PURPOSE: To evaluate deviations in the axis (intended versus achieved) and postoperative astigmatism after implantation of an Artisan toric phakic intraocular lens (IOL). SETTING: University Eye Hospital, Mainz, Germany. METHODS: This prospective study comprised 29 eyes with high ametropia and astigmatism. All eyes had uneventful implantation of a toric phakic IOL through a superior scleral tunnel incision at 12 o'clock. After a minimum of 6 months, the uncorrected visual acuity (UCVA), best correct visual acuity, refraction, and astigmatism were analyzed in all eyes. A multivariate analysis of postoperative astigmatism was performed. RESULTS: After a follow-up of at least 6 months, 95% of eyes were within +/-1.00 diopter (D) of emmetropia and 85% of eyes has a UCVA of 20/30 or better. The difference between the mean intended cylinder axis and achieved cylinder axis was 3.9 degrees (median 3 degrees; range to 13 degrees). The difference between the mean intended axis and the achieved axis between miosis and mydriasis was 1.8 degrees (median 1.5 degrees; range 0 to 5 degrees). The mean postoperative astigmatism after 6 months was 0.56 D with an axis of 31 degrees. Doubled-angle scatterplot analysis showed a tendency toward more flattening in the vertical meridian. CONCLUSIONS: During the 6-month follow-up, no significant rotation was observed after implantation of Artisan toric phakic IOLs to correct high ametropia. A sutureless sclerocorneal superior approach for phakic IOL insertion resulted in moderate to low astigmatism. Induced astigmatism should be taken into consideration during preoperative planning. 相似文献

Effect of incision types for Artisan phakic intraocular lens implantation on ocular higher order aberrations 下载免费PDF全文
AIM: To evaluate the effect of incision types for Artisan phakic intraocular lens (PIOL) implantation on ocular higher-order aberrations (HOAs).
METHODS: A retrospective review was conducted of the patients who had undergone Artisan PIOL implantation for the correction of myopia and followed up for at least 6mo. Patients are classified into 2 groups considering the incision type: cornea group with patients undergone clear corneal incision; sclera group with patients undergone sclera tunnel incision. All patients with postoperative astigmatism of under 1 diopter (D) were included to minimize the effect of residual astigmatism on postoperative HOAs. Visual acuity, special equivalents, astigmatism, predictability (±1 D from target refraction), HOAs (coma, trefoil, spherical aberration), and corneal endothelial counts were analyzed preoperatively and 6mo postoperatively.
RESULTS: At the postoperative 6mo, all patients of both groups achieved uncorrected visual acuity of 16/20 or better, and significantly decreased the spherical equivalents compared with preoperative values. The predictability of refractive correction was 96% in the former, and 94% in the latter. Unlike the sclera group, preoperative astigmatism decreased significantly in cornea group at postoperative 6mo. The HOAs increased significantly at postoperative 6mo compared to the preoperative values in both groups, and the root mean square (RMS) total and trefoil wavefront aberration of cornea group were significantly higher than those of sclera group.
CONCLUSION: Although corneal incision significantly reduces preexisting astigmatism, the postoperative 6mo of total RMS and trefoil aberration change may deteriorate the visual quality after Artisan PIOL implantation. 相似文献

