小儿支原体肺炎54例胸部CT影像分析   总被引:1,自引:0,他引:1  
目的分析总结小儿肺炎支原体肺炎(MPP)胸部CT影像学特点。方法回顾性分析本院儿科2010年9月—2011年2月间明确诊断的54例MPP患儿的临床资料及胸部CT。结果胸部CT显示病变部位:单侧者39例(72.22%),双侧者15例(27.78%);单侧中右侧者20例(51.28%),左侧者19例(48.72%);多个叶段受累30例(55.56%)多于单个叶段受累24例(44.44%),各受累叶段分布情况为:右下叶18例(33.33%),左下叶15例(27.78%),左上叶11例(20.37%),右上叶7例(12.96%),右中叶7例(12.96%),左中叶2例(3.70%);病变性质显示为大片实变影31例(57.41%),斑点状及斑片状实变影22例(40.74%),云絮状或磨玻璃样或网格样密度增高影各1例(1.85%),双肺散在斑片影7例(12.96%),其中非单个叶段受累者大片实变与斑点状及斑片状实变多同时存在。其他病变还有胸腔积液14例(25.93%),肺门淋巴结肿大7例(12.96%),肺不张7例(12.96%),心包积液1例(1.85%)。婴幼儿MMP胸部CT以散在斑片影为主,年幼儿以斑点状及斑片状实变影为主,年长儿则多表现为大片实变影。结论小儿MPP单侧病变多于双侧,右肺病变多于左肺,大片实变影最多,其次为斑点及斑片状实变影,也可见双肺散在斑片影。其影像学特点与年龄有关。胸部CT影像学特点有助于婴幼儿及年幼儿MPP的诊断及儿童MPP的早期诊断。  相似文献

Introduction and ObjectivesThe pivotal role of chest computed tomographic (CT) to diagnosis and prognosis coronavirus disease-2019 (COVID-19) is still an open field to be explored. This study was conducted to assess the CT features in confirmed cases with COVID-19.Materials and MethodsRetrospectively, initial chest CT data of 363 confirmed cases with COVID-19 were reviewed. All subjects were stratified into three groups based on patients’ clinical outcomes; non-critical group (n=194), critical group (n=65), and death group (n=104). The detailed of CT findings were collected from patients’ medical records and then evaluated for each group. In addition, multinomial logistic regression was used to analyze risk factors according to CT findings in three groups of patients with COVID-19.ResultsCompared with the non-critical group, mixed ground-glass opacities (GGO) and consolidation lesion, pleural effusion lesion, presence of diffuse opacity in cases, more than 2 lobes involved and opacity scores were significantly higher in the critical and death groups (P<0.05). Having more mixed GGO with consolidation, pleural effusion, lack of pure GGO, more diffuse opacity, involvement of more than 2 lobes and high opacity score identified as independent risk factors of critical and death groups.ConclusionCT images of non-critical, critical and death groups with COVID-19 had definite characteristics. CT examination plays a vital role in managing the current COVID-19 outbreak, for early detection of COVID-19 pneumonia. In addition, initial CT findings may be useful to stratify patients, which have a potentially important utility in the current global medical situation.  相似文献

目的 探讨新型冠状病毒肺炎(COVID-19)首次胸部高分辨率CT(HRCT)影像表现。方法 回顾性分析2020年1月3日至25日武汉市中西医结合医院确诊的106例COVID-19患者的首次胸部HRCT图像,对病灶分布、形态及周围受累情况进行统计和分析。结果 106例COVID-19患者首次肺部HRCT均发现病灶,单侧肺分布11例(10.4%),双侧肺分布95例(89.6%);肺外周分布65例(61.3%),肺外周及中心同时分布41例(38.7%);1个病灶8例(7.5%),2个病灶5例(4.7%),多发病灶93例(87.8%);结节状病灶12例(11.3%),磨玻璃状病灶94例(88.7%),条索状病灶7例(6.6%),2种及以上形态共存病灶15例(14.2%);累及一个肺叶10例(9.4%),累及2个及以上肺叶96例(90.6%);纵隔淋巴结增大24例(22.6%)(>60岁患者19例,占79.2%),胸腔积液3例(2.8%),心包积液1例(0.9%),胸膜受累/增厚2例(1.9%)。>60岁患者多表现为多病灶,多形态,肺外周及中心同时分布,累及多个肺叶,纵隔淋巴结增大。结论 COVID-19患者首次胸部HRCT检查能发现肺部病灶,是首选的影像学检查方法,胸部HRCT扫描检查在NCP的早期诊断中有重要作用。  相似文献

