PURPOSE: To intraindividually compare the enhancement pattern of focal nodular hyperplasia (FNH) after dynamic administration of two bolus-injectable liver-specific MR contrast agents, ferucarbotran and gadobenate dimeglumine. MATERIALS AND METHODS: A total of 19 patients with 24 FNHs underwent gadobenate dimeglumine- and ferucarbotran-enhanced MRI during the hepatic arterial-dominant phase (HAP; 25 seconds), the portal-venous phase (PVP; 60 seconds), and the equilibrium phase (EP; 180 seconds). Hepatospecific phases were acquired on T1-weighted images 120 minutes after gadobenate dimeglumine administration, and on T2-weighted images 10 minutes after ferucarbotran administration. Lesion enhancement was independently analyzed by two observers. The kappa statistic was determined to evaluate the agreement between the enhancement patterns of the lesions. RESULTS: On gadobenate dimeglumine-enhanced MR images during HAP, PVP, and EP, FNHs were: hyperintense (24/20/13); isointense (0/4/11); and hypointense (0/0/0). On ferucarbotran-enhanced MR images during HAP, PVP, and EP, FNHs were: hyperintense (2/0/0); isointense (16/9/14); and hypointense (6/15/10). Overall, poor agreement between both contrast agents was observed. During the hepatospecific phases, most (20/24; 83%) FNHs showed a typical enhancement pattern during the delayed hepatospecific phase. CONCLUSION: The dynamic enhancement pattern of FNHs is significantly different between gadobenate dimeglumine- and ferucarbotran-enhanced MRI. With respect to hepatospecific phase, the majority of FNHs showed a typical behavior on both contrast agents.  相似文献

PURPOSE: To compare the efficacy of two different MR contrast agents for the detection and diagnosis of focal nodular hyperplasia (FNH). MATERIALS AND METHODS: Fifty patients with 83 FNH lesions detected on spiral CT were studied in two different MRI sessions with Gd-BOPTA (MultiHance) and ferumoxides (Endorem). MRI with Gd-BOPTA was performed precontrast (T1wGRE and T2wTSE sequences) and during the dynamic and late (1-3 hours) phases after injection (T1wGRE sequences only). MRI with ferumoxides (T1wGRE and T2wTSE sequences) was performed before and at least 30 minutes after injection. Hyper- or isointensity of FNH in the late phase was considered typical for Gd-BOPTA, while isointensity or lesion hypointensity was considered typical for ferumoxides. RESULTS: With Gd-BOPTA, 83 FNH lesions (100%) appeared hyperintense during the arterial phase of dynamic MRI. All but one lesion was iso- or slightly hyperintense in the portal-venous and equilibrium phases. In the late phase, 81 FNH lesions were hyper- or isointense to the surrounding parenchyma, with two lesions appearing slightly hypointense. With ferumoxides, a significant (P < 0.001) number (21/83, 25.3%) of FNH lesions (mean diameter = 16.8 +/- 6.6 mm) were not visible. Of the visible FNH lesions, 38/62 were slightly hyperintense, and 24/62 were isointense to the surrounding parenchyma on the T2wTSE images. On the T1wGRE images, 42/62 lesions were isointense, 19/62 were slightly hyperintense, and one lesion was slightly hypointense. Seventeen lesions in 12 patients with previous neoplasia were all detected after Gd-BOPTA administration, whereas only nine of these 17 lesions (52.9%) were detected after ferumoxide administration. Two of these nine lesions showed atypical enhancement features. CONCLUSION: Gd-BOPTA-enhanced MRI is significantly better than ferumoxide-enhanced MRI for the identification and characterization of FNH.  相似文献

