An assessment of the plastic Thermafil obturation technique   总被引:1,自引:0,他引:1
Adaptation and placement of alpha-phase gutta-percha delivered with a plastic core-carrier, Thermafil, was compared to the lateral condensation of gutta-percha in a specific tooth model. Fifty-one mandibular molar roots with separate canals, patent canal orifices and curvatures greater than 15 degrees were cleaned and shaped with K-files and 2.5% sodium hypochlorite to a size 30 at the apex, and flared with Hedstrom flies to create a continuously tapering funnel preparation. Canals were randomly obturated with Sealapex root canal sealer and either alpha-phase gutta-percha on a plastic Thermafil carrier, or standard beta-phase gutta-percha with lateral condensation. Roots were radiographed from the proximal and evaluated by three examiners, based on established criteria for overall material adaptation, apical adaptation, and filling material extrusion. Thermafil provided a statistically significant better over-alt canal obturation (P < 0.001), while, in the apical third, both techniques were not significantly different (P > 0.05). When the apical orifice was patent there was a significant propensity for the extrusion of filling materials beyond the apex (P < 0.001) with the Thermafil technique.  相似文献

The aim of the present study was to evaluate the possibility of retreating teeth obturated with: (i) Thermafil with plastic carrier, (ii) Thermafil with metallic carrier and (iii) laterally condensed gutta-percha. The following aspects were analysed: removal of obturation, time required for removal, remaining filling material and apical extrusion during reinstrumentation. Thirty human teeth with a single canal were instrumented up to a size 45 K-type file, divided into three groups of 10 teeth each and obturated with Thermafil with plastic carrier (group 1), Thermafil with metallic carrier (group 2), and laterally condensed gutta-percha (group 3) with AH26 as the sealer. Reinstrumentation was performed manually after 30 days storage using Hedstroem files and xylene as a solvent. The average time needed to remove the obturation was 12 min 1 s for group 1, 14 min 36 s for group 2 and 11 min 26 s for group 3. Although no statistically significant differences were observed between groups 1 and 2, and 1 and 3 ( P > 0.05), the differences between groups 2 and 3 were significant ( P < 0.05). The amount of remaining filling material was significantly higher in group 2 when compared with groups 1 and 2 ( P < 0.01). Although the lowest incidence of apical extrusion during reinstrumentation was found in group 3, the differences among groups were not statistically significant.  相似文献

In vitro apical leakage of system B compared with other filling techniques   总被引:16,自引:0,他引:16
A fluid filtration system was used to compare the apical microleakage of roots filled with the system B (Analytic Technology), single-cone technique, lateral condensation, vertical condensation, and Thermafil (Caulk-Dentsply). After preparation of the canal system with a Profile (Caulk-Dentsply) five groups of 10 single-rooted teeth were randomly filled according to 1 of the 5 techniques. Phosphate-buffered saline was forced under a 15 cm H2O pressure, through the apex, toward the coronal part of the root. The filtration rate in L s(-1) Kpa(-1) was recorded 24 h after filling and after 1-month storage in phosphate-buffered saline. The 24 h results showed that the single cone technique provided the highest leakage (p = 0.001). At 1-month system B, Thermafil and vertical condensation had less leakage than the two other techniques (p = 0.0001): lateral condensation produced moderate apical leakage, whereas the single cone technique showed the highest leakage. Regardless of the technique the apical leakage increased after 1-month storage (p = 0.001 or p = 0.0001, depending on the technique).  相似文献

