Cancer incidence of workers in the Finnish pulp and paper industry   总被引:3,自引:0,他引:3  
The cancer incidence of 3,545 workers in the Finnish pulp and paper industry was assessed in a retrospective cohort study. The cohort included workers with continuous employment of at least one year between 1 January 1945 and 31 December 1961 and was followed until 31 December 1980. Six subcohorts were formed (sulfite mill, sulfate mill, paper mill, board mill, maintenance department, and power plant). Separate analyses were made for the 2,597 workers hired after 1 January 1945. The smoking habits were surveyed. Among the men, 196 cases of primary cancer were detected versus 203.8 expected [standardized incidence ratio (SIR) 96, 95% confidence interval (95% CI) 82-114], and there were 47 cancer cases among the women versus 57.9 expected (SIR 91, 95% CI 60-108). Lung cancer occurred in 78 men (62.6 expected, SIR 125, 95% CI 98-155), and the excess was the most prominent for the male board mill workers (40 observed, 81.1 expected, SIR 222, 95% CI 158-302), particularly after 20 year's latency (25 observed, 7.8 expected, SIR 323, 95% CI 209-476). Analogous excesses of lung cancer occurred among the men (especially the male board mill workers) who began work after 1 January 1945. The findings were not explained by smoking habits.  相似文献

Cancer incidence among Norwegian airline pilots   总被引:5,自引:0,他引:5  
OBJECTIVES: In this retrospective cohort study, the cancer incidence of commercial pilots was studied to determine whether exposure at work has any influence on the incidence of cancer. METHODS: The cohort was established from the files of the Civil Aviation Administration and included people who had valid licenses as commercial pilots between 1946 and 1994. Basic data about their flight careers were recorded, and exposure to cosmic radiation was estimated. The cohort was linked to the Cancer Register of Norway. The observed number of cases was compared with that expected based on national rates. RESULTS: A group of 3701 male pilots was followed over 70 560 person-years. There were 200 cases of cancer versus 188.8 expected, with a standardized incidence ratio (SIR) of 1.06 and a 95% confidence interval (95% CI) of 0.92-1.22. No significant decreased risk was found for any cancer site. Excess risks were found for malignant melanoma (22 cases SIR 1.8, 95% CI 1.1-2.7) and nonmelanoma skin cancer (14 cases, SIR 2.4, 95% CI 1.3-4.0). For malignant melanoma, there was a significant trend for the SIR by cumulative dose. CONCLUSIONS: For most cancer sites, the incidence among pilots did not deviate from that of the general population and could not be related to block hours of flight time or dose. It seems more likely that the excess risks of malignant melanoma and skin cancer are explained by factors related to life-style rather than by conditions at work.  相似文献

Cancer incidence was studied among 922 creosote-exposed impregnators at 13 plants in Sweden and Norway. The subjects had been impregnating wood (eg, railroad cross-ties and telegraph poles), but no data on individual exposures were available. The study population was restricted to men employed during the period 1950-1975, and their cancer morbidity was checked through the cancer registries. The total cancer incidence was somewhat lower than expected, 129 cases versus 137 expected [standardized incidence ratio (SIR) 0.94]. Increased risks in both countries combined were observed for lip cancer (SIR 2.50, 95% confidence interval (95% CI) 0.81-5.83), skin cancer (SIR 2.37, 95% CI 1.08-4.50), and malignant lymphoma (SIR 1.9, 95% CI 0.83-3.78). Exposure to sunlight may have contributed to the risk of lip and skin cancer. The small number of cancer cases does not permit valid conclusions. The findings indicate that impregnating wood with creosote in earlier decades increased the risk of skin cancer.  相似文献

