Objective: To investigate the association between secondhand smoke exposure (SHSe) and asthma symptoms, medication use, and emergency department (ED)/urgent care (UC) utilization among adolescents. Methods: We performed a secondary cross-sectional analysis of Population Assessment of Tobacco and Health Study Wave 2 (2014–2015) including asthmatic adolescents (N = 2198). Logistic regression models and Poisson regression models were built. Results: Participants with SHSe ≥1 hour in the past 7 days were at increased risk of reporting shortness of breath and harder to exercise aOR, 1.22; 95% CI, 1.04–1.43), wheezing (aOR, 1.26; 95% CI, 1.01–1.56), wheezing disturbing sleep (aOR, 1.88; 95% CI, 1.35–2.63), wheezing during/after exercise (aOR, 1.41; 95% CI, 1.19–1.66), wheezing limiting speech (aOR, 2.11; 95% CI, 1.55–2.86), dry cough at night (aOR, 1.86; 95% CI, 1.54–2.24), and asthma symptoms disturbing sleep (aOR, 2.25; 95% CI, 1.81–2.79). Participants with SHSe ≥1 hour were more likely to take asthma medications (aOR, 1.25; 95% CI, 1.03–1.52), including steroids (aOR, 1.86; 95% CI, 1.19–2.91), oxygen therapy (aOR, 2.88; 95% CI, 1.82–4.54), and controlling medications (aOR, 1.50; 95% CI, 1.24–1.82). Symptoms and medications varied by living with a smoker and home SHSe. Participants with SHSe were at increased risk of having a higher number of asthma attacks that required steroid use. Participants who lived with a smoker and had home SHSe were at increased risk of having higher ED/UC visits for asthma. Conclusions: SHSe reduction efforts are needed for asthmatic adolescents, and EDs/UCs are promising venues.  相似文献

Objective To estimate the economic burden of hypertension in a given year in rural Yunnan Province of China, including direct, indirect and intangible costs. Methods A prevalence‐based cost‐of‐illness method was used to estimate the economic burden of hypertension. Data on participants’ demographic characteristics, inpatient hospitalisation expenditures, outpatient visit expenditures, self‐medication costs and indirect costs related to hypertension were collected from a cross‐sectional health examination and questionnaire survey, involving 9396 consenting individuals aged ≥18 years and 3500 households. Blood pressure (BP) levels were determined from the average of three BP measurements. Years of life lost (YLL) because of hypertension was estimated using medical death certificates. Results The overall prevalence of and YLL/1000 population because of hypertension was 24.8% and 1.5 years for the survey population, respectively. Mean unit direct medical costs, direct non‐medical costs, morbidity costs, mortality costs, intangible costs and cost of illness were estimated to be $467.2, $20.1, $23.5, $8265.1, $417.4 and $9393.3, respectively. The total cost of hypertension was estimated to be $231.7 million. Direct costs represented the largest component of economic cost of hypertension. On average, males had higher overall direct, indirect and intangible costs of hypertension than females. Both indirect and intangible costs decreased with age, whereas direct costs increased with age. The incidence of household catastrophic health payment and household impoverishment because of hypertension was 8.9% and 4.1%, respectively. Conclusions Hypertension inflicts a considerable economic burden upon individual households and society as a whole in Yunnan Province, China.  相似文献

目的分析戊型肝炎的疾病负担,为卫生决策提供科学依据。方法采用问卷调查方式,对江苏省东台市农村人群戊型肝炎主动监测网络收集的病例进行调查,获得戊型肝炎的疾病负担情况。结果戊型肝炎的总经济负担为2 910 362.60元,人均经济负担为11 641.45元,是东台市农村居民人均纯收入的1.26倍;失能调整生命年为28.94人年,患者的负担强度为115.78人年/千人。结论戊型肝炎的疾病负担较重,应加大防制力度。  相似文献