目的 探讨新型冠状病毒肺炎(COVID-19)亚临床期高分辨率CT(HRCT)特征及短期演变规律。方法 回顾性分析2020年1月15日至31日在华中科技大学同济医学院附属协和医院及武汉市金银潭医院确诊的17例亚临床期COVID-19患者的HRCT影像资料,男4例,女13例,年龄25.0~51.0(39.8±7.5)岁,均为与确诊COVID-19患者的密切接触者。所有患者首次CT检查后3~6 d再次行CT复查,其中6例患者复查前接受抗炎抗病毒及对症治疗,11例患者未进行任何治疗。分析亚临床期和短期随访的CT影像学征象,总结其短期随访CT图像演变规律。结果 17例COVID-19患者亚临床期及短期随访CT病变主要位于双肺下叶,亚临床期左肺下叶9例,右肺下叶10例;短期随访左肺下叶9例,右肺下叶11例;病变累及肺段由亚临床期的46个增加至90个肺段。亚临床期病变以多发为主(13例),短期复查多发病例减少(7例),局灶性(6例)及弥漫分布(4例)增多。病变主要分布于胸膜下(13例)和沿支气管血管束走行(10例),短期随访1例病变分布由胸膜下进展为胸膜下及沿支气管血管束走行。病变主要有3种形态:磨玻璃密度结节影、斑片状/片状/团片状磨玻璃影和铺路石征。亚临床期及短期复查均可见磨玻璃影,随访复查显示磨玻璃结节减少4例,铺路石征增加4例。6例患者治疗后短期复查,3例由多发磨玻璃结节变为单发磨玻璃结节,3例多发磨玻璃影部分范围变小。11例未治疗患者病变范围增大。结论 亚临床期COVID-19患者肺部CT具有一定特征,多为胸膜下或沿支气管血管束走行的小磨玻璃密度结节或磨玻璃影,短期随访病变变化较大。  相似文献

The predominant pulmonary imaging findings on chest CT in the novel 2019 coronavirus infection (COVID-19) are bilateral ground glass opacities. The reverse halo sign is uncommon. This is a report of the new “bullseye sign,” which is considered a variant of the reverse halo sign and favored to represent a focus of organizing pneumonia. The specificity of this finding is unclear, however its presence should alert radiologists to the possibility of COVID-19 infection.  相似文献

ObjectiveInitial COVID-19 reports described a variety of clinical presentations, but lower respiratory abnormalities are most common and chest CT findings differ between adult and pediatric patients. We aim to summarize early CT findings to inform healthcare providers on the frequency of COVID-19 manifestations specific to adult or pediatric patients, and to determine if the sensitivity of CT justifies its use in these populations.MethodsPubMed was searched for the presence of the words “CT, imaging, COVID-19” in the title or abstract, and 17 large-scale PubMed and/or Scopus studies and case reports published between January 1, 2020 and April 15, 2020 were selected for data synthesis.ResultsInitial CT scans identified ground-glass opacities and bilateral abnormalities as more frequent in adults (74%, n = 698, and 89%, n = 378, respectively) than children (60%, n = 25, and 37%, n = 46). At 14+ days, CT scans evidenced varied degrees of improvement in adults but no resolution until at least 26 days after the onset of flu-like symptoms. In pediatric patients, a third (n = 9) showed additional small nodular GGOs limited to a single lobe 3-5 days after an initial CT scan.ConclusiónEarly adult CT findings suggest the limited use of CT as a supplemental tool in diagnosing COVID-19 in symptomatic adult patients, with a particular focus on identifying right and left lower lobe abnormalities, GGOs, and interlobular septal thickening. Early pediatric CT findings suggest against the use of CT if RT-PCR is available given its significantly lower sensitivity in this population and radiation exposure.  相似文献

Typical chest CT findings in COVID-19 have been described as bilateral peripheral ground glass opacities, with or without consolidation. Halo sign and reversed halo sign have been reported as atypical imaging findings in this disease. However, to the best of our knowledge, combined presence of these signs has never been reported before. Herein, we present a COVID-19 patient with numerous atypical target-shaped, combined halo and reversed halo pulmonary lesions, in the absence of any other underlying disease.  相似文献

