目的 :研究桡腕背侧 (dorsalradiocarpal,DRC)韧带的形态结构、分布特点及力学特性。 方法 :对76侧成人尸体上肢标本 ,解剖观察DRC韧带的形态特征 ;16侧新鲜标本用来作切片观察DRC韧带的组织结构和进行生物力学测试。结果 :DRC韧带形态有 4种类型 :⑴DRC韧带由单一部分起源于桡骨远端背侧Lister结节前及其尺侧占 3 3 % ( 2 5侧 ) ;⑵DRC韧带由两部分组成 ,起自Lister结节前及其尺侧占3 0 % ( 2 3侧 ) ;⑶DRC韧带由起源于Lister结节及其桡侧的两个部分组成占 17% ( 13侧 ) ;⑷DRC韧带由3部分组成 ,分别起始于Lister结节及其尺、桡侧占 2 0 % ( 15侧 )。DRC韧带主要以粗大纵向排列的胶原纤维组成 ,最大拉伸力是 ( 2 2 7.6± 2 6.2 )N。结论 :⑴DRC韧带存在 4种不同起始类型 ;⑵该韧带对维持腕关节稳定具有重要作用 ,应重视DRC韧带损伤的修复。  相似文献

The surface contour of articular cartilage in an intact, loaded joint   总被引:2,自引:0,他引:2  
The friction coefficients measured in diarthrodial joints are small. Theories of joint lubrication attribute this efficiency to entrapment or movement of synovial fluid, yet anatomical models of the surface are based on studies of isolated fragments of cartilage, not functional joints. To investigate the functional interrelationship of joint surfaces and synovial fluid, the ultrastructure of loaded joints was examined. Twenty-four New Zealand white rabbit knee joints were loaded either statically or moved ex vivo using simulated muscle forces and then plunge-frozen under load. After fixation in the frozen/loaded state by freeze-substitution fixation, the medial joint compartments were embedded in epoxy resin while still articulated. Bone was trimmed away from the articular surfaces, permitting the cartilage to be sectioned for light and electron microscopy. These joint surfaces were then compared with controls which were not loaded, not moved or had been disarticulated prior to embedding. Articular surfaces of loaded joints were smooth at magnifications from ×35 to ×7500, whereas the tibial surfaces of nonloaded joints were irregular. Small pools of joint fluid were observed at the meniscal edge and beneath the anterior horn of the meniscus. At magnifications of ×40000, the joint surfaces were separated by a uniform 100 nm space containing fluid. An amorphous, electron dense articular surface lamina was present but, when loaded, was thicker and flatter than previously reported. No surface pits or bumps were visible in embedded, loaded joints. This is the first ultrastructural study of intact loaded joints. The findings suggest that fluid film lubrication is present in diarthrodial joints, but the fluid sequestration postulated in several models is not apparent.  相似文献

Background

While degenerative changes to the articular cartilage of the anterior and distal portions of the femoral condyles have been well studied in the literature, the changes that occur on the posterior femoral condyle are not as clear. The purpose of this study was to assess the difference in articular cartilage thickness between the medial and lateral posterior femoral condyles in knees undergoing unicompartmental knee arthroplasty.

Methods

A retrospective review of prospectively gathered data on 107 consecutive patients undergoing unicompartmental knee arthroplasty performed by a single surgeon was performed. The remaining articular cartilage thickness after resection of the posterior femoral condyle was measured and simple analysis conducted to compare cartilage thickness between medial and lateral posterior femoral condyles.

Results

Ninety-two medial unicompartmental arthroplasties and 15 lateral unicompartmental arthroplasties were performed during the 16?month study period. The majority of lateral UKA patients were female and had lower BMI than medial UKA patients. The articular cartilage thickness on the medial posterior femoral condyle was 3?mm?±?1?mm (mean?±?standard deviation) and 1?mm?±?1?mm on the lateral side (p-value < 0.001).

