Introduction Previous studies have found inconsistent relationships of alcohol consumption with risk of hip fracture, and the importance of bone mineral density and risk of falls in mediating such a relationship has not been determined. Methods As part of the Cardiovascular Health Study, a population-based cohort study of adults aged 65 years and older from four U.S. communities, 5,865 participants reported their use of beer, wine, and liquor yearly. We identified cases of hip fracture unrelated to malignancy or motor vehicle accidents using hospitalization discharge diagnoses. A subgroup of 1,567 participants in two communities underwent dual-energy x-ray absorptiometry scans to assess bone mineral density. Results A total of 412 cases of hip fracture occurred during an average of 12 years of follow-up. There was a significant U-shaped relationship between alcohol intake and risk of hip fracture (p quadratic 0.02). Compared with long-term abstainers, the adjusted hazard ratios for hip fracture were 0.78 (95% confidence interval [CI], 0.61–1.00) among consumers of up to 14 drinks per week and 1.18 (95% CI, 0.77–1.81) among consumers of 14 or more drinks per week. Alcohol intake was associated with bone mineral density of the total hip and femoral neck in a stepwise manner, with approximately 5% (95% CI, 1%–9%) higher bone density among consumers of 14 or more drinks per week than among abstainers. These relationships were all similar among men and women. Conclusions Among older adults, moderate alcohol consumption has a U-shaped relationship with risk of hip fracture, but a graded positive relationship with bone mineral density at the hip. 目的评估骨密度在髋部脆性骨折风险预测中的临床价值。方法回顾性研究2014年6月至2019年6月在我院创伤骨科住院的老年髋部骨折患者72例,作为病例组,其中股骨转子间骨折31例,股骨颈骨折41例;对照组选择同期我院骨外科门诊老年体检者63例。使用DXA方法测量患者腰椎和健侧髋部(全髋部、转子间、股骨颈、Ward’s区)的骨密度;对照组测量腰椎和左侧髋部骨密度,统计分析测量结果。结果①骨折组腰椎、髋部骨密度均显著低于对照组,差异有统计学意义(P0.01);②转子间骨折组和股骨颈骨折组在腰椎和髋部区域骨密度比较差异均无统计学意义(P 0.05);③骨折组与对照组在转子间区的T值降低比例最大为122.1%,腰椎降低幅度最小为31.3%,余髋部的T值均有不同程度降低;④骨折后髋部和腰椎T值比存在倒置现象;⑤对照组和骨折组髋部骨质疏松程度比较,差异有统计学意义(P0.01);两组患者腰椎骨质疏松程度比较,差异无统计学意义(P0.05)。结论①髋部骨折患者骨密度均显著低于体检者,提示骨密度与髋部骨折具有一定相关性,但与髋部骨折类型无关;②在髋部骨折风险评估中,髋部骨密度相比腰椎更有价值;③当髋部与腰椎T值比出现倒置时,将不可避免发生髋部骨折;④骨量正常的部分患者发生了脆性骨折,而骨质疏松的部分患者却未发生骨折,表明影响骨折发生的因素除了骨密度外,可能和骨骼的微结构有关。  相似文献

Seventy-four postmenopausal women with nonpathological hip fracture were recruited to a study in which they were compared for lifetime factors, some biochemical measurements of bone metabolism, and bone mineral density (BMD), with 40 age-adjusted controls without fracture. The fracture patients were less independent; their walking ability was weaker; their vision was poorer; they had more general diseases (strokes, diabetes, malignant diseases, heart and vascular diseases); more of them had had deliveries; and they were using significantly more loop diuretics, and antidepressant, neuroleptic, and diabetes drugs than the controls. Thirty-seven patients and 19 controls were excluded from the statistical comparison of BMD and the biochemical measurements of bone metabolism because they had had treatments with calcium, vitamin D, bisphosphonates, estrogens, calcitonin, or corticosteroids, and one fracture patient was excluded for primary hyperparathyroidism. The BMD of the upper femur was significantly lower in the fracture group compared with the control group. Serum total calcium (S-Ca) and serum vitamin D (S-25-(OH)-D) were significantly lower and the levels of calcitonin (S-CT) significantly higher in the fracture group than in the control group, but none of the bone formation markers showed significant differences between the study groups. A comparison of patients with cervical and trochanteric fractures showed BMD to be significantly lower in the upper femur in the trochanteric fracture group. There were no significant differences in the biochemical measurements (with the exception that S-CT was higher in the cervical fracture group), nor in the lifetime factors between the fracture types. In conclusion, some lifetime factors and low S-Ca, low S-25-(OH)-D, high S-CT, and low BMD of the upper femur seem to be related to the risk of hip fracture, and low BMD and low S-CT seem to be related to the trochanteric fracture type in postmenopausal women.  相似文献