目的观察白内障超声乳化联合人工晶状体(IOL)植入术后眼高阶像差与对比敏感度(CS)、眩光对比敏感度(GS)之间的关系,探讨改善白内障术后视觉质量的方法。方法采用Zywave波前像差仪检测白内障超声乳化联合IOL植入术后129例129眼的总高阶像差、球差、彗差等各高阶像差均方根值(RMS),采用CSV-1000E对比敏感度测试仪检测其CS和GS,对白内障超声乳化联合IOL植入术后高阶像差与CS或GS间的相关性进行Spearman秩相关分析。结果白内障超声乳化联合IOL植入术后,眼高阶像差与无眩光CS之间相关性较小,总高阶像差与12 c/d CS之间(r=0.23,P〈0.05)、Z400与3 c/d CS之间(r=0.17,P〈0.05)均呈正相关;Z440与9 c/d CS之间(r=-0.21,P〈0.05)、Z550与12 c/d CS之间(r=-0.18,P〈0.05)均呈负相关。眼高阶像差与GS之间多呈负相关,包括Z331与6 c/d之间(r=-0.18,P〈0.05),Z420与9 c/d之间(r=-0.25,P〈0.05),Z420与12 c/d之间(r=-0.21,P〈0.05),Z440与9 c/d之间(r=-0.21,P〈0.05),Z440与12 c/d之间(r=-0.23,P〈0.05),Z510与9 c/d之间(r=-0.18,P〈0.05),Z530与12 c/d之间(r=-0.23,P〈0.05)均呈负相关,但Z441与9 c/d之间呈正相关(r=0.19,P〈0.05),与GS相关的高阶像差主要集中在4阶,与高阶像差相关的GS主要集中在中高空间频率段。结论白内障超声乳化联合IOL植入术后眼高阶像差的变化不影响无眩光CS,但能影响GS。 相似文献

目的 研究虹膜夹型有晶状体眼人工晶状体(iris-claw phakic intraocular lens,ICPIOL)植人术后对比敏感度(contrastsensitivity,CS)的变化。方法 2005年5月至12月于温州医学院附属眼视光医院和中山大学中山眼科中心接受ICPIOL(Verisyse,美国AMO公司)植入术的高度近视眼患者12例(23只眼),分别在术前、术后1、3、6个月采用CS测试灯箱(CSV-1000型,美国Vector Vision公司)检查暗视状态下无/有眩光时的对比敏感度函数(contrast sensitivity function,CSF)。结果 术后1、3、6个月等效球镜度数分别为(-0.60±0.77)D,(-0.48±0.77)D,(-0.34±0.53)D,与术前(-15.53D±3.65D)差异均有显著统计学意义(P均〈0.001),最佳矫正视力(IogMAR)分别为0.064-0.12,0.02±0.09,-0.003±0.08,与术前(0.21±0.12)差异均有显著统计学意义(P均〈0.001)。术后各空间频率的CS与术前比较有增高趋势。术后3个月6、12、18周/度的CS在无眩光时和有眩光时均明显提高(P均〈0.01),开始接近正常范围。术后3、6个月在各空间频率上CS差异均无统计学意义(P均〉0.05)。结论 ICPIOL植入术矫治高度近视眼可以使CSF恢复到接近正常范围,显著改善视功能,是一种适用于高度乃至超高度近视眼的可供选择的屈光手术。 相似文献

PURPOSE: To compare pre- and postoperative horizontal and vertical pupil diameters after horizontal Artisan phakic intraocular lens (PIOL) (Ophtec BV, Groningen, The Netherlands) implantation for correction of myopia. METHODS: The pre- and postoperative scotopic pupil diameters measured by the Colvard pupillometer (Oasis Medical, Glendora, Calif) were compared in 71 eyes after Artisan PIOL implantation (Artisan Colvard group). Analysis of pupil shape (vertical vs horizontal diameter, V/H ratio) after horizontal Artisan PIOL implantation was performed with the Procyon pupillometer (Procyon Instruments Ltd, London, United Kingdom) in 121 eyes (Artisan Procyon group) under scotopic, mesopic-low, and mesopic-high conditions and compared to an age- and refraction-matched control group of 121 eyes of refractive surgery candidates (Procyon control group). RESULTS: After horizontal Artisan PIOL implantation, the mean horizontal pupil diameter decreased from 6.231 +/- 0.70 mm preoperatively to 5.34 +/- 0.68 mm postoperatively in the Artisan Colvard group (P < .01). The mean horizontal scotopic diameter was 5.60 +/- 0.66 mm, the mesopic-low diameter was 4.94 +/- 0.71 mm, and the mesopic-high diameter was 3.98 +/- 0.54 mm for the Artisan Procyon group. The mean horizontal scotopic diameter was 6.29 +/- 0.91 mm, the mesopic-low diameter was 5.40 +/- 0.96 mm, and the mesopic-high diameter was 4.16 +/- 0.80 mm in the Procyon control group. A significant increase was noted in V/H ratio under scotopic, mesopic-low, and mesopic-high conditions from 1.02, 1.02, and 1.01, respectively, in the Procyon control group to 1.17, 1.12, and 1.06, respectively, in the Artisan Procyon group (P < .01). CONCLUSIONS: Horizontal pupil size was significantly decreased after horizontal Artisan PIOL implantation and might be attributed to a mechanical restriction of the iris in the horizontal meridian. 相似文献