Conclusions

There is a significant difference in articular cartilage thickness between the medial and lateral posterior femoral condyles in patients undergoing unicompartmental knee arthroplasty. This coincides with a potentially inherently different pattern of articular cartilage degeneration between the medial and lateral compartments of the knee and has implications on implant designs and resurfacing techniques about the knee.  相似文献

Studies have determined the effects of joint immobilization on the articular cartilage of sedentary animals, but we are not aware of any studies reporting the effects of joint immobilization in previously trained animals. The objective of the present study was to determine whether exercise could prevent degeneration of the articular cartilage that accompanies joint immobilization. We used light microscopy to study the thickness, cell density, nuclear size, and collagen density of articular cartilage of the femoral condyle of Wistar rats subjected to aerobic physical activity on an adapted treadmill five times per week. Four groups of Wistar rats were used: a control group (C), an immobilized group (I), an exercised group (E), and an exercised and then immobilized group (EI). The right knee joints from rats in groups I and EI were immobilized at 90 °C of flexion using a plastic cast for 8 weeks. Cartilage thickness decreased significantly in group I (mean, 120.14 ± 15.6 μm, < 0.05), but not in group EI (mean, 174 ± 2.25), and increased significantly in group E (mean, 289.49 ± 9.15) compared with group C (mean, 239.20 ± 6.25). The same results were obtained for cell density, nuclear size, and collagen density (in all cases, < 0.05). We concluded that exercise can prevent degenerative changes in femoral articular cartilage caused by immobilization of the knee joint.  相似文献

Purpose: Thicknesses of fresh equine articular cartilage surfaces from the fetlock, carpal and stifle joints were measured employing a needle probe test.

Materials and methods: Eighty-seven samples used in measurement were cultivated from fetlock, carpal and stifle joints of 12 deceased within 4?h of death. After approximately three minutes of exposure to air during dissection, all cartilage samples were preserved in a saline solution to keep the articular cartilage hydrated for testing. The thickness was measured on five different spots on the same sample. The thicknesses of the fetlock, carpus and stifle were compared.

Results: The articular cartilage of the stifle was thicker than the fetlock and carpus, while the fetlock and the carpus had similar thickness values. The average thickness of the fetlock, carpal and stifle joint are 0.86, 0.87 and 2.1?mm, respectively. They were statistically compared using the Student t-test. The frequency, nature and tissue distribution of localized amyloid deposits in articular cartilage of young and elderly patients, with and without evidence of arthritic disease, was determined. Localized amyloid deposits in articular cartilage were not found in young patients with osteoarthritis of the hip or chondromalacia/osteoarthritis of the patella. However, in elderly patients with osteoarthritis of the hip, amyloid deposits were commonly found, although at no greater frequency than in elderly patients with no evidence of arthritis. Amyloid deposits were commonly present (in 95% of cases) in osteoarthritis of the knee joint and in the articular cartilage of all joints containing pyrophosphate deposits. Similar deposits of amyloid were also found in the articular cartilage of 40-45% of rheumatoid joints. These findings indicate that localized amyloid deposits in the articular cartilage are largely age-related and not due to specific pathological alterations affecting articular cartilage.  相似文献

Purpose

This study examined the rate of degeneration of acetabular cartilage by the bipolar head according to time, and also which clinical factors are related to the degeneration of acetabular cartilage.

Materials and Methods

Among 192 patients (226 hip joints) who received bipolar hemiarthroplasty from August 1996 to August 2002, 61 patients (65 hip joints) were enrolled in this study, who were followed up for more than 2 years and showed no signs of dislocation, infection, or functional problems. A modified form of a computer assisted vector wear analysis program was used to measure the rate of degeneration of the acetabular cartilage. The factors that appeared to affect the rate of acetabular degeneration in the two groups was evaluated.

Results

The average linear degenerative change in the acetabular cartilage and the volumetric degenerative change were 0.23 ± 0.107 mm/year and 114 ± 47.2 mm3/year, respectively. The result showed significant differences in activity and HHS between the 2 groups. The HHS showed a reverse relationship with the linear degeneration and volumetric degeneration, and the activity showed a correlation with the linear and volumetric degeneration.

Conclusion

The acetabular cartilage degenerates faster as the patient'' activity increases, and slower with a higher HHS. When surgeons perform hip joint arthroplasty, it is strongly recommended that the life expectancy and the level of activity should be considered when deciding between a hemiarthroplasty and total hip arthroplasty.  相似文献

