PURPOSE: To evaluate the surgical efficacy of pars plana vitrectomy on eyes with diabetic macular edema in the presence or absence of a complete posterior vitreous detachment and with or without an epimacular membrane. METHODS: Pars plana vitrectomy was performed on 30 eyes of 29 cases with diabetic macular edema. Visual acuity was measured, and retinal thickness was determined by optical coherence tomography before and after vitrectomy. To evaluate the relationship between the effects of vitrectomy and the presence or absence of posterior vitreous detachment and/or epimacular membrane, all eyes were placed into one of four groups: group A, eyes with posterior vitreous detachment and epimacular membrane; B, eyes with posterior vitreous detachment and without epimacular membrane; C, eyes without posterior vitreous detachment and with epimacular membrane; and D, eyes without posterior vitreous detachment and without epimacular membrane. The expression of vascular endothelial growth factor and interleukin-6 was investigated immunohistochemically in epimacular membrane specimens obtained from seven eyes with diffuse diabetic macular edema. RESULTS: The postoperative mean visual acuity (0.653 +/- 0.350: mean +/- SD logarithm of minimal angle of resolution [logMAR]) was significantly better than the mean preoperative visual acuity (0.891 +/- 0.319 logMAR; Wilcoxon signed-rank test, P =.0007). The postoperative foveal thickness (264.5 +/- 118.6 microm) was significantly thinner than the preoperative foveal thickness (477.8 +/- 147.7 microm; Wilcoxon signed-rank test, P <.0001). There were no significant differences in the improvement of visual acuity and decrease of foveal thickness between the four groups (Kruskal-Wallis test, P =.13, P =.65, respectively). All of the epimacular membranes obtained at surgery expressed vascular endothelial growth factor and interleukin-6. CONCLUSIONS: These results demonstrated that vitrectomy with removal of epimacular membrane is generally an effective procedure in reducing diabetic macular edema, and the outcome does not depend on the presence absence of posterior vitreous detachment and epimacular membrane.  相似文献

目的 探讨超声乳化白内障吸除术后黄斑形态的变化及其可能的影响因素。方法 对行超声乳化白内障吸除术 ,且术前无合并症、术中无并发症发生的单纯老年性白内障患者 80例 (80只眼 )于术前和术后 1周分别行光学相干断层成像术 (opticalcoherencetomography ,OCT)检查 ;按术中使用的超声能量高低分成 2个组。观察黄斑中心凹视网膜厚度的变化及其与术中超声能量、术后前房炎性反应和视力的关系。结果  80只眼黄斑中心凹视网膜平均厚度术前为 (14 2 9± 16 7) μm ,术后为 (15 7 9± 36 7) μm ,两者比较差异无显著意义 (P >0 0 5 )。术后 3只眼出现黄斑水肿 ,11只眼出现房水中度闪光 ,黄斑中心凹视网膜平均厚度术前为 (139 9± 11 3) μm ,术后为 (197 6± 36 9) μm ,两者比较差异有显著意义 (t =2 75 1,P <0 0 5 )。低能量组术后黄斑中心凹视网膜平均厚度为 (15 6 2± 18 3) μm ,高能量组为 (172 6± 32 9) μm ,两者比较差异有显著意义 (t=2 4 11,P <0 0 5 )。术后最佳矫正视力与黄斑中心凹视网膜厚度呈负相关性 (r=- 0 82 ,P <0 0 5 )。结论 超声乳化白内障吸除术可导致术眼黄斑中心凹视网膜厚度增加及黄斑水肿 ;术中高超声能量可明显影响术后黄斑中心凹视网膜的形态 ;术后黄斑中心  相似文献

PURPOSE: To determine the difference between two types of retinal detachment surgery in postoperative visual recovery in the management of macular-off rhegmatogenous retinal detachment. METHODS: We evaluated retrospectively the postoperative visual acuity (VA) in 37 patients who underwent primary vitrectomy (group V) and 39 patients treated with conventional scleral buckling (group S). RESULTS: Although the mean preoperative VA in group V (0.03) was significantly worse (p = 0.04) than that in group S (0.06), there was no statistically significant difference between the groups in postoperative VA throughout the follow-up period. However, in the cases with poor preoperative VA (VA < 0.1), ocular hypotony (IOP < 7 mmHg), or prolonged macular detachment (duration > 7 days), the visual recovery was significantly better (p < 0.05) in group V than in group S from 1 month postoperatively. There were more eyes with a final VA of more than 0.5 in group V than in group S. When considering only the eyes with lenses spared intraoperatively, postoperative cataract progression resulting in secondary visual reduction was statistically significantly greater (p < 0.01) in group V (62%) than in group S (8%). CONCLUSIONS: Primary vitrectomy is effective to attain early visual rehabilitation, especially to manage macula-off retinal detachments with poor preoperative VA, ocular hypotony, and prolonged macular detachment. To prevent a secondary visual reduction, cataract surgery combined with vitrectomy is recommended in selected cases.  相似文献

