Nineteen patients with evidence of psoas and iliopsoas abnormalities on computed tomographic (CT) scans (12 with metastases, three with lymphoma, two with hematoma, and two with abscess) were examined with magnetic resonance (MR) imaging. The abnormal psoas could be identified on both T1- and T2-weighted spin-echo images, although T2-weighted sequences provided better contrast. The psoas muscle can be affected by one of three mechanisms: total replacement, lateral displacement, or medial displacement. In four patients in whom the CT study showed apparent enlargement of a psoas muscle, subsequent MR imaging examinations demonstrated that the psoas muscle was compressed and displaced laterally by a paraspinal mass. MR images provided better contrast between the normal and abnormal psoas than CT scans in nine cases; MR images were inferior to CT scans in two cases because calcifications (one case) and air bubbles within an abscess (one case) were not detectable.  相似文献

Temporal-lobe epilepsy: comparison of CT and MR imaging   总被引:2,自引:0,他引:2  
In 50 patients with temporal-lobe epilepsy, CT and MR findings were compared. Axial CT scans were obtained before and after administration of contrast material. Coronal MR imaging was carried out with two spin-echo (SE) sequences with a repetition time of 1600 msec and echo times of 35 or 70 msec (SE 1600/35, SE 1600/70). A focal lesion was detected by CT in 12 cases and by MR in 16 cases. If discrete attenuation or signal abnormalities are also taken into account, CT provided a positive finding in 13 cases and MR imaging in 20 cases. With the exception of a small calcification, all the lesions revealed on the CT scans were also detected on the MR images. Among the examinations assessable for temporal-lobe asymmetry, signs of a unilateral reduction in temporal-lobe size were seen on two of 35 CT scans and on 15 of 38 MR images. In three patients who had temporal-lobe resection, a subsequent comparison was made between CT, MR imaging, and pathology. Histologically proven glial reactions that could not be detected on CT were demonstrated as high-signal-intensity lesions on the SE 1600/70 image. We conclude that MR scanning, with its higher sensitivity, superior image quality, and ability of multiplanar imaging, should be the imaging technique of choice in the diagnosis of temporal-lobe epilepsy.  相似文献

PURPOSETo compare neuroimaging findings and clinical features in patients with Rocky Mountain spotted fever and to determine the impact of imaging studies in the treatment of these patients.MATERIALSWe reviewed the brain CT scans (n = 44), MR images (n = 6), or both (n = 4), and one MR spinal study in 34 patients with Rocky Mountain spotted fever, proved by definitive serologic criteria. Records were reviewed with attention to clinical symptoms and therapeutic modifications based on neuroimaging; outcomes were compared with imaging findings.RESULTSAbnormalities, consisting of infarctions, cerebral edema, meningeal enhancement, and prominent perivascular spaces, were found on four of 44 CT scans and on four of six MR studies. The spinal MR study showed abnormal enhancement of the lower spinal cord and cauda equina. Nonspecific clinical symptoms were present in all patients in whom neuroimaging findings were abnormal and in 80% of patients whose CT and/or MR findings were normal. After treatment, return to baseline clinical status was documented in 67% of patients with abnormal imaging findings and in 93% with normal findings. Death occurred in 17% of patients with abnormal neuroimaging results and in none of those with normal results.CONCLUSIONSAbnormalities on neuroimaging studies were not common in patients with Rocky Mountain spotted fever. When present, they were subtle. Symptoms at presentation and unfavorable outcomes were more prevalent when CT or MR findings were abnormal. Abnormalities identified on neuroimaging studies did not alter clinical treatment in any patient.  相似文献