PurposeOur purpose was to conduct a comprehensive systematic review of all existing literature regarding imaging findings on chest CT and associated clinical features in pregnant patients diagnosed with COVID-19.Materials & methodsA literature search was conducted on April 21, 2020 and updated on July 24, 2020 using PubMed, Embase, World Health Organization, and Google Scholar databases. Only studies which described chest CT findings of COVID-19 in pregnant patients were included for analysis.ResultsA total of 67 articles and 427 pregnant patients diagnosed with COVID-19 were analyzed. The most frequently encountered pulmonary findings on chest CT were ground-glass opacities (77.2%, 250/324), posterior lung involvement (72.5%, 50/69), multilobar involvement (71.8%, 239/333), bilateral lung involvement (69.4%, 231/333), peripheral distribution (68.1%, 98/144), and consolidation (40.9%, 94/230). Pregnant patients were also found to present more frequently with consolidation (40.9% vs. 21.0–31.8%) and pleural effusion (30.0% vs. 5.0%) in comparison to the general population. Associated clinical features included antepartum fever (198 cases), lymphopenia (128 cases), and neutrophilia (97 cases). Of the 251 neonates delivered, 96.8% had negative RT-PCR and/or IgG antibody testing for COVID-19. In the eight cases (3.2%) of reported neonatal infection, tests were either conducted on samples collected up to 72 h after birth or were found negative on all subsequent RT-PCR tests.ConclusionPregnant patients appear to present more commonly with more advanced COVID-19 CT findings compared to the general adult population. Furthermore, characteristic laboratory abnormalities found in pregnant patients tended to mirror those found in the general patient population. Lastly, results from neonatal testing suggest a low risk of vertical transmission.  相似文献

Rationale and objectivesThere is a rising onus on understanding the common features of COVID-19 pneumonia on different imaging modalities. In this study, we aimed to review and depict the common MRI features of COVID-19 pneumonia in our laboratory confirmed case series, the first comprehensive reported cohort in the literature.Materials and methodsUpon IRB approval, eight laboratory confirmed COVID-19 patients who presented to our outpatient imaging clinic underwent chest CT and, once various features of COVID-19 pneumonia were identified, a dedicated multisequence chest MRI was performed on the same day with an institutional protocol. Demographic data and the morphology, laterality and location of the lesions were recorded for each case.ResultsFive males and three females with the mean age of 40.63 ± 12.64 years old were present in this case series. Five cases had typical CT features with ground glass opacities and consolidations, readily visible on different MRI sequences. Three cases had indeterminate or atypical features which were also easily seen on MRI. The comprehensive review of MRI features for each case and representative images have been illustrated.ConclusionBecoming familiar with typical findings of COVID-19 pneumonia in MRI is crucial for every radiologist. Although MRI is not the modality of choice for evaluation of pulmonary opacities, it has similar capabilities in detection of COVID-19 pneumonia when compared to chest CT.  相似文献