文题释义: 自体软骨细胞移植:对于3.5-10 cm2的软骨缺损或多个缺损来说,自体软骨细胞移植是一种有效的软骨修复措施,取少量患者自体软骨于体外培养软骨细胞,并增殖到一定数量后植入软骨缺损处,从而达到修复缺损的目的。 基质诱导的自体软骨细胞移植:把经培养增殖后的软骨细胞接种到Ⅰ/Ⅲ型双层胶原膜上,继续培养数日,细胞与支架结合紧密之后,使用生物蛋白胶粘贴到关节软骨缺损病灶底部。术后,软骨细胞从胶原膜上游离并穿过生物胶,迁徙到软骨缺损的基底部。胶原膜和生物胶逐步降解并被吸收。接种的软骨细胞在局部生长、繁殖,并分泌基质,形成新的软骨组织修复缺损。背景:由于关节软骨具有复杂的生物学特性和高度的耐用性,自然退变或创伤引起的缺损都可能导致其结构和功能上不可逆的损害,因此关节软骨损伤后的修复治疗是临床上急需解决的问题。 目的:报告关节软骨修复技术失败最常见的危险因素及其发生率,分析影响选择特定手术治疗方法来处理软骨修复失败最重要的因素。 方法:以“articular cartilage, repair, clinic/clinical failure, surgery”为检索词,检索 PubMed和MEDLINE数据库,时限为2007至2019年,语言限制为英文。初检得到文献约343篇,根据纳入排出标准筛选,共纳入38篇文章进行分析。 结果与结论:①微骨折术和软骨镶嵌成形术在关节软骨修复后的前期和中期显示出不可忽视的失败率,而使用自体软骨细胞移植和异体骨软骨移植修复关节软骨的效果更好。②对于软骨修复失败的治疗:在以往软骨修复失败的患者中应用异体骨软骨移植可能是一个安全的选择,但对于失败的异体骨软骨移植的修复则有更高的失败率;而既往自体软骨细胞移植或基质诱导的自体软骨细胞移植失败的患者,经进一步的自体软骨细胞移植或基质诱导的自体软骨细胞移植治疗后,其治疗效果是可以接受的。此外,有软骨下骨髓刺激病史的患者,自体软骨细胞移植的失败率更高。③软骨修复失败的处理取决于手术治疗失败的类型以及软骨缺损的面积、部位的不同,异体骨软骨移植是治疗软骨下骨髓刺激患者软骨修复失败的最可靠的方法,而自体软骨细胞移植或基质诱导的自体软骨细胞移植在既往软骨修复失败的患者中显示出可以接受的治疗效果,在处理软骨修复失败的患者时,应该特别注意软骨下骨质的情况。ORCID: 0000-0002-3907-9145(张宇) 中国组织工程研究杂志出版内容重点:人工关节;骨植入物;脊柱;骨折;内固定;数字化骨科;组织工程  相似文献

背景:自体软骨因为软骨细胞密度低,新陈代谢率低以及没有血供而导致自我修复效果不理想。目的:就近年来组织工程技术的最新研究和外科治疗软骨损伤最新疗效方面一些最新研究及进展做一综述。方法:由第一作者检索PubMed数据库1992年1月至2013年12月文献,在标题和摘要中以“Articular cartilage;injury;tissue engineering;repair”为检索词进行检索。根据纳入排除标准最终选取61篇文献进行分析。  结果与结论:目前临床上常见的治疗关节软骨损伤方法有关节清理、镶嵌式成形技术、软骨膜及自体软骨细胞移植技术等,但都没研究表明某种治疗方法可以达到满意的长期疗效。其中主要的一个原因就是无法在软骨损伤处形成新的透明软骨,且修复的组织在机械性能上也无法和原有的自体软骨相比较。组织工程技术是今年来研究的热点,因其可以诱导自体软骨的形成而成为一种最有前景的治疗方法。组织工程技术利用内源性或者外源性的种子细胞,辅以诱导因子及营养因子诱导软骨的自身修复,以修复再生成有和自身软骨相似,甚至相同生物性能的透明软骨为最终的目标。如何简化治疗步骤和降低治疗费用是促进软骨再生修复进一步研究的动力。 中国组织工程研究杂志出版内容重点:组织构建;骨细胞;软骨细胞;细胞培养;成纤维细胞;血管内皮细胞;骨质疏松;组织工程全文链接:  相似文献

Controlling the chondrocytes phenotype remains a major issue for cartilage repair strategies. These cells are crucial for the biomechanical properties and cartilage integrity because they are responsible of the secretion of a specific matrix. But chondrocyte dedifferentiation is frequently observed in cartilage pathology as well as in tissue culture, making their study more difficult. Given that normal articular cartilage is hypoxic, chondrocytes have a specific and adapted response to low oxygen environment. While huge progress has been performed on deciphering intracellular hypoxia signalling the last few years, nothing was known about the particular case of the chondrocyte biology in response to hypoxia. Recent findings in this growing field showed crucial influence of the hypoxia signalling on chondrocytes physiology and raised new potential targets to repair cartilage and maintain tissue integrity. This review will thus focus on describing hypoxia‐mediated chondrocyte function in the native articular cartilage.  相似文献