BACKGROUND AND PURPOSE: Subarachnoid hemorrhage (SAH) constitutes an important neurologic emergency. Some authors have suggested that fluid-attenuated inversion recovery (FLAIR) MR imaging can detect SAH that may not be apparent on CT scans but may be revealed by lumbar puncture. We sought to determine how often FLAIR MR imaging findings are positive for SAH in cases with negative CT findings and positive lumbar puncture results. METHODS: The CT scans and FLAIR MR images of all patients with suspected SAH during a 3-year interval (2000-2002) were retrospectively reviewed by a blinded reader. Among these cases, we identified 12 with CT findings that were negative for SAH, lumbar puncture results that were positive for SAH, and FLAIR MR imaging findings that were available for review. Eleven of the 12 patients had undergone FLAIR MR imaging within 2 days of CT and lumbar puncture. The 12 patients with negative CT findings were comprised of six male and six female patients with an age range of 7 to 69 years. We evaluated the true and false negative and positive FLAIR MR imaging findings for SAH by using the lumbar puncture results as the gold standard. The FLAIR MR imaging findings of 12 additional patients without SAH (as revealed by lumbar puncture) were used as control data for a blinded reading. RESULTS: For all 12 control cases without SAH, the FLAIR MR imaging findings were interpreted correctly. Of the 12 cases that had positive lumbar puncture results but false-negative CT findings for SAH, FLAIR MR imaging findings were true-positive in only two cases and were false-negative in 10. One of the two true-positive cases had the highest concentration of RBC in the series (365 k/cc), and the other had the second highest value of RBC (65 k/cc). CONCLUSION: FLAIR MR imaging cannot replace lumbar puncture to detect the presence of SAH. FLAIR MR imaging findings are infrequently positive (16.7%) when CT findings are negative for SAH. This is likely because there is a minimum concentration of RBC/cc that must be exceeded for CSF to become hyperintense on FLAIR MR images.  相似文献

OBJECTIVE: To describe the MR findings of transient bone marrow edema (TBME) of the talus and to address the differential diagnostic considerations. DESIGN AND PATIENTS: The imaging findings of TBME of six tali were retrospectively reviewed in five patients with a clinical history of pain without trauma. Inclusion criteria were MR imaging findings that, when compared with clinical data and results of follow-up assessment, allowed the diagnosis of TBME. MR imaging, standard radiography, and bone scintigraphy were performed. The images were reviewed with particular attention to the pattern and distribution of abnormal marrow signal intensity as well as associated findings. RESULTS: In four cases the entire talus was involved, and in two cases only a portion of the bone was affected. No fractures were detected. MR imaging demonstrated diffuse decreased signal intensity of the marrow on T1-weighted images with corresponding increased signal intensity on T2-weighted images. In all six cases MR imaging detected associated findings, which included joint effusion and soft tissue edema. All patients improved clinically with conservative therapy over a period of 6 months to 1 year. CONCLUSIONS: Although unusual, TBME can involve the talus. Marrow edema without evidence of a fracture and in the absence of history of trauma is a characteristic MR imaging feature, allowing confident diagnosis and institution of conservative therapy.  相似文献

Cerebellar vermian atrophy after neonatal hypoxic-ischemic encephalopathy   总被引:3,自引:0,他引:3  
BACKGROUND AND PURPOSE: Although pathologic evidence of cerebellar injury due to birth asphyxia is well described, neuroimaging evidence is sparse. The primary purpose of this retrospective study was to evaluate the early and late imaging findings in the cerebellum of patients who had neonatal hypoxic-ischemic encephalopathy with thalamic edema shown by neonatal CT. The secondary aims were to validate thalamic edema shown by neonatal CT as a marker of thalamic injury and to assess the late cerebral cortical abnormalities associated with neonatal thalamic edema. METHODS: Fifty-five neonates with thalamic edema shown by CT performed when patients were 3 days old were identified from a cohort of full-term neonates with hypoxic-ischemic encephalopathy. Twenty-six of the 55 underwent follow-up neuroimaging. All sonograms, CT scans, and MR images of the brains of the 55 neonates were retrospectively reviewed by two pediatric neuroradiologists. The examinations were reviewed for evidence of hemorrhage, edema, atrophy, and CT attenuation or MR signal intensity abnormalities in the cerebellum, basal ganglia, and cerebral cortex. The neonatal autopsy findings in four cases were reviewed separately by a pediatric neuropathologist. RESULTS: Of the 55 neonates with thalamic edema shown by neonatal CT, 28 (51%) had thalamic edema with diffuse cerebral cortical edema, and 27 (49%) had thalamic edema without diffuse cortical edema. The cerebellar vermes appeared normal on all neonatal sonograms, CT scans, and MR images. However, atrophy of the cerebellar vermis was found in 12 (46%) of 26 patients by use of follow-up studies (95% CI, 27-65%). One of the 12 patients also had cerebellar hemispheric atrophy. Cerebellar vermian atrophy was shown at follow-up in eight (67%) of 12 patients who had neonatal thalamic edema with cortical sparing, compared with four (29%) of 14 patients who had thalamic edema with diffuse cortical edema. The difference did not reach statistical significance. The thalami appeared abnormal on follow-up neuroimages in 25 of 26 cases. Different patterns of cortical atrophy were observed on the images of patients who had thalamic edema with cortical sparing compared with those obtained in patients who had thalamic edema with cortical involvement. CONCLUSION: Cerebellar vermian atrophy is a frequent finding on follow-up images of patients in whom neonatal CT showed hypoxic-ischemic encephalopathy with abnormal thalami.  相似文献