BACKGROUND: Cancer incidence in women textile workers has not been adequately studied. The aim of this study was to examine site-specific cancer incidence patterns in a cohort of 267,400 women textile workers in Shanghai, China. METHODS: Women employed by the Shanghai Textile Industry Bureau (STIB) were followed for cancer incidence from 1989 to 1998. Age-adjusted standardized incidence ratios (SIRs) and 95% confidence intervals (CI) were computed based on Shanghai Cancer Registry (SCR) rates. RESULTS: There was a decrease in cancer incidence for the cohort compared with urban Shanghai women (SIR = 0.91, 95% CI = 0.89-0.93). There were small increased risks of other endocrine tumors (SIR = 1.31, 95% CI = 1.02-1.65). There were decreased risks for esophageal (SIR = 0.54, 95% CI = 0.44-0.66), stomach (SIR = 0.79, 95% CI = 0.73-0.85), rectal (SIR = 0.88, 95% CI = 0.78-0.98), lung (SIR = 0.80, 95% CI = 0.74-0.86), cervical (SIR = 0.37, 95% CI = 0.28-0.50), ovarian (SIR = 0.85, 95% CI = 0.75-0.96), and bladder cancers (SIR = 0.63, 95% CI = 0.46-0.85). CONCLUSIONS: Women employed in the textile industry had a lower than expected cancer experience compared with urban Shanghai women. Further research on this cohort will examine associations between site-specific cancers and occupational exposures to dusts and chemicals.  相似文献

Cancer morbidity in iron and steel workers in Korea   总被引:2,自引:0,他引:2  
BACKGROUND: In the iron and steel industry, workers are potentially exposed to a number of carcinogens and are involved in a number of processes of a hazardous nature. The cancer morbidity of iron and steel workers from modern plants in a developing country is described. METHODS: Cancer morbidity at two Korean iron and steel complexes was analyzed using Poisson regression methods. Work histories were merged with the national cancer registry for 44,974 workers who were followed from 1988-2001. RESULTS: Four hundred sixty-four cancers, in 1% of the population, were diagnosed over 14 years. Based on national cancer rates, the cohort exhibited a healthy worker effect for all cancer (SIR = 0.87, 95% CI = 0.79-0.95) reflecting relative good health, particularly for lung cancer (SIR = 0.58, 95% CI = 0. 04-0.82), stomach cancer (SIR = 0.78, 95% CI = 0.64-0.93), and liver cancer (SIR = 0.83, 95% CI = 0.68-1.01). Lung cancer morbidity was significantly elevated at the affiliated plants versus the parent plants (SRR = 2.35, 95% CI = 1.07-4.92), and all-cancer morbidity was significantly elevated for maintenance workers compared to office and production workers (SRR = 1.27, 95% CI = 1.00-1.60). Lymphohematopoietic cancer incidence was higher in the coke plants (SRR = 3.46, 95% CI = 1.02-8.91) and stomach cancer incidence was higher in the maintenance departments (SRR = 1.66, 95% CI = 1.05-2.56). CONCLUSIONS: This recent steelworker cohort exhibits possible excess cancer morbidity in some processing areas. Further follow-up of this cohort and alternate study designs such as case-control study will be needed to elucidate the relationship of exposure and health risks of iron and steel workers.  相似文献

OBJECTIVES: This study investigated the incidence of cancer and cause-specific mortality among workers in the two Lithuanian asbestos-cement factories. METHODS: The study included 1887 asbestos-cement workers, 1285 men and 602 women, and 37000 person-years. The two factories were active from 1956 (A) and 1963 (B), and the workers were observed from 1978 to 2000. The analysis was based on a comparison between the observed and expected numbers of cancer and causes of death. The observed numbers of cancer were obtained through linkage with the national cancer registry. The date and causes of death were obtained from two different sources. The expected numbers were calculated on the basis of gender- and age-specific incidence and mortality rates in 5-year periods from the whole country. Standardized incidence ratios (SIR) and standardized mortality ratios (SMR) and 95% confidence intervals (95% CI) were calculated. Duration of employment and time since first exposure were used as indicators of exposure. RESULTS: During the follow-up, 1978-2000. 473 deaths were observed versus 489 expected. There was no excess risk of deaths from nonmalignant respiratory diseases, except for an elevated risk of mortality in relation to the digestive organs other than cancer, 18 observed versus 12.2 expected (95% CI 0.9-2.3). There was no excess risk for any types of cancer, except for colorectal cancer in men, 17 observed cases (SIR 1.6, 95% CI 1.6-2.6) and one case of mesothelioma in a woman. CONCLUSIONS: This study on asbestos-exposed workers did not show any excess risk of respiratory cancer or deaths of pneumoconiosis.  相似文献