Studies on patients with degenerative joint disease of the hip show that femoral periprosthetic bone mineral decreases following total hip arthroplasty. Scarcely any osteodensitometric data exist on femoral neck fracture (FNF) patients and periprosthetic bone remodelling. In two parallel cohorts we enrolled 87 patients (mean age, 72 ± 12 years; male:female ratio, 30:57) undergoing total hip arthroplasty for either primary osteoarthritis (OA) of the hip (n = 37) or for an acute FNF (n = 50) and followed them for a mean of 5.4 years. Outcomes were bone mineral density (BMD) changes in the periprosthetic Gruen zones 1–7, the incidence of periprosthetic fractures and clinical outcome. The bone mineral loss in the fracture group was more than twice that of the osteoarthritis group, ?16.9% versus ?6.8% (p = 0.004). The incidence of periprosthetic fractures was 12% (6/50) in the fracture cohort compared with none (0%) in the OA cohort (p = 0.03). Periprosthetic bone mineral loss following total hip arthroplasty is significantly greater in patients who are treated for acute FNF than in OA patients. This decrease of BMD follows a different pattern with the FNF patients losing larger proportions of bone in Gruen zones 1, 2, 6, and 7 while the OA patients tend to have larger losses only in zones 1 and 7. © 2015 Orthopaedic Research Society. 目的对髋部、肱骨近端骨折老年女性的骨密度和骨代谢指标进行对比分析,进一步揭示上述骨折部位女性患者骨密度和骨代谢指标特征性变化情况。方法经患者及家属同意,共纳入62例老年髋部骨折女性患者(其中股骨颈骨折39例,股骨粗隆间骨折23例)、肱骨近端骨折21例,收集患者年龄、检测患者骨密度、血清骨转换指标(Ⅰ型胶原氨基端延长肽,P1NP;Ⅰ型胶原C端肽β降解产物,β-CTX)。结果肱骨近端骨折女性患者平均年龄为(66.1±8.0)岁,明显小于股骨颈骨折、粗隆间骨折女性患者(P<0.05);肱骨近端骨折女性髋部(T=-1.19±0.66)、腰椎骨密度(T=-1.67±1.00)明显高于粗隆间骨折女性髋部(T=-2.36±1.17)、腰椎骨密度(T=-2.61±1.42)(P<0.05),同时显著高于股骨颈骨折患者髋部骨密度(T=-2.33±0.99)。股骨颈骨折、股骨粗隆间骨折患者髋部、腰椎骨密度相比差异无统计学意义;三组间血清P1NP比较差异没有统计学意义,粗隆间骨折女性血清β-CTX(732.18±334.37μg/L)要明显高于肱骨近端骨折患者(529.66±292.34μg/L)(P<0.05)。结论相对于髋部骨折患者,肱骨近端骨折老年女性患者年龄较低,骨密度相对较高;骨吸收活跃可能是导致粗隆间骨折女性骨密度下降的原因。  相似文献