PURPOSE: To prospectively determine the accuracy of differentiating benign focal nodular hyperplasia (FNH) from hepatic adenoma (HA) and liver adenomatosis (LA) by using gadobenate dimeglumine-enhanced magnetic resonance (MR) imaging. MATERIALS AND METHODS: The ethics committee at each center approved the study, and all patients provided informed consent. Seventy-three patients with confirmed FNH and 35 patients with confirmed HA (n = 27) or LA (n = 8) underwent MR imaging before (T2-weighted half-Fourier rapid acquisition with relaxation enhancement or T2-weighted fast spin-echo and T1-weighted gradient-echo [GRE] sequences) and at 25-30 seconds (arterial phase), 70-90 seconds (portal venous phase), 3-5 minutes (equilibrium phase), and 1-3 hours (delayed phase) after (T1-weighted GRE sequences only, with or without fat suppression) bolus administration of 0.1 mmol per kilogram of body weight gadobenate dimeglumine. The enhancement of 235 lesions (128 FNH, 32 HA, and 75 LA lesions) relative to the normal liver parenchyma was assessed. Sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and overall accuracy for the differentiation of FNH from HA and LA were determined. RESULTS: Hyper- and isointensity on T2-weighted and iso- and hypointensity on T1-weighted GRE images were noted for 177 (88.9%) of 199 lesions visible on unenhanced images. On dynamic phase images after contrast material administration, 231 (98.3%) of 235 lesions showed rapid strong enhancement during the arterial phase and appeared hyper- to isointense during portal venous and equilibrium phases. Accurate differentiation of FNH from HA and LA was not possible on the basis of precontrast or dynamic phase images alone. At 1-3 hours after contrast material enhancement, 124 (96.9%) of 128 FNHs appeared hyper- or isointense, while 107 (100%) HA and LA lesions appeared hypointense. The sensitivity, specificity, PPV, NPV, and overall accuracy for the differentiation of FNH from HA and LA were 96.9%, 100%, 100%, 96.4%, and 98.3%, respectively. CONCLUSION: Accurate differentiation of FNH from HA and LA is achievable on delayed T1-weighted GRE images after administration of gadobenate dimeglumine.  相似文献

目的:研究肝脏局灶性结节增生(FNH)的MRI表现和病理学表现及其它们的相关性。方法:回顾性分析9例经手术病理证实的FNH的MRI表现及其病理学表现。结果:FNH平扫T1WI 5例为等信号,4例为稍低信号;T2WI 3例为等信号,6例为稍高信号。7例病灶中心或偏心有瘢痕。动态增强扫描9例病灶在动脉期均呈明显均匀强化;门脉期和延迟期为轻度增强或等信号;7例病灶在延迟期出现瘢痕强化。FNH是多血供实质性肿块,肿瘤内部结构均匀,出血和坏死少见,镜下可见典型的星状瘢痕。结论:MRI平扫和动态增强扫描能够显示FNH的特征性表现,反映FNH的病理特点,提高FNH的确诊率。  相似文献

OBJECTIVE: This study describes the findings of magnetic resonance imaging (MRI) of focal eosinophilic infiltration of the liver. METHODS: Contrast-enhanced MR images of 8 patients with focal hepatic eosinophilic infiltration were reviewed retrospectively. We evaluated the signal intensity of focal lesions in T1-weighted and T2-weighted images and the pattern of enhancement in a dynamic contrast study. RESULTS: A total 22 focal hepatic lesions were observed; the lesions were isointense (55%) or hypointense (45%) on T1-weighted images and isointense (14%) or hyperintense (86%) on T2-weighted images. The arterial phase of the contrast study revealed 11 hyperintense lesions (50%). During the portal and delayed phases, 18 (82%) and 17 lesions (77%) were hyperintense, respectively. CONCLUSION: The focal eosinophilic infiltrations showed homogeneous enhancement in the portal and delayed phases in the dynamic contrast MR study. These findings should help to distinguish focal eosinophilic infiltration, especially from metastasis in patients with malignancy.  相似文献

