目的 研究风湿性心脏病持续性心房颤动(房颤)应用厄贝沙坦联合胺碘酮的窦性心律(窦律)维持作用及复发的危险因素.方法 选择住院准备房颤复律且符合入选标准风湿性心脏病(风心病)瓣膜置换术后持续性房颤患者63例.随机分为对照组(31例)和试验组(32例).对照组给予胺碘酮,试验组用胺碘酮+厄贝沙坦.入选患者转复为窦律后即为试验起始时间,试验终点为转复后12个月.终点事件:症状或无症状房颤首次复发.结果 试验组窦律维持率显著高于对照组(68.7%与41.9%,P<0.05).治疗12个月后,试验组左心房内径(LAD)显著小于对照组[(48.6±4.6)mm与(51.5±4.2)mm,P<0.05].风心病持续性房颤复发与LAD(OR 1.242)和是否使用厄贝沙坦(OR 0.226)有关.结论 LAD是风心病持续性房颤复发的危险因素.厄贝沙坦联合胺碘酮在风心病持续性房颤复律后维持窦律的疗效优于单用胺碘酮,并能延缓左心房扩大,防止房颤复发.  相似文献

目的 了解氯沙坦联合胺碘酮对阵发性心房颤动的复律效果及复律后窦性心律维持的影响.方法 2003年1月至2005年10月将解放军421医院心内科86例非瓣膜病阵发性心房颤动患者分为胺碘酮治疗组和氯沙坦 胺碘酮治疗组,观察治疗24 h,3 d和7 d时心房颤动的转复情况.在心房颤动复律后,继续药物治疗并随访观察1年,评价两组窦性心律的维持效果.结果 胺碘酮组44例心房颤动患者治疗24 h,3 d和7 d心房颤动的转复率分别为65.90%,75.00%和86.36%,氯沙坦 胺碘酮治疗组的转复率为66.66%,80.95%和95.23%.两组在7 d时心房颤动的转复率差异有显著性意义(P<0.05).随访1年时两组窦性心律的维持率分别为71.05%和87.50%(P<0.05),两组左房内径分别为(37.45±1.44)mm和(35.83±1.38)mm(P<0.05).结论 氯沙坦联合胺碘酮对阵发性心房颤动的复律及复律后窦性心律维持均优于单用胺碘酮治疗,可能与氯沙坦抑制肾素-血管紧张素系统,降低心脏负荷,抑制心房电及结构重构有关.  相似文献

目的探讨电复律治疗风湿性心脏病瓣膜置换术后伴左房明显增大的持续性心房颤动的疗效。方法将164例风湿性心脏病瓣膜置换术后持续性心房颤动的患者(左房内径均〉50mm)随机分为3组:胺碘酮组20例;胺碘酮+雷米普利组76例;胺碘酮+厄贝沙坦组68例。所有患者在经静脉应用胺碘酮后,房颤如未转复,则行电复律治疗。复律成功者胺碘酮改为口服200mg/d,联合雷米普利及厄贝沙坦组同时口服雷米普利5mg/d、厄贝沙坦150mg/d,3~6个月后停用。结果即时成功率92.7%(152/164例)。平均随访(1.8±0.4)年,128例(78.0%)保持窦性心律。联合雷米普利组窦律维持率为86.8%(66/76例),联合厄贝沙坦组窦律维持率为75.0%(51/68例)无统计学差异,单独口服胺碘酮组55.0%(11/20例)维持窦性心律,与联合雷米普利组、厄贝沙坦组比较,有显著差异。末次随访,胺碘酮组左房内径较复律前明显增加[(60.5±3.8)mm vs(57.7±4.5)mm;P=0.04];联合雷米普利组[(58.2±4.3)mm vs(57.3±5.8)mm,P=0.283和联合厄贝沙坦组[(57.2.±5.5)mm vs(56.4±4.9)mm,P=0.373前后对照无显著差异。三组患者心功能均改善,两两比较无显著差异。结论对于房颤时间长,左房增大的患者只要正确掌握电复律的指征及方法,并予以辅助药物维持治疗,电复律的成功率较高,转复后维持率亦高,并能改善患者心功能。胺碘酮联合雷米普利或厄贝沙坦能延缓左房增大,提高窦律维持率。  相似文献