目的 探讨白内障术后早期对比敏感度(CSF)的变化.方法 试验组为超声乳化白内障摘除联合折叠型人工晶状体(introcular lens,IOL)植入手术的老年性白内障术后患者105例(150眼),术后1个月用FACT对比敏感度测试仪进行CSF测定.与试验组年龄匹配的门诊患者84例(150眼)为正常对照组.双眼矫正视力均≥0.8.结果 白内障术后患者CSF大致恢复到正常范围,但其值较正常对照组降低.三个年龄组昼及昼眩光状态低、中频(1.5、3、6cpd)白内障术后患者与正常对照组比较差异无显著性;高频(12、18cpd)比较差异有显著性(P<0.05),但70~79岁组在12cpd时无显著性差异(t=1.11、0.61,P>0.05).三个年龄组夜及夜眩光状态低频(1.5、3cpd)白内障术后患者与正常对照组比较差异有显著性(P<0.05),但70~79岁组在3cpd时无显著性差异(t=0.80、0.38,P>0.05);6cpd比较各组差异无显著性.且不论白内障术后组还是正常对照组,CSF值随年龄增长均呈逐渐下降趋势.结论 白内障摘除IOL植入可恢复患者的CSF,但尚未达到同龄人正常水平.IOL的设计原理对CSF的影响尚需进一步探讨. 相似文献

Changes in higher-order aberrations after implantation of a foldable iris-claw lens in myopic phakic eyes 总被引:2,自引:0,他引:2
PURPOSE: To evaluate the change in higher-order aberrations (HOAs) after implantation of a foldable iris-claw phakic intraocular lens (pIOL) in myopic eyes. SETTING: Department of Ophthalmology, Johannes Gutenberg-University, Mainz, Germany. METHODS: This prospective nonrandomized comparative self-controlled trial included 41 eyes that had implantation of a foldable iris-claw pIOL between July 2003 and November 2004. All patient data for HOAs (Zernike coefficient) were measured and calculated using the same pupil size preoperatively and postoperatively, and the root-mean-square (RMS) wavefront error was calculated. Examinations were performed preoperatively as well as 1 week and 3, 6, and 12 months after pIOL implantation. RESULTS: The mean preoperative sphere was -8.12 diopters (D) +/- 2.01 (SD) (range -12.25 to -3.75 D) and the mean preoperative cylinder, -0.90 +/- 0.62 D (range -2.50 to 0.00 D). The Z3(1) and Z(3)(-1) coefficients decreased after implantation of the foldable iris-claw pIOL, but the decrease was not statistically significant (P = .078 and P = .45, respectively). There was a statistically significant reduction in Z4(0) in the first postoperative week (P = .038), but the decrease was not significant thereafter. There was a statistically significant reduction in total RMS wavefront error from preoperatively to all values postoperatively (P = .008). CONCLUSION: Over a 12-month follow-up, 3rd-order and 4th-order HOAs did not increase after foldable iris-claw pIOL implantation in myopic eyes. 相似文献