目的探讨类风湿关节炎(rheumatoid arthritis,RA)患者骨密度的特点及其骨折的风险度,并分析其影响因素。方法选取2016年1月至2017年12月于我院已确诊的RA患者共70例(RA组),体检的正常健康人群共79例(对照组),收集两组人群的病史及骨折风险相关的基本临床资料,且采用双能X线骨密度仪测定两组人群股骨的3D骨密度、二维骨密度,并进行骨折跌倒评分及FRAX评分,分析RA患者的骨密度变化情况、骨折风险性和疾病的活动度,探讨影响RA骨密度减低的因素。结果与正常对照组相比,RA组患者无论是体积骨密度还是皮质厚度都明显降低(t=6.135,P0.01),且均与二维BMD的变化趋势相一致;RA患者骨折风险度明显高于正常对照组人群(t=6.663,P0.01),RA患者疾病的活动度与骨密度的改变之间无明显相关性(r=-0.085,P0.05),但激素的使用是其骨量流失的影响因素(χ~2=12.366,P0.05)。结论RA患者存在明显的骨量流失和骨折风险性增加,骨密度的改变主要表现为骨小梁和骨皮质密度均减低,因此,积极控制原发病并预防糖皮质激素性骨质疏松有重要的临床意义。  相似文献

目的血清H_2S水平与骨质疏松的相关性。方法收集2015年10月至2017年10月112例在江苏昆山地区体检和门诊就诊人员的资料,其中男82例,女30例,年龄25~86岁,平均(51.40±10.60)岁;采用DEXA骨密度仪检测该人群的骨密度,以及采用亚甲基蓝法测定血清H_2S水平。结果体内血清H_2S水平与髋部骨密度呈正相关,与腰椎骨密度无明显相关性。结论血清H_2S水平与髋部骨密度存在相关,可能是髋部骨密度下降的独立危险因素,H_2S可能是骨密度的保护因素。  相似文献

In the present study, bone mineral density (BMD) of femoral neck and lumbar spine was compared between 38 Japanese female patients with hip fracture (age 63–89 years, mean±SD 76±7 years) and 162 age-matched female controls (age 62–90 years, mean±SD 75±7 years). BMD was measured in the femoral neck and lumbar spine (L2–4) using dual-photon absorptiometry (Norland model 2600). BMD values of femoral neck as well as lumbar spine were significantly lower in patients with hip fracture than in controls (0.504±0.097 v 0.597±0.101,p<0.01, for femoral neck; 0.661±0.146 v 0.720±0.128,p<0.05, for lumbar spine). Patients with hip fracture and controls were stratified according to their BMD levels at two measuring sites, and the ratio of the number of patients and controls at each BMD level was calculated as an indicator of fracture rate. This ratio showed an exponential increase as the femoral neck BMD declined, but only a gradual increase as the lumbar spine BMD declined. Specificity-sensitivity analysis revealed that BMD values of 0.59 and 0.54 g/cm2 at the femoral neck provided a specificity of 52% and 68% with a sensitivity of 90% and 75%, respectively. These findings suggest that Japanese patients with hip fracture are more osteoporotic than age-matched controls and that the selective measurement of femoral neck would be useful for predicting the risk of hip fracture.  相似文献

