能有效的保证供受者安全.  相似文献

目的探讨成人间双活体供肝移植的手术重建技巧。方法2例成人患者,1例原发病为乙型肝炎后肝硬化、巨块型肝癌,接受其2位姐姐的左半肝移植,移植物与受者体重比(GR/WR)为1.5%,移植于原肝位置左侧的左半肝,其肝左静脉、门静脉分别于受者的肝左静脉、门静脉左支端端吻合,胆肠吻合方式重建供肝胆道;移植至右侧的左半肝,沿矢状位旋转180°,其肝左静脉、门静脉分别与受者的肝右静脉、门静脉右支端端吻合,左肝管与受者的肝总管端端吻合。另1例原发病为乙型肝炎后肝硬化,接受其母亲的右半肝移植,但由于供肝存在大泡状空泡变性,且供、受者的体重差较大,为能满足患者的生理需要,故该例同时行尸体左半肝移植,GR/WR为1.2%,右半肝的肝右动脉、肝右静脉分别于受者的同名血管端端吻合,其门静脉与受者的门静脉右支端端吻合,右肝管与受者的肝总管端端吻合;左半肝的肝左静脉、门静脉及肝动脉分别与受者的肝左静脉、门静脉左支及脾动脉端端吻合,胆肠Roux-en-Y吻合方式重建供肝胆道。结果除母亲供者术后3d出现短暂乳糜漏,经对症治疗11d后痊愈外,供者无其它并发症发生。2例患者术后恢复良好,未发生排斥反应及全身感染,现已随访10个月以上,肝功能正常,均恢复正常工作和生活。结论成人间双活体供肝移植可为受者提供更大的肝脏,又可减少供者的风险,但手术操作复杂,需要对供、受者的条件进行充分评估后施行。  相似文献

目的 探讨精准肝脏外科理念在儿童活体肝移植供肝切取术中的临床价值.方法 回顾性分析2012年12月至2014年1月上海交通大学医学院附属仁济医院收治的58例儿童活体肝移植供者的临床资料.术前对供者行CT等检查,将二维影像学数据进行三维重建,评估供者肝内胆管和血管情况,并对肝左动脉和肝左静脉解剖结构进行分型,测算供者标准肝脏体积、拟切取肝脏体积和受者标准肝脏体积,模拟手术操作,制订手术方案.采取精准肝切除切取供肝.采用门诊和电话方式进行随访,随访时间截至2014年4月.结果 58例儿童活体肝移植供者术前CT血管造影检查示肝左动脉Ⅰ型28例、Ⅱ型10例、Ⅲ型20例、无Ⅳ型供者;肝左静脉Ⅰ型35例、Ⅱ型23例.三维重建预测拟切取肝脏体积为(243±65) mL.58例供者均成功完成供肝切取术,其中7例为左半肝切取,51例为肝左外叶切取.2例供者行胆囊切除.术中实际切取肝脏体积为(255±59) mL,拟切取肝脏体积平均误差率为4.94%.移植物质量与受者体质量比为3.3%±1.0%.手术时间为(260±89)min,术中出血量为(181±35)mL,仅1例供者术中输RBC 2 U.供者术后胃肠功能恢复时间为(2.0±1.1)d,术后拔除引流管时间为(3.0±1.2)d,术后住院时间为(7±3)d,出院时所有供者血清WBC、Hb、ALT、AST、TBil、DBil、AIb等指标水平正常.2例供者术后发生并发症,分别为切口少量渗血和脂肪液化,均经对症治疗后痊愈.58例儿童活体肝移植供者术后均获得随访,中位随访时间为8.7个月.供者恢复良好,随访期间无并发症发生.结论 精准肝脏外科理念应用于儿童活体肝移植供肝切取术,切取准确率高、供者肝功能损害小、术后并发症少、恢复快.  相似文献