目的 分析白内障超声乳化人工晶状体植入术后对比敏感度与眩光对比敏感度变化.方法 选取我院2009年1月至2010年6月行超声乳化人工晶状体植入术的白内障患者45例(60眼),术后随访1~6个月,平均3.5个月.观察患者术前及术后随访末期BCVA,并于术前及术后1个月检测患者对比敏感度和眩光对比敏感度.结果 患者术后BCVA较术前显著提高,差异有统计学意义(P<0.05).明视无眩光及明视眩光条件下,患者术后各空间频率下的对比敏感度及眩光对比敏感度较术前均明显提高,差异均有显著统计学意义(均为P<0.01).暗视无眩光及暗视眩光条件下,除高空间频率(18.0c·d-1)外,患者术后其他空间频率下的对比敏感度及眩光对比敏感度较术前均明显提高,差异均有显著统计学意义(均为P <0.01).明视及暗视条件下,眩光对于患者术前、术后高空间频率(12.0 c·d-1、18.0c·d-1)下的对比敏感度均无明显影响(P>0.05),但对术前、术后其他空间频率下的对比敏感度均有明显影响(P<0.01).与暗视眩光条件相比,明视眩光条件下的患者术前、术后各空间频率对眩光比敏感度均较高,差异均有统计学意义(均为P <0.05).结论 对比敏感度及眩光对比敏感度检查可以作为白内障患者更客观有效的视功能检查方法. 相似文献

PURPOSE: To evaluate higher order aberrations and contrast sensitivity after LASIK, implantation of the Verisyse phakic intraocular lens (IOL), and refractive lens exchange with the Array multifocal IOL. METHODS: In a prospective, non-randomized case series, LASIK was performed in 20 eyes with the Technolas 217z excimer laser (Bausch & Lomb, Rochester, NY), a Verisyse phakic IOL (AMO Inc, Santa Ana, Calif) was implanted in 11 eyes, and refractive lens exchange with implantation of a multifocal IOL (Array IOL, AMO Inc) was performed in 12 eyes. Wavefront error (Zywave aberrometer; Bausch & Lomb) at two pupil sizes (4 and 6 mm) and photopic contrast sensitivity (CVS-1000) was measured preoperatively and 2 months postoperatively in all eyes. RESULTS: Photopic contrast sensitivity remained unchanged in the LASIK and the Verisyse groups, and decrease was significant in the Array group at three cycles per degree only. Higher order aberrations with a 4-mm pupil were increased in the Array group only. With a 6-mm pupil, they were increased in all groups. Comparing groups, surgically induced higher order aberrations were highest after refractive lens exchange with the Array multifocal IOL and lowest after implantation of the Verisyse IOL. CONCLUSIONS: Laser in situ keratomileusis, the Verisyse IOL, and the Array IOL increase higher order aberrations at large pupil sizes, but no increase occurs at small pupil sizes with LASIK or the Verisyse IOL. Contrast sensitivity in photopic conditions is normal with LASIK and the Verisyse IOL, but slightly reduced with the Array IOL due to the multifocal optic. 相似文献

PURPOSE: To determine whether implantation of an intraocular lens (IOL) with a modified posterior aspherical surface (AcrySof IQ, Alcon Surgical Laboratories) results in reduced ocular aberrations (spherical aberration) and improved contrast sensitivity after cataract surgery. SETTING: Brasilia Ophthalmologic Hospital, Brasilia, and Sao Geraldo Eye Hospital, Federal University of Minas Gerais, Belo Horizonte, Brazil. METHODS: In an intraindividual randomized prospective study of 25 patients with bilateral cataract, an IOL with a modified posterior surface (AcrySof IQ) was compared with a biconvex IOL with spherical surfaces (AcrySof Natural, Alcon). Ocular aberrations with a 5.0 mm and 6.0 mm pupil were measured with a Hartmann-Shack aberrometer. Quality of vision was measured by visual acuity and contrast sensitivity under mesopic and photopic conditions. All patients were followed for 3 months. RESULTS: There were no statistically significant differences between eyes in postoperative uncorrected and best corrected distance visual acuities after a follow-up of 1 month and 3 months. Postoperatively, there was a statistically significant between-group difference in contrast sensitivity under photopic conditions without glare at 18 cycles per degree (cpd) (P = .04) and under mesopic conditions without glare at all spatial frequencies (3 cpd, 6 cpd, 12 cpd, and 18 cpd; P = .03, P = .009, P = .003, and P = .003, respectively) and with glare at 3 cpd and 6 cpd (P = .001 and P = .02, respectively). The difference in higher-order aberration (HOA) values between groups was statistically significant with a 5.0 mm and 6.0 mm pupil, with the AcrySof IQ IOL inducing less HOA than the AcrySof Natural IOL. Eyes with an AcrySof IQ IOL also had statistically significant less spherical aberration than eyes with an AcrySof Natural IOL with both pupil diameters (P<.001). CONCLUSIONS: The AcrySof IQ IOL with a modified posterior surface induced significantly less HOA and spherical aberration than the AcrySof Natural IOL. Contrast sensitivity was better under mesopic conditions with the AcrySof IQ IOL. 相似文献