Causes of secondary osteoporosis are diverse, and bone changes in this condition have been elucidated less than those in primary osteoporosis. In this study, bone mineral density (BMD) was measured in the lumbar spine, distal and proximal sites of the radius, and calcaneus in representative disorders that cause secondary osteoporosis to evaluate its changes. Also, the incidence of nontraumatic vertebral fracture was examined. The subjects were 80 patients with rheumatoid arthritis, 50 patients undergoing glucocorticoid (steroid) therapy, 20 patients with chronic hepatitis, 24 patients with liver cirrhosis, 14 patients with primary biliary cirrhosis (PBC), 26 patients with diabetes mellitus, and 20 postgastrectomy patients; all were ambulatory female outpatients. Two hundred females with primary osteoporosis were examined as a control group. The reproducibility of the measurement of the BMD was satisfactory at about 3% by all methods of measurement employed. Concerning changes in BMD, periarticular trabecular bone density was most markedly reduced in the rheumatoid arthritis group. The patients receiving steroid therapy showed the greatest decreases in the trabecular bone mineral density at the distal 4% of the radius and lumbar spinal BMD. In addition, the threshold of vertebral fracture was higher in those undergoing steroid therapy than in those with primary osteoporosis. The patients with PBC showed the greatest decreases in BMD among patients with chronic liver disorders, and no decrease in BMD was noted in the chronic hepatitis group. BMD was reduced only in the radius in the patients with diabetic mellitus, and it was generally reduced in the postgastrectomy patients. BMD of the calcaneus was not reduced in any group. 目的观察吸入糖皮质激素治疗对慢性阻塞性肺疾病(chronic obstructive pulmonary diseases,COPD)患者的骨密度及骨折风险的影响。方法 122例COPD患者,70例吸入糖皮质激素的当前使用者和52例从未接受过糖皮质激素治疗的患者,进行了骨密度(bone mineral density,BMD)和身体成分评估,并接受了椎体骨折评估(verterbral fracture assessment,VFA)。考虑用于分析的骨病的风险因素是年龄、性别、吸入糖皮质激素使用、体质量指数(body mass index,BMI)、肌肉质量指数(muscle mass index,MMI)和慢性阻塞性肺病全球倡议(GOLD)类别。同时还使用了针对汉族人群的骨折风险评估工具(FRAX)计算工具。结果这些组间在性别、BMI、MMI、GOLD等级、BMD T评分和Z评分的最低值,骨质疏松症的患病率或年龄的低BMD方面没有差异(P0.05)。在使用吸入糖皮质激素的14例患者中通过VFA鉴定椎骨骨折,并且在没有接受糖皮质激素的患者中通过VFA鉴定椎体骨折(P0.05)。MMI与骨质疏松症之间存在关联的趋势(P0.05),并且根据通过GOLD评分评估的COPD严重性,BMD Z评分逐渐降低。椎体骨折与骨质疏松症(P0.05)或低MMI(P0.05)无关。FRAX没有估计骨折风险。结论吸入糖皮质激素可导致骨密度降低,但是骨折风险未发现增加。  相似文献

Osteoporosis (OP) is one of the most prevalent bone diseases worldwide with bone fracture the major clinical consequence. The effect of OP on fracture repair is disputed and although it might be expected for fracture repair to be delayed in osteoporotic individuals, a definitive answer to this question still eludes us. The aim of this study was to clarify the effect of osteoporosis in a rodent fracture model. OP was induced in 3‐month‐old rats (n = 53) by ovariectomy (OVX) followed by an externally fixated, mid‐diaphyseal femoral osteotomy at 6 months (OVX group). A further 40 animals underwent a fracture at 6 months (control group). Animals were sacrificed at 1, 2, 4, 6, and 8 weeks postfracture with outcome measures of histology, biomechanical strength testing, pQCT, relative BMD, and motion detection. OVX animals had significantly lower BMD, slower fracture repair (histologically), reduced stiffness in the fractured femora (8 weeks) and strength in the contralateral femora (6 and 8 weeks), increased body weight, and decreased motion. This study has demonstrated that OVX is associated with decrease in BMD (particularly in trabecular bone) and a reduction in the mechanical properties of intact bone and healing fractures. 目的探讨应用FRAX联合腰椎或股骨颈骨密度评估中老年女性骨折风险。方法选取2017年3月至2018年6月在连云港市第一人民医院行骨密度检查的女性337例,收集其个人基本信息、FRAX风险评估值及腰椎和股骨颈骨密度,按照骨密度及年龄分组,根据上述资料计算10年内主要部位骨质疏松性骨折概率和10年内髋部骨折概率,比较FRAX联合腰椎或股骨颈骨密度评估骨折风险的差异。结果无论代入股骨颈骨密度还是腰椎骨密度计算骨折风险,骨质疏松组的骨折风险均高于骨量减少组(P0.05);②无论是在骨质疏松组还是在骨量减少组,采用股骨颈骨密度计算的骨折风险均高于采用腰椎骨密度计算的值(P0.05)。③进一步分析显示,不同年龄组采用股骨颈骨密度计算出的骨折风险均高于采用腰椎骨密度计算的值(P0.05)。结论对于不同年龄组的骨量异常女性,FRAX联合股骨颈骨密度预测的骨折风险高于联合腰椎骨密度预测的骨折风险。  相似文献