目的 探讨成人间活体供肝移植中切取供者右半供肝(含或不含肝中静脉)的安全性及临床效果.方法 2007年6月至2008年9月,单小组实施成人间活体供肝切取手术78例;76例行右半供肝移植,其中供肝含肝中静脉30例(含肝中静脉组),不含肝中静脉46例(不含肝中静脉组).对两组供者的基本资料、手术相关资料以及术后肝功能恢复情况进行了评估和比较.结果 CT计算供者残留肝脏体积比为29.40%~50.99%;供肝重量与受者体重的比例(GRWR)为0.74%~1.76%.两组供者(含与不含肝中静脉组)在年龄、体重身高指数(BMI)、手术时间、术中失血量和输注红细胞量、拔除引流管时间、住院时间以及供者存活率方面比较,差异均无统计学意义;含肝中静脉组供者体重小于受者体重所占的比例(75.0%)明显高于不含肝中静脉组(40.0%),差异有统计学意义(P<0.05);含肝中静脉组切取的供肝重量、实际GRWR以及供肝冷保存时间明显低于不含肝中静脉组(P<0.05);两组供者术后肝功能恢复情况比较.差异无统计学意义.结论 供者经过严格的术前评估,切取含或不含肝中静脉的右半供肝均是安全的,临床效果满意.  相似文献

目的 探讨成人间活体右半肝移植术中变异门静脉支(APVB)切取与重建的技巧.方法 2002年1月至2007年4月,共实施70例成人间活体右半肝移植.术前肝脏血管三维CT成像显示供肝动脉及静脉走向,70例右半供肝中有9例门静脉分支变异,其中7例为Ⅱ型变异,2例为Ⅲ型变异.除1例供者行狭窄桥状连接单口切取APVB外,其余8例均采用供者优先的原则即距门静脉主干2~3mm处双口切断APVB.Ⅱ型变异中有2例双口切取其右前、右后支成形为一个开口后与受者门静脉主干吻合,4例右前、右后支分别与受者门静脉左、右支吻合,1例行右前、右后支间狭窄桥状组织连接单口切取后与受者门静脉主干单口吻合.Ⅲ型变异中有1例双口切取其右前、右后支分别与受者门静脉支双口吻合,1例双口切取后行新型的U形血管移植物间置与受者门静脉主干单口吻合.结果 9例受者均无门静脉狭窄或血栓、肝动脉狭窄或血栓以及肝静脉流出道狭窄等血管并发症发生.1例供者术后3 d并发门静脉血栓,手术取栓及门静脉壁修补成形后痊愈.新型的U形血管移植物间置重建术后通畅,无并发症发生.结论 成人间活体右半肝移植术中采用供者优先的原则双口切取APVB、双口吻合重建以及新型的U形血管间置等门静脉重建技术是安全可行的,未增加手术难度,且临床效果良好.  相似文献

目的 探讨开展成人活体肝移植初始阶段如何确保供、受者安全.方法 回顾性分析我院2007年4月至11月71例成人活体肝移植供、受者临床资料,分析评估方案、手术策略和并发症.结果 切取供者右半肝68例,其中带肝中静脉4例;切取带肝中静脉左半肝3例.术后出现并发症2例,1例胆漏,1例腹腔内出血,无供者死亡.受者外科并发症18例,其中胆道并发症12例,血管并发症3例,小肝综合征3例;病死率为10%(7/71).结论 在开展成人活体肝移植的初始阶段,采用严格的供、受者评估、选择合理的手术方式和术后处理可以最大程度地保证供、受者安全.  相似文献

目的:分析10例活体肝移植术中的血管变异,总结其外科处理经验,进一步提高手术成功率,减少并发症。方法:2001年1月至12月,行活体肝移植10例,其中左半肝8例,左外叶1例,右半肝1例,供肝者均为其母,经术中B超及胆管造影以确定肝切线。供体单支肝动脉分支与受体肝动脉吻合,两支肝动脉分别与受体肝左、右动脉吻合。门静脉分支与受体门静脉主干吻合。供体肝静脉与受体下腔静脉行端侧吻合。胆管重建均采用肝管分支与受体胆总管端端吻合,置T管引流。结果:10例活体肝移植,1例因肝动脉血栓形成,术后5天需次肝移植;1例发生排斥;其余8例均康复出院,5例已上学。结论:活体肝移植术中血管重建技术是其重要环节,术前和术中了解供受体解剖变异并正确处理,可减少术后血管和胆道的并发症。  相似文献