目的:探讨神经节细胞胶质瘤(ganglioglioma,GG)的影像学特点,提高对这一少见脑肿瘤的认识。方法:回顾性分析经病理证实的10例GG的影像学表现。10例中,1例仅行CT平扫,5例行MR平扫+增强扫描(其中1例同时行MR灌注成像(peffusion weighted imaging,PWI),1例同时行PWI及MR波谱成像(MR spectroscopy,MRS)),3例同时行CT及MPd扫描(包括平扫、增强及1例PWI)。结果:10.例中,1例为脑内多发病变,9例单发病变中,5例位于颞叶,2例于小脑,2例于基底节区。所有病变未见明显占位征象,瘤周水肿无或轻微。CT平扫3例中2例呈低密度,1例因病变小且位于左颞叶边缘,CT检查未发现病变;CT增强扫描2例见病变实性部分明显强化;MRI平扫8例病变呈T1WI低信号为主、T2WI高信号为主;DWI表现多样;MRI增强扫描8例呈环形强化、团片状强化、弥漫强化或无明显强化;PWI检查3例未见局部脑血容量(rCBv)升高;MRS检查1例仅显示NAA轻度减低,未见异常代谢产物。结论:GG的临床和影像表现有一定特点,联合应用多种影像学检查方法有助于该病变的术前诊断。  相似文献

目的:探讨妊娠期高血压疾病合并可逆性后部脑病综合征(RPES)的影像学特征,提高对此病的认识。方法:回顾性分析35例妊娠期高血压合并RPES患者的临床及影像学特征。首次影像学检查:CT检查19例,MRI检查16例。影像学复查:17例患者于首次检查后3~14d进行复查,平均复查间隔时间为6d,其中CT复查7例,MRI复查10例。结果:RPES主要表现为对称性皮层下脑水肿,于CT图像上呈不规则形低密度区,于MRT。wI上呈高信号,主要位于双侧顶枕叶(32/35),其次是额叶(23/35)和颞叶(16/35),也累及深部脑白质(15/35)和基底节区(11/35),小脑(5/35)和脑干(3/35)偶有累及;合并蛛网膜下腔出血2例。MRI增强扫描:5例患者病灶均未见强化;MRA:左侧大脑中动脉闭塞1例,正常3例;2例患者MRV检查均正常;DWI:病灶多呈等信号或低信号(10/12),ADC图上呈高信号;DWI示2例病灶中出现斑点状高信号,ADC图提示此区域扩散受限。复查:17例患者治疗后CT或MRI复查显示病灶完全消失13例,病灶范围明显缩小4例。结论:妊娠期高血压疾病合并RPES的影像学表现有一定特征性,主要表现为皮层下脑白质血管源性水肿,以累及后循环供血区域为主,双侧较对称,治疗后病变范围迅速减小甚至消失。  相似文献