目的观察培垛普利和厄贝沙坦分别与胺碘酮联合运用在持续性心房颤动复律中的作用。方法持续性心房颤动患者216例,按入院顺序随机分成A(胺碘酮组72例)、B(胺碘酮+培垛普利组72例)、c(胺碘酮+厄贝沙坦组72例)三组。A组抗凝3周后给予胺碘酮静脉450mg负荷,同时口服胺碘酮0.2gTid×7d,0.2gBid×7d,随后0.2gqd。B组开始抗凝3周时给予培垛普利4mgqd,C组开始抗凝3周时给予厄贝沙坦150mgqd,3周后加用胺碘酮复律(剂量和服法同A组)。4周后观察各组转复为窦性心律的例数和已复律者6个月时窦性心律维持情况。结果厄贝沙坦和培垛普利+胺碘酮组较仅用胺碘酮组转复成功率(72.86%、70.42%比54.29%,P〈0.01)及中期(6个月)维持窦性的情况均优于单用胺碘酮组(90.00%、84.31%比68.42%,P〈0.05)。结论复律前短期服用厄贝沙坦或培垛普利均能提高胺碘酮对持续性房颤复律效果,中期使用厄贝沙坦和培垛普利能防止房颤的复发。  相似文献

AIMS: Amiodarone and verapamil have been employed to treat immediate recurrences of AF (IRAF) after cardioversion. This study compares the efficacy of these agents for the treatment of IRAF. METHODS AND RESULTS: One hundred and eighty-five patients underwent transthoracic cardioversion (CV) for AF. AF recurred within 10 min in 20 patients (10.8%). These patients were randomized to verapamil (seven patients), or amiodarone (13 patients). After administration of verapamil and repeat CV, five patients (71%) experienced IRAF, compared with seven patients (54%) receiving amiodarone (P = 0.4). Including the results after crossover, IRAF occurred in 8/10 patients (80%) who received verapamil, compared with 7/15 patients (47%) who received amiodarone (P = 0.1). The combination of these agents prevented IRAF in 10/20 patients (50%). After a follow-up of 319+/-189 days, 42% of the IRAF patients treated with verapamil and/or amiodarone remained in sinus rhythm, which did not differ from patients without IRAF (53%, P = 0.7). CONCLUSIONS: IRAF occurs in 10% of patients undergoing CV. Amiodarone and verapamil are effective in preventing IRAF and result in a sinus rhythm maintenance rate of 50%. Since there is no difference in the long-term maintenance of sinus rhythm between patients with and without IRAF, attempts to restore sinus rhythm after pharmacological pretreatment are justified.  相似文献

探讨胺碘酮在心房颤动 (简称房颤 )复律和预防复发中的作用。 10年间共 10 8例房颤患者给予胺碘酮负荷剂量治疗共 10天进行房颤复律 ,9例房颤 (8.3% )持续 2周以内的患者自动复律 ,99例房颤再进行改良的胸背前后位贴片法电复律 ,94例 (94.9% )复律成功 ,复律成功能量为 78.1± 40 .6J。复律成功的 10 3例给予胺碘酮 0 .2 ,每日一次 ,预防复发 ,其后 1个月 ,0 .5年 ,1年 ,2年 ,维持窦律者分别为 98.1% ,88.3% ,71.8% ,5 6 .3% ;而早期用奎尼丁标准法复律共 36例 ,2 0例 (5 5 .6 % )自动复律 ;另 16例再行电复律 ,12例 (75 .0 % )复律成功 ,复律后在相应随访时间维持窦律者分别为 6 5 .0 % ,2 0 .0 % ,0 % ,0 %。结论 :胺碘酮药物复律房颤成功率低于奎尼丁 ,而预防复发却远优于奎尼丁 ;低能电复律法辅助胺碘酮复律是房颤复律的良好选择。慢性房颤心功能良好可用胺碘酮复律并预防复发。  相似文献