BackgroundMany patients with coronavirus disease 2019 (COVID-19) have been diagnosed with computed tomography (CT). A prognostic tool based on CT findings could be useful for predicting death from COVID-19.ObjectivesTo compare the chest CT findings of patients who survived COVID-19 versus those of patients who died of COVID-19 and to determine the usefulness the clinical usefulness of a CT scoring system for COVID-19.MethodsWe included 124 patients with confirmed SARS-CoV-2 infections who were hospitalized between April 1, 2020 and July 25, 2020.ResultsWhereas ground-glass opacities were the most common characteristic finding in survivors (75%), crazy paving was the most characteristic finding in non-survivors (65%). Atypical findings were present in 46% of patients. The chest CT score was directly proportional to mortality; a score  18 was the best cutoff for predicting death, yielding 70% sensitivity (95%CI: 47%-87%).ConclusionsOur results suggest that atypical lesions are more prevalent in this cohort. 目的 探讨新型冠状病毒肺炎(COVID-19)患者初诊临床特征及胸部高分辨率CT(HRCT)影像表现。方法 回顾性搜集2020年1月20日至28日武汉大学人民医院经临床及核酸检测确诊的141例COVID-19患者,男77例,女64例,中位年龄49(9, 87)岁。分析141例COVID-19患者的临床特征、实验室检查指标以及胸部HRCT影像学表现。结果 141例COVID-19患者实验室检查发现白细胞计数降低38例(26.95%)、淋巴细胞比率降低71例(50.35%)。141例COVID-19患者中发热(>37.5 ℃) 139例(98.58%)、咳嗽106例(75.18%)、头痛11例(7.80%)、咳痰41例(29.08%)、胸闷93例(65.96%)、呼吸道以外症状主要有腹泻等4例(2.84%)。141例COVID-19患者HRCT均有异常,52例(36.88%)胸部HRCT图像表现为磨玻璃影(GGO)和斑片影,以胸膜下分布为主;23例(16.31%)GGO合并局灶实变影;27例(19.15%)小片状模糊影;20例(14.18%)大片状实变影;48例(34.04%)支气管血管束增粗和血管穿行征;5例(3.55%)有空气支气管征象;7例(4.96%)小结节影;5例(3.55%)纤维化、网格影或条索影;7例(4.96%)双侧胸腔积液,4例(2.84%)纵隔或双侧肺门淋巴结肿大。结论 COVID-19的临床特征及HRCT影像表现多样,在COVID-19特定的流行病学背景下,应及时开展胸部HRCT扫描,对疾病做出早期预警。  相似文献

目的探讨新型冠状病毒肺炎(COVID-19)病人首诊胸部CT特征性表现,评价CT评分与临床检验结果的相关性。方法回顾性分析75例确诊COVID-19病人首诊胸部CT、发病时间及临床检验结果。由2名高年资影像诊断医师进行阅片,CT异常表现包括单发或多发磨玻璃影(GGO)或实变影,单肺或双肺受累,分布以胸膜下为主或肺中央为主。同时对COVID-19胸部CT表现进行半定量CT评分,采用Spearman相关分析比较CT评分与发病时间、淋巴细胞计数、中性粒细胞计数及C反应蛋白(CRP)的相关性。结果75例COVID-19首诊胸部CT正常者4例(5.33%),异常者71例(94.67%)。影像表现为多发GGO(26例,36.62%)、多发实变为主伴GGO(25例,35.21%)、多发GGO为主伴部分实变(12例,16.90%)、单发或散在GGO(8例,11.27%)。多数病人双肺受累(65例,91.55%),少数为单肺受累(6例,8.45%)。病变多位于胸膜下(66例,92.96%),少数位于肺内(5例,7.04%)。首诊CT评分与发病时间(rs=0.701,P<0.05)和CRP呈正相关(rs=0.522,P<0.05),与淋巴细胞计数呈负相关(rs=-0.303,P<0.05),与中性粒细胞计数无明显相关性(P>0.05)。结论COVID-19首诊胸部CT表现具有一定特征性,且与淋巴细胞计数、CRP有一定相关性,可能对临床预后有提示作用。  相似文献

