抑郁症患者SPECT研究   总被引:7,自引:0,他引:7  
目的 探讨抑郁症脑血流灌注的特点及与临床症状的相关性。方法 开放式收集门诊及住院诊断为抑郁症患者37例,进行治疗前后的^99mTc-双半胱乙脂(ECD)单光子发射型计算机断层扫描(SPECT)技术,测量患者治疗前后脑血流灌注(rCBF)显像,半定量测定rCBF,并与12例正常对照者比较。结果 37例抑郁症患者双侧额叶、双侧颞叶、双侧顶叶,双侧枕叶、左基底核有脑低灌注现象。治疗后临床康复者各部位脑灌注明显改善,差异有显著性(P〈0.05)。结论 抑郁症患者存在额叶、颡叶、顶叶及枕叶多区域的脑血流低灌注,其中以左侧较为明显,治疗后明显改善。  相似文献

目的 了解老年期抑郁症(ED)的脑血流灌注特点,为相关神经科学理论提供实验室数据。方法 对26名健康老人、22例ED及26例阿尔茨海默病(Alzheimer’s disease,AD)患者作脑血流灌注检查。结果 与健康老人相比,ED存在右顶叶、双侧额叶和双侧枕叶低灌注(P<0.05),与AD相比,ED的扣带回、双侧颞叶、双侧基底节和双侧丘脑灌注明显较高(P<0.05)。ED在颞叶、额叶、枕叶和丘脑处的双侧灌注不对称(P<0.05),除丘脑灌注为左侧高于右侧外,其余均为左侧低于右侧。判别分析结果为右顶叶、左颞叶、左额叶、左枕叶和扣带回进入判别方程,ED的判别方程对ED诊断的敏感性和特异性分别为100%和94.2%。结论 ED脑血流灌注兼具一般抑郁症和本身的特点,右顶叶、左颞叶、左额叶、左枕叶和扣带回灌注对ED的诊断有重要意义,支持额叶、边缘系统及其它相关区域共同构成情绪调节回路的假设。  相似文献

目的 分析抑郁症患者单光子发射型计算机断层显像(SPECT)局部脑血流(SCBF)灌注变化的特点,并初步探讨rCBF显像在抑郁症患者中的诊断价值. 方法 对10例正常人及临床诊断为抑郁症的32例患者分别进行SPECT局部脑血流灌注显像,利用感兴趣区(ROI)法观察rCBF情况;11例患者同期行MRI普通扫描.结果 32例抑郁症患者中31例发现rCBF显像异常,共检出71个病灶,分别位于双侧额叶、颞叶、基底节、海马、扣带回及左侧岛叶,其中57个病灶位于颞叶、海马、扣带回及岛叶,即边缘系统,9个病灶位于额叶,5个病灶位于基底节;基底节区病灶局部脑血流灌注均增高,其他部位病灶均降低;双侧大脑半球rCBF病灶构成比差异无统计学意义(χ~2=409,P=0.790).11例患者同期做MRI扫描,3例发现异常(2例轻度脑萎缩,1例海马萎缩),而此11例患者rCBF显像均发现异常.1例正常人rCBF未发现异常显像.结论 大多数抑郁症患者存在脑边缘系统rCBF降低,部分患者存在基底节区rCBF增高.SPECT能敏感地发现抑郁症患者rCBF灌注的变化,对抑郁症的诊断具有一定的价值.  相似文献