OBJECTIVE: To investigate outcomes of vitreoretinal surgery for complications of branch retinal vein occlusion (BRVO). DESIGN: Retrospective, noncomparative case series. PARTICIPANTS: The medical records of all patients who underwent vitreoretinal surgery for complications of BRVO at Bascom Palmer Eye Institute between January 1, 1991 and December 31, 1998 were reviewed. Thirty-six eyes from 36 consecutive patients were identified. MAIN OUTCOME MEASURES: Visual acuity outcomes include preservation of preoperative visual acuity and visual acuity greater than or equal to 20/40, 20/200 and 5/200. When preoperative retinal detachment was present, the anatomic outcome assessed was complete retinal attachment. Postoperative event rates of retinal detachment, vitreous hemorrhage, epiretinal membrane (ERM), and cataract were tabulated. All outcomes were assessed at 6 months. RESULTS: Surgical indications included nonclearing vitreous hemorrhage (17 patients), traction retinal detachment involving the macula (15), and ERM (4). Mean follow-up was 19 months. Preoperatively, best-corrected vision was greater than or equal to 20/200 in 19/36 (53%) eyes. Six months postoperatively, best-corrected vision was greater than or equal to 20/40 in 12/36 (33%) eyes, greater than or equal to 20/200 in 27/36 (75%) eyes, and greater than or equal to 5/200 in 31/36 (86%) eyes. Postoperative complications included retinal detachment (2/36; 6% eyes), ERM (3; 8%), vitreous hemorrhage (2; 6%), suprachoroidal hemorrhage (1; 3%), central retinal vein occlusion (1; 3%), and central retinal artery occlusion (1; 3%). Clinical features associated with better visual outcome include better preoperative visual acuity (P: = 0.05), absence of preoperative afferent pupillary defect (P: = 0.01), and absence of preoperative macular edema (P: = 0.08). CONCLUSIONS: Following surgery, retinal attachment and improved visual acuity were achieved in the majority of patients. Pre-existing pathology and postoperative complications may limit final vision in eyes with BRVO.  相似文献

PURPOSE: To determine the early postoperative changes in retinal thickness and complications after pars plana vitrectomy for diabetic macular edema. DESIGN: Consecutive interventional case series. METHODS: Studied retrospectively, pars plana vitrectomy was performed on 65 consecutive eyes of 63 patients with diabetic macular edema. The follow-up interval ranged from 6 to 36 months (12.6 +/- 7.4 months [mean +/- standard deviation (SD)]). The indications of pars plana vitrectomy in this study were (1) diffuse diabetic macular edema, (2) preoperative visual acuity less than 20/40, and (3) noneffective macular photocoagulation therapy. Preoperative and postoperative examinations by stereoscopic biomicroscopy, color fundus photography of the macula and optical coherence tomography (OCT) were performed on all eyes. Preoperatively, direct photocoagulation to microaneurysms in the macula had been performed in 48 eyes, and focal/grid photocoagulation had been performed in five eyes. Preoperative examination showed that epiretinal membranes were observed in 20 eyes, cystoid macular edema in 40 eyes, and 23 eyes had a complete posterior vitreous detachment (PVD). Epimacular membranes, removed during surgery, were examined histopathologically. RESULTS: The postoperative mean best-corrected visual acuity (logarithm of the minimum angle of resolution [logMAR] = 0.696 +/- 0.491 [mean +/- SD]) was significantly better than the preoperative mean best-corrected visual acuity (0.827 +/- 0.361; P <.0001; Wilcoxon signed-rank test). The final visual acuity improved by 2 or more lines in 32 of 65 eyes (45%), remained unchanged in 32 of 65 eyes (49%), and exacerbated after the surgery in 4 of 65 eyes (6%) due to neovascular glaucoma (2 eyes) and residual cystoid macular edema (2 eyes). The postoperative foveal retinal thickness (224.9 +/- 116.9 microm) at the last visit was significantly thinner than the preoperative foveal retinal thickness (463.7 +/- 177.3 microm; P <.0001; Wilcoxon signed-rank test). The foveal retinal thickness did not decrease linearly but fluctuated: The mean postoperative retinal thickness had decreased significantly 7 days after surgery, then remained unchanged for approximately 1 month, and thereafter gradually decreased until 4 months. The intraoperative and postoperative complications included peripheral retinal tear in 3 of 65 (4.6%) eyes, postoperative rhegmatogenous retinal detachment in 1 of 65 (1.5%) eyes, neovascular glaucoma in 3 of 65 (5%) eyes, recurrent vitreous hemorrhage in 1 of 65 (1.5%) eyes, hard exudates in the center of the macula in 3 of 56 (4.6%) eyes, postoperative epiretinal membrane formation in 9 of 65 (13.8%) eyes, and a lamellar macular hole in 1 of 65 (1.5%) eyes. CONCLUSIONS: Vitrectomy for diabetic macular edema is an effective procedure for reducing the edema and improving visual acuity. Because the postoperative reduction in retinal thickness is not complete until 4 months, the assessment of vitrectomy on foveal thickness should not be made until this time. In addition, there are severe complications from vitrectomy for diabetic macular edema, and careful preoperative and postoperative examinations and surgical methods are required.  相似文献