目的 探讨胸部CT检查在显微镜下多血管炎(MPA)临床应用巾的价值.方法 搜集66例临床确诊为MPA的临床资料和胸部CT影像资料,将胸部受累患者的CT影像资料根据临床表现分成2组,即疾病活动组和稳定组,对两组数据进行对照统计分析(x2检验).结果 66例MPA患者中胸部CT检查发现异常43例(65.2%),其中活动组32例,稳定组11例.(1)43例MPA胸部CT表现:①肺实变10例(23.2%):均为活动组患者,病变以两中下肺较多见,实变区可见空气支气管征6例.②磨玻璃样改变23例(53.5%):活动组20例、稳定组3例;其巾两肺广泛病变18例(18/23),局限性病变5例(5/23).③磨玻璃样改变和(或)肺实变33例(76.7%):活动组30例,稳定组3例.④散在斑片影19例(44.2%):活动组17例,稳定组2例,分布缺乏特征性.⑤纤维条状及网格影28例(65.1%):活动组19例、稳定组9例,两中下肺及肺外带胸膜下多见.⑥蜂窝状改变6例(14.0%):活动组5例、稳定组1例,以两巾下肺多见.⑦胸腔积液21例(48.8%):活动组20例、稳定组1例.⑧肺大泡8例(18.6%):活动组7例、稳定组1例.⑨纵隔淋巴结肿大25例(58.1%):活动组23例,稳定组2例.(2)MPA活动组与稳定组CT征象对照分析:肺实变、磨玻璃样改变、散在斑片影、纵隔淋巴结肿大及胸腔积液征象在活动组明显多于稳定组,两组差异有统计学意义(x2值分别为4.479、4.083、4.053、9.697、9.345,P值均<0.05).结论 不同时期MPA胸部CT影像表现具有不同特征,胸部CT检查可辅助临床诊断,并对治疗有指导意义.
Abstract:
Objective To evaluate chest CT features of microscopic polyangiitis(MPA).Methods Clinical data and chest CT images of 66 patients diagnosed as MAP had been collected.Depending on clinical manifestations.the 66 cases were divided into the active group and the stable group.The data of two groups were analyzed by x2 test.Results Forty-three of 66(65.2%)CT images were abnormal including 32 of them in active group and 11 in stable group. In these patients,CT images showed ground glass opacities(53.5%),lung consolidations(23.2%),scattered patchy opacities(44.2%),ground glass opacities and/or lung consolidations(76.7%),reticular opacities(65.1%),honeycomb(14.0%),pleural effusions(48.8%),emphysema(18.6%),and mediastinal adenopathies(58.1%).Lung consolidations,ground glass opacities,scattered patchy opacities,mediastinal adenopathies,and pleural effusion in the active group are more common than that in the stable group(X2=4.479,4.083,4.053,9.697,9.345 respectively,P<0.05).Conclusion Chest CT features of in patients with MPA have difference between active and stable periods.The chest CT scan is a useful tool to diagnose MAP in different periods and guide the treatment.  相似文献

ObjectivesCoronavirus disease 2019 (COVID-19) caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). This paper aims to examine the CT imaging characteristics of COVID-19.MethodsWe evaluated CT images obtained between 10 January 2019 and 16 February 2020 at Taihe Hospital. Scans were conducted 2–6 times per patient and the re-testing interval was 2–7 days. Ninety-five patients with positive SARS-CoV-2 nucleic acid test results were included in this study and we retrospectively analysed their CT imaging characteristics.ResultsNinety-five patients underwent 2–3 SARS-CoV-2 nucleic acid tests and received a definitive diagnosis of COVID-19. Fifty-three were male and 42 were female, and their mean age was 42 ± 12 years (range: 10 months to 81 years). Sixty-nine patients (72.6%) experienced fever, fatigue, and dry cough, while 15 (15.8%) had poor appetite and fatigue, and 11 (11.6%) had a dry cough and no fever. On CT imaging, early stage patients (n = 53, 55.8%) showed peripheral subpleural ground-glass opacities; these were mainly local patches (22/53, 41.5%), while some lesions were accompanied by interlobular septal thickening. Thirty-four (35.8%) patients were classified in the ‘progression stage’ based on CT imaging; these patients typically showed lesions in multiple lung segments and lobes (21/34,61.8%), and an uneven increase in ground-glass opacity density accompanied by consolidation and grid-like or cord-like shadows(30.5%). Two patients (2.1%) showed a severe presentation on CT. These showed diffuse bilateral lung lesions, mixed ground-glass opacities and consolidation with cord-like interstitial thickening and air bronchograms, entire lung involvement with a “white lung” presentation, and mild pleural effusion. Six patients in remission (6.3%), visible lesion absorption, fibrotic lesions. Based on clinical signs, 71 (74.7%), 22 (23.2%), and 2 (2.1%) patients had mild or moderate, severe, and critical disease, respectively. Within the follow-up period, 93 patients recovered and were discharged, including the 53 early stage patients and 34 progression stage patients. The length of hospitalisation was 7–28 days (mean: 10 ± 3.5 days). On discharge, lesions were significantly reduced in area and had in many cases completely disappeared, while slight pulmonary fibrosis was present in some patients. One severe stage patient was still hospitalised at the end of the follow-up period and the other severe stage patient died. The overall mortality rate was 1.05%.ConclusionsUnderstanding the CT imaging characteristics of COVID-19 is important for early lesion detection, determining the nature of lesions, and assessing disease severity.  相似文献

