目的探讨血清25-羟维生素D[25-(OH)D]水平与儿童1型糖尿病(T1DM)及酮症酸中毒(DKA)的相关性。方法选取2006年1月—2009年12月期间152例住院患儿,其中52例为首次发病的T1DM患儿,包括酮症酸中毒(DKA组)21例,以及非酮症酸中毒(非DKA组)31例,其余100例为非T1DM组。检测并比较三组患儿的血清25-(OH)D水平,分析血清25-(OH)D水平与儿童T1DM及DKA的相关性。结果 DKA组患儿的血清25-(OH)D平均为(53.6±27.8)nmol/L,显著低于非DKA组的(69.7±27.9)nmol/L和非T1DM组的(81.8±28.3)nmol/L(P<0.05);非DKA组患儿的血清25-(OH)D水平显著低于非T1DM组(P<0.05)。结论 T1DM患儿的血清25-(OH)D水平低,尤以DKA患儿最为明显,维生素D在儿童T1DM发病中的潜在保护效应值得关注。  相似文献

目的 探讨浙江省新诊断儿童1型糖尿病(DM)患儿流行病学特征及血脂特点。方法对1999年1月1日~2004年12月31日住院且为首次发病的101例1型DM患儿的年龄发病时间、出生时间、酮症酸中毒(DKA)及血脂特点进行回顾性分析。结果2004年新诊断的1型DM病人与总住院人数之比高于1999年;男童中12岁以上组病例所占同性别比例高于同年龄组女童(27.5%vs4.9%).新诊断1型DM患儿出生于10月份至次年1月份者多于2~5月份出生者,同期发病的病例与总住院人数之比也明显升高。新诊断病例DKA组血糖和糖化血红蛋白(HbAlc)较非DKA组明显升高。两组总胆固醇(TC)和三酰甘油(TG)有显著性差异。血脂异常组与血脂正常组并DKA的比率有显著性差异,两组血糖、HbAlc和住院时间差异有显著性。结论儿童1型DM与总住院人数之比早逐年上升趋势;其发病可能与青春发育有关。初发1型DM患儿的DKA发生率又有回升。1型DM患儿TC、TG与血糖和HbAlc水平相关。  相似文献

胰岛素泵治疗儿童1型糖尿病酮症酸中毒32例临床分析   总被引:2,自引:0,他引:2  
目的 观察胰岛素泵持续皮下注射胰岛素对儿童1型糖尿病酮症酸中毒(DKA)的疗效.方法 将2005-2008年收治的1型DKA患儿64例分为治疗组32例和对照组32例.治疗组予胰岛素泵治疗,对照组予小剂量胰岛素持续静脉滴注治疗.比较两组患儿血精变化、DKA纠正时间及住院时间.结果 治疗组血糖下降相对稳定,酸中毒纠正时间治疗组[(16.91±4.223)h]短于对照组[(23.31±3.797)h](P<0.001),且无反复.治疗过程中治疗组未出现低血糖,对照组出现1例.住院时间治疗组[(15.63±2.458)d]短于对照组[(20.88±3.348)d](P<0.001).结论 胰岛素泵持续皮下注射胰岛索治疗儿童1型糖尿病酮症酸中毒安全有效.  相似文献

目的 总结儿童1型糖尿病酮症酸中毒(DKA)合并低磷血症的临床特点,探讨DKA治疗期间低磷血症的发生率及预后。方法 回顾性分析2016年1月至2020年6月于广州市妇女儿童医疗中心遗传与内分泌科住院治疗的133例合并DKA的1型糖尿病患儿临床资料,分析入院后4~24h血磷水平,并对影响儿童血磷变化的相关因素进行分析。结果 133例DKA患儿中男55例(41%),女78例(59%),年龄6月龄至15岁,平均年龄(6.7±3.9)岁,新发1型糖尿病占72%。75例(56%)患儿出现血磷降低,其中41例(31%)患儿血磷0.81~1.29 mmol/L,22例(16%)患儿血磷0.61~0.81mmol/L,10例(7.5%)患儿血磷0.38~0.61mmol/L,2例(1.5%)患儿血磷<0.38mmol/L。正常血磷组与低血磷组之间pH值、碳酸氢根(HCO3-)、碱剩余(BE-B)、血钾、血氯差异均有统计学意义。酸中毒、年龄≤5岁为影响儿童DKA时血磷降低严重程度的危险因素。75例低血磷患儿均未出现低磷血症相关临床症状,未补充磷酸盐治疗。34例血磷<0.81mmol/L的患儿...  相似文献