BACKGROUND: A pathological vitreomacular adhesion is a common pathogenetic mechanism of various clinical entities such as idiopathic epimacular membrane, vitreomacular traction syndrome, and macular hole. Vitrectomy is recommended for these disorders. Anatomical and functional results in 207 operated eyes are discussed. PATIENTS AND METHODS: The results of a vitrectomy in 3 groups of patients were compared: idiopathic epimacular membrane (group 1, n = 52), vitreomacular traction syndrome (group 2, n = 48); macular hole (group 3, n = 107; 33 eyes without and 74 eyes with retinal detachment). After excision of the vitreous gel, a thin layer of epimacular vitreous cortex was identified and excised by gentle aspiration under continuous air infusion. In 50 of the 107 eyes of group 3, the vitrectomy was combined with the application of a drop of autologous blood to the macular hole. RESULTS: A vitreomacular adhesion existed in 56% of group 1, 74% of group 2, and 84% of group 3. The visual acuity improved in 54%, 62% and 50% of eyes of the 3 groups, respectively. More eyes with an initial visual acuity of at least 40/200 achieved final vision of 80/200 or better (57%, 65% and 48% of group 1 to 3, respectively) than eyes with acuities of less than 80/200. A postoperative cataract was the main reason for reduced visual results in all cases. In group 3-eyes with retinal detachment and/or myopia, a significant postoperative visual improvement was achieved only after application of autologous blood to the macular hole. CONCLUSION: A pathological vitreomacular adhesion was identified in the majority of patients with idiopathic epimacular membranes, vitreomacular traction syndrome, and macular hole, respectively. Vitreoretinal surgery for syndromes with vitreomacular traction is indicated as it warrants a significant improvement of visual function and relief of metamorphopsia.  相似文献

PURPOSE: To evaluate visual outcome after epiretinal membrane surgery. MATERIAL: and method: Retrospective study of 50 consecutive epiretinal membranes (ERM) (23 idiopathic and 27 secondary) with 12 to 42-month follow-up (mean 24 months). Functional evaluations were performed before and after surgery according to the idiopathic or secondary nature of the epiretinal membranes. RESULTS: For idiopathic ERM, vision improved by more than two lines in 43% of the eyes (33% after ERM surgery alone and 55% after ERM followed by cataract surgery) with final visual acuity of 4/10 for 66% of the patients (58% and 82% respectively); there was visual degradation for the two ERM on highly myopic eyes. For the secondary ERM, vision improved by more than 2 lines for 41% of the eyes and final visual acuity of 4/10 for 33% of the patients; visual degradation for almost all ERM secondary to uveitis was associated with preoperative cystoid macular edema. CONCLUSION: After surgery for idiopathic or secondary ERM, visual acuity is good with the exception of the idiopathic ERM on highly myopic eyes and of the uveitic ERM with preoperative cystoid macular edema. Progression of cataract often perturbs visual outcome analysis.  相似文献

