Objectives

This study was performed to analyze the aging-related changes of the female condylar bone mineral density (BMD) and trabecular structure by cone-beam computed tomography (CBCT), and determine whether the condylar structure shows obvious changes after menopause.

Methods

The CBCT images of 160 female patients who met the inclusion criteria for the study were collected and divided into four groups by age (20–29 years, 30–39 years, premenopausal, and postmenopausal groups). Computer processing software CT-Analyser (Version 1.15.2.2+; SkyScan, Antwerp, Belgium) was used to measure the condylar BMD and related indexes, namely the bone volume/tissue volume ratio (BV/TV), trabecular number (Tb.N), trabecular thickness (Tb.Th), trabecular separation (Tb.Sp), trabecular structure model index (SMI), and bone surface area/volume ratio (BS/BV). SPSS 12.0 (SPSS Inc., Chicago, IL, USA) was used to analyze the radiographic findings and statistical differences.

Results

No significant differences were found between the bilateral condyles in each group (P?>?0.05). BV/TV, Tb.N, and Tb.Th of the condyle decreased with age, and the postmenopausal group showed significantly different values for each index compared with the other groups (P?<?0.01). Tb.Sp, SMI, and BS/BV of the condyle increased with age, and the postmenopausal group showed significantly different values for each index compared with the other groups (P?<?0.01).

Conclusions

With increasing age, the female condylar bone volume decreases, the Tb.N and Tb.Th decrease, the gap between the trabecular bone increases, and plate-like trabecular bone gradually transforms into a rod-like form. These changes are much more obvious in postmenopausal women.

J Oral Pathol Med (2011) 40 : 111–120 Background: In juvenile idiopathic arthritis involvement of the temporomandibular joints (TMJs) is often associated with mandibular growth deviations. The relation between the growth deviations and severity of the inflammation, condylar shape, the micro‐architecture, and the quality of the bone has not previously been investigated. This paper studies the effect on the bony structures in mandibular condylar development in rabbits with antigen‐induced arthritis. Methods: Included were 42 juvenile rabbits with ovalbumin‐induced arthritis of the TMJs treated with intraarticular saline, intraarticular etanercept or subcutaneous etanercept. A TMJ from each animal was scanned using micro‐computed tomography and structural parameters were calculated. Three‐dimensional reconstructions of the mandibular condyle were scored blindly as normal or abnormal. TMJs were stratified for condylar morphology and were evaluated against data on trabecular structural parameters, inflammation, degree of mineralization, overall mandibular growth, and mineral apposition rate. Results: Abnormal morphology were seen in 15/32 animals available for data analysis. Erosions were an uncommon finding. Abnormal morphology was strongly related to the degree of inflammation. The trabecular separation was larger in group with abnormal morphology than in the group with normal morphology. Abnormal condylar morphology was not associated with overall mandibular growth. No differences were observed in mineral apposition rate. No differences in structural parameters were seen according to treatment modality. Conclusion: We showed that severe inflammation in the TMJs during mandibular development was associated with morphological changes in the mandibular condyle. These changes were predominantly seen at the macro‐morphological level and only very few differences were structural.  相似文献

Forward mandibular displacement in animal models is associated with faster and/or redirected condylar growth. Here the effect of forward displacement induced with an intraoral appliance on modelling/remodelling of the mandibular condyle was investigated in eight, 4-month-old, castrated male Merino sheep, randomly allocated to experimental and control groups (n=4 in each group). The study period was 15 weeks, during that time, (1). calcein, (2). tetracycline, and (3). alizarin red S fluorochromes were given to all animals from day 1. Midsagittal sections of the temporomandibular joints were selected for analysis. Dynamic variables of bone formation, static indices of bone-forming and -resorbing activity, and structural indices of trabecular bone were estimated histomorphometrically. The sampling site was divided into two regions for analysis: (a). a 'subchondral region' (2 and 3 labels only), believed to be the bone newly formed during the experimental period; (b). a 'central region' (labelled by all three fluorochromes), believed to be the bone that existed before the experiment. Regional differences in adaptive response were found. In the experimental group, the bone-volume fraction (BV/TV) of the subchondral regions had decreased, although the specific bone-surface and bone-formation rates had increased. This low BV/TV was associated with decreased trabecular thickness and increased trabecular separation. In the central condylar region of the experimental group, BV/TV was unchanged, but an increased osteoid surface was apparent when the eroded surface was taken into consideration. These adaptive condylar responses to forward mandibular displacement appeared to be the result of increased osteoblastic activity. Further studies are recommended to examine why the subchondral and central regions responded differently.  相似文献