AIM: To compare different combinations of root canal preparation and obturation technique. METHODOLOGY: A total of 48 freshly extracted human maxillary central incisors were divided into two identical groups on the basis of root canal length and apical diameter. The root canals were prepared by manual crown-down pressureless technique or with a rotary system (ProFile; Dentsply Maillefer, Ballaigues, Switzerland) in a crown-down technique. The coronal diameter and apical size of the root canals were standardized in both preparation techniques. Each main group was then divided into two subgroups and obturated with either cold lateral compaction or continuous wave of obturation with System B (EIE-Analytic Technology, Orange, CA, USA). The distribution of filling material in each canal was assessed by stereomicroscopic examination of eight cross-sections on each tooth. Areas of sealer, gutta-percha and voids were measured on the digital images of a total of 384 samples. Manipulation time and apical extrusion for each group was also determined. Data were statistically analysed using Kruskal-Wallis, anova (Bonferroni/Dunn) or Student's t-test. RESULTS: The percentage of sealer, gutta-percha and voids area between the obturation techniques was not significantly different (P > 0.05). Continuous wave groups had significantly more apical extrusion of sealer (P < 0.05), while none of the obturation techniques had gutta-percha extrusion. Rotary instrumentation was significantly faster than the manual technique (P < 0.05); continuous wave obturation was significantly faster than lateral compaction (P < 0.05). Total manipulation time in the rotary/continuous wave group was significantly shorter than the other groups (P < 0.0001). CONCLUSION: The distribution of filling materials was similar in all combinations of instrumentation and obturation techniques. The continuous wave technique was faster than lateral compaction and it extruded more sealer.  相似文献

Aim  To investigate the technical quality of nonsurgical root canal treatment performed by endodontists in Melbourne, Australia.
Methodology  Clinical and radiographic records of 100 sequential nonsurgical patients were obtained from each of six endodontists working in private practice. The following variables were analysed: proximity of root filling to radiographic apex; homogeneity and radiodensity of root filling; lateral adaptation of the root filling to the canal walls; taper; extrusion of material; small, appropriate or excessive apical enlargement; presence of lateral canals; transportation; procedural errors. The radiographs were assessed by three independent evaluators. Exploratory data analysis was undertaken using simple frequencies and cross-tabulations. A generalised linear mixed model (GLMM) was used for the formal statistical modelling.
Results  Of the 1351 canals that were examined, 91.7% were filled within 2 mm of the radiographic apex and 74% were within 1 mm. Homogeneity and adequate density were found along the entire length of the canal in 86.1% and 88.6% of cases respectively. Lateral adaptation was adequate in 95.6% of cases and the taper was 'smooth and continuous' in 83.8% of roots. No and/or small extrusion of sealer was noted in 98.3% of cases. Apical enlargement was 'appropriate' in 85% of roots. Both transportation (1.1%) and procedural errors (1.3%) were rare occurrences.
Conclusions  The technical quality of root fillings performed by endodontists in Melbourne, Australia complied with current guidelines in 77.4%–91.0% of roots. All variables examined confirmed high levels of technical proficiency. There were very few instances of canal transportation and/or procedural errors.  相似文献

The aim of this study was to compare the apical sealing ability of three different obturation techniques using an electrochemical evaluation and evaluating dye penetration. One hundred and thirty-two maxillary anterior teeth were randomly divided into six groups. There were 20 teeth in each group. The teeth were obturated as follows: Groups 1 and 4 with Thermafil, Groups 2 and 5 with System B, and Groups 3 and 6 with cold lateral condensation (CLC). The apical leakage in these groups was evaluated using an electrochemical method for Groups 1, 2, 3 and a dye penetration method for Groups 4, 5 and 6. In both methods, the lowest mean leakage values were observed for Thermafil and the highest were observed for the CLC groups. The difference between Thermafil and CLC was statistically significant (P < 0.05). In both groups, System B fillings showed moderate leakage and the difference was not significant with Thermafil and CLC groups.  相似文献

