Maintenance of interdental soft tissue and the need for esthetics are being increasingly recognized as important criteria for implant success. Statistically significant correlations have been found between the incidence of implant failure and vertical bone loss adjacent to implants. Thus, it is widely recognized that peri-implant bone resorption before loading may compromise implant success. This study aims to evaluate the effect of flap elevation on peri-implant bone loss during the healing period. Twenty sites around 10 implants were included in the study, and the effect of 2 different flap designs on the crestal height of bone was evaluated. The results of the study have shown that flap elevation can lead to increased bone loss during the healing period, with statistically significant results up to the 90-day period.  相似文献

The number of obese adolescents is increasing in the Western society. For a deeper understanding of the mechanisms underlying this pathology, the quantitative characteristics of the facial soft tissues should also be investigated. The three-dimensional coordinates of 12 soft tissue facial landmarks were obtained by computerized digitizers in 11 male and 14 female adolescents aged 13-17 years, all with a body mass index larger than 30 kg/m2 (mean 31.67 kg/m2, SD 1.58). From the landmarks, several facial dimensions were calculated. Data were compared with those collected in normal individuals of the same age, ethnicity, and sex by computing z scores. Significant (paired Student's t-test, P < .05) larger dimensions were found for skull base width (girls), mandibular width (both sexes), lower face depth (girls), and mandibular corpus length (girls). In the pooled sample (boys plus girls), the faces of obese adolescents were significantly wider transversally (skull base width, mandibular width), deeper sagittally (mid and lower face depth, mandibular corpus length), and shorter vertically (upper facial height) than those of their normal school companions. "Borderline" obese adolescents possessed some facial characteristics typical of patients with more substantial obesity. The effect of an increased body weight-per-height was therefore present also in subjects not already referred to a medical control.  相似文献

OBJECTIVES: The aim of this prospective study was to evaluate clinically and radiographically the success and esthetic result of immediate implant placement at the time of extraction. MATERIAL AND METHODS: Twelve patients with 14 titanium screw-shaped implants (13-16 mm length and 4.3 or 5 mm diameters) were placed in the extraction sockets. Defects after implant placement were recorded, and then filled up with deproteinized bovine bone mineral, bioabsorbable collagen membrane, and absorbable pins. The defect was again re-evaluated at second-stage surgery. Clinical and radiographic parameters of the peri-implant conditions were assessed at the moment of prosthesis placement and at 1-year follow-up. RESULTS: The cumulative implant survival and success rate was 100% after a 1-year observation period. Analysis of the esthetic result showed that the mean pink esthetic score (PES) was 11.1 (SD 1.35) at 1-year follow-up. At 1 year, 64.3% papillae had a score of 2 and the remaining 35.7% score 3 according to the Jemt (1997) papillary index. Optimal value of width of the keratinized mucosa was recorded in 13 (92.9%) implant cases in both periods of follow-up. At 1-year follow-up, the linear distance between implant-shoulder to the bone peaks remains stable with a mean of 2.62+/-0.2 mm at the mesial and 2.9+/-0.58 mm at the distal aspect. CONCLUSION: Careful evaluation of potential extraction sites before immediate implant installation promotes optimal implant esthetics.  相似文献