A statistical model for predicting a woman's lifetime risk of hip fracture using her bone mineral density at menopause has been proposed by Black et al. (1992b). We made an additional assumption concerning the correlation of bone mineral density between any two ages among postmenopausal women and applied the modified model to baseline ages between 50 and 85 years and any bone mineral density level likely to be observed in the population. 目的 探讨高龄、跌倒、骨密度等对老年脊柱骨折的预测诊断价值分析。方法 选择2014年4月至2016年4月在我院体检中心就诊的212名老年女性为研究对象,根据两年后是否发生脊柱骨折分为骨折组79例和未骨折组133例,比较两组患者的高龄、跌倒、骨密度等因素的危险指数,采用 SPSS 22.0和STATA 14.0软件对所得数据进行统计分析。结果 骨折组患者较未骨折组患者具有更高的年龄和跌倒倾向,而两组的骨密度无显著差异(P=0.0614),骨密度对老年脊柱骨折的预测价值有限,高龄和跌倒对老年脊柱骨折的预测有重要价值。结论 通过骨密度检测评估是否干预老年人脊柱骨折意义有限,高龄和跌倒是老年人脊柱骨折的独立危险因素。  相似文献

Structural failure becomes increasingly likely as the load on bone approximates or exceeds the bones ability to withstand it. The vertebral fracture risk index (FRI) expresses the risk for structural failure as a ratio of compressive stress (load per unit area) to estimated failure stress, and so should be a more sensitive and specific predictor of vertebral fracture than spine areal BMD (aBMD) or volumetric BMD (vBMD), surrogates of bone strength alone. To address this issue, we analyzed the results of a case-control study of 89 postmenopausal women with vertebral fractures and 306 controls in Melbourne, Australia, and a 10-year community-based prospective study in which 30 postmenopausal women who had incident vertebral fractures were compared with 150 controls in Lyon, France. The FRI and vBMD of the third lumbar vertebral body and spine aBMD were derived using dual X-ray absorptiometry. In the cross-sectional analysis, each SD increase in FRI was associated with 2.1-fold (95% confidence interval [CI], 1.55–2.73) increased vertebral fracture risk, while each SD decrease in aBMD or vBMD was associated with 4.0-fold (95% CI, 2.69–6.18 and 2.65–6.94, respectively) increase in risk. Using receiver operating characteristic (ROC) analysis, the FRI was less sensitive and specific than aBMD in discriminating cases and controls (area under ROC, 0.76 vs 0.84, p <0.01). The area under ROC curve did not differ between FRI and vBMD (0.76 vs 0.79, NS). In the prospective data set, the FRI was not predictive [hazard ratio, HR, 1.20 (95% CI, 0.9–1.7)] and was in contrast to aBMD [HR, 2.4 (95% CI, 1.5–3.8)] and vBMD [HR, 2.1 (95% CI, 1.39–3.17)]. There was also lower sensitivity using a cutoff value of FRI 1 compared with aBMD T -score of –2.5 SD in both studies. There was poor agreement (kappa=0.13–0.18) between FRI and aBMD T -scores in detecting fractures; each method only identified around 50% of fractured cases. Within the constraints of the sample size, we concluded that applying a biomechanical index such as FRI at the spine is no better in discriminating fracture cases and controls than conventional aBMD or vBMD. The FRI may not predict incident vertebral fractures.  相似文献