目的观察23G玻璃体切除手术治疗孔源性视网膜脱离的临床效果。方法对23例(23眼)孔源性视网膜脱离应用23G玻璃体切除手术联合全氟丙烷(C3F8)或硅油填充,下方视网膜裂孔者联合巩膜外加压,合并白内障者同时对白内障行超声乳化吸出术。随访3~10个月,观察术后视网膜复位、视力等情况。结果23眼手术顺利完成。术后3个月时视网膜复位21眼(91.30%),2眼(8.70%)视网膜脱离复发,再次玻璃体手术填充硅油视网膜复位。未发生其他并发症。术后3个月时视力:(1)黄斑区未脱离眼:术前最佳矫正视力0.1~0.5,LogMAR视力:1.0~0.3,平均0.42±0.29。术后最佳矫正视力0.2—0.5,LogMAR视力:0.7~0.3,平均0.38±0.19,t=1.83,P〉0.05,与术前比较差异无统计学意义。(2)黄斑区脱离眼:术前最佳矫正视力0.08~0.2,LogMAR视力:1.1~0.7,平均0.81±0.13。术后最佳矫正视力0.15—0.5,LogMAR视力:0.8~0.3,平均0.47-e0.26,t=5.78,P〈0.05,与术前比较差异有统计学意义。结论23G玻璃体切除手术治疗孔源性视网膜脱离效果良好,并发症少。  相似文献

目的:探讨高度近视眼合并白内障行超声乳化白内障吸除人工晶状体植入术治疗的临床疗效。方法:27例(37眼)因白内障合并高度近视行超声乳化白内障吸除+人工晶状体植入术。记录术前眼轴长度和术后视力,屈光度数,观察手术并发症术后眼部情况。术后随访时间为6~24mo。结果:术前平均眼轴长度为28.3mm。术后最佳矫正视力≥0.2共35眼(95%),≥0.5者共33眼(89%)。术中无后囊膜破裂;术后角膜水肿2眼,术后一过性高眼压1眼,后发性白内障2眼;术后发现黄斑变性和眼底出血共4例,无视网膜和脉络膜脱离者。结论:超声乳化白内障吸除低度数折叠式人工晶状体植入术是治疗白内障合并高度近视眼安全、有效的方法。  相似文献

目的:探讨不同类型的黄斑前膜手术的成功率和手术损伤视力的风险。方法:对56例患有不同类型的黄斑前膜患者的57只手术眼的结果进行回顾性分析,观察手术前后视力改变;不同类型黄斑前膜术后视力改善的比较;视物变形改善率;术中并发症;术后并发症。结果:视力改善2行以上41只眼占78.8%,改善率高的为扣带术后黄斑前膜和黄斑裂孔合并黄斑前膜,其次为特发性黄斑前膜,玻璃体黄斑牵引综合征的视力改善较差。总体近视力改善高于远视力。视物变形改善率为65.0%。术中并发症为原裂孔开放4只眼和新裂孔形成5只眼。结论:扣带术后黄斑前膜,黄斑裂孔合并黄斑前膜以及特发性黄斑前膜手术后的视力改善率较高,手术具有一定的操作风险。  相似文献

PURPOSE: To assess the outcome of patients who underwent instantaneous pars plana vitrectomy for dislocated lens fragments during cataract surgery. METHODS: The medical records of 29 patients who underwent immediate pars plana vitrectomy for dislocated lens fragments at the same sitting of phacoemulsification surgery were reviewed. Data including patients' demographics, preoperative and postoperative vision, risk factors for lens fragment dislocation, and intraoperative and postoperative complications were recorded. RESULTS: The mean follow-up period of the patients was 12.3 months (range, 3-47 months). The median preoperative visual acuity was 6/90 (range, LP to 6/15). The median final postoperative visual acuity was 6/18 (range, NLP to 6/9) and the mean improvement in vision after the operation was 4.5 lines. The commonest risk factors for dislocation of lens fragment were late detection of posterior capsule tear, splitting of anterior capsulorrhexis, and hard nucleus. After excluding nine eyes with other pre-existing ocular comorbidities, 10 (50%) eyes had a final visual acuity of 6/12 or better. Complications after surgery included three (10%) cases of retinal detachment and one (3%) patient developed epiretinal membrane. None of the patients in the study developed secondary glaucoma or cystoid macular oedema following surgery. CONCLUSION: Immediate vitrectomy in the same sitting of the cataract surgery is a surgical option in the management of dislocated intravitreal lens fragments when vitreoretinal support is available. Most patients achieve a good visual outcome with reduced risk of secondary glaucoma and cystoid macular oedema after surgery.  相似文献

