OBJECTIVE: The objective was to compare agreement on the diagnosis of insulin resistance (IR) among insulin sensitivity indexes in both ovulatory women and those with polycystic ovary syndrome (PCOS). STUDY DESIGN: In an observational study, the 75-g oral glucose tolerance test was performed in 105 women with PCOS and 51 ovulatory women. The insulin sensitivity indexes used were insulin quantitative sensitivity check index (QUICKI), 1/homeostasis model assessment-insulin resistance (1/HOMA-IR), area under curve for insulin (AUC-I), and the Matsuda insulin sensitivity index (COMP). For the IR diagnosis we used cut-off values described in recent publications (insulin >12 microIU/ml, 1/HOMA-IR <0.47, QUICKI < or =0.333, AUC-I > or =7000 microIU/ml 120 min, and COMP <4.75. RESULTS: Excellent agreement was assessed among insulin, QUICKI, and 1/HOMA-IR. However, the rate of IR detected by these indexes in the PCOS group (44.8-51.4%) was lower than expected. New cut-offs were then determined based on COMP results. Using these values, 1/HOMA-IR and QUICKI showed excellent agreement (kappa=0.83) with COMP. CONCLUSION: The observed agreements among insulin, QUICKI and 1/HOMA-IR were higher than 93%. Therefore, clinicians may choose any of those obtaining similar results. For clinicians who prefer COMP, but are looking for a simpler test to detect IR in PCOS women, the use of QUICKI and 1/HOMA-IR with the new cut-offs seems reasonable.  相似文献

目的:探究抗苗勒管激素(AMH)与非肥胖型多囊卵巢综合征(PCOS)合并胰岛素抵抗(IR)的关系。方法:研究纳入102例非肥胖型PCOS患者为PCOS组,其中PCOS合并IR组(n=48),单纯PCOS组(n=54),50名非肥胖型健康人作为对照组。比较3组血清AMH水平以及AMH水平与PCOS合并IR的关系。结果:PCOS合并IR组血清AMH水平和HOMA-IR高于单纯PCOS组和对照组,差异有统计学意义(均P0.001)。PCOS组AMH水平与HOMAIR呈正相关(r=0.704,P0.001)。结论:非肥胖型PCOS患者合并IR血清AMH水平升高,AMH水平与PCOS合并IR相关。  相似文献

Objective

To analyze potential interactions of visfatin and retinol-binding protein 4 (RBP4) levels with body mass, metabolic, and hormonal status in normal weight and obese women with PCOS.

Study design

Body composition was determined by bioimpedance in 83 women (44 obese) diagnosed with PCOS and in 67 women (36 obese) without PCOS. In addition, serum glucose, lipids, androgens, FSH, LH, SHBG, insulin, visfatin, and RBP4 were measured in a fasting state and the free androgen index (FAI) was calculated, as was insulin resistance using the HOMA-IR assessment.

Results

Plasma RBP4 levels were significantly higher in women of normal weight compared to obese subjects when both were diagnosed with PCOS (14.1 ± 4.6 vs.10.9 ± 4.5 ng/mL, p < 0.001); while in non-PCOS subjects the opposite was found (10.8 ± 4.5 vs. 18.4 ± 11.6 ng/mL, p < 0.01; respectively). Plasma visfatin levels were similar in PCOS and non-PCOS subjects. In non-PCOS subjects, positive correlations between RBP4 level and anthropometric parameters were observed. In PCOS, RBP4 levels inversely correlated with serum insulin levels and HOMA-IR values. No correlation was found between plasma visfatin levels and anthropometric parameters in all study groups. Similarly, no correlation was found in PCOS and non-PCOS subgroups. Additionally, there was an inverse correlation between RBP4 and LH concentrations and LH/FSH ratio in all study subjects.