目的应用单光子发射计算机断层扫描(SPECT)成像术研究非痴呆型血管性认知障碍(VCIND)患者的局部脑血流(rCBF)改变,并分析其与神经心理评分的关系。方法纳入1VCIND患者45例(VCIND组),其中男25例、女20例;2认知功能正常脑卒中(SNC)患者28例(SNC组),其中男16例、女12例;3健康对照者15例(HC组),其中男6例、女9例。所有研究对象行一般检查、神经心理评定(MMSE量表、MoCA量表、CDR量表、NIHSS量表、ADL量表、Loeb量表、Hamilton抑郁量表)及SPECT rCBF检查。结果 1与HC组比较,VCIND组两侧额叶、两侧顶叶上部、左侧丘脑rCBF明显降低(P0.05),SNC组左侧额叶、左侧顶叶下部、左侧颞叶、两侧丘脑以及两侧纹状体rCBF明显降低(P0.05);2与SNC组比较,VCIND组左侧额叶、左侧顶叶上部以及左侧丘脑rCBF明显降低(P0.05);3VCIND组内,左侧额叶、左侧顶叶上部、左侧丘脑均较右侧相应部位rCBF明显降低(P0.05);4相关分析显示VCIND组MMSE评分与左侧额叶、左顶叶上部、两侧颞叶内侧、左侧纹状体rCBF呈正相关(r=0.73~0.84,P0.05),MoCA评分与左侧额叶、右侧顶叶下部、两侧颞叶内侧rCBF呈正相关(r=0.65~0.85,P0.05)。结论 SPECT探查VCIND患者rCBF改变具有重要临床意义,MMSE及MoCA评分与局部rCBF改变具有一定相关性。  相似文献

目的用SPECT测定抑郁症患者的局部脑血流(rCBF),比较乙酰唑胺脑负荷试验后脑血流灌注变化,观察抑郁症患者脑血管的调节能力,以及是否存在潜在缺血灶。方法以18例未经抗抑郁治疗的抑郁症患者为研究对象,19名正常人作为对照组,行单光子发射计算机断层扫描(SPECT)检查。抑郁症患者48h后口服乙酰唑胺2g,再行SPECT检查。观察服用乙酰唑胺前后脑内血流的变化。结果抑郁组患者双侧额叶、颞叶的rCBF显著下降(P<0.01~0.05),左顶叶、右基底节rCBF也明显降低(P<0.05);同时,抑郁症患者局部脑血流低灌注存在不对称性,左侧灌注更低。服用乙酰唑胺后,原脑内各血流灌注下降部位恢复正常血供,未发现潜在缺血病灶。结论抑郁症患者某些特定部位存在脑血流灌注下降;乙酰唑胺脑负荷SPECT试验未发现抑郁症患者存在潜在缺血部位,而且使其局部脑血流低灌注状态恢复正常。  相似文献

目的分析脑CT灌注成像在首发抑郁症患者中的诊断价值。方法选取本院2020年1月~2021年1月期间收治的180例首发抑郁症患者作为观察组,同时选取同期门诊体检的180例健康人员作为对照组,均进行脑CT灌注成像检查,比较两组平扫、增强扫描后颅脑各脑叶CT值。结果 (1)两组左顶叶、右顶叶、左颞叶、右颞叶、左枕叶、右枕叶平扫后CT值、TIP、MTT、rCBF、rCBV差异无统计学意义(P0.05),观察组左额叶、右额叶平扫CT值均低于对照组(P0.05)。(2)两组左顶叶、右顶叶、左颞叶、右颞叶、左枕叶、右枕叶增强扫描后CT值、TIP、MTT、rCBF、rCBV差异无统计学意义(P0.05),观察组左额叶、右额叶增强扫描CT值均低于对照组(P0.05)。结论首发抑郁症患者左右额叶脑组织密度CT值均低于正常对照,可能存在大脑额叶功能改变,对首发抑郁症诊断具有一定的临床价值。  相似文献