Is scleral fixation a safe procedure for intraocular lens implantation?   总被引:1,自引:0,他引:1  
Purpose: No consensus currently exists on the optimal method for intraocular (IOL) implantation without capsular support. We evaluated the outcome and angiographic findings of eyes that underwent the implantation of scleral fixated IOLs. Methods: Iris and retinal fluorescein angiography were performed in 13 eyes that had received posterior chamber IOL implantation with scleral fixation. Follow-up examinations also assessed visual acuity (VA), intraocular pressure (IOP), IOL decentration and complications related to the procedure. Results: Mean visual acuity was 0.29 preoperatively and 0.71 postoperatively after a mean follow-up of 14.2 months. A best corrected visual acuity of 0.5 or better was obtained in 12 eyes. Iris fluorescein angiography did not show major vascular abnormalities. Retinal angiography showed 5 cases of macular edema. In 6 eyes light-induced retinal lesions occurred. Cellophane maculopathy was disclosed in 4 eyes. Macular edema was associated with photic injury in 4 cases and with cellophane maculopathy in 2 cases. Mean postoperative visual acuity was 0.6 in eyes with macular edema and 0.88 in eyes without (SD 0.18; range 0.5–1.0). Four of 5 eyes with macular edema had a postoperative visual acuity of 0.5 or better. There was no evidence of persistent IOP elevation or IOL decentration. No serious complications were recorded during surgery. Conclusions: Transscleral fixation of posterior chamber IOLs provides adequate visual acuity in most patients. Macular edema was frequently associated with the procedure. Although this complication was a cause of low visual recovery after implantation, the majority of eyes with macular edema achieved a visual acuity of 0.5 or better. PURPOSE: To describe the results of a pars plana vitrectomy, combined with phacoemulsification, using a sutureless, superotemporal, clear corneal incision for patients with a macular hole. METHODS: This study reviewed the records of 22 patients (22 eyes) who underwent a phacoemulsification with the insertion of an acrylic intraocular lens, using a 3.2 mm superotemporal clear corneal incision and a pars plana vitrectomy with an internal limiting membrane peeling in one session, for the treatment of a macular hole. RESULTS: All 22 patients had their macular holes closed using the combined surgical procedures. The mean preoperative visual acuity was 0.086, and the mean postoperative visual acuity was 0.173. This improvement was statistically significant (paired Student's t-test, p < 0.05). No patients developed posterior capsular opacity, retinal detachment, or a cystoid macular edema. The surgically induced astigmatism (SIA) was 0.808 diopters (0.808 +/- 0.761) two months after surgery. CONCLUSIONS: Combining cataract surgery with vitrectomy can achieve visual rehabilitation in the early postoperative period without requiring post-vitrectomy cataract surgery. A sutureless clear corneal incision, used in this procedure, can minimize the SIA and promote postoperative wound healing. If sutureless, transconjunctival, pars plana vitrectomy can be used more widely in the future, then the simplified, combined cataract surgery using a small clear corneal incision will also become more common, hence decreasing operation time, and hastening postoperative recovery.  相似文献

PURPOSE: To determine the visual outcomes of eyes that underwent macular translocation surgery with 360 degrees peripheral retinectomy and silicone oil tamponade (MTS360) following ocular photodynamic therapy (OPT) with verteporfin for subfoveal choroidal neovascularization (CNV) associated with age-related macular degeneration (ARMD). DESIGN: Observational case series. METHODS: A retrospective review of patients who underwent MTS360 with silicone oil tamponade from August 5, 1998 through December 1, 2002. Patients who had at least one episode of OPT with verteporfin before surgery were identified. The number of OPT session, best-corrected Early Treatment Diabetic Retinopathy Study (ETDRS) visual acuity, complications (including postoperative CNV, retinal detachment [RD]), the presence or absence of cystoid macular edema (CME) by optical coherence tomography (OCT) were recorded. RESULTS: Eight eyes of eight patients were identified that fulfilled the inclusion criteria. All eyes had at least one episode of OPT (mean, 1.5 treatments). All of these patients at the time of MTS360 demonstrated continued visual loss following the most recent OPT session. The mean preoperative visual acuity was 56 letters. Four of the eight eyes demonstrated CME by OCT on preoperative examination. There were no significant postoperative complications other than one eye that had a successful repair of an RD. At 3 months, the mean visual acuity was 54 letters. At last follow-up (mean, 10 months), the mean visual acuity was 61 letters (P =.5). The final mean visual acuity change for the patients who had only one prior OPT (five eyes) was +10 letters. The mean final visual acuity change for the patients who had multiple OPT sessions (three eyes) was -1 letter. Three of the four eyes that had preoperative CME continued to demonstrate CME at last follow-up. Two eyes that had concurrent cataract extraction surgery with MTS360 did not develop CME. No eyes developed a recurrent CNV during the postoperative period. CONCLUSIONS: MTS360 with silicone oil tamponade for CNV associated with ARMD appears to be a viable option for patients who are continuing to lose vision in their better seeing eye following OPT with verteporfin. MTS360 appears to be effective in stabilizing vision, especially in patients who had previously undergone only one OPT session. Further studies are indicated to evaluate the efficacy of MTS360 in this current era of OPT.  相似文献