PURPOSE: To determine the higher order aberrations following implantation of four foldable intraocular lens (IOL) designs. METHODS: One hundred twenty-two eyes were randomly assigned to undergo implantation with one of four foldable IOL designs--Collamer (STAAR Surgical, Monrovia, Calif), plate haptic silicone (STAAR Surgical), AcrySof (Alcon, Ft Worth, Tex), or the Sensar (Advanced Medical Optics, Santa Ana, Calif). All enrolled eyes underwent wavefront assessment using the Tracey Visual Function Analyzer (Tracey Technologies, Houston, Tex). RESULTS: Preoperatively, no significant differences were noted in higher order aberrations between the four lens groups. No significant differences were noted among the plate haptic silicone, Sensar, or AcrySof lenses at 1 week or 1 month postoperatively, or among the Collamer, plate haptic silicone, or Sensar lenses, with regard to total higher order aberrations, although differences were observed in individual aberration terms. At 1 week and 1 month postoperatively, the Collamer IOL had significantly less higher order aberrations than the AcrySof lens (P < .01). Significantly less third and fourth order aberrations, coma, trefoil, spherical aberration, and tetrafoil were observed postoperatively with the Collamer than the AcrySof IOL. There were no parameters where the AcrySof had significantly less optical aberrations than the Collamer. CONCLUSIONS: The Collamer IOL design is associated with fewer higher order aberrations than the AcrySof lens. 相似文献

PURPOSE: To evaluate contrast sensitivity at distance and near after multifocal intraocular lens (IOL) implantation. SETTING: Ophthalmologic Institute of Alicante, University Miguel Hernández, Alicante, Spain. METHODS: Contrast sensitivity was measured with the Stereo Optical Functional Acuity Contrast Test at distance and near in 21 patients with a refractive multifocal IOL (Array SA-40N, AMO). A control group with a monofocal IOL (SI-40NB, AMO) was also studied to allow comparison of results. Contrast sensitivity was measured 1, 3, 6, 12, and 18 months after IOL implantation. RESULTS: There was a statistically significant greater reduction in contrast sensitivity at distance at all spatial frequencies in the multifocal group than in the monofocal group during the first month. At 3 months, contrast sensitivity at 12 and 18 cycles per deg remained reduced in the multifocal group; contrast sensitivity at the other frequencies did not differ from that in the monofocal group (P > 0.1). At 6, 12, and 18 months, contrast sensitivity at all spatial frequencies was not significantly different between groups (P > 0.1). There was a statistically significant greater reduction in near contrast sensitivity in the multifocal group than in the monofocal group at all spatial frequencies during the first and third month after surgery (P <.01). No statistically significant differences were found between groups after 6 months (P > 0.1). Contrast sensitivity at distance and near in the multifocal group improved over time (P <.01). CONCLUSIONS: The Array IOL provided contrast sensitivity at distance comparable to that obtained with the monofocal IOL between 3 and 6 months after implantation. Near contrast sensitivity improved over time but was always lower than at distance and in the monofocal near-corrected patients, which is acceptable to avoid near visual function degradation. 相似文献