This paper reports incidence of cancer in employees of the Australian petroleum industry from 1981 to 1989. Two surveys by personal interview incorporated more than 15,000 employees, representing 92% of the eligible population. Subjects were included in the analysis after completing five years of service in the industry. At the time of this report the cohort did not include sufficiently large numbers of women for useful analysis; results presented are restricted to the men. On 31 December 1989, 50,254 person-years of observation had accumulated in the men with 152 incident cancers reported. The standardised incidence ratio (SIR) analysis showed overall cancer rates close to those of the national population. Whereas deficits were seen in some cancer sites, notably lung cancer (SIR 0.5, 95% confidence internal (95% CI) 0.3-0.9), incidence rates for some other cancer sites suggested increased risk. An excess of observed over expected cases was present in all subcategories of lymphohaematopoietic cancer except Hodgkin's disease (no cases), and was most apparent in myeloid leukaemia (SIR 4.0, 95% CI 1.6-8.2). The other major site with a raised number of cases observed over expected was melanoma (SIR 1.4, 95% CI 0.8-2.1).  相似文献

Hip fractures in women with breast cancer   总被引:2,自引:0,他引:2  
The authors investigated whether the incidence of first hip fracture, an indicator of osteoporosis, is lower in breast cancer patients, who are assumed to have higher levels of endogenous estrogens, than in other women. A population-based Swedish cohort comprising 9,673 women with invasive breast cancer diagnosed from 1958 through 1983 was followed up with respect to the occurrence of a first hip fracture during the period 1965-1983. Overall, the number of observed acute fractures (n = 387) was slightly higher than expected (n = 348.6) (standardized incidence ratio (SIR) = 1.1, 95% confidence interval (CI) 1.0-1.2). Risk for trochanteric fractures was slightly higher than expected (SIR = 1.2, 95% CI 1.0-1.4), but risk for cervical fractures was not (SIR = 1.0, 95% CI 0.9-1.1). Risk for trochanteric fracture decreased with increasing age at breast cancer diagnosis, reaching standardized incidence ratios close to unity after the age of 70 years. Duration of follow-up appeared to be unrelated to the risk of either type of fracture. The authors conclude that the incidence of first hip fracture is not lower in breast cancer patients than in other women.  相似文献

Despite 200 years of efforts to regulate safety in this occupation, chimney sweeps have increased mortality from cancer, ischaemic heart disease, and respiratory disease. Mortality and incidence of cancer were examined in a cohort of 5542 Swedish chimney sweeps employed through their national trade union at any time between 1918 and 1980. Previous studies of this cohort found increased risks of ischaemic heart disease, respiratory disease, accidental deaths, and various neoplasms. By increasing follow up, we sought to increase the power of the study and examine disease time trends. Mortality analysis was extended 7.5 years to cover the period 1951-90; cancer incidence analysis was extended six years to cover the period 1958-87. New findings include increased incidence and mortality of prostate cancer (SMR 169, 95% CI 106-256, 22 observed) and increased incidence of total haematolymphatic cancers (SIR 151, 95% CI 106-209, 36 observed). When only the most recent follow up period was analysed, previously observed risks persisted for total lung cancer (SIR 178, 95% CI 99-293), oat cell lung cancer (SIR 240, 95% CI 103-472), bladder cancer (SIR 247, 95% CI 131-422), and oesophageal cancer (Obs/Exp = 2/1.1). Mortality from ischaemic heart disease (SMR 98, 95% CI 76-123) and respiratory disease (SMR 111, 95% CI 56-199) declined during recent follow up, although significant excess mortality remained during analysis of the entire study period (ischaemic heart disease SMR 128, 95% CI 112-145; respiratory disease SMR 159, 95% CI 115-213). In analyses of the entire study period, risks of ischaemic heart disease and lung, bladder, and oesophageal cancer were adjusted for smoking; oesophageal cancer was also adjusted for use of alcohol. All risks remained significantly raised. Exposure-response analyses showed significant positive associations between duration of employment and risks for mortality from lung, oesophageal, and total cancer. Chimney sweeps remain at increased risk for cancers of the lung, oesophagus, and bladder. Our study supports a casual role for exposure to chimney soot, which contains carcinogens including polycyclic aromatic hydrocarbons. Extended follow up of this cohort now shows increased risks of prostate and haematolymphatic cancers.  相似文献