Conclusions

Plasma visfatin level is not a useful biomarker of insulin resistance and hyperandrogenism. RBP4 level reflects visceral body fat content in non-PCOS women. Decreasing RBP4 release along with increasing insulin resistance and hormonal disturbances may be a compensatory mechanism preventing deterioration in obese PCOS.  相似文献

It has been reported that polycystic ovary syndrome (PCOS) is very frequently associated with obesity, insulin resistance and hyperinsulinemia. However, metabolic disorders may lead to suppression of reproductive hormone secretion during undernutrition and in obesity. Some neuropeptides, such as neuropeptide Y (NPY) and galanin, modulate the control of appetite and play an important role in the mechanism of luteinizing hormone-releasing hormone (LHRH) secretion. NPY and galanin regulate appetite via both central and peripheral mechanisms. The interaction between central and peripheral signals for the control of food intake is due to leptin. Leptin can modulate the activity of NPY and other peptides in the hypothalamus that are known to affect eating behavior. In order to evaluate the relationship between NPY, galanin and leptin, 28 women with PCOS, 32 obese women (non-PCOS) and 19 lean healthy women (control group) were investigated. Obese women with PCOS were divided into two groups: PCOS (A) overweight (body mass index, BMI 26-30 kg/m2), and PCOS (B) obese (BMI 31-40 kg/m2). Plasma NPY, galanin and leptin concentrations were measured by radioimmunoassay. Plasma leptin levels in obese women with PCOS (groups A and B) were significantly higher than those in the control group (p < 0.05, p < 0.05, respectively). A significant positive correlation between plasma leptin and BMI in women with PCOS was found (r = 0.427, p < 0.01). A positive correlation was demonstrated between leptin and testosterone in PCOS (r = 0.461, p < 0.01). Plasma galanin concentrations in PCOS were higher than in the control group but the differences were not significant. Plasma NPY levels were significantly elevated in both non-obese (normal) and obese women with PCOS (group A) (p < 0.01, p < 0.005, respectively). However, in obese non-PCOS women plasma NPY levels gradually increased with increase in BMI. No significant correlations were found between galanin, NPY and percentage change in response of LH to LHRH, as well as between NPY and insulin, and galanin and testosterone. Plasma insulin concentrations in women with PCOS (group B) were significantly higher than in the control group (p < 0.001). Increased plasma NPY levels are found in both obese and non-obese women with PCOS. The increase in NPY is independent of the increase in BMI. In obese women with PCOS, plasma leptin is increased compared with control lean women. Serum insulin concentration is increased in obese women with PCOS. A positive correlation exists between leptin and BMI as well as between leptin and testosterone in women with PCOS. These results may suggest that the feedback system in the interaction between leptin and NPY is disturbed in PCOS.  相似文献

Abstract

The role of insulin resistance (IR) is well-documented in obese women with polycystic ovary syndrome (PCOS). Controversies exist concerning the presence of IR in idiopathic hirsutism (IH) or if it is a manifestation of high body mass index (BMI). We aimed to investigate the presence/absence of IR in lean hirsute women. One-hundred fifty-one lean women with hirsutism [96 PCOS (group 1) and 55 IH (group 2)] and 58 age-and BMI-matched healthy controls (group 3) were recruited in the study (mean age 25.21?±?6.1 versus 26.26?±?4.6years; BMI 21.79?±?1.7 versus 22.02?±?2.2?kg/m2, respectively). Significantly higher insulin and HOMA-IR, and significantly lower fasting glucose insulin ratio (FGIR), quantitative insulin sensitivity check index (QUICKI), reciprocal insulin, and Raynaud index were detected in groups 1 and 2 than in group 3 (p?<?0.05). These IR indices were similar between groups 1 and 2. The number of patients with IR (HOMA-IR?>?2, FGIR?<?7.2, or QUICKI?<?0.357) was significantly higher in groups 1 and 2 than in group 3, but was similar between groups 1 and 2. A higher frequency of IR occurs in lean hirsute women regardless of they having PCOS or IH. IR may contribute to aetiopathogenesis of IH, or may cause some metabolic abnormalities in these patients.  相似文献