PURPOSE: To prospectively compare intraindividual differences in enhancement patterns between gadolinium- and superparamagnetic iron oxide (SPIO)-enhanced magnetic resonance (MR) imaging in patients with histologically proved hepatocellular carcinoma (HCC). MATERIALS AND METHODS: Institutional review board approval and informed consent were obtained. Twenty-two patients (18 men, four women; mean age, 58.9 years) with 36 pathologically proved HCC lesions underwent contrast material-enhanced dynamic T1-weighted gradient-echo MR imaging twice. Gadopentetate dimeglumine was used at the first session. After a mean interval of 5 days, a second session was performed with a bolus-injectable SPIO agent, ferucarbotran. Qualitative analysis of contrast enhancement patterns with each agent during hepatic arterial, portal venous, and equilibrium phases was performed by two readers who classified lesions as isointense, hypointense, or hyperintense compared with surrounding liver parenchyma and searched for presence of hyperintense peritumoral ring enhancement. Results of signal intensity analysis during different vascular phases at both sessions were compared by using the McNemar test, and kappa statistic was used to evaluate agreement between signal intensity and enhancement pattern of lesions during different vascular phases. RESULTS: On gadolinium-enhanced hepatic arterial phase images, HCC lesions (n = 36) were hyperintense in 21 (58%) cases, hypointense in 10 (28%), and isointense in five (14%). On ferucarbotran-enhanced hepatic arterial phase images, HCC lesions were isointense in 18 (50%) cases, hypointense in 11 (31%), and hyperintense in seven (19%). On gadolinium-enhanced portal venous and equilibrium phase images, respectively, HCC lesions were hypointense in 17 (47%) and 21 (58%) cases, hyperintense in 10 (28%) cases and one (3%) case, and isointense in nine (25%) and 14 (39%) cases. On ferucarbotran-enhanced portal venous and equilibrium phase images, respectively, HCC lesions were hypointense in 15 (42%) and 11 (31%) cases, hyperintense in three (8%) and three (8%) cases, and isointense in 18 (50%) and 22 (61%) cases. CONCLUSION: For HCC, contrast enhancement pattern on T1-weighted gradient-echo MR images shows marked variability with gadolinium or SPIO contrast agents.  相似文献

目的:评估CT、MRI对肝脏局灶结节性增生(focal nodular hyperplasia,FNH)的诊断价值.材料和方法:回顾性分析9例FNH(6例经病理证实,3例经磁共振特异性对比剂Resovist证实)的CT、MRI表现和术前或穿刺前诊断.结果:8例为单发病灶、1例为3个病灶.10个病灶呈稍低密度,其中7个T1WI为稍低信号、T2WI稍高信号,3个病灶T1WI和T2WI为稍高信号;1个病灶为等密度和等信号;CT动脉期明显增强、门静脉期轻度增强或无增强;MRI的增强方式与CT相似,但10个病灶存在明显的延迟增强.7个病灶检出瘢痕,T1WI呈低信号、T2WI高信号,动脉期和门静脉期无增强、延迟期轻至中度增强.8例单发病灶均正确诊断为FNH.结论:CT和MRI能显示FNH的特征性改变并提高诊断的准确性.  相似文献

PURPOSE: To compare the potentials of AMI-25 (Endoren) to those of Gadolinium with the dynamic contrast-enhanced technique in the differential diagnosis of focal liver lesions. MATERIAL AND METHODS: Forty patients with at least one focal liver lesion diagnosed at US underwent MRI. We used a 1.5 T unit and employed single-shot half-Fourier T2-weighted FSE and spoiled gradient-echo T1-weighted sequences before and after Gadolinium injection. Multiple acquisitions were obtained during the arterial, portal and delayed phases. Twenty-four to 48 hours later T2*-weighted GRE and SPGR/90 degrees sequences were obtained after AMI-25 administration. In the characterization of solid lesions the gold standard was biopsy performed with a shearing needle; for the diagnosis of angiomas and of 11 metastatic lesions we considered follow-up and clinical data as important diagnostic elements. RESULTS: We found 12 hepatocarcinomas, 14 metastases, 4 cases of focal nodular hyperplasia (FNH), 4 adenomas and 6 angiomas. The diagnosis was correct and confirmed by the conventional examination in all cases but 2 adenomatous lesions and 2 angiomas. Precontrast studies showed slight hyperintensity in 2 of 4 cases of FNH, while the other 2 lesions appeared isointense and were therefore detected only on postcontrast images, where there was contrast agent uptake during the arterial phase and rapid washout. We found only one central scar hyperintense on T2- and hypointense on T1-weighted images. After AMI-25 administration all lesions appeared isointense to surrounding parenchyma on T2* GRE sequences. Adenomas were isointense in the precontrast phase and postcontrast 3 of them showed strong Gadolinium uptake and rapid washout. After AMI-25 two of the 4 lesions were hyperintense while the other two were isointense to the parenchyma. Four of 6 angiomas exhibited a typical pattern characterized by signal hyperintensity on T2-weighted sequences and on AMI-25-enhanced T1- and T2-weighted sequences. Two angiomas were supposed to be of malignant nature but histology showed the presence of a strong fibrotic component. Hepatocarcinomas could be detected on precontrast images. After Gadolinium administration 10 lesions appeared hyperintense in the arterial phase and 2 were hypointense. After AMI-25 all lesions exhibited homogeneous signal hyperintensity and appeared slightly bigger than on Gadolinium-enhanced images. The metastases were only partly demonstrated by MRI. Postgadolinium studies showed 13 lesions with hyperintense signal in the portal phase. AMI-25 administration detected 14 lesions that appeared slightly bigger than on Gadolinium-enhanced images. CONCLUSIONS: AMI-25 can help also in characterizing primary lesions with an atypical signal pattern after contrast agent administration thanks to its intrinsic capability of accumulating in benign lesions. However it remains difficult to characterize well differentiated hepatocarcinomas and adenomas. Finally, AMI-25 improves MR capabilities in detecting secondary lesions and possible satellite nodules.  相似文献

