目的研究嘉兴地区幽门螺杆菌(Hdicobacter pylori,Hp)临床菌株cagA和vacA优势基因型状况及与疾病的关系。方法从胃、十二指肠疾病患者的胃粘膜活检组织中分离培养出44株HD。采用PCR检测cagA基因和vacA基因的信号区(s)和中间区(m)亚型的分布,用卡方检验分析各基因在不同消化性溃疡或慢性胃炎中的Hp差异。结果44株Hp中cagA的阳性率为100%(44/44),vacA sla/m2基因型阳性率为61.4%(27/44),vacA sla/mlb为25.0%(11/44),vaeA sla/mlb-m2为9.0%(4/44),其中有2株HD同时检出sla/m2和sh/mlb-m2基因,另有2株Hp未检测到m区基因。慢性胃炎患者Hp的vacA sla/m2和vacA sla/mlb基因阳性率赐显高于消化性溃疡患者的Hp(P〈0.05),其余基因在不同类型疾病中的分布无统计学差异(P〉0.05)。结论cagA+vacA sla/m2、cagA+vacA sla/mlb均为嘉兴地区消化性溃疡或慢性胃炎患者感染的Hp的优势基因型,部分患者可混合感染多株不同vacA基因型的HD。  相似文献

目的调查分析来自浙江人群的Hp临床菌株Vac A基因m1b变异体的序列特点及其空泡毒素表达活性.方法取Hp临床菌株及标准菌标培养于改良的布氏肉汤,在倒置显微镜下观察空泡变性的细胞数.结果 Vac A基因存在于所有Hp菌株中,但只有部分菌株有空泡毒活性,其表达差异与Vac A基因型的不同有关,本研究中m2和 m1b型Hp菌株占优势,而m1型少见.结论 m1b型比m2型菌株更适于在人群定植.  相似文献

目的 了解消化性溃疡和慢性胃炎患者感染的幽门螺杆菌 (Hp)cagA vacA优势基因型及不同基因型Hp感染、混合感染与疾病的关系。 方法 选择胃窦、胃体双份活检标本均培养出Hp的 42例慢性胃炎 (CG)和 3 6例消化性溃疡 (PU )患者作为研究对象 ,采用聚合酶链反应 (PCR)检测156份Hp分离株的cagA基因、vacA基因的信号区 (s)和中间区 (m )亚型 ,分析Hp基因型及多株Hp混合感染在CG和PU中的分布。结果 胃窦标本cagA基因的检测中 78例患者中有 75例 (96.2 % )为cagA阳性 ,相应的胃体标本中 ,76例患者 (97.4% )为cagA阳性 ,有 1例 (1.3 % )患者胃窦、胃体检出cagA状态不一的Hp混合菌株。在胃窦标本的vacA基因分型中 ,s1a m1、s1a m2、s1a m1b、s1a m1b m2 4种vacA基因型在 78例患者中所占比例分别为 6.4% (5 78)、55.1% (43 78)、2 6.9% (2 1 78)和 1.3 % (1 78) ,多株混合感染为 3 .8% (3 78) ;而相应的胃体标本中 ,前述四种vacA基因型所占比例依次为 6.4% (5 78)、53 .8% (42 78)、2 5.6% (2 0 78)和 3 .8% (3 78) ,多株混合感染为 5.1% (4 78)。cagA+ s1a m2和cagA+ s1a m1b在胃窦标本中占 51.3 % (40 78)和 2 6.9% (2 1 78) ,而在相应的胃体标本中占 52 .6% (41 78)和 2 5.6% (2 0 78)。少量胃窦、胃体  相似文献