目的探讨多囊卵巢综合征(PCOS)患者血清一氧化氮(NO)水平与其血管并发症的关系。方法采用硝酸还原酶法测定27例PCOS患者(PCOS组),21例因男方因素或输卵管因素不孕患者(对照组)血清NO水平,测定并计算体重指数(BMI)、腰臀比(WHR)、收缩压(SBP)、舒张压(DBP)、体脂含量、胰岛素抵抗指数(HOMA-IR)、胰岛素敏感指数(IAI)、血清性激素、空腹血糖(FPG)和胰岛素(FINS)水平。结果非肥胖PCOS组的HOMA-IR高于非肥胖对照组,IAI、血清NO水平低于非肥胖对照组,差异均有显著性(P〈0.05)。非肥胖PCOS组IAI明显高于肥胖PCOS组,差异有显著性(P〈0.01)。血清NO水平在PCOS组和其对照组间差异无显著性(P〉0.05)。PCOS组中,血清NO水平与HOMA-IR(r=-0.317,P〈0.05)、体脂含量(r=-0.346,P〈0.05)呈负相关。结论PCOS患者血清NO水平降低,提示PCOS患者可能存在血管内皮功能受损。  相似文献

目的:研究多囊卵巢综合征(PCOS)患者血清抗苗勒管激素(AMH)水平的变化及其与胰岛素抵抗(IR)和雄激素的关系。方法:回顾性分析85例PCOS患者及79例正常女性的体质量指数(BMI)、血清AMH、血糖、胰岛素、雄激素、稳态模型胰岛素抵抗指数(HOMA-IR)间的相互关系。血清AMH采用ELISA法测定,空腹血浆葡萄糖(FPG)采用葡萄糖氧化酶法测定,空腹胰岛素(FIns)采用化学发光法测定,睾酮(T)采用酶联免疫法测定。结果:PCOS组血清AMH水平明显高于对照组;Person相关分析表明,PCOS组血清AMH水平与HOMA-IR及BMI不相关,与雄激素、年龄相关。多元线性回归分析显示,PCOS组中血清雄激素水平是血清AMH的影响因素。结论:PCOS人群中血清AMH较正常女性升高,但其升高与IR无关,而与雄激素水平密切相关。  相似文献

Purpose  To investigate the correlation between serum 25-hydroxyvitamin D (25-OH-VD) concentrations and metabolic parameters in obese and non-obese women with polycystic ovary syndrome (PCOS). Methods  One hundred women with PCOS were divided into two groups, obese and non-obese, according to their body mass index (BMI). Waist-to-hip ratio (WHR), Ferriman–Gallwey score, homeostasis model assessment of insulin resistance (HOMA-IR), total cholesterol, triglycerides, calcium, 25-OH-VD, LH/FSH, total testosterone, and DHEAS were measured. Results  The serum 25-OH-VD mean levels were 56.31% lower in the obese PCOS patients. There was an association of increased HOMA-IR, BMI, WHR, triglycerides, total testosterone, and DHEAS with decreased 25-OH-VD concentrations in the obese PCOS patients. Conclusion  Low serum 25-OH-VD concentrations result from the presence of obesity and insulin resistance. However, the dependency between PCOS and hypovitaminosis D is questionable. Hypovitaminosis D should be kept in mind while managing obese women with PCOS.  相似文献

目的:比较肥胖与非肥胖多囊卵巢综合征(PCOS)患者血清肿瘤坏死因子-α(TNF-α)水平的差异。方法:55例PCOS患者,根据体质量指数(BMI)分为肥胖组(BMI>25,n=31)和非肥胖组(BMI≤25,n=24);同期选择50例非PCOS育龄妇女,分为肥胖对照组(BMI>25,n=25)和非肥胖对照组(BMI≤25,n=25)。应用酶联免疫吸附法(ELISA)测定血清TNF-α的含量,分析TNF-α与胰岛素抵抗指数(HOMA-IR)的相关性。结果:PCOS肥胖组与PCOS非肥胖组的TNF-α水平分别显著高于其相应的对照组(P<0.01),PCOS非肥胖组的TNF-α水平也显著高于肥胖对照组(P<0.05)。PCOS肥胖组与PCOS非肥胖组之间的TNF-α水平无显著差异(P>0.05)。PCOS组TNF-α与HOMA-IR呈显著正相关(P<0.05)。结论:肥胖与非肥胖PCOS患者的血清TNF-α水平均升高,可能存在肥胖以外升高TNF-α的途径;TNF-α与PCOS的IR发生有密切联系。  相似文献

Objective

Comparing oxytocin level and some other parameters between infertile women with or without polycystic Ovary Syndrome (PCOS), to evaluate the correlation between oxytocin with anti-mullerian hormone (AMH), Body Mass Index (BMI) and insulin resistance (IR).