PURPOSE: To evaluate the different signal characteristics of focal hepatic lesions on ferumoxides-enhanced MR imaging, including T1-weighted spoiled gradient recalled echo (GRE) images using different echo times (TE) and T2- and T2*-weighted images. MATERIALS AND METHODS: Ferumoxides-enhanced MR imaging was performed using a 1.5-T system in 46 patients who were referred for evaluation of known or suspected hepatic malignancies. One hundred and seven lesions (42 hepatocellular carcinomas [HCC], 40 metastases, 13 cysts, eight hemangiomas, three focal nodular hyperplasias [FNHs], and one cholangiocarcinoma) were evaluated. Postcontrast MR imaging included 1) T2-weighted FSE; 2) T2*-weighted GRE; 3) T1-weighted spoiled GRE using moderate (TE = 4.2-4.4 msec) TE; and 4) minimum (TE = 1.8-2.1 msec) TE. Signal intensities of the focal lesions were rated by two radiologists in conference as follows: hypointense, isointense or invisible, hyperintense, and markedly hyperintense. Lesion-to-liver contrast-to-noise ratio (C/N) was measured by one radiologist for a quantitative assessment. RESULTS: On ferumoxides-enhanced FSE images, 92% of cysts were "markedly hyperintense" and most of the other lesions were "hyperintense", and the mean C/N of cysts was significantly higher than that of other focal lesions. T2*-weighted GRE images showed most lesions with similar hyperintensities and the mean C/N was not significantly different between any two types of lesion. T1-weighted GRE images using moderate TE showed all FNHsand hemangiomas, 29 (69%) HCCs and eight (20%) metastases as "hyperintense". On T1-weighted GRE images using minimum TE, however, all HCCs and metastasis except one were iso- or hypointense, while all of the FNHs and hemangiomas were hyperintense. Ring enhancement was highly suggestive of malignant lesions, and was more commonly seen on the minimum TE images than on the moderate TE images. CONCLUSION: Addition of T1-weighted GRE images using minimum and moderate TE is helpful for characterizing focal lesions in ferumoxides-enhanced MR imaging.  相似文献

OBJECTIVES: To evaluate the value of 1-hour delayed phase imaging (DPI) of gadobenate dimeglumine (Gd-BOPTA)-enhanced MR imaging for the characterization of hepatocellular carcinoma (HCC) and dysplastic nodule (DN) in patients with cirrhosis. MATERIALS AND METHODS: A total of 37 patients with 42 HCCs and 13 DNs were included in this study and all lesions were histopathologically confirmed except for 15 HCCs. T1-weighted 3-dimensional gradient-echo images were acquired before, immediately after (30, 60, 180 s), and 1 hour after bolus injection of gadobenate dimeglumine at a dose of 0.1 mmol/kg. The lesions were classified as isointense, hypointense, or hyperintense compared with the surrounding liver parenchyma on DPI for qualitative assessment. We performed quantitative analyses of the contrast-to-noise ratio (CNR) and of the relative contrast enhancement of the lesion on the DPI. RESULTS: In the qualitative analysis, among 42 HCCs, 30 (71.4%) were hypointense on DPI, and 10 (23.8%) and 2 (4.8%) were isointense and hyperintense, respectively; only 1 of 13 DNs (7.7%) was hypointense and 10 (76.9%) and 2 (15.4%) were isointense and hyperintense, respectively. In contrast, 25 HCCs (71.4%) of 35 hypervascular HCCs were hypointense on DPI, and no hypervascular DN (0/7) was hypointense with statistical significance (P = 0.0007). When we considered the hypointensity of the hepatic lesions on delayed phase as a sign of HCC in cirrhotic liver, our results gave a sensitivity of 71.4% and a specificity of 91.7%. In the quantitative analysis, the mean CNR of the HCCs and the DNs on the 1-hour DPI was -6.32 +/- 6.27 and -0.07 +/- 3.28, respectively; the difference between the HCCs and the DNs was significant (P < 0.05). CONCLUSIONS: Delayed gadobenate dimeglumine-enhanced MR imaging allows improved characterization of HCC in cirrhotic liver. The relative hypointensity to adjacent normal liver parenchyma is a reliable predictor that this lesion favors HCC rather than DN in cirrhotic liver.  相似文献