PURPOSE: We evaluated the visual outcome of vitrectomy for diabetic cystoid macular edema. METHODS: Visual outcome and factors which may influence final visual acuity were assessed and documented retrospectively in 45 eyes of 40 patients, all of whom were followed for at least 6 months postoperatively. RESULTS: Final postoperative visual acuity was two or more lines better than preoperative visual acuity in 58%, within one line in 40%, and had decreased by two lines or more in 2%. A final postoperative visual acuity of 0.5 or better was achieved in 38%. Preoperative visual acuity and the extent of the cystoid space on fluorescein angiography were significantly related to final visual acuity. A final postoperative visual acuity of 0.5 or better was noted in 8% of eyes with a preoperative visual acuity below 0.1, in 50% of eyes with preoperative visual acuity of 0.1 or better, in 71% of eyes with a cystoid space smaller than 5 disc areas, and in 20% of eyes with a cystoid space of 5 disc areas or more. The posterior vitreous membrane condition did not influence final visual acuity. There were no complications which decreased visual acuity. CONCLUSION: Based on the above results, we conclude that diabetic cystoid macular edema is a good indication for vitrectomy regardless of the posterior vitreous membrane condition. A preoperative visual acuity of 0.1 or better and/or a cystoid space smaller than 5 disc areas may be indications for surgery aimed at achieving a final postoperative visual acuity of 0.5 or better.  相似文献

目的:观察手术治疗先天性白内障的临床效果。方法:对97例(136眼)先天性白内障实行白内障摘除、后囊连续环形撕囊、前部玻璃体切除术,部分联合Ⅰ期或Ⅱ期IOL植入。结果:术后最佳矫正视力>0.5者46眼,>0.3者25眼,>0.1者22眼,<0.1者17眼,26眼因年龄小无法进行视力表检查,但能够跟随光亮或有注视目标,较术前有提高。主要并发症有:后囊膜混浊、葡萄膜炎、角膜水肿、IOL偏中心及青光眼等,未见IOL夹持、黄斑囊样水肿、视网膜脱离及眼内炎等严重并发症。结论:白内障摘除、后囊连续环形撕囊、前部玻璃体切除联合IOL植入是治疗先天性白内障安全有效的手段,配合术后戴镜矫正和弱视治疗,可以使大多数先天性白内障致盲儿童达到脱盲及脱残。  相似文献

BACKGROUND: Cataract remains a challenge for ophthalmologists in uveitic eyes. The aim of this study is to report the clinical course of phacoemulsification with intraocular lens implantation in eyes suffering from uveitis. PATIENTS AND METHODS: Patients presenting a uveitis were prospectively followed from June 2001 to June 2003. Ocular surgery was performed according to a standard protocol, autoimmune follow-up visits were focused on the early detection of complications of uveitis: increased ocular inflammation, synechiae, retraction of the rhexis, opacification of the posterior capsule or onset of cystoid macular edema. RESULTS: Thirty-two eyes of 24 patients suffering from uveitis were operated with cataract surgery between June 2001 and June 2003. The mean age at surgery was 56 years (range 24 - 86 years). Mean preoperative visual acuity in uveitis patients presenting cataract was 0.3 +/- 0.3, and final visual acuity was 0.8 +/- 0.3. Three patients presented minor postoperative complications. One patient had a cystoid macular edema that appeared 5 months after surgery and one patient had a relapse of herpetic dendritic keratopathy despite topical antiviral therapy combined with steroid drops. The latter presented a slight increase of intraocular pressure (24 mm Hg). CONCLUSIONS: In patients with uveitis requiring cataract surgery, intraocular lens implantation is safe. Visual prognosis is better when pre- and postoperative inflammation is minimized. Macular scars or other retinal lesions are poor prognostic indicators.  相似文献