Materials and methods

This cross–sectional study was performed on 80 PCOS and 81 non-PCOS women as the control group. Oxytocin, various hormones, Oral glucose tolerance test (OGTT) and Homeostatic model assessment of insulin resistance (HOMA-IR) were compared between two groups. Correlations between parameters were assessed by the spearman's rank correlation coefficient. Cutoff values for oxytocin and AMH in PCOS were calculated by the ROC-Curve and DeLong method.

Results

The mean oxytocin level was statistically lower in the case group (p ≤ 0.001). The mean BMI, AMH, HOMA-IR, fasting insulin and insulin 2-h after 75-g glucose were significantly higher in the PCOS group. Oxytocin was negatively correlated to AMH when evaluated for all participants or only among controls. Moreover oxytocin was negatively correlated to HOMA-IR among all participants. However the relationship between oxytocin and BMI was not statistically significant. The calculated cutoff value for oxytocin was 125 ng/L and for AMH was 3.6 ng/mL in the PCOS group.

Conclusion

The mean oxytocin level in the PCOS infertile women was lower than non-PCOS women. Oxytocin showed a significant reverse correlation with AMH and HOMA-IR.  相似文献

ObjectivesTo study the value of measuring ghrelin – a new marker for obesity – in Egyptian women with and without polycystic ovarian syndrome.DesignCross sectional study.SettingKasr El-Aini Hospital, Cairo University.Materials and methodsThe study was conducted at Kasr Al-Aini Hospital, Faculty of medicine, Cairo University from February 2008 to January 2009. Sixty eight Egyptian females were divided into two groups according to their body mass index (BMI): average weight (N=20) and obese (N=48). This group was further subdivided into 2 subgroups (non-PCOS) group and PCOS group. Fasting leptin, ghrelin, insulin, glucose, lipid, urea and creatinine concentrations were determined. HOMA-IR was calculated.ResultsGhrelin concentration decreased in obese women compared to normal weight subjects. Ghrelin levels were negatively correlated with BMI (r=0.759), insulin (r=?0.319), HOMA-IR (r=?0.372). Leptin levels showed a positive correlation with BMI, insulin, HOMA-IR (r=0.723, 0.284, 0.345, respectively). Glucose and insulin levels were significantly higher in the obese group compared to controls. To estimate the value of using ghrelin in predicting PCOS, a receiver operating characteristic curve (ROC) was plotted showing a sensitivity of 70% and specificity of 86% with a cut-off value of 34.1.ConclusionsGhrelin is a new marker that may be used for diagnosis of PCOS. Available data demonstrates a significant inverse relationship between ghrelin and leptin levels in obese women.  相似文献

The study aimed to investigate insulin resistance (IR) status in polycystic ovary syndrome (PCOS) women with normal glucose tolerance (NGT), and further to evaluate feasible diagnostic method for those patients. Three hundred and twenty-five PCOS women with NGT and ninety-five healthy age-matched controls were recruited with Rotterdam criterion and 75 g oral glucose tolerance test (OGTT). IR status was estimated following a glycemic and insulinemic OGTT (0, 30, 60, 120, and 180 min). A modified HOMA-IR formula was applied to each time-course value of glycemia and insulinemia. 目的探讨多囊卵巢综合征(PCOS)患者脂肪组织胰岛素受体底物1(IRS1)的蛋白表达及酪氨酸磷酸化在PCOS发病中的作用。方法采用免疫沉淀法、Western印迹法和增强化学发光蛋白免疫印迹法及图像分析半定量检测24例PCOS患者[PCOS组,根据体重指数分为肥胖(体重指数≥24kg/m2)和非肥胖(体重指数<24kg/m2)者各12例]及同期因卵巢囊肿或输卵管阻塞行开腹手术的非PCOS患者24例(对照组,根据体重指数分为肥胖和非肥胖者各12例)的脂肪组织中IRS1的蛋白表达及酪氨酸磷酸化程度。结果PCOS组肥胖者IRS1的蛋白表达为(82±15)%,PCOS组非肥胖者为(79±18)%;对照组肥胖者为(75±19)%,对照组非肥胖者为(70±19)%,各组间比较,差异均无统计学意义(P>005)。脂肪组织中IRS1的酪氨酸磷酸化程度在PCOS组肥胖者为(52±23)%,对照组非肥胖者为(88±12)%,两者比较,差异有统计学意义(P<001);对照组肥胖者为(45±22)%,PCOS组非肥胖者为(70±25)%,与对照组非肥胖者比较,明显降低,差异均有统计学意义(P<001,P<005);PCOS组肥胖者和对照组肥胖者间以及PCOS组肥胖者和PCOS组非肥胖者间比较,差异均无统计学意义(P>005)。结论PCOS组脂肪组织IRS1的蛋白表达及酪氨酸磷酸化程度明显减弱,可能参与胰岛素受体后信号传导抑制,并与PCOS胰岛素抵抗的发生有  相似文献