硒对幽门螺杆菌空泡毒素及其重组影响   总被引:1,自引:1,他引:1  
目的 研究硒(Se)对幽门螺杆菌(Hp)空泡毒素(VacA)及重组VacA的影响.方法 采用盐析、透析、柱层析、浓缩、抽滤等方法提纯幽门螺杆菌分泌的空泡毒素.通过琼脂糖凝胶电泳、基因序列分析鉴定重组载体,重组VacA产物经Ni-TEDTM>蛋白纯化系统纯化,十二烷基硫酸钠-聚丙烯酰胺凝胶电泳(SDS-PAGE)、及免疫印迹(Western blot)鉴定,间接酶联免疫吸附(ELISA)法检测其抗原特异性.进行细胞培养、细胞爬片;采用分光光度计、倒置显微镜、透射电镜及流式细胞仪等方法检测硒对幽门螺杆菌分泌的VacA及重组毒素蛋白功能的影响.结果 12 mmol/L硒即可使幽门螺杆菌形态发生球形变.鞭毛及菌毛出现脱落,溶血作用减弱,硒对VacA空泡毒作用无直接影响(P>0.05);硒通过抑制Hp生长减少VacA产生量从而使空泡毒作用减弱;通过抑制表达VacA的大肠埃希菌生长,减少重组VacA表达量;能增强重组VacA对人胃上皮癌细胞株(SGC-7901)生长抑制作用,使细胞分裂指数、集落形成率、S期细胞数量降低,G1期细胞总数增加;使表达重组VacA的大肠埃希菌产生脂溶性砖红色色素,并随硒浓度增加颜色加深.结论 硒能抑制Hp生长,改变Hp的形态及结构;可使重组vacA表达量减少;能增强重组VacA对SGC-7901细胞的生长抑制及诱导细胞凋亡作用;可使表达重组VacA的大肠埃希菌产生脂溶性砖红色色素;不能直接抑制VacA的空泡毒作用.  相似文献

CYP1A1、GSTM1基因多态性与食管癌遗传易感性   总被引:7,自引:3,他引:7  
目的探讨细胞色素氧化酶P450(CYP)1A1、谷胱苷肽硫转移酶(GSTM1)的基因多态性与食管癌遗传易感性关系.以及基因-基因、基因-环境之间的交互作用。方法收集新发食管癌患者89例(病例组)及同期非食管疾患患者98例(对照组),调查与食管癌相关的危险因素;基因多态性检测采用聚合酶链式反应(PCR)和限制性片段长度多态性(RFLP)方法;比较病例组和对照组CYP1A1的MspI多态、Ile/Val多态和GSTM1基因多态性的分布情况,并结合环境因素进行单因素、多因素Logistic回归分析以及联合作用分析。结果MspI多态的突变型杂合子m1/m2(基因型B)和突变型纯合子m1/m2(基因型C)在病例组与对照组的分布差异有统计学意义,ORB值为1.93(95%CI=1.01—3.84),ORC值为3.62(95%CI=1.61~8.14),Ile/Val多态的突变型和GSTM1缺失型在两组的分布差异无统计学意义:MspI多态的突变型和GSTM1缺失型对食管癌发生的交互作用差异有统计学意义,OR值为3.57(95%CI=1.36~9.41);MspI多态的突变型与摄入较多新鲜蔬菜水果和蛋类呈拮抗作用,联合作用指数(S)分别为0.26和0.48.MspI多态的突变型与食管癌家族史呈协同作用,S为2.36。结论MspI多态的突变型与食管癌易感性有关,它与摄入较多新鲜蔬菜水果和蛋类及食管癌家族史对食管癌的发生存在交互作用;具有MspI突变基因型和或食管癌家族史的个体属食管癌高危险人群,在肿瘤防治方案中应加以注意。  相似文献

目的分析2000—2007年佛山市甲1亚型(H1N1)毒株流行情况及其HA1基因变异情况。方法用MDCK细胞分离流感病毒,采用血球凝集抑制试验进行型别鉴定,选取每年2~4株甲1亚型毒株的细胞培养物提取病毒核酸,进行HA1基因的逆转录,对其核苷酸和氨基酸序列及亲缘关系进行分析。结果病毒分离结果显示2000—2007年间甲1亚型流感病毒在佛山市人群中的流行是间断性的,只在2000、2001、2005、2006年4个年份分离到甲1亚型流感毒株。毒株的HAl区氨基酸序列与流感疫苗推荐株A/NewCMedonia/20/99和A/Solomon Islands/3/2006相比,同源性分别为97.2%~99.7%和97.2%-98.5%。氨基酸残基平均替换数随年份的推移而增加,只有2006年的毒株在抗原决定簇发生了替换。结论与疫苗株比较,佛山市的甲1亚型流感毒株抗原性已逐渐发生了改变,应密切注意疫苗对毒株的免疫效果。  相似文献