Vitrectomy for diabetic cystoid macular edema   总被引:6,自引:0,他引:6  
PURPOSE: We evaluated visual outcomes following vitrectomy for diabetic cystoid macular edema. METHODS: Visual outcomes and factors possibly influencing final visual acuity were assessed and documented retrospectively in 45 eyes of 40 patients, all of whom were followed up for at least 6 months postoperatively. RESULTS: Compared with the preoperative logarithm of the minimum angle of resolution (logMAR) visual acuity, final logMAR visual acuity improved 0.2 or more in 51% of the eyes, was unchanged in 47%, and decreased 0.2 or more in 2%. A final postoperative visual acuity of 0.5 or better was achieved in 38%. Preoperative visual acuity and the extent of the cystoid space on fluorescein angiography were significantly related to final visual acuity. A final postoperative visual acuity of 0.5 or better was noted in 8% of eyes with a preoperative visual acuity below 0.1, in 50% of eyes with a preoperative visual acuity of 0.1 or better, in 71% of eyes with a cystoid space smaller than 5 disc areas, and in 20% of eyes with a cystoid space of 5 disc areas or more. The state of the posterior vitreous membrane did not influence final visual acuity. There were no complications that decreased visual acuity. CONCLUSIONS: We conclude that diabetic cystoid macular edema is a good indication for vitrectomy, regardless of the state of the posterior vitreous membrane. A preoperative visual acuity of 0.1 or better and/or a cystoid space smaller than 5 disc areas may be indications for surgery aimed at achieving a final postoperative visual acuity of 0.5 or better.  相似文献

PURPOSE: To present preoperative strategies and results of cataract surgery in patients with different types of uveitis. PATIENTS AND METHODS: The results of 18 cases of extracapsular cataract extraction with or without intraocular lens implantation were retrospectively analysed. Age of patients ranged from 10 to 71 years (mean age: 35 years). Etiology of uveitis was determined in 10 cases (63%). Preoperative management was determined by the diagnosis of uveitis. The postoperative observation time ranged from 3 years to 6 months (mean: 25 months). RESULTS: Visual acuity improved postoperatively in 17 eyes (96%), and was better than 0.5 in 10 eyes (63%). The reasons for lower visual acuity were: cystoid macular edema in 6 cases (33%), band keratopathy, amblyopia and floaters in the vitreous. CONCLUSIONS: Establishing the diagnosis and controlling the inflammation pre- and postoperatively are the basic conditions for the surgical outcomes in cataract surgery in patients with history of uveitis.  相似文献

PURPOSE: To evaluate the results of a third macular hole surgery in eyes with recurrent macular holes and two prior macular hole surgeries. DESIGN: Retrospective consecutive noncomparative case series. PARTICIPANTS: Sixteen eyes of sixteen patients with two prior macular hole surgeries with recurrent macular hole. INTERVENTION: A third vitreous surgery was performed in each eye using a long-acting gas bubble. MAIN OUTCOME MEASURE: Closure of the macular hole and change in visual acuity. RESULTS: The macular hole was closed in 12 of 16 eyes (75%) at 3 months after the third surgery. Visual acuity improved 2 or more Snellen lines in 9 of 16 eyes (56%), and 5 of 16 eyes (31%) achieved 20/40 or better vision. Six eyes (37.5%) had cataract surgery after the third macular hole surgery, and visual acuity results were similar in eyes with or without cataract surgery. Successful closure of the macular hole improved the visual acuity from 20/80 -1 to 20/50 +1 (P < 0.001). Eyes in which one of the previous surgeries had been temporarily successful in closing the macular hole improved from a mean of 20/80 to 20/40 (P = 0.003). Eyes in which both prior macular hole surgeries had been primary failures had minimal benefit with a preoperative visual acuity of 20/100 +1 and a postoperative visual acuity of 20/100 +2 (P = 0.67). CONCLUSIONS: Repeat macular hole surgery should be considered in eyes with recurrent macular holes and two prior surgeries when the macular hole was temporarily closed by at least one of the two previous surgeries. Successful closure of a macular hole in such cases usually results in significant visual acuity improvement.  相似文献