目的:探讨血清脂肪细胞因子抵抗素(resistin)、C-反应蛋白(CRP)、白细胞介素6(IL-6)水平与PCOS发生的相关性。方法:收集PCOS患者45例,再根据体质量指数(BMI)分为肥胖亚组(≥25kg/m2,22例)和非肥胖亚组(<25kg/m2,23例)。正常对照组45例,同样按BMI分为肥胖亚组(14例)和非肥胖亚组(31例)。空腹采集血清,采用酶联免疫分析法测定抵抗素、免疫比浊法测定CRP、放射免疫法测定IL-6,全自动生化分析仪测定血糖、血脂、化学发光法测定内分泌水平和血清胰岛素水平,同时测量身高、体质量、腰围、臀围,计算BMI和腰臀比值(WHR)。结果:与对照组非肥胖者相比,PCOS组肥胖、非肥胖者及对照组肥胖者抵抗素水平均显著增高(P<0.05);PCOS组和对照组肥胖者的CRP水平均高于对照组非肥胖者(P<0.05);PCOS组肥胖和非肥胖者的IL-6水平高于对照组非肥胖者(P<0.05)。抵抗素和CRP均与BMI、WHR、HOMA-IR呈显著正相关(P<0.05);IL-6与BMI、WHR有显著相关性,与HOMA-IR无相关性。结论:脂肪细胞因子抵抗素、CRP及IL-6参与PCOS患者肥胖和胰岛素抵抗的发生发展。  相似文献

目的探讨纤溶酶原激活物抑制因子1(PAI-1)基因启动子区4G及5G的多态基因型分布,及其与多囊卵巢综合征(PCOS)发病的关系。方法应用聚合酶链反应-限制性片段长度多态性技术,检测101例PCOS患者(PCOS组)与42例因男性或单纯输卵管因素不孕患者(对照组)PAI-1基因启动子区4G基因型及5G多态基因型分布,测定并计算两组患者的体重指数、胰岛素抵抗指数、胰岛素敏感指数及自然流产数等临床指标。根据体重指数将PCOS患者分为肥胖者(48例)与非肥胖者(53例)。结果PCOS组PAI-1基因启动子区4G基因型频率为57%(58/101),对照组为38%(16/42);PCOS组5G基因型频率为43%(43/101),对照组为62%(26/42)。两组4G及5G基因型频率分别比较,差异均有统计学意义(P〈0.05)。肥胖者胰岛素抵抗指数、胰岛素敏感指数与非肥胖者比较,差异均有统计学意义(P〈0.05,P〈0.01)。肥胖者4G基因型频率为48%(23/48),非肥胖者为68%(36/53);肥胖者5G基因型频率为52%(25/48),非肥胖者为32%(17/53)。两者4G及5C基因型频率分别比较,差异均有统计学意义(P〈0.05)。27例有妊娠史的PCOS患者中,自然流产14例,4G基因型频率为79%(11/14),5G基因型频率为21%(3/14);无自然流产13例,4G基因型频率为38%(5/13),5G基因型频率为62%(8/13),两者4G与5G基因型频率分别比较,差异均有统计学意义(P〈0.05)。结论PAI-1基因启动子区4G基因型频率高,可能与PCOS的发病,尤其是与非肥胖者发生PCOS有关,并与PCOS患者的自然流产率高有关。  相似文献