Aim: The aim of this study is to evaluate the influence of final canal taper on the sealing ability of Thermafil by using microcomputed tomography (micro-CT). Materials and methods: Fifty-four single-rooted teeth were instrumented to apical size 40 taper 4, 6 and 8%. Teeth were divided into three groups. All teeth were filled with Thermafil (Dentsply,Tulsa Dental Products). Using micro-CT, roots were scanned and volume measurements of voids at 1, 3 and 5 mm from the apex were calculated using specialized CT software. Measurements were analyzed statistically with ANOVA followed by Bonferroni multiple comparison correction. Results: Data analysis showed that 0.08 preparations provided better results than 0.06 and 0.04% tapered samples especially at 1 mm from the apex. Mean percentage of voids were significantly higher with Thermafil taper 0.06% (p = 0.05). None of the root canals filled teeth were gap free. Root canal preparations with bigger taper exhibited less voids than smaller tapered preparations. Clinical significance: In our daily practice, enlarging the apical third (last 3 mm) of root canals to an 8% taper gives a better sealing ability and thus long-term success for our root canal obturations. AIM: The aim of the present study was to evaluate the long-term sealing ability (both apical and coronal) of an epoxy resin root canal sealer (AH26) when used with different gutta-percha obturation techniques. METHODOLOGY: Straight single-rooted teeth with mature apices were divided into 10 groups of 75 teeth and one group of 40 control teeth. Root canals were prepared according to the crown-down/step-back technique and using both 2.5% sodium hypochlorite and an EDTA paste. Root canals were obturated using cold lateral condensation, warm vertical condensation and hybrid condensation of gutta-percha; and with Thermafil and Soft-Core obturators. AH26 was used as the sealer in all cases. After root canal filling, each group was divided into five individual groups of 15 teeth. The first group of 15 teeth was kept for 1 day, the second for 1 week, the third for 4 months, the fourth for 6 months and the fifth for 12 months at 37 degrees C in vacutainers in 80% relative humidity. The teeth were immersed in India ink for 90 h, each root was split and sectioned longitudinally, and the maximum extent of leakage was measured using a stereomicroscope at x6 magnification. RESULTS: Leakage occurred whatever filling technique was combined with AH26. The number of teeth with gross leakage increased with time up to 4 months, but did not appear to increase between 6 and 12 months storage. The degree of apical leakage and the number of leaking teeth in the Soft-Core obturator groups was significantly higher than all four other gutta-percha obturation techniques. Up to 4 months the degree of coronal leakage and the number of leaking teeth in the Thermafil groups was significantly higher than the other gutta-percha obturation techniques, except for Soft-Core. In the Soft-Core obturator groups the degree of coronal leakage and the number of leaking teeth was significantly higher than the other gutta-percha obturation techniques, except for Thermafil at all evaluation times. CONCLUSION: The hybrid gutta-percha condensation technique was superior to the other four obturation techniques in respect of apical leakage. Coronal leakage was significantly greater during the first 4 months for the Thermafil system as compared to the three condensation techniques; coronal leakage was significantly greater at all time periods for the Soft-Core system. There were no significant differences between the Thermafil and the Soft-core system.  相似文献

The aim of this in vitro study was to assess the quality of root canal obturation and degree of apical dye penetration in teeth root-filled with either laterally condensed cold gutta-percha or Thermafil obturators. A total of 144 extracted teeth with single roots were included in the study. All canals were prepared with a standard technique using a sonic handpiece with copious irrigation. Radiographs taken of the teeth to show the maximum degree of canal curvature were then exposed and the angle, radius and position of curvature determined. This information about the canals, together with their working length and diameter at end-point was used to divide the teeth into two experimental and two control groups. A total of 71 teeth were filled with Thermafil obturators, 69 with lateral condensation while four teeth remained unfilled and acted as controls. Following obturation, the teeth were radiographed in two planes and the technical quality of fill assessed on a four-point scale. All access cavities were then sealed and the teeth immersed in dye for 48 h before being split longitudinally. Linear dye penetration via the apical foramen was then assessed. In both straight and curved canals obturation with Thermafil obturators was significantly quicker (P< 0.001) than lateral condensation. Apical extrusion of gutta-percha occurred significantly more often in straight canals obturated with lateral condensation (P< 0.005) but there was no difference in curved canals. There was little difference in the radiographic quality of fill in the apical halves of the root fillings but Thermafil obturators were significantly better in the coronal halves when viewed from the proximal. Apical leakage of dye was significantly worse (P< 0.05) in curved canals filled with lateral condensation with a similar, but not significant, trend in straight canals. Under the conditions of this study, Thermafil obturators proved a satisfactor alternatie to lateral condensation of gutta-perch.  相似文献