Three patients presented with spontaneous myopic shift 6 weeks to 6 months after implantation of the Artiflex phakic intraocular lens (pIOL). The corrected distance visual acuity (CDVA) was maintained in all cases. Cyclopentolate drops successfully reverted the myopic shift in all cases; however, 2 cases required surgical reenclavation of the pIOL haptics to achieve long-term refractive stability. At the last follow-up, all patients had returned to emmetropia, with uncorrected distance visual acuities of 20/20. A slight decrease in objective optical quality was observed. No eye lost a line of CDVA. In 1 eye, optical coherence tomography scans showed slight posterior positioning of the pIOL. These cases show that spontaneous myopic shift is a potential complication of iris-claw pIOL implantation. That cyclopentolate drops and/or reenclavation of the haptics were effective in reverting the situation suggests a mechanical mechanism related to iris and ciliary body dynamics. PURPOSE: To evaluate contrast sensitivity after posterior chamber phakic intraocular lens (PIOL) implantation for the correction of high myopia. METHODS: Twenty eyes of ten patients had a posterior chamber phakic intraocular lens (Staar ICL) implanted to correct high myopia. Mean preoperative myopia was -14.10+/-2.70 D. Follow-up was 24 months for all patients. Contrast sensitivity was tested with best spectacle-corrected visual acuity preoperatively and 3, 6, 12, 18, and 24 months postoperatively. RESULTS: Contrast sensitivity increased after surgery in all spatial frequencies. Normal values were achieved for low and intermediate spatial frequencies (3 and 6 c/deg). However, in spite of the improvement, values were still below normal for high spatial frequencies (12 and 18 c/deg). When the first postoperative examination was not considered, there were no statistically significant differences in contrast sensitivity values at different postoperative periods. CONCLUSION: Contrast sensitivity increased after posterior chamber phakic intraocular lens implantation (Staar ICL) in all spatial frequencies when compared to preoperative contrast sensitivity (best spectacle-corrected). 相似文献

目的:探讨有晶状体眼人工晶状体术矫正高度近视患者术后的视觉质量。方法:30例高度近视患者(近视屈光度数为-8.25~-21.0D)植入有晶状体眼Verisyse人工晶状体矫正高度近视。术后至少3mo以上进行问卷调查,包括三部分,手术原因问卷,视功能问卷和术后满意度问卷。结果:100%患者均以提高裸眼视力为最主要手术目的,97%是为了去除眼镜,90%以上患者术后视功能好或很好,其中,以中距离视力好或很好的百分比较高。93%患者对该手术表示满意,并愿意推荐给家人或朋友。结论:有晶状体眼人工晶状体植入术矫正高度近视患者术后有较好的视觉质量,术后视功能和满意度均较好。 相似文献

A 49-year-old patient developed pupillary block glaucoma with an intraocular pressure (IOP) of 29 mm hg after implantation of a phakic intraocular lens (IOL) (NuVita, Bausch & Lomb) in the left eye. the anterior chamber deepened again, and the iop decreased to 16 mm hg after a neodymium: YAG iridotomy. Pupillary block glaucoma may occur after phakic IOL implantation without iridotomy, and we advocate that routine iridotomy be performed during phakic IOL surgeries. 相似文献

PURPOSE: To report three cases of Artisan phakic intraocular lens (PIOL) implantation to correct myopic refractive error after previous retinal detachment surgery treated with scleral encircling. METHODS: Artisan PIOLs were implanted in a 29-year-old man with -21.0 -2.0 x 180 manifest refraction and best spectacle-corrected visual acuity (BSCVA) of 20/40 (case 1), a 28-year-old woman with BSCVA of 20/20 and -8.5 -1.0 x 180 manifest refraction (case 2), and a 44-year-old man with BSCVA of 20/32 and -11.75 -1.75 x 10 manifest refraction (case 3). RESULTS: In case 1, 24 months after implantation of the Artisan PIOL, uncorrected visual acuity (UCVA) was 20/40. In case 2, 24 months after surgery, UCVA was 20/32. In case 3, 3 months after surgery, UCVA was 20/32. There was no formation of new breaks, progressive vitreoretinal traction, or complications. CONCLUSIONS: The Artisan PIOL may provide an alternative method to correct high myopia after retinal detachment surgery. 相似文献