目的探讨AD患者脑血流灌注的特征性变化。方法选择20例确诊AD患者和10例认知正常者,对所有入组对象行SPECT脑血流灌注显像检查,对所得图像进行视觉分析和半定量分析,比较两组对象各脑区血流灌注情况的差异,并比较AD患者左、右脑叶脑血流灌注的差异。结果 (1)视觉分析结果显示,AD组以颞叶或顶叶脑血流灌注减低为主,各占55%,其中双侧颞顶叶血流灌注减低者占15%,单侧颞顶叶血流灌注减低占20%。正常认知组SPECT影像学表现各异,额叶、颞叶、顶叶、枕叶、丘脑、基底节血流灌注减少情况均存在,各占10%。但无双侧颞顶叶血流灌注减少者,40%表现完全正常。(2)半定量分析结果显示,AD组在右额叶、双侧颞顶叶、右枕叶血流灌注显著低于正常认知组(P<0.05),以右侧颞叶血流灌注降低最明显,左右颞顶叶血流低灌注程度对比无显著性差异。结论 AD患者的SPECT特征性表现为双侧对称性颞顶叶血流低灌注,其中右侧颞叶血流灌注下降最严重。  相似文献

目的 探讨强迫症患者各脑叶的代谢状态及脑功能异常与其发病的关系。  方法 采用单光子发射计算机扫描 (SPECT)技术 ,对未服药的 2 2例强迫症患者于静息及症状诱发状态下行局部脑血流 (rCBF)动态显像研究。  结果  1 8例有rCBF异常 ,表现为皮层内局限性放射性分布稀疏、缺损区 ,异常脑叶主要为顶叶、额叶、颞叶 ,并发现脑功能异常与强迫症状的严重程度及药物治疗反应有关。  结论 大脑顶叶、额叶等脑叶的功能异常是部分强迫症发病的神经病理学基础  相似文献

目的 探讨强迫症(OCD)患者局部脑血流灌注(rCBF)特点及临床症状与rCBF的关系。方法 对2 8例符合国际疾病分类第1 0版(ICD 1 0 )强迫障碍诊断标准患者和1 5名正常人进行单光子发射计算机断层扫描(SPECT)脑显像分析。用耶鲁 布朗强迫评定量表(YBOCS)、Hamilton焦虑量表(HAMA)和Hamilton抑郁量表(HAMD)对患者临床症状进行评定。结果 OCD患者两侧丘脑、顶叶和基底神经节的平均放射性计数百分数(PMRC)显著高于正常组;右颞PMRC明显低于正常组;且正常组两侧颞叶和顶叶PMRC值差异非常显著。在强迫恐惧/洗涤、回避组,YBOCS强迫行为分量表评分与右基底PMRC显著正相关(r =0 .70 1 )。结论 OCD患者两侧丘脑、顶叶和基底神经节的血流灌注增加和功能亢进;右颞血流灌注减少和功能低下,两侧颞叶和顶叶血流灌注的不对称性与正常人显著不同,后者的不对称性非常显著;在强迫恐惧/洗涤、回避组,强迫行为与右基底节功能增强有关。  相似文献

目的用单光子发射计算机断层扫描(SPECT)评价复智散对阿尔茨海默病(AD)患者局部脑血流灌注影响。方法对AD患者24例(AD组)、健康对照者12名(NC组)进行神经量表测定,AD组复智散治疗前和规范治疗40d后行SPECT评价局部脑血流和MMSE、ADAS-noncog神经量表测定。结果复智散能改善AD患者所观察脑区的脑血流灌注(P<0.05),以顶叶、额叶和扣带回区尤为明显(P<0.01),并能改善记忆力、语言、定向力及行为和情感等临床症状(P<0.01)。结论复智散可通过提高AD患者局部脑血流灌注而改善其认知和非认知功能。  相似文献

目的 探讨血管性痴呆 (VD)患者 SPECT局部脑血流灌注显像特点 ,为 VD的早期诊断和痴呆严重程度的评估寻找客观的生物学指标。方法 分别对 45例 VD、3 0例卒中无痴呆 (SWD)和 3 0例正常对照者 (NC)3组进行 SPECT局部脑血流灌注显像 ,半定量分析各脑区血流灌注情况。结果  VD组额叶、顶叶、颞叶和基底节局部脑血流灌注比 SWD组减少 (P<0 .0 5 ) ;并以额叶、颞叶血流灌注的减少最为显著 (P<0 .0 1)。与 NC组相比 ,SWD组额叶、顶叶、颞叶和基底节局部脑血流灌注均减低 (P<0 .0 5 )。额叶、颞叶血流灌注减低与 MMSE评分间存在正相关关系 (r=0 .75 5 ,P<0 .0 1)。结论  VD患者存在明显的脑血流灌注减低 ,以额叶、颞叶最为显著 ,且与MMSE评分间存在正相关 ,SPECT脑血流灌注显像有助于 VD的诊断和病情评估  相似文献