PURPOSE: To retrospectively determine what information, if any, magnetic resonance (MR) imaging of the cervical spine in obtunded and/or "unreliable" patients with blunt trauma adds to multi-detector row computed tomography (CT) of the entire cervical spine (including routine multiplanar sagittal and coronal reformations) when the CT findings are normal. MATERIALS AND METHODS: The study was HIPAA compliant and institutional review board approved. Informed consent was not required. From April 2001 to November 2003, 1400 trauma patients underwent MR imaging of the cervical spine to evaluate potential cervical spine injuries. Multi-detector row CT of the cervical spine was performed with a four- or 16-detector row scanner. MR imaging of the cervical spine was performed with transverse gradient-echo, sagittal intermediate-weighted, sagittal short inversion time inversion-recovery, and sagittal T1- and T2-weighted fast spin-echo sequences. Many MR examinations were performed to exclude soft-tissue injuries in the cervical spine of obtunded patients with blunt trauma in whom cervical spine injury could not be excluded with physical examination. Complete cervical spine MR studies were obtained to evaluate soft-tissue injuries in 366 obtunded patients with blunt trauma (281 male and 85 female patients; age range, 13-92 years; mean age, 42.1 years). The patients had previously undergone total cervical spine multi-detector row CT with normal findings. The results obtained with these two modalities were compared. RESULTS: MR images were negative for acute injury in 354 of the 366 patients and negative for cervical spine ligamentous injury in 362. Seven of the 366 patients had cervical cord contusions, four patients had ligamentous injuries, three patients had intervertebral disk edema, and one patient had a cord contusion, a ligamentous injury, and an intervertebral disk injury. Four patients had ligamentous injuries; however, all of these patients had ligament injuries limited to only one of the three columns of cervical spine ligament support. Multi-detector row CT had negative predictive values of 98.9% (362 of 366 patients) for ligament injury and 100% (366 of 366 patients) for unstable cervical spine injury. CONCLUSION: A normal multi-detector row CT scan of the total cervical spine in obtunded and/or "unreliable" patients with blunt trauma enabled the authors to exclude unstable injuries on the basis of findings at follow-up cervical spine MR imaging.  相似文献

BACKGROUND AND PURPOSE: Chordoid glioma is a new clinicopathologic entity that occurs in the region of the hypothalamus/anterior third ventricle. The aims of this study were to describe the characteristic radiographic features of chordoid glioma, identify specific imaging features that may enable differentiation of chordoid glioma from other suprasellar tumors, and increase neuroradiologists' awareness of this newly described tumor, facilitating prospective diagnosis. METHODS: CT scans and/or MR images of six patients with chordoid glioma were reviewed retrospectively to determine whether any characteristic radiographic features would emerge. Reports of the clinical presentation, pathologic findings, and radiographic findings of another six patients were reviewed and included, for a total patient population of 12 (mean age +/- SD, 46 +/- 13 years). RESULTS: Imaging features were strikingly similar for all tumors. In each case, the mass was ovoid, was well circumscribed, was located in the region of the hypothalamus/anterior third ventricle, and enhanced uniformly and intensely. Tumors were hyperdense to gray matter on CT scans and were isointense on T1-weighted MR images and slightly hyperintense on long-TR MR images. In two patients, vasogenic edema extended into the optic tracts, and in three, there was hydrocephalus. CONCLUSION: Chordoid glioma is a recently described unique histopathologic entity that has been added to the World Health Organization glioma classification scheme and must be included in the differential diagnosis of a suprasellar mass. Distinctive imaging features are its location, ovoid shape, hyperdensity on CT scans, and uniform intense contrast enhancement.  相似文献

