Objective:

To evaluate the diagnostic accuracy of three-dimensional (3D) enhanced T1 high-resolution isotropic volume excitation (eTHRIVE) shoulder MR for the detection of rotator cuff tears, labral lesions and calcific tendonitis of the rotator cuff in comparison with two-dimensional (2D) fast spin echo T2 fat saturation (FS) MR.

Methods:

This retrospective study included 73 patients who underwent shoulder MRI using the eTHRIVE technique. Shoulder MR images were interpreted separately by two radiologists. They evaluated anatomic identification and image quality of the shoulder joint on routine MRI sequences (axial and oblique coronal T2 FS images) and compared them with the reformatted eTHRIVE images. The images were scored on a four-point scale (0, poor; 1, questionable; 2, adequate; 3, excellent) according to the degree of homogeneous and sufficient fat saturation to penetrate bone and soft tissue, visualization of the glenoid labrum and distinction of the supraspinatus tendon (SST). The diagnostic accuracy of eTHRIVE images compared with routine MRI sequences was evaluated in the setting of rotator cuff tears, glenoid labral injuries and calcific tendonitis of the SST.

Results:

Fat saturation scores for eTHRIVE were significantly higher than those of the T2 FS for both radiologists. The sensitivity and accuracy of the T2 FS in diagnosing rotor cuff tears were >90%, whereas sensitivity and accuracy of the eTHRIVE method were significantly lower. The sensitivity, specificity and accuracy of both images in diagnosing labral injuries and calcific tendonitis were similar and showed no significant differences. The specificity of both images for the diagnosis of labral injuries and calcific tendonitis was higher than the sensitivities.

Conclusion:

The accuracy of 3D eTHRIVE imaging was comparable to that of 2D FSE T2 FS for the diagnosis of glenoid labral injury and calcific tendonitis of SST. The 3D eTHRIVE technique was superior to 2D FSE T2 FS in terms of fat saturation. Overall, 3D eTHRIVE was inferior to T2 FS in the evaluation of rotator cuff tears because of poor contrast between joint fluid and tendons.

Advances in knowledge:

The accuracy of 3D eTHRIVE imaging is comparable to that of 2D FSE T2 FS for the diagnosis of glenoid labral injury and calcific tendonitis of SST.Decreasing MR examination time is important for improving patient comfort and reducing motion artefact.1 Three-dimensional (3D) MRI with isotropic resolution can create multiplanar reformatted (MPR) images and enable the creation of similar sequences with different image planes, resulting in decreased scan time.24 3D isotropic MR can minimize partial volume artefact using thin slice thickness (SL), which can analyse lesions without interslice gaps.5 Recently, a new 3D imaging sequence called 3D enhanced T1 high-resolution isotropic volume excitation MR (eTHRIVE) was developed.6 The eTHRIVE is a 3D T1 weighted gradient sequence with dual half-scan and improved fat suppression that enables fast dynamic scanning with submillimetre in-plane resolution.7 Lee et al6 reported that eTHRIVE has the advantage of high-contrast resolution that reveals better diagnostic results through multiplanar reconstruction. These techniques were used to evaluate ligaments of the wrist within a shorter imaging time. However, to the best of our knowledge, no preliminary study has used eTHRIVE to evaluate shoulder joint pathology. Purpose

To compare the diagnostic accuracy of three-dimensional (3D) fast spin echo (FSE) intermediate-weighed (IW-3D) and 3D hybrid double-echo steady-state T1-weighted sequences (Hy-3D) and two-dimensional (FSE) images (2D) at shoulder MR arthrography (MRA).

Materials and methods

Institutional review board approval was obtained and informed consent was waived for this retrospective study. From September 2011 to October 2014, 102 patients who had undergone 1.5 Tesla MRA of the shoulder, including conventional 2D-FSE and IW-3D and Hy-3D images were included in our study. The mean interval between MRA and surgery was 21 days (range 2–70 days). MR images were retrospectively and independently reviewed by two experienced radiologists blinded to the clinical and surgical data. Supraspinatus tendon (SST), infraspinatus tendon (IST) and subscapularis tendon (SCT) tears, as well as antero-inferior, superior and posterior labral lesions were assessed, using surgery as the reference standard. Each reader’s performance in assessing rotator cuff and labrum abnormalities was evaluated using the area under the receiver operating characteristic curve (AUC) and 95% confidence intervals (CIs). The difference was evaluated using a univariate z test. The sensitivity, specificity, positive predictive values (PPV), negative predictive values (NPV) and accuracy (Acc) for all types of rotator cuff tears and labral lesions were calculated. A value of p < 0.05 was considered statistically significant. Inter-observer agreement was calculated using kappa statistics.