AIMS: Earlier studies have suggested that the cue-induced urge to smoke depends on the expectation of the availability of smoking. The present study investigated whether a 'room context' change could undo the learned discrimination between two stimuli, respectively, predicting smoking availability or smoking unavailability. DESIGN: A 2 (smoking cue) x 2 (availability context cue) x 6 (trial) x 2 (room context change) within-subjects design was used. Participants were repeatedly presented with a context cue predicting smoking availability (blue serving tray) and a context cue predicting unavailability (yellow serving tray) in one room and tested for an effect of context change in a different room. SETTING: Two distinct rooms located in different department buildings of Maastricht University. PARTICIPANTS: Seventeen daily smokers who had smoked at least five cigarettes a day for at least 2 years. MEASUREMENTS: Self-reported urge to smoke using a visual analogue scale (VAS). FINDINGS AND CONCLUSIONS: Results replicated the finding that a context cue that predicted smoking elicited greater urges to smoke than a context cue that predicted no smoking, irrespective of the presence of smoking cues. In addition, this study showed that this differential effect on the urge to smoke was generalized to a context other than the context in which learning took place. These findings are discussed in relation to the significance of a context change regarding the predictive value of smoking availability.  相似文献

Cysteinyl leukotrienes promote airway inflammation, bronchoconstriction and mucus hypersecretion. Cigarette smoking and respiratory syncytial virus (RSV) bronchiolitis are known to increase urinary cysteinyl leukotriene E4 (uLTE4), the end product of the cysteinyl leukotriene biosynthetic pathway. We tested the following hypotheses: (1) Secondhand smoke (SHS) exposure increases uLTE4 in well infants and in those hospitalized for RSV bronchiolitis; (2) Length of hospital stay for those with RSV bronchiolitis correlates with uLTE4; and (3) Infants with parent(s) with asthma will have higher uLTE4. Parental asthma for infants hospitalized with RSV bronchiolitis (n = 79) and Well babies (n = 31) was determined by questionnaire. Urine was analyzed for LTE4, cotinine, and creatinine. SHS exposure was determined by cotinine to creatinine ratio. Chi square, or t-tests were used to determine significant differences between two groups. A three-way analysis of variance compared the effects of SHS exposure and parental asthma on uLTE4 in Well versus RSV babies. Independent variables predicting length of hospital stay were determined by stepwise multiple regression. High SHS exposure and RSV significantly increased uLTE4. The SHS induced increase in uLTE4 was seen in infants with no parental asthma but not in those with parental asthma. Length of hospital stay positively correlated with uLTE4. We concluded that SHS exposure may increase the severity of bronchiolitis in RSV-infected infants by enhancing production of cysLTs in infants with no parental asthma.  相似文献

About one-third of the world's tobacco is produced and consumed in China. Despite existing tobacco control policies and activities, the prevalence of smoking in China remains high with 350 million smokers and 740 million passive smokers. Furthermore, smoking rates in the young population and in females are increasing. The number of deaths attributed to tobacco use has reached 1.2 million per year, whereas the death toll is expected to rise to 2 million annually by 2025. Sociocultural factors favouring smoking initiation, lack of awareness among the public about the hazards of smoking, weak support from the government and strong resistance from the tobacco industry are major reasons for the lack of effectiveness of current tobacco control measures. Effective intervention efforts are urgently required. Commitments from the government are crucial in tobacco control. Firm action should be taken on tobacco control issues at multiple levels including a reduction in tobacco supply, increased tobacco taxation, increased education, tobacco advertising limitations, decreased second-hand smoke exposure and smoking cessation support. The health-care community should also play a leading role in anti-tobacco campaigns and take a more active role in smoking cessation programmes.  相似文献

Objectives: Prenatal and postnatal smoke exposures are associated with many lung diseases in children due to impaired lung function, increased inflammation, and oxidative stress. We aimed to determine the influence of secondhand tobacco smoke exposure on the levels of nasal glutathione, IL-8, IL-17, MMP-9, and TIMP-1, as well as serum surfactant protein-D (SP-D) in wheezy children. Methods: We enrolled 150 children with recurrent wheezing and recorded wheezing characteristics at enrollment. We measured the levels of serum cotinine, SP-D, nasal glutathione, IL-8, IL-17, MMP-9, and TIMP-1. Serum cotinine levels between 3 and 12 ng/mL, and above 12 ng/mL were defined as lower and higher level secondhand tobacco smoke exposure, respectively. The ANOVA test, Pearson's correlation analysis and multivariate analysis with a linear regression test were used for the statistical analysis. Results: Ninety-one children had been exposed to lower level secondhand tobacco smoke, while 24 children were exposed to higher level secondhand tobacco smoke. Thirty-five children were not exposed to cigarette smoke. Wheezing symptom scores were higher in exposed children (p = 0.03). Levels of other biomarkers showed no significant difference. Conclusions: Secondhand tobacco smoke exposure is associated with more severe respiratory symptoms in wheezing children. However, levels of nasal or serum inflammatory markers fail to explain this association, either because of different mechanical factors in the process or due to low levels of the biomarkers especially in nasal secretions.  相似文献

