目的 探讨肠缺血 再灌流过程中肠局部血管内皮细胞的细胞间粘附分子 1(intercellu laradhesionmolecule 1,ICAM 1)mRNA和蛋白表达的变化与中性粒细胞聚集的关系及其在肠损伤中的作用。方法 采用肠系膜上动脉夹闭的方法制作大鼠肠缺血 再灌流模型 ,应用逆转录 聚合酶链反应及免疫组织化学的方法观察小肠ICAM 1表达的变化 ,同时检测组织髓过氧化物酶活性及反映肠功能的指标D 乳酸、二胺氧化酶活性的变化。结果 肠缺血 0 5~ 1 5h ,小肠ICAM 1mRNA表达增加 ,免疫组织化学显示其蛋白表达在肠缺血 1h及再灌流 1h时增多 ;肠组织髓过氧化物酶活性同时升高 (P <0 0 5 ) ,并持续至再灌流后 6h ;血浆二胺氧化酶活性在肠缺血 1h和再灌流后 2h升高 (P<0 0 5 ) ,血浆D 乳酸浓度在肠缺血 1 5h时开始增加 ,再灌流后 0 5h和 1h时大幅度升高 (P <0 0 1,P <0 0 5 )。结论 肠缺血 再灌流时肠血管内皮细胞ICAM 1表达增加 ,由此介导的中性粒细胞在局部聚集、活化可能是造成肠粘膜上皮损伤和肠通透性增加的病理生理学基础  相似文献

OBJECTIVE: To determine the kinetics of plasma D(-)-lactate levels in both portal and systemic circulations, and to examine whether elevated plasma D(-)-lactate would correlate to intestinal injury in rats subjected to acute intestinal ischemia-reperfusion. METHODS: Anesthetized rats underwent 75 minutes of superior mesenteric artery occlusion followed by 6 hours of reperfusion. Plasma D(-)-lactate levels were measured by an enzymatic spectrophotometric assay. RESULTS: It showed that intestinal ischemia for 75 minutes resulted in a significant elevation in D(-)-lactate levels in portal vein blood compared to baseline values (P < 0.05). Plasma D(-)-lactate levels had a tendency to further increase after reperfusion up to 6 hours. Similar alterations in D(-)-lactate were also found in systemic circulation, there were no significant differences between the portal and systemic circulation at any time point. Moreover, the histopathological evaluation scores were significantly correlated to the portal D(-)-lactate levels in animals at various time points (r = 0.415, P < 0.01). In addition, a remarkable rise of endotoxin concentration within portal vein was already found at the end of 75-minute ischemia (P < 0.05), reaching a peak at 2 hours post-reperfusion. CONCLUSION: These data suggest that acute intestinal ischemia is associated with failure of mucosal barrier resulting in increased plasma D(-)-lactate levels in both portal and systemic blood. The subsequent reperfusion might cause further increase in D(-)-lactate levels, which correlated to the histopathological alterations. Plasma D(-)-lactate may be a useful marker of intestinal injury following both ischemia and reperfusion insults.  相似文献

目的 评价缺血预处理.后处理对大鼠肠缺血再灌注损伤的影响.方法 清洁级成年雄性SD大鼠40只.体重225~275 g,随机分为5组(n=8):假手术组(S组)仅分离肠系膜上动脉(SMA),不夹闭;肠缺血再灌注组(IIR组)采用夹闭SMA 60 min,再灌注60 min的方法制备肠缺血再灌注损伤模型;缺血预处理组(IPr组)夹闭SMA 10 min,再灌注10 min,余同IIR组;缺血后处理组(IPo 组)夹闭SMA 60 min后,再灌注30 s,缺血30 s,反复3次,再灌注60 min;缺血预处理.后处理组(IPr-IPo组)先行缺血预处理,再行缺血后处理,操作过程同IPr组和IPo组.于再灌注60 min时各组取肠粘膜组织,观察肠粘膜形态并行Chiu评分,检测丙二醛(MDA)含量,超氧化物歧化酶(SOD)及髓过氧化物酶(MPO)活性,同时采集动脉血样检测血浆肿瘤坏死因子α(TNF-α)及白细胞介素6(IL-6)浓度.结果 与S组比较,其余各组Chiu评分、MDA含量、MPO活性、血浆TNF-α与IL-6浓度升高,SOD活性降低(P<0.05).与IIR组比较,IPr组、IPo组及IPr-IPo组Chiu评分、MDA含量、MPO活性、血浆TNF-α和IL-6浓度降低.SOD活性升高(P<0.01).与IPr组和IPo组比较,IPr-IPo组Chiu评分和MDA含量降低,SOD活性升高(P<0.05).IPr组与IPo组各指标比较差异无统计学意义(P>0.05).结论 缺血预处理-后处理可减轻大鼠肠缺血再灌注损伤,较单独应用时效果好.  相似文献