目的 分析初发1型糖尿病(T1DM)患儿伴发糖尿病酮症酸中毒(DKA)的严重程度与甲状腺功能状态的相关性。方法 回顾性分析2015年1月至2020年12月内分泌遗传代谢科收治的167例初发T1DM患儿临床资料,比较T1DM患儿中非DKA,轻度、中度、重度DKA四组临床特点、实验室指标、甲状腺功能状态差异,分析血清游离三碘甲状腺原氨酸(FT3)水平的影响因素。结果 167例初发T1DM患儿中,男81例、女86例,中位年龄6.5(4.0~9.9)岁,中位病程14.0(7.0~28.0)d,非DKA组104例、轻度DKA组20例、中度DKA组16例、重度DKA组27例。各DKA组(FT3)、总三碘甲状腺原氨酸(TT3)、总甲状腺素(TT4)、pH值及HCO-3显著低于非DKA组,而阴离子间隙(AG)、血糖水平显著高于非DKA组;重度DKA组的白细胞计数、中性粒细胞百分比显著高于其他三组,差异均有统计学意义(P<0.05)。167例初发T1DM儿童中,甲状腺...  相似文献

目的 研究儿童青少年1型糖尿病(type 1 diabetes mellitus, T1DM)发生糖尿病酮症酸中毒(diabetic ketoacidosis, DKA)的危险因素,并建立DKA风险预测模型,以期降低该类患儿DKA的发生率,提高患儿生存质量。方法 回顾性选择2018年1月—2021年12月宁夏医科大学总医院收治的217例T1DM患儿,其中169例发生DKA患儿为DKA组,48例未发生DKA患儿为非DKA组。分析T1DM患儿发生DKA的危险因素,并建立预测T1DM患儿发生DKA风险的列线图模型。结果 217例T1DM患儿中DKA发生率为77.9%(169/217)。多因素logistic回归分析显示,T1DM患儿入院随机血糖高、糖化血红蛋白高(hemoglobin A1c, HbA1c)、血酮高、甘油三酯高与发生DKA密切相关(分别OR=1.156、3.203×1015、20.131、9.519,P<0.05)。列线图预测模型C-统计量为0.95,列线图模型预测T1DM患儿发生DKA的风险与实际发生DKA的风险平均绝对误差为0.004,说明模型整体预测能力较好。结论...  相似文献

目的了解儿童糖尿病合并酮症酸中毒(DKA)和高血糖高渗状态(HHS)的临床特征。方法依据诊断标准,从1995年1月至2009年12月新确诊的糖尿病患儿中筛选DKA和HHS患儿,比较糖尿病同时合并DKA和HHS(DKA-HHS)与单纯DKA患儿的临床特点。结果 1 065例新诊断儿童糖尿病患儿中有483例合并DKA,占45.35%;16例符合DKA-HHS,占1.50%;无单纯HHS糖尿病患儿。HHS在DKA患儿中的发生率为3.31%,其中1型糖尿病14例,2型糖尿病2例。HHS在年龄≥10岁的DKA患儿中的发生率明显高于小年龄患儿(χ2=6.05,P<0.05)。DKA-HHS患儿中重度脱水的比例、平均有效渗透压、血糖、三酰甘油及尿素氮水平均显著高于单纯DKA患儿,且酸中毒程度明显重于单纯DKA患儿,差异均有统计学意义(P均<0.05);DKA-HHS患儿的校正血清钠水平显著高于单纯DKA患儿,差异有统计学意义(P<0.05)。结论新诊断糖尿病患儿的HHS发生率较DKA低,且均与DKA合并存在;更常见于≥10岁患儿;DKA-HHS较单纯DKA患儿的代谢紊乱程度、脱水及酸中毒程度更严重,肾功能异常率更高。  相似文献