OBJECTIVE: To compare the findings of magnetic resonance (MR) imaging with those of computed tomography (CT) of focal liver lesions related to peripheral eosinophilia. METHODS: For 12 patients with peripheral eosinophilia (>7%) examined with hepatic MR imaging and CT, 52 focal hepatic lesions larger than 0.5 cm, including 31 lesions simultaneously found on the 2 imaging modalities, were subjected to a comparative analysis of their imaging features. RESULTS: The total number of lesions distinguished from background liver was 39 (75%) on MR imaging and 44 (85%) on CT scans. On arterial phase images of 10 patients with comparable data, homogeneously hyperintense lesions were demonstrated more frequently (P = 0.006) on MR imaging (16 [50%] of 32 lesions) than on CT scans (4 [13%] of 32 lesions). Only 7 (22%) of the 32 hypoattenuating lesions on portal phase CT were depicted as hypointense lesions on portal phase MR images in 12 patients. On delayed phase images in 8 patients, the number of hyperintense lesions on MR images (9 [56%] of 16) was greater (P = 0.077) than that seen on the CT scans (4 [25%] of 16). CONCLUSIONS: For many focal hepatic lesions related to peripheral eosinophilia, dynamic MR imaging more easily demonstrates lesional enhancement on arterial and delayed phases than CT scans. Because of the higher degree of lesional enhancement of MR imaging compared with CT, the lesion-to-liver contrast may not be sufficient to distinguish the lesion from the background liver, resulting in decreased sensitivity of portal phase dynamic MR imaging.  相似文献

CNS sarcoidosis: evaluation with contrast-enhanced MR imaging.   总被引:1,自引:0,他引:1  
Reports of findings on unenhanced MR images and contrast-enhanced CT scans in patients with intracranial sarcoidosis have suggested that MR imaging without contrast enhancement may miss meningeal involvement, which is a frequent and prominent finding in neurosarcoidosis. We studied 14 patients with CNS sarcoidosis with T1- and T2-weighted pre- and postcontrast sequences and T1-weighted postcontrast sequences. Eight of 12 patients with intracranial sarcoidosis and one of two with spinal sarcoidosis had meningeal involvement that was not apparent on the unenhanced scans. Eight of 12 patients had intraaxial areas of high signal intensity on T2-weighted images, although only two of these lesions enhanced. Three patients had enhancing extraaxial masses mimicking meningiomas on postcontrast T1-weighted images. In two patients, the lesions decreased markedly in size after steroid treatment. In one patient with sarcoidosis of the optic nerve, the lesion decreased in size and the patient's vision returned to normal after Cytoxan therapy. In five of 14 patients, CNS findings were the initial clinical manifestation of the disease. In nine of 14 patients, the diagnosis of neurosarcoidosis was suggested only after administration of contrast agent. Use of gadopentetate dimeglumine greatly enhances the sensitivity of MR imaging in the detection of CNS sarcoidosis.  相似文献

PURPOSETo evaluate imaging findings in MELAS (mitochondrial myopathy, encephalopathy, lactic acidosis, strokes) syndrome for the presence and location of infarctions and the presence of lactate.METHODSEight patients were studied with MR (n = 8) and CT (n = 2). One patient underwent single-photon emission CT with technetium 99m hexamethyl-propyleneamine oxime and one patient had conventional catheter angiography. One fixed brain was studied with MR imaging. Five patients underwent single volume proton MR spectroscopy. Imaging studies were evaluated for atrophy, edema, and infarctions. Proton MR spectroscopy was visually analyzed for presence or absence of lactate.RESULTSOne patient showed a cerebral infarction, and later a second distant infarction developed. One patient showed a transient area of cortical edema. Two patients had small nonspecific periventricular white matter abnormalities and one patient had diffuse white matter hyperintensities. Two patients had nonspecific MR abnormalities (probably age-related changes), and two had normal MR findings. None had basal ganglia involvement. Proton MR spectroscopy showed presence of lactate in one case with transient cortical edema; in two cases with nonspecific (probably age-related) brain findings; and in two patients with normal MR findings.CONCLUSIONSPatients with MELAS have a variety of MR findings. The fact that proton MR spectroscopy showed lactate in all five cases studied, regardless of MR findings, indicates that proton MR spectroscopy may be more sensitive in the detection of MELAS-associated abnormalities than MR imaging.  相似文献