AIM: The aim of this project was to evaluate and compare the radiographic quality and sealability of root fillings in extracted human teeth using lateral condensation of gutta-percha or multiphase gutta-percha obturation (Alphaseal). METHODOLOGY: A total of 108 freshly extracted human, mature single-rooted teeth were divided into two identical groups of 54 teeth on the basis of root canal shape. The canals were prepared to a minimum 0.055 taper and enlarged to size 35 at the apex. All root canals were flushed with 17% EDTA solution and 2.5% NaOCl to remove the dentinal smear layer. The canals of one group were obturated using cold lateral condensation of gutta-percha and the canals of the other group were filled using a warm multiphase gutta-percha obturation technique. The extrusion of sealer and/or gutta-percha through the apex of the teeth was recorded using a simple yes/no scheme. The sealability of each technique was assessed by a dye penetration method. The radiographic quality of obturation was determined for each canal using a four-point scale. RESULTS: Root canals filled by multiphase obturation had significantly more extrusion of sealer (P < 0.001) and gutta-percha (P < 0.001) than canals filled by lateral condensation. Canals filled by multiphase gutta-percha obturation had significantly less apical dye leakage than those obturated by lateral condensation (P < 0.05). Lateral condensation achieved significantly better scores for radiographic quality than multiphase obturation from the bucco-lingual view (P < 0.01). CONCLUSIONS: Under laboratory conditions multiphase gutta-percha had better sealability but poorer radiographic quality than lateral condensation.  相似文献

Abstract— This study compared the apical sealing ability, obturation time and extrusion of gutta-percha and sealer when root canals were ohturated using either cold lateral condensation or one of the three methods using thermoplasticised gutta-percha (Alpha Seal, Thermafil or JS Quick Fill) in vitro . One hundred and thirty-one root canals from 78 extracted human teeth were used; 116 canals were divided into five groups so that they were balanced with respect to prepared canal anatomy, and the remaining 15 canals were used as positive and negative controls. The canals in the first four groups were prepared with hand files using the step-down technique to a standard apical size and flare. The last group was prepared using engine-driven rotary nickel-titanium files (McSpad-den) to a similar apical size and Hare. One of the four obturating techniques was used to fill the canals in each of the first four groups. The fifth group was obturated using the Alpha Seal technique. The roots were immersed in india ink, demineralised and rendered transparent to assess the extent of maximum linear dye penetration. The Alpha Seal groups had the highest number of specimens without any leakage. There was a significant difference in the proportions of specimens that did not leak when the Alpha Seal ( P <0.01) and cold lateral condensation groups ( P <0.05) were compared with JS Quick Fill. Cold lateral condensation had a higher proportion of specimens with leakage in canals with curvature greater than 20' than in canals with curvatures less than 20 ( P <0.05). The curvature of canals had no effect on the sealing ability of the other techniques. The method of canal preparation had no effect on the sealing ability of Alpha Seal. Alpha Seal, Thermafil andJS Quick Fill were significantly quicker to perform than cold lateral condensation.  相似文献

AIM: To compare the area of sealer surrounding root fillings completed by two coated carrier systems (Thermafil Obturator and System GT Obturator) and the cold lateral compaction technique, following root canal preparation with two different NiTi rotary systems (Profile ISO and System GT). METHODOLOGY: Sixty extracted human mandibular premolars were instrumented with ProFile ISO 0.06 taper and System GT instruments (n = 30 each). The teeth were divided into four subgroups (n = 15) for filling as following: group 1: ProFile + Thermafil Obturator, group 2: ProFile + Cold Lateral Compaction, group 3: System GT + GT Obturator, and group 4: System GT + Cold Lateral Compaction. In all groups the canals were prepared to a final size of 40, 0.06 taper in accordance with the manufacturer's instructions and Topseal was used as a sealer. Horizontal sections were obtained every 1 mm up to 12 mm from the apical foramen. Sections were digitally photographed under a stereomicroscope and the images were transferred to an IBM-compatible PC for image analysis. The cross-sectional area of the root canal and the area filled by sealer were calculated for each section and compared statistically both at all levels and by grouping the data as apical, middle and coronal segments; using the Kruskal-Wallis test with Bonferroni correction and chi-square tests (P = 0.05). RESULTS: In the apical third (1-4 mm), the GT Obturator (group 3) had significantly less area of sealer (P < 0.05), while the difference between groups 1 and 2 was not significant (P > 0.05). For the middle and coronal thirds, both coated-carrier systems had significantly less area of sealer compared with their laterally compacted counterparts (P < 0.05). CONCLUSIONS: With the combined use of ProFile + Thermafil Obturator and System GT + GT Obturator, significantly less area of sealer occurred than that achieved with both NiTi preparation systems followed by cold lateral compaction.  相似文献