BACKGROUND: It has been previously reported that the efficacy of class I antiarrhythmics in preventing the recurrence of symptomatic paroxysmal and persistent atrial fibrillation (AF) is limited when AF lasts for 48 h or more. However, it is unclear whether the efficacy of amiodarone, a class III drug, is superior to class I antiarrhythmics in patients with long-lasting AF. METHOD AND RESULTS: The relationship between the duration of tachycardia and the efficacy of amiodarone in preventing recurrence of tachycardia was examined in 55 patients (37 men, 18 women, mean age 68+/-9 years) to whom amiodarone was administered after electrical or pharmacological cardioversion for paroxysmal and persistent AF. In 26 patients, paroxysmal and persistent AF ceased within 48 h after onset (Group A), and in the other 29 patients, it ceased after 48 h (Group B). Patient characteristics and actuarial recurrence-free rates were compared between the 2 groups. The mean follow-up period was 30+/-11 months. No statistically significant difference between the groups was found in patient characteristics. Actuarial recurrence-free rates in Group A and B at 1, 3, 6, 9, and 12 months were 100%, 81%, 69%, 62%, and 54%, and 93%, 79%, 66%, 52%, and 48%, respectively (p=NS at 12 months). The period of maintenance of sinus rhythm was 14.7+/-3.2 months in group A and 13.3+/-3.3 months in group B (mean+/-SE, p=NS). CONCLUSION: In the case of amiodarone, efficacy for maintaining sinus rhythm after cardioversion of AF was not biased by the duration of arrhythmia. This observation suggests amiodarone is effective in maintaining normal sinus rhythm after cardioversion, even in patients with long-lasting AF and electrical atrial remodeling.  相似文献

BACKGROUND: Conversion to sinus rhythm (SR) is rarely attempted in patients with rheumatic atrial fibrillation (AF) because the length of AF duration and the dilation of left atrium (LA) make maintenance of SR difficult. In this study, predictors of the successful maintenance of SR with amiodarone and electrical cardioversion in rheumatic AF patients receiving percutaneous transluminal mitral valvuloplasty (PTMV) were identified. METHODS AND RESULTS: This study included 23 consecutive patients undergoing PTMV for rheumatic AF (6 men, 53+/-11 years; AF duration 25 +/-24 months; LA diameter 44+/-6 mm; mitral valve area (MV) 1.1+/-0.2 cm(2)). Electrical cardioversion was required for the successful conversion to SR in all patients regardless of whether they had received amiodarone (400 mg/day) 2 months before PTMV (n=8) or 2 months after (n=15). After cardioversion, all patients received amiodarone 200 mg/day. With a follow-up period of 35+/-8 months, 14 patients (61%) remained in SR. A greater reduction in LA size (-4+/-3 mm vs 1+/-1 mm; p=0.004) and an greater increase in MV area (0.8+/-0.4 cm(2) vs 0.5+/-0.2 cm(2); p=0.01) by PTMV, not AF duration, were found to be the independent predictors for patients with successful maintenance of SR as compared with patients with recurrence of AF. CONCLUSION: In rheumatic AF patients receiving PTMV, the successful maintenance of SR with amiodarone and electrical cardioversion can be predicted by the degree to which LA size is reduced and MV area is increased.  相似文献

目的:探讨小剂量胺碘酮联合缬沙坦或贝那普利预防老年持续性房颤转复后房颤复发的作用及其机制。方法:150例持续性房颤患者(持续超过7d),经药物或电复律后随机分为三组:胺碘酮组(Ⅰ组),胺碘酮+缬沙坦组(Ⅱ组),胺碘酮+贝那普利组(Ⅲ组),各50例;治疗随访时间为1年,比较三组治疗后房颤的复发情况以及治疗前,治疗后第2周、3月、6月、12月的左心房内径。结果:142例完成治疗,随访12月,与I组比较,Ⅱ组和Ⅲ组心房颤动复发率(37.50%比17.39%比18.75%)明显降低,左房内径[(38.11±1.24)mm比(34.11±1.37)mm比(34.13±1.36)mm]明显缩小(P均〈0.05),而Ⅱ组和Ⅲ组间差异无显著性(P均〉0.05)。结论:胺碘酮分别与缬沙坦和贝那普利联合用于持续性房颤复律后预防房颤复发,较单用胺碘酮有效,长期服用可逆转左房扩大,抑制电重构,从而防止房颤复发。  相似文献