Degradation of the articular cartilage is a hallmark of osteoarthritis, a progressive and chronic musculoskeletal condition, affecting millions of people worldwide. The activation of several signalling cascades is altered during disease development: among them, the Wnt signalling plays a pivotal role in the maintenance of tissue homeostasis. Increasing evidence is showing that its activation needs to be maintained within a certain range to avoid the triggering of degenerative mechanisms. In this review, we summarise our current knowledge about how a balanced activation of the Wnt signalling is maintained in the articular cartilage, with a particular focus on receptor-mediated mechanisms.  相似文献

This review discusses modern methods used for the synthesis of articular cartilage ex vivo. The value of culturing articular chondrocytes as a monolayer and in three-dimensional lattices is discussed. Of particular interest are techniques involving seeding of chondrocytes onto synthetic, biodegradable, polymeric scaffolds, and natural materials, such as collagen and agarose. Also discussed is the use of bioreactors to modulate the fluid-flow-induced shear environment of cell-seeded scaffolds. Biodegradable scaffolds are central to the efforts to tissue engineer articular cartilage ex vivo. A review of salient efforts to design and use such scaffolds is presented, along with our thoughts on potential future improvements.  相似文献

目的使用修饰后的原子力显微镜(atomic force microscopy,AFM)探针研究关节软骨的微摩擦接触力学性能。方法使用微操作器对AFM氮化硅探针进行修饰处理,具体操作为在探针上粘贴玻璃微球作为针尖,然后使用修饰后的探针研究人体和牛关节软骨的微摩擦接触力学性能。结果人体和牛软骨的粗糙度分别为(68.63±6.22)、(50.16±6.47)nm,随着载荷的增大,人体和牛软骨的摩擦力逐渐增大。当探针滑动速度从0增加到100μm/s时,试样与探针之间的摩擦力增速很快;当速度从100μm/s增加到300μm/s时,摩擦力上升缓慢。结论软骨表面具有明显的纤维状结构,软骨的粗糙度与测量范围直接相关。随着速度或载荷的增大,人体和牛软骨的摩擦力增大,变化范围相同。探讨关节软骨在微摩擦试验中的力学和摩擦学性能表现,对于认识软骨损伤机制和医用人工关节抗磨材料的开发具有重要意义。  相似文献

目的:构建膝关节关节软骨的三维模型,为开展膝关节数字医学研究进行模型构建的探索。方法:人体成年新鲜膝关节标本1例,行CT、MRI扫描,利用三维重建软件Mimics及逆向工程软件Geomagic对图像进行三维重建及图像配准,构建膝关节骨、关节软骨结构。结果:利用Mimics 10.01软件构建关节软骨的三维模型,并形成骨-关节软骨复合模型,导入Geomagic软件中与CT构建的骨三维模型相配准,再次导入CT影像中进行逐层微修饰,验证关节软骨分割效果。最终建立具有骨、关节软骨的三维膝关节模型。结论:三维图像重建技术与图像配准技术可将CT、MRI图像两者结合起来,发挥其各自优势,构建出形态较好的膝关节三维模型。  相似文献

关节是人体重要的运动器官,其组织结构特殊,成熟关节软骨组织没有血供,一旦损伤难以自愈.临床上常用的关节软骨损伤修复手段有微骨裂术、关节软骨移植、关节置换术、软骨组织工程等,但这些方法的修复效果都不理想.生长因子是体内组织分泌的一种具有生物活性的物质,可促进细胞生长、增殖、迁移和分化.软骨发育过程中有许多生长因子参与,如成纤维细胞生长因子(FGF)、骨形态发生蛋白(BMP)、胰岛素样生长因子(IGF)等.研究显示,在关节软骨修复过程中加入外源性生长因子可有效促进关节软骨损伤的修复.对目前应用于关节软骨损伤修复研究中的生长因子进行综述,讨论这些生长因子在关节软骨发育及其在关节软骨修复中的作用,分析生长因子在关节软骨修复应用中存在的问题.  相似文献