目的探讨近年新诊断儿童1型糖尿病的临床特征。方法对1999年1月1日~2005年4月30日浙江大学医学院附属儿童医院住院且为首次发病的109例1型糖尿病的临床资料进行回顾性分析,按照有无酮症酸中毒和有无低钾血症分别进行分组比较,正态分布的数据用x±s表示,采用为卡方及t检验,非正态分布的资料用中位数表示,采用Mann-WhiteyU检验进行组间比较。结果新诊断1型糖尿病儿童非酮症酸中毒(DKA)组病程明显较DKA组长,平均住院时间和糖化血红蛋白明显低于DKA组。入院时有感染诱因者21例(19.3%);伴低钾血症48例(44%),非低钾血症61例(56%);DKA组低钾血症的比率为26/47(55.3%),明显高于非DKA组的22/62(35.5%)。低钾组出现心悸、胸闷及倦怠症状出现频率为39.6%、35.4%,而非低钾组上述症状的频率分别是11.5%、16.4%,两组差异有显著性。所有患儿无1例死亡,出院后治疗依从性不一致。结论感染可能导致患儿糖尿病的进展和临床表现的出现。低钾血症组的症状较重。可能是临床症状的不典型导致了糖尿病不能及时被发现。  相似文献

目的观察胰岛素泵持续皮下注射胰岛素对儿童1型糖尿病并酮症或酮症酸中毒(DK/DKA)的疗效。方法本院内分泌科2003~2005年收治的1型糖尿病并DK/DKA患儿43例,分为治疗组26例和对照组17例。治疗组予胰岛素泵治疗,对照组予小剂量胰岛素持续静脉滴注。比较二组患儿血糖、尿酮体、血pH值变化,住院时间长短。结果1.治疗组血糖下降相对稳定,纠正酸中毒后无反复。2.治疗过程中治疗组未出现低血糖,对照组2例出现。3.住院时间治疗组[(11.92±4.72)d]较对照组[(17.35±4.83)d]治疗组较对照组明显缩短(P<0.001)。结论胰岛素泵持续皮下注射胰岛素治疗儿童1型糖尿病并DK/DKA是安全有效的。  相似文献

目的探讨儿童糖尿病酮症酸中毒并发脑水肿的临床特点及危险因素。方法对糖尿病酮症酸中毒并发脑水肿患儿的临床特点及病因进行分析,并与无脑水肿的患儿资料进行对比。结果在47例糖尿病酮症酸中毒患儿中,并发脑水肿4例(8.51%),均为重度酮症酸中毒。与未并发脑水肿同等程度的酮症酸中毒患儿相比,并发脑水肿者在治疗期间血钠上升缓慢、甚至降低,尿素氮水平较高。4例患儿中有3例应用了碳酸氢盐,用量大于未并发组。结论糖尿病患儿并发重度酮症酸中毒时易并发脑水肿。血钠上升缓慢甚或降低、血尿素氮升高及碳酸氢盐的不适当应用,可增加脑水肿发生的危险。  相似文献

OBJECTIVE: To investigate the role of serum interleukin (IL-18) in children with type 1 diabetes mellitus (T1DM) and diabetic ketoacidosis (DKA). DESIGN: Case-control study. SUBJECTS: Sixty-one children with T1DM including 28 with DKA and 33 without DKA and 30 age - and sex-matched healthy controls were recruited. METHODS: Serum IL-18, IL-12, and IFN-gamma levels were measured in all subjects by enzyme linked immunosorbent assay. RESULTS: Serum IL-18 levels were significantly higher in patients with DKA than those in patients without DKA (759.2 +/- 353.8 pg/mL vs. 634.9 +/- 399.7 pg/mL, P = 0.001) and healthy controls (310.0 +/- 265.3 pg/mL). The serum IL-12 and IFN-gamma levels were not different between patients and controls (277.5 +/- 207 pg/mL vs. 351.4 +/- 223.4 pg/mL, P = 0.45 and 7.02 +/- 7.53 pg/mL vs. 5.59 +/- 5.34 pg/mL, P = 0.21, respectively). CONCLUSION: Serum IL-18 levels are increased in children with type 1 diabetes mellitus and could be a predictor of diabetic ketoacidosis.  相似文献