Purpose:

To characterize imaging features of histologically proven hepatic adenoma (HA) as well as histologically and/or radiologically proven focal nodular hyperplasia (FNH) using delayed hepatobiliary MR imaging with 0.05 mmol/kg gadoxetic acid.

Materials and Methods:

Five patients with six HAs with histological correlation were retrospectively identified on liver MRI studies performed with gadoxetic acid, and T1‐weighted imaging acquired during the delayed hepatobiliary phase. Additionally, 23 patients with 34 radiologically diagnosed FNH lesions (interpreted without consideration of delayed imaging) were identified, two of which also had histological confirmation. Signal intensity ratios relative to adjacent liver were measured on selected imaging sequences.

Results:

All six hepatic adenomas (100%), which had histological confirmation, demonstrated hypointensity relative to adjacent liver on delayed imaging. Furthermore, all of the FNH (including 34 radiologically proven, 2 of which were also histologically proven) were either hyperintense (23/34, 68%) or isointense (11/34, 32%) relative to the adjacent liver on delayed imaging. None of the FNHs were hypointense relative to liver.

Conclusion:

Distinct imaging characteristics of HA versus FNH on delayed gadoxetic acid‐enhanced MRI, with adenomas being hypointense and FNH being iso‐ or hyperintense on delayed imaging may improve specificity for characterization, and aid in the differentiation of these two lesions. Twenty-two patients with 25 cases of focal nodular hyperplasia (FNH) proved with pathologic study were imaged with a TurboFLASH (fast low angle shot) sequence combined with bolus administration of gadolinium tetraazacyclododecanetetraacetic acid (DOTA), spin-echo (SE) T2-weighted sequences, and postcontrast T1-weighted sequences. FNH-liver signal-difference-to-noise ratios were quantified; the features of the central scar were qualitatively analyzed. On SE T2-weighted images, all FNHs were hyperintense; in two cases the central scar exhibited a high signal intensity associated with hypointense areas corresponding to fibrous tissue within the branches of the scar. Unenhanced TurboFLASH images always demonstrated the FNHs as hypointense and always depicted the central scar as a hypointense area within the lesion. After bolus injection, arterial enhancement of FNH was clearly seen, and in 10 of 25 lesions, enhancement within the scar was seen 40-80 seconds after injection. Both unenhanced and enhanced TurboFLASH sequences produced the best signal-difference-to-noise ratios in comparison with T2-weighted images.  相似文献

目的 总结肝脏孤立性坏死结节(SNN)平扫和动态增强MRI特征性表现.方法 回顾性分析经手术病理证实的15例SNN的MRI征象,对病灶数目、形状、大小、部位、边界、平扫和增强后信号及强化方式进行评价.结果 病灶单发14例,另1例有2个病灶.平扫T1WI及T2WI各发现15和14个病灶,增强扫描发现16个病灶.14个病灶最大径≤3 cm.平扫T1WI病灶呈低信号5个,略低信号9个,1个呈等信号伴有周边低信号包膜及内部点状低信号.在T2WI病灶呈高信号5个,略高信号4个,略低信号3个,明显低信号2个,其中2个病灶内见点状或细线样极高信号.16例在增强扫描后各期均呈低信号,尤其在门静脉期及延迟期呈明显低信号,边界及形态显示清楚.12个病灶形状不规则,4个病灶呈圆形或卵圆形.增强后病灶内部均未见强化,3个病灶在门静脉期及延迟期可见细环状轻度强化的包膜.结论 SNN特征性MRI表现有助于与肝脏其他肿瘤鉴别.  相似文献