AIMS: (i) To compare the prevalence of extrusion of root filling material when placed using different root filling techniques, with or without customization of the master gutta-percha (GP) cone; and (ii) to investigate the effects of some factors influencing root filling extrusion and presence of voids. METHODOLOGY: A total of 180 roots were selected, prepared and randomly allocated to three groups. Five general dental practitioners performed the root fillings; each filled one group of roots (n = 60) using each of three techniques; 'cold lateral compaction' (n = 20), 'warm vertical compaction' (n = 20) and 'continuous-wave' (n = 20) techniques. For each obturation technique, the master GP cone was customized using chloroform in 10 samples. Two groups of the roots were recycled to allow all five operators to fulfill their remit. Two observers, blind to operator and obturation technique, examined the radiographs (master apical file, post-obturation) to determine the presence of root filling extrusion and voids within the apical 5 mm, independently. Root filling extrusion was also confirmed by direct inspection of the root apex after obturation. The data were analysed using logistic regression models. RESULTS: A total of 300 root fillings were performed; nine were excluded from the analysis. Most of the root fillings (80%, n = 233) were placed within 0.5 mm of the working length; only 20% (n = 58) were placed >0.5 mm beyond the working length. The odds of prevalence of extrusion (>0.5 mm) were significantly reduced by about 50% when cold lateral compaction or customization of GP were used. One operator produced 2.5 times more extruded root fillings than others. Curvature & length of root canal, apical size of prepared canal, as well as operator's preferred obturation technique had no significant influence on the prevalence of extrusion. Customization of GP was the sole factor to significantly reduce the prevalence of voids within the apical 5 mm of working length. CONCLUSIONS: Root filling extrusion was significantly influenced by 'operator' and was reduced by cold lateral compaction and customization of the master cone. Customization of master cone was the only factor that reduced voids apically.  相似文献

Objectives

This study was aimed to evaluate different endodontic obturation techniques (Thermafil, lateral condensation, and Tagger’s hybrid technique) regarding the homogeneity of the obturation radiopacity.

Materials and methods

Seventy roots of human upper central incisors were filled using the Thermafil system, lateral condensation. and Tagger’s hybrid technique. Radiopacity of the filling was evaluated based on mean of grey levels, and its homogeneity was assessed by the coefficient of variation (CV), analyzing the histograms obtained of digitized and digital radiographs.

Results

The increase in mean grey levels (p?<?0.001) and reduction in the CV (p?<?0.05) were higher for Tagger’s hybrid technique compared with other methods.

Conclusions

Tagger’s hybrid technique provided better homogeneity of the obturation radiopacity and better apical sealing compared with lateral condensation technique.

Clinical relevance

The results suggest that Tagger’s hybrid technique provided the best compaction of the root canal filling material, an important factor for the sealing of obturations and, consequently, for the effectiveness of treatment.  相似文献

The aim of this in vitro study was to assess the quality of root canal obturation and degree of linear apical dye penetration in teeth root filled with either laterally condensed gutta-percha or Thermafil obturators, A total of 150 teeth with single roots were included in the study. All canals were prepared using a standard step-back technique with anticurvature filing. Radiographs taken of the teeth to show the maximum degree of canal curvature were then exposed and the angle, radius and position of curvature determined. This information about the canals, together with their working length and diameter at end-point, was used to divide the teeth into two experimental and two control groups. A total of eight teeth were excluded because of technical difficulties, 65 were filled with Thermafil obturators, 63 were filled with laterally condensed gutta-percha whilst 14 teeth remained unfilled and acted as controls. Following obturation, the teeth were radiographed in two planes and the technical quality of fill assessed on a four-point scale. All access cavities were then sealed and the teeth immersed in dye for 48 h before being split longitudinally. Linear dye penetration via the apical foramen was then assessed. Canal obturation with Thermafil obturators (0.7 min) was significantly quicker (P > 0.001) than lateral condensation (6.4 min). Apical extrusion of sealer and gutta-percha occurred significantly more often with Thermafil obturators but there was little difference in the technical quality of the fillings and no significant difference in dye penetration. Under the conditions of this study, Thermafil obturators proved a satisfactory alternative to lateral condensation of gutta-percha.  相似文献