目的 获得散发性庚型肝炎病毒杭州株E2/NS1区部分核酸序列。方法 选择1例浙江杭州散发性庚型肝炎患,从其血清中分离HGVRNA,通过逆转录套式聚合酶链反应(RT-nPCR)法,扩增该散发性HGV(HZ-2株)E2/NS1区部分cDNA片段,然后克隆到质粒pGEX-4T-3中,并进行核苷酸序列分析。结果 HGV杭州株与河北株(HGVC964、美国株)(HGU44402)的核苷酸同源性为92.5%和89.8%,氨基酸同源性为98.0%和93.9%。结论 该项研究将为HGV诊断试剂盒的研制及基因工程疫苗的研制提供资料。  相似文献

目的 为了对2004年浙江省报告的输入性疑似登革热病例查明病因,从分子水平分析流行毒株的生物学特征,追踪传染源,为疾病的预防控制提供实验室依据。方法 对疑似患者血清标本采用ELISA、IFA和荧光RT—PCR方法分别检测登革病毒IgM、IgG抗体和病毒核酸,并用C6/36和BHK-21细胞分离登革病毒。采用RT—PCR方法扩增病毒E基因和NS1基因后分别进行序列测定,并与不同国家和地区的登革热毒株进行同源性和进化树分析。结果 从患者血清中检测到登革病毒IgM、IgG抗体和登革2型病毒核酸,并分离到登革2型病毒;浙江省登革2型病毒分离株(ZJ/01/04)与登革2型国际标准株Jamaica、国内Fujian/10/99株和Saudi/92株在E基因上氨基酸的同源性分别为97.1%、99.2%和99.6%,而与登革1、3、4型毒株相同区域氨基酸同源性分别为68.7%、66.9%和63.6%。基因进化树显示ZJ/OI/04分离株与Saudi/92株亲缘关系最近,在进化树的同一分支上。结论 从病原学、血清学和分子生物学特征上均证实该输入病例是由登革2型病毒引起,该毒株最有可能来源于沙特阿拉伯。  相似文献

目的探讨季节性流感监测的同时,针对人群中循环的优势毒株开展基因变异分析的意义。方法以2007年深圳市流感监测系统累积的数据为基础,按周分析不同监测医院流感样病例占门诊和急诊就诊者的比例,并对病毒型别进行常规分离鉴定,对优势毒株扩增HA1区基因片段,并进行基因变异分析。结果深圳市流感样病例2007年流感样病例(ILI)比例为6.67%,以低年龄为高发人群;按市、区医院和社区健康服务中心等分别分析,ILI的比例按周分布一致,呈现明显的夏季高峰,最活跃季节在5~7月;ILI暴发疫情比散发病例提早2周出现。流感病毒分离率为11.63%,优势株为H3N2亚型(占59.5%),B型次之(占37.0%)。H3N2亚型流感病毒HA1基因进化分析分成两支,WHO当年疫苗株和我国代表株仅与1~4月毒株接近,而与5月之后分离株产生了遗传距离,虽然还不能确定新的变异株出现,但提示A/sz/9/2007和A/sz122/2007两个分离株值得高度关注。B型流感病毒HAl基因进化分析发现,2007年以Yamagata系最活跃。结论季节性流感监测中开展病毒分子变异分析,可以进一步查明人群中所发生的优势毒株的特性,便于流行病学综合分析。  相似文献