Results

The difference of diagnostic accuracy achieved was not significant (p > 0.05). In particular, differences in AUC values ranged from 0.002 (p = 0.98) to 0.014 (p = 0.82) as regards the comparison between 2D and IW-3D images, from 0.002 (p = 0.98) to 0.034 (p = 0.08) concerning the comparison between 2D and Hy-3D images and from 0.010 (p = 0.82) to 0.032 (p = 0.09) when comparing Hy-3D to IW-3D images. Accuracy values in evaluating RC lesions and labral lesions were 95.1, 92.1, 91.2, 93.1, 93.1 and 94.1% by reading 2D, Hy-3D and IW-3D images, respectively. The difference of diagnostic accuracy achieved using the datasets analyzed was not significant (p > 0.05). Inter-observer agreement was very good for each of the datasets that were evaluated, with near-perfect agreement for 2D dataset (k = 0.86), Hy-3D (k = 0.81) and IW-3D (k = 0.83).

Conclusions

The accuracy of IW-3D and Hy-3D images was not significantly higher than the 2D sequences in evaluating RC and labral lesions.

PURPOSE: To evaluate the accuracy of signal intensity (SI) analysis on unenhanced fat-suppressed T1-weighted MR images in the diagnosis of pancreatic disease and to compare subjective interpretation with different quantitative measurements. MATERIALS AND METHODS: The pancreas was evaluated in 159 patients (86 normal and 73 with pancreatic disease) with spoiled gradient echo (GRE) T1-weighted fat saturation MR images. The relative SI of the pancreas to liver and spleen was quantitatively measured using regions of interest (ROIs) and qualitatively assessed by two independent observers. RESULTS: The mean values between a normal and an abnormal pancreas with pancreas-liver ratios of 0.14 +/- 0.37 vs. -0.32 +/- 0.24, respectively, and pancreas-spleen ratios of 0.89 +/- 0.55 vs. 0.02 +/- 0.43, respectively, were significantly different (P < 0.001). The pancreas-liver SI ratio was significantly better than the pancreas-spleen ratio throughout the disease group (area under the receiver operating characteristic (ROC) curve +/- SD; 0.92 +/- 0.02 for pancreas-liver vs. 0.86 +/- 0.03 for pancreas-spleen, P < 0.01), and after excluding cases of acute pancreatitis (0.96 +/- 0.02 for pancreas-liver vs. 0.89 +/- 0.03 for pancreas-spleen, P < 0.01). There was no statistically significant difference between quantitative and qualitative analysis (area under the ROC curve +/- SD; 0.93 +/- 0.02 vs. 0.93 +/- 0.02 for the entire disease group; excluding acute pancreatitis 0.96 +/- 0.02 vs 0.97 +/- 0.02) for the diagnosis of pancreatic disease when using liver as internal standard. The interobserver concordance was very good (kappa > 0.71). The sensitivity of visual liver comparison was 80% in the entire disease group and 91% after the cases of acute pancreatitis were excluded, while specificity was 93%. CONCLUSION: The pancreas-liver ratio is the best quantitative means of distinguishing normal from abnormal pancreas. Visual observation by experienced observers (qualitative measurement) was just as accurate as quantitative measurement. Detection of pancreatic pathology can be made with high accuracy by visually comparing the SI of the pancreas with that of the normal liver.  相似文献

