目的通过流行病学调查了解绝经前截瘫女性血脂水平,探讨截瘫致缺乏运动对雌激素保护期女性血脂代谢的影响。方法2004~2009 年中国康复研究中心截瘫女性住院患者183 例入院时行血脂状态调查。血脂观察指标:甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白(LDL)和高密度脂蛋白(HDL)。结果截瘫女性TG、TC 水平显著高于女性全国平均水平,分别达到1.44mmol/L 和4.45 mmol/L;HDL、LDL 平均水平均与女性全国平均水平无显著性差异。年龄与TG(r=0.158, P<0.05)、TC(r=0.271, P<0.01)、LDL(r=0.207, P<0.01)呈正相关;与HDL 无显著性相关。总体血脂异常发生率60%,依次为低HDL(33%)、高TC(25%)、高TG(24%)、高LDL(21%)。结论在雌激素保护作用下,HDL 代谢受缺乏运动的影响较小,但TG、TC代谢仍有一定程度紊乱,心血管疾病风险增加。  相似文献

摘要 目的:通过回顾性调查资料比较脊髓损伤所致四肢瘫和截瘫青壮年男性血脂水平,探讨在缺乏运动状态下不同运动能力对青壮年男性血脂代谢的影响和差异形成机制,为血脂异常防治提供指导。 方法:对中国康复研究中心2004—2010年因脊髓损伤致瘫痪进行康复治疗的青壮年男性患者进行入院状态回顾性调查,入选要求:①诊断明确(脊髓损伤致瘫痪);②资料完整;③18周岁≤年龄≤60岁;④非卒中患者。共1812例患者入选,对血脂指标[甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白(LDL)、高密度脂蛋白(HDL)]进行比较分析。 结果:截瘫组平均TG、TC、HDL水平显著高于四肢瘫痪组(分别为TG 1.79±1.22mmol/L比1.56±0.87mmol/L;TC 4.42±0.92mmol/L比4.25±0.93mmol/L;HDL 1.14±0.35mmol/L比1.06±0.35mmol/L, P<0.01),高TG发生率显著高于四肢瘫痪组(分别为36%比28%,P<0.01),而低HDL血症发生率显著低于四肢瘫痪组(47%比57%,P<0.01), 两组患者LDL水平及异常发生率均无显著差异,TC异常发生率两组无显著差异。 结论:男性截瘫患者与四肢瘫患者TG、TC、HDL水平存在显著差异,提示上肢自主活动功能对TG、TC和HDL代谢有显著影响,既有HDL水平提高而获益的一面,也同时有TG、TC异常升高的不利面,其机制可能与运动量和饮食摄入差异有关;LDL的代谢并未受运动量和饮食因素的影响。  相似文献

青壮年男性四肢瘫痪患者血脂状况调查   总被引:3,自引:3,他引:0  
目的:通过病例回顾性调查来了解青壮年男性四肢瘫痪患者血脂特点,探讨这一特殊人群在绝对缺乏运动的生活方式下血脂代谢情况和可能的心血管疾病风险及对策。方法:对中国康复研究中心2004年1月—2008年12月因四肢瘫痪入院进行康复治疗的男性患者进行入院状态调查。共466例患者入选,对血脂指标进行分析。结果:四肢瘫痪男性平均甘油三酯(TG)全国水平显著高于男性平均水平,达到1.49mmol/L;平均高密度脂蛋白(HDL)水平显著低于全国男性平均水平,只有0.92mmol/L;总胆固醇(TC)、低密度脂蛋白(LDL)平均水平与全国男性平均水平无显著差异。年龄与TC、LDL显著正相关(分别为r=0.160, P<0.01; r=0.142, P<0.01);年龄与TG、HDL无显著相关。瘫痪病程与TG、HDL显著正相关(分别为r=0.097, P<0.05; r=0.095, P<0.05);瘫痪病程与TC、LDL无显著相关。血脂异常者393例(占84%);HDL异常降低者357例(占77%)。结论:青壮年四肢瘫痪男性患者血脂异常发病率很高(达84%);主要表现为HDL水平显著降低,发病率高(达77%),但随瘫痪病程延长有升高趋势,而其水平与年龄无关;TG水平显著升高且与瘫痪病程正相关,但与年龄不相关;TC、LDL水平无显著升高,但与年龄正相关,而与瘫痪病程不相关。  相似文献