In the present study, we tested the hypothesis that the cancellous bone of the mandibular condyle is inhomogeneous and anisotropic. For this purpose, 11 mandibular condyles from embalmed human cadavers were scanned in a micro-CT system. Within each condyle, 9 volumes of interest were selected from different mediolateral and supero-inferior regions. Several bone parameters were calculated to describe the morphology. It appeared that the cancellous bone of the condyle could be approximated by parallel plates. These plates were almost vertically oriented at an angle of 17 degrees relative to the sagittal plane, i.e., perpendicular to the condylar axis. In the superior regions of the condyle, the cancellous bone had the largest bone volume fraction (0.19), associated with the thickest trabeculae (0.11 mm), and the highest trabecular number (1.72 mm(-1)). The lowest bone volume fraction (0.15) was found more inferiorly. The degree of anisotropy increased from superior to inferior across the condyle. No mediolateral differences in bone morphology were found, but superiorly central regions contained more bone than peripheral regions. The plate-like trabeculae could indicate that the condyle is optimally adapted to sustain loads from all directions in a plane perpendicular to the condylar axis. The high bone mass and lower anisotropy in the superior regions could enable the condyle to sustain multiple load directions. Toward the collum, the trabeculae are more aligned. This could point to stresses acting predominantly in one direction.  相似文献

Background: The aim of the study is to determine if bone quality evaluation of surgically obtained bone core specimens using a stereomicroscope is reliable for determining bone quality at implant recipient sites. Methods: Bone quality was presurgically assessed in 122 edentulous ridges obtained from 62 patients using periapical radiographs and categorized according to the Lekholm and Zarb classification. During surgery, bone specimens were trephined, and bone types were immediately classified using a stereomicroscope. Microarchitectural characteristics of bone cores were evaluated after being scanned using microcomputed tomography (micro‐CT). Results: Bone types of implant sites categorized from radiography and stereomicroscope had statistically similar distribution but poor interrater agreement. Using micro‐CT, maxillae and mandibles showed significant differences in microarchitectural characteristics of bone cores. Bone volume (BV), total volume (TV), and trabecular thickness (Tb.Th) increased, whereas bone surface density (BS/BV) and open porosity (Po.[Op]) decreased in mandibular bone cores compared with those in maxillary bone cores. Moreover, micro‐CT values of BV/TV and Po.(Op) statistically correlated with bone types assessed by stereomicroscopy, particularly in mandibles (adjusted means of BV/TV of Type 2 to 4 versus Type 1 decreasing from ?9.88%, ?15.09%, ?29.31%; those of Po.(Op) ranged from 9.77%, 15.06%, 29.52% in an upward trend). However, such correlations were not found in maxillae or when bone types were classified using periapical radiographs. Conclusions: Caution is needed when using presurgical periapical radiographs to predict bone quality at implant recipient sites. Surgically preserved bone core specimens, whenever obtainable, might offer additional information to accurately assess bone quality, particularly at mandibular implant sites.  相似文献

A rapidly growing postnatal animal model was used to study changes in the calcified tissue of the mandibular condyle during altered muscle function. A maxillary occlusal splint was designed to shift the mandible laterally (left) during closure. Groups of 5 Wistar rats were killed at 5, 9, 15, 21, 30, and 40 weeks (n = 30), with an equal number of controls. The experimental animals developed shorter, asymmetrical mandibles compared with the control animals. The left condyle became larger and thicker than the right condyle. Microcomputed tomography assessment of the left and right condylar trabecular bone indicated that both had less bone volume than the control condyle. The right masseter muscle significantly lost fiber size and type IIA oxidative fibers, suggesting that the right masseter muscle was used with less tension development. In contrast, the left masseter maintained its fiber size and was similar to the control masseter fiber diameters. Comparison in the sequence of changes indicated that the morphologic changes occurred first in the ramus (age, 5 weeks), before the corpus (age, 15 weeks), and before changes in masseter fiber size and composition (age, 9 weeks). This study showed that both the mandible and the condyle modified their shape and size, as well as the trabecular bone of the condyle, during shifting of the mandible to one side as it closed.  相似文献