Introduction: Smoking habits among university students in Lebanon are not clearly identified, and studies correlating these habits to asthma and allergic diseases are lacking. The primary objective of this study is to assess asthma and allergic diseases' predictors, particularly cigarette and/or waterpipe smoking and other environmental exposures, among university students. A second objective is to evaluate the potential role of these predictors as correlates of health-related self-assessment. Methods: A cross-sectional study, using a proportionate cluster sample of 3000 Lebanese students in both public and private universities, was conducted between January 2015 and December 2015. Results: The number of smokers at home, living close to an electricity generator and exposure to sand and dust significantly increased the odds of having asthma or allergic diseases (p = 0.015; OR = 1.183; p = 0.01; OR = 2.062; p = 0.001; OR = 3.558 respectively). Having tried cigarette smoking and having an air conditioner inside the means of transportation would decrease the odds of having asthma or allergic diseases by around 68% and 56.1% respectively (p = 0.009; ORa = 0.320; p = 0.01; ORa = 0.439 respectively). Conclusion: Although students with asthma or allergic diseases tended to avoid all identifiable atopic risk factors, lesser known environmental factors such as living close to an electricity generator, exposure to sand and dust, and exposure to car exhaust fumes were associated with a higher risk of asthma and a decrease in health related self-assessment. Students with asthma and allergic diseases smoked cigarettes and waterpipe at similar percentages, but cigarette smokers had a lower health related self-assessment.  相似文献

Background: Exposure to secondhand tobacco smoke (SHS) can be a risk factor for chronic obstructive pulmonary disease (COPD), but its role among relatively heavy smokers with potential co-exposure to workplace vapors, gas, dust, and fumes (VGDF) has not been studied. Methods: To estimate the contribution of SHS exposure to COPD risk, taking into account smoking effects and work-related exposures to VGDF, we quantified SHS based on survey responses for 1400 ever-employed subjects enrolled in the COPDGene study, all current or former smokers with or without COPD. Occupational exposures to VGDF were quantified based on a job exposure matrix. The associations between SHS and COPD were tested in multivariate logistic regression analyses adjusted for age, sex, VGDF exposure, and cumulative smoking. Results and Discussion: Exposures to SHS at work and at home during adulthood were associated with increased COPD risk: odds ratio (OR) = 1.12 (95% confidence interval [CI]: 1.02–1.23; p = 0.01) and OR = 1.09 (95%CI: 1.00–1.18; p = 0.04) per 10 years of exposure adjusted for smoking and other covariates, respectively. In addition, subjects with employment histories likely to entail exposure to VGDF were more likely to have COPD: OR = 1.52 (95%CI: 1.16–1.98; p < 0.01) (adjusted for other covariates). While adult home SHS COPD risk was attenuated among the heaviest smokers within the cohort, workplace SHS and job VGDF risks persisted in that stratum. Conclusion: Among smokers all with at least 10 pack-years, adult home and work SHS exposures and occupational VGDF exposure are all associated with COPD.  相似文献

目的探讨香烟暴露及戒烟对大鼠肺部炎症的影响。方法 40只健康雄性SD大鼠随机分为对照组、吸烟组(1月、2月组)、戒烟组(1月、2月组)。每组各8只,吸烟组及戒烟组大鼠每天上下午在自制熏烟盒中各给予烟熏两次,吸烟组在分别吸烟1月、2月后取材;戒烟组在给予2月烟熏后停止,分别正常饲养1月、2月取材。活杀大鼠并通过肺组织HE染色观察气道炎症改变、收集支气管肺泡灌洗液(BALF),用酶联免疫吸附实验(ELISA)检测支气管肺泡灌洗液(BALF)中白细胞介素-8(IL-8)、谷胱甘肽(GSH)含量表达。结果吸烟组及戒烟组BALF中炎症指标IL-8浓度明显高于正常组,GSH浓度明显低于正常组,但戒烟后IL-8,渐减低,GSH渐升高,但仍不能达到正常。结论吸烟可致肺部炎症加重氧化反应,戒烟可减轻上述反应。  相似文献