AIM:To investigate the relationship between the patients with cerebral infarction(CI)in different ages and blood lipid levels.METHODS:The serum concentration of blood lipid levels were measured in 211 patients with CI and 144 control cases.RESULTS:The level of lipoprotein(a)[Lp(a)] in the young adults with CI and the levels of triglyceride(TG),total cholesterol(TC),low-density lipoprotein-cholesterol(LDL-C),apolipoproteinB100(ApoB100) and Lp(a) in the subgroup of mid-adults with CI were much higher than corresponding controls.The levels of TC,LDL-C ApoB100 and Lp(a) in the subgroup of olds were significantly higher when compared with the corresponding controls,while the high-density lipoprotein-cholesterol(HDL-C) level was lower.Multiple linear regression showed that age had positive correlation to TC and ApoB100 and negative correlation to HDL-C.CONCLUSION:There are different disturbances of blood lipid metabolism in the patients with CI in different ages.The serum Lp(a) levels may be an independent risk factor for CI.  相似文献

BACKGROUND: Although pneumonia severity index(PSI) is widely used to evaluate the severity of community-acquired pneumonia(CAP), the calculation of PSI is very complicated. The present study aimed to evaluate the role of B-type natriuretic peptide(BNP) in predicting the severity of CAP.METHODS: For 202 patients with CAP admitted to the emergency department, BNP levels, cardiac load indexes, inf lammatory indexes including C-reactive protein(CRP), white blood cell count(WBC), and PSI were detected. The correlation between the indexes and PSI was investigated. BNP levels for survivor and non-survivor groups were compared, and a receiver operating characteristic(ROC) curve analysis was performed on the BNP levels versus PSI.RESULTS: The BNP levels increased with CAP severity(r=0.782, P0.001). The BNP levels of the high-risk group(PSI classes IV and V) were signifi cantly higher than those of the low-risk group(PSI classes I–III)(P0.001). The BNP levels were signifi cantly higher in the non-survivor group than in the survivor group(P0.001). In addition, there were positive correlations between BNP levels and PSI scores(r=0.782, P0.001). The BNP level was highly accurate in predicting the severity of CAP(AUC=0.952). The optimal cut-off point of BNP level for distinguishing high-risk patients from low-risk ones was 125.0 pg/m L, with a sensitivity of 0.891 and a specifi city of 0.946. Moreover, BNP level was accurate in predicting mortality(AUC=0.823). Its optimal cut-off point for predicting death was 299.0 pg/m L, with a sensitivity of 0.675 and a specifi city of 0.816. Its negative predictive cut-off value was 0.926, and the positive predictive cut-off value was 0.426.CONCLUSION: BNP level is positively correlated with the severity of CAP, and may be used as a biomarker for evaluating the severity of CAP.  相似文献

Objective To investigate the variation of bone marrow complement level in cytopenia pa-tients with positive BMMNC-Coombs test(CBCPC), and probe the role of complement in destroying hemato-poietic cells of CBCPC patients. Methods One hundred and twenty-four patients with CBCPC and twenty-three healthy donors as controls were enrolled in this study. The levels of CI-150, C3, C4, C5b-9 were tested with ELISA. The auto-antibodies on bone marrow hematupoietic cells (BMHC) were examined with flow cy-tometry. Results The level of C5b-9 in bone marrow(BM) of untreated CBCPC patients [(119.8 ± 54.0)μg,/L] was significantly higher than that of recovered patients [(100.7 ± 33.4) μg/L] or normal controls [(93.9 ± 28.8) μg/I.] (P < 0.05). The levels of CH50 in BM of untreated or recovered CBCPC patients [(33.3 ± 11.5) kU/L, (30.8 ± 10.3) kU/L] were significantly higher than that of normal controls [(24.1 ±6.4) kU/L] (P < 0.05). The level of C3 in BM of untreated or recovered CBCPC patients [(4.9 ± 2.2) mg/ L], (5.0 ± 3.5) mg/L] was significantly lower than that of normal controls [(7.0 ± 5.6) mg/L] (P < 0.05). The level of complement in peripheral blood was consistent with that in BM. CH50 in BM of CBCPC patients was negatively correlated with their C3 (r = - 0. 303, P = 0. 007) and positively correlated with their C5b-9(r = 0. 241, P = 0. 003) levels.