It is suggested that osteopontin may promote osteoclast binding to resorptive sites by interacting with the αvβ3 receptor on osteoclasts. However, the role of osteopontin in functional remodeling of bony structures remains unclear. The present study was conducted to examine the distribution of osteopontin on the condyle and explore the role in condylar remodeling in growing rats using an immunohistochemical method. Twenty Wistar strain male rats aged 7, 14, 28 and 56 days were used. In 7‐ and 14‐day‐old rats, no immunoreaction to osteopontin was detected in the cartilage cells. In 28‐day‐old rats initiating mastication, the thickness of condylar cartilage was decreased abruptly as compared to the younger rats. High immunoreaction to osteopontin was found in the cytoplasm of hypertrophic chondrocytes and on the trabecular bone surfaces of primary spongiosa adjacent to the osteoclasts or chondroclasts. The immunoreactions to osteopontin in the cytoplasm of hypertrophic chondrocytes were less in 56‐day‐old rats than in 28‐day‐old rats. It is shown that the alteration in mechanical loading on the mandibular condyle due to functional changes from weaning to mastication correlates with the localization of osteopontin in growing rats. Furthermore, it is suggested that osteopontin may stimulate osteoclastic resorption of calcified matrix by mediating the attachment of osteoclasts and/or chondroclasts during growth‐related functional remodeling of the condyle.  相似文献

Temporomandibular joint (TMJ) displays a high remodelling capability. The current purpose was to investigate the differences between mandibular condylar remodelling responses of growing mice to installation and removal of unilateral anterior crossbite (UAC) prosthesis. Twenty‐four mice were divided into one mock control group and two UAC groups. Unilateral anterior crossbite was created by installing a pair of prosthesis to left‐side maxillary and mandibular incisors. Unilateral anterior crossbite was removed in removal group at 3 weeks but remained in UAC group. Temporomandibular joints were sampled at 7 weeks. Changes in condylar cartilage and subchondral bone were assessed by histology and in vivo micro‐CT. Real‐time PCR and immunohistochemistry were performed to evaluate expression changes in ADAMTS‐5, MMP‐3, MMP‐9, MMP‐13, IL‐1, TNF‐α, OPG and RANKL. Statistical analysis was performed at α = 0·05. Temporomandibular joint cartilage degradation was induced by UAC as previously reported but was reversed by removal of UAC. The dropped cartilage thickness, chondrocyte number and collagen II‐positive area, the increased expression levels of Adamts‐5, Mmp3, 9, 13, Tnf‐α and Il‐1β in cartilage, the decreased ratio of OPG/RANKL in both condylar cartilage and subchondral bone, the loss of TMJ subchondral bone and the increase in the TRAP‐positive cells in subchondral bone were all reversed in the removal group (P < 0·05). The growing mouse TMJ condyle displays a high remodelling capability which can be degenerative and rehabilitative, respectively, in response to placement and thereafter removal of the aberrant prosthesis. Eliminating aberrant prosthesis is helpful to promote the degraded condyle to recover.  相似文献

Background

Mandibular asymmetry is commonly caused by disproportionate mandibular growth due to unilateral condylar hyperactivity. The current standard for mandibular condyle bone scintigraphy uses single photon emission computed tomography (SPECT) imaging after intravenous administration of technetium-99m-labeled diphosphates. To our knowledge, the use of hybrid SPECT integrated with multidetector computed tomography (SPECT/CT) imaging in the scintigraphic quantitative functional assessment of mandibular condyle growth has not yet been described.

Case report

A 22-year-old male with mandibular asymmetry due to suspected unilateral condylar hyperactivity underwent SPECT/CT imaging of the skull and facial bones after intravenous administration of technetium-99m methylene diphosphonate. Using CT to guide anatomical contouring, precise regions of interest were drawn over the mandibular condyles and clivus in adjacent SPECT/CT transaxial slices to calculate the mean radiotracer count ratios. For comparison of quantitative results, conventional SPECT images were obtained from the SPECT/CT data and processed according to conventional methods. Planar images were also obtained for visual assessment of mandibular condyle activity. All three methods, SPECT/CT, conventional SPECT, and planar imaging, found unilateral condylar hyperactivity of the left mandibular condyle. The condyles and clivus were easily and confidently identified on SPECT/CT images.