A cohort of 736 male and 167 female workers of two anthophyllite mines in Finland was followed up through the Finnish Cancer Registry for cancer in 1953-91. Compared with the total cancer incidence of the east Finnish population, the men had a raised risk of total cancer (standardised incidence ratio (SIR) 1.7; 95% confidence interval (95% CI) 1.4-1.9), mainly attributable to an excess in lung cancer (SIR 2.8; 95% CI 2.2-3.6). The risk of lung cancer was somewhat higher among workers classified as heavily exposed (SIR 3.2; 95% CI 2.4-4.1) than among those moderately exposed (SIR 2.3; 95% CI 1.5-3.6) and the risk increased with increasing smoking and with increasing time of work with exposure. There were four cases of mesothelioma v 0.1 expected, all in men who smoked and had had a long and heavy asbestos exposure. Among women, a non-significant excess in total cancer (SIR 1.5; 95% CI 0.9-2.4) was found in the subgroup with heavy exposure to asbestos. Anthophyllite asbestos seems to have high potency in the carcinogenesis of lung cancer and low potency in carcinogenesis of mesothelioma in comparison with the other types of asbestos.  相似文献

OBJECTIVES: To examine the association between cancer incidence, in particular incidence of lung cancer, and duration of work among employees in eight Norwegian plants producing ferrosilicon and silicon metal. METHODS: Among men first employed during 1933-91 and with at least 6 months in these plants, the incident cases of cancer during 1953-91 were obtained from The Cancer Registry of Norway. The numbers of various cancers were compared with expected figures calculated from age and calendar time specific rates for Norwegian men during the same period. Internal comparisons of rates were performed with Poisson regression analysis. The final cohort comprised 8530 men. RESULTS: A total of 832 cases of cancer were observed against 786 expected (standardised incidence ratio (SIR) 1.06). Among the furnace workers an increased incidence of lung cancer (SIR 1.57) and testicular cancer (SIR 2.30) was found. Internal comparisons of rates by Poisson regression analysis among the rural furnace workers showed a positive trend between incidence of lung cancer and duration of work of 1.05 (95% confidence interval (95% CI) 1.00 to 1.10)/work-year. Excess cases of prostate and kidney cancer were found among blue collar non-furnace workers, in particular among the mechanics. CONCLUSION: The results suggest associations between furnace work and lung and testicular cancer, and between non-furnace work and prostate cancer.

OBJECTIVES: This study evaluated the influence of occupational exposure on cancer risk among female Norwegian nurses. METHODS: A historical prospective cohort study was performed. The cohort was established from the Norwegian Board of Health's registry of nurses and included women who graduated from a nursing school before 1985. The cohort (N=43 316) was linked to the Cancer Registry of Norway. The observed number of cases was compared with the expected number on the basis of national rates. Time since first employment, period of first employment, and duration of employment were used as indicators of exposure. Poisson regression analyses were used for internal comparisons, adjusting for age, period, and fertility. RESULTS: The nurses were followed over 1473931 person-years. During the follow-up (1953-2002), 6193 cancer cases were observed. The standardized incidence ratio (SIR) for all cancers combined was close to unity. Significantly lower risks were found for cancers with a known association with alcohol and tobacco consumption and sexual activity. A significantly increased risk was found for breast cancer (SIR 1.14, 95% confidence interval (95% CI) 1.09-1.19), ovarian cancer (SIR 1.14, 95% CI 1.04-1.25), malignant melanoma (SIR 1.15, 95% CI 1.04-1.28), and borderline significant risk appeared for other skin cancer (SIR 1.12, 95% CI 0.98-1.29). A positive trend for increasing time since first exposure was found for breast cancer and malignant melanoma. CONCLUSIONS: The results indicate an association between working as a nurse and an increased risk of breast cancer and malignant melanoma. Decreased risks, found for several cancers, indicate favorable lifestyle habits among nurses.  相似文献