PURPOSETo evaluate a high-resolution, thin-section fast spin-echo MR imaging technique of the inner ear to identify the large vestibular aqueduct syndrome seen on temporal bone CT scans.METHODSWe retrospectively reviewed the temporal bone CT scans of 21 patients with hearing loss and enlarged bony vestibular aqueducts by CT criteria. High-resolution fast spin-echo MR imaging was then performed on these patients using dual 3-inch phased-array receiver coils fixed in a temporomandibular joint holder and centered over the temporal bones. MR imaging included axial and oblique sagittal fast spin-echo sequences. The diameter of the midvestibular aqueduct on CT scans and the signal at the level of the midaqueduct on MR images were measured on axial sequences, then compared. High-resolution MR imaging with the same protocol was performed in 44 control subjects with normal ears, and similar measurements were taken.RESULTSThe average size of the enlarged bony vestibular aqueduct on CT scans was 3.7 mm, and the average width of the signal from within the enlarged aqueduct on MR images was 3.8 mm. Statistical analysis showed excellent correlation. MR images alone displayed the enlarged extraosseous endolymphatic sac, which accompanies the enlarged aqueduct in this syndrome. Five ears in three patients with enlarged bony vestibular aqueducts on CT scans showed no evidence of an enlarged endolymphatic duct or sac on MR images. An enlarged endolymphatic sac was seen on MR images in one patient with a bony vestibular aqueduct, which had normal measurements on CT scans. MR imaging alone identified a single case of mild cochlear dysplasia (Mondini malformation). In the 88 normal ears studied, the average size of the endolymphatic sac at its midpoint between the common crus and the external aperture measured on MR images was 0.8 mm (range, 0.5 to 1.4 mm). In 25% of the normal ears, no signal was seen from within the vestibular aqueduct.CONCLUSIONThin-section, high-resolution fast spin-echo MR imaging of the inner ear is complementary to CT in studying patients with the large vestibular aqueduct syndrome, as MR imaging better displays the soft tissue and fluid of the membranous labyrinth.  相似文献

Twenty-four patients with 29 cerebrovascular malformations were evaluated with a combination of computed tomography (CT), angiography, and magnetic resonance (MR) imaging. Characteristics of the malformations on MR images were reviewed retrospectively, and a comparative evaluation of MR and CT images was made. Of 14 angiographically evident malformations, 13 intra-axial lesions were detected on both CT and MR images, and one dural malformation gave false-negative results on both modalities. The appearance of parenchymal lesions on MR images closely mirrored characteristic CT findings. Calcific foci were difficult to separate from vessels on both images. Clot was more easily identified on MR images. In the detection of 15 angiographically occult malformations, CT proved more sensitive when focal calcification was the only evidence of their presence. MR study failed to detect two small supratentorial lesions evidenced by faint calcifications on CT scans. In two patients, MR images showed small hemorrhages not detectable by CT, and MR provided strong evidence for the diagnosis of hematoma for 12 lesions. Angiographically evident malformations have a highly characteristic appearance on MR images. MR may be more sensitive than CT in the detection of small hemorrhagic foci associated with cryptic arteriovenous malformations and may add specificity in the diagnosis of occult malformations in some cases, but MR is less sensitive than CT for the detection of small calcified malformations.  相似文献

Twenty-seven patients with chronic middle ear infection were prospectively studied with pre- and post-contrast magnetic resonance (MR) images to assess the role of MR imaging in the recognition of middle ear tissue abnormalities. The findings were correlated with computed tomographic (CT) scans and surgical and pathologic data. Granulation tissue constantly appeared enhanced on studies done with gadolinium diethylenetriaminepentaacetic acid (DTPA), unlike cholesteatoma, cholesterol granuloma, or brain herniation into the middle ear cavities. Evaluation of extension and thickness of the inflammatory tissue with MR imaging was in accordance with surgical findings. In six cases, isolated granulation tissue misdiagnosed as either a cholesteatoma or herniated brain on CT scans was accurately evaluated on postcontrast MR images. When granulation tissue was associated with other soft-tissue masses, Gd-DTPA-enhanced MR images allowed accurate definition of the site and the extension of each lesion. Furthermore, abnormal meningeal enhancement was precisely depicted by MR images in two cases.  相似文献