Discussion

We speculate that SPECT/CT imaging will improve the diagnostic accuracy of unilateral condylar hyperactivity. More studies are necessary to further define its role, obtain baseline reference values, and establish analysis protocols.  相似文献

Osteochondroma is the most common benign tumor of the axial skeleton, though it is rarely found in the oral and maxillofacial regions. Reported is a case of osteochondroma affecting the mandibular condyle of a 22-year-old Japanese woman. The patient visited the hospital with the chief complaint of facial asymmetry. She had pain with clicking on her right temporomandibular joint (TMJ), resulting in trismus and facial asymmetry with a chin deviation to the left side. Her maximum jaw opening was 34 mm, with moderate left TMJ tenderness. There was Class I occlusion without open bite. Panoramic radiography and computed tomography (CT) revealed an enlarged right mandibular condylar head. Magnetic resonance imaging (MRI) also showed an enlarged condyle with hyperintense bone marrow on a T2-weighted image. Condylectomy and condyloplasty were performed. Surgical specimen of the lesion revealed osteochondroma of the mandibular condyle head. Two months after the initial surgery, facial asymmetry was surgically corrected by Le Fort I osteotomy and chin reduction. The patient was discharged from the hospital with no subjective complaints. At the time of this report, the patient had been followed up for seven months, and there had been no evidence of recurrence.  相似文献

目的:比较下颌骨髁突骨软骨瘤和单侧下颌骨髁突增生的CT表现特点。方法:对2005—2010年上海交通大学医学院附属第九人民医院口腔颌面外科收治的下颌骨髁突骨软骨瘤11例和单侧下颌骨髁突增生8例患者的CT影像学资料进行评价,评价指标包括病变髁突大小、病变范围以及病变周围软硬组织改变。结果:11例下颌骨髁突骨软骨瘤病例CT显示肿瘤与病变髁突无明显分界(8/11)或与患侧髁突有蒂相连(3/11);肿瘤骨皮质及骨髓腔均与患侧髁突相续,瘤体表面均有特征性薄层软骨帽覆盖,瘤体外周密度通常高于中心;瘤体周围均有薄层软组织包绕。患侧颞骨关节面表面均有明显矿化,且因受瘤体压迫改建而较对侧平坦,患侧关节上、下腔间隙较对侧明显变窄;肿瘤生长方向不尽相同。8例单侧下颌骨髁突增生病例CT显示髁突颈部和(或)下颌支延长,髁突形状改变;增生的髁突外周骨皮质均有不同程度的骨化,硬化层厚度较对侧大;骨髓腔密度较不均匀。结论:CT检查能有效提供病变髁突及其周围软硬组织情况,为鉴别诊断下颌骨髁突骨软骨瘤和单侧下颌骨髁突增生提供良好的依据。  相似文献

目的 :探索常规X线、二维和三维CT成像 3种影像学检查在下颌骨髁突骨折诊断中的应用价值。方法 :对 2 7例 38侧不同类型的髁突骨折的常规X线、二维和三维CT显示情况进行对比分析 ,并与手术结果相对照。结果 :常规X线对各种类型的髁突骨折均有显示不准确现象 ,横断位的二维CT对 3例髁突横行骨折有不准确显示 ,三维CT可清楚地显示各种类型的髁突骨折。结论 :常规X线对髁突各种类型骨折均有误诊 ,三维CT图像结合二维图像可准确显示各种类型的髁突骨折及移位情况  相似文献

There are case reports of sagittal fractures of the condylar head leading to bifid condyle. However bifid condyles maybe found in patients with no history of trauma. A split in the saggital plane of the condyle is not visible with a lateral, oblique or panaromic radiographs but only with anteriorposterior, transorbital projections or CT scan of the temperomandibular joint. The chances of condyle being split in the sagittal plane may be due to the medial pole extending beyond the condylar neck, moreover the condyle is composed of cancellous bone covered by a thin layer of cortical bone. Here we are presenting three case reports of Saggital split condyles and stress the need for inclusion of these type of fractures in the classification of condylar fractures.  相似文献