目的了解江苏省宜兴地区丙型肝炎病毒(HCV)基因型分布特征和病毒变异情况。方法对宜兴市人民医院收治的158例HCV抗体阳性患者血清进行HCV RNA检测,对阳性标本进行Simmonds分型,采用5’非编码区(5’NCR)1、2、3、1b型特异性引物进行PCR扩增,并对1b型和2型各1株的PCR产物测序验证。分析不同性别、临床类型的丙型肝炎患者基因型分布差异。结果158份血清中,有95份为HCV RNA阳性,其中1b型80份(84.2%),2型5份(5.3%),1b/2型的混合感染5份(5.3%),不能分型的5份(5.3%),1b型的序列与GenBank J-4株的同源性为99.2%,2型与GenBank J-6株的同源性为97.7%。男性和女性在基因型总体分布上存在差异(P〈0.05),并且在单一型感染和混合型感染的分布上差异亦有统计学意义,不同临床类型的丙型肝炎中HCV基因型分布未显示差异。结论江苏省宜兴地区HCV以1b型为主,男女患者HCV基因型分布差异有统计学意义,而不同临床类型的HCV基因型分布无显著差异,基本反映了本地区HCV感染的特点。  相似文献

The Helicobacter pylori vacuolating cytotoxin is a putative pathogenicity factor encoded by vacA, a mosaic gene with a global distribution. The vacA type prevalence and diversity of H. pylori isolated from antral gastric biopsies of 360 dyspeptic patients in mid-Essex, and of 79 patients from other locations, were investigated in order to test for links with disease severity. Mid (m)-region genotyping and subtyping by vacA HaeIII RFLP (restriction fragment length polymorphism) analysis showed that the m1 and m2 alleles were diverse, with 191 different subtypes. Variation in 44% of strains was accounted for by ten subtypes of which subtype v-1 represented a conserved core (33%) of the m1 form. Prevalence rates for combined mid and signal (s)-region genotypes were 40% for s1/m1, 46% for s1/m2, and 11% for s2/m2. Overall, vacA genotyping provided high typability and discrimination, but no specific RFLP markers could reliably predict a clinically significant presentation due to an H. pylori infection.  相似文献

目的 分析幽门螺杆菌(HP)中国菌株vacA基因多态性.方法 对分离自中国7个不同地区、不同胃十二指肠相关疾病患者的119株HP,采用特异引物聚合酶链反应(PER)方法,对其vacA基因进行PCR扩增.根据核酸电泳中产物片段大小确定vacA等位基因类型并统计分析各型分布.对vacA基因核心片段进行PCR扩增和DNA测序,利用软件MEGA4.0对DNA测序结果进行聚类分析.结果 119株HP的vacA基因以sla、m2和il型为主,分别为97.5%(116/119)、68.9%(82/119)和91.6%(109/119).26.1%(31/119)为mlb;slb,mla未检出.vacA组合基因型以sla/m2/il为主(62.2%,74/119),sla/mlb/il次之(25.2%,30/119).不同地区、不同疾病来源菌株sla分布的差异无统计学意义(P>005).而m区基因多态性在疾病类型及分离地区间差异有统计学意义(P<0.01).不同疾病来源菌株间i区基因分型分布的差异无统计学意义,但不同分离地区菌株间差异有统计学意义(P<0.01).119株的vacA基因序列聚类为三个不同组群.结论 HP中国分离株vacA基因型以sla/m2/il组合型为主,sla分型结果与菌株分离地区及疾病类型无关.不同地区、不同疾病来源菌株m区基因分型不同.不同地区菌株vacA基因i区分型不同,但i区基因分型与菌株的疾病来源无显著相关性.
Abstract:
Objective To understand the polymorphism of Helicobacter pylori (H. pylori) vacA alleles in China. Methods A total of 119 H. pylori strains were isolated from different gastroduodenal diseases in 7 different geographic regions in China. vacA and its alleles were identified according to the length of PCR products with DNA electrophoresis. The distributions of vacA alleles were statistically analyzed. The core fragment of vacA was sequentially analyzed by software MEGA4.0. Results The alleles in vacA dominantly belonged to sla, m2 and il in the tested strains.The distribution appeared to be 97.5%(116/119) ,68.9%(82/119) and 91.6%(109/119),respectively.The mlb allele appeared to be 26.1% (31/119). slb and mla were not found. The major vacA recombination was between slaim2/il and 62.2% , followed by sla/mlb/il (25.2% , 30/119). No association was found between the distribution of sla allele and the clinical outcome, as well as the geographical regions (P>0.05). However, the distribution of m alleles showed significant difference both among the types of disease and the geographic regions (P<0.01), The present of i alleles did not show significant differences among disease patterns, but had significant differences between different geographic groups (P<0.01). Three clusters were identified among these 119 isolates according to the DNA sequence of vacA. Conclusion sla/m2/il appeared to be the main allele in H. pylori vacA isolates from China in this study. The distribution of m alleles in vacA was correlated both to the regions and the disease patterns. The presence of i allele was associated to the regions but not the disease patterns.  相似文献