We have attempted to identify any characteristics which could be used to predict the development of cerebral edema in four children under 5 years of age with new onset insulin-dependent diabetes mellitus and diabetic ketoacidosis. We retrospectively analysed and compared the concentration of serum sodium (corrected for serum glucose value) and effective serum osmolality of these 4 children with values of 10 age-matched controls with new onset insulin-dependent diabetes mellitus who did not develop cerebral edema during treatment of diabetic ketoacidosis. The initial serum sodium values of the two groups were not statistically different. Patients who developed cerebral edema had lower initial serum glucose values and effective serum osmolality. During treatment, patients who developed cerebral edema had consistently lower mean serum sodium and osmolality than controls at each 4-h interval after the first 4 h of therapy. Serum sodium and osmolality declined progressively after the initiation of therapy in cerebral edema patients, while remaining stable in controls. These data suggest that children who develop cerebral edema during treatment for diabetic ketoacidosis initially may have a relatively normal serum osmolality and subsequently develop progressive hyponatremia and/or a trend of declining serum sodium before developing cerebral edema.  相似文献

In 1982 Johnson et al. described a simple colorimetric assay for measuring glycated proteins, termed fructosamine, in the serum of adults with diabetes mellitus and demonstrated this to be a useful index of intermediate glucose control (1-3 weeks). Our study was designed to show this as well in children and adolescents with type I diabetes mellitus. Serum fructosamine was determined in 76 children and adolescents with diabetes mellitus, and 111 age-matched controls. In the controls an age-dependency but not sex-dependency could be demonstrated. In the diabetic patients we found a significant correlation between serum fructosamine and HbA1 values (r = 0.87, p less than 0.001). In 6 patients with newly diagnosed diabetes mellitus serum fructosamine concentrations decreased at a faster rate than HbA1 values. The fructosamine assay is rapid, technically simple and inexpensive, and is at least a useful addition or perhaps an alternative to HbA1 estimation.  相似文献

Endothelial-cell specific adhesion molecules are reported to be elevated in patients with diabetes mellitus and related to diabetic vascular complications. We studied serum concentrations of soluble intercellular adhesion molecule-1 (sICAM-1), vascular cell adhesion molecule-1 (sVCAM-1), endothelial-leukocyte adhesion molecule (sE-selectin) in 30 healthy children and 35 children with type 1 diabetes without symptomatic vascular complications. sE-selectin levels were higher in diabetics than in controls (p < 0.001). sVCAM-1 and sICAM-1 levels were not different between the groups (p > 0.05). In seven newly diagnosed diabetics with ketoacidosis, concentrations of these molecules were not different before and after one month of insulin therapy (p > 0.05). In the combined group, only sE-selectin was correlated positively with serum glucose, HbA1c (r = 0.3, p < 0.05 for both) and negatively with C-peptide levels (r = -0.4, p < 0.05). In diabetic children without symptomatic vascular complications, sE-selectin but not sICAM and sVCAM levels was elevated; this finding might reflect ongoing endothelial-cell activation rather than endothelial damage.  相似文献

Serum levels of cholesterol (C), triglycerides (TG), lipoprotein-C and apolipoproteins (apo) A-I, A-II and B were measured in 30 children with type I diabetes mellitus (16 boys, 14 girls, aged 11-14 years) and in 26 healthy controls (15 boys, 11 girls, aged 10-13 years). For 19 diabetics controls matched for age, sex and relative body weight were selected. The diabetic patients were considered to be in fair metabolic control according to HbA1 levels and glycosylated serum protein concentrations. Mean serum apo A-I, A-II and B, C, TG, low density lipoprotein cholesterol (LDL-C) and high density lipoprotein cholesterol (HDL-C) did not differ significantly between diabetic nondiabetic children. Very low density lipoprotein cholesterol (VLDL-C) was significantly higher in diabetic children than in controls. Serum C and LDL-C levels showed close univariate linear correlations with glycosylated serum protein (LDL-C: r = 0.53, p less than 0.01, C: r = 0.58, p less than 0.01) in diabetics. The ratio LDL/HDL-C was significantly correlated to HbA1 levels (r = 0.47, p less than 0.01). By canonical and multiple linear correlation analysis significant relations of a selected set of variables concerning the control and therapy of diabetes (serum glucose, HbA1, glycosylated serum protein, insulin dose) with a set of lipoprotein variables (C, TG, VLDL-C, HDL-C, LDL-C, apo A-I, A-II, B) could be demonstrated. From these data we conclude that significant relations between atherogenic serum lipids and lipoproteins (C, LDL-C) and the degree of metabolic control exist in diabetic children, even in the absence of marked dyslipoproteinemia.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