AIMS: To evaluate the impact, on atrial fibrillation (AF) recurrences, of verapamil addition to a class IC or III antiarrhythmic drug in patients, with persistent AF, who underwent an electrical cardioversion (EC). METHODS AND RESULTS: Three hundred sixty-three patients were randomized to receive four different pre-treatment protocols: oral amiodarone (group A), oral flecainide (group F), oral amiodarone plus oral verapamil (group A+V), oral flecainide plus oral verapamil (group F+V). Patients who showed an AF recurrence within 3 months were assigned to the alternative group and underwent a second EC after 48h. During 3 months of follow-up, 89 patients (27.5%) had an AF recurrence. By univariate analysis, verapamil reduced AF recurrences if added to amiodarone or flecainide (from 35% to 20%, P=0.004). Applying Cox proportional hazards regression model, only the younger age, the shorter duration of AF, and the use of verapamil were predictor of maintenance of sinus rhythm after cardioversion. In patients with primary AF recurrence, verapamil addition to group A and F patients, significantly decreased secondary AF recurrence rate as compared to group A+V and F+V patients who stopped the verapamil therapy (68% vs 88%, P=0.03). CONCLUSIONS: The addition of verapamil to class IC or III antiarrhythmic drug significantly reduced the AF recurrences, that were more frequent in older patients and in patients with longer lasting AF; moreover, verapamil was effective in reducing the secondary AF recurrences, too.  相似文献

AIMS: Direct current cardioversion of persistent atrial fibrillation is one of the most widely used and effective treatments for the restoration of sinus rhythm, but may be hampered by a low success rate and a high percentage of early recurrence. Pre-treatment with amiodarone or a glucose-insulin-potassium solution could improve the efficacy of electrical cardioversion by reversing the partially depolarized diastolic potential of the subsidiary pacemakers in atrial fibrillation. In a controlled randomized study, we assessed the effectiveness of electrical cardioversion in patients with persistent atrial fibrillation after pre-treatment with amiodarone or potassium infusion and the efficacy of amiodarone in maintaining sinus rhythm after electrical cardioversion. METHODS AND RESULTS: Ninety-two patients with persistent atrial fibrillation (>2 weeks duration) were prospectively randomized into three matched groups: A (n=31, oral amiodarone 400 mg. day(-1)1 month before and 200 mg. day(-1)2 months after cardioversion), B (n=31, 180 mg. day(-1)oral diltiazem 1 month before and 2 months after cardioversion and 80 mmol potassium, 50 UI insulin in 500 ml 30% glucose solution 24 h before cardioversion) and C (n=30, control patients, 180 mg. day(-1)oral diltiazem 1 month before and 2 months after cardioversion). Before cardioversion all patients were under 4 weeks effective oral anticoagulant therapy (warfarin). Before electrical cardioversion, the rate of spontaneous conversion to sinus rhythm was higher in group A (25%) than groups B (6%) or C (3%) (P<0.005). Electrical cardioversion was more successful in group A (88%) than groups B (56%) or C (65%) (P<0.05), while the electrical thresholds for effective cardioversion were lower in group B than the other groups (P<0.05). Twenty-four hours after cardioversion, the early recurrence of atrial fibrillation was similar in the three groups (P=ns), while at 2 months the recurrence rate was lower in group A (32%) than groups B (56%) or C (52%) (P<0.01). CONCLUSION: Pre-treatment with low-dose oral amiodarone, compared with oral diltiazem or glucose-insulin-potassium treatments, induces a significantly high percentage of instances of spontaneous conversion, increases electrical cardioversion efficacy and reduces atrial fibrillation recurrence.  相似文献