目的:研究肠缺血-再灌流过程中PMN聚集、活化在局部及远隔脏器损伤中的作用及机制.方法:检测大鼠肠缺血再灌流后不同时相全血和组织MPO活性、脏器功能、血浆LPS浓度,观察肠静脉血清对PMN上CD11b和CD18表达的影响.结果:全血和组织MPO活性在再灌流后0.5 h大幅度升高,再灌流后1 h,全血、小肠和肝组织MPO活性达峰值(P<0.05~P<0.01);肺组织MPO活性于再灌流2 h达峰值,再灌流6 h仍维持高水平(P<0.01);脏器功能指标的异常在再灌流后1~2 h最明显(P<0.05),并与相应组织MPO活性变化正相关.再灌流后肠静脉血清刺激PMN上CD11b和CD18表达增加,分别与肝、肺组织MPO活性变化呈正相关关系(r=0.9852,r=0.9874,P<0.02).结论:PMN的聚集、活化参与肠缺血-再灌流过程中局部及远隔组织的损伤过程;PMN上粘附分子表达上调是肠缺血-再灌流后PMN聚集、活化的分子机制之一.  相似文献

目的:观察缺血后处理减轻肠缺血再灌注引起的小肠及远隔脏器损伤的效果,并探讨其机制。方法将家兔48只随机分为假手术组、缺血再灌注组、缺血后处理组,每组16只。再灌注2 h 后采集各组动脉血、静脉血及部分肠道组织、肝、肺组织,测动脉血中 TNF-α、IL-1β、IL-6、IL-10水平,测静脉血中 ALT、AST、BUN,Cr、LDH、CK-MB 活性,测内毒素水平,测定血清及小肠、肝、肺组织 MDA、MPO、CAT、SOD 水平,HE 染色,观察肠黏膜损伤情况,细菌培养观察细菌易位率。结果与缺血再灌注组比较,缺血后处理组血清及小肠、肝、肺组织中 MDA、MPO 水平明显降低, SOD、CAT 水平明显升高,静脉血 ALT、AST、LDH、CK-MB、BUN 下降;动脉血中 TNF-α、IL-1β、IL-6、内毒素降低,IL-10水平升高,肠黏膜损伤评分明显降低。结论缺血后处理可以减轻肠黏膜损伤,减少内毒素易位,促进抗炎因子的激活,抑制炎性介质的过度释放,提升小肠组织及远隔脏器的氧自由基的抗氧化能力,减轻小肠及远隔脏器组织损伤。  相似文献

目的:观察细胞间黏附分子-1(intercellular adhesion molecule-1,ICAM-1)在大鼠肾缺血再灌注(ischemia/reperfusion,I/R)不同时间点表达的变化并与肾功能恶化程度做相关性分析,从而探讨ICAM-1在肾I/R损伤中所起的作用.方法:应用苦味酸法和二乙酰一肟反应法测定大鼠肾I/R不同时间点血肌酐和尿素氮值, HE染色光镜下观察石蜡包埋切片肾组织损伤的形态学改变,免疫组化法分析ICAM-1在肾I/R不同时间点表达的变化.结果:肾功能检测发现, 再灌注1 h后血肌酐和尿素氮值开始增高和正常对照组比较有统计学差异(P<0.05),12 h达峰值.HE染色光镜下观察肾组织细胞以近端肾小管变性、坏死为主,I/R各时间点肾组织病理损伤变化与肾功能变化规律相一致.再灌注后早期ICAM-1表达以内皮细胞为主,后期肾脏各部均可表达,小管上皮尤甚.肾脏表达的ICAM-1与肾功能恶化程度存在明显的正相关(P<0.01).结论:I/R早期肾脏即有ICAM-1表达,其表达量与肾功能恶化程度呈正相关,提示ICAM-1在肾I/R损伤发病机制中起促进作用.  相似文献