OBJECTIVES: The aim of this study was to define the cancer pattern in an Icelandic cohort of deck officers while indirectly controlling for their smoking habits. METHODS: A cohort of 3874 male deck officers was followed from 1966 to 1998. It was record-linked by the deck officers' personal identification numbers to population-based registers containing each person's vital and emigration status and cancer diagnosis. Standardized incidence ratios (SIR) were calculated for all cancers and different cancer sites in relation to different lag times and years of birth. Information on smoking habits was obtained by from a questionnaire administered to a sample of the cohort (N=728). RESULTS: In the total cohort 436 cancers were observed, equal to the expected 436 [SIR 1.0, 95% confidence interval (95% CI) 0.9-1.1], and a significantly increased risk of soft-tissue sarcoma (SIR 2.7, 95% CI 1.2-5.1) was found together with a decreased risk of testis cancer (SIR 0.0, 95% CI .. -0.7) and urinary bladder cancer (SIR 0.5, 95% CI 0.3-0.8). The deck officers' smoking habits were similar to those found in a sample of the general population. The predictive value was 1.11 for lung cancer CONCLUSIONS: An increased incidence of soft-tissue sarcoma was found among deck officers. This increase resembles that previously found for Swedish deck officers, and it warrants further study.  相似文献

The risk of cancer was analysed in a cohort of 20,245 licensed pesticide applicators in agriculture who had licences issued between 1965 and 1976. Most were men (99%) and about 50% had been born in 1935 or later. The cohort was followed up in the Swedish Cancer Register from date of licence until death or 31 December 1982. The mean follow up time was 12.2 years. Average time since first exposure was longer, however, since one fifth of the cohort was exposed in the 1950s. A total of 558 malignant tumours was found compared with 649.8 expected, which resulted in a statistically significantly decreased standardised incidence ratio (SIR) of 0.86 (95% confidence interval (CI): 0.79-0.93). Significantly decreased risks of cancer were also found for liver (SIR = 0.45, 95% CI: 0.18-0.93), pancreas (SIR = 0.50, 95% CI: 0.26-0.87), lung (SIR = 0.50, 95% CI: 0.35-0.68), and kidney (SIR = 0.53, 95% CI: 0.32-0.84). No statistically significantly increased risks or any time trends were observed. SIR for testicular cancer was increased (SIR = 1.55, 95% CI: 0.92-2.45) and increased with period since licence. Eighteen cases with testicular cancer were found. For those born in 1935 or later a non-significant increased overall risk of cancer was observed (SIR = 1.07, 95% CI: 0.82-1.37). Comparisons were made with agricultural workers in general since pesticide applicators are mainly farmers that use or have used pesticides to a greater extent. Higher risks for pesticide applicators were found for testicular cancer, tumours of the nervous system and endocrine glands, and Hodgkin's disease.  相似文献

BACKGROUND: The aim of the present study was to investigate cancer risk among women working in the Norwegian pulp and paper industry. The cohort included a total of 4,247 workers employed for at least one year between 1920 and 1993 (108,095 person-years), 85% of them as paper or administration workers. METHODS: The follow-up period for cancer was from 1953-1993. No data of exposure measurements were available. The analyses were based on comparisons of standard incidence ratios. The expected numbers of cancer cases were calculated using the five-year age-specific incidence rates for the entire female population. RESULTS: During the follow-up period, 380 new cases of cancer were observed vs. 322 expected (SIR = 1.2, 95% CI = 1.07-1.30). An excess risk of ovarian cancer was found (SIR = 1.5, 95% CI = 1.07-2.09). The SIR was highest among those younger than 55 years, and mostly among those working in paper departments. Short-term workers showed increased risk of lung and bladder cancer. CONCLUSIONS: Based on results from the present study, the increased risk of ovarian cancer is difficult to interpret, since existing knowledge of its etiology is limited. However, these women might have been exposed to various work-related agents such as talc, microbes, and different types of paper dust.  相似文献