BACKGROUND AND PURPOSE: Diffusion-weighted MR imaging of the spine has been used to differentiate benign from pathologic vertebral body compression fractures. We sought to determine the utility of diffusion-weighted MR imaging in the detection of vertebral metastases and to compare it with conventional noncontrast T1- and T2-weighted MR imaging. METHODS: Fifteen patients with metastases to the spine were studied using conventional MR imaging and diffusion-weighted imaging. Blinded review of all images was undertaken, and patients were categorized according to whether they had focal or multiple lesions. The signal intensity of the lesions was compared on T1-, T2- (fast spin-echo), and diffusion-weighted images. RESULTS: In five patients with focal disease, metastases were hypointense on T1-weighted images; hypointense (n = 2), isointense (n = 1), or hyperintense (n = 2) on T2-weighted images; and hypointense (n = 3) or hyperintense (n = 2) on diffusion-weighted images with respect to presumed normal bone marrow. In 10 patients with disease in multiple sites, all lesions were hypointense on T1-weighted images; hypointense (n = 2), isointense (n = 4), hyperintense (n = 2), or mixed (n = 2) on T2-weighted images; and hypointense (n = 5), hyperintense (n = 3), or mixed (n = 2) on diffusion-weighted images with respect to presumed normal bone marrow. CONCLUSION: As used in this study, diffusion-weighted MR imaging of the spine showed no advantage in the detection and characterization of vertebral metastases as compared with noncontrast T1-weighted imaging, but was considered superior to T2-weighted imaging.  相似文献

PURPOSEWe evaluated the appearance of enhancing multiple sclerosis (MS) lesions on unenhanced T1-weighted MR images and the natural course of enhancing MS lesions on serial unenhanced T1-weighted and magnetization transfer (MT) MR images.METHODSOne hundred twenty-six enhancing lesions were followed up monthly for 6 to 12 months to determine their signal intensity on unenhanced T1-weighted and MT MR images. At the time of initial enhancement, the size of the lesion and the contrast ratio of enhancement were calculated for each enhancing lesion. During follow-up, the contrast ratio on the corresponding unenhanced T1-weighted image was measured, and an MT ratio (MTR) was calculated.RESULTSTwenty-five enhancing lesions (20%) appeared isointense and 101 lesions (80%) appeared hypointense relative to normal-appearing white matter on unenhanced T1-weighted images. During 6 months of follow-up, four MR patterns of active lesions were detected: initially isointense lesions remained isointense (15%); initially isointense lesions became hypointense (5%, most of which reenhanced); initially hypointense lesions became isointense (44%); and initially hypointense lesions remained hypointense (36%). MTR was significantly lower for hypointense lesions as compared with isointense lesions at the time of initial enhancement. For lesions that changed from hypointense to isointense, MTR increased significantly during 6 months of follow-up. Multiple regression analysis showed that strongly decreased MTR at the time of initial enhancement and enhancement duration of more than one scan were predictive of a hypointense appearance on unenhanced T1-weighted images at 6 months'' follow-up. Ring enhancement was found to be the only (weak) predictor of persistently hypointense signal intensity.CONCLUSIONMost enhancing lesions appear slightly to significantly hypointense on unenhanced T1-weighted images. Although most hypointensities are reversible, only those lesions that fail to recover on unenhanced T1-weighted and MT images may have considerable irreversible structural changes.  相似文献

Spin-echo MR imaging at 0.35 T was used to image hepatic focal nodular hyperplasia (FNH) and to attempt to distinguish it from primary malignant hepatic tumors. There were six FNH and 10 malignant tumors including seven hepatocellular carcinomas, two cholangiocarcinomas, and one hepatoblastoma. Our results show that FNH has a fairly consistent appearance, dissimilar from that of malignant primary hepatic tumors. Four of six FNH lesions were isointense (except for a central scar in three) and indistinguishable from normal hepatic parenchyma on all pulse sequences, whereas two of six were homogeneous but slightly hyperintense on T2-weighted sequences. Furthermore, a central hyperintense scar was seen in three of six lesions on T2-weighted sequences. In contrast, each of the malignant primary hepatic tumors was hyperintense on T2-weighted sequences and seven of 10 were hypointense on T1-weighted sequences; in nine of 10, heterogeneous areas of intensity were noted. In two fibrolamellar hepatocellular carcinomas a central scar was seen that was hypointense on all pulse sequences. By using quantitative data, the best characterization was obtained by using lesion/normal-liver intensity ratios from a T2-weighted sequence; all FNH had a ratio less than 1.33, while in nine of 10 primary malignant tumors it was greater than 1.41. We conclude that focal nodular hyperplasia may have a consistent appearance on spin-echo MR imaging and probably can be distinguished from primary malignant lesions in most instances.  相似文献