The single-tooth implant: a viable alternative for single-tooth replacement   总被引:2,自引:0,他引:2  
BACKGROUND: The use of dental implants for single-tooth replacement cases introduces a challenge to the performance of these restorations. Clinical data suggest that single-tooth implant restorations do not benefit from lateral support from the surrounding dentition as do bridged implants, and that these restorations are subjected to greater masticatory forces. Therefore, single-tooth implants are exposed to an increased risk of failure. This prospective clinical trial was designed to evaluate the cumulative success rate of dual acid-etched single-tooth replacement implants in two clinical centers. METHODS: The study protocol included 59 patients between 19 and 73 years. Implants were evaluated for mobility, infection, and crestal bone loss from 30.9 to 60 months. RESULTS: Seventy-one (71) implants were placed in 59 patients, 20 male and 39 female. Forty-seven (47) were placed in the maxilla and 24 in the mandible. The percentages of anterior and posterior implants were 45.1% and 54.9%, respectively. A total of 13 (18.4%) implants were placed in soft, poor quality (Type IV) bone. Beginning 1 month post-implant placement, cases were followed for 30.9 to 60 months with a mean follow-up of 45.9 months. One implant failed (peri-implant infection) yielding an overall success rate of 98.6%. CONCLUSIONS: This success rate for single-tooth replacement dual acid-etched implants compares favorably with bridged implants and with success rates of other single-tooth implant studies. Dual acid-etched implants performed well even under conditions of poor quality bone, where concomitant bone augmentation was performed, and when used for immediate replacement of extracted teeth.  相似文献

BACKGROUND: The reactive correction of a single tooth anterior crossbite following periodontal therapy is described. This case report provides new information regarding correction of a crossbite relationship and con- firms existing reports of tooth movement following periodontal therapy. METHODS: A 39-year-old woman in good general health presented with a history of recurrent periodontal abscesses of a maxillary incisor. Probing depths of the abscessed tooth ranged from 5 to 12 mm, and class 1 mobility was noted. Radiographs revealed that the tooth had previously been treated endodontically. The patient's periodontal diagnosis was generalized chronic moderate to severe periodontitis. Treatment considerations were complicated by a single-tooth crossbite relationship of the involved incisor and clinical evidence that the periodontal abscess communicated with an apical infection. Treatment of the abscess consisted of cause-related therapy, bone grafting, and occlusal adjustment. RESULTS: Five months after surgical treatment, an edge-to-edge incisal relationship was observed, the first indicator of tooth movement. Further correction to a normal incisal relationship resulted 1 year after modification of the proximal contact. At this time, there was normal probing depth with only slight recession and mobility. Bone fill was radiographically noted. CONCLUSION: It appears that some cases of maxillary incisor crossbite that are complicated by periodontal disease may be corrected, without orthodontic appliances, following periodontal treatment.  相似文献

Orthognathic surgery is undergone to improve facial and dental aesthetics and to improve function. Three dimensional (3D) soft tissue analysis based on stereophotogrammetry provides a realistic measurement of facial morphology. There is a need for objective assessment of surgery outcomes. The study aim was to evaluate the 3D naso-maxillary complex soft tissue morphology following Le Fort I maxillary advancement and compare the findings with a local reference group. 3D images of 112 volunteers were captured using stereophotogrammetry and viewed by 8 lay people; 40 images (16 males and 24 females) were chosen as the reference group to have harmonious facial appearance. The linear and angular measurements of this group were compared with 35 patients (19 female and 16 male) who had maxillary advancement in the post-surgical group. Facial morphology post-surgery was similar to the reference group, except the nasal base width which was wider by 2.3mm in males and 2.6mm in females. In the orthognathic group, the females had a smaller nasolabial angle by 9.7° than the reference group. In conclusion, 3D imaging is a sensitive tool for analysing facial appearance. Compared with a control group, statistical differences were identified in soft tissue morphology which should be considered in surgical planning and patient consent.  相似文献

Predictability of soft tissue form around single-tooth implant restorations   总被引:4,自引:0,他引:4  
The objective of this study was to answer important questions about gingival responses to single-tooth implants: (1) Are papilla regeneration and sulcular recession expected results? (2) Do soft tissue profiles retain their sulcular form over an extended period? and (3) Do single-implant replacements require special restorative handling to achieve predictable soft tissue form? A retrospective, photographic examination was used to follow 55 single-implant restorations in 51 patients for a period from 1 to 9 years (mean 3.5 years). Papillae regenerated in 83.9% of implants for a mean growth of 0.65 mm mesially and 0.62 mm distally. The sulcular apex receded in 59% of patients for a mean of 0.06 mm. Complete papilla fill was noted in 75% of patients examined. Short- to long-term measurements revealed that papilla regrowth continued slightly and that sulcular recession abated. Papilla regeneration around single implants was a predictable outcome in this population; sulcular recession was not a predictable finding. Papilla levels demonstrated a tendency toward increasing height, and sulcular levels remained fairly constant over the long term. Predictable soft tissue profiles were achieved with a simplified implant prosthetic protocol, progressing directly from healing abutments to definitive crowns in most cases.  相似文献