The objective of the study was to investigate whether immunologic and biochemical events occurring in the course of type 1 diabetes mellitus might play a role in the development of the celiac disease. The study was carried out on 223 children with long-standing diabetes mellitus type 1 (DM1). All the patients had TSH, fT4, fT3, urinary albumin secretion rate, IgA, level of antigliadin antibodies (AGA) IgA and IgG, antitissue transglutaminase IgA antibodies, antiendomysium (EmA) IgA and IgG antibodies and antitireoglobulin antibodies, antithyroid peroxidase antibodies evaluated. Serum TNF-alpha, IL-6, and IL-10 levels were also measured. The group of children with coincident DM1 and celiac disease and without autoimmune thyroiditis was characterized by significantly higher glycosylated hemoglobin, higher serum TNF-alpha, IL-6 but lower serum IL-10 in relation to the remaining diabetic patients. A statistically significant positive correlation was observed between IgA-anti-tTG and serum TNF-alpha (R = 0.28, p = 0.026); between IgG AGA and serum IL-6 (R = 0.31, p = 0.023); and between glycosylated hemoglobin and IgA-anti-tTG (R = 0.21, p = 0.001) and IgA antiendomysium (R = 0.22, p = 0.001). Poor metabolic control, persistent elevated levels of proinflammatory cytokines, and decreased level of antiinflammatory cytokines occurring in the course of type 1 diabetes mellitus might influence the incidence of celiac disease.  相似文献

Latent autoimmune diabetes mellitus in adults (LADA) is characterized by clinical presentation as type 2 diabetes mellitus after 25 years of age, initial control achieved with oral hypoglycemic agents for at least 6 months, presence of autoantibodies and some immunogenetic features of type 1 diabetes mellitus. An 8.3 year-old girl was referred to our pediatric endocrinology department because of incidental glucosuria. She did not complain of polyuria, polydipsia, or weight loss. Her body mass index (BMI) was at the 80th percentile. Fasting glucose was 126 mg/dl, and OGTT glucose level at 120 min was 307 mg/dl. Although C-peptide levels were normal, her first phase insulin response (FIR) was lower than the 1st percentile. Anti-insulin antibody (AIA), islet cell antibody (ICA), and anti-glutamic acid decarboxylase (antiGAD) were negative. According to the clinical and laboratory findings, she was diagnosed as having type 2 diabetes mellitus. She was started with oral anti-diabetic treatment for a period of 1 year. Insulin had to be initiated for worsening of HbA1c levels. In the fourth year of follow-up, she was admitted to our hospital with diabetic ketoacidosis although she was on an intensive insulin regimen. At this time, C-peptide levels were low, antiGAD and AIA were positive with HLA DR3/DQ2 haplotype. In addition, her thyroid peroxidase antibody and endomysium antibody were found to be high at follow-up. Small intestinal biopsy revealed celiac disease. This patient may represent the first case of latent autoimmune diabetes mellitus in children (LADC) with autoimmune thyroiditis and celiac disease.  相似文献

Hyperglycaemia has been reported to cause increased production of oxygen free radicals. Oxidative stress may contribute to the pathogenesis of diabetic complications. Coenzyme Q(10) (CoQ(10)) is known for its key role in mitochondrial bioenergetics and is considered as a potent antioxidant and free radical scavenger. This study was conducted to evaluate plasma and blood cell concentrations of CoQ(10) in accordance to its redox capacity in children with diabetes mellitus type 1. CoQ(10) plasma and blood cell concentrations and redox status were measured using high-performance liquid chromatography with electrochemical detection in 43 children with diabetes mellitus type 1 and compared with 39 healthy children. In addition, the diabetic patients were subdivided according to their haemoglobin A1c (HbA1c) values into two groups, that is, those with good control (<8%) and those with poor control (>8%), and the CoQ(10) status was compared between the two groups. Children with type 1 diabetes showed increased plasma levels of CoQ(10) in comparison to healthy children. While CoQ(10) erythrocyte and platelet concentrations did not differ, in the diabetes group, the platelet redox status differed with a significantly increased part of reduced CoQ(10). This difference in concentration and redox status in comparison to healthy controls may be attributed to the subgroup of patients with poor control, as the subdivision of diabetic patients according to their HbA1c values shows. In diabetic children, especially in those with poor control, an increase in plasma concentration and intracellular redox capacity of the antioxidant CoQ(10) may contribute to the body's self-protection during a state of enhanced oxidative stress.  相似文献
