Phantomschmerzen sind ein in vielen chirurgischen Disziplinen bekanntes Symptom. Bei ophthalmologischen Patienten wurde das Auftreten von Phantomschmerzen nach Enukleation mit Ausnahme einer Pilotstudie [19] bislang nicht systematisch untersucht.

Unilaterale retinale Pseudoarteritis nach Contusio bulbi

This article reports an unusual case of unilateral frosted branch angiitis. Three major groups of this disease are known: secondary frosted branch angiitis due to viral infection or autoimmune diseases, frosted branch-like angiitis related to malignant diseases and the rare entity of idiopathic frosted branch angiitis. A 58-year-old patient presented with strictly unilateral sheathing and partly occluded retinal arteries, vitritis and macular edema and 4 months prior to presentation he suffered a contusion of the eye with traumatic cataract and vitreous hemorrhage followed by cataract extraction and vitrectomy. The general medical history revealed that the contralateral eye and biochemical analyses were unremarkable despite slightly elevated antinuclear antibody (ANA) levels. Under high dose steroid therapy the patient showed slow improvement in all clinical findings. This case does not fit into any of the known groups and can therefore be defined as pseudoarteritis.

Phthisis bulbi in retinoblastoma

Background: Phthisis bulbi is a relatively uncommon and atypical clinical presentation of retinoblastoma. Design: Retrospective study conducted at a tertiary care hospital. Participants: Eighteen consecutive retinoblastoma patients with primary phthisis bulbi. Methods: Retrospective analysis of clinical, imaging and histopathological features of all retinoblastoma patients with primary phthisis bulbi, treated at our centre between January 2005 and December 2009. Main outcome Measure: Clinical and histopathology features. Results: Eighteen (3.5%) retinoblastoma patients developed primary phthisis bulbi. The median age of presentation was 1.5 years. The median duration of symptoms before presentation was 6 months. In total, 15 out of 18 (83%) cases had bilateral disease. Among these, 80% (12/15) had advanced intraocular disease in the fellow eye. Most common first symptom was white reflex. History of orbital inflammation was present in 12/18 cases. Computed tomographic scan of orbit showed intraocular mass with calcific densities in 16 eyes. In two cases, hyperdense mass was seen without any calcification. On histopathology, residual viable tumour cells with characteristics of poorly differentiated retinoblastoma were found in 67% (12/18) eyes. High‐risk factors were present in six cases with microscopic residual disease in three cases. Conclusions: This is the largest case series of retinoblastoma patients with primary phthisis bulbi. Phthisis bulbi in retinoblastoma may be associated with bilateral disease in most cases and advanced intraocular disease in the fellow eye in a significant number of cases. Regression is incomplete in majority of these cases; therefore, enucleation must definitely be done in all cases of retinoblastoma presenting with phthisis bulbi.

A 21-year-old man presented in the emergency department with noticeable deterioration of vision and a feeling of pressure in the right eye. He stated that he had injured his eye when a shot was fired while cleaning a blank cartridge pistol. Ultrasound examination revealed no evidence of intravitreal hemorrhage or detachment of the retina. Examination by computed tomography did not reveal the presence of a foreign body. The severely reduced visual acuity could most likely be attributed to a traumatic cataract. An in-depth psychiatric examination of the anamnesis revealed that the patient had deliberately injured himself with the weapon. The severe traumatic eye injury was the result of an unrecognized hebephrenic schizophrenia.

本文对52例眼球铁质沉着征进行了回顾性研究，其主要临床表现有：虹膜异色、瞳孔散大、晶体前囊铁锈颗粒沉着、白内障形成、玻璃体液化及视网膜电流图异常等。其中大泡状角膜病变、瞳孔缩小、晶体全脱位、视乳头充血为作者首次报告。同时，对其发病机制进行了探讨。

It is reported about a 14 year old girl, sustaining an avulsion of the left eye-ball by a coat-rack. The optic nerve was cutted 18 mm retrobulbarly. In perimetry of the fellow-eye a temporo-superior defect was found as proof of traumatisation of the anterior-loop-fibers in the chiasm.