MR arthrography of the shoulder: comparison with conventional MR imaging   总被引:5,自引:0,他引:5  
Twenty-three patients underwent both conventional MR imaging of the shoulder and MR shoulder arthrography for clinically suspected labral or rotator cuff abnormalities. Images obtained before and after contrast administration were studied independently, and without knowledge of clinical findings, by two radiologists for the presence of abnormalities of the glenoid labrum or rotator cuff. Results were correlated with surgical findings in all patients. Nine patients had surgically proved labral tears. MR arthrography detected all nine labral abnormalities, while six of the nine were missed on conventional MR imaging. Fourteen patients had surgically proved rotator cuff tears. MR arthrography detected 11 of the 14 tears and missed three partial tears on the bursal surface. Conventional MR imaging detected nine cuff tears and missed five tears; two of these were complete rotator cuff tears and three were partial tears of the undersurface of the rotator cuff. Our results suggest that MR arthrography enhances the accuracy of MR in the evaluation of the glenoid labrum and rotator cuff tendon.  相似文献

目的通过规范扫描时相、增加三维采集厚度及采用减影后处理技术,评价三维动态增强MR血管成像(3D DCE MRA)对显示门静脉高压症患者侧支循环的作用。方法30例门静脉高压症患者进行3D DCE MRA,其中12例采用75~90 mm的三维采集厚度,扫描时相以呼吸次数粗略估计,采集3-5次,18例采用150~180 mm三维采集厚度,扫描时相以秒表计时确定,分别于0、20、40、60、90 s采集5次。比较减影前后的最大强度投影(MIP)重组图像对门静脉系统血管主干及侧支的显示能力(以4级法评价),并统计患者显示的所有侧支血管及其显示的最佳时间。结果减影技术会降低门静脉主干的显示效果[减影前(2.74±0.31)级,减影后(2.53±0.49)级],差异有统计学意义(t=2.65,P<0.05),而提高食管胃底曲张静脉的显示[减影前(1.63±0.50)级,减影后(2.58±0.30)级],差异有统计学意义(t=-12.56,P<0.01)。门静脉主干与多数侧支循环为20 s左右显示最佳,个别侧支血管显影延迟。结论采用减影后处理技术能提高对门静脉高压症患者侧支血管的显示,规范的扫描时相不会遗漏显影较晚的侧支循环,增大采集厚度能保证前后腹壁侧支循环的显示。  相似文献

PURPOSE: To determine the usefulness of high-resolution three-dimensional (3D) constructive interference in steady state (CISS) MRI for evaluating mural invasion and morphologic features in esophageal carcinomas. MATERIALS AND METHODS: Twenty-four esophageal specimens with carcinomas were studied with a 1.5-T system using a 4-cm-diameter loop coil. High-resolution 3D-CISS MR images were obtained with a field of view (FOV) of 80 mm, matrix of 256 x 256, and section thickness of 0.5 mm (voxel size of 0.05 mm(3)). 3D-CISS MR images were compared with histopathologic findings, and virtual MR endoscopic images were compared with macroscopic findings at surgery. RESULTS: 3D-CISS MR images clearly depicted the normal esophageal wall as consisting of eight layers, which correlated well with the histologic layers. In 22 of 24 esophageal carcinomas (92%), the depth of mural invasion visualized with 3D-CISS MRI correlated well with the histopathologic staging. In all 24 carcinomas (100%), virtual MR endoscopic images clearly depicted the macroscopic types of the carcinomas, including adjacent lymph node swelling. CONCLUSION: High-resolution 3D-CISS MRI has a high diagnostic accuracy for evaluating mural invasion and macroscopic findings in esophageal carcinomas, and may be applicable to preoperative histopathologic staging and morphologic evaluation.  相似文献