Methods Forty healthy male SD weighing 225-275 g were randomly assigned into 5 groups ( n = 8 each): group I sham operation (group S) ; group II intestinal IR (group IIR); group Ⅲ ischemic preconditioning (group Ipr); group IV ischemic postconditioning (group Ipo); group V Ipr+ Ipo. The rats were anesthetized with intraperitonel 20% urethane 5 ml/kg. Superior mesenteric artery (SMA) was occluded for 60 min followed by 60 min reperfusion. In group S, SMA was isolated but not occluded. In group Ipr, SMA was occluded for 10 min followed by 10 min reperfusion, and the rest procedures were performed using the method described in group IIR. In group Ipo, 60 min ischemia was followed by three 30 s episodes of ischemia at 30 s intervals for reperfusion. In group Ipr+ Ipo, Ipr was performed followed by Ipo and the procedures were performed using the methods described in group Ipr and Ipo. The animals were killed at 60 min of reperfusion. The intestinal tissues were immediately removed for determination of MDA content, SOD and MPO activities and the degree of damage to intestinal mucous membrane was scored according to Chiu score. Arterial blood samples were taken for determination of plasma concentrations of TNF-α and 1L-6. Results Compared with group S, Chiu score, MDA content, MPO activity, and plasma concentrations of TNF-α and IL-6 were significantly increased, whereas SOD activity decreased in the other 4 groups ( P < 0.05). Chiu score, MDA content, MPO activity, and plasma concentrations of TNF-α and IL-6 were significantly decreased, whereas SOD activity increased in group Ipr, Ipo and Ipr + Ipo as compared with group IIR ( P < 0.05). Chiu score and MDA content were significantly lower, whereas SOD activity higher in group Ipr + Ipo than in group Ipr and Ipo ( P < 0.05). No significant differences were detected in the indices between group Ipr and group Ipo ( P > 0.05). Methods Forty healthy male SD weighing 225-275 g were randomly assigned into 5 groups ( n = 8 each): group I sham operation (group S) ; group II intestinal IR (group IIR); group Ⅲ ischemic preconditioning (group Ipr); group IV ischemic postconditioning (group Ipo); group V Ipr+ Ipo. The rats were anesthetized with intraperitonel 20% urethane 5 ml/kg. Superior mesenteric artery (SMA) was occluded for 60 min followed by 60 min reperfusion. In group S, SMA was isolated but not occluded. In group Ipr, SMA was occluded for 10 min followed by 10 min reperfusion, and the rest procedures were performed using the method described in group IIR. In group Ipo, 60 min ischemia was followed by three 30 s episodes of ischemia at 30 s intervals for reperfusion. In group Ipr+ Ipo, Ipr was performed followed by Ipo and the procedures were performed using the methods described in group Ipr and Ipo. The animals were killed at 60 min of reperfusion. The intestinal tissues were immediately removed for determination of MDA content, SOD and MPO activities and the degree of damage to intestinal mucous membrane was scored according to Chiu score. Arterial blood samples were taken for determination of plasma concentrations of TNF-α and 1L-6. Results Compared with group S, Chiu score, MDA content, MPO activity, and plasma concentrations of TNF-α and IL-6 were significantly increased, whereas SOD activity decreased in the other 4 groups ( P < 0.05). Chiu score, MDA content, MPO activity, and plasma concentrations of TNF-α and IL-6 were significantly decreased, whereas SOD activity increased in group Ipr, Ipo and Ipr + Ipo as compared with group IIR ( P < 0.05). Chiu score and MDA content were significantly lower, whereas SOD activity higher in group Ipr + Ipo than in group Ipr and Ipo ( P < 0.05). No significant differences were detected in the indices between group Ipr and group Ipo ( P > 0.05). Conclusion Ischemic preconditioning-postconditioning can attenuate the intestinal IR injury in rats, and the efficacy is better than that of either Ipr or Ipo alone.  相似文献

目的 了解缺血预处理(IP)对小肠缺血的作用。方法 在不同缺血预处理的大鼠模型上,观察缺血再灌注后肠组织腺苷酸含量和肠粘膜损伤情况。结果 IP1组(预处理:10分钟缺血15分钟再灌注和IP2组(3分钟缺血5分钟再灌接以7分种10分钟再灌)小肠的ATP和总腺苷酸含量较对照组明显增高,肠粘膜损伤亦明显减轻(P〈0.05)。.结论 IP1和IP2组的缺血预处理对小肠缺知再灌注损伤有一定的保护作用。  相似文献