The purpose of this study was to assess the seal obtained in straight and curved root canals filled with either laterally compacted gutta-percha or Thermafil obturators. Each technique was used in combination with three different sealers (RSA RoekoSeal, AH Plus, AH 26). Thermafil obturators were also used without sealer, resulting in a total of 14 test groups of 16 teeth each. Sixty teeth served as positive or negative controls. Included in the study were 142 extracted teeth with straight and 142 with curved root canals. All canals were enlarged up to size 40. After obturation, all roots were placed in India ink for 48 h and rendered transparent to measure the maximum linear dye penetration. Canals filled with Thermafil obturators had significantly more extrusion of filling material than canals filled by lateral compaction (p < 0.01). Thermafil without sealer showed significantly greater dye penetration compared with all other groups both in straight and in curved canals (p < 0.05). As long as a sealer was used, the seal obturated with Thermafil was equivalent in terms of dye penetration to lateral compaction. There were no statistical differences in the mean apical dye penetration among the three sealers. The differences between the dye penetration in straight and in curved canals were insignificant for all groups (p > 0.05). Under the conditions of this study, Thermafil obturators achieved seals comparable to lateral compaction, as long as a sealer was used.  相似文献

This study investigated the sealing properties of cold laterally compacted gutta percha and Thermafil applied over different apically fractured rotary nickel-titanium files. Extracted human premolars were prepared by using ProTaper (Dentsply-Maillefer, Ballaigues, Switzerland) or ProFile (Dentsply-Maillefer) systems (n = 40 for each), after which half of the specimens in each subgroup were subjected to instrument separation at the apical level. For each rotary system, roots with and without apically separated instruments (n = 10 for each) were filled with the two obturation systems and the levels of fluid conductance were recorded. In the absence of instrument separation, the leakage of Thermafil (Dentsply Maillefer) was significantly less than that achieved with cold lateral compaction (p < 0.05), whereas Thermafil yielded similar amounts of leakage in roots prepared with the ProTaper and ProFile systems (p < 0.05). Roots with fractured ProTaper instruments displayed significantly less leakage than those filled without ProTaper fragments, regardless of the obturation technique used (p < 0.05). However, intracanal separation of ProFile instruments increased the leakage (p < 0.05), but the obturation method did not influence fluid conduction (p > 0.05). These results reveal the variability of leakage associated with apically fractured ProFile and ProTaper files and different obturation methods used.  相似文献

AIM: To compare the sealing ability and the canal-filled area (gutta-percha + sealer) of three current filling techniques in oval-shaped canals. METHODOLOGY: A total of 50 oval-shaped root canals were prepared and root filled as follows: G1: lateral condensation (n = 10), G2: System B (n = 10) and G3: Thermafil system (n = 10). All teeth were mounted in a two-chamber apparatus and the coronal access was exposed to human saliva. The appearance of turbidity in the BHI broth over a 15-week period was observed. A cross section of each tooth was made 5 mm from the apex and the samples were prepared for microscopic analysis. Digital image measurements of the cross-sectional area and the area filled by gutta-percha and sealer were performed. Log-rank and Fisher's exact tests were used to analyse the leakage data. Student's t-test was used to analyse the filled-area data. RESULTS: Overall, 30% of the specimens of G1 and G2 and 20% of G3 demonstrated leakage after 15 weeks and no significant difference was found amongst the groups (P > 0.05). The percentage of canal-filled area was 68, 70 and 78%, respectively (P > 0.05). The correlation analysis revealed no significant relation between the pattern of bacterial leakage and canal-filled area (P = 0.128). CONCLUSIONS: No significant difference in apical sealing and canal-filled area in oval-shaped canals was seen between the three filling techniques. No significant correlation was found between the quality of the apical seal and the filled-area of the root canal space.  相似文献