BACKGROUND: Cabin crews are exposed to cosmic radiation at work and this may increase their incidence of radiation-induced cancers. Former studies indicate an increased risk of breast cancer. METHODS: A retrospective cohort study was performed. The cohort was established from the files of the Civil Aviation Administration and included people with a valid licence as a cabin attendant between 1950 and 1994. The cohort was linked to the Cancer Registry of Norway. Observed number of cases was compared with expected, based on national rates. Breast cancer incidence was analysed, adjusting for individual fertility variables. RESULTS: A group of 3693 cabin attendants were followed over 72 804 person-years. Among the women, 38 cases of breast cancer were observed (standardized incidence ratio (SIR) = 1.1, 95% CI : 0.8-1.5). Among men excess risks were found for cancers in the upper respiratory and gastric tract (SIR = 6.0, 95% CI : 2.7-11.4) and cancer of the liver (two cases, SIR = 10.8, 95% CI : 1.3-39.2). For both sexes elevated risks were found for malignant melanoma and non-melanoma skin cancer; for men these were SIR = 2.9 (95% CI : 1.1-6.4) and SIR = 9.9 (95% CI : 4.5-18.8) respectively, while for women these were SIR = 1.7 (95% CI : 1.0-2.7) and SIR = 2.9 (95% CI : 1.0-6.9) respectively. For no cancer site was a significant decreased risk found. CONCLUSIONS: An increased risk of radiation-induced cancers was not observed. The excess risks of some other cancers are more probably explained by factors related to lifestyle.  相似文献

BACKGROUND: A classification of 12 work categories was used to evaluate the cancer incidence and mortality among a cohort of Swedish rubber tire workers. METHODS: Cancer incidence and mortality in the cohort was compared with expected values from national rates. Standardized incidence and mortality ratios were calculated for the total cohort, for sub-cohorts and with the inclusion of a latency requirement. RESULTS: Among men, increased incidence and mortality risks were found for cancer in the larynx; SIR=2.10; 95% confidence intervals (95% CI): 1.05-3.76, SMR=2.08; 95% CI: 0.42-6.09. Increased risks were also seen for cancer in the trachea, bronchus, and lung; SIR=1.62; 95% CI: 1.28-2.02, SMR=1.54; 95% CI: 1.21-1.94, the incidence risk was highest among those with the longest exposure duration and among workers in compounding/mixing, milling, and maintenance. Decreased incidence risks were seen for cancer of the prostate (SIR=0.74; 95% CI: 059-0.92) and skin (SIR=0.57; 95% CI: 0.36-0.84). CONCLUSIONS: The finding of an excess of tumors in the respiratory system is in agreement with earlier findings in other studies on rubber tire workers. The results on other cancer types are compared to earlier findings and related to work processes and chemical exposures of possible causal importance.  相似文献

OBJECTIVES: Occupational radiation exposure was estimated, and the cancer incidence among physicians working with radiation was compared to that of unexposed physicians. METHODS: A cohort of 1312 physicians was identified from the Finnish occupational radiation exposure registry. Radiation exposure data were obtained from 1970 to 2001 on the basis of individual dosimeters. Never-monitored Finnish physicians (N=15 821) were used as a reference group, identified from census data of Statistics Finland. Incident cancer cases were identified by record linkage with the Finnish Cancer Registry. RESULTS: The cumulative radiation dose exceeded the recording level (0.3-3.0 mSv during a 3-month period for 1029 radiation-exposed physicians (59.8%). Six percent of the radiologists had received a cumulative dose of 50 mSv or more. Altogether there were 41 cancers observed among the radiation-exposed physicians and 998 cases found in the never-monitored group. Standardized incidence ratios (SIR) for all cancers were comparable with those of the general population among physicians monitored for radiation [SIR 1.0, 95% confidence interval (95% CI) 0.7-1.4] and other physicians (SIR 1.0, 95% CI 1.0-1.1). For specific cancer sites, a slightly elevated risk of female breast cancer was found among monitored physicians when compared with other physicians (rate ratio 1.7, 95% CI 1.0-3.1). No obvious dose-response relationship was found for the overall cancer incidence. CONCLUSIONS: According to the results from a nationwide cohort, occupational exposure to medical radiation is not a strong risk factor for cancer among physicians. Possible excess risk could not be reliably demonstrated even after the follow-up of a nationwide cohort for up to 30 years.  相似文献