This study was performed to assess the impact of lipid accumulation product (LAP) and visceral adiposity index (VAI) on clinical, hormonal, and metabolic parameters in lean women with PCOS. Retrospective analysis of 120 consecutive lean PCOS subjects was performed. Subjects were divided into two groups according to HOMA-IR, as IR?+?and IR?. A HOMA-IR value above 2.5 was used to indicate IR. Clinical, hormonal and metabolic parameters were compared between the two groups. Correlations between LAP and VAI and clinical, hormonal, metabolic features in women PCOS were analyzed. One hundred twenty lean PCOS subjects were enrolled, of which 39 were insulin resistant. Comparison of group means showed significantly higher values for TG levels, FAI, FGS, TG/HDL-c, TyG, LAP, and VAI indexes and lower values for glucose/insulin ratio and QUICKI in the IR?+?group. LAP and VAI were both found to be positively correlated with each other and with WC, FAI, FGS, TG, TC levels, lipid ratios, TyG index, and HOMA-IR and negatively correlated with Glucose/Insulin ratio, QUICKI, and HDL-c in lean women with PCOS. LAP and VAI may be promising in early identification of IR and cardiometabolic risk and may be useful for the assessment of hyperandrogenism in lean women with PCOS.  相似文献

Polycystic ovary syndrome (PCOS) is commonly associated with insulin resistance, obesity, dyslipidemia and hypertension. Adiponectin, an adipocyte-specific protein with important roles in glucose and lipid homeostasis, possesses antidiabetic and insulin-sensitizing properties. Ghrelin, a protein ligand for the growth hormone secretagog receptor, has been shown to stimulate food intake and to influence energy balance, insulin signaling and glucose metabolism. We aimed to evaluate the relationships between metabolic alterations and adiponectin and ghrelin levels in lean PCOS women, compared with lean and obese women. The study was carried out on 20 non-obese PCOS women aged 20 – 48 years and age-matched groups of 45 healthy lean and 37 obese women. Hormonal and biochemical parameters, adiponectin and ghrelin concentrations and anthropometric data were determined. In PCOS subjects, we found increased homeostasis model assessment – insulin resistance index (HOMA-IR) with non-significant differences in adiponectin and ghrelin concentrations compared with healthy women, although the PCOS group showed a tendency to lower adiponectin levels. However, ghrelin levels in PCOS women were significantly higher than in obese women. Moreover, we observed a negative correlation between adiponectin and testosterone, cholesterol, triglycerides, glucose and diastolic blood pressure in PCOS. In conclusion, it can be suggested that higher values of HOMA-IR with lower adiponectin levels may indicate future development of metabolic syndrome or other metabolic disturbances in lean PCOS women.  相似文献

The objective of this study is to measure serum chemerin levels in women with polycystic ovary syndrome (PCOS) and assess their relationship with clinical, metabolic, and hormonal parameters. One hundred eighteen PCOS women and 114 healthy women were recruited in this study. Their blood pressure, body mass index (BMI), waist circumference and waist-to-hip ratio (WHR), fasting insulin (FIN), fasting plasma glucose (FPG), blood serum sex hormone, and blood lipid were measured. Serum chemerin, leptin, and adiponectin were measured by ELISA. Nesfatin-1, encoded by the nucleobindin-2 (NUCB2) gene, is an anorexigenic protein related to energy metabolism, obesity, and insulin resistance. The aim of this study was to evaluate NUCB2 gene polymorphism (rs757081) and its association with serum levels of nesfatin-1 in obese and non-obese women with polycystic ovary syndrome (PCOS). In the study population, we analyzed 60 patients with PCOS and 26 age-matched healthy women as controls. The patients with PCOS were divided into two groups based on body mass index (BMI): obese group (n?=?28) or non-obese group (n?=?32). NUCB2 was genotyped using the polymerase chain reaction-restriction (PCR) technique. Serum nesfatin-1 level was measured by enzyme-linked immunosorbent assay (ELISA). Nesfatin-1 levels in the obese PCOS group were significantly lower than those in the non-obese PCOS and control groups (p?p?>?0.05), whereas nesfatin-1 levels in the CC and CG genotypes were lower than those in the GG genotype. Nesfatin-1 decreases in PCOS, especially in obese women, and is negatively correlated with cardiometabolic risk factors. Although genotype disturbances of NUCB2 were similar among the groups, CC and CG genotypes accompanied lower nesfatin-1 levels.