PURPOSE: To determine thin-section computed tomography (CT) characteristics of acute eosinophilic pneumonia (AEP). MATERIALS AND METHODS: Thin-section CT scans of 29 patients (14 males, 15 females; mean age, 26+/-15 years; age range, 15-72 years) with AEP were included this retrospective study. The clinical diagnosis of AEP was established by Allen's criteria. Each thin-section CT was reviewed by two observers. RESULTS: Bilateral areas with ground-glass attenuation were observed on thin-section CT in all patients. Areas of air-space consolidation were present in 16 (55%) of 29 patients. Poorly defined centrilobular nodules were present in 9 patients (31%). Interlobular septal thickening was present in 26 patients (90%). Thickening of bronchovascular bundles was present in 19 patients (66%). Pleural effusions were present in 23 patients (79%) (bilateral=22, right side=1, left side=0). The predominant overall anatomic distribution was central in only 2 (7%) of 29 patients, peripheral in 9 patients (31%), and random in 18 patients (62%). The overall zonal predominance was upper in 4 patients (14%), lower in 8 patients (28%), and random in 17 patients (58%). CONCLUSION: CT findings in AEP patients consisted mainly of bilateral areas of ground-glass attenuation, interlobular septal thickening, thickening of bronchovascular bundles, and the presence of a pleural effusion without cardiomegaly. The most common overall anatomic distribution and zonal predominance of the abnormal CT findings were random.  相似文献

ObjectiveThe purpose of this study was to investigate the chest CT imaging features and clinical outcome of coronavirus disease 2019 (COVID-19) in Ningbo, China.MethodsIn this retrospective study, twenty-eight confirmed and seven highly suspected cases of COVID-19 were enrolled in Ningbo first hospital from January 26, 2020 to March 5, 2020. Cases were confirmed by real-time polymerase chain reaction (RT-PCR). The initial and follow-up chest CT imaging features, epidemiological history, and outcome were analyzed.ResultsThe average age of the patients was 57.3 ± 15.3 years (range: 27–96 years), including 25 females and 10 males. On CT images, 89.3% (25/28) confirmed and 100% (7/7) suspected patients had ground-glass opacities (GGOs), and GGOs with mixed consolidations were observed in 35.7% (10/28) confirmed and 42.9% (3/7) suspected cases, most of these lesions were distributed under the peripheral of both lungs. 17 confirmed and 4 suspected cases had a history of participating in Ningbo Tian-tong Temple rituals and all had GGOs in their lungs during the initial CT scan. As of March 25, 2020, the lung lesions of our cases were significantly resolved and all patients have been discharged from the hospital.ConclusionThe most common chest CT features are multiple bilateral and peripheral GGOs with mixed consolidations or not in the lungs of patients with COVID-19. Chest CT plays an important role in the diagnosis and monitoring treatment response of this disease. There was no reported death in our cases.  相似文献

Objectives:Although chest CT has been widely used in patients with COVID-19, its role for early diagnosis of COVID-19 is unclear. We report the diagnostic performance of chest CT using structured reporting in a routine clinical setting during the early phase of the epidemic in Germany.Methods:Patients with clinical suspicion of COVID-19 and moderate-to-severe symptoms were included in this retrospective study. CTs were performed and reported before RT-PCR results (reference standard) became available. A structured reporting system was used that concluded in a recently described five-grade score (“CO-RADS”), indicating the level of suspicion for pulmonary involvement of COVID-19 from 1 = very low to 5 = very high. Structured reporting was performed by three Radiologists in consensus.Results:In 96 consecutive patients (50 male, mean age 64), RT-PCR was positive in 20 (21%) cases. CT features significantly more common in RT-PCR-positive patients were ground-glass opacities as dominant feature, crazy paving, hazy margins of opacities, and multifocal bilateral distribution (p < 0.05). Using a cut-off point between CO-RADS 3 and 4, sensitivity was 90%, specificity 91%, positive predictive value 72%, negative predictive value 97%, and accuracy 91%. ROC analysis showed an AUC of 0.938.Conclusions:Structured reporting of chest CT with a five-grade scale provided accurate diagnosis of COVID-19. Its use was feasible and helpful in clinical routine.Advances in knowledge:Chest CT with structured reporting may be a provisional diagnostic alternative to RT-PCR testing for early diagnosis of COVID-19, especially when RT-PCR results are delayed or test capacities are limited.  相似文献