OBJECTIVE: The purpose of this study was to assess the accuracy of routine T2-weighted MR imaging in detecting and grading articular cartilage lesions in the knee compared with arthroscopy. SUBJECTS AND METHODS: We examined 130 consecutive patients who underwent MR imaging and arthroscopy of the knee for suspected internal derangement. MR imaging consisted of axial and coronal T2-weighted fast spin-echo sequences with fat saturation and sagittal T2-weighted spin-echo sequences. Each single plane was evaluated and graded for the presence and appearance of articular cartilage defects using a standard arthroscopic grading scheme adapted to MR imaging. RESULTS: Of the 86 arthroscopically proven abnormalities, 81 were detected on MR imaging. Sensitivity of the T2-weighted fast spin-echo sequence with fat saturation was 61% for the coronal plane alone and 59% for the axial plane alone. Specificity for each plane was 99%. Sensitivity for the sagittal T2-weighted spin-echo sequence was 40%, and specificity was 100%. Sensitivity of the combination of axial and coronal T2-weighted fast spin-echo sequences with fat saturation and sagittal T2-weighted spin-echo sequence compared with arthroscopy for revealing cartilage lesions was 94%, specificity was 99%, and accuracy was 98%. Sensitivity of coronal and axial T2-weighted fast spin-echo sequences with fat saturation was 93%, and specificity was 99%. Fifty-five lesions (64%) were identically graded on MR imaging and arthroscopy. Seventy-eight lesions (90%) were within one grade using MR imaging and arthroscopy, and 84 lesions (97%) were within two grades using MR imaging and arthroscopy. CONCLUSION: T2-weighted fast spin-echo MR imaging with fat saturation is an accurate and fast technique for detecting and grading articular cartilage defects in the knee. The combination of the axial and coronal planes offers sufficient coverage of articular surfaces to provide a high sensitivity and specificity for chondral defects.  相似文献

PURPOSE: To compare fat suppression methods by using spectrally selective fat saturation and section-selective water excitation in standard magnetic resonance (MR) imaging sequences used in day-to-day musculoskeletal practice. MATERIALS AND METHODS: Eighty-three patients underwent MR examination with a 1.5-T system. The two methods were compared by using three common sequences: T1-weighted spin-echo (SE) imaging performed after contrast material injection (n = 24), intermediate-weighted fast SE (n = 36) imaging, and T2-weighted fast SE (n = 36) imaging. Acquisition times of the sequences and signal-to-noise and contrast-to-noise ratios of bone, muscle, fat, and water for the two methods were compared quantitatively. Images were then qualitatively reviewed by two radiologists who were blinded to the type of fat suppression used. Image quality was scored according to four criteria (homogeneity of fat suppression, susceptibility and foldover artifacts, conspicuousness of lesion, and overall image quality) by using a five-point scale (0, bad; 1, poor; 2, fair; 3, good; and 4, excellent). A paired Student t test was used to compare the quantitative data, and a nonparametric paired-data Wilcoxon signed rank test was used for qualitative analysis. RESULTS: Water excitation allowed a substantial decrease in acquisition time (by up to 50%) for T1-weighted sequences. Quantitative measurements revealed a greater signal-to-noise ratio (P <.01) with water excitation for all three sequences, whereas the contrast-to-noise ratio was greater with water excitation only in intermediate-weighted sequences (P <.01). Qualitatively, water excitation proved statistically better than or equal to fat saturation for all criteria in all imaging sequences (P <.05). Mean scores of overall image quality ranged between 2.5 and 3.0 for fat saturation and 3.4 and 3.7 for water excitation, respectively (P <.05). CONCLUSION: Section-selective water excitation is faster than conventional fat saturation and produces images of better quality.  相似文献

Magnetic resonance images were obtained from 32 patients with acute myocardial infarction, using a four-echo technique (echo time (TE) = 30, 60, 90, and 120 ms) pregadolinium(Gd)-DTPA injection and a TE = 30 ms sequence pre- and post-Gd-DTPA. Intensity ratios of infarcted and normal myocardium were calculated, as were contrast-to-noise and signal-to-noise ratios. The four intensity ratios pre-Gd-DTPA were 1.20 +/- 0.15, 1.42 +/- 0.22, 1.78 +/- 0.38, and 1.99 +/- 0.60 for TE = 30, 60, 90, and 120 ms, respectively, and 1.42 +/- 0.19 post-Gd-DTPA (p = NS for post-Gd-DTPA vs TE = 60, p = 0.007 for TE = 90 vs TE = 120, p less than 0.0001 for all other comparisons). The four contrast-to-noise ratios pre-Gd-DTPA were 1.69 +/- 0.97, 2.69 +/- 1.13, 3.17 +/- 1.15, and 2.90 +/- 1.09 for TE = 30, 60, 90, and 120 ms, respectively, and 2.71 +/- 1.26 post-Gd-DTPA (p = NS for post-Gd-DTPA vs TE = 60, 90, and 120, p = NS for TE = 120 vs TE = 60 and 90, p less than 0.01 for all other comparisons). The four signal-to-noise ratios pre-Gd-DTPA were 8.67 +/- 1.47, 6.52 +/- 0.76, 5.20 +/- 0.64, 4.17 +/- 0.53 for TE = 30, 60, 90, and 120 ms, respectively, and 9.17 +/- 1.92 post-Gd-DTPA (p = 0.03 for post-Gd-DTPA vs TE = 30, p less than 0.0001 for all other comparisons). In conclusion, the detectabilities of acute myocardial infarction were similar at TE = 60 ms and at Gd-DTPA enhanced short-TE MR imaging. However, image quality proved to be superior using the Gd-DTPA enhanced short-TE technique.  相似文献