Data on functional imaging of bipolar disorder (BD) utilizing single photon emission computerized tomography (SPECT) is limited. This study assessed regional cerebral blood flow (rCBF), using 99mTc-ECD SPECT, among patients with BD, with mania (N=10) or depression (N=10), compared with 10 patients with unipolar depression and 10 normal controls. Regions of interest were analysed using a semi-automatic brain quantification programme. Compared to controls, patients with mania had significantly reduced perfusion mainly in the left frontal area, also in the left anterior cingulate and parietal cortices; those with bipolar depression had significantly lowered rCBF principally in the anterior temporal regions bilaterally, as well as the left parietal area. Patients with unipolar depression had significantly lowered perfusion than controls in most of the regions examined, chiefly in the anterior temporal and frontal cortices bilaterally; they also had lowered perfusion in the right anterior temporal and frontal areas, as well as the right middle temporal area and the right thalamus, compared to patients with mania. Increased severity of psychotic symptoms was associated with reduced rCBF in patients. These results indicate that altered blood flow in the frontal-subcortical systems characterises patients with BD, as well as those with unipolar depression.  相似文献

Background

Studies of mental disorders using single photon emission computed tomography (SPECT) have been done for many years in China. Many results have been obtained. We review these findings and introduce them to the outside world.

Methods

SPECT papers available on the Chinese Biomedical Bibliographic Database, focusing on depression, schizophrenia, Alzheimer’s disease (AD), vascular dementia (VD), anxiety disorder, and obsessive compulsive disorder (OCD) in China, were reviewed and the results were compared with those obtained outside China.

Results

We found that regional cerebral blood flow (rCBF) was abnormal in mental disorders, but the specificity of the abnormality is not yet consistent. Lower perfusion of rCBF could be seen in frontal, temporal, and parietal lobes of patients with depression, AD, schizophrenia, and VD. It seems that abnormality of the frontal lobe is more common in depression and schizophrenia, but temporal lobe abnormalities are more common in AD and VD. The perfusion of rCBF in the parietal lobe seems to be related to aging. Abnormalities in the occipital lobe and basal ganglia seem to be more associated with vascular problems. Thalamic dysfunction was mainly correlated with VD, and that of the cingulate largely with depression and schizophrenia. Hippocampal abnormalities were associated with AD. There were few reports on changes in anxiety disorders and other mental problems.

Conclusion

There is no specific biological marker of SPECT for individual mental disorders. Further study is needed to provide more specific information on the pathophysiology of mental disorders. It seems that brain abnormalities are similar in Chinese and non Chinese psychiatric patients.  相似文献

Summary. Objective: To evaluate the patterns of regional cerebral blood flow (rCBF) in cortical and subcortical regions by Brain SPECT imaging, in children and adolescents with obsessive-compulsive disorder (OCD) before and after treatment.Method: Fourteen OCD patients (6 to 17 years old) underwent brain SPECT; ten of those subjects were reexamined after successful treatment. rCBF ratios were correlated with clinical parameters on the 14 patients in symptomatic state, and we compared rCBF ratios of the ten patients before and after treatment.Results: There was no statistically significant difference in average ratios of rCBF before and after treatment. There were significant clinical correlations between current age and age of onset of OCD and rCBF in the bilateral superior frontal, and bilateral parietal cortical regions.Conclusions: Further investigations on abnormal neurodevelopment of cortical-subcortical circuits possibly involved in symptomatology of paediatric OCD are warranted.  相似文献