目的研究淮南地区学生幽门螺杆菌(Helicobacter pylori,H.pylori)cagA和vacA基因分布特征,为防治工作提供理论依据。方法对74例有消化道症状,年龄在7~24岁的在校学生进行胃镜检查,并在胃窦部取活检粘膜作H.pylori的分离培养,利用聚合酶链反应技术(PCR)测定分离培养出的H.pylori菌株的cagA和vacA基因并进行分型。结果74例学生中,分离培养出H.pylori菌株24例,基因测定结果显示,24株H.pylori临床分离株中,94.7%(23/24)含vacA基因,70.8%(17/24)含cagA基因;其中慢性胃炎vacA和cagA基因检出率分别为94.7%(18/19),70.6%(12/17);2例胃溃疡及3例十二指肠球部溃疡均全部为vacA和cagA阳性;进一步分型发现89.5%(17/19)的慢性胃炎为vacA 和cagA ,而5例溃疡患者均为vacA 和cagA 。结论淮南地区学生H.pylori感染多为vacA 和cagA 菌株,应充分重视H.pylori毒力因子的监测。  相似文献

A clinical trial was conducted in Argentina to determine the efficacy of clarithromycin plus lansoprazole for the treatment of Helicobacter pylori in duodenal ulcers and non-ulcer dyspepsia. PCR-RFLP was conducted on an 820-bp amplified product of the ureC gene of H. pylori to determine the genetic heterogeneity of 83 pretreatment and 21 post-treatment isolates. Twelve different restriction patterns were observed when digested with Sau 3A or Hha I, resulting in 40 different RFLP types. Comparison of isolates before treatment to after treatment showed that 20 of 20 patients had the same RFLP type. In addition, the presence of the cytotoxin-associated gene (cagA) and the vacuolating gene (vacA) were determined. All pretreatment isolates were positive for vacA whereas 75% of the pretreatment isolates were positive for cagA. The results of this study indicate that a high degree of heterogeneity exists among H. pylori and that infection is not limited to a small number of RFLP types.  相似文献

目的 研究c9,t11-共轭亚油酸(CLA)对人胃腺癌细胞(SGC-7901)侵袭能力的影响,探讨其抑制肿瘤转移的可能机制。方法 用重组基底膜侵袭实验评价癌细胞侵袭能力;用逆转录聚合酶链反应(RT-PCR)检测SGC-7901细胞中组织基质金属蛋白酶抑制剂(TIMP)-1、TIMP-2和nm23-H1mRNA的表达。结果 在200、100和50μmol/L浓度时,c9,t11-CLA对SGC-7901细胞侵袭重组基底膜的抑制率分别为53.7%、40.9%和29.3%。c9,t11-CLA可诱导SGC-7901细胞中TIMP-1、TIMP-2和nm23-H1mRNA的表达。结论 c9,t11-CLA抑制SGC-7901细胞侵袭重组基底膜。c9,t11-CLA的抗侵袭活性与诱导肿瘤细胞中TIMP-1、TIMP-2和nm23-H1mRNA的表达等有关。  相似文献