Facial soft tissue injuries are common in pediatric trauma patients. Early diagnosis and definitive treatment as well as good postoperative wound care are important when dealing with soft tissue injuries, such as facial nerve and parotid injuries, animal bites, avulsive skin wounds and eyelid and ear lacerations. Children heal quickly, but they also tend to develop hypertrophic scars. Proper wound management during the healing period can help to minimize the risk of adverse scar formation. Dentists may be involved in the initial assessment of these patients. Knowledge of the diagnosis and management of soft tissue trauma is useful when dealing with pediatric patients and their parents postoperatively.  相似文献

This case report describes the treatment of an adolescent girl with a skeletal Class II Division 2 malocclusion and impinging overbite. One of 2 previously extracted premolars had to be replaced by a single-tooth implant after adequate space reopening. An optimal overbite-overjet relationship was achieved through significant intrusion and proclination of maxillary and mandibular incisors. A horizontally impacted mandibular second molar was repositioned to ensure a 2-molar arch integrity. Resolution of the gingival smile line and favorable facial changes were also obtained.  相似文献

BACKGROUND: The purpose of this study was to compare and evaluate bone and soft tissue levels in immediately restored single implants positioned in the esthetic anterior region. METHODS: Forty-five patients requiring single-implant crowns in the anterior region received 52 implants, which were restored immediately. Periapical radiographs and digital images of 368 interproximal sites were taken during regular follow-ups from 24 to 72 months after surgery. The interproximal papilla was assessed and compared to the distances between the bone crest and the contact point between the natural teeth and the restoration crown. RESULTS: Bone loss in the bone peak was statistically significantly greater in the postextraction sockets compared to the healed sites. No significant correlation was found between bone loss and papilla presence. The interproximal papilla showed average growth and decreased, in percentage terms, when the distance between the bone crest and the contact point between the tooth and the restoration crown was >7 mm. CONCLUSIONS: In the interproximal area between the implant and the natural tooth, the papilla does not seem to be affected by the peri-implant bone loss. The contact point between the natural tooth and the restoration crown should be < or =7 mm from the bone peak. Immediate restoration did not seem to cause a greater average amount of bone loss compared to that reported previously for one- and two-stage surgical procedures after the first year of function.  相似文献

AIM: In this study, the reproducibility of a newly developed pink esthetic score (PES) for evaluating soft tissue around single-tooth implant crowns was assessed. The effect of observer specialization was another point of interest. MATERIAL AND METHODS: Twenty observers (five prosthodontists, five oral surgeons, five orthodontists and five dental students) were given photographs of 30 single-tooth implant crowns. Seven variables were evaluated vs. a natural reference tooth: mesial papilla, distal papilla, soft-tissue level, soft-tissue contour, alveolar process deficiency, soft-tissue color and texture. Using a 0-1-2 scoring system, 0 being the lowest, 2 being the highest value, the maximum achievable PES was 14. Each observer was requested to make two assessments at an interval of 4 weeks. At the second assessment, the photographs were scored in the reverse order. RESULTS: The mean PES of evaluations at the first assessment (n=600) was 9.46 (+/-3.81 SD), and 9.24 (+/-3.8 SD) at the second one. The difference between these two means was not significant statistically (P=0.6379). Implant-related mean PES for single-tooth implants varied from 2.28 to 13.8, with standard deviations between 0.46 and 3.51. Very poor and very esthetic restorations showed the smallest standard deviations. The mean total PES was 10.6 for the prosthodontists, 9.2 for the oral surgeons, 9.9 for the dental students and 7.6 for the orthodontists. CONCLUSIONS: The PES reproducibly evaluates peri-implant soft tissue around single-tooth implants. Thus, an objective outcome of different surgical or prosthodontic protocols can be assessed. Orthodontists were clearly more critical than the other observers.  相似文献