BACKGROUND: The relation between heart rate variability (HRV) and occurrence of atrial fibrillation (AF) in paroxysmal AF has been well studied, but there are controversial observations regarding the relation of HRV parameters to the recurrence of chronic AF after cardioversion. HYPOTHESIS: The present study compared HRV parameters of patients with chronic AF on the second day of cardioversion with a healthy control group and investigated their predictive value for AF recurrence. METHODS: Forty-one patients with chronic AF (> 3 months), who had various underlying cardiovascular disorders, were enrolled to the study. Of these, 31 patients were successfully cardioverted by external direct current shock, but 27 patients fulfilled the entry criteria. Twenty healthy subjects served as a control group. On the second day of restoration of sinus rhythm, 24-h Holter recording was obtained and the following time-domain indices of HRV were measured: SDNN (the standard deviation of the mean RR interval expressed in ms), SDANN (the SD of the averages of RR intervals in all 5-min segments of the 24-h recording), rMSSD (the root mean square of differences of successive RR intervals), and pNN50 (the percentage of adjacent RR intervals that differed by more than 50 ms). Patients were followed-up for 6 weeks for recurrence of AF. RESULTS: After cardioversion, SDNN and SDANN were found to be significantly lower in the AF group than in the control group (86.4 +/- 31.7 ms vs. 142.1 +/- 40.2 ms, and 57 +/- 17.4 ms vs. 124.4 +/- 37.7 ms, p < 0.001 and p < 0.001, respectively). The indices of vagal modulation of heart rate (rMSS and pNN50) were not different between the AF group and the control group. Recurrence of AF was observed in 15 patients. In these patients, all HRV parameters were significantly depressed compared with those with maintained sinus rhythm. Logistic regression analysis revealed that only decreased pNN50 was an independent predictor of AF relapse (relative risk = 1.5, p = 0.02, 95% confidence interval 1.1-2.2). There was also a trend toward a shortened SDNN as a predictor of AF recurrences. CONCLUSION: Suppressed HRV parameters and decreased vagal tone are probably a risk factor for AF recurrences after cardioversion to sinus rhythm in a specific subset of patients with chronic AF.  相似文献

BACKGROUND. Delivery of shocks within the right atrium has been reported to be more effective than conventional external shocks in converting atrial fibrillation (AF), but these two cardioversion techniques have never been compared prospectively. The purpose of this study was to compare the efficacies of external and internal cardioversion in patients with chronic AF unresponsive to prior attempts at electrical and/or pharmacological cardioversion. Low-dose amiodarone was used in all patients after cardioversion to suppress recurrences of AF. METHODS AND RESULTS. One hundred twelve patients with AF of at least 1 month in duration were randomly assigned to undergo external cardioversion with 300-360-J shocks or internal cardioversion with 200-300-J shocks delivered through a standard electrode catheter within the right atrium. The patients were treated with amiodarone (200 mg/day 5-7 days/week) for 1 month before electrical cardioversion and afterward if the cardioversion was successful. The patients were evaluated at regular intervals during 1 year of follow-up. The efficacy of internal cardioversion was significantly greater than that of external cardioversion (91% versus 67%, p = 0.002). The only variable that was associated with the outcome of cardioversion was body weight. Among patients in whom sinus rhythm was restored, AF recurred as often after internal and external cardioversion; at 1 year of follow-up, 37% of patients in whom external or internal cardioversion had been effective were still in sinus rhythm. Patients who had undergone an attempt at electrical cardioversion before entry into this study were less likely to remain in sinus rhythm after cardioversion. The only complications of cardioversion were one instance of cerebral thromboembolism after external cardioversion and one instance of transient pulmonary edema after internal cardioversion. Therapy with amiodarone was discontinued because of an adverse drug effect in only three patients. CONCLUSIONS. Internal cardioversion is more effective than external cardioversion in restoring sinus rhythm and is as safe as external cardioversion in patients with chronic AF. The recurrence rate of AF is the same after both types of cardioversion. If conventional electrical cardioversion is ineffective, internal cardioversion should be attempted. The combination of low-dose amiodarone and external or internal cardioversion may result in maintaining sinus rhythm long-term in patients with refractory AF.  相似文献