In developing countries, clarithromycin resistance and frequency of re-infection are factors that contribute to high prevalence of Helicobacter pylori infection. The aim of this research was determine the prevalence of clarithromycin resistance and its relation with A2142G, A2142C and A2143G mutations in the domain V of the 23S rRNA gene of H. pylori isolates in patients from Southern Mexico with chronic gastritis. Another purpose of this work was to study the prevalence of virulent genotypes and distribution of resistant strains according to the vacA/cagA/babA2 H. pylori genotypes. One hundred forty-four patients with chronic gastritis were studied. Forty-five H. pylori strains were isolated and clarithromycin susceptibility was determined by the disk-diffusion method. The 82.2% of the strains had the combination of alleles vacA s1 m1 and the cagA gene was detected in 77.8% and 40% of the strains were babA2 positive. The vacA s1 m1 genotype was detected more frequently in cagA+ strains, vacA s1m1/cagA+/babA2- genotype was more frequent than vacA s1m1/cagA+/babA2+, 37.8% and 33.3%, respectively. Eight strains were clarithromycin resistant, in three of these, point mutations were identified, but only in one strain the A2143G mutation associated with clarithromycin resistance was found. Other point mutations (A1821G, G1826A, T1830C, A2089G, T1600C, C1601T, C1602T, T1610C, A1611C and T1633G) that have not been associated with clarithromycin resistance were identified. The highest proportion of resistant strains was vacA s1m1/cagA+ (62.5%). In patients from southern Mexico with chronic gastritis, the prevalence of clarithromycin resistance is within internationally accepted range (17.8%) and allows continued use of triple therapy for H. pylori eradication. However, it is necessary to monitor the evolution of clarithromycin resistance in this area. The largest proportion of resistant H. pylori strains is not harboring the A2142G, A2142C and A2143G mutations in the 23S rRNA gene (87.5%). The vacA s1m1/cagA+ genotype was the most prevalent and among clarithromycin-resistant strains, this was the predominant.  相似文献

目的:研究幽门螺杆菌(Hp)的重要毒力因子vacA,cagA,iceA及插入序列IS在中国香港地区分离菌株中的分布特征及其与十二指肠溃疡的关系。方法:采用聚合酶链反应(PCR)和Southern blot方法,对72例证实为Hp感染的胃十二指肠疾病患者的胃黏膜标本直接进行检测。结果:72例患者中,69例(95.8%)感染的Hp菌株为vacA sIc型,3例(4.2%)为sIa型;23例(31.9%)为vacAm1b型,46例(63.9%)为vacAm2型;6例(8.3%)为混合型。63.9%(46/72)的患者感染菌株为iceA1型,29.2%(21/72)为iceA2型。CagA的阳性率为88.9%(64/72)。结论:Hp毒力因子vacA,cagA和iceA在香港菌株中的分布有自己的特点;未发现特定cagA,vacA和iceA基因型别与DU相关。  相似文献

目的通过对胃十二指肠疾病患者幽门螺杆菌(H.pylori)多种抗体的蛋白芯片检测,评价H.pylori的尿素酶(Ure)、细胞毒素相关蛋白(CagA)、空泡毒素(VacA)、热休克蛋白60(Hsp60)与慢性胃炎、消化性溃疡、胃癌的相关性,并了解H.pylori的硝基还原酶(RdxA)表达与甲硝唑耐药的相关性。方法经胃镜检查确诊的300例慢性胃十二指肠疾病患者,抽静脉血3ml离心取血清,用蛋白芯片技术检测H.pylori的Ure、CagA、VacA、Hsp60、RdxA五种抗体。结果慢性胃炎、消化性溃疡、胃癌中H.pylori感染率为34.0%、58.0%、34.O%(P〈0.01);在H.pylori感染的慢性胃炎、消化性溃疡、胃癌中CagA抗体阳性率分别为54.9%、75.9%、64.7%(P=0.070);VacA抗体在三种疾病中阳性率分别为31.4%、22.4%、17.6%(P=0.412);HspS0抗体阳性率分别为56.9%、48.3%、41.2%(P:0.466);RdxA抗体总阳性率为4.O%(5/126)。结论消化性溃疡与H.pylori感染及毒力因子CagA的作用密切相关(r=0.221,P=0.021);而VacA、Hsp60与三种胃十二指肠疾病间未显示明确相关性(r=0.118、-0.110,P=0.187、0.220);RdxA抗体水平不能完全代表甲硝唑耐药水平。  相似文献