Langerhans cells (LC) are dendritic cells of the immune system able to capture intraepithelial pathogens and migrate to regional lymph nodes to present them to naive T cells. Up to now immunohistological studies on human gingival LC have been carried out using antibodies against HLA-DR or CD1a molecules. A new marker of LC called Langerin (CD207) and described, among other subcellular localisations, in the Birbeck granules is now available in immunohistochemistry. The purpose of this in situ study was to quantify and to compare Langerin+ versus CD1a+ LC number in order to show differences in the expression of these molecules, if any, and to determine which marker is the most specific. The present study was conducted using nine frozen healthy gingival samples. Double immunofluorescence procedures were performed with an anti-Langerin antibody revealed by FITC and with an anti-CD1a-PE antibody. Mounted slides were analysed by fluorescence microscopy and quantifications were performed on projected slides associated with a grid of 0.015 mm(2). Our results have shown that 1/ the number of CD1a+ LC was significantly increased (P=0.01) when compared with Langerin+ LC 2/ 92% of Langerin+ LC co-expressed CD1a 3/ only 82% of CD1a+ cells co-expressed Langerin 4/ a positive correlation was noted between CD1a+ and Langerin+ LC numbers. The present study has revealed the heterogeneity in the phenotype of gingival LC population and shown that Langerin seems the most specific marker for the study of LC.  相似文献

Fifty-nine commercially pure titanium implants in 59 subjects were compared with internal control teeth for 3 years. Nineteen coated implants of identical design were placed in 17 of the subjects and compared with the titanium implants. Demographic data, microbial DNA, aspartate aminotransferase levels, Plaque Index, width of adjacent keratinized tissue, probing depths, bleeding on probing, relative attachment levels, mobility, and radiographic bone height were studied. The only statistically significant changes over time were improved plaque scores in the subjects and slight bone loss around the implants. There were no differences between the 2 types of implants. Mobility was less and probing depth and bleeding on probing were greater in the implant sites than in the control sites.  相似文献

Bilateral temporomandibular joint ankylosis in childhood can result in severe facial deformity characterized by both hard and soft tissue changes. The soft tissue deformity manifests as a submental hump of redundant soft tissue which has been displaced inferiorly due to deficiency of skeletal attachments. Improved results can be produced in these cases if the orthognathic correction is complemented by correction of the soft tissues. The presence of microgenia makes it possible to reach the subplatysmal plane through an intraoral incision allowing for the plication of platysma and relocation of the displaced soft tissues from the cervical to the mental region.  相似文献

AIM: To investigate patient centred outcomes, soft tissue morphology, and bone levels. MATERIAL AND METHODS: Sixty-six subjects, who had completed treatment for a single implant restoration at least l year previously. Appearance was recorded photographically and bone levels and interdental contact points measured from intra-oral radiographs using a x 7 scale loupe. Subjects completed a satisfaction questionnaire. RESULTS: Subjects were highly satisfied with all aspects of the restoration including the appearance of the soft tissue (median shape/colour score 6 on scale 1-6). Twenty-eight sites in 20 subjects had no contact point between implant crown and adjacent tooth. A normal height papilla was judged to be present in 19 of these sites. These were excluded from the subsequent analysis. In the remaining 46 subjects with contact points the presence (JEMT score 3) or deficiency (score 1/2) of the papilla was significantly related to the distance to the bone level on the adjacent tooth and implant head. Differences were observed between the mesial and distal aspects of the implant restoration. CONCLUSIONS: Examining clinicians were more critical of the restorations than the patients. The presence of a complete papilla was associated with a slightly greater distance from contact point to bone level than previously reported.  相似文献