The value of MR imaging was compared with that of high-resolution CT in assessing chronic infiltrative lung disease in 25 patients. The cases included nine patients with usual interstitial pneumonia, six with sarcoidosis, four with hypersensitivity pneumonitis, and six with miscellaneous conditions. The diagnosis was proved by biopsy (n = 17) or by means of clinical, laboratory, and radiologic criteria (n = 8). All patients had 1.5-T MR imaging and CT of the chest. Cardiac-gated T1-, proton density-, and T2-weighted spin-echo sequences were obtained. Initially, the MR images were assessed independently; later they were compared directly with the corresponding CT scans. In six patients, MR images and CT scans were obtained before open lung biopsy, and the images and scans were assessed prospectively. CT was superior to MR imaging in the anatomic assessment of the lung parenchyma and in showing fibrosis. However, areas of air-space opacification (ground-glass opacities) were seen as well on MR as on high-resolution CT. In the six patients who had open lung biopsy, areas of air-space opacification seen on MR and on CT corresponded to areas of active alveolitis or air-space infiltrates pathologically. Follow-up in six patients showed equal degrees of change in the air-space opacification over time on MR and CT. We conclude that, although MR imaging is inferior to high-resolution CT in the assessment of chronic infiltrative lung diseases, it may play a role in the assessment and follow-up of patients with air-space opacification.  相似文献

Invasion of laryngeal cartilage by cancer: comparison of CT and MR imaging   总被引:2,自引:0,他引:2  
Forty-two patients with laryngeal carcinomas were examined with computed tomography (CT) and magnetic resonance (MR) imaging. The accuracy of both CT and MR imaging in the depiction of cartilage invasion was evaluated in 16 patients by comparing findings at CT and MR with pathologic findings. Calcified cartilage that has been invaded by cancer is frequently seen on CT scans as having an intact contour. Tumor approaching nonossified cartilage may simulate cartilage invasion. On T1-weighted MR images, invaded marrow of ossified cartilage is of intermediate signal intensity, allowing it to be differentiated from normal bone marrow. On proton-density images, tumor is of increased signal intensity, which allows it to be differentiated from nonossified cartilage. In our experience, the specificities of CT and MR imaging were approximately equal (91% and 88%, respectively), but CT had a considerably lower sensitivity than MR (46% vs. 89%). Gross movement artifacts, which resulted in nondiagnostic images, occurred in 16% of the MR examinations. MR imaging is recommended as the modality of choice in the diagnosis of cartilage invasion.  相似文献

The role of MR in evaluating tuberous sclerosis is reviewed in 15 patients. These studies were compared with CT scans, which were available in 14 patients. Four characteristic findings were noted on the MR images obtained. Subependymal nodules projecting into the lateral ventricles were seen in 12 of 15 patients on T1-weighted images. This was the most specific finding. Distortion of the normal cortical architecture was seen in 10 of 11 patients in whom T1-weighted images were obtained using a 256 X 256 matrix. These foci corresponded to multiple cortical areas of increased signal on T2-weighted images. Dilated ventricles were seen in five patients. In one patient, a known astrocytoma showed increased signal on the T2-weighted images, allowing differentiation from a benign subependymal nodule. MR depicted the cortical hamartomas more completely than did CT. The MR scans were abnormal in all cases, and a diagnosis could be confidently made in all 11 cases scanned using a 256 X 256 matrix. Our preliminary experience suggests that MR will at least equal and probably exceed CT, both for sensitivity and specificity, given the use of a 256 X 256 matrix.  相似文献

目的 通过分析非酮症高血糖性舞蹈病的CT和MRI表现与临床表现之间的关系,以探讨该疾病的影像诊断价值.方法 回顾性分析6例非酮症高血糖性舞蹈病的影像表现和临床资料.6例均行CT平扫,其中1例行MR扫描.6例中3例行CT复查,1例行MR复查.结果 6例患者CT均表现为一侧或两侧纹状体密度增高,均累及壳核,其中5例同时累及苍白球外侧部或尾状核;3例CT复查表现为受累纹状体密度减低或密度增高影消失.1例T1WI表现为两侧豆状核高信号,T2WI表现为低信号;MRI复查示受累豆状核T1WI和T2WI信号无变化.所有患者的舞蹈病症状均在血糖控制后2~6 d内消失.结论 典型的CT和MRI表现有助于非酮症高血糖性舞蹈病的诊断  相似文献