Liver transplantation in patients with biliary atresia and polysplenia syndrome requires some technical adjustments. During eight yr, 75 biliary atresia patients underwent living donor liver transplantation. Of these, two patients were complicated by absent inferior vena cava with azygos continuation. Both patients received hemi-liver grafts with short hepatic veins. Outflow was reconstructed using a cryopreserved vena cava. In both patients, the short hepatic vein was anastomosed to the vein graft end-to-end and the end of the vein graft was anastomosed with the common orifices of recipient's hepatic veins. Both of the patients survived the operation with satisfactory graft function. A hemi-liver graft with short hepatic veins can be used for patients with biliary atresia with inferior vena cava agenesia using the current technique.  相似文献

BACKGROUND: Absence of the portal bifurcation is exceptional and characterized by an absent extrahepatic portal vein bifurcation, the right portal vein only being at the porta hepatis. There is no extraparenchymal left portal vein. This may represent a problem in liver splitting, reduction, and living related transplantation. METHOD: A case was encountered during reduction of a cadaveric liver allograft to a left lateral segmental graft from a 40-kg cadaveric donor to a 15-kg recipient. The portal venous inflow was reconstructed with a vein graft via a novel extrahilar approach to the left portal vein at the umbilical fissure. RESULTS: This graft was used successfully in a 3-year-old child requiring transplantation for a failed Kasai operation for extrahepatic biliary atresia. The child is now well, 1 year posttransplant, after an uneventful postoperative course with good portal flow within the graft. CONCLUSION: The situation of an absent left portal vein extrahepatic course should not preclude splitting or reduction procedures. The innovative technical solution, we propose, should add to the armamentarium of the liver transplant surgeon contemplating a left lateral segmental graft for the paediatric liver transplant recipient.  相似文献

The authors describe new technic of non-auxiliary orthotopic transplantation of segmental liver harvested from living dogs. 32 dogs were utilized. In the donor dogs, the left medial and lateral lobes were mibilized. The left portal branch, left hepatic artery, left biliary branch and the left hepatic vein were dissected free. The segmental liver graft was perfused and cooled in-situ through the left portal vein. The recipient dogs underwent two steps total hepatectomy: First the segments I, II, III, IV and V were resected. The segments VI and VII were maintained as well as the right portal vein and the retro-hepatic inferior vena cava order to keep the splanchnic and caval flux and to avoid the spleno-cavo-jugular by-pass. The segmental liver graft was then transplanted in an orthotopic position. Termino-lateral hepatico-caval anastomosis and left porto-portal anastomosis as well as arterial and biliary reconstruction were executed. All the donors survived more than 30 days. Nine recipient dogs died during the first three post-operative days from hemorrhage (3 dogs), fibrinolysis (2 dogs), primary non function of the graft (2 dogs) and hepatic artery thrombosis (1 dog). A successful experience with auxiliary partial orthotopic liver transplantation (APOLT) for acute liver failure is reported in a 29-year-old woman who experienced jaundice, generalized erythema for 7 days, and decreased mentation for 3 days. Two months prior, she suffered pulmonary tuberculosis, being currently treated with antituberculous medications, which caused the fulminant hepatic failure. We decided to perform APOLT based on two facts. The first was the possibility that the diseased native liver may recover sufficiently to withdraw the immunosuppressants. Second, the pulmonary tuberculosis may have been worsened by immunosuppression. We removed the extended lateral section of the recipient for the graft. The left hepatic vein of the extended left lateral graft was anastomosed to the left hepatic vein of the recipient. The left portal vein of the graft was anastomosed to the left portal vein of the recipient. The right portal vein of the recipient was left without any manipulation. A duct-to-duct anastomosis was performed. On postoperative day 3, antituberculous medications were started. On the postoperative day 37, she was discharged without any problems. On the postoperative day 120, she showed no event of rejection, and her pulmonary symptoms improved. We performed the operation without transection of the portal branch to the native liver, but no functional competition has been discovered.  相似文献