A large number of studies have demonstrated that depression patients have cognitive dysfunction. With recently developed brain functional imaging, studies have focused on changes in brain function to investigate cognitive changes. However, there is still controversy regarding abnormalities in brain functions or correlation between cognitive impairment and brain function changes. Thus, it is important to design an emotion-related task for research into brain function changes. We selected positive, neutral, and negative pictures from the International Affective Picture System. Patients with major depressive disorder were asked to judge emotion pictures. In addition, functional MRI was performed to synchronously record behavior data and imaging data. Results showed that the total correct rate for recognizing pictures was lower in patients compared with normal controls. Moreover, the consistency for recognizing pictures for depressed patients was worse than normal controls, and they frequently recognized positive pictures as negative pictures. The consistency for recognizing pictures was negatively correlated with the Hamilton Depression Rating Scale. Functional MRI suggested that the activation of some areas in the frontal lobe, temporal lobe, parietal lobe, limbic lobe, and cerebellum was enhanced, but that the activation of some areas in the frontal lobe, parietal lobe and occipital lobe was weakened while the patients were watching positive and neutral pictures compared with normal controls. The activation of some areas in the frontal lobe, temporal lobe, parietal lobe, and limbic lobe was enhanced, but the activation of some areas in the occipital lobe were weakened while the patients were watching the negative pictures compared with normal controls. These findings indicate that patients with major depressive disorder have negative cognitive disorder and extensive brain dysfunction. Thus, reduced activation of the occipital lobe may be an initiating factor for cognitive disorder in depressed patients.  相似文献

1. The objective of this study was to compare the relative regional cerebral blood flow (rCBF) patterns of a group of adolescents with major depressive disorder (MDD) to a group of normal controls. 2. Seven adolescent patients with symptomatic MDD and 7 age- and gender-matched normal controls, underwent SPECT imaging with 99mTc-HMPAO while unmedicated and in a resting state. These subject's data were normalized to whole brain counts, oriented in Talairach space, and analyzed using a voxel-based, t-image approach. 3. The authors found relative rCBF increases in the depressed group as compared to normals in the right mesial temporal cortex, the right superior-anterior temporal lobe, and the left infero-lateral temporal lobe. We found rCBF decreases in the depressed group as compared to normals in the left parietal lobe, the anterior thalamus and the right caudate. 4. Adolescents with MDD show rCBF abnormalities similar to those found in adult MDD rCBF studies. Further controlled studies with larger numbers of MDD subjects and normal age- and gender-matched controls are necessary before any definitive conclusions can be made from these findings.  相似文献

Abstract We examined patterns of regional cerebral blood flow (rCBF) abnormalities in 18 patients with major depressive disorder in late life using single photon emission computed tomography (SPECT) and (99mTc-hexamethyl-propylenamine oxime (99mTc-HMPAO). Compared with 13 age-matched controls, relative rCBF was significantly decreased bilaterally in the anterior cingulate gyrus, the prefrontal cortex, the temporal cortex, the parietal cortex, the hippocampus and the caudate nucleus. However, it was not correlated with the severity of depression or global cognitive dysfunction. In 10 patients with a prolonged depressive episode or prolonged residual symptoms (the refractory subgroup), robust and extensive decreases in rCBF were found compared with controls and the rCBF decreased significantly in the anterior cingulate gyrus and the prefrontal cortex compared with that in the non-refractory subgroup. In the non-refractory subgroup, rCBF decreased significandy in the caudate nucleus and tended to decrease in the anterior cingulate gyrus compared with controls. These findings indicate that dysfunction of the limbic system, the cerebral association cortex and the caudate nucleus may be implicated in late-life depression and that robust and extensive hypoperfusion, especially in the anterior cingulate and the prefrontal regions, may relate to refractoriness or chronification of depression.  相似文献