During segmental ostial ablation for pulmonary vein isolation, pulmonary vein potentials are easily identified during sinus rhythm or left atrial pacing. Therefore, maintenance of atrial fibrillation (AF) during the procedure is desirable. However, cardioversion is occasionally followed by an immediate recurrence of AF. This study compared the efficacy of ibutilide and amiodarone in preventing immediate recurrences of AF in patients who underwent pulmonary vein isolation. The subjects of this study were 25 patients (mean age 56 +/- 10 years) who underwent pulmonary vein isolation for AF who had an immediate recurrence of AF within 60 seconds after 2 transthoracic cardioversions. The patients were randomized to receive an infusion of either 300 mg of amiodarone over 10 minutes or 1 mg of ibutilide over 5 minutes. Cardioversion was repeated 15 minutes after the drug infusion. If immediate recurrences of AF occurred 2 more times, the alternative study drug was administered, and cardioversion was repeated. Immediate recurrences of AF were suppressed by amiodarone in 8 of 10 patients (80%), and by ibutilide in 9 of 15 patients (60%, p = 0.4). After crossover, immediate recurrence of AF was suppressed in 2 of 6 patients (33%) by amiodarone, and in 1 of 2 patients (50%) by ibutilide (p = 0.6). Ibutilide and amiodarone, when used alone or in combination, prevented immediate recurrences of AF in 20 of 25 patients (80%). There were no adverse drug effects. Ibutilide and amiodarone were equally effective in suppressing immediate recurrences of AF. Overall, immediate recurrences of AF can be prevented by amiodarone and/or ibutilide in 80% of patients.  相似文献

目的:观察电复律+胺碘酮维持治疗对持续性心房颤动(房颤)节律控制的效果及安全性。方法:回顾分析2007年8月至2010年12月在我院行心脏电复律+胺碘酮维持治疗的53例持续性房颤患者的资料,对复律≥2年的24例患者进行随访,其中维持窦性心律1.5年者16例(复律组),房颤复发者8例(复发组)。结果:53例有50例达到复律早期成功,早期成功率为94.3%,与电复律前比较,电复律后患者的总心室率[(109777±6757)次/min比(81083±5036)次/min]、平均心率[(81±8)次/min比(62±6)次/min]、最快心室率[(145±13)次/min比(123±11)次/min]、最慢心室率[(67±7)次/min比(45±6)次/min]明显减低(P〈0.05);与复律组比较,复发组的房颤持续时间[(4.36±1.47)月比(8.7±2.15)月]、左房内径[(35.85±2.07)mm比(43.15±1.95)mm]、年龄[(54.3±11.7)岁比(72.1±8.3)岁]均明显增大(P〈0.01~〈0.001)。所有病人无明显副作用。结论:电复律联合小剂量胺碘酮维持治疗对大部分持续性房颤患者有效、安全、简便,应该推广。  相似文献

The study was carried out to evaluate the effect of prophylactic single-dose intravenous amiodarone in patients undergoing valve replacement surgery. Maintenance of sinus rhythm is better than maintenance of fixed ventricular rate in atrial fibrillation (AF) especially in the presence of irritable left or right atrium because of enlargement. Fifty-six patients with valvular heart disease with or without AF were randomly divided into two groups. Group I or the amiodarone group (n=28) received amiodarone (3 mg/kg in 100 ml normal saline) and group II or the control group received same volume of normal saline. The standardized protocol for cardiopulmonary bypass was maintained for all the patients. AF occurred in 7.14% patients in group I, and in group II, 28.57% (P=0.035); ventricular tachycardia/fibrillation was observed in 21.43% patients in group I and 46.43% patients in group II (P=0.089) after release of aortic clamp. Most of the patients in group I (92.86%) maintained sinus rhythm without cardioversion or defibrillation after release of aortic cross clamp (P=0.002). Defibrillation or cardio version was needed in 7.14% patients in group I and 28.57% patients in group II (P=0.078). A single prophylactic intraoperative dose of intravenous amiodarone decreased post bypass arrhythmia in this study in comparison to the control group. Single dose of intraoperative amiodarone may be used to decrease postoperative arrhythmia in open heart surgery.  相似文献