目的明确骨质疏松女性股骨颈骨折与股骨转子间骨折的髋部骨密度差异,探讨骨质疏松患者发生髋部骨折(股骨颈骨折、股骨转子间骨折)与骨折部位骨密度的相关性。方法回顾性分析2015年1月1日至2016年12月31日期间于福建省某三级甲等医院的住院治疗的骨质疏松及髋部骨质疏松性骨折患者172例,其中无病史的原发骨质疏松患者109例、骨质疏松性股骨颈骨折患者39例、骨质疏松性转子间骨折24例。分别统计3组患者年龄、体质指数、糖尿病患病情况、骨折侧别、髋部各部位骨密度、血清Ⅰ型胶原交联C末端肽(C-terminal crosslinking telopeptide of type Ⅰ collagen,CTX)、Ⅰ型原胶原N-端前肽(procollagen type ⅠN propeptide,PINP)、25羟基维生素D(25-OH-D)。分别进行三组间及两两组间比较。结果三组间年龄、体质指数、糖尿病患病率差异无统计学意义(F=2.667,P=0.072; F=0.882,P=0.416;χ~2=3.216,P=0.232),股骨颈骨折组与股骨转子间骨折组组间骨折侧别差异无统计学意义(χ~2=0.958,P=0.328),三组间髋部Ward区骨密度差异无统计学意义(F=2.937,P=0.056),髋部骨密度比较,股骨颈、股骨大转子、股骨转子间、髋部整体差异有统计学意义(F=7.825,P=0.001; F=8.668,P0.001; F=9.657,P0.001)。股骨颈骨折组、转子间骨折组股骨颈、股骨大转子、股骨转子间、髋部整体骨密度均小于骨质疏松组,差异均有统计学意义(P0.05);股骨颈骨折组与转子间骨折组股骨颈、大转子、转子间、髋部整体骨密度差异均无统计学意义(P0.05)。三组间β-CTX、P1NP、维生素D差异均有统计学意义(P0.05)。股骨颈骨折组、转子间骨折组β-CTX、维生素D均小于骨质疏松组,差异均有统计学意义(P0.05);转子间骨折与骨质疏松组P1NP差异无统计学意义,股骨颈骨折组与转子间骨折组股骨颈、大转子、转子间、髋部整体骨密度差异均无统计学意义(P0.05)。结论女性骨质疏松患者发生髋部骨折的类型可能并不取决局部的骨密度,可能与骨微结构等因素相关,要得到明确、可靠的结果仍需进一步研究证实。  相似文献

目的 分析营养指标对维持性血液透析患者骨密度及骨折风险的影响。方法 横断面+前瞻观察性队列研究。设非慢性肾脏病对照,测定跟骨超声骨密度,透析患者同时评估臀围等营养指标,随访骨折发生情况。结果 入组透析患者131例、对照77例;透析患者骨密度T值低于对照(中位值分别为?2.9和?2.0, P<0.001),多种因素校正后仍如此(F=11.042,P=0.001);多元线性回归分析显示骨密度与透析、年龄、性别相关(P值均<0.05),与腰围、体质指数等无关,对血液透析患者单独分析则骨密度只与年龄、性别相关(P 分别为0.02,0.005)。平均随访81.0月,透析患者发生骨折10例(7.6 %);Logistic回归分析显示在调整了骨密度、糖尿病、甲状旁腺激素等指标后透析龄长、臀围大、体质指数低、贫血重的患者骨折风险高[OR((95% CI)分别为1.029(1.007~1.051),1.485(1.016~2.171),0.414(0.193~0.891),0.827(0.691~0.989);均P <0.05]。结论 血液透析患者骨密度低于对照,并且独立于营养指标等多种因素;臀围、体质指数等指标影响新发骨折风险,提示透析患者减小臀围、提高体质指数可能减少骨折风险。  相似文献

 We sequentially measured the periprosthetic bone mineral density (BMD) of the femur after cementless total hip arthroplasty, using dual-energy X-ray absorptiometry, over a 3-year period. The periprosthetic bone was divided into three regions (proximo-medial, middle, and distal to the prosthetic stem). After the insertion of a fully porous coated stem in 21 patients, the BMD was measured within 3 weeks, and 6, 12, 24, and 36 months after surgery. At 6 months, all zones showed a decrease in BMD relative to the BMD within 3 weeks, but subsequently the BMD was unchanged. The lower the BMD within 3 weeks of surgery, or the lower the body weight, the higher the percent loss of BMD at 6 months.