Fractures of the mandibular condyle are common and include diacapitular fractures that affect the condylar head. The medial part of the condylar head is least commonly fractured, possibly due to decreased propensity for lines of force to run in the region. Micro-computed tomography (X-ray microtomography) of five temporomandibular joint specimens was conducted to explore whether trabecular bone structure correlates positively with fracture prevalence, which could reflect adaptation in response to lower exposure to physiological loads throughout life. Models of trabecular bone, and graphic representation of bone density indicated least dense bone medially, but a statistically significant ANOVA result was not obtained. Further study is required to verify whether a relationship between bone microstructure and fracture frequency exists, and whether or not this is the product of association between the directions of physiological and traumatic forces.  相似文献

Oral Diseases (2011) 17 , 499–507 Objective: Injection of complete Freund’s adjuvant (CFA) into the temporomandibular joint (TMJ) causes acute swelling around the joint and subsequent morphological alterations in the condyle. We aimed to evaluate changes in the three‐dimensional architecture of the condyle induced with CFA. Materials and methods: The CFA was injected into the unilateral TMJ of rats and morphological changes in the condyle were assessed repeatedly for 14 days by in vivo micro‐CT. Results: Osseous abnormalities of condyle were first observed at 3–5 days after CFA injection on the tomographic images, and the condylar deformation became more obvious thereafter. Among 12 condyles examined at 14 days postinjection, osteophytosis was observed in all of the specimens and bone erosion coexisted in five condyles. None of the saline‐treated condyles showed architectural changes. Significant changes were detected in the mesiolateral and rostrocaudal widths of the CFA‐treated condyles at 10–14 days postinjection (P < 0.01). The extent of both condylar bone formation and resorption was greater in the CFA‐injected TMJs than in saline‐injected TMJs (P < 0.05). Conclusion: These results indicate that CFA causes dynamic morphological changes in the condyle and that our experimental approach will provide new insights into the subacute inflammatory processes in the TMJ.  相似文献

Variability in growth response of the mandible after replacement of the mandibular condyle with the costochondral junction of a rib (CCJ) is common. Other donor graft sites that are more similar to the mandibular condyle might be more suitable for mandibular condylar replacement. Previous studies have shown the histomorphologic and developmental similarities between the sternoclavicular joint (SCJ) and temporomandibular joint (TMJ). The purpose of this study was to evaluate histologically short-term adaptations within the TMJ after replacement of the mandibular condyle with the autogenous sternal head of the clavicle, and to compare these adaptations with autogenous CCJ and mandibular condyle (surgical control) transplants. Bilateral vertical ramus osteotomies were performed in 12 juvenile Macaca mulatta with the left condyle being immediately replaced and the right condyle removed and replaced with either the sternal head of the clavicle or costochondral junction of a rib. All grafts were stabilized with maxillomandibular fixation for five weeks. Two animals in each group were killed at five, 11, and 17 weeks postsurgery and prepared for histologic analysis. The results indicate that: 1) incorporation of all grafts into the recipient site occurred and all animals had good mandibular function and occlusion throughout the follow-up period; 2) the clavicular and mandibular condylar grafts were incorporated sooner than the CCJ grafts; 3) clavicular graft changes resembled those of the condylar grafts histologically while the costal grafts remained inert and unchanged; and 4) a new condylar process with cartilage similar to that of a normal mandibular condyle regenerated in the costal graft animals from cells presumably contributed by the periosteum of the mandibular ramus and TMJ capsule.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