目的观察缬沙坦联合胺碘酮对持续性心房颤动复律及复律后窦性心律维持率的影响。方法选择持续性心房颤动患者96例,随机分为两组,治疗组49例,给予缬沙坦及胺碘酮治疗,对照组47例给予安慰剂及胺碘酮治疗。随访1年,观察两组心房颤动转复率和转复后3、12个月时心房颤动复发率,以及转复时及转复后12个月左心房内径(LAD)、左心室射血分数(LVEF)变化。结果心房颤动转复率治疗组(87.8%)与对照组(85.1%)差异无统计学意义(P〉0.05)。心房颤动复发率转复后3个月及12个月治疗组(14.0%、2312%)均小于对照组(32.5%、54.5%)(P〈0.05或0.01),转律后12个月LAD治疗组[(44.3±065)mm)]较对照组[(469±0.54)mm]变小(P〈0+05),LVEF治疗组[(56.8±8.64)%]较对照组[(52.3±908)%]升高(P〈0.05)。结论缬沙坦联合胺碘酮治疗持续性心房颤动在减少心房颤动的复发率以及改善心功能方面优于单独使用胺碘酮。  相似文献

AIMS: Paroxysmal atrial fibrillation (AF) is usually preceded by a premature atrial complex (PAC). We hypothesized that patients with a high frequency of atrial ectopic activity after restoration of sinus rhythm following direct current cardioversion would be more likely to experience recurrence of AF. METHODS AND RESULTS: Forty-four patients with documented persistent AF were studied. A 24 h Holter recording was performed from the day of external direct current cardioversion. Patients were reviewed at 1 week, 1 month, and 6 months. After 6 months, 59% of patients had experienced a recurrence of AF. Neither the frequency of PACs nor the frequency or duration of supraventricular tachycardia (SVT) episodes predicted AF recurrence (P=0.60, 0.30, and 0.42, respectively). There was a trend towards maximum rate of SVT predicting recurrence of AF (P=0.08). CONCLUSION: Frequency of supraventricular ectopy or the number and length of SVT runs in the 24 h after restoration of sinus rhythm are not strong predictors of recurrence of AF after electrical cardioversion. A larger study would be required to detect a small predictive effect.  相似文献

目的:观察厄贝沙坦联合胺碘酮对阵发性心房颤动复律后维持窦性心律的临床效果及其对左心房内径的影响。方法:选择阵发性心房颤动患者89例,分为胺碘酮治疗组(44例,单纯服用胺碘酮),联合治疗组(45例,服用胺碘酮及厄贝沙坦),随访12个月,观察两组患者治疗前后窦性心律维持率,左房内径的变化,并进行比较分析。结果:治疗12个月后,与胺碘酮组比较,联合治疗组窦性心律维持率明显提高(64.3%比81.4%),左房内径明显减小[(40.12±10.6)mm比(34.10±10.11)mm],P均〈0.05。结论:厄贝沙坦联合胺碘酮对阵发性心房颤动复律后维持窦性心律的作用明显优于单用胺碘酮,且明显抑制左房扩大,无明显不良反应。  相似文献

BACKGROUND: The aim of this study was to evaluate pharmacological cardioversion of long-lasting persistent atrial fibrillation (AF) using bepridil in terms of recovery of atrial mechanical function and maintenance of sinus rhythm. Bepridil alone or in combination with aprindine is effective for termination of persistent AF. METHODS AND RESULTS: The study group comprised 38 consecutive patients (24 men, 58.8+/-9.3 years) with successful conversion of persistent AF lasting >1 month either pharmacologically (Group I) or electrically (Group II). Fast Fourier transform analysis of fibrillation waves was performed and fibrillation cycle length (FCL) was calculated from the peak frequency. In Group I, sinus rhythm was pharmacologically restored in 22 patients after an average 30 days (7-49 days) of bepridil administration, either alone (11) or in combination with oral aprindine (11); they were followed up while using the same drugs. In Group II, electrical conversion restored sinus rhythm in 16 patients, and they were followed up with conventional antiarrhythmic drugs other than bepridil and aprindine. After bepridil treatment FCL increased and became significantly longer in Group I than in Group II (190+/-39 vs 150+/-29 ms, p<0.001). Atrial peak velocity in transmitral flow within the first week after cardioversion was greater in Group I than in Group II (68+/-35 vs 32+/-20 cm/s, p<0.05). By Kaplan-Meier analysis, 83% of Group I patients were free of AF recurrence at the 12-month follow-up, compared with 36% in Group II (p<0.005). CONCLUSIONS: In patients with long-lasting AF, pharmacological conversion with bepridil alone or in combination with aprindine recovered atrial mechanical function better and maintained sinus rhythm longer than electrical conversion.  相似文献