Orthotopic liver transplantation requires matching of the donor and recipient for size. The rarity of suitable pediatric and infant donors in many countries has resulted in the acceptance of the principle of a reduced-sized adult liver graft for children. A new method of segmental orthotopic liver transplant is described. This method involves resecting the recipient liver off the inferior vena cava, which is left in situ, and the donor hepatic vein is anastomosed end to side to the inferior vena cava. The technique allows orthotopic liver transplantation with widely mismatched sizes from adults to infants.  相似文献

We report a transplant of the left lateral liver segments with 3 arteries for a pediatric recipient from a living donor. A 6-month-old female infant was diagnosed with liver cirrhosis secondary to biliary atresia and scheduled for living donor liver transplantation (LDLT; mother as donor). Left lateral hepatectomy was performed at the donor site. The dissection of the left hepatic artery (HA), which was divided immediately after its origin, showed 3 branches for segments II, III, and IV. The arteries for segment II, segment III, and segment IV were anastomosed to the recipient HA. No postoperative complications were observed. The outcome of this case demonstrates that left lateral segments with 3 arteries can be successfully used if proper surgical techniques are applied. From this experience we can recommend “3-in-1 segmental resection” in the donor can be safely done by skilled microvascular surgeons and this technique should be considered for selected cases where multiple tiny arteries supply the graft.  相似文献

目的:探索肝中叶作为独立供肝进行移植的可行性,以进一步拓宽供肝来源。
方法:普通级健康犬分成供体组(n=12,20~25 kg)和受体组(n=12,10~15 kg),供受体随机配对。供体手术将位于中央区的方叶、右中叶在体内劈离,原位灌注后保留其专属的门静脉中支、肝中动脉和中肝管,得到独立的中央区供肝并称重。受体手术先建立临时性门腔分流,供肝背驮式植入,流出口和腔静脉壁端侧吻合,供肝门静脉中支和受体门静脉主干行端端吻合后恢复新肝血流,重建动脉和胆管。术后观察受体腹腔及胆汁引流,每日检测肝功能,死亡后行尸检,移植物取标本行病理检查。
结果:犬肝被深陷的叶间裂分隔成7叶,各叶间由较少的肝桥连接,方叶和右中叶由门静脉中支、中肝动脉营养血回流至肝中静脉,胆汁引流至中肝管。供体组体内劈离技术全部得以完成,手术时间(215.0±67.7)min,失血量(229.3±66.5)mL。比较GRWR,中央区供肝[(1.3±0.3)%]和假设的左侧区供肝的[(2.1±0.4)%]及右侧区供肝的[(0.9±0.1)%]之间差异均有统计学意义(均P<0.01)。受体组手术时间(327.6±75.3)min,无肝期(33.6±7.5)min,失血量(415.5±79.8)mL。12个供肝均成功植入,冷缺血时间为(41.9±12.1)min,(8.3±3.6)min后排泌胆汁。受体肝功能指标在术后第1天发生明显变化,随后逐渐恢复,中位存活时间92.5(18~272)h,未发现有因吻合口出血、血栓等外科技术性并发症而死亡受体。
结论:犬动物模型证实肝中央区可以劈离出来作为一个独立的供肝器官,为将来拓宽供肝来源提供了另外一种思路。  相似文献

Abnormalities of recipient or donor vascular structures are associated with reconstructive difficulties in liver transplantation. A patient with thrombosis of the right hepatic vein and associated stricture of the inferior vena cava (IVC), portal vein thrombosis and multiple aberrant arteries underwent orthotopic liver transplantation. The donor's suprahepatic IVC was anastomosed to the recipient's intrathoracic IVC. The portal vein flow was restored by venous graft interposition, while the arterial flow was ensured by interposing an iliac arterial graft anastomosed to the infrarenal aorta. In conclusion, graft function remains excellent more than 5 years postoperatively.  相似文献
