The effectiveness of a rational emotive mental health program on children's emotional adjustment was examined. Fourth and eighth grade students (n = 204) were given 15 weekly classroom sessions of rational emotive education. Comparisons were made with alternate treatment and no treatment controls at each grade level. In general, the results supported the hypotheses that rational emotive principles can be acquired by school children, and that such acquisition corresponds to lowered trait anxiety and neuroticism scores. Contrary to prediction, younger students benefited more than older students. Implications and suggestions for further research are discussed.  相似文献

This article describes the ways in which I have tried, primarily as a policy researcher, to bring an advocacy perspective to my own work and, in collaboration with others, engage in efforts to improve the lives of children experiencing emotional and behavioral challenges as well as the lives of their families. It is organized in 3 sections. The 1st section highlights my early career path and its relation to advocacy. The 2nd section summarizes the policy research I have conducted that has given me a broad understanding of children's mental health. The 3rd highlights lessons for advocates and my view of critical recommendations for the future.  相似文献

Over the past three years the Worcester Youth Guidance Center has developed a series of mental health programs directed to the Hispanic community of Worcester, Massachusetts. The model for these programs has been that of strategic intervention. This approach was based upon knowledge gained from the literature and extensive exploratory contacts with key Hispanic leaders. The Center's Hispanic Program consists of five components: (1) community education and training. (2) clinical consultation, (3) program consultation, (4) research, and (5) clinical intervention and case management. This article describes these components in detail and presents a rationale for strategic intervention.  相似文献

This study evaluated variation at the individual and work unit levels in the relations of job control, hostility, and trait anxiety to mental health and job satisfaction. Questionnaire data from a sample of 2,900 employees working at 152 hospital wards were analyzed by means of multilevel regression analyses. Results showed that mental health (General Health Questionnaire-12), varying mainly at the individual level, was explained mostly by hostility and trait anxiety. Job satisfaction varied significantly at the individual and the ward level. Job control accounted for most of this multilevel variation. Thus, this study demonstrated the significance of individual characteristics and organizational effects in explaining the mental health and job satisfaction of employees.  相似文献

The current third-party fee-for-service mode of reimbursement for mental health services emphasizes individual, pathology-oriented intervention and thus has disadvantages for clients and carriers. This article focuses on the mode's effects on outpatient services for children, youth, and families. It examines the ways in which common payment modes seriously limit services and create an incentive for psychologists to take a narrow, often nonempirical, perspective. Suggestions that allow for multisystem, ecological skill building hinge on building relationships between psychologists and the carriers. Desired results take two forms: modification of contingencies between providers and carriers and modification of contingency between insured and carrier. Examples of each are presented.  相似文献

Objective

The paper presents an application of the “Fit between Individuals, Task and Technology” (FITT) framework to analyze the socio-organizational-technical factors that influence IT adoption in the healthcare domain.

Method

The FITT framework was employed as the theoretical instrument for a retrospective analysis of a 15-year effort in implementing IT systems and eHealth services in the context of a Regional Health Information Network in Crete. Quantitative and qualitative research methods, interviews and participant observations were employed to gather data from a case study that involved the entire region of Crete.

Results

The detailed analysis of the case study based on the FITT framework, showed common features, but also differences of IT adoption within the various health organizations. The emerging picture is a complex nexus of factors contributing to IT adoption, and multi-level interventional strategies to promote IT use.

Conclusion

The work presented in this paper shows the applicability of the FITT framework in explaining the complexity of aspects observed in the implementation of healthcare information systems. The reported experiences reveal that fit management can be viewed as a system with a feedback loop that is never really stable, but ever changing based on external factors or deliberate interventions. Management of fit, therefore, becomes a constant and complex task for the whole life cycle of IT systems.  相似文献

The efficacy of a community-based psychosocial program in Bosnia-Herzegovina during the war and immediate postwar years (1994-1999) was described in this article. Ten centers provided various kinds of psychological help in the besieged city of Sarajevo and the towns of Zenica, Travnik, and Vitez. Since 1994, an intensive monitoring system has documented data on clients, interventions, and outcomes. This study focused on the systematic evaluation of counseling interventions aimed to alleviate the distress in wartime. The sample consisted of 3,283 and 1,785 inhabitants of Sarajevo, Zenica, Travnik, and Vitez who filled out the GHQ-28 and IES respectively. Pre- and post-assessments were compared throughout consecutive years (1994-1999) and across age groups and both sexes. Outcomes of these scales reflected very high scores, especially among people between 30 and 40 years of age. Furthermore, intake scores increased in time rather than decreased. Differences between pre- and postmeasurements are highly significant--throughout the years. Analyses revealed substantial proportions of clinically recovered or generally improved individual functioning, although some clients revealed no improvement.  相似文献