The purpose of this study was to determine the usefulness of magnetic resonance (MR) arthrography for diagnosing adhesive capsulitis. Shoulder MR images of 28 patients with (n=14) and without (n=14) adhesive capsulitis were retrospectively analyzed. MR images were assessed for capsule and synovium thickness as well as the width of the axillary recess on oblique coronal fat-suppressed T1-weighted images and T2-weighted images, respectively. On oblique sagittal fat-suppressed T1-weighted images, the width of the rotator interval and the presence of abnormal tissue in the interval were evaluated. Significant differences were found between the two groups in capsule and synovium thickness on both sides of the recess on oblique coronal T2-weighted images (P=0.000), whereas thickness on the humeral aspect showed no significant difference on oblique coronal fat-suppressed T1-weighted images (P=0.109). On oblique coronal T2-weighted images, a cut-off value of 3-mm thickness gave the highest diagnostic accuracy for adhesive capsulitis with sensitivity, specificity, and accuracy of 79% (11/14), 100% (14/14), and 89% (25/28) at the humeral side and 93% (13/14), 86% (12/14), and 89% (25/28) at the glenoid side, respectively. There were significant differences in rotator interval width, presence of abnormal tissue in the rotator interval, and axillary recess width between the two groups (P<0.05). Thickness of capsule and synovium of the axillary recess greater than 3 mm is a practical MR criterion for diagnosing adhesive capsulitis when measured on oblique coronal T2-weighted MR arthrography images without fat suppression. The presence of abnormal tissue in the rotator interval showed high sensitivity but rather low specificity.  相似文献

PURPOSE: The aim of the study was to analyse the accuracy of MR arthrography in the evaluation of capsulo-labro-ligamentous lesions of the shoulder in patients with glenohumeral joint instability. MATERIALS AND METHODS: From 1999 to 2001 fifty-eight patients with glenohumeral joint instability were studied by MR arthrography. Twenty-seven patients underwent surgical repair: 11 by arthroscopic and 16 by arthrotomic approach. All shoulder evaluations were performed with T1 and T2 weighted axial, coronal and sagittal oblique images, before and after intra-articular injection of gadolinium contrast. RESULTS: Forty capsulo-ligamentous lesions (including 14 capsular ruptures with extravasation of the contrast medium) were detected by MR arthrography. Fifty-two labral tears (36 of the anterior, 13 of the superior and 3 of the posterior glenoid labrum) were identified: 11 out of 52 were not recognized before gadolinium contrast injection. Five rotator cuff tears were identified, one of which was not shown in the pre-injection examination. Surgical results confirmed the MR arthrographic findings in 25/27 patients. In one case MR arthrography did not recognize a SLAP lesion; in another case it identified a tear of the capsule but not of the glenoid labrum. CONCLUSIONS: In many cases of subacute glenohumeral joint lesions with intracapsular fluid, MR may accurately evaluate capsulo-labral-ligamentous lesions. Indeed, the examination of lesions is limited by the absence of the natural contrast determined by fluid; in such cases, intra-articular injection of gadolinium contrast is necessary. MR arthrography evaluates the degree of capsulo-labro-ligamentous tears and may guide the surgical approach.  相似文献