Typical ocular and CNS melanomas are hyperintense on T1-weighted MR images and hypointense on T2-weighted MR images. We performed MR imaging in 48 patients with melanoma metastatic to visceral organs. Images were reviewed retrospectively in order to determine whether there were predominant MR features specific for visceral melanoma and to see if visceral metastases have MR characteristics similar to metastases in the CNS. Eleven patients also were examined after injection of gadopentetate dimeglumine to evaluate the enhancement characteristics of these tumors. Two hundred sixty-one lesions were found. Lesions were classified according to their signal intensities relative to uninvolved liver on T1-weighted, T2-weighted, and short TI inversion recovery (STIR) pulse sequences. Most commonly, lesions were either hypointense or isointense on T1-weighted sequences and hyperintense on T2-weighted and STIR sequences (185 lesions). Less frequently, lesions were hyperintense on T1-weighted sequences and hypointense or isointense on T2-weighted and STIR sequences (59 lesions). A mixed pattern was seen on T1- and T2-weighted sequences in 17 lesions. The patterns did not correlate with lesion size. Of the three sequences studied by subjective comparison, the STIR sequence in our series had the highest sensitivity for lesion detection and yielded the highest lesion conspicuity. Injection of gadopentetate dimeglumine in 11 patients did not increase either the number or the conspicuity of lesions seen. Our results show that visceral metastases from melanoma have a wide variety of appearances on MR images. The STIR sequence appears to be optimal, and the metastases do not enhance with gadopentetate dimeglumine.  相似文献

Hamartomas of the spleen usually appear isointense on T1-weighted MR images and hyperintense on T2-weighted images. We describe a histologically proven case which presented as a small (2.5 cm) focal mass isointense to splenic parenchyma on T1-weighted images and hypointense on both turbo-spin-echo T2 and short T1 inversion recovery images. Dynamic MRI revealed a delayed enhancement during the arterial phase becoming isointense and prolonged on subsequent images. This prolonged enhancement has previously been described as a characteristic pattern in these tumours. The lack of oedema and necrosis and the presence of fibrous tissue in the hamartoma at histopathology likely account for the low signal intensity on all sequences. Focal nodular hyperplasia of the liver: MR findings in 35 proved cases   总被引:6,自引:0,他引:6  
MR images of 28 patients with 35 lesions of hepatic focal nodular hyperplasia were reviewed to determine the frequency of findings considered typical of this condition (isointensity on T1- and T2-weighted pulse sequences, a central hyperintense scar on T2-weighted images, and homogeneous signal intensity). Fifteen lesions were imaged at 0.6 T with T1- and T2-weighted spin-echo (SE) pulse sequences; 20 lesions were imaged at 1.5 T with T1-weighted SE and gradient-echo pulse sequences and T2-weighted SE pulse sequences. Diagnosis of focal nodular hyperplasia was made pathologically in 25 patients, with nuclear scintigraphy in four, and with follow-up imaging in six. Only seven lesions (20%) were isointense relative to normal liver on both T1- and T2-weighted images. On T1-weighted SE images, 21 lesions (60%) were isointense relative to normal liver, 12 (34%) were hypointense, and two (6%) were hyperintense. On T2-weighted SE images, 12 lesions (34%) were isointense and 23 (66%) were hyperintense relative to normal liver. A central scar was present in 17 lesions (49%) and was hypointense relative to the lesion on T1-weighted images and hyperintense on T2-weighted images. Twenty lesions (57%) were of homogeneous signal intensity throughout the lesion, except for the presence of a central scar. All three MR imaging characteristics were present in three cases (9%). We conclude that hepatic focal nodular hyperplasia has a wide range of signal intensity on MR imaging.  相似文献