Psychoeducational programs for families of the mentally ill became widespread during the 1980s as a means of providing a forum for the relevant education and mutual support of participants. While these programs are thought to be extremely useful as interventions, very little emphasis has been placed on evaluation as a means of demonstrating their effectiveness in achieving goals. There is a possibility, then, that psychoeducation will continue to flourish with little direct evidence of positive outcomes for its family participants. This article consists of a literature review of existing methods of psychoeducational program evaluation, both quantitative and qualitative, all of which may be applicable in certain circumstances. The process by which an evaluation instrument was developed for a program with families of the mentally ill is then presented in some detail.  相似文献

PurposeImplementations of health information technologies are notoriously difficult, which is due to a range of inter-related technical, social and organizational factors that need to be considered. In the light of an apparent lack of empirically based integrated accounts surrounding these issues, this interpretative review aims to provide an overview and extract potentially generalizable findings across settings.MethodsWe conducted a systematic search and critique of the empirical literature published between 1997 and 2010. In doing so, we searched a range of medical databases to identify review papers that related to the implementation and adoption of eHealth applications in organizational settings. We qualitatively synthesized this literature extracting data relating to technologies, contexts, stakeholders, and their inter-relationships.ResultsFrom a total body of 121 systematic reviews, we identified 13 systematic reviews encompassing organizational issues surrounding health information technology implementations. By and large, the evidence indicates that there are a range of technical, social and organizational considerations that need to be deliberated when attempting to ensure that technological innovations are useful for both individuals and organizational processes. However, these dimensions are inter-related, requiring a careful balancing act of strategic implementation decisions in order to ensure that unintended consequences resulting from technology introduction do not pose a threat to patients.ConclusionsOrganizational issues surrounding technology implementations in healthcare settings are crucially important, but have as yet not received adequate research attention. This may in part be due to the subjective nature of factors, but also due to a lack of coordinated efforts toward more theoretically-informed work. Our findings may be used as the basis for the development of best practice guidelines in this area.  相似文献

The present study was a comprehensive, quantitative synthesis of the literature examining the effects of physical activity on children's mental health outcomes. The final analysis included 73 published and unpublished studies, totaling 246 effect sizes. Various study and participant characteristics were coded to assess moderator effects, including type of physical activity, mental health outcome, gender, cognitive ability, mental status, and implementer of the physical activity, etc. Results demonstrated varying effects depending on the methodology of the examined study [i.e., correlational vs. randomized controlled trial (RCT)/non-RCT] and characteristics of the participants, although overall effects of physical activity on children's mental health were small but significant, indicating that on average physical activity led to improved mental health outcomes for all children.  相似文献

Systems of care (SOCs) have been developed throughout the country to meet the needs of children with severe emotional disturbances (SED) and their families. In these SOCs, multiple agencies and disciplines are expected to work together with informal community supports to address families' needs (Stroul & Friedman, 1986a). A review of the literature on the impact of SOCs suggests: (a) communities' service delivery systems change; and (b) children experience modest improvements in symptomatology and functioning. At the same time, little is known about (a) which components of the SOC approach, at what levels, are necessary to impact child and family outcomes; (b) the degree to which SOCs affect other family members, beyond the target child; and (c) the impact of community contexts and supports in SOCs. Future research should improve measurement of key SOC constructs, examine the relation between specific levels of implementation and outcomes for the entire family, and investigate the impact of broader community systems and supports on families within SOCs. © 2004 Wiley Periodicals, Inc. Disaster victims typically do not request services from the mental health system. Victims must be contacted through innovative outreach programs linked to the disaster relief network. Advance planning for disaster programs by mental health centers is highly desirable, but barriers may be encountered related to (a) external support, (b) information on victim needs, (c) intervention methods, (d) linkage with disaster agencies, or (d) planning guidelines. Advice and information are offered on how to overcome each potential barrier. A model disaster plan is outlined that is consistent with NIMH guidelines for disaster preparedness planning, and with three criteria considered essential for an effective plan.  相似文献