PURPOSE: Distraction osteogenesis (DO) is commonly performed for mandibular reconstruction during the growth period. We tested the hypothesis that parathyroid hormone-related protein (PTHrP) in mandibular condylar cartilage and underlying trabecular bone in growing individuals undergo changes in response to distraction forces. MATERIALS AND METHODS: Forty-eight 6-week-old male Sprague-Dawley rats were used. Animals underwent unilateral mandibular distraction using a distractor that we devised, and unoperated animals were evaluated as controls. DO procedure was performed: 3 days' latency period, 0.4 mm/day rate, total 4.0 mm. Changes in cartilage morphology, PTHrP activity, and 3-dimensional trabecular bone structure changes measured by micro-computed tomography were examined at 0, 2, 4, and 6 weeks of consolidation. RESULTS: A marked irregularity was noted in the superior portion of the distracted side's condylar cartilage that resolved after distraction ceased. PTHrP was more strongly expressed in the hypertrophic layer of condylar cartilage on the distracted side than in controls, up to 6 weeks after the end of distraction. Subchondral trabecular bone volume, percent bone volume, and trabecular number in the superior and posterior regions of the condyle decreased significantly by 2 weeks after distraction. These parameters returned to normal in the posterior condyle, but not in the superior part of the condyle by 6 weeks following distraction. CONCLUSION: These results suggest that unilateral mandibular distraction in growing rats causes temporary morphologic alterations of trabecular bone structure on the distracted side accompanied by increased production of PTHrP in the mandibular condyle.  相似文献

OBJECTIVE: To examine the effects of mandibular lateral shift on the growth and morphology of the condyle and mandibular bone in growing rats. MATERIALS AND METHODS: A maxillary resin plate was designed to displace 4-week-old rat mandibles 2 mm to the left during closure. The plate was cemented, and the rats were killed after 2, 4, 8, or 12 weeks. Gross dimensions of the condylar head were measured. Radiographic films of the mandibles were exposed, and selected measurements were made. The newly formed bone in the condyle was evaluated with periodic acid and Schiff's reagent (PAS) staining. RESULTS: The length of the condylar head was greater on the ipsilateral side compared with the contralateral side. The experimental rats developed an asymmetric mandible, shorter in horizontal dimension but longer in the vertical dimension on the ipsilateral side. The mandibular growth direction was also affected, with the mandible on the ipsilateral side growing in a more anterior and superior direction. The amount of newly formed bone was greater in the superior subchondral region but lower in the posterior subchondral region on the ipsilateral side compared with the contralateral side. CONCLUSIONS: Mechanical stimuli delivered by a functional shift produce a series of morphological and histological responses in the condyle and lead to condylar and mandibular asymmetry in rats.  相似文献

It is well known that the size of the condyle is different between males and females in humans. It has also been documented that ovariectomy (OVX) and orchiectomy (ORX) influence the remodeling of the condylar head in experimental animals. However, influences of sex hormones on the condyle in terms of its breadth have not been examined. With these considerations, this study was designed to examine the breadth of the mandibular condyle in OVX and ORX mice by histomorphometry 8 weeks after surgery. In the OVX mice, the condylar breadth was significantly larger than in the controls, while no significant differences in the breadth were found between the ORX mice and the controls. The thickness of the condylar cartilage layers exhibited similar findings to the breadth of the condyle. On the contrary, the trabecular bone volume was significantly smaller in the OVX and ORX mice than in the corresponding controls, and the difference was more substantial in female mice than in the males. These findings emphasize that a reduction of sex hormone levels, induced by OVX and ORX, significantly influences condylar morphogenesis.  相似文献

目的:建立髁突囊内矢状骨折动物模型,观察髁突骨折后不同时间的病理形态改变。方法:对10只绵羊进行实验,右侧关节为手术侧,从耳前区切开皮肤,到达颞下颌关节,切断关节盘前后附着,从髁突外侧嵴到内侧髁颈锯开髁突,建立斜形矢状骨折。髁突碎片和关节盘推向前下内侧,左侧关节为对照侧。分别于术后1周、4周和12周处死2只、4只和4只动物。记录术前和术后每组动物的体质量、最大开口度、侧方活动度,进行X线、CT和组织病理学观察。采用SPSS12.0软件包对数据进行统计分析。结果:各手术组体质量、左侧向活动度未发现显著差异,但是在4周和12周组中,最大开口度和右侧向活动度明显减小。在这2组中,X线检查发现髁突残端外侧有新骨生成,并向外侧生长,同时关节间隙变狭窄和模糊。三维CT检查发现,髁突残端、髁突碎片和关节结节均发生改建。组织病理学观察发现,在4周和12周组动物,关节腔充满纤维组织、软骨和骨组织。结论:该研究表明,此类骨折可以导致髁突骨关节病理改变,颞下颌关节有发展为关节强直的可能。  相似文献
